Associação entre obesidade e fatores sociodemográficos, médico-odontológicos e comportamentais em adultos : um estudo transversal

Obesidade é o acúmulo excessivo ou anormal de gordura em relação à massa magra, podendo ser modificada por fatores ambientais, genéticos e comportamentais. Na literatura, é bem estabelecida a associação entre diabetes, tabagismo e, mais recentemente, obesidade e periodontite. Entretanto, estudos de prevalência e de associação entre obesidade e gengivite ainda são bastante escassos, especialmente em indivíduos adultos. O presente estudo objetivou verificar a associação entre obesidade e diferentes fatores sociodemográficos, médico-odontológicos e comportamentais em uma amostra de indivíduos adultos da cidade de Porto Alegre, Brasil. Esse estudo, de caráter epidemiológico transversal, avaliou 614 indivíduos maiores de 18 anos. Foram incluídos apenas os indivíduos com pelo menos quatro dentes em boca e que não utilizem aparelhos ortodônticos fixos. Os participantes foram identificados por meio de uma amostragem probabilística multi-estágios, considerando também a distribuição de acordo com o gênero e faixa etária. Dez setores censitários foram visitados. Uma entrevista estruturada foi realizada por pesquisadores treinados, obtendo-se informação de gênero, idade, hábitos comportamentais e estado de saúde sistêmica. Um examinador treinado e calibrado realizou exames de Índice de Placa Visível, Índice gengival de Löe-Silness, modificado por Löe, e cálculo. Além disso, peso, altura e percentual de gordura, obtido através de um aparelho portátil de microcorrente elétrica, também foram coletados. Regressão multivariada de Poisson foi utilizada para avaliar a associação entre obesidade e os diferentes fatores médico-odontológicos e sociodemográficos. Dos indivíduos examinados, 33,89% foram classificados como obesos por apresentarem índice de massa corporal pelo menos de 30 kg/m². Observou-se que gênero feminino, hipertensão autorreportada, não fumar, maiores percentuais de placa visível e maior número de dentes presentes apresentaram associação estatisticamente significativa com obesidade quando comparados com indivíduos não obesos. Concluiu-se que a ocorrência de obesidade, na presente amostra, foi alta e que o gênero feminino, não fumar e maiores percentuais de placa estiveram associados com a obesidade. Contudo, nenhuma associação estatisticamente significativa foi estabelecida entre gengivite e obesidade. / Obesity is the excessive or abnormal accumulation of fat in relation to lean body mass that may be modified by environmental, genetic and behavioral factors. O aumento da prevalência do sobrepeso e da obesidade tem se revelado um problema de saúde pública mundial. O desenvolvimento da obesidade envolve múltiplos fatores, como padrão de atividade física, padrão de alimentação, variáveis sociais e ambientais, fatores biológicos e genéticos. O objetivo deste estudo, que está aninhado no Projeto de Pesquisa OBEDIARP, foi avaliar a prevalência do sobrepeso e da obesidade na população adulta residente no município de Ribeirão Preto, estado de São Paulo, no ano de 2006 e estimá-la segundo variáveis sóciodemográficas, padrão de atividade física, padrão alimentar e saúde auto-referida. O estudo teve um delineamento transversal e foi desenvolvido no período de março de 2006 a junho de 2009. O processo de amostragem foi desenvolvido em três estágios, sendo o setor censitário a unidade primária de amostragem. Utilizou-se amostra ponderada de 2197 participantes. As variáveis do estudo foram: estado nutricional, sexo, idade, nível educacional, padrão de atividade física, tempo sentado, escores de freqüência de consumo de alimentos e auto-percepção do estado de saúde. A coleta dos dados ocorreu mediante a aplicação de entrevistas estruturadas que foram aplicadas nos domicílios dos elegíveis por uma equipe de entrevistadores previamente treinada. A taxa de resposta foi de 78%. Médias e proporções foram estimadas por pontos e por intervalos, com 95% de confiança. Para a avaliação de associações globais utilizou-se a estatística \"F\", adotando-se nível de significância de 5%. Para avaliar as relações entre o Índice de Massa Corporal e variáveis relacionadas ao padrão alimentar, ao gasto metabólico em atividade física e ao tempo sentado, utilizou-se modelo de regressão linear, adotando-se nível de significância de 5%. Todas as análises foram desenvolvidas no software STATA para Windows, versão 8.2. As estimativas levaram em consideração o efeito de desenho. Do total dos participantes do estudo 69.96% eram do sexo feminino e 30.04% do sexo masculino, com média de idade de 48.52 anos. A prevalência do excesso de peso encontrada foi de 64.65%, sendo que 39,23% foram classificados como pré-obesos e 25.42% como obesos. Os indivíduos classificados como insuficientemente ativos foram 66.29% da amostra. As variáveis sexo, faixa etária, escolaridade, estado de saúde auto-referida e estado de saúde comparado aos amigos apresentaram associação com o estado nutricional. Foi possível identificar que o consumo de alimentos de alta densidade energética e a média diária de tempo sentado estiveram positivamente associados ao índice de massa corporal, embora apenas em alguns estratos das variáveis sóciodemográficas. Conclui-se que a elevada prevalência de excesso de peso e de inatividade física na população adulta de Ribeirão Preto, apontam para a necessidade de implementação de medidas específicas de promoção e prevenção em saúde. / The increased prevalence of overweight and obesity has revealed to be a global public health problem. The development of obesity involves multiple factors, such as physical activity pattern, eating pattern, social and environmental variables, biological and genetic factors This study, part of the OBEDIARP Research Project, aimed to assess the prevalence of overweight and obesity in the adult population living in Ribeirão Preto, a city in the state of São Paulo, Brazil in 2006 and to estimate it according to socio-demographic variables, physical activity pattern, eating pattern and self-referred health. The research, with a cross-sectional design, was developed between March 2006 and June 2009. The sampling process was developed in three stages, with the census sector serving as the primary sampling unit. A weighted sample of 2197 participants was used. The study variables were: nutritional state, gender, age, education level, physical activity pattern, sitting time, food consumption frequency scores and self-perception of health state. Data were collected through structured interviews, held at the eligible participants\' homes by a previously trained team of interviewers. The response rate was 78%. Means and proportions were estimated by points and intervals, at a 95% confidence level. \"F\" statistics were used to assess global associations, adopting a 5% significance level. To assess the relations between Body Mass Index and food pattern-related variables, metabolic rate in physical activity and sitting time, a linear regression model was used, adopting a 5% significance level. All analyses were developed in STATA for Windows software, version 8.2. The effect of design was taken into account in all estimates. Of all study participants, 69.96% were women and 30.04% men, with a mean age of 48.52 years. The prevalence rate of overweight was 64.65%, with 39.23% classified as pre-obese and 25.42% as obese. Individuals classified as insufficiently active corresponded to 66.29% of the sample. The variables gender, age range, education level, self-referred health state and health state compared to friends were associated with the nutritional state. It could be identified that consumption of high-energy foods and mean daily sitting time were positively associated with the body mass index, although in some socio-demographic layers only. It is concluded that the high prevalence of overweight and physical inactivity in the adult population of Ribeirão Preto point towards the need for specific health promotion and prevention measures.

Clinical and kinematic assessments of upper limb function in persons with post-stroke symptoms

Stroke is a common and multifaceted disease that often involves motor deficits in the upper limb. This thesis investigated reliability and validity of existing clinical assessments of upper limb function in persons with post-stroke symptoms and in non-disabled controls. Study I was conducted in a clinical setting where the Motor Evaluation Scale of Upper Extremity in Stroke patients (MESUPES) was assessed in persons post-stroke by two physiotherapists selected from a group of four.  Study II-IV took place in a motion analysis laboratory with an optoelectronic system. Kinematic measures and clinical measures were used to investigate the validity of the Arm Posture Score (APS), the Finger-to-Nose test (FNT), and the Standardised Nine Hole Peg test (S-NHPT) in persons post-stroke and non-disabled controls.   The results showed that the MESUPES had a high inter-rater reliability while the concurrent validity was not fully confirmed. MESUPES has a maximum score of 58 and the minimal detectable change ranged from 5 to 8 for a confidence level of 80% and 95%. The Arm Posture Scores, which include either four or six arm movement variables, were able to distinguish between the affected and non-affected arms, as well as between the affected arm and the non-dominant arm of the controls. The total movement time of the FNT, which is a coordination test, was able to distinguish persons post-stroke from controls, at least at a group level. Movement smoothness, accuracy and compensation, obtained from kinematic analysis, were the most discriminative variables for the FNT. Smoothness was most strongly correlated with the timed FNT and had the greatest association with the variance of the timed FNT. For the S-NHPT, which is a dexterity test involving grasping and reaching, the movement times, smoothness and compensation discriminated between the stroke group and the control group. Persons post-stroke spent considerably more time in the grasp-related parts of the task compared to controls. Smoothness and upper limb impairments had the strongest correlation with the S-NHPT.   In conclusion, the clinical measures used within stroke rehabilitation seem valid and reliable, although some limitations are highlighted by the kinematic assessment. MESUPES was shown to be a reliable assessment of upper limb movement quality after stroke. The kinematic analysis revealed that the timed FNT does not have sufficient discriminative validity at an individual level. The timed FNT reflected speed-related aspects of pointing movements such as smoothness and length of the deceleration phase, but should not be used as an overall measure of upper limb coordination after stroke. The timed S-NHPT demonstrated sufficient discriminative validity and reflected smoothness and upper limb impairments. For both the FNT and S-NHPT, kinematic analysis showed that the clinical outcomes of those tests (time of performance) did not adequately detect qualitative aspects of the upper limb movements after stroke such as possible compensatory movements. Therefore, clinical assessments that capture qualitative aspects of upper limb movements would improve the assessment of upper limb coordination and dexterity after stroke. In addition, 3D movement analysis provided unique information about upper limb function after stroke, not least in persons with mild post-stroke impairments. The APS, for instance, which quantifies how much the arm swing during gait deviates from normal, discriminated between persons with stroke and non-disabled persons. Such subtle deviations are not possible to quantify with the human eye.

Objetivos: Conhecer a prevalência de sobrepeso e obesidade em escolares de Chilpancingo, México, e investigar o efeito independente de alguns fatores de risco ou de proteção para estes desfechos. Métodos: O estudo teve um delineamento transversal e a coleta de dados foi realizada no período de março a maio de 2004. A população de referência foi constituída pelo universo de alunos matriculados em escolas primárias de localização urbana, e a população do estudo foi composta por 700 escolares, de ambos os sexos, através de amostragem probabilística. Além das prevalências de sobrepeso e obesidade, que foram estimadas segundo o sexo e a idade, investigaram-se os fatores de risco ou de proteção para estes desfechos, utilizando-se a técnica estatística de regressão logística multivariada, com nível de significância a £ 0,05. As medidas de associação utilizadas foram os \"Odds Ratios\", estimados por pontos e por intervalos com 95% de confiança. As análises foram desenvolvidas no software STATA, versão 7.0. Resultados: Para o conjunto dos escolares, as prevalências de sobrepeso e obesidade foram, respectivamente, 28,1% e 13,7%. No sexo masculino, as respectivas prevalências foram 23,0% e 16,0%, e no feminino, 33,9% e 12,1%. Entre os escolares com menos de 10 anos de idade, a prevalência de sobrepeso foi 25,7% e a de obesidade, 16,3%. Nos escolares com mais de 10 anos, as respectivas prevalências foram 32,5% e 10,8%. Os fatores de risco para o sobrepeso, nos escolares de Chilpancingo, compuseram o seguinte elenco: \"nº de anos de escolaridade da mãe\"; \"hábito de comer enquanto estudavam ou assistiam TV\"; \"escores de alimentos de risco classificados no 2º ou 3º tercil de consumo\";\"atividade sedentária classificada no 2º ou 3º tercil\" e \"peso a nascer no 2º ou 3º tercil\". Rates among boys corresponded to 23.0% and 16.0% and, among girls, to 33.9% and 12.1%, respectively. Estudo epidemiológico descritivo, transversal, com 1 560 idosos de amostra representativa da cidade de São Paulo.Testes de avaliação cognitiva associados a escalas de atividades diárias compuseram o instrumento de screening aplicado porta a porta. A prevalência de suspeitos foi de 16,1%. O teste Chi2 seguido de análise por regressão logística mostraram relação das variáveis idade, analfabetismo ou baixa escolaridade, AVC e diabetes mellitus com suspeita de demência; as variáveis sexo, classe socioeconômica, cor, HAS, colesterol, depressão e TCE não apresentaram relação com suspeita de demência / Descriptive cross-sectional epidemiological study with 1 560 elderly in representative sampling of São Paulo city. Cognitive evaluation tests associated with scales of daily activities composed the screening and were carried out door to door. Suspect prevalence rate was 16,1%. The Chi2 test with logistic regressive analysis showed a relationship between age, illiteracy or few years of study, stroke and diabetes mellitus with dementia suspicion. The variables gender, socioeconomic class, skin color, systemic arterial hypertension, cholesterol, depression and cranioencephalic traumatism showed no relationship with dementia suspicion
