Avaliação do efeito do metotrexato na reparação tecidual em um defeito ósseo simulando fratura de mandíbula em ratos / Influence of methotrexate on bone healing of a defect simulating a fracture in rat mandibleLeonardo Toledo de Aguiar 17 December 2008 (has links)
Objetivo: O presente estudo teve como objetivo verificar os efeitos do tratamento com altas e baixas doses de metotrexato (MTX) na reparação de fraturas mandibulares, num modelo experimental com ratos. Métodos: O modelo experimental empregado consiste na criação de um defeito ósseo na mandíbula do rato, semelhante a uma fratura. Oitenta ratos foram distribuídos em 4 grupos de 20 animais que receberam, por via intraperitoneal: soro fisiológico (controle) (1 ml, após a cirurgia); dexametasona (DX) (0,15 mg / kg, dose única após a cirurgia); alta dose MTX (1,6 mg / kg, semanalmente); baixa dose MTX, (0,25 mg / kg, semanalmente). Os animais foram sacrificados no dia 1º, 7º, 15º e 30º dia após a cirurgia. As mandíbulas foram submetidas à análise radiográfica para medir a distância entre os cotos ósseos e a área de osteotomia. Avaliação histomorfométrica foi realizada usando um software analisador de imagens digitalizadas para verificar a formação de cartilagem e óssea. Resultados: Os resultados revelaram não haver alterações significantes entre os tratamentos nos parâmetros avaliados nos 10 e 70 dias após a cirurgia. Animais do grupo controle sacrificados no 15º dia após a cirurgia tiveram uma redução da distância entre as extremidades ósseas e na área da osteotomia, bem como um grande aumento na formação de cartilagem. O padrão desses parâmetros nos animais tratados com baixas doses de MTX e DX não foram significativamente diferentes do grupo controle neste período. No entanto, os animais tratados com alta dose de MTX tiveram aumento da distância entre os cotos ósseos e da área da osteotomia, bem como foi praticamente nulo o aumento da formação de cartilagem. Sobre o 30º dia após a cirurgia, os animais do grupo controle tinham praticamente recuperado a região da fratura, bem como aqueles tratados com doses baixas de MTX. Os grupos tratados com a alta dose de MTX e DX mantiveram abertos os defeitos ósseos. Conclusões: Este estudo mostra claramente que a baixa dose de MTX não afetou a reparação óssea de fraturas mandibulares em ratos, em contraste com a alta dose de MTX, que afeta desfavoravelmente a regeneração óssea. / Purpose: The present study aims to verify the effect of high and low dose of methotrexate (MTX) treatment on bone repair of mandibular fractures in rats. Methods: The experimental model employed consists in creating a defect in rat mandible, similar to a fracture. Eighty rats were distributed in 4 groups of 20 animals that received, intraperitoneally: saline (1 ml, after surgery); dexametazone (DX, 0.15 mg/Kg, one dose at surgery); high dose MTX (1.6 mg/Kg, weekly); low dose MTX, (0.25 mg/Kg, weekly). Groups of five animals were sacrificed on the 1st, 7th, 15th and 30th day after surgery. Mandibles were submitted to radiographic analysis to measure the distance between bony edges and the area of osteotomy. Histopathological evaluation was performed in digitalized images using an analyzer software to examine cartilage and bone formation. Results: The treatments did not alter any evaluated parameters on days 1 and 7 after surgery. Control animals sacrificed on the 15th day after surgery had a reduction of the distance between bony ends and in the area of osteotomy, as well as a great increase in cartilage formation. The pattern of these parameters in animals treated with low dose MTX and DX did not differ from control group. However, animals treated with higher dose of MTX kept increasing the distance between bony ends and the area of osteotomy, and the increase in cartilage formation was practically inexistent. On the 30th day after surgery, control animals had pratically recovered the fracture region, as well as those treated with low dose MTX. The group treated with higher dose MTX and DX still had an open bone defect. Conclusions: This study clearly shows that low dose MTX did not affect bone healing of mandibular fractures in rats, in contrast to a higher dose, which promotes impairment of bone regeneration.

O advento da osteossíntese biológica é importante avanço no desenvolvimento da ortopedia. As vantagens que as hastes de Interlocking Nail têm demonstrado clinicamente, como método de fixação interna que promove formação de calo ósseo adequado, respeitando as premissas básicas da ortopedia moderna, com paciente retomando suas atividades de forma ativa, precoce e indolor, transportam esta técnica à condição de principal em termos de tratamento de fraturas cominutivas de fêmur em pacientes humanos e animais de companhia. No entanto, face a pouca disponibilidade de dados biomecânicos disposta em bibliografia, buscou-se, através deste estudo biomecânico não destrutivo, disponibilizar o padrão de movimentação interfragmentária do foco da fratura, mediante osteossíntese com hastes bloqueadas. Foi realizado estudo contemplando cinco grupos diferentes, com seis elementos cada, em análise in vitro, em relação a quatro diferentes possibilidades de bloqueio transcortical. Avaliou-se também comportamento do sistema quando utilizada haste de comprimento menor ao ideal, submetidos aos ensaios de compressão excêntrica, encurvamento crânio-caudal e rotação, através de análise fotogramétrica e confrontamento de dados aos obtidos pela análise de rigidez disponibilizada pela máquina universal de ensaios mecânicos Kratos® modelo 5002 dotada de célula de carga de 981 N (100 kgf). A análise dos resultados obtidos não apresentou diferença significativa entre os grupos nos ensaios propostos, tanto para análise fotogramétrica quanto análise de rigidez, sendo menor que 1,0 mm a média da movimentação interfragmentária de todos os grupos estudados. Estes dados demonstram biomecanicamente a aplicabilidade da técnica em fraturas cominutivas de terço médio de diáfise femoral em cães, possibilitando, frente à necessidade do cirurgia, optar por qualquer das técnicas de bloqueio aqui estudadas. / The advent of biological osteosynthesis is an important advance in the orthopedics development. The advantages that the rods of Interlocking Nail have clinically demonstrated, as method of internal fixation that promotes adequate bone callous formation, respecting the basic premises of the modern orthopedics, with patient retaking its activities of active, precocious and painless form, carry this technique to the first choice in terms of treatment of femoral comminutive fractures in human and small animal patients. However, face to few availability of biomechanical data disposed in bibliography, this non destructive biomechanical study had the aim to demonstrate the standard of interfragmentary motion, in Interlocking nails osteosynthesis. This research was developed contemplating five different groups, with six elements each group, in an in vitro analysis, in relation to four different possibilities of transcortical blockade. The behavior of the system when the osteosynthesis was made through one lesser length to the ideal interlocking rod was also evaluated. All groups were submitted to the assays of eccentric compression, bending and torsion, through photogrammetric analysis and comparison to the analysis of rigidity data, disposed for the Kratos® universal machine of mechanical assays, model 5002 endowed with 981 N (100 kgf) load cell. The analysis of disposed results did not present significant difference between the groups in the considered assays, neither for photogrammetric analysis nor for rigidity analysis, being the average of the interfragmentary motion lesser that one millimeter on all of the studied groups. These data biomecanically demonstrate the applicability of the interlocking nail technique in medial third cominnutive femoral fractures in dogs, including, front to the necessity of the surgeon, the possibility to opt to any of the techniques of blockade studied.

Previous issue date: 2013 / Resumo: As fraturas de ângulo mandibular são comuns, apresentando alta prevalência entre as fraturas da face. Estudos epidemiológicos apresentam informações sobre o perfil dos pacientes vítimas de trauma de face, auxiliando em métodos de prevenção e aperfeiçoando condutas de tratamento. Este trabalho é um estudo retrospectivo, longitudinal e observacional, com o objetivo de avaliar a epidemiologia das fraturas de ângulo mandibular, etiologia, frequência e modalidade de tratamento empregado, assim como complicações, dos pacientes atendidos pela Área de Cirurgia Buco-Maxilo-Facial da Faculdade de Odontologia de Piracicaba - FOP/Unicamp, no período compreendido entre abril de 1999 a dezembro de 2012. Foram incluídos 201 pacientes que sofreram 204 fraturas de ângulo mandibular, com maior prevalência no lado esquerdo (55,9%), cuja análise estatística descritiva demonstrou uma prevalência de indivíduos do gênero masculino (88,6%), cor branca (52,2%), com média de 27,95 anos ± 9,9 anos. A maioria apresentava algum tipo de vício (56,7%), a etiologia mais frequente foi agressão física (33%) e a má-oclusão foi o sinal com maior prevalência (72,6%). O sintoma mais frequente foi a dor (75,6%), e em 111 fraturas (54,4%) havia a presença de terceiros molares na linha de fratura. O tratamento cirúrgico foi empregado na maioria das fraturas (78%), sendo o acesso intraoral o mais frequentemente utilizado (60,0%). A técnica de fixação mais empregada foi a utilização de uma miniplaca do sistema 2,0mm (42,0%), havendo um baixo índice de complicações (9,5%), tendo sido a infecção a mais frequente. Concluiu-se que o grupo mais afetado foi constituído por adultos jovens do gênero masculino, vítimas de agressão física e que possuíam algum vício; a maioria dos pacientes foi submetida a tratamento cirúrgico, por acesso intraoral e utilização de uma única miniplaca, e houve baixa incidência de complicações. Estes dados epidemiológicos e das características do tratamento das fraturas de ângulo mandibular poderão auxiliar na instituição de campanhas preventivas, e de protocolos clínicos para o tratamento destas lesões, e também no direcionamento de futuras pesquisas. / Abstract: Mandibular angle fractures are common, with a high incidence among facial fractures. Epidemiological studies provide information on the profile of patients victims of facial trauma, assisting in methods of prevention and perfecting treatment procedures. This is a retrospective, longitudinal, and observational study, aiming to evaluate the epidemiology, etiology, frequency and type of treatment used, as well as complications, regarding mandibular angle fractures in patients treated at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry of Piracicaba - FOP/Unicamp, from April 1999 to December 2012. 201 patients with 204 mandibular angle fractures were included, showing higher incidence on the left side (55.9%), and a descriptive statistical analysis revealed the following prevalence: male subjects (88.6%), white subjects (52.2%), and mean age of 27.95 years ± 9.9 years. The majority had some kind of addiction (56.7%), the most common etiology was physical aggression (33%) and malocclusion was the sign with the highest incidence (72.6%), while the most frequent symptom was pain (75.6%) and 111 fractures (54.4%) had third molars in the line of fracture. Surgical treatment was employed in most fractures (78%), and intraoral access was the most frequent access used (60.0%). The fixation technique most often employed was the use of one 2.0 mm miniplate system (42.0%), with a low complication rate (9.5%) in the treatment of patients in this study. It was concluded that the most affected group consists of young adults male gender, victims of physical aggression and that most subjects underwent surgical treatment with few complications. Genetic factors in bone disorders:osteogenesis imperfecta, juvenile osteoporosis and stress fracturesHartikka, H. (Heini) 16 May 2005 (has links)
Abstract
Genetic factors and their resulting phenotypes were evaluated in three different bone disorders: osteogenesis imperfecta (OI), juvenile idiopathic osteoporosis (JIO), and stress fractures.
The spectrum of the OI phenotypes caused by mutations in the COL1A1 and COL1A2 genes is well defined, but the mechanisms by which the variations affect the hearing phenotype are not well-known. A total of 54 Finnish OI patients with previously diagnosed hearing loss, or aged 35 or more years, were analyzed here for mutations in COL1A1, or COL1A2. Altogether, 49 mutations were identified, of which 41 were novel. No correlation was observed between the mutated gene, or the mutation type, and the hearing pattern. This indicates that the basis of hearing loss in OI is complex, and is a result of multifactorial, still unknown genetic effects, or of variable expressions of the COL1A1 and COL1A2 genes.
JIO presents peri-pubertally as an acute symptomatic osteoporosis (bone pain and fractures) in otherwise healthy children, and no underlying cause has yet been identified for this disorder. Here, the analysis of the low-density lipoprotein receptor-related protein 5 gene (LRP5) in 20 patients with JIO revealed two missense mutations (A29T and R1036Q) and one frameshift mutation (C913fs) in 3 of the patients. The LRP5 gene has recently been shown to be also involved in osteoporosis-pseudoglioma syndrome and a high-bone-mass phenotype.
Stress fractures are a significant problem among athletes and soldiers. Genetic factors may increase the fracture risk, but no susceptibility genes have yet been identified. Seven genes involved in bone metabolism, or pathology, were studied in terms of their roles in stress fracture. No disease-causing, or predisposing variations were found in the candidate gene, or association analyses, but a highly significant association was found between the phenotype and a vitamin D receptor (VDR) haplotype, TGT, which is composed of three polymorphic sites, FokI, BsmI and TaqI. We showed that femoral neck stress fractures are associated with a certain VDR haplotype, accounting for a five-fold increase in the risk of developing stress fractures, with an associated attributable risk of 12%.
The results of this study show that genetic factors play a role in different pathological bone phenotypes. These findings provide new information on the pathogenesis of the disorders and for the development of genetic testing and targeted treatment for the disorders.

A three-month prospective study of risk factors for stress fractures sustained by soldiers during basic trainingWood, Paola Silvia 31 May 2009 (has links)
Stress fractures represent one of the most common and serious overuse injuries in the military environment. The aim of this prospective study was to determine the incidence of stress fractures during 12 weeks of Basic Training (BT) by comparing the results of the intrinsic risk indicators obtained from a group of participants who suffered stress fractures, with the rest of the original group (controls) who did not suffer from any stress fractures, and to assess any changes in physical markers whilst following a progressive, scientifically designed, Physical Training (PT) Programme during the BT. The intrinsic risk factors investigated included sex, age, race (measured via questionnaire), foot morphology (wet test), Q angle, leg length discrepancy, bone density (dual-energy X-ray absorptiometry(DEXA), physical fitness (standardized military fitness test, isokinetic upper and lower leg strength, handgrip strength), flexibility (ankle plantarflexion and dorsiflexion, hip internal and external rotation), anthropometry (skinfold method and DEXA), female menstrual disturbances and lifestyle behaviours including smoking, female contraception use and medical history of previous injury (questionnaire). The cohort (n=183), also refered to as the Experimental Group (EG), was measured at the beginning and at the end of the BT period. The standardized physical fitness test was also completed in the fifth week of training. The latter’s results were compared to the results obtained by a Control Group (CG), who had undergone BT the year prior to this cohort. The size of the cohort, the intrinsic risk factor profile and the control of certain extrinsic risk factors may have contributed to zero incidences of stress fractures found. Within the intrinsic risk factor profile, sex, age, race, foot morphology, Q angle, hip external rotation and bone density were normal whilst the measured leg discrepancy and limited ankle dorsiflexion appeared to not have a sufficient risk for stress fracture development. The small sample of the cohort that reported having menstrual irregularities, smoked and had a history of previous fractures, did not place this cohort at risk for stress fracture development. The cohort did, however have lower isotonic, isokinetic and isometric strengths than the other cohorts who reported a relatively high stress fracture incidence. The BT period found statistically significant changes in bone density, flexibility, body composition, muscle strength and endurance. Female participants showed an increase in the T- and Z-scores of the left femur area, a deterioration in left ankle dorsiflexion and hip external rotation, whilst their plantarflexion increased. Their mesomorph component increased, and decreases in % body fat (BF) as well as in the ectomorph and endomorph component were also found. Male participants’ plantarflexion and hip external rotation decreased whilst their dorsiflexion increased. Lean body mass and mesomorph component increased whilst %BF, ectomorph and endomorph component decreased. The new cyclic-progressive PT programme controlled for risk of injury by allowing sufficient periods of recovery, by gradually increasing the duration, frequency, and intensity of training, by reducing repetitive weight-bearing activities and by including a variation of exercises. Running shoes, rather than combat boots, were also worn during PT. Marching on concrete was eliminated. Significant improvements were shown by both male and female participants in aerobic fitness and muscular endurance and muscular strength. Future research should include a larger size cohort, who developed stress fractures utilising BT groups from different corps and units in the South African Military environment. Previous issue date: 2015 / Resumo: O risco de fratura osteoporótica pode ser avaliado clinicamente baseado em fatores clínicos e pela densidade mineral óssea (DMO), entretanto esses parâmetros não são bons preditores do risco de fratura. Recentemente, o Brasil foi incluído no instrumento fracture risk assessment tool- FRAX-BRASIL, porém seu uso tem sido limitado na prática clínica. OBJETIVO: Avaliar o grau de concordância entre o risco de fratura em 10 anos calculado pelo FRAX-BRASIL com e sem densitometria em mulheres brasileiras na pós-menopausa. MÉTODO: Realizou-se um estudo de corte transversal no período de novembro de 2014 a fevereiro de 2015, com 402 mulheres em acompanhamento no Ambulatório de Menopausa do Hospital da Mulher Prof. Dr. José Aristodemo Pinotti em Campinas-SP. Foram incluídas mulheres com 40 anos ou mais, em amenorreia há pelo menos 12 meses e com exame de densitometria óssea prévio a qualquer tratamento medicamentoso para osteopenia ou osteoporose. As mulheres foram entrevistadas por um pesquisador durante a consulta de rotina, na qual foram coletadas informações sobre fatores de risco necessários para o questionário FRAX-BRASIL e dados da densitometria óssea. Os dados obtidos foram inseridos na plataforma online FRAX-BRASIL, em que foi calculado o risco para uma fratura maior e de quadril, utilizando-se somente os fatores de risco clínicos e o risco incluindo valores de DMO do colo do fêmur em g/cm2. ANÁLISE ESTATÍSTICA: Para análise do grau de concordância entre os riscos de fraturas com e sem densitometria óssea foi utilizado o coeficiente de correlação intraclasse (ICC). O Teste de Mann-whitney foi utilizado para comparação entre as médias do risco de fratura calculado com e sem DMO; para comparação entre as frequências de alto risco calculadas com e sem DMO foi utilizado o Teste de comparação entre duas proporções. Para análise da associação entre as variáveis clinico/demográficas e a variação do risco de fratura foi utilizada a análise de regressão linear. O nível de significância adotado foi <0,05. RESULTADOS: A probabilidade de fratura em 10 anos calculada pelo FRAX-BRASIL para fratura de quadril e para fratura maior somente pelos fatores de risco clínicos foi de 0,84% ±1,92 e 4,03% ±2,98 e com DMO foi de 0,83% ±1,76 e 4,05% ±2,98 respectivamente. O coeficiente de correlação intraclasse entre o FRAX-BRASIL com e sem DMO foi de 0,76 (IC95% 0,716-0,799) para uma fratura maior e de 0,644 (IC95% 0,583-0,698) para fratura de quadril. Ao avaliar as mulheres utilizando o FRAX com DMO 0,75% e 5,22% excederam os limiares de alto risco para fratura maior e de quadril, respectivamente. Sem o acréscimo da densidade óssea 1% e 11,44% apresentaram alto risco para fratura maior e de quadril, respectivamente. Dessa forma a recomendação de tratamento foi concordante entre o FRAX com e sem DMO em 99,75% dos casos de alto risco de fratura maior e de 93,78% para o quadril. Os fatores associados a menor variação FRAX com e sem foram maior idade, menor DMO, menor T-score e ausência de fratura previa tanto para fratura maior como para quadril. O menor IMC esteve associado a menor variação do FRAX apenas para fratura maior. CONCLUSÃO: O risco de fratura maior ou de quadril foi baixo na população estudada. O FRAX-BRASIL apresentou alta concordância para estimar o risco de fratura maior e concordância moderada para fratura de quadril apresentando uma estimativa de risco para fratura semelhante com ou sem DMO em nossa população / Abstract: The risk of osteoporotic fracture can be clinically evaluated based on clinical factors and by the bone mineral density (BMD), but these parameters are not good predictors of fracture risk. Recently, Brazil was included in the fracture risk assessment tool- FRAX-BRAZIL, but its use has been limited in clinical practice. GOAL: To evaluate the degree of correlation between the degree of correlation between the risk of fracture in 10 years calculated by FRAX-BRAZIL with and without densitometry in Brazilian postmenopausal women. METHODS: A cross-sectional study was conducted with 402 women followed up at the Menopause Ambulatory at the Women's Hospital Prof. Dr. José Aristodemo Pinotti in Campinas-SP. Women were included with 40 years or more in amenorrhea for at least 12 months and with bone densitometry exam prior to any drug treatment for osteopenia or osteoporosis. A researcher interviewed the women during a routine visit, where information about risk factors necessary for the FRAX-BRAZIL questionnaire and data of bone densitometry were collected. The collected data were inserted on the online platform FRAX-BRAZIL where the risk for major fractures and of the hip using only clinical risk factors and the risk including femoral neck BMD values in g / cm2. STATISTICAL ANALYSIS: To analyze the degree of correlation between the risk of fractures with and without bone densitometry was used the intraclass correlation coefficient (ICC). The Mann-Whitney test was used to compare the averages of fracture risk calculated with and without BMD; to compare the frequencies of high risk calculated with and without BMD was used the compare Test between two proportions. For analysis of the association between clinical / demographic variables and the change of the fracture risk was used linear regression analysis. The significance level was <0.05. RESULTS: The fracture probability calculated in 10 years by using the FRAX-BRAZIL for hip fracture and major fracture only by clinical risk factors was 0.84% ± 1.92 and 4.03 ± 2.98% and BMD was 0.83% ± 1.76 and 4.05 ± 2.98%, respectively. The intraclass correlation coefficient between the FRAX-BRAZIL with and without BMD was 0.76 (IC95% 0.716-0.799) for a major fracture and 0.644 (IC95% 0.583-0.698) for hip fracture. When evaluating women using FRAX with BMD 0.75% and 5.22% exceeded the high-risk thresholds for major and hip fracture, respectively. Without the increase of the bone density 1% and 11.44% presented high risk for major fractures and of hip, respectively. Then the treatment recommendation was consistent between the FRAX with and without BMD in 99.75% of cases of high risk of major fracture and 93.78% for the hip. Factors associated with less variation FRAX with and without were older, lower BMD, lower T-score, and no previous fracture both for major fracture as to hip fracture. The BMI was associated with lower variation in the FRAX only to major fracture. CONCLUSION: The risk of major fracture or of the hip was low in the study population. The FRAX-BRAZIL presented a high correlation to estimate the risk of major fractures and moderate agreement for hip fracture presenting a risk estimate for similar fracture with or without BMD in our population. Orientador: Norma Sueli Pinheiro Módolo / Resumo: Introdução: A última atualização demográfica do Instituto Brasileiro de Geografia e Estatística realizada em 2017 mostrou que a população acima de 60 anos representava 14,6% dos 207,1 milhões de habitantes do Brasil, o que corresponde a aproximadamente 30 milhões de idosos, com expectativa de vida de 75,8 anos. Entre 2007 e 2011, o número acumulado de internações por fraturas de fêmur em pacientes acima de 60 anos, nos hospitais do Sistema Único de Saúde, foi de 175.781 O delirium pós-operatório é uma complicação comum nos pacientes idosos, e está associado a hospitalizações prolongadas, maiores taxas de institucionalização após a alta, deterioração cognitiva prolongada, diminuição da capacidade funcional, além de ser fator independente de mortalidade em 6-12 meses. Estudo disponível na literatura mostra que 14 a 24% dos pacientes idosos com fratura de quadril apresentam delirium já na admissão hospitalar, sendo que a prevalência durante a internação chega a 56%. Os mecanismos fisiopatológicos do desenvolvimento do delirium pós-operatório (DPO) e do déficit cognitivo pós-operatório têm sido estudados em nível molecular, porém ainda com pouco ganho. From 2007 to 2011, 175.781 elder patients were admitted after a hip fracture in brazilian public hospitals. Post-operative delirium (POD) is an usual complication in the elderly and is associated to longer hospital stay, cognitive impairment, functional decline and increased 6-12 months mortality rate. Delirium rates between the elder population can be as high as 14 to 24% already at admission, being the prevalence as high as 56% during hospital stay. Pathophysiologic mechanisms of delirium have been studied at molecular level, but research efforts are still needed in order to develop sensitive and specific early markers for this condition, so that diagnosis and effective treatment would be readily given. Objectives: This research project aims to correlate two neuroinflamation biomarkers serum levels (S100B protein and Neuron Specific Enolase - NSE) and the incidence of pre and postoperative delirium in elderly patients undergoing hip fracture repair. Sensitivity and specificity of thoracic radiography relative to computed tomography in dogs affected by blunt trauma caused by a motor vehicle accidentDancer, Sumari Constance January 2019 (has links)
Thoracic injuries caused by blunt trauma are commonly encountered emergencies in veterinary medicine. However, no literature exists comparing radiography to computed tomography (CT) in blunt thoracic trauma caused by motor vehicle accidents in canine patients. The aim of this prospective case series was to estimate the sensitivity (Se) and specificity (Sp) of thoracic radiography relative to CT for detecting lung contusions, pneumothorax, pleural effusion and rib fractures. The study further aimed to establish a severity scoring system for radiography and CT and to compare the findings between the two modalities. The hypothesis was that radiography would be less sensitive than CT at detecting these injuries and that radiography would underestimate the severity of lung contusions compared to CT. Fifty-nine patients met the inclusion criteria. Radiography underestimated the presence of lung contusions (Se = 69%, 95% Confidence interval (CI)) and overestimated the severity of the contusions relative to CT. There was also high interobserver variability in evaluating lung contusion severity (coefficient of variation = 91%). Both the three-view thoracic and horizontal beam radiography had poor sensitivities for the detection of pneumothorax (Se = 19% and 63% respectively) and pleural effusions (Se = 43% and 71% respectively). Similarly, the sensitivity (56%) of three-view thoracic radiographs for the detection of rib fractures was poor relative to CT. Outcomes of double miniplate osteosynthesis in the immediate management of infected mandible fracturesDangor, Zain January 2020 (has links)
Magister Chirurgiae Dentium (MChD) / Introduction: A common complication of poorly managed mandible fractures is infection. There is a consensus amongst clinicians in treating infected mandible fractures in an immediate setting. The approach includes drainage of the purulent discharge, debridement of the fracture, removal of teeth in the fracture line and immediate fixation. Fixation can be load bearing or load sharing in nature. Although clinicians advocate for the use of a reconstruction load bearing plate, a double miniplate fixation could be an alternative.
Aim: The aim is to assess the outcomes of double miniplate osteosynthesis in the immediate management of infected mandible fractures Material and method: A prospective cohort study was conducted. The sample size was 20 (n =20). Infections were treated with an incision and drainage and the fractures fixated with a double miniplate fixation system. Pain scores, fracture union, fracture stability and surgical times were measured. Follow–up visits included one week, six weeks and three months respectively.
Results: Mandibular angle fractures were most commonly infected. The most common cause of infection were teeth in the line of fracture. The buccal fascial space was the most commonly involved fascial space in regard to spread of infection. Nineteen of the twenty cases attained fracture stability. However, complications commonly encountered were persistent infection and wound dehiscence.
Conclusion: Although a limited number of clinical cases were treated, the results correspond with the current literature when an immediate miniplate fixation protocol is used in infected mandible fractures. However, complications encountered in the study included the persistence of infection and wound dehiscence.

Calcitonin is a hormone secreted by the C-cells of the thyroid gland in response to elevations of the plasma calcium level. It reduces bone resorption by inhibiting mature active osteoclasts and increases renal calcium excretion. It is used in the management of postmenopausal osteoporosis, Paget's disease of bone, and malignancy-associated hypercalcemia. Synthetic and recombinant calcitonin preparations are available; both have similar pharmacokinetic and pharmacodynamic profiles. As calcitonin is a peptide, the traditional method of administration has been parenteral or intranasal. This hinders its clinical use: adherence with therapy is notoriously low, and withdrawal from clinical trials has been problematic. An oral formulation would be more attractive, practical, and convenient to patients. In addition to its effect on active osteoclasts and renal tubules, calcitonin has an analgesic action, possibly mediated through β-endorphins and the central modulation of pain perception. It also exerts a protective action on cartilage and may be useful in the management of osteoarthritis and possibly rheumatoid arthritis. Oral formulations of calcitonin have been developed using different techniques. The most studied involves drug-delivery carriers such as Eligen® 8-(N-2hydroxy-5-chloro-benzoyl)-amino-caprylic acid (5-CNAC) (Emisphere Technologies, Cedar Knolls, NJ). Several factors affect the bioavailability and efficacy of orally administered calcitonin, including amount of water used to take the tablet, time of day the tablet is taken, and proximity to intake of a meal. Preliminary results looked promising. Unfortunately, in two Phase III studies, oral calcitonin (0.8 mg with 200 mg 5-CNAC, once a day for postmenopausal osteoporosis and twice a day for osteoarthritis) failed to meet key end points, and in December 2011, Novartis Pharma AG announced that it would not pursue further clinical development of oral calcitonin for postmenopausal osteoporosis or osteoarthritis. A unique feature of calcitonin is that it is able to uncouple bone turnover, reducing bone resorption without affecting bone formation and therefore increasing bone mass and improving bone quality. This effect, however, may be dose-dependent, with higher doses inhibiting both resorption and formation. Because so many factors affect the pharmacokinetics and pharmacodynamics of calcitonin, especially orally administered calcitonin, much work remains to be done to explore the full pharmacologic spectrum and potential of calcitonin and determine the optimum dose and timing of administration, as well as water and food intake.
