Pregnancy in the mouse : the effect of retinoic acid and gestagens

The formation of the corpus luteum, its maintenance throughout pregnancy and the secretory activity of this transient endocrine gland receive special emphasis because of its role in the secretion of progesterone. An inadequate luteal phase has been associated with an increased incidence of congenital abnormalities. Anything which interferes with the hormonal environment may lead to fetal death or abnormal embryogenesis. Retinoic acid was administered to mice early in pregnancy to induce neural tube defects. Plasma concentrations were determined by high pressure liquid chromatography. Distribution of the retinoic acid was determined by wholebody autoradiography. The hormonal profile during pregnancy was determined in control and retinoic acid treated dams by specific radioimmunoassay for progesterone and 20 alpha dihydroprogesterone. There was no significant effect of retinoic acid on the hormonal profile of treated dams. This suggests that the teratogenic effects of retinoic acid are not due to a change in the hormonal environment. Progesterone and 20 alpha dihydroprogesterone concentrations were significantly increased on days 10 and 10.5 of pregnancy in both treated and control dams. This is the point when pseudopregnancy would end and pregnancy maintenance comes under the control of luteinizing hormone. The increase in 20 alpha dihydroprogesterone concentration at this point in pregnancy was similar to that observed after the administration of prostaglandin F<sub>2alpha</sub> (PGF<sub>2alpha</sub>). It seems likely that PGF<sub>2alpha</sub> and luteinizing hormone act antagonistically during midpregnancy. Concomitant treatment revealed an interaction resulting in an increased progesterone secretion and the conversion of progesterone to 20 alpha dihydroprogesterone. Hence, PGF<sub>2alpha</sub> may be released at the point in pregnancy when luteinizing hormone takes over as the major luteotrophin. Luteinizing hormone thus maintains pregnancy in the mouse in the face of an endogenous PGF<sub>2alpha</sub> release.

FundaÃÃo de Amparo à Pesquisa do Estado do Cearà / INTRODUÃÃO: O misoprostol (CytotecÂ) à um anÃlogo de prostaglandina E1, inicialmente comercializado com finalidades terapÃuticas do trato gastrintestinal, de uso proscrito em gestantes pelo risco de abortamento. A realidade do aborto e o ineficiente controle de medicamentos em nosso paÃs tornaram generalizado seu uso como mÃtodo abortivo. Estudos demonstram que o potencial abortivo do misoprostol à variÃvel e seu uso sem acompanhamento mÃdico pode levar a uma gestaÃÃo que nÃo se perde, gerando risco para o embriÃo/feto. VÃrios relatos de casos e estudos epidemiolÃgicos associam o uso de misoprostol durante a gestaÃÃo com crianÃas malformadas, especialmente seqÃÃncia de MÃebius, defeitos de reduÃÃo de membros e diversas anomalias do sistema nervoso central. O risco do surgimento destes defeitos nÃo pÃde ainda ser determinado com os dados atualmente disponÃveis, fazendo-se necessÃrio a continuidade de pesquisas. OBJETIVOS: Identificar em recÃm nascidos (RNs) malformados nas principais maternidades de Fortaleza e em controles normais a freqÃÃncia de exposiÃÃo ao misoprostol e apresentar o espectro de MFs dos neonatos expostos. MÃTODOS: Estudo de caso controle, com busca ativa diÃria de RNs malformados e controles pareados em quatro maternidades pÃblicas de Fortaleza â CE, no perÃodo de julho a novembro de 2005, para entrevista com a parturiente utilizando questionÃrio estruturado, na busca por exposiÃÃes diversas durante a gestaÃÃo, inclusive ao misoprostol. RESULTADOS: Os grupos eram homogÃneos quanto ao perfil sÃcio-demogrÃfico e principais caracterÃsticas, apresentando diferenÃas somente em desfechos desfavorÃveis mais esperados em RNs MFs, como menor peso (p=0,0001), menor adequaÃÃo peso x idade gestacional (p=0,006), maior natimortalidade (p=0,024) e maior mortalidade perinatal (p=0,0001). Ambos os grupos tiveram Ãndice de assistÃncia prÃ-natal no mÃnimo adequada (69%), diferindo somente na quantidade de ultra-sonografias (US) realizadas a mais pelos casos (p=0,003). Quanto aos fatores de risco, a Ãnica diferenÃa consistiu no grupo caso ter apresentado maior freqÃÃncia em apresentaÃÃo pÃlvica (19,8%, p=0,037). Aproximadamente 84% das mÃes relataram exposiÃÃo a pelo menos um medicamento durante a gestaÃÃo, e a mÃdia de medicamentos consumidos por gestante foi de 3,72. O uso de fumo, Ãlcool e drogas ilÃcitas foi pouco reportado, e somente o fumo apresentou-se mais freqÃente no grupo controle (p=0,0171). A maior parte das gestaÃÃes foi declarada como planejada ou desejada (70% do total). O relato de tentativa de aborto foi de 6,8% do total da amostra, havendo maior citaÃÃo de uso de misoprostol no grupo caso, embora nÃo tenha sido estatisticamente significativo. CONCLUSÃO: Os achados deste estudo sugerem que ocorre uma maior exposiÃÃo de misoprostol durante a gestaÃÃo em RNs malformados comparados a RNs saudÃveis, Odds Ratio (OR) = 3,65 [IC95%: 0,74;17,91]. O espectro de MFs encontradas, associadas ao misoprostol foi: hidrocefalia, mielomeningocele, agenesia de quadril, pà equinovaro, luxaÃÃo de joelho, luxaÃÃo de quadril, agenesia de falanges distais, sindactilia, polidactilia, imperfuraÃÃo anal, ausÃncia de testÃculos no bolso escrotal e higroma cÃstico. / INTRODUCTION: Misoprostol (CytotecÂ) is a prostaglandin E1 analog, previously commercialized with therapeutic purposes, with proscribed use in the gravid due to the risk of miscarriage. The common practice of abortion and the inefficient control on access to drugs in our country has lead to the generalized use of this drug as an abortive method. Studies have shown that the abortive potency of misoprostol is variable and its use without medical assistance may lead to thriving gestation, and a consequent risk of malformation to the fetus. Various case reports and epidemiological studies associate the use of misoprostol during gestation with offspring bearing malformations, especially with the MÃebius sequence, terminal transverse-limb defects and diverse other anomalies of the central nervous system. The risk of acquiring these malformations cannot however be determined with available data, thus, there is a need for continuous research. OBJECTIVE: Identifying newborns bearing malformations in the leading maternities in Fortaleza and selecting their respective study controls, determining the frequency of exposition to misoprostol and identifying the spectrum of anomalies in the exposed group. METHODS: A case control-study, with a daily active search within the four largest maternities in Fortaleza, Ceara for newborn with congenital defects and a paired control, from July to November 2005. Every mother was interviewed with a questionnaire in the search for diverse expositions during pregnancy, including misoprostol. RESULTS: The groups were homogenous in respect to their socioeconomic profile and other main characteristics. Differences were found in respect to unfavorable outcomes among newborn with congenital defects like low weight (p=0,0001), low weight in relation to gestational age (p=0,006), increased natimortality (p=0,024) and increased perinatal mortality (p=0,0001). Both groups had adequate pre-natal assistance (69%), deferring only in the amount of ultrasonographic exams carried out in the case group (p=0,003). In regards to risk factors, the only deference encountered was the case group having more breech presentation (19,8%, p=0,037). Approximately 84% of mothers had been exposed to at least one medication during gestation, and the average number of drugs consumed was 3, 72. The use of tobacco, alcohol and illicit drugs was rarely informed, and only tobacco smoking was more frequent in the control group (p=0,0171). Most gestation were reported as planed or desired (70% of the total). Attempted abortion was reported in 6,8% of whole sample, with the use of misoprostol in the case group, although without statistic significance. CONCLUSION: The findings of these study suggest that exposition to misoprostol during gestation occurs more frequently in newborn with congenital defects compared with healthy ones, Odds Ratio (OR) = 3,65 [IC95%: 0,74;17,91]. The spectrum of malformations found and associated with misoprostol was hydrocephalus, meningomyelocele, agenesis of hip, equinovarus, luxation of the knee, hip congenital dislocation, absence of distal phalanges, syndactyly, polidactyly, anal imperforation, criptorchidism and cystic higroma.

Houve diferença significante no escore Apgar e reflexos entre os grupos, demonstrando uma maior vitalidade neonatal do grupo não clampeado. O segundo estudo avaliou e determinou a incidência de defeitos congênitos encontrados em cães neonatos, relacionou as possíveis causas e determinou a taxa de mortalidade nos recém-nascidos acometidos. Do total de 74 ninhadas com 355 neonatos atendidos, 20 (27%) das ninhadas apresentaram malformações congênitas. As 20 ninhadas possuíam 98 neonatos, dos quais 29 (29,6%) estavam acometidos, e destes, 20 vieram a óbito. O defeito congênito mais comumente encontrado foi a fenda palatina (10/29) 34,5%, seguido pela hidrocefalia (6/29) 20,7%. The neonatal period in dogs has a high mortality rate and the causes are attributed to several factors, including hypoxemia during birth and congenital malformations. The objective of the study was to evaluate two factors for neonatal viability: hypoxemia and congenital defects. In the first study, neonatal viability was determined and compared with the Apgar score and neonatal reflexes, between two groups, neonates who underwent clamping and neonates submitted to non-clamping of the umbilical cord. There was a significant difference in the Apgar score and reflexes between the groups, demonstrating a greater neonatal vitality of the non-clamped group. The second study evaluated and determined the incidence of congenital defects found in neonatal dogs, related the possible causes and determined the mortality rate in newborns affected. Of the total of 74 litters with 355 newborns attended, 20 (27%) of the litters presented congenital malformations. The 20 litters had 98 neonates, of which 29 (29.6%) were affected, and of these, 20 died. Veterinary neonatology is a growing area, research in the area will aid in clinical intervention, better quality of medical treatments, prevention and with this greater neonatal survival. The information obtained in the present studies helps in the diagnosis, prognosis and in the accomplishment of preventive measures during the gestation, the birth and the neonatal period, minimizing the rate of canine neonatal mortality.

Title: Prevalence of asymmetric lateral disability on musculoskeletal apparatus in the neonatal age and infancy. Objective: The purpose of this research thesis was to compile issues of possible types of asymmetric musculoskeletal impairment observed in neonates and infants, identify potential causes of these asymmetries and lateral predilection for reported types of asymmetries. The main subject of discussion are just found lateral predilection of asymmetries and relationship of their causes with prenatal, perinatal or postnatal period. Methods: This thesis is structured in the form of literary research and has descriptively- analytical character. Results: On the basis of the analysis of available sources about the asymmetric musculoskeletal impairment in neonates and infants a comprehensive picture of possible types of these impairment, their causes and lateral predilections was made. The occurrence of congenital defects and/or hereditary diseases in cattle, sheep and buffaloes were 0.89%, 0.36% e 7.55% respectively from all received material. The defects were classified according the affected system. From 48 of congenital defects and/or hereditary diseases observed in cattle 21 (43.75%) affected the skeletal system (chondrodysplasia, scoliosis, lateral deviation of mandible, palatoschisis and unclassified malformation) nine (18.75%) the central nervous system (hypoplasia of olfatory and frontal lobes, cerebellar cortical degeneration, spina bifida, congenital hypomielinogenesis, hereditary hypermetria, cerebellar hypoplasia and pachygiria), nine (18.75%) the muscular system (arthrogryposis), three (6.25%) the cardiovascular system (patent ductus arteriosus and unclassified malformation), one (2.08%) the lymphatic system (hereditary lymphatic hypoplasia), one (2.08%) o alimentary system (atresia ani) and one (2.08%) the eye (congenital cataract). In five cases (10.42%) different systems were affected (diprosopus and decephalus dipos dibraqius). In sheep all observed defects affected various systems (anomalous twins and aprosopia). Of eight cases of congenital defects/hereditary diseases diagnosed in buffaloes three (37.5%) affected the muscular system (arthrogriposis and double muscle), two (25%) the integument (mechanobullous dermatosis and albinism), one (12.5%) the central nervous system (hydranencephaly), one (12.5%) the central nervous system/ integument (hydranencephaly/albinism) and one (12.5%) the alimentary system (megaesofagus). It was concluded that the sporadic congenital defects cause little economical losses in cattle. Environmental congenital defects can cause losses in certain localized geographic areas or farms. The hereditary diseases were important by mortality of animals and by spread of undesirable genes in cattle breeding from milk production region in Southern Brazil. In sheep the congenital defects are rare. In water buffaloes the high prevalence of hereditary diseases was a consequence of the high consanguinity of herd and management measures need to be taken to avoid the spread of recessive genes that are difficult to control. / Foi realizado um estudo das malformações congênitas/doenças hereditárias diagnosticadas em bovinos, ovinos e bubalinos através da revisão dos protocolos de necropsia do Laboratório Regional de Diagnóstico (LRD) da Faculdade de Veterinária da Universidade Federal de Pelotas (UFPel) entre 1978 e 2009. A ocorrência de defeitos congênitos e/ou doenças hereditárias em bovinos, ovinos e bubalinos representou 0,89%, 0,36% e 7,55% respectivamente, de todos os materiais dessas espécies recebidos. Os defeitos foram classificados de acordo com o sistema afetado. Dos 48 casos de defeitos congênitos e/ou doenças hereditárias diagnosticados em bovinos 21 (43,75%) afetaram o sistema esquelético (condrodisplasia, escoliose, desvio lateral da mandíbula, fenda palatina e malformação não classificada) nove (18,75%) o sistema nervoso central (hipoplasia dos lobos frontais e olfatórios, degeneração cerebelar cortical, espinha bífida, hipomielinogênese congênita, hipermetria hereditária, hipoplasia cerebelar e paquigiria), nove (18,75%) o sistema muscular (artrogripose), três (6,25%) o sistema cardiovascular (persistência do ducto arterioso e malformação não classificada), um (2,08%) o sistema linfático (hipoplasia linfática), um (2,08%) o sistema gastrintestinal (atresia anal) e um (2,08%) o olho (catarata congênita). Em cinco casos (10,42%) vários sistemas estavam afetados (diprosopo e dicéfalo dipos dibráquio). Todos os casos de defeitos congênitos observados em ovinos (gêmeos anômalos e aprosopia) afetaram vários sistemas. Dos oito casos de defeitos congênitos e/ou doenças hereditárias diagnosticados em búfalos três (37,5%) afetaram o sistema muscular (artrogripose e hiperplasia muscular), dois (25%) o sistema tegumentar (dermatose mecânico-bolhosa e albinismo), um (12,5%) o sistema nervoso central (hidranencefalia), um (12,5%) o sistema nervoso central/tegumentar/fotorreceptor (hidranencefalia/albinismo), e um (12,5%) o sistema gastrintestinal (megaesôfago). Concluiu-se que os defeitos congênitos esporádicos têm pouca importância em bovinos. Defeitos congênitos de causas ambientais podem trazer prejuízos econômicos importantes em determinadas regiões ou estabelecimentos. As doenças comprovadamente hereditárias são importantes não só pela mortalidade mas, também, pela possibilidade de disseminação de genes indesejáveis nas diferentes raças, principalmente aquelas criadas em pequenas propriedades rurais da bacia leiteira da região. Em ovinos os defeitos congênitos são raros. Em bubalinos a alta frequência de doenças hereditárias na raça Murrah foi atribuída a alta consanguinidade do rebanho e medidas de controle devem ser tomadas para evitar-se a contínua disseminação, principalmente dos genes recessivos que são mais difíceis de controlar.

Objetivo: Verificar se há diferença entre o consumo habitual de folato entre mães de crianças com Síndrome de Down e mães de crianças sem malformações. Métodos: Foi realizado um estudo de caso-controle, com um total de 100 mães das quais 50, incluídas no grupo caso, apresentavam filhos com Síndrome de Down e 50, consideradas grupo controle,tinham filhos sem malformações congênitas. Aplicou-se um questionário de consumo habitual de alimentos contendo questões relativas à classificação sócio-econômica e ao consumo de alimentos-fonte e alimentos fortificados com esta vitamina. Todas participantes assinaram o Termo de Consentimento Livre e Esclarecido. Resultados: Entre as variáveis analisadas, somente a quantidade de ácido fólico consumida habitualmente e a idade no momento do nascimento do filho, diferiram-se significantemente entre os grupos. Mães caso consumiram em média 359,1 μg/dia (dp ± 91,9) de folato, enquanto as mães do grupo controle, 425,5 μg/dia (dp± 104,3), (p=0,001). A idade que tiveram seus filhos foi no grupo caso 27, 5 anos (dp± 4,8) de e no grupo controle de 25,4 (dp ±5,3), (p=0,042). Somente 3,4% das entrevistadas relataram uso de ácido fólico ou polivitamínicos de forma periconcepcional ou em algum momento da gestação. Conclusões: Embora pelo presente estudo tenha havido diferença significativa entre o consumo de folato no grupo caso e no controle é importante que não se descarte possíveis fatores bioquímicos envolvidos e aqui não avaliados. / Objective: Check if exists difference between the usual consumption of folate in mothers of children with Down Syndrome and in mothers of children without congenital abnormalities. Methods: A case-control study was accomplished, with a sample of 100 mothers from which, 50 had children with Down Syndrome and 50, considered group control, children without congenital abnormalities. A questionnaire of quantitative frequency was applied containing questions related to the socioeconomic class and the food-source consumption, and foods fortified with folic acid. All participants signed the Informed Consent Form. Results: Among the variables in analysis, only the daily amount of folic acid consumed and the age that the mothers had their babies were significant different between the two groups. Mothers of children with Down Syndrome consumed 359,1 μg/day (dp ± 91,9) of folate, while the mothers of control group consumed, 425,5 μg/day (dp± 104,3), (p=0,001). The age that they had their babies were in group case 27, 5 years (dp± 4,8) and in the control group, 25,4 (dp ±5,3), (p=0,042). Only 3,4% of the interviewee related use of folic acid or others vitamines before or during the pregnancy. Conclusion: Although in the present study has been a significant difference between the consumption of folate in the case group and in the control, is important not to discard possible biochemical factors involved and here not evaluated.
