Apesar das evidências que associam a ocorrência de IU ao diabete, pouco se sabe dos mecanismos pelos quais o diabete leva à IU. Objetivo: verificar a associação entre a ocorrência de IU e função muscular do AP com os distúrbios hiperglicêmicos gestacionais. Materiais e Métodos: As gestantes foram avaliadas em dois momentos. O primeiro momento (M1) entre a 24a e 32a, e o segundo (M2) entre a 34ª e 38a semanas gestacionais. A análise estatística foi realizada com nível de significância de 5%. Resultados: As hiperglicêmicas apresentaram idade cronológica e gestacional mais avançadas, além de menor estatura. Houve diferença significativa na distribuição peso-altura entre os grupos no M1. Não houve diferença entre os grupos, no M2 quanto à ocorrência de IU. Na análise intergrupos da palpação vaginal, observamos redução da força muscular, enquanto que algumas mantiveram ou aumentaram. Na comparação das avaliações com perineômetro e eletromiógrafo, entre e intragrupos, nos grupos, não houve diferenças significativas. Conclusão: Devido às limitações do estudo, principalmente a dificuldade de conscientização e aprendizado da contração do AP, ressaltamos a importância de estudos nessa área, e padronização dos métodos de avaliação, para possivel comparação com diferentes estudos, tendo em vista à qualidade de vida da gestante / The pelvic floor (PF) can suffer alterations in the different stages of women life that may predispose to the Urinary Incontinence (UI). Despite the evidences which associate the UI to the diabetes, there is little information about the mechanisms which take diabetes to the UI. Objective: to analyze the association between the UI occurrence and the PF muscle function with the gestational hyperglycemic disorders. Material and Methods: the pregnant women were evaluated in two moments. The first moment (M1) was between the 24th and 32nd, and the second one (M2) was between the 34th and 38th gestational week. The statistical analysis was realized considering 5% relevance. Results: The hyperglycemic ones presented more advanced chronological and gestational ages, besides they were shorter. There was relevant difference in the weight-height distribution between the groups in the M1. There was no difference between the groups in the M2 considering the UI occurrence. In the intergroup analysis of digital palpation, muscle strength reduction was observed, although for some there was an increase and some kept the same. In the comparison of the evaluations with perineometer and electromyographer, inter and intra groups, there were no relevant differences. Conclusion: Due to the limitations of the study, mainly the awareness and learning of the PF contraction difficulty, we emphasize the importance of developing studies in this area and standardization of the evaluation methods to enable the comparisons with different studies aiming the pregnant women life quality

Desenho Proposta de padronização de protocolo de avaliação da contração muscular do assoalho pélvico feminino por meio da eletromiografia de superfície. Procedimentos Para elaboração do protocolo considerou-se dados da literatura e a experiência dos pesquisadores em execução da EMG na prática clínica. Para verificar a aplicabilidade do protocolo proposto, selecionou-se amostra de 35 mulheres nuligestas que foram submetidas,em momento único, ao protocolo. Resultados O protocolo foi padronizado em quatro etapas: 1ª Etapa: Adequação do ambiente, 2ª Etapa: Conscientização e avaliação da contração muscular do assoalho pélvico, 3ª Etapa: Eletromiografia do assoalho pélvico e 4ª Etapa: Interpretação do sinal eletromiográfico. O protocolo aplicado na amostra descrita demonstrou que as participantes tinham consciência de como realizar a contração muscular do assoalho pélvico, as interferências na EMG foram mínimas, não prejudicando a qualidade do exame e demonstrou-se executável de modo geral. Conclusão A proposta de padronização de protocolo de EMG do assoalho pélvico feminino foi composta por quatro etapas com descrição detalhada de todos os procedimentos, mostrouse aplicável e com qualidade dos dados coletados

Os exercícios para o assoalho pélvico podem promover a recuperação da ereção após a prostatectomia radical. Trata-se de um ensaio clínico prospectivo e randomizado em oitenta pacientes com diagnóstico clínico de adenocarcinoma de próstata localizado, randomizados em três grupos: grupo controle (n=26) onde os pacientes não receberam tratamento específico, grupo orientação (n=30) onde os pacientes foram orientados a fazer exercícios diariamente em suas residências e o grupo de eletroestimulação (n=24), onde os pacientes foram submetidos à eletroestimulação anal e exercícios para o assoalho pélvico diariamente em suas residências. Os grupos foram comparados quanto à idade, escolaridade, radioterapia no pós-operatório, estadiamento patológico, escore de Gleason, comorbidades e IMC, somente apresentando diferenças entre os grupos a cardiopatia. Quando realizada a evolução da variável IIEF-5, força e tempo de contração no músculo do assoalho pélvico ao longo dos doze meses, nenhuma das variáveis apresentaram p-valor significativo referente à interação, ou seja, os grupos possuem a mesma performance. Verificamos a correlação entre os valores obtidos do questionário IIEF-5 com a força de contração do assoalho pélvico a cada medida do tempo analisada, estas medidas não apresentaram correlação. Os resultados demonstraram que a reabilitação do assoalho pélvico não interferiu na recuperação da função erétil dos pacientes ao longo de doze meses após da prostatectomia radical / Radical prostatectomy is one of the treatment options for prostate cancer being carried out by various techniques, but with very high rate of postoperative erectile dysfunction, worsening the quality of life. The exercises for the pelvic floor may promote recovery of erections after radical prostatectomy. This prospective, randomized clinical trial was performed in 80 patients with clinical diagnosis of localized prostate adenocarcinoma who were randomized into three groups: the control group (n = 26) where patients received no specific treatment, the exposure group (n = 30) where patients were instructed to exercise daily in their homes, and the electrical stimulation group (n = 24), where patients underwent anal electrical stimulation and exercises for the pelvic floor daily in their homes. The groups were compared for age, education, postoperative radiotherapy, pathological staging, Gleason score, comorbidities, and BMI, and the only difference among them was heart disease. When carried out the evolution of the IIEF-5 variable strength and timing of contraction in the muscle of the pelvic floor over the twelve months, none of the variables showed significant p-value related to the interaction, that is, the groups have the same performance. We verified the correlation between the values of the IIEF-5 questionnaire and the force of contraction of the pelvic floor, to every analyzed timing measurement, these measures were not correlated. The results showed that rehabilitation of the pelvic floor did not affect the recovery of erectile function in patients over twelve months following radical prostatectomy

Tem-se na literatura atual que, em mulheres jovens, fatores gineco-obst?tricos e h?bitos de vida influenciam nessa perda de fun??o, no entanto, ainda h? incertezas sobre a influ?ncia dessas vari?veis no funcionamento da MAP de mulheres na p?s-menopausa. Assim, o presente estudo visou avaliar se h? influ?ncia da paridade, tipo de parto e n?vel de atividade f?sica sobre a MAP em mulheres na p?s-menopausa. Objetivou-se ainda comparar a for?a da MAP em mulheres na p?s-menopausa submetidas a parto normal e ces?rea, com diferentes n?veis de atividade f?sica, e com menopausa artificial e natural nas fases inicial e tardia, al?m de correlacionar o teste de for?a muscular e perineometria e analisar a confiabilidade inter-examinador da perineometria. Atrav?s de um estudo observacional, anal?tico e transversal foram estudadas 100 mulheres no per?odo da p?s-menopausa, com idades entre 45 e 65 anos, divididas, de acordo com o estado menopausal, em tr?s grupos: histerectomizadas, p?s-menopausa inicial e tardia. As pacientes foram questionadas sobre fatores sociodemogr?ficos e hist?ria gineco-obst?trica. Todas as volunt?rias foram submetidas ao exame f?sico, no qual foi mensurado o peso e altura para c?lculo do ?ndice de massa corporal e a circunfer?ncia cintura. A avalia??o do assoalho p?lvico foi realizada investigando a for?a e a press?o da MAP por meio do teste de for?a muscular e da perineometria. Para an?lise dos resultados foram utilizados a estat?stica descritiva, os testes de compara??o como ANOVA e a regress?o m?ltipla, al?m do teste Kolmogorov-Smirnov aplicado para testar a normalidade das vari?veis. Nos resultados, observou-se que em rela??o ?s caracter?sticas sociodemogr?ficas e antropom?tricas na amostra final (n = 85). Sendo observado que a maioria das mulheres nos tr?s grupos eram casadas (p=0.51) e cat?licas (p=0.13). A renda per capta variou de R$ 585,47 ? 466,67 a R$1.271,83 ? 1.748,97, sendo sem signific?ncia a diferen?a entre os grupos (p=0.05). O G>6apresentou uma m?dia de idade de 58,95 ?3,96 significativamente maior que o G<6 (53,21? 3,88) (p=0.000). Quanto as caracter?sticas antropom?tricas, n?o houve diferen?a entre os grupos em rela??o ao peso (p=0.32), altura (p=0.72), IMC (p=0.34) e circunfer?ncia de cintura (p=0.33). Obteve-se que o tipo de parto, paridade e n?vel de atividade f?sica n?o apresentaram influ?ncia sobre a fun??o da musculatura do assoalho p?lvico nas mulheres estudadas (p= 0.794). N?o foram vistas diferen?as na fun??o da MAP de mulheres submetidas a partos normais, cesarianos e normais + cesarianos (TFM p= 0.897; perineometria p = 0.502), de mulheres com 1 parto, 2-3 partos ou 4 partos ou mais (TFM p=0.28; perineometria p=0.13), e entre aquelas com diferentes n?veis de atividade f?sica (TFM p= 0.663; perineometria p=0.741) Acredita-se que o decl?nio da fun??o muscular em mulheres na p?s-menopausa esteja relacionado, fundamentalmente, ao processo de envelhecimento feminino. / The post-menopause stage is characterized by hormonal and organic alterations of ovarian failure. One of the most important of these is muscles alterations of the pelvic floor (MPF). According to current literature, in young women gynecological and obstetric factors, as well as lifestyles and habits influence that loss of function. However, there is still uncertainty about the influence of those variables in the MPF functions in post-menopause women. Thus, this study aimed at seeing if there is an influence from number of births, the type of birth and the level of physical activity on the MPF of post-menopause women. Another objective of this study was to compare MPF force in women who had had vaginal births with those who had been subjected to cesarean sections, those with different levels of physical activity and those with artificial and natural menopause in the initial and latter stages. Furthermore, the test of muscular force was compared to perineometry. Using observational, analytical and transversal observations, 100 women in the post-menopausal stage of life, between the ages of 45 and 65, were examined. They were divided according to the menopausal stage into three groups: women who had undergone hysterectomies, those in the initial stages of postmenopause and those in the late stage of postmenpause. The patients were questioned about social, demographic, gynecological and obstetric factors. All the volunteers were submitted to a physical examination where their height and weight were measured to arrive at the corporal mass index and their waist measurements were taken. The evaluation of the pelvic floor was conducted with muscular force tests and perineometry. These results were analyzed with statistical description and ANOVA statistical tests, multiple regression and Kolmogorov-Smirnov evaluations. The results showed homogeneity with regard to social demographic and anthropometric characteristics among the women in the final test sample (n=85). It was also seen that most of the women in all three groups were married (p=0.51) and catholic (p=0.13). The average per capital income varied between $R585.47 (+/-466.67) and $R1,271.83 (+/-1,748.95), with no significant difference between the groups (p=0.05). The G>6 group presented an average age between 58.95 (+/-3.96) which was significantly greater that the G<6 group?s average age (53.21+/- 3.88) (p=0.000). There was no difference between the groups? anthropometric characteristics of weight (p=0.32), height (p=0.72) and corporal mass index (p=0.34), nor in the waist measurements (p=0.33). Furthermore, no significant difference was noted in the MPF function of women who had had normal births, cesarean sections or a combination of the two (TFM p=0.897; perineum measurement p=0.502). Likewise, no differences were seen in the MPF function of women who had one, two to three or four or more births (TFM p=0.28, perineum measurement p=0.13). Finally, no difference was perceived among those with different levels of physical activity (TFM p=0.663; perineum measurement p=0.741). Therefore, we found that the type of delivery, number of births and physical activity had no influence on the muscular function of the pelvic floor among the women studied. It is believed that decline in muscular function in post-menopause women is fundamentally related to the process of aging.

Estes m?sculos s?o ativados em diferentes situa??es em que h? o aumento da press?o intra-abdominal. Sua integridade e bom funcionamento s?o fundamentais na manuten??o da contin?ncia urin?ria e fecal e na sustenta??o aos ?rg?os p?lvicos. Existem diversas t?cnicas de avalia??o da MAP respaldadas pela literatura, dentre elas a perineometria. Os perine?metros t?m por objetivo medir as altera??es de press?o na vagina em resposta ? contra??o volunt?ria dos MAP. Os equipamentos de perineometria s?o simples, minimamente invasivos e de baixo custo. Apesar de ser uma medida objetiva do grau de press?o desempenhado pela contra??o volunt?ria dos MAP, a perineometria n?o possui intervalos de classifica??o que guiem a interpreta??o dos seus resultados cl?nicos. Objetivo: Desenvolver uma escala de classifica??o da perineometria. Metodologia: Estudo de car?ter observacional, transversal. A amostra foi resultante de um processo de amostragem do tipo n?o probal?stico. As mulheres foram recrutadas por demanda espont?nea. Foi realizada uma avalia??o perineom?trica e o Teste Muscular Manual dos MAP, por meio do toque bidigital, categorizando-se a for?a pela Escala Modificada de Oxford. Os dados coletados foram tabulados e analisados no programa IMB Statistical Package for the Social Sciences vers?o 20.0. Para determinar os valores de classifica??o da perineometria foi realizada uma regress?o linear simples, tendo como vari?vel explicativa a Escala Modificada de Oxford e como vari?vel resposta a m?dia aritm?tica das tr?s medidas da perineometria. A regress?o linear foi realizada no programa estat?stico R vers?o 3.2.4. Resultados: Foram inclu?das na an?lise 259 mulheres, que apresentaram m?dia de idade de 52,80 (? 8,78) anos. A perineometria apresentou uma m?dia de 35,1? 22,7 (IC: 32,1 ? 38,0) cmH2O. A mediana do grau de for?a dos MAP foi tr?s (Q25: 2; Q75: 3). Houve uma correla??o forte, positiva e estatisticamente significativa entre o grau de for?a pela escala modificada de Oxford e a perineometria (r = 0,846, p < 0,01). A estratifica??o das medidas perineom?tricas foi realizada em uma escala de cinco pontos, que varia de uma press?o muito fraca a uma forte press?o. De acordo com a escala proposta, valores entre 7,5 e 14,5 cmH2O correspondem a uma press?o muito fraca; o intervalo de 14,6 a 26,5 cmH2O ? equivalente a uma press?o fraca; uma press?o moderada est? no intervalo de 26,6 a 41,5 cmH2O; valores de 41,6 a 60,5 cmH2O representam uma press?o boa e valores acima de 60,6 uma press?o forte. Conclus?o: Os valores de perineometria foram estratificados em uma escala de cinco pontos. Para a literatura, promove-se, pela primeira vez, a apresenta??o de uma escala com valores padr?es da perineometria. / Introduction: Pelvic floor muscles (PFM) correspond to a group of skeletal muscles that are located at the basis of the pelvic cavity. These muscles are activated in different situations in wich there is an increase in intra-abdominal pressure. Its integrity and good functioning are fundamental in maintaining urinary and fecal continence and in sustaining the pelvic organs. There are several PFM evaluation techniques supported by the literature, such as the perineometry. Perineometers are aimed to measure pressure changes in the vagina in response to voluntary contraction of PFM. Perineometry equipments are simple, minimally invasive and inexpensive. Althought it is an objective measure of the degree of pressure performed by the PFM voluntary contraction, perineometry does not have rating ranges that guide the interpretation of its results. Objective: To develop a classification scale for perineometry. Methodology: A cross-sectional observational study. The sample was the result of a non-probalistc type sampling process. Women were recruted on spontaneous demand. A perineometric evaluation and the manual strength test of the PFM were performed by means of the bidigital touch, and the force was categorized by the Oxford Modified Scale. The collected data were tabulated and analyzed in the program IMB Statistical Package for the Social Sciences version 20.0. To determine the classification ranges of perineometry, a simple linear regression was performed, using as an explanatory variable the Oxford Modified Scale and as a response variable the arithmetic mean of the three measurements of perineometry. The linear regression was performed in the statistical program R version 3.2.4. Results: The 259 women included in the analysis had a mean age of 52.80 (? 8.78) years. Perineometry presented an average of 35.1 ? 22.7 (CI: 32.1 - 38.0) cmH2O. The median PFM strength level was 3 (Q25: 2; Q75: 3). There was a strong, positive and statistically significant correlation between the degree of strength by the Oxford Modified Scale and perineometry (r = 0.846, p <0.01). The stratification of the perineometric measurements was performed on a five-point scale, ranging from very weak pressure to strong pressure. According to the proposed scale, values between 7.5 and 14.5 cmH2O correspond to a very weak pressure; The range of 14.6 to 26.5 cmH2O is equivalent to a weak pressure; A moderate pressure is in the range of 26.6 to 41.5 cmH2O; Values of 41.6 to 60.5 cmH2O represent a good pressure and values above 60.6 is a strong pressure. Conclusion: Perineometric values were stratified on a five-point scale. By observing the results, from this scale it is possible to establish the ranges of pressure exerted by the voluntary contraction of the PFM. The achieved result in this work has implications in clinical practice, since it will guide the specialists and the patients about the degree of muscular strength. For the literature, is promoted for the first time the presentation of a scale of perineometry classification standards.
