Disordered Eating Behaviour, Unhealthy Eating Patterns and the Association with Body Mass Index among Canadian Youth Aged 12-17: A Cross-sectional Study

This is the final submission of my Masters Thesis. / OBJECTIVES: Cognitive disordered eating behaviours (body shape concerns) are prevalent among youth with higher unhealthy body mass indexes (BMIs), while physical disordered eating behaviours (unhealthy changes in eating habits) are common mechanisms in determining future BMI. This thesis investigates the associations between these disordered eating behaviours and BMI categories and evaluates potential covariate modification. As a secondary objective, this thesis investigates the association between unhealthy eating patterns and BMI categories. METHODS & DATA ANALYSIS: This study was conducted among Canadian youth aged 12-17 (n=12,350). Body shape concerns were measured as the frequency of having a preoccupied desire to be thin while changing eating habits was measured as the frequency of changing eating habits to manage weight. Unhealthy eating patterns were defined as frequent restaurant eating, energy drink consumption, and skipping breakfast. Categorical BMI classifications consisted of obesity, overweight and normal/underweight. Adjusted multinomial logistic regression models were constructed. Covariates were independently tested for statistical interaction in the primary relationships. Measures of association were reported as adjusted odds ratios (ORs) with 95% confidence intervals. RESULTS: Having a preoccupied desire to be thin was associated with increased odds of being overweight and having obesity. These relationships were modified by sex, physical activity, and sleep. Changing eating habits to manage weight was associated with increased odds of being overweight and having obesity. These relationships were modified by sex. Skipping breakfast increased the odds of being overweight and having obesity, while consuming energy drinks increased the odds of having obesity. CONCLUSION: Over 40% of the sample experienced some frequency of having body shape concerns or changing eating habits to manage weight. These disordered eating behaviours increase the odds of being overweight and having obesity. Continued monitoring through national–level surveys, will be crucial in understanding changes in rates of disordered eating behaviour. / Thesis / Master of Public Health (MPH)

A Foundation for Learning: Parent's Perceptions of the McMaster Children and Youth University (MCYU) / Addressing the Factors That Shape Parent Involvement in the McMaster Children and Youth University (MCYU)

The McMaster Children and Youth University (MCYU) was founded at McMaster University in Hamilton Ontario, Canada. The MCYU delivers free educational programming to children and youth to empower and prepare youth, especially those at a socioeconomic disadvantage, to aspire to higher education (MCYU, 2022). Parents are invited to attend the lectures alongside their children, making parent involvement a key component of the program (MCYU, 2018; Paquin et al, 2018). Participation in educational activities, such as out-of-school programming, school-run field trips, or extracurriculars have been correlated with improved academic achievement and social adjustment (Holloway & Pimlott-Wilson, 2014; Weininger et al., 2015). However, children and youth from disadvantaged backgrounds are less likely to participate in learning enrichment activities outside of school (Siddiqui et al., 2019). These barriers to participation have often been attributed to lower economic status (Chanfreau, et al., 2016; Farthing, 2014; Holloway & Pimlott-Wilson, 2014; Siddiqui et al., 2019). A grounded theory study was employed addressing the following question: How do socio-ecological factors shape parental decision-making for childhood involvement in the MCYU? Twenty parents who participate in the MCYU were interviewed and asked to complete an accompanying survey. After thorough data collection and analysis, the results indicated that positive educational experiences in the lives of parents shape their approach to their children’s education. Their life experiences have led to an intrinsic and extrinsic desire to learn, and the MCYU is an invaluable tool for parents to instill these values in their children through 1) education, 2) exposure, 3) enjoyment, and 4) engagement. This research may inform an expansion of MCYU type programs across Canada as well as aid the MCYU in reaching members of the community who are unable to benefit from the program. Using health-related quality of life instruments for children with long-term conditions : On the basis of a national quality registry system

Introduction: There has been a continuous development of new technologies in healthcare that are derived from national quality registries. However, this innovation needs to be translated into the workflow of healthcare delivery, to enable children with long-term conditions to get the best support possible to manage their health during everyday life. Since children living with long-term conditions experience different interference levels in their lives, healthcare professionals need to assess the impact of care on children’s day-to-day lives, as a complement to biomedical assessments. Aim: The overall aim of this thesis was to explore and describe the use of instruments about health-related quality of life (HRQOL) in outpatient care for children with long-term conditions on the basis of a national quality registry system. Methods: The research was conducted by using comparative, cross-sectional and explorative designs and data collection was performed by using different methods. The questionnaire DISABKIDS Chronic Generic Measure -37 was used as well as semi-structured interviews and video-recordings from consultations. Altogether, 156 children (8–18 years) and nine healthcare professionals participated in the studies. Children with Type 1 Diabetes (T1D) (n 131) answered the questionnaire DISABKIDS and children with rheumatic diseases, kidney diseases and T1D (n 25) were interviewed after their consultation at the outpatient clinic after the web-DISABKIDS had been used. In total, nine healthcare professionals used the HRQOL instrument as an assessment tool during the encounters which was video-recorded (n 21). Quantitative deductive content analysis was used to describe content in different HRQOL instruments. Statistical inference was used to analyse results from DISABKIDS and qualitative content analysis was used to analyse the interviews and video-recordings. Results: The findings showed that based on a biopsychosocial perspective, both generic and disease-specific instruments should be used to gain a comprehensive evaluation of the child’s HRQOL. The DISABKIDS instrument is applicable when describing different aspects of health concerning children with T1D. When DISABKIDS was used in the encounters, children expressed positive experiences about sharing their results with the healthcare professional. It was discovered that different approaches led to different outcomes for the child when the healthcare professionals were using DISABKIDS during the encounter. When an instructing approach is used, the child’s ability to learn more about their health and how to improve their health is limited. When an inviting or engaging approach is used by the professional, the child may become more involved during the conversations. Conclusions: It could be argued that instruments of HRQOL could be used as a complement to biomedical variables, to promote a biopsychosocial perspective on the child’s health. According to the children in this thesis, feedback on their results after answering to web-DISABKIDS is important, which implies that healthcare professionals need to prioritize time for discussions about results from HRQOL instruments in the encounters. If healthcare professionals involve the child in the discussion of the results of the HRQOL, misinterpreted answers could be corrected during the conversation. Concurrently, this claims that healthcare professionals invite and engage the child.

Targeting efficiency and take-up of Oportunidades, a conditional cash transfer, in urban Mexico in 2008

Oportunidades is a Conditional Cash Transfer (CCT) that uses a proxy means-test targeting model to select eligible households for the programme. According to the Income and Expenditure Household Survey of 2008, approximately two in every three eligible rural households participate in Oportunidades, whereas only one in every three eligible urban households receives the Oportunidades cash transfer. This research explores the factors behind this lack of take-up, the costs of participation and the implications of targeting inefficiency on the programme’s impact on income poverty. It argues that a sample selection model is a pertinent tool of analysis as it informs on the distribution of cash transfers conditional on household eligibility. This conditional distribution is also used to understand the costs of participation as a latent variable. Eligible households are less likely to invest in human capital and neither the cash transfer nor the income forgone by children and teenagers are sufficient to overcome these costs of participation. By identifying a method to quantify behavioural change of households, I associate the costs of participation to the difficulties of inducing health-related behavioural change among recipients and eligible non-recipients. At an aggregate state level, targeting inefficiency is not fully explained by only looking at the budget constraints of the programme. In fact, targeting efficiency is positively associated to aggregate behavioural change and negatively associated to aggregate costs for participation at state level. Yet, targeting efficiency does not guarantee impact on income poverty and Oportunidades’ highest impact on income poverty also associated with the inclusion of non-eligible households in the programme. This research reconsiders the importance of the context in which CCTs are implemented and informs on the conflicting aims of CCTs: providing income poverty relief via cash transfers and incentivizing behavioural change by conditioning the cash transfer in health and education investment.

Preventing forced marriage : a comparative analysis of France and Great Britain

This study aims at ascertaining via a cross-country/cross-city comparison why different national contexts characterized by allegedly opposite ideologies concerning the incorporation of immigrants (namely, the British Race Relations/multicultural model and French republicanism) have led to the adoption of similar policy tools in the prevention of forced unions amongst young people of ethnic minority background. In order to do so, the study will examine French republican and British multicultural rhetoric and policies aimed at the prevention of forced marriage at different institutional levels, with a focus on the preventive role played by the educational sector and within a historical institutionalist theoretical framework. The comparison begins with a consideration of French and British national rhetoric and policies against forced marriage from 1997 to 2008 to develop an adequate framework for the analysis of the preventive role attributed to educational policies in four major localities (the capital cities, Paris and London, and the second two largest cities per population size, Lyon and Birmingham). Despite differences in the policies and rhetoric adopted by multicultural Britain and republican France to tackle forced unions, the study hypothesizes a common trend in the ways French and British public authorities conceptualize the practice of forced marriage - intended mainly as the product of cultural difference. Similarities in the conceptualization of the practice, in turn, have contributed to the identification of similar policy tools despite dissimilar institutional contexts. Such a hypothesis contrasts with one of the key claims of historical institutionalism, according to which dissimilar institutions lead to different policy outcomes across different countries. The study will introduce the role of ideas – in the form of frames (Bleich 2003) – as a tool to explain the reasons why French and British policies aimed at the prevention of forced unions have led to similar policy outcomes despite dissimilar institutional contexts.

Why child health policies in post-apartheid South Africa have not performed as intended : the case of the School Health Policy

The unprecedented scale of health sector reform in the course of radical political transformation in post-apartheid South Africa is well-documented. This thesis examines child health policy reform as a crucial part of this process. The goals of broader health sector reform were to improve the overall health status of citizens, in particular those most vulnerable, and eliminate inequities in health service provision and health status outcomes. Although children were accorded explicit prioritisation during this time, child health indicators remain poor and some have worsened. Amidst the documented explanations for the poor progress with child health indicators, the specific role and contribution of child health policies had not been interrogated. The thesis examines the development, design and implementation of national child health policies, with particular focus on equity. The National School Health Policy serves as a case-study for the analysis. Three complementary policy analysis frameworks guide the enquiry. Findings are based on a documentary analysis of key policies and 81 qualitative interviews with national policy makers and managers, provincial and district managers, and service providers in three socioeconomically different provinces of South Africa. The common assertion by South African health system analysts, that "policies are good, but implementation is poor", is refuted by this research. The findings show that child health policies have many deficiencies in their design and development. These "poor policies" contribute to inadequate child health service provision, which in turn have a bearing on poor child health outcomes. In particular the failure in clearly defining and conceptualising equity in policy development and design contributed to the absence of equity considerations in the implementation phase. The explanations for these policy failures include: lack of strategic direction for child health services; poor policy making capacity; a lack of clear policy translation; and the diverse politics, power and passion of policy actors. Broader health system factors, such as an immature and poorly functioning district health system, compound these policy failures. The thesis deepens the understanding of child health policy reform through a retrospective policy analysis and so contributes to the body of knowledge on policy reform in South Africa and in low- and middle-income countries more generally.

Peace as societal transformation : intergenerational power-struggles and the role of youth in post-conflict Sierra Leone

Intergenerational solidarity and reciprocity are fundamental building blocks of any society. At the same time, socio-generational groups constantly struggle for influence and authority. In Sub-Saharan Africa, disproportionately male, gerontocratic and patrimonial systems governing economic, social and political life lend a special explosiveness to the social cleavage of generation. This dissertation draws on the concept of the generational contract to explore whether Sierra Leone’s decade-long civil war (1991-2001) – labelled a ‘revolt of youth’ – catalysed changes in the power-asymmetries between age groups. Based on fieldwork conducted between 2007 and 2010, I argue that youth in post-war Sierra Leone question fundamental norms of intergenerational relations and challenge local governance structures demanding changes to the generational contract. Amidst a strong continuity of gerontocratic dominance and counter-strategies from elders, youth draw on organisational forms and a local rights discourse to create spaces for contestation and negotiation. These openings hold potential for long-term rearrangements of societal relations in the medium to long-term future.

"Foreign talent" : desire and Singapore's China scholars

This thesis addresses the “foreign talent” situation in Singapore with an ethnographic account of the lived experiences of immigrant PRC students on scholarships, or “PRC scholars.” For some two decades, the Singapore government has annually recruited middle school students from China in their hundreds, selecting them through tests and interviews, granting them full scholarships at either pre-undergraduate or undergraduate level, and, very often, “bonding” them to work subsequently in Singapore for a number of years. Wooed and appropriated in such a way as prized potential human capital, PRC scholars exemplify the Singapore state’s desire for “foreign talent.” In the first decade of the twenty-first century, as the influx of all manners of “foreign talent” into the small city-state gathered pace, local sentiments and discourses of resentment arose. The local-vs-“foreign talent” problem became a serious strain on a city and people proud of their cosmopolitanism. This thesis analyzes the “foreign talent” situation through the ethnographic “macro-trope” of desire. It argues that “foreign talent” is a site of convergence and divergence, collusion and collision, accommodation and contestation, fulfillment and failure of various individual, sociocultural, and political desires and longings. Through the lens of desire, and its psychoanalytic undertones and insights, this thesis looks ethnographically into the PRC scholars’ “foreign talent” journeys in nuanced ways. Based on ethnographic fieldworks carried out in a Chinese middle school and a Singaporean university, the thesis shows how Chinese students are constituted as specific subjects of desire, and how they subsequently develop certain perceptions, attitudes, and stereotypes about the local “other” as well as about themselves after arriving in Singapore as “foreign talent.” Infused with multifarious desires, the PRC scholars’ experiences are often characterized by angst and dissatisfaction; yet it is also argued that generative subjective transformations take place precisely amidst these dynamics and pragmatics of desiring. Ultimately, this thesis seeks to make possible an ethical re-imagination of the “foreign talent” situation in Singapore from the perspective of desire; to provide an account of the so far little-studied Chinese migrant students in the context of Singapore; and to speak more broadly to the cultural and subjective dimensions of human experiences in the context of educational mobility, identity politics, and globalization.

The subject of the thesis Evaluation criteria for the evaluation of directors of houses for children and youth is an analysis of the current status of the evaluation process directors by founders of houses for children and youth (DDM). The aim of this thesis is to determine how the assessment is carried out DDM founder of directors, including the sources, methods and criteria that are used in the evaluation. My fundamental goal by this thesis: To determine whether the results of the evaluation are used in the further development directors DDM and whether the directors and founders believe that the evaluation of the founder directors is necessary for improving the quality of DDM. The theoretical part deals with the definition houses for children and youth, director DDM, founder DDM, staff evaluation and training and development. In the practical part of the research - descriptive type of research, conducted through questionnaires on the basis of three hypotheses. Conclusion the paper provides an overview of the current condition.
