Entretanto, nos cães com degeneração mixomatosa da válvula mitral (DMVM), valvulopatia caracterizada pela sobrecarga do átrio esquerdo (AE), a avaliação da sobrecarga atrial é realizada através da obtenção do diâmetro do AE e sua relação com o diâmetro da artéria Aorta (Ao) (AE:Ao), auxiliando na classificação da insuficiência cardíaca congestiva (ICC). Portanto, procurou-se mensurar os valores referentes ao VAE indexados à ASC de cães sadios e portadores da DMVM, utilizando o método biplanar de Simpson. Para isso, foram avaliados 107 cães sadios (controle) e 81 portadores da DMVM classes funcionais Ia, Ib e II da ICC (ISACHC), com o cálculo do VAE nos momentos da diástole (d) e sístole (s) atrial utilizando o método biplanar de Simpson. As imagens apicais quatro (4C) e duas câmaras (2C) foram obtidas por meio da ecocardiografia bidimensional via janela paraesternal esquerda (JPE) e direita (JPD) em todos os cães avaliados. Os valores referentes aos cães sadios foram correlacionados com o peso corporal utilizando o teste linear de Pearson e submetidos ao teste de normalidade estatística. Na comparação dos cães sadios com os portadores da DMVM, bem como entre as classes funcionais de ICC, utilizou-se análise de variância (ANOVA) e o teste de Tukey quando p<0,05. Os resultados demonstraram uma correlação alta e positiva das variáveis VAEd (r>0,77) e VAEs (r>0,73) com o peso corporal nos cães sadios, com posterior indexação dos valores na ASC e aprovação no teste de normalidade. Já os cães ... / The measurement of left atrial volume (LAV) indexed to body surface area ( ASC ) is obtained by the biplane Simpson method via echocardiography , and considered a prognostic marker in assessing left atrial cardiomyopathy and valvular heart disease in medicine . However , in dogs with myxomatous mitral valve (MMVD), valvular heart disease characterized by an overload of the left atrium (LA) , the assessment of atrial enlargement is accomplished by obtaining the LA diameter and its relation to the diameter of the aorta (Ao ) (LA:Ao) , assisting in the classification of congestive heart failure (CHF) . Therefore, we sought to measure the values for LAV indexed to ASC of healthy dogs and MMVD carriers using the biplane Simpson method. . Thus, 107 healthy dogs (control) and 81 patients with functional MMVD classes Ia, Ib and II of the CCI (ISACH) were evaluated by calculating the time of VAE in diastole (d) and systole (s) fibrillation using biplane Simpson’s method. The framework (4C) and apical two-chamber images (2C) were obtained by two-dimensional echocardiography via left parasternal (JPE) in all dogs evaluated. The values for the healthy dogs were correlated with body weight using the Linear Test Pearson and submitted to statistical normality test. In comparison with healthy carriers of MMVD dogs, as well as between functional classes of ICC, we used analysis of variance (ANOVA) and Tukey's test at p<0.05. The results showed high positive correlation between LVAd (r>0.77) and LVAs (r > 0.73) variables with body weight in healthy dogs, with subsequent indexation of amounts on ASC and passing the normality test correlation. Recently, studies have shown that inorganic nitrate (NO3-) from diet is an alternative source of NOproduction, especially in situations of low oxygenation and reduced pH, as in exercise muscle. Patients with coronary artery disease (CAD) or even cardiovascular risk factors (CRF) have endothelial dysfunction, characterized by reduced bioavailability of ON, which increases the risk of triggering cardiovascular events. Objective: The aim of this study was to investigate the acute effect of physical exercise combined with inorganic nitrate supplementation on endothelial function in patients with CAD. Methods: Nine men with recent AMI (54.2 ± 5.6 years) were randomized in a cross-over / open label study and participated in two exercise sessions with supplementation of nitrate derived from beet juice (450mL, ~8mmol NO3-) or Placebo (water). Plasma nitrite, flow-mediated dilation (FMD) and blood pressure (BP) were measured before and after the exercise session. After a washout period of 7 days, the groups were crossed.After a washout period (seven days), the groups were crossed.Results:The plasma nitrite concentration was 14.0 to 5.9 μM / g protein (p = 0.042) after the exercise session. In those supplemented with beet juice, nitrite levels were preserved. Regardless of this variation, there was no significant difference in endothelial function. In relation toBP, there was no reduction in systolic (SBP) and diastolic (DBP) pressure in the exercise or in this ally of the supplement. Conclusion: In patients with CAD, supplementation of beet juice did not increase nitrite concentration. Parallel to this, no changes were found in FMD, SBP and DBP values. / Introdução: O Oxido Nítrico (ON),sintetizado a partir da L-arginina pela enzima NO-sintase (NOS),tem importante papel na modulação vascular, em especial durante o exercício físico. Recentemente, estudos têm demonstrado que o nitrato inorgânico (NO3-), proveniente da dieta, se configura como fonte alternativa de produção de ON, principalmente em situações de baixa oxigenação e pH reduzido, como no músculo em exercício. Portadores de doença arterial coronariana (DAC) ou mesmo fatores de riscocardiovascular(FR) apresentam disfunção endotelial, caracterizada pela redução na biodisponibilidade de ON, e tem neste cenário uma elevação nas chances de desencadear eventos cardiovasculares. Objetivo: Verificar o efeito agudo do exercício físico aliado àsuplementação de nitrato inorgânico na função endotelial em portadores de DAC. Métodos: Nove homens com IAM recente (54,2± 5,6anos), foram randomizados em estudo cruzado/aberto e participaram de duas sessões de exercícios, com suplementação de nitrato derivado do suco de beterraba (450mL, ~8mmol de NO3-)ou placebo (água). Dosagem plasmática de nitrito, dilatação mediada pelo fluxo (DMF) e pressão arterial (PA) foram mensuradas antes e após a sessão de exercício. Após um período de washout de 7 dias, houve cruzamento dos grupos. Resultados: A concentração de nitritoplasmático foide 14,0 para 5,9 μM/g de proteína(p=0,042) após a sessão de exercícios.Naqueles suplementados com suco debeterrabaos níveis de nitritosforam preservados. Independente desta variação, não houve diferença na função endotelial. Com relação a PA, não houve redução da pressão arterial sistólica (PAS)ediastólica (PAD) no exercício ou neste aliado ao suplemento. Conclusão: Emportadores de DAC, a suplementação de suco de beterraba não elevou a concentração de nitrito. Paralelo a isso, não foram encontradas alterações nos valores de DMF, PAS e PAD.

Avaliação comparativa do uso da furosemida em bolus ou infusão contínua no tratamento de cães com doença degenerativa valvar /

Entretanto, estudos em seres humanos com ICC demonstraram que a furosemida em taxa infusão contínua (TIC) apresenta maior eficácia e segurança em relação à administração em BI. Portanto, o objetivo deste estudo foi comparar a eficácia do diurético furosemida em TIC e BI em cães com degeneração mixomatosa valvar mitral. Para este fim, o estudo incluiu 20 cães, entre 8 e 12 anos, com peso entre 8 e 15 kg, e ICC classes Ia, Ib e II - ISACHC, e o grupo controle. Para o tratamento TIC, os cães receberam 0.66mg/kg IV furosemida como uma dose de carga seguida de 0.66mg/kg/h durante 8 horas. No tratamento BI os cães receberam 3 mg/kg IV furosemida na 0 e 4 horas. Os cães foram submetidos a tratamento por TIC e 15 dias mais tarde, os mesmos cães foram submetidos a tratamento por BI. Para ambos os tratamentos, foi administrado o mesmo volume de fluido e a produção de urina foi quantificada a cada hora. A ingestão hídrica, densidade urinária, pH urinário foram avaliados por hora. As variáveis sanguíneas, hematócrito (Ht), hemoglobina (Hb), creatinina (creat), ureia, proteína total (PT), albumina (alb) e os eletrólitos sódio (Na), potássio (K), magnésio (Mg), cálcio (Ca) e fosforo (P) foram avaliados a cada 2 horas. O peso e as variáveis ecocardiográficas foram avaliados antes e depois do tratamento. A produção urinaria por hora (mL/kg/h) e total (mL/kg/8h) foi superior no tratamento TIC em todos os animais (p<0,05). Os parâmetros ingestão hídrica, densidade urinária, diferença de pH urinário, diferença de peso, Ht, Hb, ureia, creat e fósforo junto com as diferenças das variáveis ecocardiográficas não apresentaram diferenças significativas entre os tratamentos (p>0,05). A furosemida... / Abstract: Congestive heart failure (CHF) is a syndrome that leads to inadequate blood supply to the tissue due to heart dysfunction and is treated with furosemide in intermittent bolus (BI). However, studies in humans with CHF showed that furosemide continuous rate infusion (CRI) showed better efficacy and safety for the administration in BI. Therefore, the aim of this study was to compare the effectiveness of the diuretic furosemide in CRI and BI in dogs with mitral valve myxomatous degeneration. To this end, the study included 20 dogs between 8 and 12 years old, weighing between 8 and 15 kg, and ICC classes Ia, Ib and II - ISACHC, and the control group. CRI for the treatment, the dogs received 0.66mg/kg IV furosemide as a loading dose followed by 0.66mg/kg/h for 8 hours. BI treatment the dogs received 3 mg / kg IV furosemide at 0 and 4 hours. The dogs were treated by CRI and 15 days later, the same dogs were treated by BI. For both treatments, the same volume of fluid and urine production was quantitated was administered hourly. Variabilidade da frequência cardíaca e a dinâmica miocárdica em cães com degeneração mixomatosa da valva mitral submetidos a um programa de condicionamento físico /

Embora as estratégias de tratamento mudem de acordo com o estágio da doença, programas de exercício não integram a terapia padrão da enfermidade. Em seres humanos já se demonstrou que o treinamento apropriado pode ser, inclusive, benéfico como adjuvante do tratamento de algumas enfermidades cardiovasculares. Nesse sentido, objetivou-se com este estudo avaliar os efeitos promovidos pelo condicionamento sistematizado em esteira em cães portadores de DMVM. Para tanto, 19 cães foram selecionados em quatro grupos conforme a severidade da enfermidade (fundamentada no tamanho da relação átrio esquerdo/aorta - Grupo A: controle; Grupo B: Ae/Ao<1,4; Grupo C: Ae/Ao1,4-1,8; e, Grupo D: Ae/Ao>1,8). O teste de esforço incremental foi realizado para detecção do limiar de lactato. Na sequência, foram realizadas oito semanas consecutivas de condicionamento, com base em 70% da velocidade do limiar de lactato. Os animais foram avaliados quanto à variabilidade da frequência cardíaca, além da dinâmica miocárdica, por meio de ecocardiografia convencional e tecidual. Foi empregado o teste de normalidade dos resíduos e análise de variância, seguida do teste de Tukey para comparação das médias dos diversos parâmetros estudados. As análises estatísticas foram realizadas com nível de significância de 5%. A análise estatística multivariada exploratória foi utilizada para a extração dos fatores, segundo critério de Kaiser. Os resultados de ecocardiografia demostraram aumento do diâmetro interno do ventrículo esquerdo em diástole, oito semanas após o condicionamento na comparação com avaliação inicial no grupo C, assim como, aumento do septo interventricular em sístole quatro semanas e oito semanas em relação ao valor basal no grupo A. Although treatment strategies change according to the stage of the disease, exercise programs are not part of the standard therapy of the disease. In human beings, it has been demonstrated that proper training can be beneficial as adjunctive treatment of certain cardiovascular diseases. This study evaluated the effects of systematic training on a treadmill in dogs with MMVD. Nineteen dogs were selected and divided into four groups according to the severity of illness (based on the size of the left atrium-to-aorta ratio - Group A: control; Group B: LA/Ao<1.4; Group C: LA/Ao 1.4 to 1.8, and Group D: LA/Ao>1.8). The incremental exercise test was performed to detect the lactate threshold. Eight consecutive weeks of training were performed based on 70% of the lactate threshold velocity. The animals were evaluated for the heart rate variability, as well as myocardial dynamics, by standard and tissue Doppler echocardiography. Test for residual normality, analysis of variance, and Tukey's test were all used to compare the means of several parameters. Statistical analyses were performed at a level of significance of 5%. Exploratory multivariate statistical analysis was used for the extraction of factors, according to the Kaiser criteria. The echocardiography results showed increased left ventricular internal diameter in diastole eight weeks after training as compared to the baseline value in group C, as well as an increased interventricular septum in systole at four weeks and eight weeks as compared to the baseline value in group A. Biomarcadores cardíacos em cães com degeneração mixomatosa da valva mitral submetidos a um programa de condicionamento físico /

A utilização de biomarcadores cardíacos pode ser indicada como meio para monitorizar respostas a tratamentos e atividade física. Assim, o estudo em questão teve como escopo avaliar os biomarcadores cardíacos em cães com doença mixomatosa da valva mitral (DMVM), submetidos ao exercício físico, correlacionando os valores das concentrações de tais substâncias com os índices ecocardiográficos estudados. Para tanto, 15 cães com DMVM foram selecionados e alocados em três grupos, conforme a severidade da enfermidade (baseada no tamanho da relação átrio esquerdo/aorta). O teste de esforço incremental foi realizado para detecção do limiar de lactato e velocidade correspondente. Na sequência, foram realizadas oito semanas consecutivas de treinamento, com base em 70% da velocidade do limiar de lactato. Foram documentas as concentrações sanguíneas dos biomarcadores cardíacos (NT-proANP, NT-proBNP, troponina I cardíaca, CK-MB, LDH I e AST) e as variáveis ecocardiográficas nos momentos inicial (M0), 4 (M4) e 8 (M8) semanas após o início do protocolo de exercício. No grupo AE/Ao<1,4 foi identificada diferença significativa no NT-proANP em M8 comparado ao M0. Para os demais biomarcadores não houve diferença entre grupos e momentos ao longo do condicionamento. Por meio de análise estatística multivariada, foram extraídos 8 fatores, sendo que o fator 1, composto por FEC% e FEJ%, representou 17,05% da variância total dos dados. Já os fatores compostos por biomarcadores apresentaram menor representatividade. Os resultados desta pesquisa permitem concluir que o exercício físico sistematizado, da forma como realizado, não promoveu alteração nas... / Abstract: As opposed to human medicine, which already uses the exercise as a therapeutic tool in several heart diseases, it is not common sense for small animal practitioners to recommend such treatment for dogs with heart disease owing to an absence of studies. Cardiac biomarkers may be used to monitor response to treatment and physical activity. Therefore, this study had to goal of assessing how these substances perform in dogs with myxomatous mitral valve disease (MMVD) undergoing a physical exercise program. For this purpose, 15 dogs were ascribed to three groups (in accordance with the leftatrium- to-aorta ratio). Incremental exercise test was performed to detect the lactate threshold. In the following eight weeks, the animals underwent training exercises based on 70% of the lactate threshold velocity. Echocardiographic parameters and cardiac biomarkers (NT-proANP, NT-proBNP, Cardiac troponin I, CK-MB, LDH I and AST) were evaluated at the starting time (M0), and at 4 (M4) and 8 (M8) weeks after the beginning of the exercise protocol. In the LA/Ao <1.4 group, a significant difference was found in NT-proANP at M8 as compared to M0. No differences were documented for the remaining biomarkers throughout the training. Using multivariate analysis, 8 factors were extracted. Factor 1, which comprises fractional shortening and ejection fraction, represented 17.05% of the data variability, while the factors composed by biomarkers showed lower representativeness. Based on these results, it was concluded that the systematic exercise, at least the way it was performed in this investigation, did not promote changes in the concentrations of both the stretch and myocytes injury biomarkers. Os microRNAs regulam a expressão gênica pós-transcricionalmente, primariamente pela associação com a região 3' e/ou 5' não traduzida (3' UTR, 5' UTR) de seus RNAs mensageiros alvos geralmente reprimindo e, em alguns casos, estimulando a tradução. Atualmente, embora o número de genes relacionados com o desenvolvimento de doenças musculares aumente a cada ano, as vias moleculares envolvidas permanecem pobremente compreendidas. Porém, estudos recentes demonstram a importância da regulação mediada por microRNAs em diversos processos biológicos e em algumas doenças do músculo esquelético. A Insuficiência cardíaca (IC) é uma síndrome clínica associada à disfunção cardíaca, diminuição da expectativa de vida e intolerância aos exercícios físicos. Essa intolerância aos exercícios físicos, bem como a redução da atividade locomotora, estão associados aos principais sintomas dos pacientes com IC: a fadiga e a fraqueza muscular. Esse fenômeno é decorrente, em parte, da mudança no metabolismo de oxidativo para glicolítico e da presença de atrofia e alterações nos tipos de fibras musculares. Nesse estudo, analisamos a expressão de 303 microRNAs no músculo sóleo da ratos Wistar macho com IC induzida por monocrotalina (60 mg/kg). O músculo sóleo dos animais com IC apresentaram uma alteração na expressão relativa de 19 microRNAs, com diminuição na expressão de 05 microRNAs (< 50%; P<0,05) e aumento na expressão de 14 microRNAs (> 150%; P<0,05) quando comparado com o grupo controle. Nossos dados mostram que os miRNAs miR-29b, miR-27a-5p, miR-434-3p, miR-539, miR-489, miR-214, miR-632 e miR-146b são regulados na miopatia esquelética que ocorre na insuficiência cardíaca, sugerindo que essas moléculas possam atuar ... / Abstract: MicroRNAs (miRNAs) are small (~17-25 nt) non-coding RNAs regulating mRNAs involved in various biological processes, including the pathophysiology of striated muscles. miRNAs suppress gene expression through their complementarity to the sequence of one or more mRNAs, usually at a site in the 3′ untranslated region. The formation of an miRNA-target complex results either in inhibition of protein translation or in degradation of the mRNA transcript. There is no doubt that the formation, maintenance, and physiological and pathophysiological responses of skeletal muscles, with all their complex regulatory circuits, are subject to regulation by non-coding RNAs. Heart failure (HF) is characterized by a skeletal muscle myopathy with increased expression of fast myosin heavy chains (MyHCs) and atrophy. The skeletal muscle-specific molecular regulatory mechanisms controlling MyHC expression during HF have not been described and microRNAs may be related to these alterations. Using RT-qPCR TaqMan Low Density Arrays, we measured the expression of 303 microRNAs in soleus skeletal muscle from Wistar rat with monocrotaline (60 mg/kg)-induced HF. RNA isolated from soleus muscle of rat with HF showed increased expression of 14 microRNAs (> 150%; P<0,05) compared with controls. Similarly, HF-induced atrophy were accompanied by reduced expression of 05 microRNAs (< 50%; P<0,05). Interestingly, we outline the miRNAs miR-29, miR-214, and miR-489 that have been found to be dysregulated in diseases associated with skeletal muscle or are changed during muscle adaptation. Incidência de parada cardíaca e mortalidade durante anestesia em hospital universitário de atendimento terciário no período de 2005 a 2012 /

A parada cardíaca (PC) e o óbito são os piores desfechos clínicos. A investigação das incidências dessas complicações é difícil devido à diferença metodológica encontrada na literatura. Assim, comparações das incidências e causas entre os estudos devem ser feitas com ressalvas. Nos últimos anos, melhorias técnicas na prática anestesiológica podem ter contribuído para a diminuição na incidência de PC e de óbito no Hospital das Clínicas da Faculdade de Medicina de Botucatu, Universidade Estadual Paulista. Objetivou-se com este estudo reavaliar a incidência, as causas e os fatores desencadeantes de PC e de óbito em pacientes anestesiados em hospital universitário de atendimento terciário no período de 2005 a 2012. Método. Os casos de PC e óbito em 55.002 anestesias consecutivas foram identificados em banco de dados de complicações anestésicas. A incidência de PC e de óbito foi calculada em relação aos atributos: faixa etária, sexo, classificação do estado físico da American Society of Anesthesiologists (ASA), tipo de atendimento, técnica anestésica, especialidade cirúrgica e fatores desencadeantes. Uma comissão classificou todas as PC e os óbitos, de acordo com os seguintes fatores desencadeantes: 1) totalmente relacionada à doença/condição do paciente; 2) totalmente relacionada à cirurgia; 3) totalmente relacionada à anestesia; e 4) parcialmente relacionada à anestesia. As incidências foram expressas para 10.000 anestesias, juntamente com o intervalo de confiança (IC) de 95%. Resultados. Ocorreram 139 PC (25,1 por 10.000 anestesias; IC 95%: 21,4 - 29,8) e 88 óbitos (16,0 por 10.000 anestesias; IC 95%: 13,0 - 19,7). Cardiac arrest (CA) and death are the worst outcomes in medicine. To survey these complications is difficult because of the methodological variation in literature. As a result, the comparison of incidence and triggering factors between studies should be made sparingly. Technical improvement in our institutional anesthesia care may have declined the CA and death incidences in the last years. Then we intended to reexamine the CA and mortality incidence, causes and triggering factors in a Brazilian tertiary teaching hospital from 2005 to 2012. Methods. Cardiac arrest and death were identified from an anesthetics complications database. Cardiac arrest and death incidence was calculated for the following characteristics: age, sex, ASA physical status classification, surgical procedure, anesthesia technique, surgery specialty, and triggering factors. All CA and death were reviewed and classified into following four groups of triggering factors: 1) totally patient disease/condition-related; 2) totally surgery-related; 3) totally anesthesia-related; or 4) partially anesthesia-related. The incidences were expressed per 10,000 anesthetics with 95% confidence interval (95% CI). Results. A total of 139 CA (25.1 per 10,000; 95% CI: 21.4 - 29.8) and 88 deaths (16.0 per 10,000; 95% CI: 13.0 - 19.7) were counted. The major CA and death incidence occurred in neonates and elderly (p = 0.03 and p = 0.028, respectively), ASA physical status ≥ III (p = 0.01 and p = 0.028, respectively), emergency surgery (p = 0.023 and p = 0.03, respectively), general anesthesia (p = 0.03 and p = 0.001, respectively), and multispecialty, cardiac and vascular surgery (p = 0.04 and p = 0.01, respectively). São escassos na literatura trabalhos que avaliam a associação do hábito alimentar e adiposidade corporal com fatores de risco cardiometabólico em crianças, principalmente nas fases pré-escolar e escolar. O objetivo deste estudo foi investigar a associação do padrão alimentar e adiposidade corporal com fatores de risco cardiometabólico em crianças de 4 a 7 anos de idade. Trata-se de um estudo de coorte retrospectiva, conduzido com 403 crianças nascidas no único hospital maternidade de Viçosa, Minas Gerais, Brasil, e que foram acompanhadas pelo Programa de Apoio à Lactação (PROLAC) no primeiro ano de vida. Foram coletados dados nos prontuários do PROLAC referentes ao nascimento e os primeiros meses de vida, como peso ao nascer e tempo de aleitamento materno exclusivo (AME). Na coleta de dados relativa às idades entre 4 e 7 anos, foram obtidas informações sociodemográficas, de estilo de vida e de consumo alimentar. O hábito alimentar das crianças foi avaliado por meio da identificação de padrões alimentares, utilizando-se a Análise de Componentes Principais (ACP). Foram aferidas as medidas de peso, estatura e perímetro da cintura das crianças, procedendo-se ao cálculo do índice de massa corporal por idade (IMC/I) e da relação cintura-estatura (RCE). A composição corporal foi avaliada pela técnica Dual Energy X-ray absorptiometry (DEXA). Realizou-se coleta de sangue para dosagem dos seguintes parâmetros bioquímicos: colesterol total, HDL, LDL, triglicerídeos, glicemia de jejum e insulina de jejum (em uma subamostra). Para avaliação da resistência à insulina, calculou-se o índice triglicerídeos-glicemia (TyG). Foi também avaliada a pressão arterial das crianças, adotando-se protocolo estabelecido. Observou-se que o aumento da adiposidade abdominal (avaliada pelo perímetro da cintura, percentual de gordura central e RCE) foi mais prevalente entre as crianças com excesso de peso. O índice RCE foi o que apresentou maior acurácia na predição do excesso de peso entre as crianças do estudo (AUC = 0,91; IC95%: 0,86-0,96). Os indicadores de adiposidade corporal total e central associaram-se positivamente com a pressão arterial sistólica e diastólica, após ajuste por outras variáveis. Foram identificados neste estudo cinco padrões alimentares, que explicaram 42,3% da variância dos dados, sendo denominados: “Tradicional”, “Não saudável”, “Leite e achocolatado”, “Lanche” e “Saudável”. Encontrou-se que as crianças mais velhas, as que não receberam AME até, pelo menos, o quarto mês de vida e as de maior renda, apresentaram maior probabilidade de consumir alimentos do padrão alimentar “Não saudável. Além disso, verificou-se associação entre menor tempo de AME e moderado consumo de alimentos do padrão “Lanche”. Ainda, as crianças com maior renda per capita também demonstraram maior adesão aos padrões “Lanche” e “Saudável”. Quanto à associação entre os indicadores de adiposidade corporal total e central e os escores dos padrões alimentares, observou-se que a maior adesão ao padrões “Tradicional” e “Não saudável” relacionou-se a maiores valores dos indicadores de adiposidade. Na investigação dos fatores associados ao índice TyG, observou-se que quanto menor o tempo diário em atividades ativas, maior o valor de TyG; e o aumento nos valores do IMC, RCE, percentual de gordura total e central, associou-se positivamente com o TyG. Ainda, pela análise da curva ROC, o ponto de corte do índice TyG para predição da resistência à insulina entre as crianças do estudo, com melhor equilíbrio entre os valores de sensibilidade e especificidade, foi o valor de 7,88. Os resultados deste estudo apontam que o excesso de adiposidade corporal total e central está associado à fatores de risco cardiometabólico, como aumento da pressão arterial e risco de resistência à insulina. Além disso, a maior adesão a padrões alimentares compostos por alimentos de alto valor energético e ricos em carboidratos simples e gorduras, relaciona-se ao aumento da adiposidade corporal na infância. / Due to changes in the dietary habits of the population, there is a greater consumption of foods rich in fats and simple carbohydrates, which has contributed to the increase in the prevalence of overweight and other cardiometabolic risk factors. There are few studies in the literature that evaluate the association of dietary habits and body adiposity with cardiometabolic risk factors in children, especially in the preschool and school phases. The objective of this study was to investigate the association of dietary pattern and body adiposity with cardiometabolic risk factors in children from 4 to 7 years of age. This is a retrospective cohort study, conducted with 403 children born in the only maternity hospital in Viçosa, Minas Gerais, Brazil, and who were followed up by the Lactation Support Program (PROLAC) in the first year of life. Data were collected in the PROLAC records regarding birth and the first months of life, such as birth weight and exclusive breastfeeding time (EB). Data on the ages ranging from 4 to 7 years were obtained from sociodemographic, lifestyle and food consumption data. The dietary habits of the children were evaluated through the identification of dietary patterns, using Principal Component Analysis (PCA). The measures of weight, height and waist circumference of the children were measured, and the body mass index by age (BMI for age) and waist-height ratio (WHR) were calculated. Body composition was evaluated by Dual Energy X-ray absorptiometry (DEXA). Blood samples were collected for the following biochemical parameters: total cholesterol, HDL, LDL, triglycerides, fasting glucose and fasting insulin (in a sub-sample). For the evaluation of insulin resistance, the triglyceride-glycemia index (TyG) was calculated. The children's blood pressure was also evaluated, adopting an established protocol. It was observed that the increase in abdominal adiposity (evaluated by waist circumference, percentage of central fat and WHR) was more prevalent among overweight children. The WHR index was the one that presented the greatest accuracy in the prediction of overweight among the children of the study (AUC = 0.91, 95% CI: 0.86- 0.96). The indicators of total and central body adiposity were positively associated with systolic and diastolic blood pressure, after adjusting for other variables. Five food standards were identified in this study, which explained 42.3% of the variance of the data: "Traditional", "Unhealthy", "Milk and chocolate", "Snack" and "Healthy". It was found that older children, those who did not receive EB until at least the fourth month of life and those with higher income, were more likely to consume food of the "unhealthy" diet. In addition, there was an association between shorter SMA time and moderate food consumption of the "Snack" standard. Still, children with higher per capita income also showed greater adherence to the "Snack" and "Healthy" standards. As for the association between the indicators of total and central body adiposity and the scores of the dietary patterns, it was observed that the greater adherence to the "Traditional" and "Unhealthy" standards was related to higher values of the adiposity indicators. In the investigation of factors associated with the TyG index, it was observed that the lower the daily time in active activities, the higher the TyG value; And the increase in BMI, WHR, percentage of total and central fat, was positively associated with TyG. Also, by the analysis of the ROC curve, the TyG index cut-off point for predicting insulin resistance among the children in the study, with a better balance between the sensitivity and specificity values, was the value of 7.88. The results of this study indicate that excess of total and central body adiposity is associated with cardiometabolic risk factors, such as increased blood pressure and insulin resistance. In addition, the higher adherence to dietary patterns composed of foods of high energy value and rich in simple carbohydrates and fats, is related to the increase of body adiposity in childhood.

Embora as estratégias de tratamento mudem de acordo com o estágio da doença, programas de exercício não integram a terapia padrão da enfermidade. Em seres humanos já se demonstrou que o treinamento apropriado pode ser, inclusive, benéfico como adjuvante do tratamento de algumas enfermidades cardiovasculares. Nesse sentido, objetivou-se com este estudo avaliar os efeitos promovidos pelo condicionamento sistematizado em esteira em cães portadores de DMVM. Para tanto, 19 cães foram selecionados em quatro grupos conforme a severidade da enfermidade (fundamentada no tamanho da relação átrio esquerdo/aorta - Grupo A: controle; Grupo B: Ae/Ao<1,4; Grupo C: Ae/Ao1,4-1,8; e, Grupo D: Ae/Ao>1,8). O teste de esforço incremental foi realizado para detecção do limiar de lactato. Na sequência, foram realizadas oito semanas consecutivas de condicionamento, com base em 70% da velocidade do limiar de lactato. Os animais foram avaliados quanto à variabilidade da frequência cardíaca, além da dinâmica miocárdica, por meio de ecocardiografia convencional e tecidual. Foi empregado o teste de normalidade dos resíduos e análise de variância, seguida do teste de Tukey para comparação das médias dos diversos parâmetros estudados. As análises estatísticas foram realizadas com nível de significância de 5%. A análise estatística multivariada exploratória foi utilizada para a extração dos fatores, segundo critério de Kaiser. Os resultados de ecocardiografia demostraram aumento do diâmetro interno do ventrículo esquerdo em diástole, oito semanas após o condicionamento na comparação com avaliação inicial no grupo C, assim como, aumento do septo interventricular em sístole quatro semanas e oito semanas em relação ao valor basal no grupo A. Although treatment strategies change according to the stage of the disease, exercise programs are not part of the standard therapy of the disease. In human beings, it has been demonstrated that proper training can be beneficial as adjunctive treatment of certain cardiovascular diseases. This study evaluated the effects of systematic training on a treadmill in dogs with MMVD. Nineteen dogs were selected and divided into four groups according to the severity of illness (based on the size of the left atrium-to-aorta ratio - Group A: control; Group B: LA/Ao<1.4; Group C: LA/Ao 1.4 to 1.8, and Group D: LA/Ao>1.8). The incremental exercise test was performed to detect the lactate threshold. Eight consecutive weeks of training were performed based on 70% of the lactate threshold velocity. The animals were evaluated for the heart rate variability, as well as myocardial dynamics, by standard and tissue Doppler echocardiography. Test for residual normality, analysis of variance, and Tukey's test were all used to compare the means of several parameters. Statistical analyses were performed at a level of significance of 5%. Exploratory multivariate statistical analysis was used for the extraction of factors, according to the Kaiser criteria. The echocardiography results showed increased left ventricular internal diameter in diastole eight weeks after training as compared to the baseline value in group C, as well as an increased interventricular septum in systole at four weeks and eight weeks as compared to the baseline value in group A. Entretanto, estudos em seres humanos com ICC demonstraram que a furosemida em taxa infusão contínua (TIC) apresenta maior eficácia e segurança em relação à administração em BI. Portanto, o objetivo deste estudo foi comparar a eficácia do diurético furosemida em TIC e BI em cães com degeneração mixomatosa valvar mitral. Para este fim, o estudo incluiu 20 cães, entre 8 e 12 anos, com peso entre 8 e 15 kg, e ICC classes Ia, Ib e II - ISACHC, e o grupo controle. Para o tratamento TIC, os cães receberam 0.66mg/kg IV furosemida como uma dose de carga seguida de 0.66mg/kg/h durante 8 horas. No tratamento BI os cães receberam 3 mg/kg IV furosemida na 0 e 4 horas. Os cães foram submetidos a tratamento por TIC e 15 dias mais tarde, os mesmos cães foram submetidos a tratamento por BI. Para ambos os tratamentos, foi administrado o mesmo volume de fluido e a produção de urina foi quantificada a cada hora. A ingestão hídrica, densidade urinária, pH urinário foram avaliados por hora. As variáveis sanguíneas, hematócrito (Ht), hemoglobina (Hb), creatinina (creat), ureia, proteína total (PT), albumina (alb) e os eletrólitos sódio (Na), potássio (K), magnésio (Mg), cálcio (Ca) e fosforo (P) foram avaliados a cada 2 horas. O peso e as variáveis ecocardiográficas foram avaliados antes e depois do tratamento. A produção urinaria por hora (mL/kg/h) e total (mL/kg/8h) foi superior no tratamento TIC em todos os animais (p<0,05). Os parâmetros ingestão hídrica, densidade urinária, diferença de pH urinário, diferença de peso, Ht, Hb, ureia, creat e fósforo junto com as diferenças das variáveis ecocardiográficas não apresentaram diferenças significativas entre os tratamentos (p>0,05). A furosemida... / Congestive heart failure (CHF) is a syndrome that leads to inadequate blood supply to the tissue due to heart dysfunction and is treated with furosemide in intermittent bolus (BI). However, studies in humans with CHF showed that furosemide continuous rate infusion (CRI) showed better efficacy and safety for the administration in BI. Therefore, the aim of this study was to compare the effectiveness of the diuretic furosemide in CRI and BI in dogs with mitral valve myxomatous degeneration. To this end, the study included 20 dogs between 8 and 12 years old, weighing between 8 and 15 kg, and ICC classes Ia, Ib and II - ISACHC, and the control group. CRI for the treatment, the dogs received 0.66mg/kg IV furosemide as a loading dose followed by 0.66mg/kg/h for 8 hours. BI treatment the dogs received 3 mg / kg IV furosemide at 0 and 4 hours.
