Custo-efetividade da estimulação ovariana mínima versus estimulação ovariana convencional em pacientes más respondedoras submetidas à fertilização in vitro

Introdução: com o adiamento da maternidade está aumentando a necessidade e busca por serviços de Reprodução Assistida. No entanto, esses tratamentos ainda são bastante onerosos e acabam, muitas vezes, sendo inacessíveis para grande parte da população. Objetivo: avaliar o custo-efetividade da estimulação ovariana mínima versus estimulação convencional, no que se refere à taxa de gestação por transferência de embrião, em pacientes más respondedoras submetidas à Fertilização in vitro (FIV). Delineamento: estudo de coorte prospectiva. Método: foram incluídas no estudo, 101 pacientes - classificadas como más respondedorasque realizaram FIV no período de dezembro de 2016 a setembro de 2017 no Centro de Reprodução Humana Insemine e no Projeto Maternus – Porto Alegre. Dessas 101 pacientes, 9 foram excluídas por apresentarem 35 anos de idade ou menos. Cinqüenta e sete pacientes foram submetidas à estimulação ovariana convencional e 35 à estimulação mínima. Resultados: Todos os resultados serão expressos em média e desvio-padrão (DP). Não houve diferença estatisticamente significativa entre o grupo da estimulação ovariana convencional e o da estimulação mínima no que se refere à idade da paciente (40,4 ± 2,8 versus 41,3 ± 2,4, respectivamente, p= 0,112), ao índice de massa corporal (24,05 ± 2,7 versus 24,11 ± 2,9, p=0,918) e ao valor do Hormônio Anti-Mulleriano (0,58 ±0,7 versus 0,81 ± 0,96, p=0,198). Em relação à dose total de gonadotrofinas utilizada, o grupo da estimulação convencional utilizou em média 2628UI ± 788,37 por ciclo enquanto que as pacientes do grupo da estimulação mínima usaram em média 409UI ± 495,60, apresentando uma diferença estatisticamente significativa (p=0,001). O número de oócitos capturados na punção ovariana foi significativamente maior no grupo da estimulação convencional (2,04 ± 1,8) quando comparado ao grupo da estimulação mínima (1,09 ± 1,2), p= 0,003. O mesmo ocorreu com o número total de embriões, 1,26 ± 1,3 versus 0,54 ± 0,9, p= 0,002, respectivamente. O grupo da estimulação convencional teve um custo total com medicação para indução de ovulação de R$ 3.932,25, enquanto que o grupo da estimulação mínima, de R$ 858,05. A taxa de gestação por transferência embrionária foi de 17,9% versus 8,3%, nos grupos de estimulação 9 tradicional e mínima, respectivamente (p=0,423). O risco relativo (RR) foi de 0,46, mostrando que o grupo da estimulação convencional apresentou 2,1 vezes mais gravidez por transferência embrionária que o da estimulação mínima, mas com um custo de gonadotrofinas 4 vezes maior. Dessa forma, o custo de uma gestação com estimulação tradicional por ciclo iniciado foi de R$ 31.458,00 e no grupo da estimulação ovariana mínima, cada gestação custou R$ 27.457,60. O incremento do custo-efetividade foi de R$32.791,47, ou seja, esse é o custo para se ter um desfecho de gestação a mais. Extrapolando nossos dados para o estado do Rio Grande do Sul (RS), numa tomada de decisão para o gasto com medicação em pacientes más respondedoras teríamos, se adotada estimulação convencional, 608 gestações com um custo total de R$ 13.318.531,00, enquanto que se usássemos a estimulação mínima, teríamos 282 gestações totalizando um custo de R$ 2.906.215,00. Conclusão: Embora o RR de gestação após o uso de tratamento convencional para induzir a ovulação em mulheres submetidas à FIV seja de 2,1, houve um gasto com medicação 4 vezes maior. Esses achados podem auxiliar na tomada de decisões, sejam elas de cunho individual ou de estratégias em saúde pública. Considerando o custo-efetividade (C/E) de se utilizar ciclos de estimulação mínima, em pacientes más respondedoras, um maior número de mulheres pode ser tratado e, consequentemente, pode-se obter um número maior de gestações, apesar da menor taxa de gravidez por transferência embrionária. / Introduction: with the delay of maternity, the need and demand for assisted reproduction services have been on the rise. However, these treatments are still quite expensive and often are not available to a large part of the population. Aim: to assess the cost-effectiveness of minimal ovarian stimulation versus conventional stimulation regarding the rate of pregnancy due to embryo transfer in poor responders submitted to in-vitro fertilization (IVF). Study Design: prospective cohort study. Method: 101 patients -classified as poor responders - who underwent IVF from December 2016 to September 2017 at Centro de Reprodução Humana Insemine and Maternus Project, in Porto Alegre, were included in the study. Of these 101 patients, nine were excluded because they were 35 years old or younger. Fiftyseven patients were submitted to conventional ovarian stimulation and 35 to minimal stimulation. Results: All results were expressed as mean and standard deviation. There was no statistically significant difference between the conventional ovarian stimulation and the minimal stimulation groups regarding patient age (40.4 ± 2.8 versus 41.3 ± 2.4, respectively, p = 0.112), body mass index (24.05 ± 2.7 versus 24.11 ± 2.9, p = 0.918) and anti-müllerian hormone value (0.58 ± 0.7 versus 0.81 ± 0.96, p = 0.198). Regarding the total dose of gonadotropins used, the conventional stimulation group took on average 2628.07 IU ± 788.37 per cycle, while the patients in the minimal stimulation group took on average 409.29 IU ± 495.60, showing a statistically significant difference (p = 0.001). The number of oocytes captured by ovarian puncture was significantly higher in the conventional stimulation group (2.04 ± 1.8) when compared with the minimal stimulation group (1.09 ± 1.2), p= 0.003. The same happened to the total number of embryos, 1.26 ± 1.3 versus 0.54 ± 0.9, p= 11 0.002, respectively. The conventional stimulation group had a total cost with ovulation inducing drugs of BRL 3,932.25, while for the minimal stimulation group it was BRL 858.05. The embryo transfer pregnancy rate was 17.9% versus 8.3% in the traditional and minimal stimulation groups, respectively (p=0.423). The relative risk was 0.46, showing that the conventional stimulation group had 2.1 times as many embryo transfer pregnancies as minimal stimulation, but at a four-fold cost for gonadotropins. Thus, the cost of a traditional stimulation pregnancy by initiated cycle was BRL 31,458.00, and in the minimal ovarian stimulation group, each pregnancy cost BRL 27,457.60. The increase in cost-effectiveness was BRL 32,791.47, that is, this is the cost for an additional pregnancy outcome. Extrapolating our data to the state of Rio Grande do Sul (south of Brazil), in a decision to spend on medication in poor responders; the choice of a conventional stimulation would reach a total of 608 pregnancies with a total cost of BRL 13,318,531.00. However, using minimal stimulation, we would have 282 pregnancies with a cost of BRL 2,906,215.00. Conclusion: Although the RR of pregnancies after use of conventional treatment to induce ovulation in women submitted to IVF was 2.1, there was an expense with drugs 4 times greater. These findings can help in a decision making, either individually or in public health strategies. Considering the cost-effectiveness of using minimal stimulation protocol in patients with poor responders, a greater number of women could be treated and, consequently, a greater number of pregnancies achieved, in spite of the lower embryo transfer pregnancy rate.

Procedimento para análise de decisão quanto à prevenção de doenças em animais: uma aplicacão da Teoria dos Jogos / Procedure for decision analysis on prevention against animal diseases: an application of Game Theory

A hipótese básica do presente trabalho é que embora a ocorrência de uma determinada doença possa apresentar impactos econômicos expressivos para a pecuária de um país - onerando não apenas a atividade produtiva - alguns produtores não reconhecem, individualmente, a importância devida em adotar medidas preventivas, uma vez que a ocorrência da doença não é frequente. Ademais, a existência de externalidades faz com que a ação de um produtor em relação às medidas sanitárias de seu rebanho afete os demais produtores da mesma região, o que também pode não ser percebido por eles. Diante disso, o presente trabalho desenvolve um procedimento teórico para a análise de decisões estratégicas dos produtores quanto à prevenção contra doenças em animais, face ao risco de contaminação de seu rebanho. O desenvolvimento do modelo é baseado nos preceitos da Teoria dos Jogos, e na análise econômica de benefício/custo como suporte da tomada de decisão do produtor. Depois de modelado o problema e determinados os possíveis equilíbrios do jogo, infere-se sobre as possíveis atuações do governo, no uso de incentivos econômicos (como indenizações e multas) para que o equilíbrio seja o da prevenção. Em seguida, o modelo teórico é aplicado a um caso específico de surto de febre aftosa no Brasil. Os resultados mostram que quando não se tem um mercado diferenciado para animal não vacinado, o equilíbrio do jogo tende para a situação onde os produtores decidem vacinar seu rebanho, sugerindo que não existe a necessidade de intervenção do governo para estimular a prevenção. Na prática, no entanto, o governo brasileiro estabelece políticas de incentivo, visto que alguns produtores não vacinam seu rebanho mesmo conhecendo os riscos, o que se traduz na falta de racionalidade dos mesmos. Considerando essa irracionalidade por parte de alguns produtores, o presente estudo é importante para que estes possam delinear panoramas de decisão, de forma a identificar as melhores ações a serem tomadas de forma coletiva. / The basic hypothesis of this study is that producers do not perceive, individually, the importance of adopting prevention measures, even when aware of the economic risks for them and for the local economy, because the outbreaks of some diseases are not frequent. Moreover, the presence of externalities is such that the action of one producer regarding sanitary measures of the herd affects other producers in the same region, which also cannot be perceived by them. This study aims to develop a theoretical procedure to infer about strategic decisions taken by producers to prevent animal disease in their herds, in face of the risk of contamination. The development of the model is based on the Game Theory, and the benefit cost analysis as support for producer\'s decision. After modeling the problem and determining the equilibriums, these are used to elaborate inferences about possible actions of the government through economic incentives (such as indemnities and fines) to encourage the prevention. Then, the theoretical model is applied to a specific case of a foot-and-mouth disease (FMD) outbreak in Brazil. The results indicate that in the absence of a differential market for non-vaccinated animals, the game equilibrium tends to the situation where producer\'s decisions are to vaccinate their herd, suggesting that government intervention is not necessary. Even though, the Brazilian government uses incentive policies for the cattle vaccination since some producers do not vaccinate their animals, despite of their awareness of the risks, which suggests lack of rationality. Considering the irrationality of some producers, this study is important for them to delineate decisions scenes, identifying the best actions to be taken in the collective context.

Travel time savings are usually the most important utility component of transport projects. This paper shows estimation of the value of travel time savings (VTTS) with particular focus on random parameter models. Employing these models commuters’ VTTS in the German City Dresden is estimated. Results show that travel time savings of motorised and non-motorised modes are valued differently, VTTS derived in this study are higher than values currently used in Germany’s federal transport infrastructure plan and VTTS are dependent on travel time.

近年來,電動車與電動機車發展及應用儼然已成為世界潮流,在機車密度極高且擁有「機車王國」稱號的台灣,電動機車技術日新月異,也發展出多樣化商業模式,其中最著名莫過於電動機車電池租賃商業模式,以及共享商業模式。 本研究旨在利用成本效益分析中的淨現值法及益本比法,模擬分析機車使用者以電動機車代替傳統燃油機車,為使用者自己及整體社會帶來之淨現值。在電動機車方面,本研究分析兩種電動機車商業模式,分別為「電動機車電池租賃商業模式」及「共享電動機車商業模式」。 本研究結果顯示,在使用者立場下,目前電動機車成本依舊大於傳統125C.C.燃油機車之成本,且「電動機車電池租賃商業模式」較適合每個月騎乘里程較長之使用者,而「共享電動機車商業模式」較適合每個月騎乘里程較短之使用者。此外,敏感度分析顯示出,在「電動機車電池租賃商業模式」下,騎乘里程長度與NPV及BCR均呈現正相關。在「共享電動機車商業模式」下,每個月騎乘里程為100公里之使用者,在共享電動機車計價方式為每分鐘2.25元之方案下,使用者之NPV>0及BCR>1,並且騎乘里程越短之使用者,其對共享電動機車計價變動的益本比敏感程度越高(當價格下降時,益本比上升較高)。 最後,在整體社會立場下,利用「電動機車電池租賃商業模式」替換傳統燃油機車且騎乘里程越高之使用者,對整體社會帶來之淨現值越高。而利用「共享電動機車商業模式」替換傳統燃油機車且騎乘里程越低之使用者,越具有經濟效益。 / In recent years, the development and application of electric vehicles and electric scooters have become popuplar. In Taiwan, where scooter density is very high and is also called a "scooter kingdom", electric scooter technology is not only improving, but also developing a diversified business model. Particularly, two of the most famous business models are “Electric scooter battery rental business model” and “Sharing electric scooter business model”. The purpose of this study is to use the net present value method and the benefit ratio method in the cost-benefit analysis. We analysis the user's own benefits and the overall social net benefits which are generated from the scooter users replacing traditional fuel scooter with electric scooter. In the field of electric scooters, this study analyzes two business models of electric scooters, “Electric scooter battery rental business model” and “Sharing electric scooter business model”. According to the simulation result of empirical analysis, for the users, the current cost of electric scooters is still higher than the cost of traditional 125C.C. fuel scooters. “Electric scooter battery rental business model” is more suitable for people who have higher accumulated distance per month, and “Sharing electric scooter business model” is more suitable for people who have lower accumulated distance per month . On the other hand, according to the result of sensitivity analysis, the accumulated distance is positively correlated with NPV and BCR in the “Electric scooter battery rental business model”. In the “Sharing electric scooter business model”, those who ride 100 km per month have NPV> 0 and BCR> 1 in the pricing of NT$2.25 per minute. Besides, those who ride 100 km per month have higher sensitivity of pricing. For the overall society, those who use the "Electric scooter battery rental business model" to replace the traditional fuel scooter and have higher accumulated distance per month can generate higher net benefits to the whole society. Those who use the "Sharing electric scooter business model" to replace the traditional fuel scooter and have lower accumulated distance per month can generate higher net benefits to the whole society.

The test was conducted at Agrícola Famosa, municipality of Tibau/RN, in a split-plot, distributed in a randomized block design with four replications. The main plots received two tillage systems (direct and conventional) and the subplots received twelve combinations of plants for green manure (Crotalaria; Millet; Sunnhemp + millet, more braquiaria Corn, spontaneous vegetation + compound more polyethylene film, Solo maintained without vegetation ; spontaneous vegetation; Guandu; Guandu + millet; Pork Beans, Beans pig-+ millet and soil without removing the cover (natural vegetation) + corn + Braquiaria + polyethylene film), incorporated (conventional tillage) or maintained on the soil (tillage). In each subplot, the dry weight of biomass yield was evaluated, using a framework of 0.25 m². In the first stage of the experiment, using the data obtained from melon, we determined the number of fruits per plant; average fruit weight and total and commercial productivity for export, besides the remaining dry mass of plant ground covers (straw). In the second stage, we evaluated the quality characteristics of fruits of melon (number of fruits per plant, average mass of fruits, longitudinal and transverse lengths, pulp thickness, firmness, pH, soluble solids (SS), acidity total (AT) and soluble / total acidity solid ratio (SS/TA) and total sugars) cultivated with different incorporated green manures or maintained on the ground in conventional tillage systems and direct, respectively. In the third step, we assessed the cost of production of yellow melon in each combination of plants to incorporated green manure or maintained on the ground, in conventional and no-till systems. The cost for cultural practice were estimated from the technical coefficients (inputs and operations) recorded in the experiment and extrapolated to one hectare. We considered economically viable those cultural practices which obtained a favorable benefit/cost to 1.0. It was found that the highest production of dry straw was obtained in treatments with more braquiaria followed corn millet consortium + Crotalaria and the single millet, which also showed low rate of decomposition. There was no variation between green manures incorporated on the production of melons. The no-tillage system produced higher number of fruits per plant, however smaller. The conventional tillage system presented higher yield compared to tillage. In the second trial, it was found that no-till system kept most of the qualitative characteristics of melon fruit in comparison to conventional tillage and no variation between most of the qualitative characteristics of melon fruit recommended for the green manure. With regard to production costs, treatments at the till system showed lower cost compared to conventional tillage due to the cost of polyethylene film and plowing and harrowing operations and placing the film on conventional tillage. All cultural practices in both tillage systems were considered economically viable, because they have a favorable benefit/cost of more than 1.0. However, the treatments with the production of corn for the production of green ears, intercropped with Braquiaria, stood out the best benefit / cost ratio due to the increase of revenue from the sale of green ears, in both tillage systems. The best benefit/cost ratio was observed in the treatments with conventional tillage due to the lower productivity of melon at the till / O presente trabalho teve o objetivo de avaliar a produtividade, o custo de produção e a qualidade dos frutos da cultura do melão amarelo com diferentes espécies de plantas destinadas à adubação verde, incorporadas ou mantidas sobre o solo nos sistemas de plantio convencional e direto, respectivamente. O ensaio foi conduzido na Fazenda Agrícola Famosa, município de Tibau/RN, no esquema de parcelas subdivididas, distribuídas no delineamento experimental em blocos casualizados, com quatro repetições. Nas parcelas, foram avaliados dois sistemas de plantio (direto e convencional) e nas subparcelas, doze combinações de plantas para adubação verde (Crotalária; Milheto; Crotalária + milheto; Milho mais braquiária; Vegetação espontânea + composto mais filme de polietileno; Solo mantido sem vegetação; Vegetação espontânea; Guandu; Guandu + milheto; Feijão de porco; Feijão-de-porco + milheto e Solo sem retirar a cobertura (vegetação natural) + milho + braquiária + filme de polietileno), incorporadas (plantio convencional) ou mantidas sobre o solo (plantio direto). Em cada subparcela, foi avaliado o rendimento de massa seca da parte aérea, utilizando-se um quadro de 0,25 m². Na primeira etapa do experimento, a partir dos dados obtidos com o melão, determinou-se o número de frutos por planta; peso médio de frutos e produtividade total e comercial para exportação, além da massa seca remanescente das plantas de coberturas do solo (palhada). Na segunda etapa, avaliaram-se as características qualitativas dos frutos de melão (número de frutos por planta, massa media dos frutos, comprimentos longitudinal e transversal; espessura de polpa; firmeza da polpa, pH, teor de sólidos solúveis (SS), acidez total (AT) e relação sólidos solúveis/acidez total (SS/AT) e açúcares totais) cultivado com diferentes adubos verdes incorporados ou mantidos sobre o solo nos sistemas de plantio convencional e direto, respectivamente. Na terceira etapa, avaliou-se o custo de produção do melão amarelo em cada combinação de plantas para adubação verde incorporadas ou mantidas sobre o solo, nos sistemas de plantio convencional e direto. Os custos por prática cultural foram estimados a partir dos coeficientes técnicos (insumos e operações) registrados no experimento e extrapolados para um hectare. Foram consideradas como viáveis economicamente aquelas práticas culturais que obtiveram relação benefício/custo superior a 1,0. Verificou-se que a maior produção de matéria seca de palhada foi obtida nos tratamentos com milho mais braquiária seguidos do consórcio milheto + crotalária e o milheto solteiro, que também apresentaram menor taxa de decomposição. Não houve variação entre os adubos verdes incorporados sobre a produção de melão. O sistema de plantio direto produziu maior número de frutos por planta, porém de menor tamanho. O sistema de plantio convencional apresentou maior produtividade em relação ao plantio direto. No segundo ensaio, verificou-se que o sistema de plantio direto manteve a maioria das características qualitativas dos frutos de melão em relação ao plantio convencional e não houve variação entre a maioria das características qualitativas dos frutos de melão para os adubos verdes avaliados. Com relação aos custos de produção, os tratamentos no sistema de plantio direto apresentaram menor custo em relação ao plantio convencional, em decorrência do custo do filme de polietileno e das operações de aração e gradagem e colocação do filme de polietileno no plantio convencional. Todas as práticas culturais nos dois sistemas de plantio foram consideradas viáveis economicamente, por apresentarem uma relação benefício/custo superior a 1,0, porém os tratamentos com a produção de milho, destinado à produção de espigas verdes, consorciado com braquiária, se destacaram pela melhor relação benefício/custo, em função do incremento da receita com a comercialização das espigas verdes, nos dois sistemas de plantio. A melhor relação benefício/custo foi verificada nos tratamentos com plantio convencional em razão da menor produtividade do melão no plantio direto

This thesis deals with the issue of assessing the effectiveness of public investment. The first part of the thesis is theoretical. The life cycle of the building is defined in the introduction. The feasibility studies and methods for evaluating the effectiveness of public projects are subsequently described. The practical part of the thesis is aimed at assessing the effectiveness of the public investment project. The evaluated project is the building of a kindergarten in a small village. The eCBA application was used to evaluate the efficiency. A quantitative analysis of the risks related to the project is carried out at the end of this thesis.

Annotation This Master’s thesis deals with obtaining grants from EU funds that are specified for railroad development in the Czech Republic – modernization, reconstruction, interconnection and making the railroad traffic accessible for the whole Europe. In parlance of the Czech Republic the word modernization means speeding up the trains. In my Master’s thesis I would like to describe you the way how to evaluate the efficiency of the project “Reconstruction of Railroad Station in Olomouc” by means of main economic indexes (IRR, BCR, NPV) and further familiarize you with the Benefit 7+ form used for raising money from EU structural funds. I also would like to introduce you the procedure of elaborating required scheme used in economic evaluation – cost and benefits analysis (CBA). This scheme is absolutely necessary in order to get the grants from abovementioned EU structural funds concerning any investment project in the Czech Republic.

Water surface profile modelling for Pinjarra flood diversion channel and economic evaluation

Shire of Murray has concerns regarding the negative impact that a 100 year flood could have on existing structures built before 1997. The increase cost in construction due to landfill has an adverse effect on development in Pinjarra. Feasibility of constructing a diversion channel at upstream of Murray River to attenuate the flood level from 1 in 100 year ARI to 1 in 50 year ARI, was investigated by Kiong (2003). The Murray River Water Surface Profile along three kilometres south of Greenlands Road was modelled. Flood damages on each flood occurrence were assessed and Average Annual Damage (AAD) was calculated. The AAD is used to estimate the monetary benefit against the construction cost of the diversion channel. Groundwater along Greenlands and Fauntleroy Drains was also modelled to determine the viable depth of the designed channel, as well as the analysis of backwater. The proposed channel is designed at different scenarios (invert level at breakout point, culvert or causeway design, and diversion channel variations). The benefit cost ratio of the proposed diversion channel is calculated. Other mitigation options are suggested including detention basins for structural measure, or building a new flood-proof township for non-structural measure.

An explorative study of the technology transfer coach as a preliminary for the design of a computer aid

The university technology transfer coach has an important role in supporting the commercialization of research results. This thesis has studied the technology transfer coach and their needs in the coaching process. The goal has been to investigate information needs of the technology transfer coach as a preliminary for the design of computer aids.Using a grounded theory approach, we interviewed 17 coaches working in the Swedish technology transfer environment. Extracted quotes from interviews were openly coded and categorized. The analysis show three main problem areas related to the information needs of the technology transfer coach; awareness, communication, and resources. Moreover, 20 features for future computer aids were extracted from the interview data and scenarios and personas where developed to exemplify the future use of computer aids.We conclude that there is a need for computer support in the coaching process. Such systems should aid the coach in; awareness, aiding the coach to focus on meetings; communication, aid the coach to transfer commercialisation knowledge; and resources, aid the coach in accessing and delivering of resources to the coachee. However, it is imperative that the computer aids do not interfere with the coach current process; and that the computer aid is not seen as the sole solution.
