Apesar de existir um número considerável de estudos sobre o tema, ainda não há um consenso sobre sua extensão, manejo e repercussões para os pacientes e seus pais. Este estudo buscou investigar as representações de mães e pais, de crianças com ADS, sobre a identidade de gênero de seus filhos. Foram entrevistados 3 mães e 3 pais de filhos nascidos com ADS, de diferentes idades. A entrevista narrativa e a análise de conteúdo foram utilizadas, respectivamente, como forma de coleta e análise dos dados. Os resultados mostraram que, tanto mães quanto pais, atribuem fundamental importância à anatomia da genitália como critério para definição da identidade sexual da criança, base sobre a qual se constitui, ao longo de seu desenvolvimento, a identidade de gênero da mesma. O estudo também possibilitou o levantamento e discussão de outros aspectos considerados importantes na opinião das mães e dos pais entrevistados como, por exemplo, o impacto emocional do diagnóstico quando do nascimento da criança, a investigação etiológica, o tratamento da ADS, o manejo médico e questões acerca do futuro dos filhos com ADS, que foram discutidos na pesquisa. / The disorders of sexual diferentiation which result from genital ambiguity in children, nowadays called Disorders of Sex Development (DSD), have become a problem and a challenge for the professional team who deals with these cases. Quando essa linhagem não é detectada, esses indivíduos deixam de ser investigados em relação a uma série de problemas clínicos. Este trabalho teve como objetivo verificar se a hibridação in situ com fluorescência (FISH) em células de mucosa oral poderia ser empregada para detectar criptomosaicismo com linhagem 45,X em indivíduos com cariótipo 46,XY. A casuística foi composta por 19 homens saudáveis com idades entre 20 e 30 anos e cinco pacientes com distúrbios da diferenciação do sexo (DDS) com idades entre 5 e 23 anos, quatro com mosaico 45,X/46,XY e um com disgenesia testicular 46,XY associada a déficit de crescimento. Após confirmar que os jovens saudáveis tinham cariótipo 46,XY em 50 metáfases de linfócitos de sangue periférico, foi realizada análise por FISH com sondas específicas para os cromossomos X e Y em 1.000 núcleos interfásicos de linfócitos de sangue periférico e 1.000 de mucosa oral, seguida de comparação da proporção de núcleos contendo apenas o sinal do cromossomo X nos dois tecidos. A mesma análise foi feita nos cinco pacientes com DDS. A distribuição da proporção dos núcleos interfásicos de linfócitos e mucosa oral contendo apenas o sinal do X nos jovens saudáveis foi compatível com a distribuição normal, e número superior a 12:1.000 em linfócitos e 13:1.000 em mucosa oral devem ser considerados indicativos de mosaicismo em nosso laboratório. A frequência desses núcleos nos dois tecidos não diferiu significativamente (p=0,6855). Nos cinco pacientes com DDS a frequência de núcleos contendo apenas o sinal do X diferiu significativamente da observada em indivíduos normais em linfócitos (p=0,0008) e mucosa oral (p=0,0008). No paciente com cariótipo prévio 46,XY a linhagem 45,X foi confirmada por FISH em metáfases, e em um dos casos de mosaicismo foram detectadas linhagens celulares adicionais. Também não houve diferença significativa entre a frequência de núcleos contendo apenas o sinal do X nos dois tecidos desses pacientes (p=0,3750). Estes resultados indicam que a pesquisa de mosaicismo com linhagem 45,X em indivíduos com DDS ou esterilidade e cariótipo 46,XY pode ser feita por meio de FISH em mucosa oral, com vantagens evidentes em termos de custo e rapidez, além de ser feita a partir de tecido obtido de modo não invasivo / Abstract: Ambiguous genitalia and sterility with a 46,XY karyotype may be due to mosaicism with a 45, X karyotype not detectable in peripheral blood lymphocytes. When this cell line is not detected, these individuals fail to be investigated over a range of clinical problems. This study aimed to verify whether fluorescence in situ hybridization (FISH) in cells from buccal smear could be employed to detect cryptomosaicism with a 45,X cell line in individuals with a 46,XY karyotype. The sample consisted of 19 healthy men aged 20 to 30 years and five patients with disorders of sex development (DSD) aged 5 to 23 years, four with mosaicism 45,X/46,XY and one with testicular dysgenesis 46, XY associated with growth deficiency. After confirming that the healthy young men had a 46,XY karyotype in 50 metaphases from peripheral blood lymphocytes, FISH analysis with probes specific for chromosomes X and Y was done in 1,000 nuclei from peripheral blood lymphocytes and 1,000 from buccal smear, followed by comparison of the proportion of nuclei containing only the signal of the X chromosome in these tissues. The same analysis was performed in five patients with DDS. The distribution of the proportion of interphase nuclei of lymphocytes and buccal smear containing only the X signal in healthy young was consistent with normal distribution; a number greater than 12:1,000 in lymphocytes and 13:1,000 in buccal smear should be considered indicative of mosaicism in our laboratory. The frequency of these nuclei in both tissues did not differ significantly (p = 0.6855). In patients with DDS the frequency of nuclei containing only the X signal differed significantly from that observed in normal individuals both in lymphocytes (p = 0.0008) and buccal smear (p = 0.0008). In the patient with a prior 46,XY karyotype, a 45,X cell line was confirmed by FISH in metaphases, and in one case of mosaicism additional cell lines were detected. There was also no significant difference between the frequency of nuclei containing only the X signal in the two tissues of these patients (p = 0.3750). These results indicate that investigation of mosaicism with 45,X cell line in individuals with 46,XY DSD or sterility can be done by FISH in cells from buccal smear, with obvious advantages in terms of cost and speed, using a tissue obtained noninvasively / Mestrado / Ciencias Biomedicas / Mestra em Ciências Médicas

Etude des gènes impliqués dans le déterminisme gonadique chez l'homme / Genetic study of gonadal development in human

Les anomalies du développement sexuel recouvrent un spectre phénotypique large. Les hommes XX présentent dans la majorité des cas un développement testiculaire normal, lié à la présence SRY sur un des chromosomes X. Dans 10% des cas, aucune cause n’est retrouvée. Chez les femmes, l’origine de l’insuffisance ovarienne prématurée (IOP) n’est identifiée que dans 20% des cas. L’objectif de cette thèse a été d’identifier de nouveaux mécanismes moléculaires impliqués dans le développement gonadique, testiculaire et ovarien, ainsi que dans son fonctionnement. L’étude d’une cohorte de patients DSD 46,XX SRY négatifs a mis en évidence une duplication de la région RevSex dans un désert génique en amont de SOX9 chez quatre patients. Ceci a permis de redéfinir la région minimale impliquée dans l'activation de l'expression de SOX9 à une taille maximale de 41.9 kb et de proposer un mécanisme permettant cette expression chez des hommes XX. Le séquençage d’exome chez dix patients de la cohorte n’a pas mis en évidence de mutations dans des gènes d’intérêts. L’ensemble de ces résultats pose la question du rôle des régions régulatrices dans la survenue des DSD.L’analyse d’une cohorte de patientes ayant une IOP a permis d'identifier des délétions incluant le gène CPEB1. Des études précédentes chez la souris ont montré son implication dans le développement folliculaire. Le séquençage du gène CPEB1 dans la cohorte n'a pas mis en évidence de mutation pathogène. Ce travail a permis de montrer que la délétion impliquant le gène CPEB1est une cause rare mais récurrente d'IOP et concerne environ 1% des patientes. Une microdélétion contenant le gène CASP3 un gène de la voie des caspases impliquée dans la régulation du pool folliculaire a également été identifiée chez une patiente. L'ensemble de ces résultats montre l'intérêt de l'étude génétique des patients présentant une anomalie du développement de la gonade ou de son fonctionnement par des techniques d'étude globale du génome. / Disorders of Sex Development (DSD) can be identified in new-born and during infancy but also in adults because of infertility. Most 46,XX testicular DSD have a normal testicular development due to the presence of the SRY gene at the tip of one of their X chromosome. However, the genetic causes of 46,XX-SRY negative testicular DSD remain poorly defined. In women, disorders of gonadal development can be responsible for primary ovarian insufficiency (POI) and genetic causes are identify in only 20% of cases. The aim of this thesis was to identify molecular mechanisms involved in gonadal development and in its functioning. The cohort study of 46,XX testicular DSD identified four patients with a duplication in the previously reported RevSex region located about 550 kb upstream of SOX9. One duplication allowed us to refine the minimal region associated with 46,XX-SRY negative DSD to a 40.7–41.9 kb element. Exome sequencing of 10 patients from the cohort did not show any mutation in genes implicated in DSD or in new candidate genes. These results raise questions about the role of the regulatory sequences in the onset of DSD.The cohort study of POI patients identified three patients carrying a microdeletion including CPEB1 a good candidate gene for POI as study in mice showed the implication of CPEB1 in follicular development. Sequencing CPEB1 gene did not identified any mutation. Therefore, heterozygous deletion of CPEB1 gene leading to haploinsufficiency could be responsible for POI in humans. This microdeletion is rare but recurrent and was identified in about 1% of patients with POI. Another microdeletion containing CASP3 gene that belongs to the caspase family, which is implicated in the regulation of the follicular pool, was identified in a patient. Further studies are needed to confirm the role of CASP3 in POI. These results demonstrate the importance of genetic study of patients presenting with DSD or POI using whole genome techniques.

Introdução: As doenças crônicas que envolvem tratamento clínico e cirúrgico podem comprometer a qualidade de vida. Poucos estudos analisam a qualidade de vida de pacientes com distúrbios do desenvolvimento sexual (DDS). O objetivo foi avaliar a qualidade de vida de pacientes com DDS com o diagnostico etiológico estabelecido, seguidos até a idade adulta em um único centro terciário. Pacientes e Métodos: 144 pacientes adultos com DDS (56 pacientes com DDS 46,XX - 49 com sexo social feminino e 7 com o sexo social masculino, bem como, 88 pacientes com DDS 46,XY - 54 com sexo social feminino e 34 com sexo social masculino). Instrumento: A avaliação da qualidade de vida foi realizada através do questionário WHOQOL-Bref. Resultados: Os pacientes com DDS 46,XX e 46,XY apresentaram escores de qualidade de vida semelhante e comparáveis aos escores da população brasileira geral. Os pacientes com DDS do sexo social masculino tiveram melhores escores no domínio psicológico do que os pacientes do sexo social feminino, da mesma forma que a população geral brasileira. Dentro do grupo DDS 46,XY, também os pacientes com o sexo social masculino tiveram melhores escores de qualidade de vida em comparação aos do sexo social feminino. Para avaliar o impacto na qualidade de vida dos pacientes com DDS 46,XY criados no sexo social feminino, foi comparado os escores de qualidade de vida nos pacientes registrados no sexo social masculino com aqueles dos pacientes registrados no sexo social feminino e que mudaram para o sexo social masculino. Ambos os grupos apresentaram escores semelhantes de qualidade de vida. Comparou-se ainda, a qualidade de vida de pacientes com DDS 46,XY com deficiência de 5alfa-RD2 e pacientes com DDS, devido à defeitos na secreção ou ação da testosterona. Ambos os grupos apresentaram qualidade de vida semelhante entre si para as questões gerais e nos quatro domínios. A maioria das variáveis que influenciaram a qualidade de vida foram saúde geral, sentimentos positivos e espiritualidade, religião e crenças pessoais, cada um deles contribuindo com 18% da variabilidade da pontuação da qualidade de vida geral. O desempenho sexual teve pouca interferência na qualidade de vida geral, explicando apenas 4% da variabilidade deste escore. O tratamento tardio foi associado negativa e significativamente com pior qualidade de vida geral. Conclusão: Esta larga coorte de pacientes adultos com DDS, que foi seguida por uma equipe multidisciplinar em um único centro terciário, teve boa qualidade de vida na idade adulta. Ressalta-se que o tratamento tardio comprometeu a qualidade de vida dos pacientes com DSD, ao passo que o desempenho sexual teve pouca influência na qualidade de vida geral / Objective: Chronic diseases involving medical and surgical treatment may compromise the quality of life. Few studies have focused on the quality of life of patients with disorders of sex development (DSD). The aim was to evaluate quality of life in DSD patients with defined diagnoses followed until adulthood in a single tertiary centre. Patients and Methods: 144 Adult DSD patients (56 patients with 46,XX DSD - 49 with female social sex and 7 with male social sex as well as 88 patients with 46,XY DSD - 54 with female social sex and 34 with male social sex). Measurements: Quality of life using WHOQOL-Bref questionnaire Results: Both 46,XX and 46,XY DSD patients had similar quality of life scores on the WHOQOL-Bref, comparable to the scores of the Brazilian general population. Male social sex DSD patients had better scores on the psychological domain than female social sex DSD patients, as found in the Brazilian general population. In addition, among the 46,XY DSD group, the male social sex patients had better quality of life compared to the female social sex patients. To estimate the impact on quality of life of patients with DDS 46, XY raised in females social sex, we analyzed the quality of life scores of patients raised with male social sex with those patients registered with female social sex who changed to male social sex. Both groups had similar quality of life. We also evaluate the impact of testosterone in the quality of life of patients with 46,XY DSD with 5alfa-RD2 deficiency and patients with DSD due to testosterone secretion or action defects. Both groups showed similar quality of life. The most influencing variables on quality of life of all group of patients were general health, positive feelings and spirituality, religion and personal beliefs, each of them contributing with 18% of the variability of the general quality of life score. There was a positive and significant correlation between sexual performance and general quality of life, although it explained only 4% of the variability of the general quality of life score. Late treatment was associated negatively and significantly with poorer overall quality of life. Conclusion: This large cohort of adult DSD patients, which was followed by a multidisciplinary team in a single tertiary centre, had good quality of life in adulthood; in addition, late treatment compromised the quality of life of DSD patients, whereas sexual performance had little influence on quality of life

Patients and Methods: 144 Adult DSD patients (56 patients with 46,XX DSD - 49 with female social sex and 7 with male social sex as well as 88 patients with 46,XY DSD - 54 with female social sex and 34 with male social sex). Measurements: Quality of life using WHOQOL-Bref questionnaire Results: Both 46,XX and 46,XY DSD patients had similar quality of life scores on the WHOQOL-Bref, comparable to the scores of the Brazilian general population. Male social sex DSD patients had better scores on the psychological domain than female social sex DSD patients, as found in the Brazilian general population. In addition, among the 46,XY DSD group, the male social sex patients had better quality of life compared to the female social sex patients. To estimate the impact on quality of life of patients with DDS 46, XY raised in females social sex, we analyzed the quality of life scores of patients raised with male social sex with those patients registered with female social sex who changed to male social sex. Both groups had similar quality of life. We also evaluate the impact of testosterone in the quality of life of patients with 46,XY DSD with 5alfa-RD2 deficiency and patients with DSD due to testosterone secretion or action defects. Both groups showed similar quality of life. The most influencing variables on quality of life of all group of patients were general health, positive feelings and spirituality, religion and personal beliefs, each of them contributing with 18% of the variability of the general quality of life score. There was a positive and significant correlation between sexual performance and general quality of life, although it explained only 4% of the variability of the general quality of life score. Late treatment was associated negatively and significantly with poorer overall quality of life. Conclusion: This large cohort of adult DSD patients, which was followed by a multidisciplinary team in a single tertiary centre, had good quality of life in adulthood; in addition, late treatment compromised the quality of life of DSD patients, whereas sexual performance had little influence on quality of life

Os distúrbios do desenvolvimento sexual (DDS) 46,XY por anormalidades do desenvolvimento gonadal são doenças genéticas heterogêneas. Os testes genéticos que utilizam o sequenciamento de genes candidatos pelo método de Sanger é efetivo para o diagnóstico etiológico em menos de 40% desses pacientes. O sequenciamento de nova geração é uma ferramenta eficaz no diagnóstico de pacientes com DDSs. No presente estudo, utilizamos o sequenciamento exômico paralelo em larga escala (SEPLE) para identificar variantes alélicas potencialmente causadoras dos DDSs. Pacientes com síndrome de regressão testicular embrionária (SRTE) familial, membros de duas famílias brasileiras não relacionadas (família 1: dois irmãos afetados e pais não afetados e família 2: indivíduo índice, irmã não afetada e sobrinho afetado, filho da irmã do índice) foram selecionados inicialmente. Estes pacientes apresentavam micropenis, testículos ausentes ou disgenéticos e ausência de útero. O DNA genômico foi extraído de leucócitos de sangue periférico e o SEPLE foi realizado usando a plataforma Illumina-HiSeq-2500. Uma nova variante em heterozigose, c.923C > T, (p.Arg308Gln), no DHX37 foi identificada em todos os membros afetados das duas famílias brasileiras. O sequenciamento por Sanger confirmou a presença da variante em todos os afetados, no pai não afetado da F1 e na mãe não afetada do F2. A presença de efeito fundador da variante nestas duas famílias foi excluído, através da análise de 29 marcadores localizados no cromossomo 12q. Trinta e cinco pacientes com 46, XY DDS por anormalidades do desenvolvimento gonadal (6 com SRTE e 29 com disgenesia gonadal) esporádicos foram incluídos na pesquisa de variantes no DHX37 por Sanger ou por painel de genes candidatos (Plataforma Illumina-HiSeq-2500). As variantes c.923C > T, (p.Arg308Gln) em heterozigose e a variante c.451G > A, (p.Arg151Trp) em homozigose foram identificadas no DHX37 em dois pacientes com SRTE esporádicos. As variantes c.142C > T (p.Ala48Thr) e c.2020G > A (p.Arg674Trp) foram identificadas em heterozigose em dois pacientes esporádicos com disgenesia gonadal (DG). De acordo com o American College of Medical Genetics e Genomics guideline (ACMG), as variantes foram classificadas como provavelmente patogênicas (p.Arg308Gln e p.Arg674Trp), variante de significância clínica incerta (p.Arg151Trp) e variante benigna (p.Ala48Thr). Três variantes identificadas no DHX37 eram causadoras de doença em mais de três das ferramentas de previsão in silico utilizadas. Nenhuma das novas variantes no DHX37 foi encontrada nas bases de dados da população disponíveis (EXAC, 1000GENOME, ESP6500, gnomAD e abraOM) com uma frequência maior do que 0,01%. O padrão de metilação da região promotora do DHX37 dos membros afetados e não afetados das famílias 1 e 2 e controles femininos e masculino foi analisado utilizando o ensaio Infinium Methylation EPIC BeadChip (Illumina). Um sítio CpG hipometilado na região promotora do DHX37 foi observado apenas no pai da F1 quando comparado com as amostras dos filhos afetados, membros da F2 e controles. A análise imuno-histoquímica do padrão de expressão da proteína DHX37 foi realizada em amostras de testículos de oito indivíduos com diferentes idades cronológicas. Esta analise revelou que no tecido testicular de recém-nascidos, indivíduos em idade puberal e adultos, o DHX37 estava expresso em células germinativas em diferentes estágios de maturação (presentes nas espermatogonias, espermatócitos e espermátides e ausente nos espermatozoides). Este achado sugere que a proteína DHX37 pode ter um papel no processo de desenvolvimento das células germinativas. O DHX37 (12q24.31) codifica uma proteína DEAH (Asp-Glu-Ala-His) que pertence às famílias RNA helicases cujos membros estão envolvidos em quase todos os processos relacionados ao RNA, bem como ao controle do crescimento, proliferação e processos de diferenciação celular. Pacientes descritos com deleções na região 12q24.31, que contém o DHX37, apresentavam características sindrômicas e anormalidades genitais, como hipospadias ou micropenis. A presença de anormalidades genitais nestes pacientes reforça a hipótese de que o DHX37 está envolvido na etiologia dos DDSs 46,XY. Em conclusão, identificamos fortes evidências de um novo gene, o DHX37, envolvido no processo de desenvolvimento e manutenção das gônadas masculinas. Next-generation sequencing has been a powerful genetic diagnostic tool in the investigation of DSD patients. In the present study, we used the whole exome sequencing (WES) to identify potentially DSD-causing allelic variants. Patients with familial embryonic testicular regression syndrome (ETRS) of two non-related Brazilian families (Family 1 (F1)- two affected brothers and the unaffected parents; Family 2 (F2)- index patient, his unaffected sister and an affected nephew, the son of the index\'s sister) were initially selected. The patients had micropenis, absent or dysgenetic testes and no uterus. Genomic DNA was extracted from peripheral blood leucocytes and WES was performed using the Illumina-HiSeq-2500 platform. A novel heterozygous variant, c.923C > T, (p.Arg308Gln), in the DHX37 gene was identified in all affected members of the two Brazilian families and in F1\'s father. Sanger sequencing confirmed the presence of the variant in the all of the affected members and in F1\'s father and in F2\'s mother. The presence of a founder effect in these families was investigated using 29 markers located on chromosome 12q and was ruled out. Thirty-five additional patients with sporadic 46,XY DSD associated with abnormalities of gonadal development (6 with ETRS and 29 with gonadal dysgenesis) were screened for variants in this novel candidate gene using Sanger or NGS-based targeted sequencing panel of DSD candidate genes (Illumina-HiSeq-2500 platform). The heterozygous variant c.923C > T (p.Arg308Gln) and the homozygous variant c.451G>A, (p.Arg151Trp) in the DHX37 were identified in two sporadic SRTE patients. The heterozygous variants c.142C > T (p.Ala48Thr) and c.2020G > A (p.Arg674Trp) were identified in two sporadic 46,XY gonadal dysgenesis (GD) patients. According to the American College of Medical Genetics and Genomics guidelines (ACMG) the variants were classified as likely pathogenic (p.Arg308Gln, p.Arg674Trp), variant of uncertain clinical significance (p.Arg151Trp and) and benign variant (p.Ala48Thr); and all three of them were disease causing in more than three of the in silico prediction tools used. None of the novel DHX37 variants identified were found in the available population databases (EXAC, 1000GENOME, ESP6500, gnomAD and abraOM) in more than 0.01%. Segregation analysis of DHX37 allelic variants was performed in 5 families and resulted in the autosomal dominant with incomplete penetrance and recessive inheritance patterns. DNA methylation pattern of the DHX37 promotor region of F1 and F2\'s affected and unaffected members and gender-matched controls was analyzed using the Infinium Methylation EPIC BeadChip (Illumina). One hypomethylated CpG site in the promoter region of the DHX37 was only observed in F1\'s father compared with the affected siblings, F2\'s members and controls. Immunohistochemical analysis of DHX37 protein expression pattern was performed on testis samples of eight individuals with different chronological ages. This analysis revealed that in the testicular tissue of newborns, pubertal and adults, the DHX37 was expressed in different stages of germ cells maturation (present in spermatogonia, spermatocytes and spermatids and absent in spermatozoa). This finding suggests that DHX37 protein might have a role in the process of germ-cells development. The DHX37 (12q24.31) encodes a DEAH (AspGlu- Ala-His) protein of the RNA helicase families, whose members are involved in almost all RNA-related process such as the control of cell growth, proliferation and differentiation processes. Previously described patients with 12q 24.31 deletions, which contains DHX37, showed syndromic features and genital abnormalities as hypospadias or micropenis. This phenotype reinforces the hypothesis that DHX37 is involved in the etiology of disorders in male sex development. In conclusion, a novel gene, the DHX37, involved in the process of development and maintenance of male gonads was identified. Further studies will be necessary to clarify the exact role of the DHX37 in the gene regulatory networks controlling human sex development

O intuito geral deste trabalho foi o de identificar as angústias, dúvidas e ansiedades dos pacientes e seus pais, bem como da equipe médica que os assiste. O objetivo específico do trabalho foi fazer uma avaliação do que se passava nas entrelinhas do atendimento, para compreender o que o paciente e o familiar entendem ou não entendem sobre atendimento médico e vice-versa. Para isso, foram ouvidos sete médicos especialistas de cinco instituições do Sistema Único de Saúde (SUS), nove familiares e seis pacientes portadores de hiperplasia adrenal congênita (HAC) por deficiência de 21-hidroxilase. / The current scenario regarding treatment of patients with Anomalies of Sex Differentiation (ASD) brings the challenge to put face-to-face the medical knowledge and their patients and/or relatives acquaintance — distinct speeches — so that both these knowledges need to be heard. The general purpose of this work was to identify the anguishes, doubts, distress, and anxieties from patients and their parents, as well as from the medical team that attend them. The specific aim was to evaluate the scenario beyond the medical service in order to appreciate what patients and parents understand or do not understand within the medical attendance and vice-versa.
