Efeitos de diferentes concentrações de isofluoretano sobre a perfusão tecidual em cães /

Oito cães da raça Beagle foram anestesiados em múltiplos de CAM correspondentes a 1,5 (G1,5) e 2,0 (G2,0) CAM por duas horas, sendo mensuradas a cada 20 minutos as variáveis frequência cardíaca (FC), pressão sistólica (PAS), média (PAM) e diastólica (PAD), pressão da artéria pulmonar média (PAPm), pressão capilar pulmonar média (PCPm), pressão venosa central (PVC), índice cardíaco (IC), índice sistólico (IS), índice de trabalho ventricular esquerdo (ITVE), índice de resistência periférica total (IRPT), índice de resistência vascular pulmonar (IRVP), índice de oferta de oxigênio (IDO2), índice de consumo de oxigênio (IVO2), taxa de extração de oxigênio (TeO2), saturação venosa mista de oxigênio (SvO2) e lactato sanguíneo arterial. As médias de PAM, IC, IS e ITVE, diferiram significativamente entre os grupos, o que não foi observado nas variáveis IRPT e IRVP, as quais apresentaram diferença somente entre momentos de um mesmo grupo. Da mesma forma, houve diminuição progressiva da IRPT em G1,5, mas não em G2,0. Houve redução de IDO2 em G2,0 quando comparada a G1,5, mas não de IVO2. O lactato apresentou valores acima dos limites de referência, não diferindo entre grupos e reduzindo-se ao longo do tempo, enquanto que SvO2 foi menor em G2,0, acompanhada de maior TeO2 neste grupo. Concluiu-se que a perfusão global é prejudicada por concentrações maiores de isofluorano, ao passo que a perfusão periférica é mantida. Existe um limite para o grau de vasodilatação periférica causada pelo isofluorano em cães e o tempo de exposição ao anestésico interfere com a dinâmica cardiovascular tanto quanto sua concentração / Abstract: The aim of this study was to evaluate the effects of isoflurane and anesthetic time period on tissue perfusion of dogs under two different concentrations. Eight beagle dogs were anesthetized in MAC multiples correspondent to 1.5 (G1.5) e 2.0 (G2.0) MAC for two hours, being the following variables collected every 20 minutes: heart rate (HR), arterial systolic (SAP), mean (MAP) and diastolic (DAP) pressures, pulmonary artery mean pressure (PAPm), pulmonary artery wedge pressure (PAWP), central venous pressure (CVP), cardiac index (CI), systolic index (SI), left ventricle work index (LVWI), total peripheral resistance index (TPRI), pulmonary vascular resistance index (PVRI), oxygen delivery index (IDO2), oxygen consumption index (IVO2), oxygen extraction (TeO2), mixed venous oxygen saturation (SvO2) and arterial blood lactate. Mean values of MAP, CI, SI and LVWI were different between groups, and such differences were not observed in TPRI and PVRI, which differences were only significant between time points in each group. In addition, there was progressive reduction of TPRI in G1.5, but not G2.0. There was a reduction of IDO2 in G2.0 when compared to G1.5, but not IVO2. Lactate levels were above reference limits, with no difference between groups and decreasing over time, whereas SvO2 was lower in G2.0, accompanied by higher TeO2 in that group. We conclude that global perfusion is impaired by higher concentrations of isoflurane, while peripheral perfusion is maintained. Oito bezerros receberam infusão contínua de lidocaína (GL) ou salina 0,9% (GC). Após MPA com xilazina 0,05 mg/kg/IV e indução anestésica com quetamina 2mg/kg associada com midazolam o,1mg/kg, com os bezerros em decúbito lateral procedeu-se a intubação e administrou-se isofluorano 1,3%. Subsequentemente institui-se a VPPI com PPI de 15 cmH2O e fR de 6mpm. Decorridos 40 minutos sob anestesia com isofluorano com ventilação mecânica, aplicou-se lidocaína 2mg/kg e iniciou-se infusão contínua na taxa de 100 µg/kg/minuto (GL). Anotaram-se as variações da FC, PA, fR, SpO2, BIS e TR antes da MPA (MB) e 15 minutos após a MBA (MX) antes da administração da lidocaína (M0) e em intervalos de 20 minutos após o início da infusão dos fármacos (M20, M40, M60 e M80). As demais variáveis foram mensuradas a partir de M0. Após o final da infusão e desconexão, foram avaliados o período de tempo para a adoção de decúbito esternal e posição quadrupedal. Também foi mensurada a concentração sérica da lidocaína. A infusão continua de lidocaína não alterou as variáveis ventilométricas, hemogasométricas, índice biespectral e recuperação, porém diminuiu FC e IC. Conclui-se que apesar de ter causado redução da atividade cardíaca, a infusão contínua de lidocaína pode ser empregada como técnica anestésica em bezerros / Cardiorespiratory effects and bispectral index were analised after continuous rate infusion of lidocaine in calves isoflurane-anesthetized under controlled ventilation. Eight calves received continuous rate infusion of lidocaine (LG) or saline 0.9% (CG). After premedication with xylazine 0.05 mg kg -1 IV and induction of anesthesia with ketamine 2 mg kg -1 associated with the midazolam, 1 mg kg -1 , with the calves in lateral recumbence preceded the intubation and was administered isoflurane 1.3%. IPPV was instituted with PIP of 15 cmH2O and RR of 6 beats min -1 . After 40 minutes with isoflurane anesthesia and mechanical ventilation was applied lidocaine 2 mg kg -1 and continuous rate infusion was started at a rate of 100 mg kg -1 min -1 (LG). Were recorded variations in HR, AP, fR, SPO2, BIS and RT before administration of premedication (MB) and 15 minutes after administration (MX) before administering lidocaine (M0) and every twenty minutes after the start of the infusion of drugs (M20, M40 M60 and M80).The other variables were measured from M0. After the end of infusion and disconnection, were evaluated the time to adopt sternal recumbency and standing position. Also measured the serum concentration of lidocaine. Continuous rate infusion of lidocaine did not change the ventilometry, blood gas, bispectral index and recovery, but decreased HR and CI. We conclude that although there was a reduction in cardiac activity, the continuous infusion of lidocaine may be used as anesthesia in calves

Pesquisas recentes têm dado atenção especial às citocinas envolvidas na resposta inflamatória, mas ainda não há consenso quanto a influência do sevoflurano (SVF) no perfil de citocinas. Esse estudo analisou as citocinas IL-1, IL-2, IL-4, IL-5, IL-6, IL-8, IL-10, IL-12, TNF- e INF- durante a anestesia com SVF em pacientes com estado físico ASA I, submetidos a cirurgias de pequeno porte, e idade de 18 a 55 anos. Os indivíduos foram distribuídos em dois grupos de 15, sendo os do grupo SVF pacientes submetidos a cirurgia de timpanoplastia ou septoplastia com duração de pelo menos 120 minutos, sob anestesia geral com SVF em 1,0 a 1,5 concentração alveolar mínima (CAM). As amostras de sangue foram coletadas antes da indução anestésica (M1-pré-cirurgia), após indução anestésica e imediatamente antes do início da cirurgia (M2), 120 minutos após o início da anestesia inalatória (M3), e no 1º dia do pósoperatório (M4). Outros 15 indivíduos constituíram o grupo controle, voluntáriosdos quais foi obtida uma única amostra de sangue venoso. A análise dascitocinas foi feita pelo método da citometria de fluxo utilizando o kit BD Cytometric Bead Array. Observou-se diferença significativa em relação a IL-4 (p=0,02), maior entre os pacientes, e IL-5 (p<0,001), maior entre os voluntários. No grupo de pacientes, houve aumento significativo de IL-5 em M3 em relação a M1 (p=0,03), e de IL-6 no momento M4 em relação aos momentos M1 e M2 (p<0,001). Em conclusão, em pacientes adultos com estado físico ASA I submetidos a cirurgias de timpanoplastia ou septoplastia, a anestesia com sevoflurano, com exceção de aumento de IL-5 no intraoperatório... / Patients have stress when submitted to anaesthesia and surgery, characterized by inflammatory response because of the surgery trauma, and possibly due to the anaesthetics. Literature shows controversial data as most studies have been designed using large surgery models. Using minor surgical procedures, in order to reduce tissue injury influence on inflammatory response, this study aimed to evaluate the effects of anaesthesia with sevoflurane (SVF) on plasma cytokine profile from 15 ASA I patients of both genders, aged from 18 to 55, scheduled for tympanoplasty or septoplasty lasting, at least, 120 minutes, with 1.0 to 1.5 minimum alveolar concentration of SVF. Blood samples were drawn at 4 time points: before anaesthesia induction (T1 - baseline), before surgery (T2), at 120 minutes after anaesthesia induction (T3), and on the postoperative first day (T4). The study also included 15 healthy volunteers, not submitted to anaesthesia and surgery, as controls (C) to T1, and from whom blood sample was collected a single time. Plasma IL-1, IL-2, IL-4, IL-5, IL-6, IL- 8, IL-10, IL-12, TNF- e INF-) were assessed in plasma from patients and volunteers by flow cytometry, using the BD® Cytometric Bead Array kit.IL-4 (p=0,02) and IL-5 (p<0,001) differed between groups at T1. In the patient group there were differences on IL-5 (p=0,03) and IL-6 (p<0,001) levels, with peakvalues at M3 and M4, respectively. Efeitos hepaticos da associação de anestesicos volateis e paracentamol; estudo experimental em ratos

Os anestésicos voláteis também podem causar hepatotoxicidade, por efeito direto ou através dos metabólitos que produzem. Neste trabalho, procurou-se avaliar os efeitos da associção dos voláteis, halotano, isoflurano e sevoflurano, com o paracetamol, em ratos não subp1etidos à hipoxia e mantidos em ventilação espontânea. Foram incluídos no estudo 94 ratos machos adultos, da linhagem Wistar, com peso entre 260 e 290 g. Destes, 38 foram utilizados na determinação dos valores e intervalos de referência laboratoriais. Os demais foram distribuídos em sete grupos de oito animais cada. Nos grupos um, dois e três, foram administradas, respectivamente, uma CAM (concentração alveolar -mínima) de halotano, de isoflurano e de sevoflurano. Esses anestésicos foram vaporizados em oxigênio a 100%, durante 60 minutos, por cinco dias consecutivos. Nos grupos quatro, cinco e seis, após mesmo procedimento com halotano, isoflurano e sevoflurano, respectivamente, foi administrado paracetamol na dose diária de 500 mg/kg de peso, através de sonda orog~ca, por cinco dias consecutivos. No grupo sete, administrou-se paracetamol isoladamente, em dose d$i.a única de 500 mg/kg. No quinto dia de acompanhamento, os ratos foram anestesiados com tiopental sódico endovenoso (20 mg/kg). Após laparotqmia, 3 ml de sangue foram coletados da aorta, para dosagens das enzimas (aspartato aminotransferase - AST, alanina aminotransferase - ALT, fosfatase alcalina - FA e gama-glutamiltransferase - GGT) e para determinação das concentrações plasmáticas de paracetamol. Fragmentos de :flgado foram fIxados em formalina para posterior estudo histológico. Na análise clínica dos re~ultados, observou-se a ocorrência de alterações enzimáticas isoladas em todos os grupos estudados, quando os intervalos de referência laboratoriais foram utilizados como parâmetros. Estatisticamente, a associação halotano e paracetamol aumentou a AST e a ALT, mas não alterou a FA. Houve aumento da GGT, devido ao efeito isolado do halotano. Não houve aumento da necrose nem da reação inflamatória portal. O aumento da vacuolização citoplasmática foi devido ao efeito isolado do halotano. Houve aumento da congestão, da reação inflamatória lobular e da concentração plasmática de paracetamol. A associação isoflurano e paracetamol aumentou a ALT, mas não alterou a FA. Houve aumento da AST e da GGT, devido ao efeito isolado do isoflurano. Não houve aumento da necrose nem das reações inflamatórias lobular e portal. Houve aumento da vacuolização citoplasmática, devido ao efeito isolado do isoflurano. Houve aumento da congestão e da concentração plasmática de paracetamol. A associação sevoflurano e paracetamol não alterou as enzimas estudadas. Não houve aumento da necrose nem das reações inflamatórias lobular e portal. Houve aumento da vacuolização citoplasmática, devido ao efeito isolado do sevoflurano. Houve aumento da congestão, mas não da concentração plasmática de paracetamol / Abstract: Large doses ofparacetamol can cause hepatic toxic effects. Volatile anesthetics can also cause hepatotoxicity directly or through their metabolites. ln this study, we have tried to evaluate the effects of the association of volatile anesthetics, halothane, isoflurane and sevoflurane with paracetamol, in rats which were not submitted to hypoxia and remained with spontaneous ventilation. Ninety four adult male Wistar rats, weighing 260 - 290 g, were used. Thirty eight of them were used to estabilish reference values and intervals. The rest was divided into seven groups with eight rats each one. ln groups one, two and three the rats were anesthetized one hour a day, for tive consecutive days with one MAC (minimum alveolar concentration) ofhalothane, isoflurane and sevoflurane, respectively, vaporized in one hundred per cent oxygen. In groups faur, tive and six, afier one hour a day of anesthesia with halothane, isoflurane and sevoflurane (1 MAC), respectively, paracetamol (500 mg/Kg) Was administered with a stomach tube, for tive consecutive days. ln group seven, only paracetamol was administered (500 mg/kg) with a stomach tube for tive consecutive days. On the fifth day, afier the normal daily treatment, the rats were ~esthetized with thiopental sodium (20 mg/kg, intravenously). Afier laparotomy, three mililiters of blood were collected :ltom the aorta, for enzymatic (aspartate aminotransferase - AST , alanine aminotransferase - ALT, alkaline phosphatase - AP and gamma-glutamyltransferase - OGT) and paracetamol plasmatic doses. Then, the histological study was done. The clinical analyses ofthe results, showed the ocurrence af isolated enzymatic alterations in alI the groups, when the reference intervals were used as parameters. There was an increase in AST in groups one, two, four and tive. ln the saroe groups, GGT increased to the superior limit of the reference intervalo AL T increased in groups four, tive and seven, but AP did not altero StatisticalIy, the association of halothane and paracetamol increased the AST and ALT, but did not alter alkaline phosphatase. There was an increase in GGT due to the effect of just halothane. There was no increase of necrosis and portal intlammatory reaction. The increase of vacuolation was due to the effect of just halothane. There was also an increase in the congestion, lobular inflammatory reaction and in the paracetamol plasmatic concentration. The association ofisotlurane and paracetamol increased ALT, but did not alter aIkaline phosphatase. There was an increase in AST and in GGT due to the effect of only isotlurane. There was no increase of necrosis, lobular and portal inflammatory reactions. However, there was an increase of the vacuolation due to the effect of isotlurane. An increase of congestion and in the plasmatic concentration of paracetamol was observed. The association of sevotlurane and paracetamol caused no alteration in any of the enzymes studied. Nor was there any increase in necrosis, lobular and portal reactions. The increase in vacuolation was due to the effects of only sevotlurane. Comparação da umidade e temperatura dos gases inalados com alto ou baixo fluxo de gases frescos e com ou sem permutador de calor e umidade em anestesia pediátrica /

Como as crianças possuem menor volume minuto, a umidificação dos gases no sistema ventilatório circular pode ser menor na criança que no adulto. O permutador de calor e umidade (PCU) é dispositivo utilizado para umidificar e aquecer adicionalmente os gases inalados durante a ventilação artificial. Com o objetivo de avaliar as propriedades de umidificação de sistema ventilatório circular durante anestesia pediátrica, realizou-se estudo para comparar a temperatura e umidade dos gases inalados com baixo ou alto fluxo de gases frescos (FGF) e com ou sem PCU no circuito ventilatório. Quarenta crianças foram alocadas aleatoriamente em um de quatro grupos segundo a ventilação pulmonar com sistema circular com baixo (1L/min) ou alto (3L/min) FGF sem PCU (grupos 1L e 3L) ou com PCU (grupos PCU1L e PCU3L). A temperatura e UA dos gases inalados foram mensuradas durante 80 minutos após a conexão do paciente ao circuito ventilatório. As médias da temperatura do gás inalado foram maiores nos grupos que utilizaram PCU (PCU1L: 30,3 ± 1,1°C; PCU3L: 29,3± 1,2oC) quando comparadas às dos grupos que não utilizaram PCU (1L: 27,0 ± 1,2°C; 3L: 27,1 ± 1,5oC) (p < 0,0001). As médias de UA dos gases inalados foram maiores nos grupos com PCU em relação aos grupos sem PCU e maiores nos grupos de baixo FGF que nos grupos de alto FGF [(PCU1L: 25 ± 1 mg H20/L) > (PCU3L: 23 ± 2 mg H20/L) > (1L: 17 ± 1 mg H20/L) > (3L: 14 ± 1 mg H20/L)] (p<0,0001). Em sistema ventilatório circular pediátrico, o uso de alto ou baixo FGF não propicia umidificação mínima necessária dos gases inalados para redução do risco de desidratação das vias aéreas. A adição de PCU no circuito ventilatório aumenta a umidade ... / Abstract: An inhaled gas absolute humidity (AH) of 20 mg H2O.L-1 is the minimum value most considered as the threshold necessary for preventing the deleterious effects of dry gas on the epithelium of the airways during anesthesia. Because children have small minute ventilation, we hypothesized that the humidification of a circle breathing system is lower in children compared with adults. A heat and moisture exchanger (HME) is a device that can be used to further humidify inhaled gases during anesthesia. In order to evaluate the humidifying properties of a circle breathing system during pediatric anesthesia we compared the temperature and humidity of inhaled gases under low or high fresh gas flow (FGF) conditions and with or without an HME in the breathing circuit. Forty children were randomly allocated into one of four groups according to the ventilation of their lungs by a circle breathing system with low (1L.min-1) or high (3L.min-1) FGF without an HME (1L and 3L groups) or with an HME (HME1L and HME3L groups). The temperature and AH of inhaled gases were measured for 80 minutes after connecting the patient to the breathing circuit. The mean inhaled gas temperature was higher in HME groups (HME1L: 30.3 ± 1.1°C; HME3L: 29.3 ± 1.2°C) compared with no-HME groups (1L: 27.0 ± 1.2°C; 3L: 27.1 ± 1.5°C) (P < 0.0001). The mean inhaled gas AH was higher in HME than no-HME groups, and higher in low-flow than high-flow groups [(HME1L: 25 ± 1 mg H2O.L-1) > (HME3L: 23 ± 2 mg H2O.L-1) > (1L: 17 ± 1 mg H2O.L-1) > (3L: 14 ± 1 mg H2O.L-1)] (P < 0.0001). A análise dascitocinas foi feita pelo método da citometria de fluxo utilizando o kit BD Cytometric Bead Array. Observou-se diferença significativa em relação a IL-4 (p=0,02), maior entre os pacientes, e IL-5 (p<0,001), maior entre os voluntários. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Patients have stress when submitted to anaesthesia and surgery, characterized by inflammatory response because of the surgery trauma, and possibly due to the anaesthetics. Literature shows controversial data as most studies have been designed using large surgery models. Using minor surgical procedures, in order to reduce tissue injury influence on inflammatory response, this study aimed to evaluate the effects of anaesthesia with sevoflurane (SVF) on plasma cytokine profile from 15 ASA I patients of both genders, aged from 18 to 55, scheduled for tympanoplasty or septoplasty lasting, at least, 120 minutes, with 1.0 to 1.5 minimum alveolar concentration of SVF. Blood samples were drawn at 4 time points: before anaesthesia induction (T1 - baseline), before surgery (T2), at 120 minutes after anaesthesia induction (T3), and on the postoperative first day (T4). The study also included 15 healthy volunteers, not submitted to anaesthesia and surgery, as controls (C) to T1, and from whom blood sample was collected a single time. Plasma IL-1, IL-2, IL-4, IL-5, IL-6, IL- 8, IL-10, IL-12, TNF- e INF-) were assessed in plasma from patients and volunteers by flow cytometry, using the BD® Cytometric Bead Array kit.IL-4 (p=0,02) and IL-5 (p<0,001) differed between groups at T1. Tentativas precoces de retornar à posição quadrupedal são normalmente fracassadas podendo ocasionar quedas, injúrias e até mesmo fraturas. Técnicas para melhorar a qualidade de recuperação incluem o uso de agonistas adrenérgicos do tipo α2, os quais objetivam, neste contexto, a sedação do paciente, aumentando o tempo de decúbito e possibilitando maior tempo de eliminação do anestésico inalatório residual. O doxapram é um fármaco caracterizado por provocar aumento na frequência respiratória e, potencialmente, pode aumentar o clearance dos anestésicos inalatórios. Neste contexto, objetivou-se verificar a influência da administração dos fármacos xilazina e doxapram no tempo e na qualidade da recuperação de equinos submetidos à anestesia geral inalatória com isofluorano. Seis cavalos foram anestesiados durante 90 minutos quatro vezes, com intervalo de duas semanas entre os procedimentos, recebendo aleatoriamente os seguintes tratamentos após a interrupção do anestésico inalatório: xilazina 0,2 mg/kg, ou xilazina e doxapram (0,2 e 0,1 mg/kg, respectivamente), ou xilazina e doxapram (0,2 e 0,2 mg/kg, respectivamente), ou solução NaCl 0,9%. Early attempts to assume upright position are seldom successful and frequently result on falls, injuries and even fractures. Techniques to improve anesthetic recovery include the use of α2- adrenergic agonists at recovery in order to increase the recumbency time permitting clearance a broader residual inhalational anesthetic. Doxapram is characterized by increasing respiratory rate. Potentially it can increase inhaled anesthetics clearence. The purpose of the present study was to verify to influence of xylazine and doxapram administration on time and quality of recovery of horses submitted to isoflurane inhalational anesthesia. Six horses were anesthetized four times, with a wash out period of two weeks. Each animal received one of the following randomized treatments following discontinuation of isoflurane anesthesia: xylazine 0.2 mg/kg, or xylazine plus doxapram (0.2 and 0.1 mg/kg, respectively), xylazine plus doxapram (0.2 and 0.2 mg/kg, respectively), or NaCl 0.9% solution. Recovery was filmed and posterior evaluated by two blind observes by a visual analogue scale and a qualitative descriptive scale. The scores were analyzed for agreement by Bland-Altman and for the difference by RM ANOVA followed by Tukey test. The parameters measured were analyzed with RM ANOVA followed by Tukey test (p < 0.05). There was no difference regarding times recovery between groups. The animals who received just xylazine presented better scores on quality analysis, followed by doxapram 0.2 mg/kg group, control group, and 0.1 mg.kg of doxapram group. Como as crianças possuem menor volume minuto, a umidificação dos gases no sistema ventilatório circular pode ser menor na criança que no adulto. O permutador de calor e umidade (PCU) é dispositivo utilizado para umidificar e aquecer adicionalmente os gases inalados durante a ventilação artificial. Com o objetivo de avaliar as propriedades de umidificação de sistema ventilatório circular durante anestesia pediátrica, realizou-se estudo para comparar a temperatura e umidade dos gases inalados com baixo ou alto fluxo de gases frescos (FGF) e com ou sem PCU no circuito ventilatório. Quarenta crianças foram alocadas aleatoriamente em um de quatro grupos segundo a ventilação pulmonar com sistema circular com baixo (1L/min) ou alto (3L/min) FGF sem PCU (grupos 1L e 3L) ou com PCU (grupos PCU1L e PCU3L). A temperatura e UA dos gases inalados foram mensuradas durante 80 minutos após a conexão do paciente ao circuito ventilatório. As médias da temperatura do gás inalado foram maiores nos grupos que utilizaram PCU (PCU1L: 30,3 ± 1,1°C; PCU3L: 29,3± 1,2oC) quando comparadas às dos grupos que não utilizaram PCU (1L: 27,0 ± 1,2°C; 3L: 27,1 ± 1,5oC) (p < 0,0001). As médias de UA dos gases inalados foram maiores nos grupos com PCU em relação aos grupos sem PCU e maiores nos grupos de baixo FGF que nos grupos de alto FGF [(PCU1L: 25 ± 1 mg H20/L) > (PCU3L: 23 ± 2 mg H20/L) > (1L: 17 ± 1 mg H20/L) > (3L: 14 ± 1 mg H20/L)] (p<0,0001). A adição de PCU no circuito ventilatório aumenta a umidade... / An inhaled gas absolute humidity (AH) of 20 mg H2O.L-1 is the minimum value most considered as the threshold necessary for preventing the deleterious effects of dry gas on the epithelium of the airways during anesthesia. Because children have small minute ventilation, we hypothesized that the humidification of a circle breathing system is lower in children compared with adults. A heat and moisture exchanger (HME) is a device that can be used to further humidify inhaled gases during anesthesia. In order to evaluate the humidifying properties of a circle breathing system during pediatric anesthesia we compared the temperature and humidity of inhaled gases under low or high fresh gas flow (FGF) conditions and with or without an HME in the breathing circuit. Forty children were randomly allocated into one of four groups according to the ventilation of their lungs by a circle breathing system with low (1L.min-1) or high (3L.min-1) FGF without an HME (1L and 3L groups) or with an HME (HME1L and HME3L groups). The temperature and AH of inhaled gases were measured for 80 minutes after connecting the patient to the breathing circuit. The mean inhaled gas temperature was higher in HME groups (HME1L: 30.3 ± 1.1°C; HME3L: 29.3 ± 1.2°C) compared with no-HME groups (1L: 27.0 ± 1.2°C; 3L: 27.1 ± 1.5°C) (P < 0.0001). Oito bezerros receberam infusão contínua de lidocaína (GL) ou salina 0,9% (GC). Após MPA com xilazina 0,05 mg/kg/IV e indução anestésica com quetamina 2mg/kg associada com midazolam o,1mg/kg, com os bezerros em decúbito lateral procedeu-se a intubação e administrou-se isofluorano 1,3%. Subsequentemente institui-se a VPPI com PPI de 15 cmH2O e fR de 6mpm. Decorridos 40 minutos sob anestesia com isofluorano com ventilação mecânica, aplicou-se lidocaína 2mg/kg e iniciou-se infusão contínua na taxa de 100 µg/kg/minuto (GL). Anotaram-se as variações da FC, PA, fR, SpO2, BIS e TR antes da MPA (MB) e 15 minutos após a MBA (MX) antes da administração da lidocaína (M0) e em intervalos de 20 minutos após o início da infusão dos fármacos (M20, M40, M60 e M80). As demais variáveis foram mensuradas a partir de M0. Após o final da infusão e desconexão, foram avaliados o período de tempo para a adoção de decúbito esternal e posição quadrupedal. Também foi mensurada a concentração sérica da lidocaína. A infusão continua de lidocaína não alterou as variáveis ventilométricas, hemogasométricas, índice biespectral e recuperação, porém diminuiu FC e IC. Conclui-se que apesar de ter causado redução da atividade cardíaca, a infusão contínua de lidocaína pode ser empregada como técnica anestésica em bezerros / Abstract: Cardiorespiratory effects and bispectral index were analised after continuous rate infusion of lidocaine in calves isoflurane-anesthetized under controlled ventilation. Eight calves received continuous rate infusion of lidocaine (LG) or saline 0.9% (CG). After premedication with xylazine 0.05 mg kg -1 IV and induction of anesthesia with ketamine 2 mg kg -1 associated with the midazolam, 1 mg kg -1 , with the calves in lateral recumbence preceded the intubation and was administered isoflurane 1.3%. IPPV was instituted with PIP of 15 cmH2O and RR of 6 beats min -1 . After 40 minutes with isoflurane anesthesia and mechanical ventilation was applied lidocaine 2 mg kg -1 and continuous rate infusion was started at a rate of 100 mg kg -1 min -1 (LG). Were recorded variations in HR, AP, fR, SPO2, BIS and RT before administration of premedication (MB) and 15 minutes after administration (MX) before administering lidocaine (M0) and every twenty minutes after the start of the infusion of drugs (M20, M40 M60 and M80).The other variables were measured from M0. After the end of infusion and disconnection, were evaluated the time to adopt sternal recumbency and standing position. Also measured the serum concentration of lidocaine. Continuous rate infusion of lidocaine did not change the ventilometry, blood gas, bispectral index and recovery, but decreased HR and CI. Oito cães da raça Beagle foram anestesiados em múltiplos de CAM correspondentes a 1,5 (G1,5) e 2,0 (G2,0) CAM por duas horas, sendo mensuradas a cada 20 minutos as variáveis frequência cardíaca (FC), pressão sistólica (PAS), média (PAM) e diastólica (PAD), pressão da artéria pulmonar média (PAPm), pressão capilar pulmonar média (PCPm), pressão venosa central (PVC), índice cardíaco (IC), índice sistólico (IS), índice de trabalho ventricular esquerdo (ITVE), índice de resistência periférica total (IRPT), índice de resistência vascular pulmonar (IRVP), índice de oferta de oxigênio (IDO2), índice de consumo de oxigênio (IVO2), taxa de extração de oxigênio (TeO2), saturação venosa mista de oxigênio (SvO2) e lactato sanguíneo arterial. As médias de PAM, IC, IS e ITVE, diferiram significativamente entre os grupos, o que não foi observado nas variáveis IRPT e IRVP, as quais apresentaram diferença somente entre momentos de um mesmo grupo. Da mesma forma, houve diminuição progressiva da IRPT em G1,5, mas não em G2,0. Houve redução de IDO2 em G2,0 quando comparada a G1,5, mas não de IVO2. O lactato apresentou valores acima dos limites de referência, não diferindo entre grupos e reduzindo-se ao longo do tempo, enquanto que SvO2 foi menor em G2,0, acompanhada de maior TeO2 neste grupo. Concluiu-se que a perfusão global é prejudicada por concentrações maiores de isofluorano, ao passo que a perfusão periférica é mantida. Existe um limite para o grau de vasodilatação periférica causada pelo isofluorano em cães e o tempo de exposição ao anestésico interfere com a dinâmica cardiovascular tanto quanto sua concentração / The aim of this study was to evaluate the effects of isoflurane and anesthetic time period on tissue perfusion of dogs under two different concentrations. Eight beagle dogs were anesthetized in MAC multiples correspondent to 1.5 (G1.5) e 2.0 (G2.0) MAC for two hours, being the following variables collected every 20 minutes: heart rate (HR), arterial systolic (SAP), mean (MAP) and diastolic (DAP) pressures, pulmonary artery mean pressure (PAPm), pulmonary artery wedge pressure (PAWP), central venous pressure (CVP), cardiac index (CI), systolic index (SI), left ventricle work index (LVWI), total peripheral resistance index (TPRI), pulmonary vascular resistance index (PVRI), oxygen delivery index (IDO2), oxygen consumption index (IVO2), oxygen extraction (TeO2), mixed venous oxygen saturation (SvO2) and arterial blood lactate. Mean values of MAP, CI, SI and LVWI were different between groups, and such differences were not observed in TPRI and PVRI, which differences were only significant between time points in each group. In addition, there was progressive reduction of TPRI in G1.5, but not G2.0. There was a reduction of IDO2 in G2.0 when compared to G1.5, but not IVO2. Lactate levels were above reference limits, with no difference between groups and decreasing over time, whereas SvO2 was lower in G2.0, accompanied by higher TeO2 in that group. We conclude that global perfusion is impaired by higher concentrations of isoflurane, while peripheral perfusion is maintained. There is a limit to the degree of vasodilation caused by isoflurane in dogs and the time of exposure to the anesthetic interferes with cardiovascular dynamics as much as its concentrations
