Perfil das citocinas e correlação com a morbidade no período pós-operatório em crianças com diagnostico de cardiopatias congênitas não cianosantes submetidas a cirurgia corretiva com circulação extracorpórea

Pediatric cardiac surgery with cardiopulmonary bypass (CPB) induces a complex inflammatory reaction of variable intensity that, in severe cases, may lead to multiple organ dysfunctions, resulting in considerable morbidity and mortality. Previous studies have demonstrated the involvement of cytokines with both pro-­inflammatory and anti-­ inflammatory profile in the inflammatory response in this group of patients, as well as its relationship to morbidity in the postoperative period. The present study aimed to delineate the kinetics of plasma concentrations of eight different cytokines, to determine the impact of blood transfusion on these kinetics and to evaluate the possible correlation of postoperative morbidity with these kinetics. This study evaluated a group of 19 children diagnosed with non-­cyanotic congenital heart disease with volume overload of left ventricle and increased pulmonary blood flow, classified as low risk for operative mortality, undergone corrective surgery with CPB. Blood samples were taken in seven different times: after induction of anesthesia but prior to initiation of surgery (T0), five minutes after CPB beginning (T1), five minutes after the opening of the aortic clamp (T2), at the end of surgical procedure (T3), four hours after the end of surgery (T4), at the first postoperative day (T5) and at the second postoperative day (T6). For each sample was carried out the measurement of plasma concentrations of interleukin (IL) -­2, interleukin (IL) -­4, interleukin (IL) -­6, interleukin (IL) -­‐10, interleukin (IL) -­17, tumor necrosis factor (TNF)-­α, interferon (IFN)-­γ and macrophage migration inhibitory factor (MIF). We also evaluated blood transfusion and eight morbidity related variables: CPB time, aortic cross-­clamping time, inotropic score, cumulative TISS score, oxygenation index, volume of postoperative bleeding in the first 48 hours, duration of mechanical ventilation and length of stay in intensive care unit (ICU) postoperatively. Considering the kinetics of the concentrations of the cytokines evaluated, the concentrations of IL-­6, IL-­10 and MIF significantly changed in response to the surgical procedure. The concentrations of IL-­‐6 increased in T3, T4 and T5 and returned to baseline values at T6. The concentrations of IL-­10 increased in T2, T3 and T4 and returned to baseline values at T5. Concentrations of MIF increased in T2 and T3, returned to baseline in T4 and rose again at T6. Red blood cell transfusion had no impact on plasma concentrations of IL-­6, IL-­10 and MIF. Plasma concentrations of IL-­2, IL-­4, IL-­17, IFN-­γ and TNF-­α did not change significantly since the induction of anesthesia to the second postoperative day. Regarding the correlation of the concentrations of cytokines with morbidity in the postoperative period, we detected a positive correlation between plasma concentrations of IL-­6 and inotropic score. Different from what occurred with IL-­6, there were no correlations between the concentrations of IL-­10 and MIF and inotropic score. We did not found any correlation among the concentrations of IL-­6, IL-­10 and MIF and the following variables: CPB time, aortic cross-­clamping time, cumulative TISS score, oxygenation index, volume of postoperative bleeding in the first 48 hours, duration of mechanical ventilation and ICU length of stay. Therefore, for this group of patients, characterized as low risk, we have found a significant elevation of plasma concentrations of the mediators IL-­6, IL-­10 and MIF in response to corrective surgery. Packed red blood cells transfusion had no impact on plasma concentrations of these mediators. Also, no correlation was found between elevated concentrations of these mediators and the majority of morbidity related perioperative and postoperative variables that were evaluated. / A cirurgia cardíaca pediátrica com auxílio da circulação extracorpórea (CEC) está associada a uma resposta inflamatória complexa, de intensidade variável. Em alguns casos tal reação pode desencadear uma disfunção de múltiplos órgãos resultando em morbidade e mortalidade significativas. Estudos prévios demonstraram a participação de citocinas tanto com perfil pró-­inflamatório quanto anti-­inflamatório na resposta inflamatória nesse grupo de pacientes, assim como sua relação com a morbidade no período pós-­operatório (PO). O presente estudo teve os seguintes objetivos: delinear a cinética das concentrações plasmáticas de oito diferentes citocinas, determinar o impacto da hemotransfusão nesta cinética e avaliar a possível correlação da cinética das concentrações plasmáticas das citocinas com a morbidade no PO. A casuística foi constituída por um grupo de 19 crianças com diagnóstico de cardiopatias congênitas não cianosantes com sobrecarga de volume de ventrículo esquerdo e hiperfluxo pulmonar, classificadas como de baixo risco para mortalidade operatória, submetidas a cirurgia corretiva com CEC. Foram coletadas amostras sanguíneas das crianças incluídas no estudo em sete momentos diferentes: após a indução da anestesia mas antes do início da cirurgia (T0), cinco minutos após o início da CEC (T1), cinco minutos após a abertura da pinça da aorta (T2), ao final do procedimento cirúrgico (T3), quatro horas após o final da cirurgia (T4) , primeiro dia de PO (T5) e segundo dia de PO (T6). Para cada amostra foi realizada a mensuração das concentrações plasmáticas da interleucina (IL)-­2, interleucina (IL)-­4, interleucina (IL)-­6, interleucina (IL)-­10, interleucina (IL)-­17, fator de necrose tumoral (TNF)-­α, interferon (IFN)-­γ e do fator inibidor da migração de macrófagos (MIF). Também foram avaliadas a transfusão de hemoderivados e oito variáveis relacionadas a morbidade: tempo de CEC, tempo de pinçamento aórtico, escore inotrópico, escore de TISS cumulativo, índice de oxigenação, volume de sangramento pós-­‐operatório nas primeiras 48 horas, tempo de ventilação mecânica e tempo de internação na UTI no período pós-­operatório. Considerando a cinética das concentrações plasmáticas das citocinas avaliadas, as concentrações de IL-­6, IL-­10 e MIF se modificaram significativamente em resposta ao procedimento cirúrgico. As concentrações de IL-­6 se elevaram em T3, T4 e T5 e retornaram aos valores basais em T6. As concentrações de IL-­10 se elevaram em T2, T3 e T4 e retornaram aos valores basais em T5. As concentrações de MIF se elevaram em T2 e T3, retornaram aos valores basais em T4 e se elevaram novamente em T6. A transfusão de concentrado de hemácias não teve impacto nas concentrações plasmáticas de IL-­6, IL-­10 e MIF. As concentrações plasmáticas de IL-­2, IL-­4, IL-­17, IFN-­γ e TNF-­α não se modificaram significativamente desde a indução da anestesia até o segundo dia de PO. Quanto a correlação das concentrações das citocinas com a morbidade no PO, foi detectada uma correlação positiva entre as concentrações plasmáticas de IL-­6 e o escore inotrópico. Diferentemente do que ocorreu com a IL-­6, não foram detectadas correlações entre as concentrações de IL-­10 e MIF e o escore inotrópico. Também não foram detectadas correlações das concentrações de IL-­6, IL-­10 e MIF com as seguintes variáveis: tempo de CEC, tempo de pinçamento aórtico, escore de TISS cumulativo, índice de oxigenação, volume de sangramento pós-­operatório nas primeiras 48 horas, tempo de ventilação mecânica e tempo de internação na UTI no pós-­operatório. Portanto, neste grupo de pacientes avaliados, caracterizados como de baixo risco, foi encontrada uma elevação significativa das concentrações plasmáticas dos mediadores IL-­6, IL-­10 e MIF em resposta a intervenção cirúrgica corretiva. A transfusão de concentrado de hemácias não teve impacto nas concentrações plasmáticas desses mediadores. At the time of the formulation the deficits in surgeries were expressive, being more critical in the North and Northeast. The research aims to evaluate the policy on its design, implementation and results. We used quantitative methods for analysis of primary data from questionnaires administered to surgeons and secondary collected from the MS / DATASUS and IBGE. The time frame was 1999 to 2008 for procedures in congenital heart disease and from 2001 to 2008 for the valve procedures. The qualitative analyses were used for the entrances and speeches by corporate directors and managers of MS. Analyses were buoyed on the premises of the National Health System, as fairness, integrity and regionalization in the light of some authors. The results have proved to be a landmark policy by recognizing the characteristics and particular needs in the area. However, there is lack of measures to improve the frequency of the procedures, still with significant deficits, 62% for Brazil and 91%, 76%, 55%, 36% and 47% for the north, northeast, southeast, south and central west respectively, and resolve regional inequities. Politics is, according to the the analysis employed, ineffective and inefficient. As strategies to support governance actions met: realign payment amounts per procedure; implement database in partnership with DCCVPed; tables reconcile the diagnostic procedures with the orthesis and prosthesis, identify centers of excellence in complex heart and set them as reference regional centers to sponsor professional training value fees, subsidize the restructuring of the institutions empowered and structuring those who enter the system, establish policy for regional equity, create Program Prevention and Control of Rheumatic Fever. In sum, it means the policy as a trigger of a process of structuring and continuous evaluation that will provide input and be a tool to persuade the powers that be. / A Política Nacional de Atenção Cardiovascular de Alta Complexidade com foco na Cirurgia Cardiovascular Pediátrica, formalizada em 15 de junho de 2004, por meio das Portarias Nº1169/GM e Nº 210 SAS/MS, é gestada por intervenção da SBCCV e DCCVPed junto ao Ministério da Saúde e se propõe atender a demanda em cirurgia cardiovascular pediátrica. À época da formulação, os deficits em cirurgias eram expressivos, sendo mais críticos nas regiões Norte e Nordeste. A pesquisa objetiva avaliar a política quanto ao desenho, implementação e resultados. Foram empregados métodos quantitativos para análise de dados primários dos questionários, aplicados aos cirurgiões e secundários coletados no MS/DATASUS e IBGE. O recorte temporal foi de 1999 a 2008 para os procedimentos em cardiopatias congênitas e de 2001 a 2008 para os valvares. Os qualitativos foram empregados para análise das portarias e discursos de diretores de sociedades e gestores do MS. As análises foram balizadas nas premissas do Sistema Único de Saúde, equidade, integralidade e regionalização, à luz de alguns autores. Os resultados demonstram ser a política um marco, pelo fato de reconhecer características e necessidades particulares na área. Carece porém, de medidas que visem otimizar a frequência de procedimentos ainda com deficits significativos - 62% para o Brasil e 91%, 76%, 55%, 36% e 47% para as regiões Norte, Nordeste, Sudeste, Sul e Centro-Oeste, respectivamente, e resolver iniquidades regionais. A política é, à análise empregada, ineficaz e ineficiente. Como estratégias que visam a subsidiar ações de governança, encontraram-se: realinhar valores de pagamento por procedimento; implementar banco de dados em parceria com o DCCVPed; compatibilizar tabelas de diagnóstico, procedimentos e órteses e próteses; identificar centros de excelência em cardiopatias complexas e defini-los como de referência regional; patrocinar centros para treinamento profissional; valorizar honorários; subsidiar reestruturação das instituições habilitadas e estruturação daquelas que entrarem no sistema; estabelecer política de equidade regional; criar Programa de Prevenção e Controle da Febre Reumática. Em suma, entende-se a política como desencadeadora de um processo de estruturação e que a avaliação continuada poderá trazer subsídios fundamentados e ser ferramenta de persuasão junto ao poder constituído.
