Perfil clínico dos animais e funcionalidade do uso do aparelho de fisioterapia veterinária(modelos vetcar) na reabilitação de cães e gatos acometidos por dificuldades de locomoção /

Szabó / Resumo: Problemas como a incoordenação (ataxia), paresias e paralisias são freqüentes no atendimento clínico veterinário em ambulatórios de pequenos animais. Os sinais clínicos abrangem desde dores na coluna vertebral até a ausência de dor profunda. Várias afecções podem estar associadas a esses sintomas, a saber, as discopatias, mielopatias, fusão de vértebras, displasia coxofemoral e enfermidades causadas por bactérias, vírus e protozoários. O objetivo deste estudo retrospectivo foi avaliar o uso do Aparelho de Fisioterapia Veterinária (AFV) em animais com paresias e paralisias causadas por diversas afecções, para favorecer a higiene, mobilidade e reabilitação social em casos irreversíveis, além de auxiliar no restabelecimento fisioterápico em casos reversíveis, e ainda, traçar o perfil dos pacientes que fazem o uso deste aparelho. Para tanto, foram cadastrados 1203 cães e 47 gatos oriundos de várias cidades brasileiras, por meio de fichas de avaliação previamente elaboradas com questões que traçam o perfil clínico e físico dos animais. A espécie canina foi a mais acometida por paralisias, sendo os SRDs mais representativos. A lesão medular foi a causa principal de paralisias em cães e gatos; as paresias flácidas foram as mais observadas, sendo que a média das idades em que ocorreram paralisias decorrentes de diferentes enfermidades encontrou-se entre os 3 e 9 anos. Na associação entre as categorias de raça, idade e enfermidades, os cães da raça Teckel mostraram maior incidência de lesões de medula e de discos intervertebrais quando comparados às demais categorias de raça. A idade de acometimento dos animais diferiu entre as categorias de raça. Não houve diferença na associação entre raça e sexo / Abstract: Incoordination (ataxia), paresis and paralysis are frequent in the small animal veterinary clinics. The clinical signs include from back pain to the absence of deep pain reflex. Several pathologies can be associated with these symptoms, such as the discopathies, spinal cord lesions, vertebrae fusion, hip dysplasia and others caused by bacteria, virus and protozoary agents. The objective of this retrospective study was to evaluate the use of the Veterinary Physiotherapic Equipment (AFV) in animals with paresis, paraplegy and tetraplegy to improve hygiene, mobility and social rehabilitation in irreversible cases, and to aid the physiotherapic rehabilitation in reversible cases, establishing the profile of the patients that made use of the AFV. A total of 1203 dogs and 47 cats originated from various brazilian cities were recorded by evaluation data sheets previously elaborated with questions to delineate the animals' clinical and physical characteristics. The canine species was the most affected by paralysis, and the non-specific breed (mongrel) was the most representative. The lesions that mostly caused paralysis in dogs and cats were the spinal cord lesions and those of the intervertebral disks. The partial and flaccid paralysis were the most commonly observed paralysis, and the average of ages in which paralysis caused by different pathologies occurred was between 3 and 9 years. Recentemente, foi descrita também a associação com disautonomia, alteração da motilidade gastrointestinal e dor crônica generalizada. É mais frequente em mulheres e crianças, diminuindo a incidência com a idade. Há inquéritos populacionais relatados em crianças e adolescentes, contudo a frequência e as repercussões da hipermobilidade no adulto jovem são pouco conhecidas. Objetivo: Descrever a frequência de hipermobilidade articular em adultos jovens, por meio de um inquérito, estimando o seu impacto funcional e na qualidade de vida, por meio do questionário de saúde SF-36. Método: Foram investigados estudantes voluntários entre 18 a 25 anos de idade, provenientes de duas faculdades, de medicina e fisioterapia, que responderam ao questionário auto administrado de cinco itens sobre a hipermobilidade, atividade física e a versão brasileira do SF-36. Foi solicitada a avaliação de hipermobilidade por meio de autoexame instruído, pontuando-se os critérios de Beighton, que foram registrados pelo investigador. Estes critérios pontuam a extensão de joelhos e cotovelos maior que 180 graus, a extensão dos 5o dígitos maior que 90 graus, a aposição dos polegares na face flexora do antebraço e a extensão da coluna com as mãos espalmadas no solo. Considerou-se hipermobilidade generalizada, a pontuação maior ou igual a 4, e localizada menor que 4 pontos, sendo ambas os lados do corpo pontuadas independentemente. Resultados: Foram incluídos 388 sujeitos, sendo 299 mulheres (77,06%) e 89 homens (22,94%), mediana de idade 23 anos, com as seguintes características antropométricas: peso médio 64,5±15,7 Kg, altura média 1,66±0,09 m. It has been recognized in healthy people, in ballet dancers and in connective tissue genetic diseases. More recently, it has been associated to chronic widespread pain, dysautonomia, and gastrointestinal dysmotility. Approximately 25-50% of children younger than 10 years have some degree of hypermobility, it is more common in girls decreasing incidence with age. There are population-based studies in children and adolescents, but the reports of frequency and impact of hypermobility in youngsters are scarce. Objective: Describe the frequency of joint hypermobility in youngsters through a survey, to estimate its functional impact and quality of life by SF-36 health questionnaire. Methods: Volunteers from 18 to 25 years were recruited from a local medical and a physical therapy schools. They were requested to fill out a 5-parts hypermobility questionnaire, physical activity practice and the Brazilian version of the SF-36 health questionnaire. They performed an instructed self-exam to score Beighton criteria for hypermobility. Beighton criteria scores the extension beyond range of 180 degrees in the knees and elbows, extension of 5o fingers beyond 90 degrees, extension of thumbs to touch the forearms, spine flexion to touch hands opened to the ground. All data were recorded by one assessor. Results: Of the 388 subjects included, 299 were women (77.06%) and 89 men (22.94%), with a median age 23 years, mean weight 64.5 ± 15.7 Kg, mean height 1.66 ± 0.09 m. The mean body mass index (BMI) was 23.2 ± 4.5. Generalized joint hypermobility was observed in 104 (26.8%). Localized joint hypermobility was observed in 135 (34.79%) subjects. Portanto, é importante determinar a repetibilidade dessas variáveis. O objetivo deste trabalho foi avaliar a repetibilidade das medidas dos parâmetros espaço-temporais da marcha de crianças com Paralisia Cerebral (PC) e crianças saudáveis e investigar o número mais adequado de tentativas do ciclo da marcha que devem ser registrados para que se obtenham dados acurados. A amostra foi composta por 19 crianças divididas em dois grupos. Um grupo de 9 crianças com PC do tipo hemiplegia espástica e um grupo com 8 crianças saudáveis que foram avaliadas pelo mesmo pesquisador em duas sessões diferentes, com intervalo de 1 semana entre as sessões. As crianças caminharam em velocidade auto-selecionada e foram utilizadas 3, 4, 5 e 6 tentativas, nas sessões 1 e 2, para representar a média. Para registro dos parâmetros espaço-temporais foi utilizado o sistema Peak Motus, com duas câmeras de vídeo SVHS com taxa de aquisição de 60 Hz. Coeficiente de correlação intraclasse (ICC) e limites de concordância de Bland-Altman foram escolhidos para análise dos resultados. Os resultados demonstraram que as crianças com PC apresentaram menor velocidade e comprimento do ciclo do que as crianças saudáveis para as sessões 1 e 2. Para todos os parâmetros espaçotemporais, os dois grupos apresentaram valores de ICC excelentes a moderados, ou seja maior do que 0,75. Para ambos os grupos, a medida de 3 tentativas do ciclo da marcha representando a média mostrou repetibilidade similar a de quando 6 tentativas foram utilizadas. Pode-se concluir que a utilização de 3 tentativas representando a média foi suficiente para reproduzir o padrão de locomoção das crianças e gerar dados acurados. _________________________________________________________________________________________ ABSTRACT / The spatio-temporal parameters of gait are frequently utilized to evaluate and identify possible disorders in the gait of children. Therefore, it is important to determine the repeatability of these variables. The objective of this work was to evaluate the repeatability of the measurements of the spatio-temporal parameters of the gait of children with Cerebral Palsy (CP) and of healthy children and investigate the most adequate number of trials to measure the gait cycle to be registered for obtaining accurate data. The sample was composed of 19 children divided into two groups. A group of 9 children with CP of the spastic hemiplegic type and a group of 8 healthy children were evaluated by the same researcher in two different sessions, with an interval of 1 week between sessions. The children walked at their own chosen velocity and 3, 4, 5 and 6 trials were employed, in sessions 1 and 2 to represent the average. To register the spatio-temporal parameters the Peak Motus system was used with two SVHS video cameras with an acquisition rate of 60 Hz. Intra-class Correlation Coefficient (ICC) and Bland and Altman limits of agreement were chosen to analyse the results. The results demonstrated that the children with CP showed less velocity and stride length than the healthy children for sessions 1 and 2. For all the spatio-temporal parameters the two groups presented excellent to moderate ICC values, that is to say, greater than 0,75. For both groups, the measure of 3 trials of the gait cycle representing the average demonstrated repeatability similar to that when 6 trials were utilized. One can conclude that the utilization of 3 trials representing the average was sufficient to reproduce the pattern of locomotion of the children and generate accurate data.

Orientador: Vitório Rodrigo / Resumo: Dentre os comprometimentos motores associados à doença de Parkinson (DP), os distúrbios do andar são os mais incapacitantes aos pacientes. Disfunções cognitivas e do córtex cerebral, presentes na DP, influenciam nos comprometimentos do andar. Como o tratamento tradicional para DP (uso regular de medicamentos) apresenta pouco benefício para o controle do andar, faz-se necessário o desenvolvimento de intervenções alternativas. O exercício aeróbio (EA) e a estimulação transcraniana por corrente contínua (ETCC) têm sido apontados como de grande potencial terapêutico na DP, incluindo benefícios para o andar e a cognição. Além disso, especula-se que a combinação das duas técnicas é capaz de potencializar os efeitos positivos. Entretanto novos estudos são necessários para confirmar tal evidência. . Assim, o objetivo do presente estudo foi comparar os efeitos de uma sessão combinada de EA e ETCC anódica do córtex pré-frontal com os de uma sessão de EA na atividade cortical, cognição e no andar usual e no custo da tarefa dupla no andar de pacientes com DP. Cognitive and cerebral cortex dysfunctions, present in PD, influence gait impairments. As the traditional treatment for PD (regular use of medications) has limited benefit for gait, it is necessary to develop alternative interventions. Aerobic exercise (AE) and transcranial direct current stimulation (tDCS) have been identified with great therapeutic potential in PD, including advantages for gait and cognition. However, the effects of combining such interventions remain poorly understood. The aim of the present study was to compare the effects of a combined session of AE and anodic tDCS of the prefrontal cortex with those of an AE session on cortical activity, cognition, usual gait, and the cost of the dual task on the gait of patients with PD. Lamônica / Resumo: Introdução: Pessoas com Paralisia Cerebral (PC) apresentam prognóstico e comorbidades diversificadas e suas manifestações estão relacionadas com limitações funcionais, tanto motoras globais como de controle neuromotor das funções orais, desencadeando dificuldades na comunicação, deglutição e saúde bucal. Objetivo: Este estudo teve por objetivo relacionar o risco de disfagia orofaríngea (DO) com o estado de saúde bucal na pessoa com Paralisia Cerebral. Método: Foram selecionados todos os prontuários com diagnóstico neurológico conforme CID 10, dentro da classificação de Paralisia Cerebral de distintos subtipos, do período de janeiro de 2011 a junho de 2017, do Centro de Assistência Odontológica à Pessoa com Deficiência (CAOE) da Universidade Estadual Paulista - Unesp - Campus de Araçatuba, perfazendo 1868 indivíduos. Foram incluídos nesse estudo 345 prontuários, 194 do gênero masculino e 151 do gênero feminino, faixa etária de 1 a 60 anos, com média de 13,9 anos, e função motora grossa (GMFCS) que variou de I a V. Elaborado e aplicado instrumento de rastreio para disfagia orofaríngea na PC e para a classificação da saúde bucal nessa população. Purpose: This study aimed to relate the risk of oropharyngeal dysphagia (OD) with oral health status in people with CP. Methods: All neurological records according within the classification of CP of different subtypes, from January 2011 to June 2017, were selected from the Center for the Assistance to the Individuals with Disabilities (CAOE) of the Paulista State University- Unesp - Campus of Araçatuba, comprising 1868 individuals. The study included 345 medical records, 194 males and 151 females, ranging from 1 to 60 years old, mean age of 13.9 years, and gross motor function (GMFCS), ranging from I to V. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Chronic treatment of psychotic disorders is associated with adverse effects that affect motor function. Movement disorders include Parkinsonism, akathisia, dystonia and tardive dyskinesia. Magnesium (Mg) is an essential mineral for various physiological functions in the body and its supplementation is used in several diseases. The purpose of this study was investigate the effect of Mg supplementation on the prevention and reversal of orofacial dyskinesia (OD), designated as experiment 1 and 2, respectively, as well as the effect on oxidative stress parameters. In both experiments, male Wistar adult rats were used. In experiment 1, rats were randomly divided into two groups both supplemented with oral solution of magnesium aspartate (40 mg/Kg/mL) or deionized water. After 28 days of supplementation, half of each experimental group was treated with a reserpine solution (0.7 mg/kg/ml, sc) (Mg + R and R groups) or vehicle (C and Mg groups) for 3 days (every other day). One day (24 hours) after the last administration of R/vehicle, all animals were subjected to OD and catalepsy time behavioral assessments. In the experiment 2 rats were randomly divided into two groups and treated with a solution of reserpine (0.7 mg / kg / ml, sc) (R groups) or vehicle (Group C) for 3 days (every other day). Twenty-four hours after the last administration of R/vehicle, the development of OD was quantified. One-half of each experimental group was supplemented immediately once a day (by gavage) with magnesium aspartate (40 mg / kg / ml) (Mg, and groups R + Mg) or deionized water (groups C and R). The OD was measured during subsequent days (every 48 hours). Mg supplementation was maintained throughout the time of behavioral assessment (10 consecutive days). After behavioral evaluations, all animals were euthanized by exsanguination. Blood was drawn for analysis of erythrocytes lipid peroxidation (LP). The brains were immediately dissected for separating cortex, striatum and substantia nigra for determining reactive species (RS) and protein carbonyl (PC). The results showed that Mg supplementation before reserpine administration was sufficient to prevent movement disorder observed in the vacuous chewing movement frequency (VCM) and catalepsy time and also was able to prevent the generation of RS and PC, in both the cortex and the substantia nigra regions, also preventing LP in the erythrocytes. Supplementation of Mg after treatment with reserpine was able to minimize the frequency of VCM and catalepsy time, reduce the generation of RS and PC levels in both cortex and the corpus striatum regions and also reversing the increasing the level of LP in the erythrocytes. Our results underscore the importance of including alternative therapies through supplementation of essential natural substances, like magnesium, which can prevent or ameliorate motor disturbances, often related to chronic treatment of psychotic disorders that so far have no effective treatment. / O tratamento crônico de distúrbios psicóticos está associado a efeitos adversos que afetam a função motora. Os distúrbios do movimento incluem o Parkinsonismo, acatisia, distonias e também discinesia tardia. O magnésio (Mg) é um mineral essencial para diversas funções fisiológicas no organismo e sua suplementação tem sido empregada em diversas doenças. O objetivo deste estudo foi investigar o efeito da suplementação de Mg sobre a prevenção e a reversão da discinesia orofacial (DO), designados como experimento 1 e 2, respectivamente, bem como o efeito sobre parâmetros de estresse oxidativo. Em ambos os experimentos foram utilizados ratos machos Wistar adultos. No experimento 1, os ratos foram divididos aleatoriamente em dois grupos, ambos suplementados oralmente com solução de aspartato de magnésio (40 mg/Kg/mL) ou água deionizada. Depois de 28 dias de suplementação, metade de cada grupo experimental foi tratada com uma solução de reserpina (0,7 mg/kg/mL; sc) (R e Mg + R grupos) ou veículo (C e Mg grupos) durante 3 dias (em dias alternados). Um dia (24 horas) após a última administração de R / veículo, todos os animais foram submetidos a avaliações comportamentais de DO, através da quantificação de movimentos de mascar no vazio (MMV), e tempo de catalepsia. No experimento 2 os ratos foram divididos aleatoriamente em dois grupos e tratados com solução de reserpina (0,7 mg/kg/mL; sc) (grupos R) ou veículo (grupo C) durante 3 dias (em dias alternados). Vinte e quatro horas após a última administração de R / veículo, o desenvolvimento de DO foi quantificada. Metade de cada grupo experimental foi suplementado imediatamente, uma vez por dia (por gavagem) com aspartato de magnésio (40 mg/kg/mL) (grupos Mg e R + Mg) ou água desionizada (grupos C e R). A DO foi quantificada durante os dias subsequentes (cada 48h). A suplementação de Mg foi mantida durante todo o tempo de avaliação comportamental (10 dias consecutivos). Após as avaliações comportamentais todos os animais foram eutanasiados por exsangüinação. O sangue foi retirado para análise dos níveis de lipoperoxidação (LP) eritrocitária. Os cérebros foram imediatamente dissecados para a separação da região do córtex, corpo estriado e substantia nigra para a determinação de espécies reativas (ER) e de proteína carbonil (PC). Os resultados mostraram que a suplementação de Mg antes da administração da reserpina foi suficiente para prevenir os distúrbios do movimento, observados pela frequência de MMV e tempo de catalepsia; bem como foi capaz de evitar a geração de ER e PC, tanto na região do córtex como na substantia nigra, também impedindo a LP nos eritrócitos. A suplementação de Mg após o tratamento com reserpina foi capaz de minimizar a frequência de MMV e o tempo de catalepsia, reduzir a geração de ER e os níveis de PC nas regiões do córtex e do corpo estriado, revertendo também o aumento do nível de LP nos eritrócitos. Nossos resultados ressaltam a importância da inclusão de terapias alternativas através da suplementação de substancias naturais essenciais, como o magnésio, as quais podem prevenir ou atenuar distúrbios motores, frequentemente relacionados ao tratamento crônico de distúrbios psicóticos que até o momento não dispõe de um tratamento eficaz.

Correlação entre o sinal mecanomiográfico e a escala modificada de Ashworth durante avaliação clínica da espasticidade / Correlation between the MMG signal and the modified Ashworth scale for clinical assessment of spasticity

A espasticidade é uma desordem comum em pessoas que apresentam lesão no neurônio motor superior. O acometimento pode ocorrer em diferentes níveis. A Escala Modificada de Ashworth (EMA) é o instrumento de avaliação mais utilizado para medir os níveis de acometimento, entretanto apresenta subjetividade. Já a mecanomiografia (MMG) trata-se de uma técnica objetiva, que quantifica as vibrações musculares durante os eventos de contração e alongamento e, por isso, pode dimensionar com precisão o nível de espasticidade. Diante disso, objetivou-se investigar a correlação entre os níveis de espasticidade determinados pela EMA com os sinais mecanomiográficos em músculos espásticos e não espásticos. Avaliaram-se 34 membros de 22 voluntários, de ambos os sexos, com idade média de 39,91±13,77 anos. O protocolo experimental consistiu na realização de avaliação pela EMA dos grupos musculares flexores e extensores de joelho e/ou cotovelo, onde um grupo era o agonista e o outro antagonista. Simultaneamente, captavam-se os sinais de MMG. Utilizou-se um equipamento de MMG customizado para registrar e gravar os sinais, configurados em um programa da plataforma LabView®. No programa computacional MatLab®, processaram-se os sinais da MMG no domínio temporal (mediana da energia) e espectral (mediana da frequência) dos eixos de movimento X (transversal), Y (longitudinal) e Z (perpendicular). Para delimitação da banda passante, empregou-se um filtro Butterworth de 3a ordem atuando na faixa de 5 a 50 Hz. Aplicaram-se testes estatísticos como coeficiente de correlação de Spearman, teste de Kruskall-Wallis e teste de correlação linear. Como resultados, no domínio temporal, o teste de Kruskal- Wallis mostrou diferenças na mediana da energia (MMG ME) entre cada grupo de EMA. O teste de correlação linear mostrou alta correlação entre a EMA e a MMGME, tanto no grupo muscular agonista quanto no antagonista. Assim, no domínio do tempo, a maior correlação linear ocorreu entre a EMA e a MMGME do eixo Z do grupo muscular agonista (R2 = 0,9557) e a menor correlação no eixo X, do antagonista (R 2 = 0,8862). O teste de correlação de Spearman também confirmou alta correlação em todos os eixos, nesta análise. O processamento no domínio espectral mostrou que houve aumento na mediana da frequência (MMGMF) nos maiores graus de EMA. Porém, os coeficientes de correlação linear determinados foram moderados e baixos. O maior coeficiente de correlação linear entre a EMA e a MMGMF ocorreu no eixo Z, do grupo muscular agonista (R2 = 0,4883) e o menor valor no eixo Y, do grupo antagonista (R 2 = 0,1657). No teste de Spearman, a maior correlação ocorreu entre o eixo Y do grupo agonista (0,6951; p < 0,001) e o menor valor no eixo X do grupo antagonista (0,3592; p < 0,001). Por meio dos dados obtidos, concluiu-se que entre a MMGME e a EMA houve correlação significativamente elevada em ambos os grupos musculares. Entre a MMGMF e a EMA também houve correlação significativa, porém moderada no grupo agonista, e fraca no grupo antagonista. Desta forma, dentre os descritores estudados, a MMGME mostrou ser a mais adequada para correlacionar-se com o grau de espasticidade definido pela EMA. / Spasticity is a common disorder in people who have upper motor neuron injury. The involvement may occur at different levels. The Modified Ashworth Scale (MAS) is the most used method to measure involvement levels. But it corresponds to a subjective evaluation. Mechanomyography (MMG) is an objective technique that quantifies the muscle vibration during the contraction and stretching events. So, it may assess the level of spasticity accurately. This study aimed to investigate the correlation between spasticity levels determined by MAS with MMG signal in spastic and not spastic muscles. In the experimental protocol, we evaluated 34 members of 22 volunteers, of both genders, with a mean age of 39.91 ± 13.77 years. We evaluated the levels of spasticity by MAS in flexor and extensor muscle groups of the knee and/or elbow, where one muscle group was the agonist and one antagonist. Simultaneously the assessment by the MAS, caught up the MMG signals. We used a custom MMG equipment to register and record the signals, configured in LabView platform. Using the MatLab computer program, it was processed the MMG signals in the time domain (median energy) and spectral domain (median frequency) for the three motion axes: X (transversal), Y (longitudinal) and Z (perpendicular). For bandwidth delimitation, we used a 3rd order Butterworth filter, acting in the range of 5-50 Hz. Statistical tests as Spearman's correlation coefficient, Kruskal-Wallis test and linear correlation test were applied. As results in the time domain, the Kruskal-Wallis test showed differences in median energy (MMGME) between MAS groups. The linear correlation test showed high linear correlation between MAS and MMGME for the agonist muscle as well as for the antagonist group. The largest linear correlation occurred between the MAS and MMG ME for the Z axis of the agonist muscle group (R2 = 0.9557) and the lowest correlation occurred in the X axis, for the antagonist muscle group (R2 = 0.8862). The Spearman correlation test also confirmed high correlation for all axes in the time domain analysis. In the spectral domain, the analysis showed an increase in the median frequency (MMGMF) in MAS’ greater levels. The highest correlation coefficient between MAS and MMGMF signal occurred in the Z axis for the agonist muscle group (R2 = 0.4883), and the lowest value occurred on the Y axis for the antagonist group (R2 = 0.1657). By means of the Spearman correlation test, the highest correlation occurred between the Y axis of the agonist group (0.6951; p <0.001) and the lowest value on the X axis of the antagonist group (0.3592; p <0.001). We conclude that there was a significantly high correlation between the MMGME and MAS in both muscle groups. Also between MMG and MAS occurred a significant correlation, however moderate for the agonist group, and low for the antagonist group. So, the MMGME proved to be more an appropriate descriptor to correlate with the degree of spasticity defined by the MAS.
