O cuidado de enfermagem e o cateter de Hickman: a busca de evidências / Nursing care and Hickman s catheter: the search for evidence

Os pacientes submetidos ao transplante de medula óssea (TMO) necessitam de um acesso venoso seguro para a infusão da medula óssea sem que haja comprometimento do enxerto, assim a implantação de um cateter venoso central é parte da terapêutica, sendo o de Hickman o mais utilizado atualmente; entretanto, sua presença impõe riscos aos pacientes, sendo a infecção, o mais preocupante. A implementação da prática baseada em evidências fundamenta a tomada de decisão do enfermeiro, uma vez que a utilização de resultados de pesquisa é um dos pilares desta abordagem. O presente estudo é uma revisão integrativa da literatura, que teve como objetivo buscar e avaliar as evidências sobre os cuidados de enfermagem relacionados ao cateter de Hickman após a sua implantação, no paciente submetido ao TMO. Para a seleção dos artigos utilizamos as bases de dados Lilacs, Medline, Cinahl e o periódico Bone Marrow Transplantation, e a amostra constituiu-se de 18 artigos. O curativo recomendado é o de poliuretano trocado a cada 7 dias. O método de coleta de sangue observado foi o de descarte, sendo que a dosagem de ciclosporina não deve ser coletada da mesma via onde foi infundida. A redução do número de manipulações do cateter é considerada uma medida eficaz na prevenção e no controle de infecção. As evidências extraídas dos estudos analisados podem auxiliar a implementação de cuidados de enfermagem eficazes relacionados ao cateter de Hickman visando principalmente a melhoria da assistência à saúde. / Patients submitted to Bone Marrow Transplantation (BMT) need a safe venous access for the infusion of bone marrow, without jeopardizing the graft. Thus, the implantation of a central venous catheter is a part of therapy, with Hickman s catheter as the most commonly used type; however, its presence entails risks for the patients, the most preoccupying of which is infection. The implementation of evidence-based practice, with the use of research results as one of its pillars and supports nurses decision making. This integrative literature review aimed to look for and evaluate evidence on nursing care related to Hickman s catheter after its implantation in patients submitted to BMT. Articles were selected from the Lilacs, Medline and Cinahl databases and from the journal Bone Marrow Transplantation. The sample consisted of 18 articles. Esta pesquisa teve como objeto de estudo a segurança do recém-nascido no processo de utilização do Cateter Central de Inserção Periférica (PICC) e, como objetivos: conhecer o significado de segurança para o enfermeiro no processo de utilização do PICC em recém-nascidos; descrever os cuidados prestados pelo enfermeiro no uso do PICC em recém-nascidos e analisar os nexos entre segurança e os princípios bioéticos no uso do PICC em recém-nascidos na prática assistencial dos enfermeiros. Trata-se de um estudo descritivo de abordagem qualitativa. O cenário foi a unidade de terapia intensiva neonatal de um Hospital Universitário localizado no município do Rio de Janeiro e os sujeitos, 11 enfermeiros plantonistas capacitados e que realizam a implantação do PICC em recém-nascidos. Para a coleta de dados realizou-se a entrevista semiestruturada, gravada em fita cassete, entre os meses de março e junho de 2012. Posteriormente estas foram transcritas e analisadas por meio da análise de conteúdo de Bardin, na modalidade temática e interpretada à luz dos princípios bioéticos e da segurança do paciente. Como resultados emergiram 04 categorias: Técnicas e Procedimentos, Cuidados com o recém-nascido, Aspectos relacionados à equipe e Aspectos relacionados à família. Para os enfermeiros, segurança no processo de utilização do PICC no recém-nascido, significa saber indicar o uso deste dispositivo de acordo com as peculiaridades de cada criança. Exercer cuidados antes, durante e após o uso do cateter, valorizar os cuidados técnicos relacionados ao procedimento, possuir conhecimento teórico-prático e ter disponibilidade de recursos materiais e humanos para desenvolver um cuidado seguro. Além de atentar para os registros e protocolos da unidade acerca desta prática assistencial. Para preservar a segurança do neonato, compreendem ser necessária a tomada de decisão em conjunto com o médico acerca do momento ideal para se implantar este dispositivo, bem como a escolha do tipo ideal de sedação para o mesmo, dentre outros aspectos. No processo de utilização do PICC, os enfermeiros entendem a manutenção da temperatura corporal, a realização de medidas de conforto perante a dor, a prevenção de infecções e o posicionamento adequado do recém-nascido durante o procedimento, como atitudes essenciais para a promoção de sua segurança. Buscam, também, esclarecer os pais quanto ao procedimento que será realizado com seu filho. Conclui-se que o enfermeiro, no que diz respeito à prática do PICC, atua de acordo com os princípios bioéticos de beneficência e não-maleficência, já que realiza sua assistência visando o bem-estar do neonato, procurando minimizar os desconfortos associados a esse procedimento. Apesar de esclarecerem os pais quanto ao procedimento que será realizado com seu filho, alguns enfermeiros, não os consultam previamente acerca da autorização para implantação deste dispositivo infringindo, assim, o princípio bioético da autonomia. / This research had as its object of study the safety of the newborn in the process of using the Peripherally Inserted Central Catheter (PICC) and its objectives: know the meaning of safety for nurses in the process of PICC use in newborns; describe the care provided by nurses using PICC in newborns and analyze the nexus between security and bioethical principles in the use of PICC in newborns in care practice of the nurses. This is a descriptive qualitative approach, characterized as an exploratory cross-sectional survey, depending on the object of study selected. The scenario chosen was the Neonatal Intensive Care Unit of a University Hospital located in the municipality of Rio de Janeiro and the subjects were eleven nurses on duty trained to use the PICC in newborns. To collect data, semi-structured interview was conducted between the months of march and june 2012, and the responses were recorded on cassette tapes. Thereafter began the content analysis proposed by Bardin and interpretation of results was made according to the concepts of patient safety and bioethics. As results emerged four categories: Techniques and Procedures, Care of the newborn, Aspects related to the team and Aspects related to the family. For nurses, provide security means knowing indicating the PICC according to the peculiarities of each newborn, exercising care before, during and after the use of this catheter, including technical precautions related to the procedure, in addition to having theoretical and practical knowledge and have availability of material and human resources to develop a care insurance. Besides attending to the records and protocols of the healthcare practice about this unit. To preserve the safety of the neonate, be necessary to understand the decision making together with the doctor about the ideal time to deploy this device as well as the choice of the ideal type of sedation for it, among other aspects. In the process of using the PICC nurses understand the maintenance of body temperature, performing comfort measures before pain, infection prevention and appropriate positioning of the baby during the procedure, and attitudes essential for the promotion of their safety. Seek also enlighten parents about the procedure that will be performed with your child. Avaliação das lesões obstrutivas dos principais vasos de drenagem venosa dos membros superiores nos pacientes em hemodiálise / Obstructive wounds evaluation of the upper members main drainage vessels among patients under dialysis

Introdução: A integridade do Sistema Venoso Central é uma condição fundamental para criar ou manter um acesso vascular eficiente para hemodiálise. A cateterização da veia subclávia é a principal responsável pelo desenvolvimento de obstruções nesses vasos. Porém, é preciso reavaliar a distribuição dessas lesões na atual época de priorização da cateterização da veia jugular interna. Objetivo: Avaliar a distribuição anatômica das obstruções no Sistema Venoso Central dos pacientes em hemodiálise com sinais clínicos de hipertensão venosa. Método e Material: foi realizado um estudo prospectivo de casos consecutivos no HC-FMUSP com duração de 2 anos. A amostra foi composta de 30 pacientes que estavam apresentando sinais clínicos de hipertensão venosa no membro superior portador de uma fístula arteriovenosa. Todos os doentes incluídos foram submetidos a uma angiografia do membro superior através da fístula como método diagnóstico. Coleta de dados e Procedimentos: antes da fistulografia, os pacientes encaminhados ao ambulatório de Nefrologia do HC-FMUSP foram submetidos a uma avaliação clínica com preenchimento de um questionário padronizado, aqueles que preencherão os critérios da pesquisa foram agendados para a realização do exame. Após o diagnostico as lesões foram classificadas conforme o tipo e localização. Análise Estatística: os dados foram expressos em proporção, média ou mediana e valores mínimos e máximos conforme apropriado. As variáveis foram apresentadas descritivamente em tabelas contendo freqüências absolutas (n) e relativas (%). As diferenças entre grupos foram testadas inicialmente com uso de análise univariada, com aplicação do teste t de Student ou do teste de Mann- Whitney. Resultados: nos 30 doentes avaliados foram diagnosticadas ao todo 30 lesões, sendo 20 na veia braquiocefálica, 9 na veia subclávia e 1 na veia cava superior. 70% dessas lesões estavam localizadas em veias intratorácicas. O tipo de lesão mais freqüente foi a oclusão, presente em 70% dos casos. 33% dos pacientes que desenvolveram lesões no Sistema Venoso Central tiveram antecedente de cateterização somente da veia jugular interna. Conclusão: Atualmente, no diagnostico das lesões obstrutivas do Sistema Venoso Central nos pacientes em hemodiálise é importante utilizar métodos diagnósticos que possam informar com precisão as condições dos vasos de localização intratorácica. / Introduction: Central Venous System integrity is a fundamental condition in order to create or maintain an efficient vascular access for dialysis. Subclavian vein catheterization is the foremost condition responsible for these vessels\' obstructions development. However, it is necessary to reappraise these wounds distribution as the current priorization is for catheterization of internal jugular vein. Objective: To evaluate anatomical obstructions distribution in the Central Venous System of patients under dialysis who present clinical signs of venous hypertension. Method and material: A prospective study of consecutive cases was carried out at HCFMUSP within a two years period. The sample was composed by 30 patients who showed up clinical signs of venous hypertension and were bearers of an artery-venous fistula in the upper member. All included patients were submitted to an angiography of the upper member through the fistula as a diagnosis approach. Fact-gathering and procedures: Prior to fistulography, patients were directed to the Nephrology outpatient clinic at HC-FMUSP and were submitted to a clinical evaluation, and filled in a pattern questionnaire. Those who fulfilled the research criteria were scheduled for the exam achievement. After diagnosed, all lesions were classified according to category and location. Statistical analysis: Facts were expressed in proportion, medium or average and minimum and maximum values according appropriate needs. Variables were presented descriptively in absolute tables containing frequencies (n) and relative values (%). Differences amid groups were initially quizzed using univariate analysis, with application of test t of Student or test of Mann-Whitney. Results: Among 30 evaluated patients a total 30 wounds were diagnosed, being 20 in brachiocephalic vein, 9 in subclavian vein and 1 in superior cava vein. 70% of these wounds were located within intratoracic veins. The most frequent lesion category was occlusion present in 70% of the cases. 33% of the patients who developed wounds in the Central Venous System had antecedent of catheterization only at internal jugular vein. Conclusion: Nowadays, in order to diagnose obstructive lesions within Central Venous System among patients on dialysis it is important to use diagnoses approaches which can accurately inform the conditions of intratoracic vessels location.

The major cause of morbidity and mortality in transplanted patients is infection. The insertion of a semi-implantable, long-term central venous indwelling catheter poses a risk of complications, such as obstruction, fracture or breakage, migration, infection, and death. Objectives: The aims of the present study were to characterize the patients undergoing hematopoietic stem cell transplantation and to identify complications related to the central venous catheter and to describe the associated complications between the variables (sex, age, diagnosis, kind of transplantation, type of catheter, site of catheter insertion, and the length of time with the catheter. Methods: This was a retrospective, quantitative study conducted at a teaching hospital in inland of São Paulo State. Transplanted patients’ names were retrieved from the medical records and from the nursing forms. Files were reviewed retrospectively. The study involved the analysis of 188 patients’ files from 2007 to 2011. Association between variables was compared using the Chi-square test and the univariate analysis. Continuous variables were compared using Student’s t-test. Independent variables were analyzed using the Mann-Whitney test, Anova and the Kruskal-Wallis test. P = 0.05 was considered statistically significant. We used the correspondence analysis and multivariate technique to analyze the categorical variables. Results: Most patients were married man with age ranging from 40 to 60 years. They worked in general services and came from the cities of São Paulo and Mato Grosso States. They were patients from the Unified Health System (SUS) diagnosed with Hodgkin's lymphoma, multiple myeloma and non-Hodgkin lymphoma undergoing autologous transplantation. The major complications were constipation, diarrhea, urinary tract infection, respiratory infection and graft versus host disease. However, most patients survived the transplant and were discharged. The Hickman catheter was the most used catheter, with a mean length of time with the catheter of 47.6 days. The fever/bacteremia complication was more incident in young males with non-Hodgkin lymphoma and autologous transplantation, who remained with the catheter for a long period in the subclavian vein. Conclusion: Characterizing the patients’ profile, identifying the diagnosis, complications of treatment, the current situation of the post transplanted patients, provide grants to nursing professionals in search of a safer practice with actions related to prevention. Complications of catheter and its relation to age, gender, medical diagnosis, type of transplant, catheter model, mean residence time and insertion site should be analyzed considering the conditioning protocol, which directly affects the occurrence of complications and injury Related catheter. / Introdução: O transplante de células tronco hematopoéticas é um processo longo, complexo, de alto custo, que exige infraestrutura hospitalar apropriada e equipe multiprofissional qualificada. As maiores causas de morbidade e mortalidade de pacientes transplantados são as infecções. Faz parte do transplante a inserção de um cateter venoso central semi-implantável de longa permanência, com risco de complicações como obstrução, fratura ou ruptura, migração, infecção e morte. Objetivos: Caracterizar os pacientes submetidos ao transplante de células tronco hematopoéticas; identificar as intercorrências associadas ao cateter venoso central e descrever a relação de correspondência entre as complicações e as variáveis: sexo, idade, diagnóstico médico, tipo de transplante, modelo de cateter, local de inserção e tempo de permanência com o cateter. Método: Estudo retrospectivo, quantitativo, realizado em um hospital de ensino do interior do estado de São Paulo, por meio do prontuário e planilhas de enfermagem de 188 pacientes transplantados entre 2007 e 2011. Foram realizados testes estatísticos de associação (qui-quadrado), de comparação univariada, teste T, para amostras independentes, Mann-Whitney, ANOVA e Kruskal-Wallis, todos ao nível de significância 0,05 e análise de correspondência e técnica multivariada para as variáveis categóricas. Resultados: A maioria dos pacientes era do sexo masculino, idade 40 a 60 anos, casada, trabalhavam na área de serviços gerais, procedentes de cidades do estado de São Paulo e Mato Grosso, atendidos pelo SUS, com diagnóstico de linfoma de Hodgkin, mieloma múltiplo e linfoma não Hodgkin, submetidos ao transplante autólogo. As complicações foram: constipação, diarreia, infecção urinária, infecção respiratória e doença do enxerto contra o hospedeiro, entretanto, a maioria sobreviveu ao transplante e teve alta hospitalar. O Hickman foi o cateter mais utilizado, com permanência média de 47,6 dias. A complicação febre/bacteremia mais incidente em jovens, sexo masculino, com Linfoma não Hodgkin e transplante autólogo, que permaneceram com o cateter por longo período, em veia subclávia. Conclusão: Caracterizar o perfil, identificar o diagnóstico, as complicações decorrentes do tratamento, a situação atual do paciente pós transplante, oferece subsídios aos profissionais de enfermagem na busca de uma prática mais segura. As complicações do cateter e suas relações com idade, sexo, diagnóstico médico, modalidade de transplante, modelo de cateter, tempo médio de permanência e local de inserção, devem ser analisadas considerando o protocolo de condicionamento, que interfere diretamente na ocorrência e agravo das complicações relacionadas ao cateter.

Hypnosis for Relief of Pain and Anxiety in Children Receiving Intravenous Lines in the Pediatric Emergency Department

Intravenous line placement is one of the most common procedures performed on children presenting to the Emergency Department. Anxiety about needles is widespread, and many children experience high levels of fear and/or pain with their IV line placements. Hypnosis is a behavioral intervention that shows significant promise for alleviating procedure-related pain and anxiety in children. Twenty-five developmentally normal, English-speaking children between the ages of five and fifteen who required IV line placement in the Pediatric Emergency Department at Yale-New Haven Childrens Hospital were randomized to receive either the standard of care or standard of care plus a brief hypnotic intervention. The groups were similar with regard to baseline demographic and socioeconomic status, previous experience with medical care, and presence or absence of chronic medical conditions. Childrens pre-procedural anxiety ratings on a 10cm visual analog scale (VAS) and expected procedural pain ratings by 10-point oucher and 10cm VAS were not significantly different between the groups. Children randomized to the hypnosis group reported less anxiety during the procedure (mean 5.0 vs 3.1, median 7.2 vs 2.2, p = 0.28) than children randomized to the standard of care group. Cases also had a decrease in anxiety from expected to actual of 1.6 on a 10cm scale, while those randomized to the control group had an increase from expected to actual anxiety of 1.1 (p=0.01). A smaller trend towards decreased pain in the hypnosis group was also present. As measured by VAS, cases had lower mean pain scores (3.4 vs 4.3) than controls. In a comparison of anticipated and actual pain scores between groups, the hypnosis group had a mean decrease of 0.8 on a 10cm VAS , while the control group had a mean increase of 0.5 (p=0.14). Recruitment of subjects is ongoing, but preliminary results suggest that hypnosis is effective for alleviating needle-related anxiety in children undergoing IV line placement and may be helpful for alleviating the pain of IV line placement as well.

Determinants of vascular access-related bloodstream infections among patients receiving hemodialysis

Vascular access-related bloodstream infection (BSI) is frequent among patients undergoing hemodialysis increasing significantly their morbidity and mortality. Studies assessing centre- and patient-predictors of BSI have had inadequate sample size and follow-up time. The aims of this project are: to describe the incidence rates; and to determine patient- and centre-level predictors of BSI in a cohort of incident hemodialysis patients treated in teaching or community hospitals, and in First Nation dialysis units. The rates of BSI in our population were lower than those observed in other settings. Central venous catheters were the most important risk factor for BSI and their use in our study was much higher than recommended. Some variability in BSI rates was found among centres, but no centre-related variable was found to be associated with the risk of BSI. Effort to reduce catheter use in hemodialysis patients may significantly reduce the risk of BSI in this patient population.

The process of data collection was through observation with structured form, refers to medical records and structured questionnaires with health professionals. The results were organized, tabulated, categorized and analyzed using SPSS 14.0. The characterization of the subjects was performed using descriptive and inferential statistics, taking into account the nature of the variables, with analysis of variance (ANOVA) and Spearman correlation test, it was a discussion of the information obtained, considering the mean, standard deviation, coefficient of variance and standard error. The variables that showed a higher level of correlation were treated with the application of significance tests. As the results, 71% of participants were female and 29% male, age ranged from 18 to 85 years (52.6 ? 22.5). The insertion, there was a variation from 0 to 5 errors (1.2 ? 1.4), during maintenance, the average was 2.3 ? 0.9 errors, ranging from 0 to 4. During the insertion and maintenance of CVC, patients who had been an infection ranging from 2 to 9 mistakes (4.2 ? 1.7), since those who did not show the variation goes from 0 to 5 errors (2, 8 ? 1.5). The correlation coefficient between the risk of infection throughout the process and the risk of infection at the insertion showed strong and significant (r = 0.845 p = 0.000) and in relation to risk of infection in maintenance was moderate and significant (r = 0.551 p = 0.001).
