O objetivo geral do trabalho consiste em delinear os aspectos da situação analítica para estabelecer novas compreensões da análise a partir do conceito de campo analítico de Willy e de Madeleine Baranger. A situação analítica possui duas partes que interagem entre si, o enquadre e o processo analítico. O processo analítico é a parte móvel da situação analítica que envolve a relação transferencial, a resistência, as interpretações, o insight, etc. O enquadre é a parte que tende à estabilidade e tem a finalidade de ser aquilo que sustenta o processo analítico e de permitir o seu desenrolar com o mínimo de interrupções possíveis. O enquadre é formado pelas regras analíticas, pelo contrato, o espaço físico, o tempo (duração e horário) e as técnicas. O manejo do enquadre torna possível o trabalho clínico com o núcleo psicótico do paciente e de suas partes regredidas. Nos casos de regressão, a adaptação do enquadre e do analista torna-se necessária para o bom andamento da análise. O campo analítico do casal Baranger permite novas possibilidades de compreensão da situação analítica a partir da existência de uma interligação entre todas as partes do campo e do campo com o todo. Nessa perspectiva, o discurso do paciente adquire um aspecto de narratividade em que os personagens do campo se apresentam permitindo ao analista outras formas de interpretação. A partir da ótica do campo analítico, a análise torna-se um espaço de história e de sonhar a dois. / The area of interest of this thesis is the psychoanalysis, more specifically the theoretical study of the concept of the analytical situation and the psychoanalytical field. The general objective of the work is to delineate the aspects of the analytical situation to establish new understandings of the analysis from Willy and Madeleine Baranger's psychoanalytical field concept. The analytical situation has two parts that interact with each other, the framework and the analytical process. The analytical process is the moving part of the analytic situation that involves the transferential relationship, resistance, interpretations, insight, etc. The frame is the part that tends to stability and is intended to be what sustains the analytical process and to allow its unfolding with as few interruptions as possible. The framework is formed by analytical rules, contract, physical space, time (duration and time) and techniques. The management of the frame makes possible the clinical work with the patient's psychotic nucleus and its regressed parts. In the cases of regression, the adaptation of the framework and the analyst becomes necessary for the good progress of the analysis. The analytical field of the Baranger couple allows new possibilities of understanding the analytical situation, from the existence of an interconnection between all the parts of the field and the field with the whole. In this perspective, the patient's discourse acquires an aspect of narrativity in which the characters of the field present themselves allowing the analyst other forms of interpretation. From the point of view of the analytical field, analysis becomes a space of history and of dreaming of two. / Le domaine d'intérêt de cette thèse est la psychanalyse, plus précisément l'étude théorique du concept de situation psychanalytique et de champ analytique. L'objectif de l’étude est de délimiter les aspects de la situation psychanalytique dans le but d’établir des nouvelles compréhensions de l'analyse à l'aide des outils conceptuels de champ analytique de Willy et Madeleine Baranger. La situation psychanalytique comprend deux parties qui interagissent entre elles-mêmes, c’est-à-dire le cadre et le processus analytique. D’un côté, le processus analytique est la partie mobile de la situation analytique et celle qui implique la relation transférentielle, la résistance, les interprétations, le insight, etc. De l’autre, le cadre est la partie qui tend à la stabilité et celle qui maintient le processus analytique en permettant son déroulement avec le minimum d'interruptions possibles. Le cadre est formé par les règles analytiques, le contrat, l'espace physique, le temps (la durée et l'horaire de la séance) et les techniques. L’aménagement du cadre permet le travail clinique avec le noyau psychotique du patient et ses parties régressives. Dans les cas de régression, l'adaptation du cadre et de l'analyste devient nécessaire pour la bonne progression de l'analyse. Le concept de champ analytique avancé par le couple Baranger permet de nouvelles possibilités de compréhension de la situation analytique étant donné l’existence d’une interconnexion entre toutes les parties du champ et du champ par rapport à son ensemble. Dans cette perspective, le discours du patient acquiert un aspect de narrativité dans lequel se présentent les personnages du champ auxquels permettent à l'analyste d'autres formes d'interprétation. Du point de vue du champ analytique, l'analyse devient un espace d'histoire et de rêver à deux.

Screening and alcohol brief interventions in antenatal care : a realistic evaluation

Background: Prenatal alcohol consumption is one of the leading preventable causes of birth defects, including fetal alcohol syndrome and learning disabilities. Although there is strong evidence of the benefits of screening and alcohol brief interventions (ABIs) in reducing hazardous and harmful drinking among the primary care population, evidence of its effectiveness with the antenatal care population is limited. Nevertheless, the Scottish Government is incorporating an alcohol screening and ABI programme as part of the routine antenatal care provided to women in a bid to protect the health and safety of the unborn child and improve subsequent health and developmental outcomes. This research therefore seeks to increase understanding of the factors that are likely to influence the effectiveness of this recently implemented programme. It also aims to explore the extent to which contemporary issues such as change in guidelines regarding alcohol consumption during pregnancy influences perceptions and attitudes, and the possible implications of these on the screening and ABI delivery. Methods: The study described in this thesis employed a realistic evaluation methodology. Realistic evaluation is a theory-driven approach to investigating social programmes. It is concerned with hypothesising, testing and refining programme theories by exploring the interaction of contexts, mechanisms and outcomes. To identify the relevant screening and ABI programme theories, two separate systematic reviews, a critical review and four face-to-face interviews were undertaken with health policy implementers. The findings were used to construct context, mechanism and outcomes propositions. The propositions were then tested by conducting individual interviews with seventeen pregnant women and fifteen midwives, a further six midwifery team leaders were involved in a focus group discussion. A thematic approach using a hybrid of inductive and deductive coding and theme development informed the qualitative analysis. Results: In the context of uncertainties regarding the threshold of drinking that causes fetal harm, pregnant women reported that screening assessment helped them to reflect on their drinking behaviour and facilitate behaviour change. For women who drank at hazardous and harmful levels before attending the booking appointment, screening and ABI may be helpful in terms of eliciting behaviour change. However, they may not be very beneficial in terms of reducing harm to the fetus as it has been found that drinking during the first trimester poses the most risk to the fetus. Training and resources provided to midwives as part of the screening and ABI programme were found to be facilitating mechanisms that midwives indicated improved their skills and confidence. However, most of the midwives had not subsequently employed the motivational interviewing skills required for the ABI delivery, as many of the pregnant women reported that they reduced or abstained from alcohol consumption once pregnancy was confirmed. The outcome noted was that midwives confidence decreased leading to missed opportunities to appropriately deliver the ABI to eligible women. The small numbers of women being identified for ABI meant midwives rarely delivered the ABI. This negatively influenced midwives attitudes as they then accorded ABI low priority in their workload. Other disenabling mechanisms noted to be hampering the implementation of the screening and ABI initiative included midwives contending with competing priorities at the booking appointments, and the lack of adequate rapport between midwives and pregnant women at the booking appointment to discuss alcohol issues appropriately, leading to women providing socially desirable responses to screening questions. Conclusions: The findings of this study has generated greater explanations of the working of the screening and ABI programme in antenatal care setting and has provided transferable lessons that can be used by others intending to implement similar programmes in other settings.

The efficacy of natural plant extracts (rosemary, sage, and thyme) in stabilizing hemp and soybean oils during storage

Lipid oxidation of edible oils is a concern due to undesirable effects on the organoleptic, nutritional, and toxicological aspects of fats and oils. Vegetable oils are particularly prone to lipid oxidation due to the susceptibility of the unsaturated double bonds of the fatty acids to oxidation. Hemp oil is high in unsaturated fatty acids, including gammalinolenic acid, which is an essential fatty acid. Theoretically, hemp oil would be prone to lipid oxidation; however, scientific data on the subject could not be found. Of the common vegetable oils of North America, the fatty acid composition of soybean oil most closely resembles that of hemp oil; however, gamma-linolenic acid is absent in soybean oil. The following studies monitored the oxidative stability of hemp and soybean oils based on the peroxide values, p-anisidine values, totox numbers, UV absorption measured at wavelengths of 232 and 268 nm, and fatty acid (%) compositions. A 48-week shelf-life study was performed to compare the oxidative stability of hemp and soybean oils with and without 0.125% (w/w) of rosemary, sage, or thyme extracts and stored in the dark at 38°C. Also, a 14-day accelerated storage study determined the quality of hemp and soybean oils exposed to 180°C for 0,4, 8, 12, and 24 hours and the effects of subsequent storage at 38°C in darkness. A 24-day accelerated storage study was used to monitor oxidation of hemp and soybean oils that were heated at 180°C for 4 hours prior to storage at 38°C in darkness, in order to determine the antioxidant efficacy of 0.10% and 0.05% (w/w) of rosemary, sage, or thyme extracts, and 0.10% and 0.05% (w/w) extract binary mixtures of rosemary + sage, rosemary + thyme, or sage + thyme. During long-term storage (48 weeks), the levels of lipid oxidation products (i.e. hydroperoxides, aldehydes, conjugated dienes and trienes) in hemp and soybean oils exhibited some changes during storage, but generally remained low throughout the study. This showed that hemp and soybean oils were not inherently prone to lipid oxidation. Based on the thermally-induced accelerated oxidation conditions, lipid oxidation was apparent in heated hemp and soybean oil samples as detected by the p-anisidine, totox number, UV absorption methods, and fatty acid contents. The peroxide values remained low after hightemperature heating, but progressively increased during storage. The addition of various concentrations and mixtures of rosemary, sage and thyme extracts before the thermal treatment, did not prevent oil deterioration due to heating, but effectively inhibited the generation of hydroperoxides during subsequent storage.
