The Career Development of Adolescent Mothers: Research to PracticeBarto, Heather Heinfelden 16 June 2011 (has links)
This research endeavor contributes to a better understanding of the career development of adolescent mothers by (a) highlighting the contextual considerations; (b) examining the existing body of literature; (c) contributing to the research on adolescent mothers' career adaptability, resiliency, and obstacles; and (c) proposing ways to bridge the research to practice gap. An ecological examination of the contextual considerations associated with adolescent pregnancy and motherhood provided pertinent information on which to base a culturally sensitive framework to examine the career development of adolescent mothers using the concepts of career adaptability and resiliency and an assessment of obstacles that may impede their career development.
The culturally sensitive framework was used in two studies conducted to better understand the career development of adolescent mothers. The first was a content analysis designed to provide a better understanding of the published literature and to direct a research study. Content analysis findings revealed the need to develop and implement comprehensive programs for adolescent mothers that are strength-based, responsive to areas of need, and foster career development skills/knowledge and resiliency. Using the results of the content analysis, a study was designed to examine the relationships among career adaptability, resiliency, and perceived obstacles to career development with a convenience sample of adolescent mothers. Results indicated that these adolescent mothers were similar to non-mothering peers in the planning and decision making dimensions of career adaptability but lower in exploration. Traits of personal resiliency and emotional reactivity were comparable to non-mothering peers, but relational resiliency was lower. Obstacles most often cited as impeding career development were pressing immediate needs and educational/career related concerns.
Finally, the need to bridge the research to practice gap is discussed including considerations related to practitioners, organizations, and communication. In an effort to bridge the research to practice gap, the Interactive Systems Framework (ISF) is proposed as a model for communicating with practitioners through Synthesis and Translation, Support, and Delivery systems. The model is presented as an example for sharing with practitioners a culturally sensitive framework on the career adaptability of adolescent mothers supported by dissemination and implementation of research findings. / Ph. Este estudo objetivou conhecer e compreender como foi constituída a percepção que os adolescentes têm da parentalidade recorrente na adolescência. Para compreender o significado desta experiência e a relação com seus projetos de vida utilizei-me do método qualitativo com enfoque na Fenomenologia Social. Foram realizadas 15 entrevistas com cinco pais e dez mães que haviam experienciado a parentalidade recorrente com as seguintes questões norteadoras: Como foi para você ser mãe/pai pela primeira vez? Como aconteceu o nascimento do outro filho? Como é ser mãe/pai mais de uma vez, ainda adolescente? O que você espera do futuro sendo mãe/pai tão jovem? Dos depoimentos emergiram cinco categorias concretas do vivido: Contextualizando a percepção sobre ser pai/mãe adolescente, que implicou na construção de duas subcategorias: sendo pai/mãe pela primeira vez e sendo pai/mãe mais de uma vez; Vivenciando perdas; Vivenciando ganhos, com duas subcategorias: sentindo-se amadurecidos /responsáveis e expressando satisfação com a parentalidade; Buscando segurança para o futuro e Experienciando situação ambivalente. O estudo permitiu compreender que a parentalidade adolescente recorrente é um fenômeno complexo, multifacetado, de inesgotável possibilidades perpceptivas, cujas diversas vivências são dependentes do contexto social que define os desejos, os projetos, as possibilidades e significações nas distintas classes sociais. Com este entendimento numa relação face a face, de intersubjetividade, respeitando a singularidade dos adolescentes, o enfermeiro tem uma situação de destaque no que se refere ao assistir/cuidar desta clientela, centrado na dimensão humana /existencial. / The goal of this study was to know and understand how the perception adolescents have of the recurring parenthood during adolescence was constituted. To comprehend the meaning of this experience and the relation with their life projects the qualitative method was used, focusing the social phenomenology. Fifteen interviews were carried out with five fathers and ten mothers who had experienced the recurring parenthood as an unveiling strategy of the phenomenon, using the following directive questions: How did you feel becoming a first-time mother/father? How did the birth of another child take place? How does it feel to be a mother/father more than once, still being an adolescent? What do you expect of the future, being such a young mother/father? Five concrete categories of the experienced surfaced from the subjects\' statements: Contextualizing the perception about being an adolescent mother/father with two subcategories: being a first-time parent and being a parent more than once; Experiencing losses; Experiencing gains with two subcategories: feeling more mature/responsible and expressing satisfaction with parenthood; Seeking safety for the future; and Experiencing an ambivalent situation. The study allowed the understanding that recurring adolescent parenthood is a complex, multi-faceted phenomenon, with endless perceptive possibilities, of which the diverse experiences depend on the social context that defines wishes, projects, possibilities and meanings through the distinct social classes. Based on this understanding during a face-to-face relationship of intersubjectivity, respecting the adolescents\' singularity, the nurse plays a very important role as regards assisting/taking care of this clientele, centered on the human/assisting dimension.

It occurs mainly in girls between 11 to 13-year-old with a prevalence rate of 4%. This common spinal condition can be associated with significant cosmetic and clinical morbidities in severe cases. Since the treatment for AIS remains unsatisfactory, it is imperative to elucidate the etiopathogenesis of AIS so that effective therapeutic and preventive measures can be devised. Towards this end, Cheng et al. investigated and noted that osteopenia was present in 30% of AIS subjects. Osteopenia was found to be a significant prognostic factor for curve progression in AIS. In previous studies, only Dual energy X-ray Absorptiometry (DXA) was available and the measurement was confined to areal-BMD (aBMD). For in-depth understanding of the bone quality and its link to the etiopathogenesis of AIS, three-dimensional volumetric evaluation of bone mass and measurement of key parameters of bone quality would be important. With the advancement of imaging techniques and the availability of high-resolution pQCT (HR-pQCT), it is now possible to have in vivo measurement of bone quality including Bone Morphometry, Volumetric BMD (vBMD) and Trabecular Bone Micro-architecture in human subjects. / The current study utilized HR-pQCT with the following objectives: 1) to investigate bone quality in AIS vs. age- and sex-matched normal controls and 2) to evaluate and compare the correlation of bone quality with osteopenia between AIS and non-AIS control subjects. / 214 untreated AIS and 187 non-AIS healthy girls between 11-13 years old were recruited. aBMD of bilateral femoral necks was measured by DXA. Bone Morphometry, vBMD and Trabecular Bone Micro-architecture were measured at the non-dominant distal radius using HR-pQCT. / In the first part, our findings demonstrated that AIS was associated with lower Cortical Bone Area (p=0.048), Cortical Bone vBMD (p=0.014), Trabecular Number (p=0.003) and greater Trabecular Separation (p=0.006) after adjustment for age. With multivariate linear regression analysis, after adjusted for age, calcium intake and physical activity levels, the association of AIS with lower Cortical Bone vBMD, (p=0.032), Trabecular Number (p=0.005) and greater Trabecular Separation (p=0.010) remained. In contrast, no difference was found in the Cortical Perimeter between AIS and controls. / The abnormalities in cortical bone quality in AIS suggested the possibility of defects in mineral accretion during endocortical apposition. We speculated that lower Cortical Area and vBMD could be associated with reduced bone mechanical strength thus predisposing to the development or progression of spinal deformity in AIS. Furthermore, the association between AIS and lower Trabecular Number reflected a defect in trabecular formation, which might be due to abnormal regulation and modulation of endochondral ossification and bone mineralization in AIS. / In the second part, subjects were classified into the osteopenic (Z-score≤-1) and non-osteopenic (Z-score>-1) group. Interestingly, we found that osteopenia in AIS was distinctly different from osteopenia in non-AIS controls. Alterations in trabecular bone quality in association with osteopenia were only detected in AIS. Osteopenic AIS was uniquely associated with lower Trabecular Bone vBMD, BV/TV, Trabecular Thickness (all p<0.001) and greater SMI (p=0.008) indicating predominance of rod-like trabeculae when compared with non-osteopenic AIS. / The trabecular compartment is generally more vulnerable and responsive to changes in the metabolic and biomechanical environment. The unique alterations of trabecular bone quality in osteopenic AIS suggested the presence of metabolic dysfunction resulting in abnormal modeling and remodeling processes in AIS. These altered trabecular bone micro-architecture might lead to reduced bone strength thus resulting in mechanical weakness of the spine and subsequent curve progression. Another clinical significance of the present study was the call for developing a composite prognostic factor incorporating both BMD and bone quality parameters for more accurate prediction of curve occurrence and progression in AIS in clinical practice. / In conclusion, this is the first series of in vivo studies evaluating bone quality in AIS. Our findings demonstrated abnormal bone quality in AIS and unique alteration of trabecular bone profile in osteopenic AIS. An Examination of Self-Disclosure Willingness Among AdolescentsHall, Benton G. (Benton Garrett) 05 1900 (has links)
This study surveyed the willingness of adolescents to self-disclose in different situations. One hundred and forty high school students were surveyed in a suburban, southwestern city. The survey included Gordon Chelune's Self-disclosure Situations Survey, Michael Leary's Social Anxiety Survey, and a 40-item situational survey created to test students' self-disclosure willingness in normal situations. It was hypothesized that students would more readily disclose to peers than parents or counselors; that they would more readily disclose in warm, informal settings than cold, formal ones; and that there would be a significant negative correlation between social anxiety and self-disclosure. There was a small but significant negative correlation between social anxiety and students' willingness to disclose only in a warm setting. The other hypotheses were proved.

Child and adolescence mental health is a huge concern throughout the world, especially in developing countries were children and adolescence form majority of the population. Hyperactivity is one of the most common behavioural problems seen in child and adolescent psychiatric outpatient units. This behavioural problem may be accompanied by an underlying childhood psychiatric disorders.
Objectives
To determine the number of children with hyperactivity, establish gender differences, as well as profile of psychiatric disorders associated with hyperactivity at Dr George Mukhari Hospital child psychiatry outpatient unit.
Method
This was a retrospective descriptive study that included children who presented for the first time at Dr George Mukhari outpatient child psychiatry outpatient unit from January 2009 to December 2009. Children of all genders were equally considered and were within the age group of 3 to 14 years. Data of all children included was collected from the children’s records. All the records of children with hyperactivity were separated from those without this behavioural problem. Gender differences as well as underlying psychiatric disorders were determined. Diagnoses made during the first assessment were based on Diagnostic and Statistical Manual of Mental Disorders, 4th edition text revision (DSM IV TR).
Results
A total of 49 new children who were assessed during the study period were included. Males were over-represented than females, and accounted to 79.49% and 20.4% respectively. Almost half of the children presented with hyperactivity (48.98%). Most of the children with hyperactivity were less

than 6 years (50%), followed by school aged group (41.67%) and then adolescents (8.33%). Majority of children with hyperactivity were males (83.33%) than females (16.67%).
The most common disorder associated with hyperactivity was mental retardation (62.50%), followed by attention deficit hyperactivity disorder (29.17%); and then mania due to HIV (4.17%) and sensorineural hearing deficit with hyperactivity (4.17%).
Conclusion
Hyperactivity is a common behavioural problem or complaint at Dr George Mukhari child psychiatry outpatient unit. It is common in males than females, and is associated with underlying childhood psychiatric disorders. Therefore, health professionals should be trained to identify behavioural problems, and community awareness considered since hyperactivity is associated with childhood psychiatric disorders. This is important because early identification and intervention will subsequently yield a good outcome.
