Disfagia orofaríngea em pacientes submetidos à intubação orotraqueal prolongada em UTIs / Oropharyngeal dysphagia in patients submitted to prolonged orotracheal intubation in intensive care units

INTRODUÇÃO: A deglutição é um processo complexo que requer a coordenação precisa de mais de 25 músculos, seis pares de nervos cranianos e os lobos frontais. O comprometimento neste processo, denominado de disfagia, pode aumentar a taxa de morbidade dos pacientes e também o risco para a aspiração, retardando a administração de uma nutrição adequada por via oral. A intubação orotraqueal prolongada, definida na literatura como período superior a 48 horas de intubação, poderá causar alterações na deglutição e ocasionar a disfagia após a extubação. O objetivo deste estudo foi verificar as variáveis independentes da avaliação fonoaudiológica da deglutição que são preditoras do risco de disfagia após intubação orotraqueal prolongada nas Unidades de Terapia Intensiva. MÉTODOS: Foi realizado um estudo transversal observacional. Participaram deste estudo 148 pacientes submetidos à avaliação em beira de leito da deglutição, no período entre setembro de 2009 e setembro de 2011. Todos os pacientes apresentavam histórico de intubação orotraqueal prolongada e foram admitidos em uma das Unidades de Terapia Intensiva de um grande hospital escola brasileiro. Os critérios de inclusão adotados foram: estabilidade clínica e respiratória; pontuação na Escala de Coma Glasgow acima de 14 pontos; idade acima de 18 anos; ausência de traqueostomia; ausência de doenças neurológicas; ausência de disfagia esofágica; ausência de procedimentos cirúrgicos envolvendo a área de cabeça e pescoço. Além disso, os pacientes deveriam ser submetidos à avaliação em beira de leito da deglutição no prazo de 48 horas após a extubação. A análise estatística incluiu a correlação entre os resultados obtidos no teste de deglutição de água e a pontuação do nível da deglutição. RESULTADOS: Os resultados indicaram que a presença de tosse e alteração da ausculta cervical durante a deglutição de água são variáveis preditoras independentes do risco de disfagia para o grupo testado. CONCLUSÃO: O estudo apontou as variáveis preditoras do risco de disfagia em pacientes submetidos à intubação orotraqueal prolongada. / INTRODUCTION: Swallowing is a complex process, that require the precise timing and coordination of more than 25 muscles, six cranial nerves and frontal lobes. Compromise of this process, or dysphagia, can result in profund morbidity, increasing the changes of aspiration and delaying the admistration of proper oral nutrition. It is know that an orotracheal tube might disturb these intricately choreographed events and cause post-extubation dysphagia. Prolonged intubation, typically defined as longer than 48 hours in the literature, is thought to contribute to swallowing dysfunction. The objective of this study is to elucidated independent factors that predict the risk of dysphagia after prolonged orotraqueal intubation in Intensive Care Units patients. METHODS: A cross-sectional, observational study design was used. Participants were 148 consecutive patients who underwent clinical bedside swallowing assessment, from September 2009 to September 2011. All patients presented a history of prolonged orotraqueal intubation and were admitted in one of the several Intensive Care Units of a large Brazilian school hospital. The adopted inclusion criteria were: to present clinical and respiratory stability, to present more than 14 points on the Glasgow Coma Scale; age above 18 years; absence of tracheostomy; absence of neurologic diseases, absence of esophageal dysphagia; absence of surgical procedures involving the head and neck. Also, to be included in the study, patients had to undergo a clinical swallowing assessment within 48 hours after extubation. The statistical analysis included the correlation of the results obtained on a water swallow test and the risk level for dysphagia. RESULTS: Results indicated that altered cervical auscultation and presence of cough during water swallow tests increase the likelihood of dysphagia in patients who underwent prolonged orotracheal intubation. CONCLUSION: The results of the study indicate factors that predict the risk of dysphagia after prolonged orotraqueal intubation.

Programa terapêutico fonoaudiológico ambulatorial para disfagia orofaríngea em adultos e idosos / Outpatient speech therapy program for oropharyngeal dysphagia in adults and elderly.

INTRODUCÃO: O objetivo da reabilitação em disfagia orofaríngea é estabilizar o aspecto nutricional e eliminar os riscos de aspiração laringotraqueal e consequentes complicações associadas. Um estudo sistemático que permita estabelecer o processo de reabilitação fonoaudiológica da disfagia orofaríngea, bem estruturado com base e em evidências e demonstração dos indicadores de qualidade, ainda se faz necessário para nortear a atuação clínica. O objetivo da pesquisa foi a aplicação da primeira fase de um ensaio clínico, onde o PTFDO foi avaliado em seu efeito terapêutico. A medida de efeito adotada foi a manifestação da alteração funcional considerada como mudança benéfica positiva (segurança para deglutição do alimento, por via oral, após a aplicação do tratamento). MÉTODO: Este foi um estudo longitudinal de efeito de tratamento, determinado por medidas comparativas entre pré e pós teste. A população alvo do estudo alvo foram pacientes adultos, encaminhados ao Ambulatório de Disfagia, HCFMUSP, para avaliação e tratamento fonoaudiológico. O período de seleção dos participantes foi de 24 meses, sendo avaliados para elegibilidade todos os pacientes encaminhados pelas equipes médicas do Instituto Central do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Para avaliação clínica fonoaudiológica da deglutição foram aplicados protocolos clínicos padronizados, PARD e PITA. Após a avaliação inicial, foram compostos 3 grupos de alocação, com base no padrão funcional da deglutição. A alocação dos participantes foi realizada com base na classificação do paciente segundo a Escala Funcional ASHA NOMS. Todos os participantes, independente do grupo de alocação, realizaram o mesmo programa terapêutico, composto por número de sessões fechadas, com técnicas especificas, bem como sua frequência e intensidade. As técnicas realizadas nas sessões presenciais, também foram repetidas diariamente pelos participantes. RESULTADOS: Foram incluídos de acordo com os critérios pré-estabelecidos um total de 138 participantes. Nos três grupos existe uma redução significativa dos sinais para disfagia, tendo em comum no pós tratamento a presença de deglutições múltiplas em todos os grupos CONCLUSÃO: Os dados obtidos demonstram efetividade na reabilitação da disfagia orofaríngea por meio da terapia tradicional, utilizando sessões presenciais e orientações em casa, num período de quatro semanas / INTRODUCTION: The aim of rehabilitation in oropharyngeal dysphagia is to stabilize the nutritional aspect and eliminate the risk of tracheal aspiration and subsequent complications associated. A systematic study to establish the process of voice rehabilitation of oropharyngeal dysphagia, well structured and based on evidence and statement of quality indicators are still needed to guide clinical practice. The research objective was the implementation of the first phase of a clinical trial where the PTFDO was valued at its therapeutic effect. The adopted measure of effect was the manifestation of the functional alteration considered positive beneficial change (for food safety swallowing orally, after application of the treatment). METHODS: This was a longitudinal study of treatment effect, determined by comparative measurements between pre and post test. The target study target population been adult patients referred to the Clinic of Dysphagia, HCFMUSP for evaluation and speech therapy. The period of selection of participants was 24 months, being evaluated for eligibility all patients referred by medical personnel of the Central Institute of the Hospital das Clinicas, Faculty of Medicine, University of Sao Paulo. For clinical examination of swallowing were applied standardized clinical protocols, PARD and PITA. After the initial assessment, it was made 3 allocation groups, based on functional pattern of swallowing. The allocation of participants was based on patients\' classification according to Functional Scale ASHA NOMS. All participants, regardless of the allocation group, underwent the same treatment program, consisting of number of closed sessions, with specific techniques as well as their frequency and intensity. The techniques used in classroom sessions were also repeated daily by the participants. RESULTS: We included according to pre-established criteria a total of 138 participants. Queixa de deglutição em indivíduos com Diabetes Melito tipos 1 e 2 / Swallowing complaints in individuals with type 1 and type 2 Diabetes Melittus

Introdução: O Diabetes Melito afeta o sistema digestório ao longo dos anos. Embora seja grande o relato da ocorrência de disfagia nesta população, não existem na literatura descrições da frequência de dificuldade na deglutição destes pacientes. Objetivo: Esta pesquisa teve como objetivo caracterizar a presença de sintomas autorreferidos de dificuldade de deglutição nos pacientes portadores de Diabetes Melito e relacioná-los com idade, sexo, índice de massa corporal, controle glicêmico, tratamento medicamentoso, tempo de diagnóstico do diabetes e suas complicações. Materiais e Métodos: Foram selecionados 221 indivíduos adultos portadores de Diabetes Melito para compor os grupos Diabetes Melito tipo 1 (GDM1) e Diabetes Melito tipo 2 (GDM2). O GDM1 consistiu de 112 indivíduos, 36 homens e 76 mulheres, média de 36 anos e o GDM2 de 109 indivíduos, sendo 39 homens e 70 mulheres com idade média de 57 anos. Foram selecionados ainda 217 adultos para compor o Grupo Controle (GC), divididos em Grupo Controle Diabetes Melito tipo 1 (GC1) e Grupo Controle Diabetes Melito tipo 2 (GC2) selecionados de acordo com a idade e sexo do grupo estudo. Tais participantes responderam ao questionário de autoavaliação alimentar Eating Assessment Tool (EAT-10), a fim de mensurar suas dificuldades de deglutição para identificação do risco de disfagia. A possibilidade de disfagia no EAT-10 foi definida quando os resultados foram >=3. Resultados: Os indivíduos com Diabetes Melito tipo 2 apresentaram maior frequência de disfagia, uma vez que a comparação entre os grupos de estudo GDM1 e GDM2 evidenciou que os indivíduos do grupo GDM2 pontuaram mais no protocolo EAT-10 do que os do GDM1, com uma diferença de 1,38 pontos (p 0,003). Os resultados do EAT-10 para os indivíduos com Diabetes Melito sofreram influência da idade, sexo, tempo de diagnóstico, dosagem de hemoglobina glicada, presença de neuropatia diabética, nefropatia e retinopatia, bem como medicamentos anti-hipertensivos e antiaglutinantes plaquetários. Conclusão: Os Dissertação de Mestrado 8 indivíduos com Diabetes Melito tipo 2 que apresentam maior tempo de diagnóstico da doença e com controle glicêmico alterado apresentaram maiores frequências de sintomas de disfagia em relação aos indivíduos com Diabetes Melito tipo 1. Estudos futuros são necessários para caracterizar a biodinâmica da deglutição nesses indivíduos. / Introduction: Diabetes mellitus affects the digestive system over the years. Although the occurrence of dysphagia in this population is large, there are no descriptions in the literature of the frequency of dysphagia in these patients. Objective: This study aimed to characterize the presence of self-reported symptoms of swallowing difficulty in diabetic patients and to relate them to age, sex, body mass index, glycemic control, drug treatment, time of diagnosis and others complications. Materials and Methods: Twentyone adult individuals with Diabetes mellitus were selected to form the groups Diabetes mellitus type 1 (GDM1) and Diabetes mellitus type 2 (GDM2). GDM1 consisted of 112 individuals, 36 men and 76 women, mean age of 36 years, and GDM2 of 109 individuals, 39 men and 70 women and 117 adults to be part of the Control Group (CG), selected according to age and sex of the study group. These participants answered a self-assessment questionnaire regarding their swallowing difficulties, in order to identify the risk of dysphagia. The possibility of dysphagia in EAT-10 was defined when the results were >=3. Results: Individuals with type 2 diabetes mellitus presented greater symptoms of dysphagia. The results of the EAT-10 for individuals with Diabetes were influenced by age, sex, time of diagnosis, glycated hemoglobin levels, presence of diabetic neuropathy, nephropathy and retinopathy, as well as antihypertensive drugs and platelet anti-caking agents. Conclusion: Individuals with type 2 diabetes mellitus who present a longer diagnosis and with altered glycemic control presented greater symptoms of dysphagia. Future studies are needed to characterize the biodynamics of swallowing in these individuals.

Deglutição orofaríngea e ritmos biológicos na insuficiência renal crônica /

Como as causas e possíveis consequências da disfagia na IRC ainda não foram investigadas o presente estudo contemplou também o estudo de variáveis biológicas que comprovadamente influenciam parâmetros clínicos como este, sendo estas o sono, a produção de melatonina, hormônio produzido pela glândula pineal, com papel importante na modulação do ritmo circadiano de sono-vigília e relacionado com sistema imune e os níveis de citocinas inflamatórias. A hipótese deste estudo seria de que pacientes com IRC podem apresentar quadros de disfagia concomitantemente a distúrbios de sono, sendo estes causados por déficit na produção de melatonina em consequência a altos níveis de citocinas inflamatórias. Objetivos: Caracterizar o perfil de deglutição e as váriaveis ritmicas biológicas sono, melatonina e citocinas inflamatórias em indivíduos com IRC. Metodologia: O grupo IRC foi composto por 20 indivíduos adultos, O grupo controle foi composto por 19 indivíduos adultos saudáveis, Para avaliação da deglutição foram utilizados a Videofluoroscopia (VFD) e a Functional Oral Intake Scale (FOIS) e para avaliação do sono o questionário do Índice de Qualidade de Sono de Pittsburgh (PSQI - BR). Para a dosagem de melatonina foi coletada saliva dos indivíduos em intervalos de 4 horas ao longo de 24 horas e para a dosagem de citocinas foi realizada coleta de sangue as 13:00h e a 1:00h. O objetivo desse estudo é descrever e caracterizar qualitativamente e quantitativamente os parâmetros ultrassonográficos da deglutição orofaríngea em adultos saudáveis e verificar de que forma as variáveis faixa etária e consistência de alimento, bem como suas respectivas interações, interferem nos parâmetros ultrassonográficos. Foram realizadas avaliações ultrassonográficas da deglutição (com ultrassom portátil modelo DP 6600, transdutor micro-convexo acoplado a um computador, além do estabilizador de cabeça) de 100 indivíduos (divididos em 4 grupos: GI - 20 à 30 anos, GII - 31 à 40 anos, GIII - 41 à 50 anos e GIV - 51 à 60 anos) com a consistência líquida (volume livre e controlado de 5ml) e consistência pudim (volume controlado de 5ml). Os parâmetros para a análise qualitativa do movimento de língua e do osso hióide foram adaptados de um estudo prévio. Considerou-se a presença de cinco marcadores durante a deglutição orofaríngea. Para a análise quantitativa, foram observados os parâmetros: duração da propulsão do movimento de língua durante a deglutição e a distância do deslocamento máximo do osso hióide durante o pico da deglutição. Os resultados indicaram que, em relação à descrição ultrassonográfica qualitativa, todos os indivíduos, independente da consistência e volume do alimento apresentaram o mesmo padrão de postura de língua para a recepção do alimento (fase1) e apreensão do alimento (fase 2). Em relação à propulsão do bolo alimentar (fase 3), verificou-se uma grande variabilidade inter-sujeitos do movimento ondulatório da língua. Já para o pico máximo do deslocamento... / Among different technology used in oropharyngeal swallowing instrumental evaluation, considering cost-benefit ratio, tongue movement ultrasound stand out as a viable instrument to investigate orofacial functions. This study aims to describe and to define - both qualitatively and quantitatively - ultrasonographic parameters of oropharyngeal swallowing among healthy adults and to verify in what way the variables age group and consistency of food swallowed, as well as respective interactions, might interfere with ultrasonographic parameters. Ultrasonographic evaluations of swallowing were performed (using the DP 6600 portable ultrasound machine, a micro-convex probe connected to a PC, and a Stabilization Headset) in 100 people (divided into four groups: GI - 20 à 30 age, GII - 31 à 40 age, GIII - 41 à 50 age e GIV - 51 à 60 age), using liquids (both free volume and controlled swallow of 5 ml) and pudding-consistent food (controlled swallow of 5 ml). Tongue and hyoid bone movement, used as parameters to qualitative analysis, were adapted from a previous study. During the oropharyngeal swallowing, the presence of five markers was considered. The following parameters were regarded in the quantitative analysis: tongue movement propulsion duration during swallowing, and maximum displacement of the hyoid bone during swallowing peak. Considering qualitative ultrasonographic description, the results pointed that all volunteers presented the same standard tongue posture previous to food reception (phase 1) and containment (phase 2), regardless of food consistency and volume. During the bolus propulsion (phase 3), a wide intersubjective variety of wavelike movements was observed. O comprometimento na fase oral da deglutição é ocasionado por fraqueza, fasciculação e atrofia de língua que promovem incoordenação oral gerando aumento no tempo de transito oral (TTO), comprometendo assim a alimentação destes indivíduos. Objetivo: Esta pesquisa teve por objetivo descrever e relacionar o TTO da deglutição com o desempenho funcional e com penetração e aspiração laringotraqueal na DNM. Método: Participaram desse estudo 20 indivíduos com DNM, independente do tipo ou tempo da doença. Foram incluídos 11 indivíduos, sendo nove do gênero masculino e dois do gênero feminino, faixa etária de 31 a 87 anos (média de idade de 57 anos). Foram excluídos nove indivíduos. Aplicado o questionário Amyotrophic Lateral Sclerosis Functional Rating Scale - revised (ALSFRS-r/BR) para classificação dos indivíduos de acordo com parâmetros Global, Bulbar e Bulbar/Respiratório. Realizada videofluoroscopia da deglutição com diferentes consistências de alimento no volume de cinco ml, com análise quantitativa temporal por meio de software específico para mensuração do tempo de trânsito oral total (TTOT) e análise qualitativa por meio da escala de Penetração e Aspiração Laringotraqueal proposta por Rosenbek et al (1996). Para relacionar o TTOT com os parâmetros da ALSFRS-r foi aplicado o teste de comparação de médias para dados incompletos (ANOVA), pressupondo nível de significância α≤0,05, bem como efetuados cálculos de coeficiente de correlação de Pearson para análise bidimensional. A relação entre TTOT e os níveis da escala de penetração e aspiração laringotraqueal foi descritiva. Resultados: Verificou-se que houve ampla variação do TTOT dentro da mesma consistência de alimento. Houve correlação entre o TTOT e o desempenho funcional global na consistência líquida espessada (r=-0,691) e para o TTOT e o desempenho bulbar na pastosa (r=-0,859). O nível da escala de penetração e aspiração laringotraqueal variou de 2 a 4 e o TTOT nesses níveis variou de 1222ms a 2057ms. Conclusão: Houve ampla variação do TTOT na mesma consistência de alimento na DNM e quanto maior o TTOT, independente da consistência do alimento, menor foi o desempenho funcional na DNM. / Introduction: Oropharyngeal dysphagia is a frequent symptom in individuals with motor neuron disease (MND) and one of the parameters that measure the degree of functionality in this population, and the alteration in the oral phase of swallowing is one of the first signs of dysphagia in DNM. The impairment in the oral phase of swallowing is caused by weakness, fasciculation and tongue atrophy that promote oral incoordination leading to an increase in oral transit time (OTT), thus compromising the feeding of these individuals. Objective: This study aimed to describe and correlate oral transit time (OTT) of swallowing with functional performance and with laryngotracheal penetration and aspiration in MDN. Method: Participated in this study 20 individuals with DNM, regardless of the type or time of the disease. Eleven individuals were included, nine of whom were male and two were female, ranging from 31 to 87 years old ( mean age of 57 years). Were excluded nine individuals. Applied the Amyotrophic Lateral Sclerosis Functional Rating Scale - revised (ALSFRS-r / BR) to classification of individuals according to Global, Bulbar and Bulbar / Respiratory parameters. A videofluoroscopy of swallowing with different food consistencies was performed in the volume of five ml, with qualitative analysis using the Laryngotracheal Penetration and Aspiration Scale proposed by Rosenbek et al (1996) and temporal quantitative analysis using specific software to measure total oral transit time (TOTT). In order to correlate the TOTT with the parameters of the ALSFRS-r, the test of comparison of averages for incomplete data (ANOVA) was applied, assuming a significance level α = 0.05, as well as Pearson's correlation coefficient calculations for two-dimensional analysis. The relationship between TTOT and levels of the laryngotracheal penetration and aspiration scale was descriptive. Results: It was verified that there is a wide variation of TOTT within the same food consistency. There was a correlation between the TTOT and the overall functional performance in the thickened liquid consistency (r = -0.691) and for the TOTT and the bulbar performance in the pasty (r = -0.859). The laryngotracheal penetration and aspiration ranged from 2 to 4 and the TOTT at these levels ranged from 1222ms to 2057ms. Conclusion: There is a wide variation of the TOTT in the same food consistency in the MND and higher the TOTT, regardeless of the consistency thickened of the food, the lower the functional performance in the MND.

Disfagia orofaríngea em pacientes submetidos à intubação orotraqueal prolongada em UTIs / Oropharyngeal dysphagia in patients submitted to prolonged orotracheal intubation in intensive care units

INTRODUÇÃO: A deglutição é um processo complexo que requer a coordenação precisa de mais de 25 músculos, seis pares de nervos cranianos e os lobos frontais. O comprometimento neste processo, denominado de disfagia, pode aumentar a taxa de morbidade dos pacientes e também o risco para a aspiração, retardando a administração de uma nutrição adequada por via oral. A intubação orotraqueal prolongada, definida na literatura como período superior a 48 horas de intubação, poderá causar alterações na deglutição e ocasionar a disfagia após a extubação. O objetivo deste estudo foi verificar as variáveis independentes da avaliação fonoaudiológica da deglutição que são preditoras do risco de disfagia após intubação orotraqueal prolongada nas Unidades de Terapia Intensiva. MÉTODOS: Foi realizado um estudo transversal observacional. Participaram deste estudo 148 pacientes submetidos à avaliação em beira de leito da deglutição, no período entre setembro de 2009 e setembro de 2011. Todos os pacientes apresentavam histórico de intubação orotraqueal prolongada e foram admitidos em uma das Unidades de Terapia Intensiva de um grande hospital escola brasileiro. Os critérios de inclusão adotados foram: estabilidade clínica e respiratória; pontuação na Escala de Coma Glasgow acima de 14 pontos; idade acima de 18 anos; ausência de traqueostomia; ausência de doenças neurológicas; ausência de disfagia esofágica; ausência de procedimentos cirúrgicos envolvendo a área de cabeça e pescoço. Além disso, os pacientes deveriam ser submetidos à avaliação em beira de leito da deglutição no prazo de 48 horas após a extubação. A análise estatística incluiu a correlação entre os resultados obtidos no teste de deglutição de água e a pontuação do nível da deglutição. RESULTADOS: Os resultados indicaram que a presença de tosse e alteração da ausculta cervical durante a deglutição de água são variáveis preditoras independentes do risco de disfagia para o grupo testado. CONCLUSÃO: O estudo apontou as variáveis preditoras do risco de disfagia em pacientes submetidos à intubação orotraqueal prolongada. / INTRODUCTION: Swallowing is a complex process, that require the precise timing and coordination of more than 25 muscles, six cranial nerves and frontal lobes. Compromise of this process, or dysphagia, can result in profund morbidity, increasing the changes of aspiration and delaying the admistration of proper oral nutrition. It is know that an orotracheal tube might disturb these intricately choreographed events and cause post-extubation dysphagia. Prolonged intubation, typically defined as longer than 48 hours in the literature, is thought to contribute to swallowing dysfunction. The objective of this study is to elucidated independent factors that predict the risk of dysphagia after prolonged orotraqueal intubation in Intensive Care Units patients. METHODS: A cross-sectional, observational study design was used. Participants were 148 consecutive patients who underwent clinical bedside swallowing assessment, from September 2009 to September 2011. All patients presented a history of prolonged orotraqueal intubation and were admitted in one of the several Intensive Care Units of a large Brazilian school hospital. The adopted inclusion criteria were: to present clinical and respiratory stability, to present more than 14 points on the Glasgow Coma Scale; age above 18 years; absence of tracheostomy; absence of neurologic diseases, absence of esophageal dysphagia; absence of surgical procedures involving the head and neck. Also, to be included in the study, patients had to undergo a clinical swallowing assessment within 48 hours after extubation. The statistical analysis included the correlation of the results obtained on a water swallow test and the risk level for dysphagia. RESULTS: Results indicated that altered cervical auscultation and presence of cough during water swallow tests increase the likelihood of dysphagia in patients who underwent prolonged orotracheal intubation. CONCLUSION: The results of the study indicate factors that predict the risk of dysphagia after prolonged orotraqueal intubation.

Queixa de deglutição em indivíduos com Diabetes Melito tipos 1 e 2 / Swallowing complaints in individuals with type 1 and type 2 Diabetes Melittus

Introdução: O Diabetes Melito afeta o sistema digestório ao longo dos anos. Embora seja grande o relato da ocorrência de disfagia nesta população, não existem na literatura descrições da frequência de dificuldade na deglutição destes pacientes. Objetivo: Esta pesquisa teve como objetivo caracterizar a presença de sintomas autorreferidos de dificuldade de deglutição nos pacientes portadores de Diabetes Melito e relacioná-los com idade, sexo, índice de massa corporal, controle glicêmico, tratamento medicamentoso, tempo de diagnóstico do diabetes e suas complicações. Materiais e Métodos: Foram selecionados 221 indivíduos adultos portadores de Diabetes Melito para compor os grupos Diabetes Melito tipo 1 (GDM1) e Diabetes Melito tipo 2 (GDM2). O GDM1 consistiu de 112 indivíduos, 36 homens e 76 mulheres, média de 36 anos e o GDM2 de 109 indivíduos, sendo 39 homens e 70 mulheres com idade média de 57 anos. Foram selecionados ainda 217 adultos para compor o Grupo Controle (GC), divididos em Grupo Controle Diabetes Melito tipo 1 (GC1) e Grupo Controle Diabetes Melito tipo 2 (GC2) selecionados de acordo com a idade e sexo do grupo estudo. Tais participantes responderam ao questionário de autoavaliação alimentar Eating Assessment Tool (EAT-10), a fim de mensurar suas dificuldades de deglutição para identificação do risco de disfagia. A possibilidade de disfagia no EAT-10 foi definida quando os resultados foram >=3. Resultados: Os indivíduos com Diabetes Melito tipo 2 apresentaram maior frequência de disfagia, uma vez que a comparação entre os grupos de estudo GDM1 e GDM2 evidenciou que os indivíduos do grupo GDM2 pontuaram mais no protocolo EAT-10 do que os do GDM1, com uma diferença de 1,38 pontos (p 0,003). Os resultados do EAT-10 para os indivíduos com Diabetes Melito sofreram influência da idade, sexo, tempo de diagnóstico, dosagem de hemoglobina glicada, presença de neuropatia diabética, nefropatia e retinopatia, bem como medicamentos anti-hipertensivos e antiaglutinantes plaquetários. Conclusão: Os Dissertação de Mestrado 8 indivíduos com Diabetes Melito tipo 2 que apresentam maior tempo de diagnóstico da doença e com controle glicêmico alterado apresentaram maiores frequências de sintomas de disfagia em relação aos indivíduos com Diabetes Melito tipo 1. Estudos futuros são necessários para caracterizar a biodinâmica da deglutição nesses indivíduos. / Introduction: Diabetes mellitus affects the digestive system over the years. Although the occurrence of dysphagia in this population is large, there are no descriptions in the literature of the frequency of dysphagia in these patients. Objective: This study aimed to characterize the presence of self-reported symptoms of swallowing difficulty in diabetic patients and to relate them to age, sex, body mass index, glycemic control, drug treatment, time of diagnosis and others complications. Materials and Methods: Twentyone adult individuals with Diabetes mellitus were selected to form the groups Diabetes mellitus type 1 (GDM1) and Diabetes mellitus type 2 (GDM2). GDM1 consisted of 112 individuals, 36 men and 76 women, mean age of 36 years, and GDM2 of 109 individuals, 39 men and 70 women and 117 adults to be part of the Control Group (CG), selected according to age and sex of the study group. These participants answered a self-assessment questionnaire regarding their swallowing difficulties, in order to identify the risk of dysphagia. The possibility of dysphagia in EAT-10 was defined when the results were >=3. Results: Individuals with type 2 diabetes mellitus presented greater symptoms of dysphagia. The results of the EAT-10 for individuals with Diabetes were influenced by age, sex, time of diagnosis, glycated hemoglobin levels, presence of diabetic neuropathy, nephropathy and retinopathy, as well as antihypertensive drugs and platelet anti-caking agents. Conclusion: Individuals with type 2 diabetes mellitus who present a longer diagnosis and with altered glycemic control presented greater symptoms of dysphagia. Future studies are needed to characterize the biodynamics of swallowing in these individuals.

Análise ultrassonográfica qualitativa e quantitativa da deglutição orofaríngea /

O objetivo desse estudo é descrever e caracterizar qualitativamente e quantitativamente os parâmetros ultrassonográficos da deglutição orofaríngea em adultos saudáveis e verificar de que forma as variáveis faixa etária e consistência de alimento, bem como suas respectivas interações, interferem nos parâmetros ultrassonográficos. Foram realizadas avaliações ultrassonográficas da deglutição (com ultrassom portátil modelo DP 6600, transdutor micro-convexo acoplado a um computador, além do estabilizador de cabeça) de 100 indivíduos (divididos em 4 grupos: GI - 20 à 30 anos, GII - 31 à 40 anos, GIII - 41 à 50 anos e GIV - 51 à 60 anos) com a consistência líquida (volume livre e controlado de 5ml) e consistência pudim (volume controlado de 5ml). Os parâmetros para a análise qualitativa do movimento de língua e do osso hióide foram adaptados de um estudo prévio. Considerou-se a presença de cinco marcadores durante a deglutição orofaríngea. Para a análise quantitativa, foram observados os parâmetros: duração da propulsão do movimento de língua durante a deglutição e a distância do deslocamento máximo do osso hióide durante o pico da deglutição. Os resultados indicaram que, em relação à descrição ultrassonográfica qualitativa, todos os indivíduos, independente da consistência e volume do alimento apresentaram o mesmo padrão de postura de língua para a recepção do alimento (fase1) e apreensão do alimento (fase 2). (Complete abstract click electronic access below) / Abstract: Among different technology used in oropharyngeal swallowing instrumental evaluation, considering cost-benefit ratio, tongue movement ultrasound stand out as a viable instrument to investigate orofacial functions. This study aims to describe and to define - both qualitatively and quantitatively - ultrasonographic parameters of oropharyngeal swallowing among healthy adults and to verify in what way the variables age group and consistency of food swallowed, as well as respective interactions, might interfere with ultrasonographic parameters. Ultrasonographic evaluations of swallowing were performed (using the DP 6600 portable ultrasound machine, a micro-convex probe connected to a PC, and a Stabilization Headset) in 100 people (divided into four groups: GI - 20 à 30 age, GII - 31 à 40 age, GIII - 41 à 50 age e GIV - 51 à 60 age), using liquids (both free volume and controlled swallow of 5 ml) and pudding-consistent food (controlled swallow of 5 ml). Tongue and hyoid bone movement, used as parameters to qualitative analysis, were adapted from a previous study. During the oropharyngeal swallowing, the presence of five markers was considered. The following parameters were regarded in the quantitative analysis: tongue movement propulsion duration during swallowing, and maximum displacement of the hyoid bone during swallowing peak. Considering qualitative ultrasonographic description, the results pointed that all volunteers presented the same standard tongue posture previous to food reception (phase 1) and containment (phase 2), regardless of food consistency and volume.
