[pt] ANÁLISE MULTIELEMENTAR COMO FERRAMENTA PARA A DIFERENCIAÇÃO ENTRE MARCAS DE CIGARROS COMERCIALIZADAS NO BRASIL / [en] MULTI-ELEMENT ANALYSIS AS A TOOL FOR DIFFERENTIATING BETWEEN CIGARETTE BRANDS SOLD IN BRAZIL

[pt] A presença de metais-traço nos cigarros oferece uma fonte de informação potencial do ponto de vista forense e criminológico, na determinação e classificação de marcas de tabaco, com o intuito de diferenciar as marcas legalizadas das contrabandeadas. Por isso, neste trabalho, almejou-se desenvolver e validar métodos para determinação metais e semimetais presentes nos cigarros, com o intuito de avaliar a viabilidade de discriminação entre as diferentes marcas, analisando as amostras após o processo de calcinação e dissolução ácida. Estes métodos englobam as análises químicas por técnicas espectrométricas (CV-AAS, ICP-OES e DRC-ICP-MS) e posterior interpretação de dados por ACP (Análise dos Componentes Principais) e AAH (Análise de Agrupamento Hierárquico). As concentrações de metais e semimetais (As, Ba, B, Cd, Co, Cu, Fe, Mn, Hg, Ni, Sr e Zn) foram estudadas para dez diferentes marcas de cigarros, obtidas em tabacarias e lojas locais, representando as marcas mais consumidas no país. A utilização da ACP indicou a presença de quatro componentes principais que, em conjunto, explicavam mais de 80 por cento da variabilidade dos dados amostrais. Esses resultados comprovaram a existência de diferenças significativas entre as marcas de tabaco em seus conteúdos elementares, possibilitando a diferenciação entre marcas ilegais e marcas legalizadas e o agrupamento das marcas legalizadas de acordo com as empresas de tabaco as quais pertencem, sendo corroborado pela AAH, feita pelo método da distância média de ligação (UPGMA). Os teores de umidade, cinzas totais e pH de cada marca também foram determinados, no entanto não contribuíram efetivamente para a diferenciação entre as marcas estudadas. A AAH foi utilizada para a interpretação de possíveis similaridades entre os elementos, e em conjunto com o teste de correlação linear de Pearson, indicou similaridades entre os elementos Ba e Sr . Agrupamentos entre Fe e Zn com o Sr e Co com o Mn também foram detectados. / [en] The presence of trace metals in cigarettes offers potential source of information from a forensic and criminological point of view, which deals with the determination and classification of tobacco brands, with the aim of differentiating legal brands from smuggled brands. Therefore, this work aimed to develop and validate methods for determining the metallic content present in cigarettes, in order to assess the feasibility of discrimination between different brands, analyzing cigarettes after the process of calcination and acid digestion. These methods include chemical analysis by spectrometric techniques (CV-AAS, ICP-OES and DRC-ICPMS) and subsequent data interpretation by PCA (Principal Component Analysis) and HCA (Hierarchical Cluster Analysis). The concentration of metals and semimetals (As, Ba, B, Cd, Co, Cu, Fe, Mn, Hg Ni, Sr and Zn) were studied for ten different brands of cigarettes, obtained in tobacconists and local stores, representing the brands most consumed in the country. The analysis of PCA indicated the presence of four principal components that together explained more than 80 percent of the variability of the sample data. These results proved the existence of significant differences between tobacco brands in their elemental content, allowing the differentiation between illegal brands and legalized brands and the grouping of legal brands according to the tobacco companies to which they belong, being corroborated by the HCA performed by the average link distance method (UPGMA). The moisture content, total ash and pH of each brand were also determined, however they did not contribute effectively to the differentiation between the brands studied. The HCA used for the interpretation of possible similarities between the elements, and interpreted together with Pearson s linear correlation test indicated similarities between the elements Ba, Sr, in addition to indicating the proximity between the Mn and the Cd. Clusters between Fe and Zn with Sr and Co with Mn were also detected.

Healthcare Providers’ Screening and Counseling for Tobacco Use Among Youth in the United States: Prevalence, Practices, and Providers’ Perspectives

Youth tobacco use is a public health concern in the United States (U.S.) with about 2.8 million middle and high school youth reporting current tobacco use in 2023. Healthcare providers (HCPs) can play a crucial role in addressing youth tobacco use through screening and counseling, yet rates of these interventions are suboptimal. Tennessee has a high prevalence of youth tobacco use and not much is known about these interventions in Northeast Tennessee. This study has several objectives: 1) conduct a scoping review of available evidence on screening and counseling for youth tobacco use and identify related gaps; 2) assess youth self-reported rates of HCPs’ screening and counseling for tobacco use among U.S. youth, and identify associated factors; 3) explore HCPs’ experiences and identify barriers and facilitators in implementing screening and counseling interventions for youth tobacco use in Northeast Tennessee; and 4) provide recommendations for policy and practice based on identified gaps. This multi-method study employed 1) a scoping review, adhering to the Johanna Briggs Institute methodology; 2) a cross-sectional analysis of data from the 2022 National Youth Tobacco Survey using logistic regression models; 3) a qualitative study using a blended-inductive coding of semi-structured interviews and thematic analysis, and 4) a research brief highlighting the policy and practice implications of study findings. The scoping review analyzed 22 articles and revealed disparities in HCPs’ tobacco screening and counseling interventions. The quantitative study showed tobacco screening and counseling rates of 43.7% and 3 45.6%, respectively. Younger youth (9–12 years) and non-Hispanic (NH) Black, Hispanic, and NH Asian youth were less likely to receive screening and counseling compared to NH White youth. In-depth interviews with 12 primary care providers (PCPs) revealed varying levels of confidence in screening and counseling for tobacco use. PCPs primarily conduct formal screening for older youth (12–18 years), and barriers such as time constraints, lack of confidentiality, and resistance from youth were frequently mentioned. Efforts should be made to improve screening and counseling interventions for youth tobacco use by implementing policies to reduce barriers and increase HCPs’ awareness regarding early intervention for all age groups, races, and ethnicities.

This thesis is aimed at tabacism problems encountered by 9th grade elementary school pupils in the area of Most (Czech Republic). Comparison of gender aspect is accented. The primary target was to figure out the age of first experience with tobacco smoking and to investigate the subjective reasons leading to this act. The other domain of investigation was the attitude of adolescents to tobacco attractiveness and their tolerance to smoking of their future life partners. The thesis also compares my investigation results to studies related to the influence of smoking parents on their children's early experience with tobacco, and related to the manner in which are the elementary school children informed about tabacism issues. Some results of investigation are compared with existing research in the comments. The following facts were discovered - Adolescent girls smoke more than boys, and smoking parents have substantial influence on their children. The majority of children with smoking parents has more experience with tabacism than the children of non-smokers. The primary prevention in schools is performed as part of health education lessons, or similar subjects' lessons. 5 Abstract: This thesis addresses to the cigarette demand and excise tax on cigarettes in the Czech Republic The aim is to describe behavior of smokers and especially their reaction to the changes in price of cigarettes. Data for household spending are used for this purpose. First part of this thesis is dedicated to description of the cigarette market and socio-demographic characteristics of smokers. Second part concerns with question whether smokers are rational or myopic and how price elastic is their behavior. Then the rest of the thesis describes excise tax payment mechanism and analyzes excise duty incomes. The thesis finds that cigarettes are more affordable these days than in the beginning of 90's. Czech smokers do not fully realize future impact of their behavior and they seem to be very inelastic witch respect to price. The price elasticity was estimated on value -0.2. Given low price elassticitty and increasing purchasing power, the current cigarette excise tax is considered to be too low. In three piles were used wood shavings as bulking agents and other two piles, gardening pruning. During the process, were performed physical and chemical analysis (temperature, humidity, pH, C/N ratio, ash content), biological (seed germination index) and spectroscopic (Ultraviolet-Visible and Infrared). Furthermore, they were verified the granulometry of the waste (tobacco, gardening pruning and wood shavings) to see if the particle sizes thereof were within the recommended range for composting processes. It was observed that the granulometry of the residues were adequate for achieving composting. All piles had temperatures in the thermophilic stage considered ideal for the elimination of pathogens. Phytotoxicity tests and analyzes of the C/N ratio showed that the compounds were matured at 180 days of the process. The increase in the ash content of the compounds over the course of the composting time indicated the mineralization of organic matter. The results of the spectroscopic analysis indicated that the composting process has occurred properly for all piles, because showed the degradation of the simple structures and the formation of complex groups (humic substances). The ratios E2/E4 suggested that initially occurred lignin degradation in the piles studied and that, especially in the curing phase, occurred the formation of non-condensed aromatic structures linked to oxygen function. The results of the ratios E4/E6 indicated that the piles had final values of these ratios similar to those observed for humic acids of the soil. The infrared results showed absorptions in the region 1647-1652 cm-1, this being a characteristic of humic compost. The set of results indicates that the composting using the tobacco of the contraband cigarettes and industrial sewage sludge, in the conditions developed in this work, is an environmentally suitable alternative for the treatment/reuse as much tobacco as sewage sludge. / O presente trabalho teve como objetivo estudar a compostagem de tabaco de cigarros contrabandeados misturado a lodo de estação de tratamento de esgoto industrial em cinco leiras revolvidas. Em três leiras utilizaram-se cepilhos de madeira como agentes de volume e em outras duas, poda de jardinagem. Durante o processo, foram realizadas análises de físico-químicas (temperatura, umidade, pH, razão C/N, teor de cinzas), biológicas (índice de germinação de sementes) e espectroscópicas (Ultravioleta-Visível e Infravermelho). Além disso, verificaram-se as granulometrias dos resíduos (tabaco, poda de jardinagem e cepilhos de madeira) para saber se os tamanhos das partículas dos mesmos estavam dentro da faixa recomendada para processos de compostagem. Observou-se que as granulometrias dos resíduos estavam adequadas para a realização da compostagem. Todas as leiras apresentaram temperaturas na fase termofílica consideradas ideais para a eliminação de patógenos. Os testes de fitotoxicidade e as análises da razão C/N mostraram que os compostos estavam maturados nos 180 dias do processo. Ocorreram aumentos nos teores de cinzas dos compostos com o decorrer do tempo de compostagem, o que indicou a mineralização da matéria orgânica. Os resultados das análises espectroscópicas indicaram que o processo de compostagem estava ocorrendo de forma adequada para todas as leiras, pois demonstram a degradação de estruturas simples e a formação de grupos complexos (substâncias húmicas). As razões E2/E4 sugeriram que inicialmente ocorreu a degradação de lignina nas leiras estudadas e que, principalmente na fase de cura, ocorreu a formação de estruturas aromáticas não condensadas ligadas a funções oxigenadas. Os resultados referentes às razões E4/E6 indicaram que as leiras apresentaram valores finais destas razões semelhantes aos valores observados para os ácidos húmicos do solo. Os resultados de infravermelho demonstraram absorções na região de 1647 a 1652 cm-1, sendo isto uma característica de compostos humificados. O conjunto de resultados obtidos indica que a compostagem utilizando tabaco de cigarros contrabandeados e lodo de estação de tratamento de esgoto industrial, nas condições desenvolvidas neste trabalho, constitui-se como uma alternativa ambientalmente adequada para o tratamento/reaproveitamento tanto do tabaco quanto do lodo.

Perspectives of Young Adults Toward Tobacco Use

Conditions related to tobacco use constitute the single most preventable cause of death in the United States. Approximately 443,000 U.S. adults die each year from smoking-related illnesses. During young adulthood, social and behavioral changes occur; experimentation with tobacco products such as cigarettes is common and may lead to a habit of smoking. A gap was identified in the literature on the perceived impact of family communication on young adults' decisions regarding smoking. The purpose of this descriptive phenomenological study was to address this gap by gathering information on the perspectives of young adults toward tobacco use. Fifteen young adults aged 18 to 26 from the northeastern region of New Jersey who were currently engaged in the use of tobacco products participated in open-ended interviews. Research questions were designed to investigate young adults' views and perceptions regarding cigarette use and to explore information regarding how smoking-related communication received from family members influenced young adults' decision to smoke. The theory of planned behavior and social learning theory provided the theoretical underpinnings and consistent themes by young adults from the study. Van Manen's data analysis strategy demonstrated thematic reports from young adults that behaviors, habits, attitudes, communications, including verbal and nonverbal cues and practices are learned from their parental figures in the home environment. Recommendations for future research include exploring young adults from other geographical locations regarding their perspectives toward tobacco use. This study may promote positive social change for the public and health practitioners by providing insight on family interactions regarding smoking behaviors for young adults..

Re-testing the link between youth receptivity to tobacco promotion and their susceptibility to smoke

The Index of Receptivity to Tobacco Industry Promotion (IRTIP) is a model that is used by hundreds of articles. The causal claim based on findings from this model is even more pervasive, and has resulted in much of the modern post 1998 tobacco legislation that is still enforced. This thesis tested the link between adolescent receptivity to tobacco industry promotion and susceptibility to smoking. Pierce et al. (1998) reported that they had found a positive and causal association between receptivity and susceptibility by using IRTIP. They claimed that receptivity to tobacco industry promotion was the only significant causal factor affecting adolescent susceptibility to smoking. Exposure to peer and parental smoking was not found to be a significant effect. A review of the literature found that many sections of IRTIP differ from accepted marketing theory on how cigarette advertising and promotions affect adolescent adoption of cigarette smoking. The proxy measures used in IRTIP were shown to diverge from those previously used for measuring the constructs of Attention, Intention, Desire and Action (AIDA) in marketing communications. IRTIP also differs from previous theory by including measures that attempt to quantify the effect of tobacco premiums into a model that was designed to measure the effects of advertising.

Facteurs de risque de cancer du poumon chez la femme

Le cancer du poumon chez la femme était une maladie rare au début du XXème siècle. Son incidence a fortement augmenté durant ces vingt dernières années dans les pays développés et particulièrement en France du fait de l'augmentation de la consommation de cigarettes dans la population féminine. L'étude des risques de cancer du poumon liés à la consommation de cigarettes des femmes a soulevé la question d'une éventuelle plus grande susceptibilité des femmes vis-à-vis du tabac dans la survenue du cancer du poumon par rapport aux hommes. Les résultats des études épidémiologiques sur le sujet sont divergents et l'hypothèse selon laquelle les hormones joueraient un rôle dans cette plus grande susceptibilité des femmes a été proposée pour expliquer ces différences. Objectifs : Les objectifs de cette thèse sont de donner des estimations récentes du risque de cancer du poumon chez la femme associé à la consommation de cigarettes en France, de comparer le risque de cancer du poumon chez les femmes et chez les hommes et d'étudier le rôle des hormones dans le risque de cancer du poumon.Matériel et méthodes : L'étude de la consommation de cigarettes chez les femmes et la comparaison des risques de cancer du poumon associé à cette consommation a été réalisée à partir des données de l'étude ICARE. Les cas de cancer du poumon ont été identifiés dans 10 départements incluant un registre général de cancer. Au total, 2276 cas de cancer du poumon chez les hommes et 2780 témoins ont été inclus ainsi que 650 cas de cancers du poumon féminins et 775 témoins. Les témoins étaient issus de la population générale, avec une distribution âge et sexe conforme à celle des cas et une répartition par statut socioéconomique conforme à celle des départements dont sont issus les cas. L'analyse du rôle des hormones et des facteurs reproductifs sur le risque du cancer du poumon a été menée par une analyse poolée, réunissant 9 études internationales. Cette étude a été réalisée dans le cadre du consortium ILCCO. Au total, 4547 cas et 4176 témoins ont été inclus dans l'analyse.Résultats : Notre étude a montré que le risque de cancer du poumon associé à la consommation de cigarettes est 8 fois plus élevé chez les femmes fumeuses par rapport aux non fumeuses en France. La quantité moyenne fumée, la durée totale de consommation et le délai depuis l'arrêt sont les 3 principales caractéristiques de consommation qui modulent le risque de cancer du poumon. La fraction de risque attribuable à la consommation de cigarettes est estimée à 55% [47 %-63 %]. La comparaison du risque de cancer du poumon entre les hommes et les femmes est très influencée par la présence des non fumeurs. . Bien que nous n'ayons pas trouvé de différence de risque de cancer du poumon entre les hommes et les femmes fumeurs lorsque l'on considère tous les types histologiques réunis, nous avons constaté que les femmes relativement aux hommes étaient plus à risque de cancer à petites cellules et de cancer épidermoïde, qui sont les types histologiques les plus liés à la consommation de cigarettes. Concernant l'étude sur le rôle des hormones dans la survenue de cancer du poumon, nos résultats montrent que plus l'âge à la ménopause est tardif et plus la durée des cycles menstruels est longue, moins le risque de cancer du poumon est important. Nous avons également montré que l'ovariectomie était associée positivement avec le risque de cancer du poumon. Conclusion : Nos résultats semblent indiquer que les femmes fumeuses sont peut-être plus à risque de développer un carcinome à petites cellules ou un cancer épidermoïde par rapport aux hommes fumeurs. L'étude sur les facteurs hormonaux a montré une association négative avec l'exposition prolongée aux hormones, qui n'est pas très cohérente avec un risque de cancer du poumon associé au tabac plus important chez les femmes que chez les hommes.

Porovnání spotřební daně z tabákových výrobků v ČR a státech EU / Comparison of excise duty on tobacco products in the Czech Republic and the EU

This thesis is concerned with a comparison of excise duty on tobacco products in the Czech Republic and the member countries of the European Union. Its aim is to find groups of countries with similar indicators, to explore the similarities between these countries and to try to identify the possible effects that may cause differences. The thesis is divided into two parts. The theoretical part is focused on the characteristics of excise duties and primarily on excise duty on tobacco products. It deals with European Commission directives and minimum excise rates. The practical part deals with selected indicators related to excise duty on tobacco products. These indicators are excise duty on tobacco products as percentage of GDP and percentage of total taxation, specific excise per 1 000 cigarettes as percentage of WAP and excise yield in EUR per 1 000 cigarettes of the WAP. The last indicator is cigarettes consumption per capita, which is not included in the analysis. Data is processed by cluster analysis for the years 2003 and 2014 in computer software STATISTICA 12. The task of cluster analysis is to find groups of countries which are similar. The results of cluster analysis are described in the next chapters. The differences of the individual clusters and the possible influences that these differences may have caused are also described in the last chapters.

Os pacientes foram divididos em 4 grupos: Controle; E-cig; Fumante e Ex-fumante. Os critérios de inclusão adotados foram: ausência de histórico de neoplasia maligna bucal e ausência de quaisquer sinais clínicos visíveis de alteração no local avaliado. Foi realizada avaliação quanto ao grau de dependência nicotínica, por meio do Teste de Fagerström para Dependênca à Cigarro e Escore de Consumo Situacional Issa, e quanto ao perfil tabágico dos pacientes. A mensuração de CO expirado foi utilizada como critério objetivo de avaliação de consumo de tabaco, bem como a concentração de cotinina no sangue capilar dos pacientes avaliados. As amostras foram coletadas por meio de citologia esfoliativa da região de bordo lateral de língua e soalho bucal. Os esfregaços obtidos foram fixados com spray alcoólico e as lâminas coradas pelo método de Feulgen. Posteriormente, foi realizada a investigação dos micronúcleos e das anomalias metanucleares. Para a comparação dos dados não paramétricos foram utilizados os testes Kruskal-Wallis e Mann-Whitney. Já para as correlações e associações entre os dados, foram utilizados o teste da correlação de Spearman e o Teste Exato de Fisher, respectivamente. Foi adotado um nível de significância de 5% para todos os testes. A partir dos resultados, observou-se diferença significante entre os grupos E-cig e Controle (p=0,0072), e também entre os grupos Fumante e Controle (p=0,0004), no total de MN. Em contrapartida os grupos E-cig e Ex-fumante foram semelhantes, enquanto que o grupo Fumante foi significativamente maior que o grupo Ex-fumante (p=0,0370) também no total de MN. Também foi possível observar diferença significante entre os grupos E-cig e Ex-fumante, bem como entre os grupos E-cig e Controle em todas as anomalias metanucleares, exceto cariorrexe. Os grupos Fumante e Ex-fumante foram diferentes em cariorrexe (p=0,0517), e nuclear bud (p=0,0470) apenas, e o grupo Fumante foi significativamente maior que o grupo Controle em todas as anomalias metanucleares. A comparação entre os grupos E-cig e Fumante não foi realizada, devido à ingestão de altas doses de bebidas alcoólicas no grupo E-cig. O tabagismo promove danos citotóxicos e citogenéticos e o uso de E-cig associado ao álcool parece causar genotoxicidade e citotoxicidade. A cessação tabágica promove reversibilidade dos danos causados anteriormente. / The objective of this research was to investigate the frequency of micronuclei and metanuclear anomalies in the oral mucosa of electronic cigarette vaporizers and smokers of industrialized cigarettes and to compare with former smokers and nonsmokers to determine if there is a difference in cytogenetic and cytotoxic damage, as well as if it decreases after smoking cessation. The patients were divided into 4 groups: Control; E-cig; Smoker; and Former smoker. The inclusion criteria adopted were: absence of a history of oral malignancy and absence of any visible clinical signs of alteration at the evaluated site. An assessment was made of the degree of nicotinic dependence, using the Fagerström Test for Cigarette Dependence and the Issa Situational Consumption Score, and of the smoking profile of the patients. Measurement of expired CO was used as the objective criterion for the evaluation of tobacco consumption as well as the concentration of cotinine in the capillary blood of the evaluated patients. The samples were collected from the tongue lateral region and floor of the mouth by means of exfoliative cytology. The smears obtained were fixed with alcoholic spray and the slides were stained by the Feulgen method. Subsequently, the investigation of the micronuclei (MN) and metanuclear anomalies was carried out. For the non-parametric data, the Kruskal-Wallis and Mann-Whitney tests were performed. For the correlations and associations between the data, the Spearman correlation test and the Fisher Exact Test were respectively used. A significance level of 5% was adopted for all tests. From the results, we observed a significant difference between groups E-cig and Control (p = 0.0072), and also between groups Smoker and Control (p = 0.0004), in the total of MN. In contrast, groups E-cig and Former smoker were similar, whereas the group of Smoker was significantly higher than Former smoker (p = 0.0370) also in the total of MN. It was also possible to observe a significant difference between E-cig and Former smoker, as well as between E-cig and Control in all metanuclear anomalies, except for karyorrhexis. Already Smoker and Former smoker, they were different in karyorrhexis (p = 0.0517), and nuclear bud (p = 0.0470) only, and Smoker was significantly higher than Control in all metanuclear abnormalities. The comparison between groups E-cig and Smoker could not be performed due to the ingestion of high doses of alcoholic beverages in E-cig group. Smoking promotes cytotoxic and cytogenetic damage and the use of alcohol-associated electronic cigarettes appears to cause genotoxicity and cytotoxicity. Smoking cessation promotes reversibility of previously caused harm.
