Disturbios depressivos em um centro de atenção primaria : prevalencia momentanea e analise do discurso dos pacientes

A amostra foi constituída de 103 sujeitos de ambos os sexos, com idade entre 18 e 60 anos e sócio econômico-culturalmente carentes." A metodologia utilizada abrangeu diversos procedimentos. Inicialmente, acompanhávamos o atendímento efetuado pelo" Clínico Geral, observando e anotando sua conduta durante a consulta (interação médico-paciente, características gerais do momento da consulta, solicitações de exames complementares, hipótese diagnóstica, prescrição, encaminhamentos, etc.). Em seguida, em sala contígua, os pacientes respondiam ao formulário sobre os critérios de exclusão e inclusão para de pressão da OMS - Avalia~ão Normatizada de Transtornos Depressivos ANTD/OMS Standardized Assessment of Depressive Disorders SADD/WHO; Sartorius et .al., 1983). Os pacientes que preenchiam esses critérios eram considerados "suspeitos" de depressão. A estes, aplicávamos a Escala de Zung de Auto-Avaliação de Depressão (Zung Self-Rating Depression Scale; Zung & Durham, 1965), cujo "ponto de corte" foi estabelecido em 40 pontos. Os pacientes com Zung )= 40 foram considerados deprimidos, sendo submetidos à gravação dos seus discursos. Esta ocorria durante cinco minutos e após consentimento do sujeito, que era estimulado a falar de forma não dirigida sobre suas queixas. Formulávamos perguntas como "o que o trouxe aqui?", "qual é o problema?" "0 que voei sente?", 15. Observação: O resumo, na íntegra, poderá ser visualizado no texto completo da tese digital / Abstract: The aims of this stud~ were to investigate the paint prevalence of depressive disorders in a Primar Health Care Centre and the kind of speech used b~ these patients to express their complaints. The patients were a consecutive series of males or famales attending the Centre, aged between 18 and 60 years and of lower social-economic and cultural background. The methodological procedures were as follows: the researcher accompanied the General Practitioner's (GP) consultation and took notes of his usual procedure (doctor-patient relationship, interviewing, laboratory tests, prescription, referall, ete.) towards the patients. The GP did not know the aim of the study. After the consultation, the patients were sent to the next-door room where the researcher applied the Standardized Assessment of Depresslve Disorders (SADD/WHO; Sartorius et aI., 1983) to them. To those "suspected" of depressive disorder by means of the SAOO, the researcher applied the Zung Self-Rating Oepressian Scale (Zung &Durham, 1965). The subjects who had a total score >= 40 on Zung Scale were considered as depressives and hence were submitted to a "free" interview for five minutes. This interview was recorded and care was taken for not asking questions that could hint at a symptom, e.g., it was not al10wed to ask "how does your appetite look like", "what about ~our sleep?" and so forth, but rather neutral questions were made, such as "what brought you here"?
