Alterações tomográficas e funcionais pulmonares em pacientes com hipogamaglobulinemia primária em reposição de imunoglobulina humana / Pulmonary tomographic and functional abnormalities in patients with primary hypogammaglobulinemia receiving human immunoglobulin replacement

Introdução: A Agamaglobulinemia, a Imunodeficiência Comum Variável (IDCV) e a Síndrome Hiper IgM (SHIGM) são imunodeficiência primárias predominantemente humorais que se beneficiam da reposição de imunoglobulina humana, com redução da morbimortalidade. No entanto, apesar da reposição adequada de imunoglobulina, complicações pulmonares podem ocorrer, influenciando o prognóstico destes pacientes. Objetivo: O objetivo do estudo foi descrever as alterações morfológicas e funcionais pulmonares em pacientes com hipogamaglobulinemia primária em tratamento com reposição de imunoglobulina humana. Métodos: Foram avaliados 30 pacientes (agamaglobulinemia n=14; IDCV n=9; SHIGM n=7) que receberam imunoglobulina e antibioticoterapia profilática regularmente. A avaliação utilizou dados dos prontuários sobre o início e a evolução da doença, bem como dados de espirometria e tomografia computadorizada de tórax. O escore de Bhalla foi aplicado à tomografia mais recente de cada um dos pacientes para correlacionar as alterações tomográficas pulmonares com os dados clínicos, resultados das espirometrias e ocorrência de processos infecciosos sino-pulmonares após o início da reposição de imunoglobulina. Para as análises estatísticas utilizou-se o programa SPSS 13.0, e valores de p<0,05 foram considerados estatisticamente significantes. As variáveis nominais foram comparadas através do teste de Fisher e as contínuas, através de testes não paramétricos (Mann-Whitney, Kruskal- Wallis e Wilcoxon). Para as correlações do escore de Bhalla com as demais variáveis foi utilizado o coeficiente de Spearman. Resultados: Houve diminuição na frequência de pneumonias (p<0,001) e aumento na frequência de sinusites (p<0,001) após o início da reposição de imunoglobulina. Distúrbios ventilatórios foram evidenciados em 14 dos 23 pacientes que puderam realizaram espirometria (7 obstrutivos, 5 restritivos e 2 inconclusivos). Pacientes com bronquiectasias ao diagnóstico e aqueles à primeira avaliação tomográfica apresentaram mediana de idade mais elevada ao diagnóstico (p=0,015 e p=0,001, respectivamente) e tempo mais prolongado entre o início dos sintomas e o diagnóstico que aqueles sem bronquiectasias (p=0,010 e p=0,001, respectivamente). Sete pacientes desenvolveram bronquiectasias durante o tratamento. Pacientes com bronquiectasias à avaliação final apresentaram maior frequência de sinusites antes do início da reposição de imunoglobulina que aqueles sem bronquiectasias (p=0,010). Houve correlação estatisticamente significante do escore de Bhalla com VEF1 pré e pós-broncodilatador (r= -0,778 e r= -0,837, respectivamente), CVF (r= -0,773), FEF25-75% (r= -0,571) e com a frequência de pneumonias após o início do tratamento (r= 0,561). Conclusões: O tratamento com reposição regular de imunoglobulina e antibioticoterapia profilática reduziu a frequência e gravidade das infecções pulmonares, porém não evitou a ocorrência de sinusites, o aparecimento de bronquiectasias nem de outras alterações morfológicas e funcionais pulmonares / Introduction: Agammaglobulinemia, Common Variable Immunodeficiency (CVID) and Hyper IgM Syndrome (HIGM) are predominantly antibody deficiencies that benefit from immunoglobulin replacement therapy, with reduction of their morbidity and mortality. Despite regular immunoglobulin replacement, pulmonary complications may occur in those patients, affecting their prognosis. Objective: The aim of this study was to describe tomographic and functional pulmonary abnormalities in patients with primary hypogammaglobulinemia receiving immunoglobulin replacement therapy. Methods: Thirty patients (agammaglobulinemia n=14, CVID n=9, HIGM n=7) receiving antimicrobial prophylaxis and regular immunoglobulin infusions were evaluated. Clinical records were reviewed to obtain data concerning the onset and evolution of the disease and the results of spirometry and computed tomography of the chest. Bhalla score was applied to the most recent tomography of each patient to correlate tomographic pulmonary abnormalities with clinical data, spirometry results and the occurrence of sinusal and pulmonary infections after the onset of the immunoglobulin replacement. Statistical analysis was performed using the software SPSS 13.0 and p values < 0.05 were interpreted as statistically significant. Nominal variables were tested using Fisher´s exact test and continuous variables were tested using non-parametric tests (Mann-Whitney, Kruskal-Wallis e Wilcoxon). Spearman coefficient was used to correlate Bhalla score with the other variables. Results: The frequency of pneumonias decreased (p<0.001) and the frequency of sinusitis increased (p<0.001) after the onset of immunoglobulin replacement. Pulmonary function was abnormal in 14 of 23 patients (7 obstructive, 5 restrictive, 2 inconclusive). Patients with bronchiectasis at diagnosis and those with bronchiectasis at the first tomographic evaluation presented higher median age at diagnosis (p=0.015 and p=0.001, respectively) and longer duration between the onset of symptoms and diagnosis than those without bronchiectasis (p=0.010 e p=0.001, respectively). Seven patients developed bronchiectasis during treatment. Patients with bronchiectasis at the last tomographic evaluation presented a higher frequency of sinusitis before therapy onset than those without bronchiectasis (p=0.001). Statistically significant correlation was found between Bhalla score and pre and post bronchodilator FEV1 (r= -0.778 and r= -0.837, respectively), FVC (r= -0.773) and FEF25-75% (r= -0.571) and between Bhalla score and the frequency of pneumonias after the onset of immunoglobulin replacement therapy (r=0.561). Conclusions: Immunoglobulin replacement therapy and antimicrobial prophylaxis reduced the frequency and severity of pulmonary infections but did not prevent the occurrence of sinusitis, the development of bronchiectasis or other morphological and functional pulmonary abnormalities

Efeitos agudos do flutter e da compressão torácica sobre a remoção de secreção e a impedância do sistema respiratório em pacientes com bronquiectasias / Acute flutter effects and thoracic compression over the secretion removal and impedance of the respiratory system in patients with bronchiectasis

Introdução: As bronquiectasias são caracterizadas patologicamente pela dilatação anormal e permanente dos brônquios, causadas, principalmente, pela perpetuação de processos inflamatórios e prejuízo da remoção de secreções. A fisioterapia é parte fundamental do tratamento destes pacientes, utilizando-se de suas diversas técnicas e dispositivas. Objetivo: Avaliar o efeito das técnicas flutter e compressão torácica sobre a remoção de secreção e alterações na resistência total do sistema respiratório em indivíduos com bronquiectasias. Métodos: Em estudo cruzado, randomizado e cego, indivíduos com bronquiectasias e saudáveis foram avaliados antes e após sessões de controle, exercícios com flutter, ou manobras de compressão torácica (CT). Foram avaliados o peso seco, peso total, adesividade e purulência das secreções expectoradas. Para impedância do sistema respiratório foi utilizado a oscilometria de impulso, que analisou a resistência a 5Hz (R5), reactância a 5Hz (X5) e frequência de ressonância (Fres). Adicionalmente, foram avaliados escala de dispneia do Medical Research Council (MRC); escala de aceitabilidade e tolerância; oximetria de pulso; dificuldade de expectoração e Índice de Severidade de Bronquiectasias (BSI). Resultados: Flutter e CT foram eficazes para remoção de secreção comparada a controle (p>0,05). A variação da resistência total do sistema respiratório foi favorável para técnica flutter no grupo bronquiectasias quando comparada ao grupo controle (p< 0,05). Ambas as técnicas foram bem aceitas pelos participantes. Conclusão: Os resultados sugerem que as técnicas flutter e compressão torácica são eficazes para a remoção de secreção, seguras e bem toleradas por pacientes com bronquiectasias, com efeito agudo na resistência total do sistema respiratório favorável à técnica flutter. / INTRODUCTION: Bronchiectasis is characterized by abnormal and permanent bronchi dilatation, mainly caused by the progression of inflammatory processes and loss of the ability to clear mucus. Physical therapy is a fundamental part of these patients\' treatment. In this study we aimed to evaluate the effects of the flutter technique and thoracic compression on the mucus clearance and alterations in the total impedance of the respiratory system in individuals with bronchiectasis. METHODS: In a crossover randomized blinded study, subjects with bronchiectasis and healthy (control) ones were evaluated before and after control sections, flutter exercises, or thoracic compression (TC) manoeuvres. Dry and total weights, adhesiveness, and purulence of the expectorated mucus secretions were evaluated. For respiratory system impedance measurements, impulse oscillometry was used to evaluate the resistance at 5Hz, reactance at 5Hz, and resonant frequency. Additional factors that were evaluated included the dyspnoea scale of the Medical Research Council, acceptability and tolerance scale, pulse oximetry, expectoration difficulty, and bronchiectasis severity index. RESULTS: The flutter technique and TC were efficient for mucus clearance when compared with control (p <0.05). Acute total respiratory system resistance variation responded more favourably to the flutter technique in the bronchiectasis group when compared to the control group (p <0.05). Both techniques were well tolerated by the participants. CONCLUSION: Results suggest that the flutter technique and thoracic compression are efficient methods for removal of mucus secretions. They are safe and well tolerated by patients; acute effects in the total respiratory system resistance respond favourably to the flutter technique.

ESTIMATING DISEASE SEVERITY, SYMPTOM BURDEN AND HEALTH-RELATED BEHAVIORS IN PATIENTS WITH CHRONIC PULMONARY DISEASES

Chronic pulmonary diseases include a wide range of illnesses that differ in etiology, prevalence, symptomatology and available therapy. A common link among these illnesses is their impact on patients’ vital function of breathing, high symptom burden and significantly impaired quality of life. This dissertation research evaluates disease severity, symptom burden and health behaviors of patients with three different chronic pulmonary conditions. First, alpha-1 antitrypsin deficiency (AATD) is an inherited condition that typically is associated with an increased risk of early onset pulmonary emphysema. This study examines differences in demographic, health, and behavioral characteristics and compares clinical outcomes and health related behaviors and attitudes between two severe genotypes of AATD - ZZ and SZ. The findings of the study suggest that patients with SZ genotype and less severe form of deficiency report higher number of exacerbations, comorbidities, as well as unhealthy behaviors such as lack of exercise and current smoking. In addition, individuals with the more severely deficient ZZ genotype are more adherent to disease management and prevention program recommendations and maintain a healthier lifestyle than individuals with SZ genotype. Second chronic lung disease examined in this research was chronic obstructive pulmonary disease (COPD), the fourth leading cause of death and second leading cause of disability in the United States. Prevalence and burden of cough and phlegm, two of the most common symptoms of the COPD, were assessed among participants of the COPD Foundation’s Patient-Powered Research Network (COPD PPRN). In addition, association between patient-reported levels of phlegm and cough, clinical outcomes and patients’ quality of life were evaluated. Participants’ quality of life was assessed using Patient Reported Outcome Measurement Information System instrument PROMIS-29. Association between changes in symptom severity over time and patient-reported quality of life were examined. Findings of this study indicated that severity of cough and phlegm were associated with higher number of exacerbations, greater dyspnea, and worsened patient-reported quality of life including physical and social functioning. Improvement in cough and phlegm severity over time was associated with better patient-reported quality of life. Third pulmonary illness described in this dissertation is non-cystic fibrosis bronchiectasis (NCFB), a rare and etiologically diverse condition characterized by dilated bronchi, poor mucus clearance and susceptibility to bacterial infection. Association between presence of Pseudomonas aeruginosa (PA), one of the most frequently isolated pathogens in patients with NCFFB, and disease severity was assessed utilizing enrollment data from the Bronchiectasis and NTM Research Registry (BRR). NCFB disease severity was evaluated using modified versions of validated in large international cohorts instruments, the Bronchiectasis Severity Index (BSI) and FACED. The findings of this study indicate that PA infection is common in NCFB patients, and presence of PA in patients’ sputum is associated with having moderate and high severity of bronchiectasis. In addition, the results of this study suggest that the two severity assessment instruments classify patients with NCFB differently which may be attributed to a greater number of severity markers utilized in the calculation of the BSI compared to FACED. In conclusion, the proposed dissertation aims to enhance understanding of differences in health outcomes between genotypes of AATD within AlphaNet registry, and to guide future health-promoting behaviors. It highlights the burden of common symptoms such as cough and phlegm in patients with COPD within COPD PPRN and their association with patients’ quality of life. In addition, it introduces modified indices of NCFB severity and emphasizes high burden of the disease in patients with presence of PA within the US BRR.

Bronquiectasias : tratamento cirúrgico : análise de 437 casos

-Introdução: Este trabalho apresenta a experiência cirúrgica em 437 pacientes submetidos à ressecção pulmonar para tratamento de bronquiectasias, avaliando evolução pós-operatória e o impacto quanto à melhora dos sintomas. -Métodos: Foram analisados retrospectivamente os prontuários médicos de 437 pacientes submetidos à ressecção pulmonar para tratamento de bronquiectasias em uma única instituição, entre Janeiro de 1978 e Dezembro de 2010. Os pacientes tinham 16 ou mais anos de idade, e as bronquiectasias foram diagnosticadas pelas manifestações clínicas e por tomografia computadorizada do tórax. Pacientes portadores de fibrose cística e os submetidos a transplante pulmonar foram excluídos. -Resultados: A amostra incluiu 437 pacientes (249 do sexo feminino; 188 do masculino). A idade média dos pacientes na época da cirurgia era de 38,45 anos (variando de 16 a 80 anos). O sintoma mais comum na apresentação era tosse produtiva (79,4%). A causa identificada como mais comum das bronquiectasias foi infecção respiratória na infância (49,2%). Cirurgia unilateral foi realizada em 415 pacientes (95,0%), e em 344 toda a área com bronquiectasias foi ressecada. Dentre os procedimentos cirúrgicos utilizados, o principal foi lobectomia média em 147 pacientes (33,6%) e segmentectomia (32,3%). Após a cirurgia, 94,4% de 267 pacientes questionados referiram melhora de seus sintomas, 68,9% deles considerando-se assintomáticos. Ocorreu 1 (um) óbito no pós operatório. -Conclusões: O tratamento cirúrgico das bronquiectasias mostrou-se altamente eficiente, em termos de melhora dos sintomas dos indivíduos acometidos, sendo realizado com taxas de morbidade aceitáveis, mortalidade praticamente nula, e com excelentes resultados em termos de melhora dos sintomas. / Background. This work presents the surgical experience of 437 patients undergoing to lung resection for the treatment of bronchiectasis, evaluating both the impact on postoperative clinical improvement and complications of the surgical procedure. Methods It was retrospectively analyzed the medical charts of 437 consecutive patients who underwent a surgical lung resection for treatment of bronchiectasis in a single institution, between January 1978 and December 2010. Patients were 16 years or older and bronchiectasis was diagnosed by symptoms and computed tomography. Patients with cystic fibrosis and bronchiectasis the ones that underwent to lung transplantation were excluded. Results The study sample included 437 patients (249 females; 188 males). Mean age at time of surgery was 38.45 years old (range: 16 to 80). The most common presenting symptoms were cough (79.4%). The most common etiology of bronchiectais was childhood infections (49.2%). Unilateral approach was performed in 415 (95.0%) and in 344 the total bronchiectasic site were resected. Several surgical procedures were used, the majority was medium lobectomy in 147 (33.6%) and segmentectomy (32.3%). After surgery, 94.4% of the patients improved their symptoms, 68,9% following practically assymptomatic. One postoperative death occurred. Conclusions Bronchiectasis may be treated by surgical management, particularly in developing countries where it is very prevalent. It can be performed with acceptable morbidity rates, practically without mortality, and with excelent results in terms of symptoms improvement.

Avaliação do desempenho na distância percorrida no teste de caminhada de seis minutos e qualidade de vida em pacientes com bronquiectasias não-fibrocísticas

Objetivos: Avaliar o desempenho no teste de caminhada de seis minutos (TC6M) em pacientes adultos com bronquiectasias não fibrocísticas, estabelecendo associações entre TC6M e qualidade de vida relacionada à saúde (QVRS). Secundariamente, determinar relações entre TC6M, achados clínicos e função pulmonar a fim de identificar preditores de tolerância ao exercíco. Métodos: Estudo transversal envolvendo pacientes com bronquiectasias não fibrocística, idade ≥ 18 anos, com pelo menos um sintoma respiratório por 2 ou mais anos e volume expiratório forçado no primeiro segundo (VEF1) ≤ 70% do previsto. Realizou-se avaliação clínica, função pulmonar, TC6M e QVRS pelo questionário Short-Form 36. Resultados: Foram incluídos 70 pacientes (48 mulheres; idade média 54,5±17,7 anos; VEF1 médio 44,9±14,5%). Desempenho alterado no TC6M foi observado em 23 pacientes (Grupo 1) e desempenho normal em 47 pacientes (Grupo 2). Em comparação aos pacientes do Grupo 2, os pacientes do Grupo 1 apresentaram menor idade (p<0,001), menor idade de diagnóstico das bronquiectasias (p=0,006), menor proporção de ex fumantes (p=0,048), menor índice de massa corporal (IMC) (p=0,003), pior VEF1 % previsto (p=0,041) e pior pressão expiratória máxima % previsto (p=0,021). Não houve diferença significante entre grupos nos escores de QVRS (p>0,05). No modelo de regressão logística, idade menor (p=0,002) e IMC menor (p=0,034) se associaram significantemente com desempenho precário no TC6M. Conclusão: Elevada proporção de pacientes com bronquiectasias não fibrocísticas apresenta desempenho precário no TC6M. O desempenho ao exercício não se relacionou com a QVRS. Idade e IMC se associaram com performance ao exercício. / Objective: To determine the performance in 6-minute walk test (6MWT) in adult patients with non-cystic fibrosis bronchiectasis, establishing the relationship between 6-MWT and health-related quality of life (HRQL). Secondarily, to determine the relationship between 6MWT, clinical findings and lung function in order to identify predictors for exercise tolerance. Methods: Cross-sectional study involving patients with non-cystic fibrosis bronchiectasis aged ≥ 18 years, with at least one respiratory symptom for 2 years or more, and with predicted forced expiratory volume in first second (FEV1) ≤ 70%. Patients were submitted to clinical evaluation, pulmonary function tests, 6MWT and HRQL using the Short-Form 36 Questionnaire. Results: Seventy patients were included (48 females; mean age, 54.5±17.7 years; mean FEV1, 44.9 ±14.5%). We observed poor performance in 6MWT in 23 patients (Group 1) and normal performance in 47 patients (Group 2). In comparison with Group 2 patients, Group 1 patients presented lower age (p<0.001), lower age at diagnosis of bronchiectasis (p=0.006), lower proportion of ex-smokers (p=0.048), lower body mass index (BMI) (p=0.003), worse FEV1 % predicted (p=0.041), and worse maximum expiratory pressure % predicted (p=0.021). There was no significant difference between groups on scores for HRQL (p>0.05). In a logistic regression model, lower age (p=0.002) and lower BMI (p=0.034) were significantly associated with poor performance in 6MWT. Conclusion: There is a high rate of poor performance in 6MWT in patients with non-cystic fibrosis bronchiectasis. The exercise performance was not related to HRQL. Age and BMI were associated with the exercise performance.

Análise do processo sinérgico da deglutição em pacientes portadores de bronquiectasia atendidos no Hospital Universitário Pedro Ernesto / Swallowing analize in patients carrying bronchiectasis attended at Hospital Universitário Pedro Ernesto

A respiração e a deglutição são vitais para o homem. Enquanto a primeira diz respeito a um ato primitivo da vida, a hematose, a segunda trata da manutenção da vida, oferecendo a energia necessária, nutrindo e hidratando, perpassando pelo prazer alimentar, ato tão importante na sociedade contemporânea. A relação funcional entre essas funções ainda não foi totalmente elucidada, porém é crescente o interesse e o número de estudos sobre esta temática. Considerando que a deglutição eficiente tem como pressuposto a capacidade de proteger via aérea inferior, a alteração de deglutição primária ou secundária a um dano pulmonar pode trazer repercussões severas para a integridade do sistema respiratório. O objetivo desse estudo é analisar o processo sinérgico da deglutição em portadores de bronquiectasia, a fim de verificar se há alteração na fisiologia da deglutição e caracterizá-la, assim como, identificar se há correspondência entre alteração da função pulmonar e alteração da deglutição.Para tal foram selecionados randomicamente 30 pacientes na faixa etária de 18 a 65 anos, atendidos no ambulatório de bronquiectasia do HUPE. Destes, 26 indivíduos responderam a um questionário dirigido sobre hábitos alimentares e possíveis dificuldades de alimentação; foram submetidos à avaliação clínica da deglutição; 22 fizeram espirometria no setor de prova de função pulmonar no HUPE e 17 avaliação videofluoroscópica da deglutição. Dos 26 indivíduos estudados 10 eram homens e 16 mulheres, com média de idade de 46,3 anos. Na avaliação clínica da deglutição observaram-se alterações estruturais e funcionais em todos os indivíduos estudados. As principais alterações estruturais detectadas referem-se e a dinâmica laríngea; enquanto na avaliação funcional detectou-se alterações referentes à ejeção oral, dinâmica hiolaríngea, trânsito faríngeo e presença de deglutições múltiplas. Com relação à espirometria 06 indivíduos apresentaram distúrbio obstrutivo leve; 04 distúrbio obstrutivo moderado e 09 distúrbio obstrutivo acentuado. A videofluoroscopia da deglutição corroborou os achados da avaliação clínica da deglutição e evidenciou episódios de penetração e aspiração laríngea Pode-se concluir que: (1) a avaliação clínica da deglutição associada à avaliação videofluoroscópica são métodos eficientes para a análise do processo sinérgico da deglutição; (2) identificou-se alteração do processo sinérgico da deglutição, nos indivíduos avaliados; (3) a ausculta cervical isoladamente, não demonstrou ser um método eficiente para predizer aspiração e/ou penetração laringotraqueal; (4) houve correspondência entre os resultados da avaliação clínica funcional da deglutição e videofluoroscópica, exceto quanto a presença de penetração e/ou aspiração e quanto a capacidade de avaliar a ejeção oral; (5) não foi possível identificar se há correspondência entre alteração da função pulmonar e processo sinérgico da deglutição. / Breathing, as well as swallowing, is vital to men.While the first accounts for a primitive aspect of life, the transference of gases through the blood-air barrier, the second is about the maintenance of life, supplying the necessary energy, nourishing and hydrating the organism and then touching the subject of feeding pleasure, such an important act in contemporary society. The functional relation between these functions has not been completely elucidated; however the interest is increasing and the number of studies about this theme. Considering that the efficient swallowing has the capacity of protecting the low airway as purpose, the alteration of primary or secondary swallowing to a pulmonary damage may bring severe repercussions to the integrity of the respiratory system. The aim of the study is to analyze the swallowing in patients carrying bronchiectasis, in order to verify whether there is an alteration in the swallowing physiology and characterize it, as well as to identify whether there is a correspondence between the alteration of the pulmonary function and swallowing disorders. 30 patients were randomly selected aged between 18 and 62 years old, attended at the bronchiectasis clinic of HUPE. 26 out of those individuals were evaluated. They answered a directed quiz about alimentary habits and possible alimentary difficulties; they were submitted to clinical evaluation of swallowing, 22 were orientated to spirometric evaluation in the sector of pulmonary function test at HUPE and 17 videofluorographic examination of swallowing. From the 26 individuals 10 were male and 16 were female, average age of 26 years old. As for the clinical evaluation of swallowing (structural and functional), all individuals presented some kind of laryngeal alteration in the structural evaluation. In the functional evaluation was noted that the main alterations referred to oral ejection, elevation and anterior movement of the hyoid and larynx, pharyngeal transit and multiple swallowing were also present. In relation to spirometry 06 individuals presented mild obstructive disorder 04 presented moderate obstructive disorders and 09 presented severe obstructive disorders. The videofluorograph examination confirmed the findings of the swallowing clinical evaluation and made clear an episode of larynx penetration of liquids. The present study showed as preliminary results: (1) that the clinical evaluation of swallowing associated with videofluorographic Examination are efficient methods to the analysis of the swallowing process; (2) Alteration in the synergic process of swallowing was identified in the evaluated individuals; (3) Cervical auscutating isolatedly did not prove to be an efficient method to predict larynx-tracheal aspiration and/or penetration; (4) There was a correspondence between the results of clinical evaluation of the swallowing videofluorographic, except for the presence of penetration and/or aspiration and for the capacity of evaluating oral ejection; (5) Was not possible to identify if there is a correlation between alteration of the pulmonary function and swallowing disorders.

Bronquiectasias : tratamento cirúrgico : análise de 437 casos

-Introdução: Este trabalho apresenta a experiência cirúrgica em 437 pacientes submetidos à ressecção pulmonar para tratamento de bronquiectasias, avaliando evolução pós-operatória e o impacto quanto à melhora dos sintomas. -Métodos: Foram analisados retrospectivamente os prontuários médicos de 437 pacientes submetidos à ressecção pulmonar para tratamento de bronquiectasias em uma única instituição, entre Janeiro de 1978 e Dezembro de 2010. Os pacientes tinham 16 ou mais anos de idade, e as bronquiectasias foram diagnosticadas pelas manifestações clínicas e por tomografia computadorizada do tórax. Pacientes portadores de fibrose cística e os submetidos a transplante pulmonar foram excluídos. -Resultados: A amostra incluiu 437 pacientes (249 do sexo feminino; 188 do masculino). A idade média dos pacientes na época da cirurgia era de 38,45 anos (variando de 16 a 80 anos). O sintoma mais comum na apresentação era tosse produtiva (79,4%). A causa identificada como mais comum das bronquiectasias foi infecção respiratória na infância (49,2%). Cirurgia unilateral foi realizada em 415 pacientes (95,0%), e em 344 toda a área com bronquiectasias foi ressecada. Dentre os procedimentos cirúrgicos utilizados, o principal foi lobectomia média em 147 pacientes (33,6%) e segmentectomia (32,3%). Após a cirurgia, 94,4% de 267 pacientes questionados referiram melhora de seus sintomas, 68,9% deles considerando-se assintomáticos. Ocorreu 1 (um) óbito no pós operatório. -Conclusões: O tratamento cirúrgico das bronquiectasias mostrou-se altamente eficiente, em termos de melhora dos sintomas dos indivíduos acometidos, sendo realizado com taxas de morbidade aceitáveis, mortalidade praticamente nula, e com excelentes resultados em termos de melhora dos sintomas. / Background. This work presents the surgical experience of 437 patients undergoing to lung resection for the treatment of bronchiectasis, evaluating both the impact on postoperative clinical improvement and complications of the surgical procedure. Methods It was retrospectively analyzed the medical charts of 437 consecutive patients who underwent a surgical lung resection for treatment of bronchiectasis in a single institution, between January 1978 and December 2010. Patients were 16 years or older and bronchiectasis was diagnosed by symptoms and computed tomography. Patients with cystic fibrosis and bronchiectasis the ones that underwent to lung transplantation were excluded. Results The study sample included 437 patients (249 females; 188 males). Mean age at time of surgery was 38.45 years old (range: 16 to 80). The most common presenting symptoms were cough (79.4%). The most common etiology of bronchiectais was childhood infections (49.2%). Unilateral approach was performed in 415 (95.0%) and in 344 the total bronchiectasic site were resected. Several surgical procedures were used, the majority was medium lobectomy in 147 (33.6%) and segmentectomy (32.3%). After surgery, 94.4% of the patients improved their symptoms, 68,9% following practically assymptomatic. One postoperative death occurred. Conclusions Bronchiectasis may be treated by surgical management, particularly in developing countries where it is very prevalent. It can be performed with acceptable morbidity rates, practically without mortality, and with excelent results in terms of symptoms improvement.

Objetivos: Avaliar o desempenho no teste de caminhada de seis minutos (TC6M) em pacientes adultos com bronquiectasias não fibrocísticas, estabelecendo associações entre TC6M e qualidade de vida relacionada à saúde (QVRS). Secundariamente, determinar relações entre TC6M, achados clínicos e função pulmonar a fim de identificar preditores de tolerância ao exercíco. Métodos: Estudo transversal envolvendo pacientes com bronquiectasias não fibrocística, idade ≥ 18 anos, com pelo menos um sintoma respiratório por 2 ou mais anos e volume expiratório forçado no primeiro segundo (VEF1) ≤ 70% do previsto. Realizou-se avaliação clínica, função pulmonar, TC6M e QVRS pelo questionário Short-Form 36. Resultados: Foram incluídos 70 pacientes (48 mulheres; idade média 54,5±17,7 anos; VEF1 médio 44,9±14,5%). Desempenho alterado no TC6M foi observado em 23 pacientes (Grupo 1) e desempenho normal em 47 pacientes (Grupo 2). Em comparação aos pacientes do Grupo 2, os pacientes do Grupo 1 apresentaram menor idade (p<0,001), menor idade de diagnóstico das bronquiectasias (p=0,006), menor proporção de ex fumantes (p=0,048), menor índice de massa corporal (IMC) (p=0,003), pior VEF1 % previsto (p=0,041) e pior pressão expiratória máxima % previsto (p=0,021). Não houve diferença significante entre grupos nos escores de QVRS (p>0,05). No modelo de regressão logística, idade menor (p=0,002) e IMC menor (p=0,034) se associaram significantemente com desempenho precário no TC6M. Conclusão: Elevada proporção de pacientes com bronquiectasias não fibrocísticas apresenta desempenho precário no TC6M. O desempenho ao exercício não se relacionou com a QVRS.
