Comparação do estímulo vibratório com a eletroestimulação intravaginal em mulheres com incontinência urinária : um ensaio clínico randomizado

Introdução: O treinamento dos músculos do assoalho pélvico (MAP) é a primeira linha de tratamento conservador da incontinência urinária (IU), contudo, muitas mulheres não conseguem contrair essa musculatura voluntariamente. A eletroestimulação intravaginal (EEIV) é bem documentada nestes casos e o estímulo vibratório intravaginal (EVIV) também tem sido utilizado, mas poucos estudos abordam o método. Objetivo: Comparar a EEIV com o EVIV na funcionalidade dos MAP de mulheres com IU que não conseguem contrair voluntariamente os MAP. Materiais e métodos: Ensaio clínico randomizado realizado no Hospital de Clínicas de Porto Alegre de junho de 2016 a setembro de 2017. Critérios de inclusão: mulheres maiores de 18 anos, com IU e incapazes de contrair voluntariamente os MAP. Critérios de exclusão: alergia ao látex e treinamento dos MAP nos últimos seis meses Após avaliação, as pacientes que correspondiam aos critérios de inclusão foram randomizadas em grupo EEIV e EVIV. Foram realizadas seis sessões individuais de 20 minutos, uma vez por semana. Resultados: Foram randomizadas 21 mulheres para cada grupo e 18 completaram o protocolo de EVIV e 17 o de EEIV. Ao final, o grupo EVIV apresentou aumento significativo na força dos MAP em relação ao grupo EEIV (p=0,026) e houve interação significativa do tempo e tipo de intervenção para a mesma variável (p=0,008) no GEVIV. Conclusão: O EVIV foi significativamente melhor que a EEIV na melhora da força dos MAP, mas são necessários mais estudos para consolidar seu uso na melhora da funcionalidade dos MAP e no tratamento da IU. / Introduction: Pelvic floor muscle (PFM) training is the first line of conservative treatment for urinary incontinence (UI); however, many women are unable to contract these muscles voluntarily. Intravaginal electrostimulation (IVES) is well documented in these cases and intravaginal vibratory stimulation (IVVS) is also used, but few studies address this method. Objective: To compare the use of IVES and IVVS on the function of the PFM in women with UI who cannot voluntarily contract the PFM. Materials and methods: Randomized clinical trial performed at the Hospital de Clínicas de Porto Alegre from June 2016 to September 2017. Inclusion criteria: women over 18 years of age with UI and unable to voluntarily contract their PFM. Exclusion criteria: latex allergy and PFM training over the last six months. After the assessment, the patients that met the inclusion criteria were randomized into IVES and IVVS groups. Six individual 20-minute sessions were held once a week Results: Twenty-one women were randomly assigned to each group and 18 completed the IVVS and 17 the IVES protocols. In the end, the IVVS group presented a significant increase in strength of the PFM in relation to the IVES group (p = 0.026) and there was a significant interaction between time and type of intervention for the same variable (p = 0.008) in the IVVS group. Conclusion: IVVS was significantly better than IVES in improving PFM strength, but more studies are needed to consolidate its use regarding PFM function and the treatment of UI.

Associação entre a musculatura do assoalho pélvico e a função sexual de mulheres com diferentes tipos de incontinência urinária

Introdução: Incontinência Urinária (IU) é comum na população feminina afetando um terço das mulheres adultas, podendo comprometer sua função sexual (FS). Ainda há controvérsia sobre o impacto da IU sobre a FS. A associação da FS e da funcionalidade da MAP é uma questão relevante que necessita aprofundamento. Objetivo: Comparar a FS de mulheres com Incontinência Urinária de Esforço (IUE) e Incontinência Urinária Mista (IUM), e correlacionar a funcionalidade da musculatura do assoalho pélvico (MAP) à FS destas mulheres. Método: Observacional e transversal, n=61 mulheres, de 30 a 70 anos que tiveram relação sexual nos últimos 12 meses. As participantes foram classificadas em dois grupos: IUE (n=22) E IUM (n=39). A avaliação foi constituída por ficha de anamnese, biofeedback pressórico, escala PERFECT, e questionário PISQ-12. A análise estatística foi realizada através do teste Shapiro-Wilk para verificar a normalidade dos dados. Para comparação dos dados foi utilizado o teste T de amostras independentes e o teste U de Mann-Whitney. Para correlação foi utilizado o teste de Correlação de Spearman. O nível de significância adotado foi de 5%. Resultados: Diferença significativa na paridade e duração da queixa entre os grupos; na comparação do escore total do PISQ-12 e no domínio físico; nos itens P e F da escala PERFECT. Não houve correlação significativa entre a CVM Média e o Escore Total PISQ-12. Conclusão: Não foi demonstrado correlação entre a função da MAP e a FS nos grupos. Porém, houve diferença significativa entre a FS nos grupos. / Introduction: Urinary Incontinence (UI) is common among the female population. It affects one third of adult women and can compromise their sexual function (SF). There are still controversy about the impacts of UI on the SF. The association of the SF and the functionality of the pelvic floor muscles (PMF) is a relevant issue that needs to be better comprehended. Goal: Comparing the SF of women with Stress Urinary Incontinence (SUI) and Mixed Urinary Incontinence (MUI), and correlating these women’s PMF functionality to their SF. Method: Observational and cross-sectional, n=61 women aged 30 to 70 yo, who had had sexual relations in the last 12 months. The participants were divided into two groups: SUI (n=22) and MUI (n=39). The assessment consisted of the anamnesis record, pressure biofeedback, PERFECT scale and the PISQ-12 questionnaire. Statistical analysis was carried out using the Shapiro-Wilk test to verify the normality of the data. The independent t-test and the Mann-Whitney U test were used for data comparison. Spearman’s rank correlation was used to correlate data. The adopted level of significance was 5%. Results: Significant difference between the groups regarding the parity and duration of complaints; the PISQ-12 total score and the physical domain; the items Power-Pressure and Fast of the PERFECT scale. There was no significant correlation between the Maximum Voluntary Contraction (MVC) and the PISQ-12 total score. Conclusion: The tests did not find a correlation between the PFM functions and the SF in the groups. However, there was a significant difference in the SF between groups.

Foram estudadas prospectivamente 150 mulheres multíparas, saudáveis, sem disfunções urinárias, distribuídas em quatro grupos, G1 (n=37) mulheres na faixa etária de 30 a 40 anos, G2 (n=39) na faixa de 41 a 50 anos, G3 (n=39) na faixa de 51 a 60 anos e G4 (n=35) acima de 60 anos. Os dados demográficos, como idade, número de partos, índice de massa corpórea (IMC), atividade física e sexual, foram obtidos por questionário. A avaliação subjetiva dos músculos do assoalho pélvico (MAP) foi feita por palpação digital da vagina nas porções anterior e posterior por três examinadores. Foi utilizada a classificação descrita por Amaro e col, 2003, foi garantido sigilo entre os examinadores. A média de idade foi de 35, 45, 54 e 66 anos em G1, G2, G3 e G4 respectivamente. 69,3% das mulheres tinham atividade sexual e 40,7% atividade física regular. O IMC foi classificado como normal no G1 e pré-obeso no G2, G3 e G4, sendo significativamente menor no G1 em relação ao G4 (p<0,05). O número de partos vaginais foi menor no G2 em relação ao G4 (p<0,05), e de cesáreas foi menor no G4 em relação aos grupos G2 e G3. Não houve diferença estatística significativa entre os graus de contração muscular e entre a porção anterior e posterior do AP (p>0,05). A concordância plena da palpação digital na porção anterior foi de 44,7% e na posterior 55,3%, não houve diferença estatística significativa. Houve concordância entre os diferentes examinadores do método subjetivo de palpação digital do AP na porção anterior e posterior do AP independente da faixa etária garantindo a reprodutibilidade do método. / The aim of this study is to assess the function and the reproductibility of subjective evaluation of pelvic floor muscle (PMF) using the digital vaginal palpation in continent women from different age ranges. 150 healthy multipararous women were studied with no urinary dysfunction, distributed in four different groups: G1 (n=31) from 30 to 40 years; G2 (n=39) from 41 to 50 years; G3 (n= 39) from 51 to 60 years; and G4 (n=35) over 60 years. The demographic data, such as age, number of deliveries, body mass index ( BMI ), physical and sexual activity, were all obtained using a questionnaire. The subjective assessments of the pelvic floor muscles (PFM ) were performed by three different examiners using transvaginal digital palpation (TDP) in the anterior and posterior areas. It was used tha classification as described by Amaro et al, 2003, the secret amongst the examiners was granted. The ages were in average 35, 45, 54 and 66 years old in G1, G2, G3 and G4 respectively. 69,3% of the women had sexual activity and 40,7% had regular physical activities. The BMI was classified was significantly lower in G1 in comparasion to G4 (p<0,05). The number of natural deliveries was lower in G2 in comparasion to G4 ( p<0,05), and the number of caesarian was lower in G4 in comparasion to groups G2 and G3. There was no statistical difference between the subjective evaluation of PMF, neither the anterior and posterior area Threre was 44,7% agreement among the different examiners using TDP in the anterior and of 55,3%in the posterior, there was no statistical difference. There was an agreement among the different examiners in the subjective evaluation of PMF using TDP in the anterior and posterior area independent of the age range, demonstrating the reproducibility of this method.

Sujeitos e Métodos: realizou-se um ensaio clínico com 26 mulheres que apresentavam diagnóstico de disfunção sexual (transtorno de desejo sexual, transtorno de excitação, transtorno orgástico e dispareunia) no período de fevereiro de 2008 a maio de 2009. Foi administrado um questionário denominado FSFI (Female Sexual Function Índex) para verificar a função sexual. A contração dos músculos do assoalho pélvico foi avaliada por meio da palpação vaginal (bi-digital) e, em seguida, realizou-se uma eletromiografia de superfície para avaliar amplitudes de contração. Tais procedimentos ocorreram previamente, na metade e após o tratamento. As mulheres realizaram exercícios para os músculos do assoalho pélvico em diferentes posições, uma ou duas vezes por semana, totalizando 10 sessões de tratamento. Na análise das variáveis ao longo do tempo, utilizou-se o teste não-paramétrico e o Anova de Friedman, o teste de Bonferroni e o teste de correlação de Spearman. Adotou-se um nível de significância de p<0,05 para análise dos dados. Resultados: Observou-se uma melhora significativa (p<0,0001) dos escores de todos os domínios e do escore total do FSFI ao final do tratamento quando comparadas a avaliação inicial e intermediária. Em relação a eletromiografia de superfície, as amplitudes das contrações fásicas e tônicas aumentaram significativamente (p<0,0001) ao longo do tratamento. Houve também um aumento na força do assoalho pélvico, com 69% das mulheres apresentando grau 4 ou 5 na avaliação final. Ao final do tratamento, a maioria das participantes (18) recebeu alta por não mais apresentar queixas relativas à função sexual. Conclusão: O treinamento dos MAP resultou em melhora na força e nas amplitudes da eletromiografia, acompanhadas de aumento significativo nos escores de função sexual, o que indica que essa abordagem terapêutica pode ser utilizada com sucesso no tratamento das disfunções sexuais femininas / Abstract:Objective: To evaluate the effect of pelvic floor muscle training in female sexual dysfunctions. Subjects and Methods: 26 women with a diagnosis of sexual dysfunction (sexual desire, arousal, orgasmic disorders and dyspareunia) were enrolled in a clinical trial, between February 2008 and May 2009. To evaluate the sexual function we used Female Sexual Function Index (FSFI) questionnaire. Bi-digital palpation was applied to assess muscle strength and surface electromyography to contraction amplitudes. The assessment was carried out before, during (after five sessions) and at the end of the treatment (after 10 sessions). The women underwent pelvic floor muscle training, once or twice a week, for 10 sessions in different positions. To analysis during the follow-up period, non parametric's and ANOVA's Friedman test, Bonferroni test and Spearman Correlation test were applied. Results were considered statistically significant if p<0.05. Results: All FSFI scores were higher (p<0.0001) at the end of the treatment in comparison with the values observed before and in the middle of the treatment. Regarding surface electromyography, tonic and phasic contractions amplitudes significantly increased over time (p<0.0001). Pelvic floor strength also increased in 69% of the women who had 4 or 5 grade in final evaluation. Eighteen participants had a total improvement of sexual complaints at the end of the treatment. Conclusions: The pelvic floor muscle training improved strength and surface electromyography amplitudes while the sexual function scores increased. Desenho Proposta de padronização de protocolo de avaliação da contração muscular do assoalho pélvico feminino por meio da eletromiografia de superfície. Procedimentos Para elaboração do protocolo considerou-se dados da literatura e a experiência dos pesquisadores em execução da EMG na prática clínica. Para verificar a aplicabilidade do protocolo proposto, selecionou-se amostra de 35 mulheres nuligestas que foram submetidas,em momento único, ao protocolo. Resultados O protocolo foi padronizado em quatro etapas: 1ª Etapa: Adequação do ambiente, 2ª Etapa: Conscientização e avaliação da contração muscular do assoalho pélvico, 3ª Etapa: Eletromiografia do assoalho pélvico e 4ª Etapa: Interpretação do sinal eletromiográfico. O protocolo aplicado na amostra descrita demonstrou que as participantes tinham consciência de como realizar a contração muscular do assoalho pélvico, as interferências na EMG foram mínimas, não prejudicando a qualidade do exame e demonstrou-se executável de modo geral. The study involved 31 women from the Clinic of Coloproctology Walter Cantídio University Hospital, Federal University of Ceará and the Center for Gastroenterology and Coloproctology of Ceará, in the period August 2010 to July 2012, with 28 to 64 years of age, who met the Rome III criteria and presented, eletromanométrico examination, presence of contraction to maneuver defecation. Were randomized into three groups: group I conventional treatment of constipation, group II conventional treatment of constipation associated with "biofeedback", Group III conventional treatment of constipation associated with electrical stimulation. At baseline and after six weeks of treatment, we assessed the subjective and objective parameters of anorectal function by means of the scoring system for constipation Wexner scale to Bristol stool consistency, visual analog scale and anorectal electromanometry. All patients demonstrated improvement in bowel satisfaction, stool frequency, effort and feeling of incomplete evacuation, stool-type modifications and improvement in quality of life (p <0.05). On examination there was increased pressure eletromanométrico average voluntary contraction in group III (p = 0.043), decreased sensitivity threshold in group II (p = 0.025) and in group III (p = 0.012) and decreased rectal capacity maximum in group II (p = 0.005). Only 19.4% (n = 6) normalized dynamic defecation, and 80.6% (n = 25) expressed clinical improvement and not instrumental. The results confirmed that the "biofeedback" and electrostimulation are valuable treatment options and impact on quality of life of these patients. The "biofeedback" was more effective than electrical stimulation, which in turn is more effective than conventional treatment for constipation. Further randomized, controlled, long-term are needed to confirm the results. In the future, studies with the combination of therapeutic modalities may be a therapeutic option more sustainable and effective. / Esse estudo foi realizado com o objetivo de avaliar os efeitos do “biofeedback” e da eletroestimulação na reversão da contração paradoxal da musculatura puborretal. Foram avaliadas 31 mulheres provenientes do Ambulatório de Coloproctologia do Hospital Universitário Walter Cantídio da Universidade Federal do Ceará e do Centro de Coloproctologia e Gastroenterologia do Ceará, no período agosto de 2010 a julho de 2012, com 28 a 64 anos de idade, que preenchiam os critérios de Roma III e apresentavam, ao exame eletromanométrico, presença de contração à manobra evacuatória. Foram randomizadas em três grupos: grupo I tratamento convencional da constipação, grupo II tratamento convencional da constipação associado ao “biofeedback”, grupo III tratamento convencional da constipação associado à eletroestimulação. No início e após seis semanas do tratamento, foram avaliados os parâmetros subjetivos e objetivos da função anorretal, por meio do sistema de pontuação para constipação de Wexner, escala de Bristol para consistência de fezes, escala analógica visual e eletromanometria anorretal. Todas as pacientes demonstraram melhora da satisfação intestinal, frequência evacuatória, esforço e sensação de evacuação incompleta, modificações do tipo de fezes e melhora da qualidade de vida (p<0,05). Ao exame eletromanométrico houve aumento da pressão média de contração voluntária no grupo III (p=0,043), diminuição do limiar de sensibilidade no grupo II (p=0,025) e no grupo III (p=0,012) e diminuição da capacidade retal máxima no grupo II (p=0,005). Somente 19,4% (n=6) normalizaram a dinâmica evacuatória, e 80,6% (n=25) expressaram melhora clínica e não instrumental. Os resultados confirmaram que o “biofeedback” e a eletroestimulação são opções terapêuticas valiosas e de impacto na qualidade de vida destes pacientes. O “biofeedback” foi mais eficaz do que a eletroestimulação, que é por sua vez mais eficaz do que tratamento convencional para constipação. Maiores estudos randomizados, controlados e em longo prazo são necessários para confirmar os resultados. No futuro, estudos com a associação das modalidades terapêuticas pode ser opção terapêutica mais duradoura e eficaz.

Statistical analysis was carried out using the Shapiro-Wilk test to verify the normality of the data. The independent t-test and the Mann-Whitney U test were used for data comparison. Spearman’s rank correlation was used to correlate data. The adopted level of significance was 5%. Results: Significant difference between the groups regarding the parity and duration of complaints; the PISQ-12 total score and the physical domain; the items Power-Pressure and Fast of the PERFECT scale. There was no significant correlation between the Maximum Voluntary Contraction (MVC) and the PISQ-12 total score. Conclusion: The tests did not find a correlation between the PFM functions and the SF in the groups. However, there was a significant difference in the SF between groups.

Comparação do estímulo vibratório com a eletroestimulação intravaginal em mulheres com incontinência urinária : um ensaio clínico randomizado

Introdução: O treinamento dos músculos do assoalho pélvico (MAP) é a primeira linha de tratamento conservador da incontinência urinária (IU), contudo, muitas mulheres não conseguem contrair essa musculatura voluntariamente. A eletroestimulação intravaginal (EEIV) é bem documentada nestes casos e o estímulo vibratório intravaginal (EVIV) também tem sido utilizado, mas poucos estudos abordam o método. Objetivo: Comparar a EEIV com o EVIV na funcionalidade dos MAP de mulheres com IU que não conseguem contrair voluntariamente os MAP. Materiais e métodos: Ensaio clínico randomizado realizado no Hospital de Clínicas de Porto Alegre de junho de 2016 a setembro de 2017. Critérios de inclusão: mulheres maiores de 18 anos, com IU e incapazes de contrair voluntariamente os MAP. Critérios de exclusão: alergia ao látex e treinamento dos MAP nos últimos seis meses Após avaliação, as pacientes que correspondiam aos critérios de inclusão foram randomizadas em grupo EEIV e EVIV. Foram realizadas seis sessões individuais de 20 minutos, uma vez por semana. Resultados: Foram randomizadas 21 mulheres para cada grupo e 18 completaram o protocolo de EVIV e 17 o de EEIV. Ao final, o grupo EVIV apresentou aumento significativo na força dos MAP em relação ao grupo EEIV (p=0,026) e houve interação significativa do tempo e tipo de intervenção para a mesma variável (p=0,008) no GEVIV. Conclusão: O EVIV foi significativamente melhor que a EEIV na melhora da força dos MAP, mas são necessários mais estudos para consolidar seu uso na melhora da funcionalidade dos MAP e no tratamento da IU. / Introduction: Pelvic floor muscle (PFM) training is the first line of conservative treatment for urinary incontinence (UI); however, many women are unable to contract these muscles voluntarily. Intravaginal electrostimulation (IVES) is well documented in these cases and intravaginal vibratory stimulation (IVVS) is also used, but few studies address this method. Objective: To compare the use of IVES and IVVS on the function of the PFM in women with UI who cannot voluntarily contract the PFM. Materials and methods: Randomized clinical trial performed at the Hospital de Clínicas de Porto Alegre from June 2016 to September 2017. Inclusion criteria: women over 18 years of age with UI and unable to voluntarily contract their PFM. Exclusion criteria: latex allergy and PFM training over the last six months. After the assessment, the patients that met the inclusion criteria were randomized into IVES and IVVS groups. Six individual 20-minute sessions were held once a week Results: Twenty-one women were randomly assigned to each group and 18 completed the IVVS and 17 the IVES protocols. In the end, the IVVS group presented a significant increase in strength of the PFM in relation to the IVES group (p = 0.026) and there was a significant interaction between time and type of intervention for the same variable (p = 0.008) in the IVVS group. Conclusion: IVVS was significantly better than IVES in improving PFM strength, but more studies are needed to consolidate its use regarding PFM function and the treatment of UI.

Avaliação ultrassonográfica do diâmetro uretral pós-parto e sua correlação com fatores gestacionais e incontinência urinária em seis meses após o nascimento

INTRODUÇÃO: A incontinência urinária (IU) é uma condição multifatorial, sendo que, para muitas mulheres, a gestação, o trabalho de parto e o parto constituem os eventos-sentinela para o seu aparecimento. A ultrassonografia transperineal (translabial) tem sido utilizada para avaliação das alterações anatômicas que ocorrem após o parto, sendo possível correlacionar seus resultados com os sintomas de IU. Delineamos um estudo para comparar o valor do diâmetro uretral de mulheres após o parto vaginal e após a cesariana eletiva, correlacionando estas medidas com fatores ligados à gestação e ao nascimento, e com a presença de IU no período de seis meses após o nascimento. MATERIAIS E MÉTODOS: Trata-se de um estudo transversal, composto por 205 pacientes. Foi realizada ultrassonografia transperineal para medida do diâmetro uretral, a nível do colo vesical e da uretra média, após o nascimento, e foram obtidas informações sobre a gestação e o parto, utilizando-se uma ficha específica para a coleta de dados. Seis meses após o nascimento, avaliamos a presença de IU nas pacientes, e quantificou-se a perda utrinária através do questionário ICIQ-SF (International Consultation on Incontinence – Short Form) (Tamanini, Dambros et al. 2004). RESULTADOS: Das 151 pacientes, 73 tiveram parto vaginal (grupo 1), e 78, cesariana eletiva (grupo 2). Houve diferença estatisticamente significativa na medida do diâmetro uretral no colo vesical após o parto, a qual foi menor no grupo 2 (p< 0,0001). Não houve diferença significativa na medida do diâmetro na uretra média entre os grupos (p=0,505). A medida do diâmetro uretral na uretra média apresentou correlação inversa com a presença IU em seis meses de seguimento (rs=0,219; p=0,014). Houve correlação positiva entre a presença de incontinência urinária durante a gestação e em seis meses após o nascimento (p=0,016). CONCLUSÕES: Uma diferença na medida ultrassonográfica do diâmetro uretral no colo vesical foi observada entre os grupos. Houve correlação inversa entre a medida do diâmetro uretral na uretra média e a presença de IU após seis meses de acompanhamento. / BACKGROUND: Urinary incontinence (UI) is a multifactorial condition, and for most women, pregnancy, labor and delivery are the main factors that contribute to its appearance. Transperineal ultrasound has been used to evaluate anatomic damages due to vaginal delivery, and these findings can be correlated to postpartum UI symptoms. We outlined a study to compare the measure of the urethral diameter in women who had a vaginal delivery or elective cesarean section and correlate this measure with pregnancy and labor linked factors, as well as with the presence of UI six months after birth. METHODS: A cross-sectional study was outlined, and 205 patients were recruited. Transperineal ultrasound was performed to measure the urethral diameter, both at mid urethra and at vesical bladder level, immediately after delivery, and data regarding pregnancy and labor were obtained. Six months after birth, patients evaluate the presence of UI, symptom through the ICIQ-SF (International Consultation on Incontinence – Short Form) questionnaire RESULTS: Of the 151 patients studied, 73 had a vaginal delivery (group 1) and 78, elective cesarean section (group 2). We found a significant difference between groups in urethral diameter al bladder neck level, which was smaller for the group 2 (p< 0,0001). We didn’t find any significant difference between groups in urethral diameter at the level of mid urethra (p=0,505). The urethral diameter at the level of mid urethra showed an inverse correlation with presence of urinary incontinence at six-month analysis (rs=0,219; p=0.014) and a positive correlation between UI during pregnancy and at six months after delivery (rs=0,214; p=0.016) was observed. CONCLUSIONS: A difference in the urethral diameter at the level of the bladder neck was observed between groups. We found an inverse correlation between urethral diameter at the level of the mid urethra and the presence of UI at six months after delivery.
