Com o objetivo de obter a prevalência de anticorpos anti-HIV e comportamentos de risco relacionados entre adolescentes infratores, foram entrevistados 1122 e 93 jovens do sexo masculino e feminino, respectivamente, internados na FEBEM (FundaçãoEstadual para o Bem-Estar do Menor de São Paulo). Ao mesmo tempo obteve-se uma amostra de sangue para sorologia. O questionário avaliou os seguintes comportamentos: prática sexual, uso de drogas ilícitas, conhecimento sobre a AIDS. A soroprevalência para o HIV foi obtida erelacionada com os comportamentos de risco por análise uni-, bi- e multivariada. A positividade para o HIV foi 2,6% e 10,3% para sexo masculino e feminino, respectivamente. O risco de transmissão parenteral foi maior que o de transmissão sexualentre meninos, sendo o contrário nas meninas / In order to study the prevalence of HIV and related risky behaviors among disadvantaged youth, we interviewed and bled, between December 1994 and April 1995, 1122 young male and 93 young female who were serving time in FEBEM, a state institution that cares for homeless and offender youth of São Paulo, Brazil. Our questionnaire covered the following areas: sexual practices and use of illicit drugs: knowledge of HIV and STDs and their prevention; and myths and beliefs about AIDS. Seroprevalence of HIV was assessed and related with risk-taking behaviors by means of uni-,bi- and multivariate analysis. We found 2.6% of the males and 10.3% of the females to be positive to HIV. The prevalence of HCV antibodies resulted in 5.9% for males and 4.6% for females, respectively. The risk for parenterally transmitted HIV among males was higher than that for sexually related transmission. The inverse relationship was found among females.

Innate immune mechanisms in limiting HIV-1 pathogenesis among South African adults and mother-infant pairs.

This study was conducted to investigate the role of natural killer cell surface receptors, KIRs and their cognate HLA ligands in preventing HIV-1 acquisition and disease progression in HIV-1 exposed infants. Using DBS stored for 8 years from 21 pregnant South African women we evaluated 3 methods of gDNA extraction with and without whole genome amplification (WGA) to characterize immune-related genes: IL-10, KIR and HLA class I. However, IL-10 SNP typing was only for testing the quality of gDNA. QIAamp DNA mini kit yielded the highest gDNA quality (p<0.05; Wilcoxon Signed Rank Test) with sufficient yield for subsequent analyses. In contrast, WGA was not reliable for SSP-PCR analysis of KIR2DL1, KIR2DS1, KIR2DL5, and KIR2DL3 or high resolution HLA genotyping using a sequence-based approach. A cohort of 370 infants; 124 HIV-1 perinatally infected, 120 exposed uninfected and 126 unexposed healthy infants was used for KIR and HLA genotyping. After adjustment for viral load and multiple comparisons, the frequency of HLA-Cw*04:01 allele was likely to be associated with susceptibility to mother-to-child acquisition of HIV-1 in exposed infected (EI) infants (p=0.05; Logistic Regression analysis). HLA-A*23:01 was likely to be associated with decreased CD4 T lymphocyte count in HIV-1 infected infants (p=0.01; ANOVA), whereas HLA-B*81 tended to be associated with higher CD4 T lymphocyte count (p=0.04, ANOVA). We speculate that HLA-Cw*04:01 interacts with KIR2DL1 and inhibit NK cell responses which predispose the infants to HIV-1 infection. KIR2DS1 and KIR2DL5 were both associated with faster HIV-1 disease progression. Identified protective HLA-class I alleles could be used to present viral epitopes to either NK cells via KIRs or CTLs and enhance immune activation which may promote resistance to HIV-1 infection. / Thesis (M.Med.Sc.)-University of KwaZulu-Natal, Durban, 2012.

The aim of this study was to explore the knowledge of HIV transmission and prevention among adolescents aged between 14 and 16 years at the Tshwane West District. A qualitative exploratory descriptive contextual study was conducted. The World Café approach was used to obtain data from participants in this research. A questionnaire served as the data collection tool to extract data from participants. The study consisted of 37 participants and content analysis was done to analyse data gathered from participants during the World Café. Conclusions were drawn from the study and the findings revealed that the knowledge of HIV transmission and prevention among adolescents in the Tshwane West District is poor. / Health Studies / M.P.H.

The competencies of midwives during the provision of immediate postnatal care in Swaziland

Text in English / The aim of the study was to describe the competencies of midwives during the provision of immediate postnatal care (PNC) with the intention of adapting and linking international best practice guidelines as well as a conceptual framework for the implementation of PNC in Swaziland. A quantitative cross-sectional design study was conducted to investigate the competencies of midwives during the provision of immediate postnatal care services to mothers and their infants. A systematic random sampling technique was used to select eighty-eight (88) midwives and six (6) senior midwives to participate in the study. Data collection was done using structured questionnaires. Quantitative data was complemented by data that was generated from open-ended questions at the end of the questionnaires. Data analysis was conducted using IBM SPSS Statistics version 22.0 software. The study highlighted that all the midwives who participated in this study had the relevant qualification. Of the respondents, 70.5% were state-certified midwives with a second registered certificate, 27.3% had bachelor’s degrees, while 2.3% had an advanced midwifery certificate. The study found no difference between the type of qualification of midwives and the knowledge of PNC interventions to be offered to mothers immediately post-delivery across different qualifications held by the midwives (Kruskal-Wallis test: x2=5.498, df=2, p=0.064). Gaps were identified in their knowledge and practices. There were discrepancies in the level of knowledge and practices regarding maternal vital sign assessment immediately after delivery (within 30 minutes). It was noted that these vital signs, i.e. blood pressure (12.5%), temperature (50.0%), pulse (54.5%), respiration (63.6%) were not taken after delivery. It was also noted that 15.0%, 58.0%, 64.8% of the respondents were not aware of the importance of assessing newborns for APGAR, skin-to-skin contact and drying the neonate. The study found that there were no postnatal care guidelines in Swaziland. The findings of the study led to the adaption and linkage of the latest international evidence-based guidelines and a conceptual framework for the implementation of immediate PNC to mothers and their infants in Swaziland. / Health Studies / D. An evaluation of postnatal care rendered to HIV positive women and their infants

The purpose of this study was to evaluate care rendered to HIV positive women and their infants during the first six weeks of postpartum. Quantitative, descriptive, cross sectional and analytic study was conducted to investigate postnatal care services provided to HIV positive mothers. Data collection was done using structured questionnaires. 372 respondents participated in the study. Descriptive data analysis was used; Epi info version 3.5.2 software was used. The study highlighted that the quality of PNC was compromised, in all levels including the critical immediate postnatal care, 3-14 days and 6 weeks postnatal care services. All health facilities that were involved in the study had the basic resources to render quality postnatal care. Negative attitudes of staff and long waiting time (16.7%), were the most deterrents to postnatal care. / Health Studies / M.A. (Health Studies)

The views of midwives regarding the implementation of PMTCT programmes in public health centres in Soweto

M.Cur. (Maternal and Child Nursing Science) / The implementation of the PMTCT programme was conceptualised in 2000. The aim of this study was to determine the views of midwives regarding the implementation of the PMTCT programmes in the public health centres in Soweto. Data was collected from midwives involved in the implementation of PMTCT in antenatal clinics by means of semi-structural interviews. Of the 20 midwives (N=20) interviewed, only five (5) midwives attended formal training. The discussion of the views of midwives regarding the implementation of PMTCT centered around the lack of formal training, lack of resources, lack of infrastructure, negative attitude of the community, lack of budgeting, lack of support by colleagues and poor management support, language barriers in communication and religious beliefs. This study found that midwives need to go for formal training on PMTCT programmes and that more research need to be done in this field. Moreover, the findings from this study have implications for clinical practice. In addition, guidelines were formulated to help midwives and other health care practitioners to implement the PMTCT programmes in antenatal care.

Intimacy, sex and sexuality : the experiences of vertically-infected HIV-positive adolescents

This research explores the lived experience of being a vertically or prenatally-infected HIV-positive adolescent. It looks specifically at how the participants experience intimacy, sex and sexuality. HIV/AIDS remains a global pandemic and vertically-infected adolescents are a growing new demographic. However, there is a poverty of research, and therefore interventions and support, for this demographic. This qualitative research conducted six individual, in-depth, semi-structured, psychoanalytic research interviews with three participants. The interviews were structured around projective drawings that the participants did in the course of each interview. The interviews were transcribed and analysed using psychodynamic object relations theory and organised through interpretative phenomenological analysis. Every effort was made to ensure that the research was conducted ethically and validly. The analysis found that the participants' experience of intimacy has resulted in a self that is constantly under threat. This in turn has negatively impacted on the participants' experience of sex and sexuality. The implication of this research is that more in-depth research needs to be done into this demographic so that better interventions and support may be offered.

The results of this qualitative study provided descriptive data from university administrators within the athletics setting who are responsible for providing adequate student athlete health care services from developed and implemented administrative policies that directly or indirectly affect a student athlete's physiological and psychological well-being.

Factors influencing men's involvement in prevention of mother-to-child transmission (PMTCT) of HIV programmes in Mambwe district, Zambia

The study aimed at assessing the factors influencing the low involvement of men in prevention of mother-to-child transmission (PMTCT) of HIV programmes in the Mambwe district, Zambia. The factors studied were grouped as knowledge and awareness, socio-cultural, programmatic and demographic characteristics. A quantitative, exploratory study was used and 127 men were interviewed. The major findings were: Knowledge of PMTCT was the strongest factor which was positively associated with the level of men's involvement in PMTCT. Socio-cultural and programmatic factors were found to negatively influence men's involvement (although weakly); and among the demographic characteristics, age and level of education were positively associated with an increase in the level of involvement, while the duration of the relationship with the female partner was negatively associated with the level of men involvement. / Health Studies / M.A. (Public Health)
