An integrated socio-dental approach to dental treatment need estimation

To improve on the traditional approaches of dental treatment need estimation, a new theoretical model of treatment needs composed of 'Normative', 'Impact-Related', 'Effective' and 'Accessible' treatment needs, is proposed. The aim of the present study was to develop an appropriate socio-dental indicator to generate 'Impact-Related Treatment Need', compare them with traditional professionally assessed normative need, and to explore the preliminary approach to assess 'Effective Treatment Need' for periodontal treatment. An oral health survey was carried out in Ban Phang district, Khon Kaen, Thailand. 501 people aged 35-44 years in 16 villages, selected by cluster random sampling process, were interviewed about oral impacts on daily performances and their oral health behaviours, and then were had clinical examinations. A concise indicator 'Oral Impacts on Daily Performances' (OIDP) was developed, based on measuring impacts on eight physical, psychological and social aspects of daily activities. Scores were calculated by multiplying the frequency by the severity scores of each performance, then scores were summated. The indicator was tested for psychometric properties on the study sample, using interviews and clinical examinations. Cronbach alpha of internal consistency was 0.65. Kappa of test-retest reliability ranged from 0.6 to 1.0. Criterion validity was shown by a strong relationship with general perceptions of oral impacts (p < 0.001). Construct validity was demonstrated by significant differences of scores between groups with high and low prevalence of deep periodontal pockets, decayed and missing teeth, and number of functional teeth (p < 0.001). The combination of specific OIDP scores and normative needs generated the impact-related need for various dental treatments. As expected, the percentage of people with need was decreased from normative need, when Impact-Related Treatment Need was applied. The differences were large in needs for prostheses, orthodontics and periodontal treatment (ranging from 21.7% to 40.2% of normative need); moderate for restorations (64%); and low for pulp care, extractions and treatment for deep periodontal pockets (81.7%-91.7%). A similar pattern of need reduction was obtained when increasing cut-off points of OIDP scores. An exploratory study to develop 'Effective Treatment Need' for periodontal treatment was done. It was shown, using logistic regression that plaque control and smoking were the behaviour-oriented risk factors which affected behavioural propensity for periodontal destruction. Receiver Operating Curve showed the appropriate cut-off point of plaque level at 0.8. Behavioural propensity was determined from people who are non-smokers with plaque index of 0.8 or lower, to generate Effective Treatment Need. The results indicated that the majority of people who had Impact-Related Treatment Need for periodontal treatment did not have the acceptable level of behavioural propensity required for effectiveness of treatment. The advantages of the socio-dental needs approach in dental care planning were demonstrated and discussed. It was concluded that the OIDP is a valid and reliable indicator of oral outcomes. Integrating it into Impact-Related Treatment Need, as well as further integration of behavioural propensity to generate Effective Treatment Need could provide alternative improved dental treatment need estimations.

Objetivos: Neste estudo objetivou-se avaliar e comparar a condição periodontal de dentes pilares e não pilares de indivíduos com fissura labiopalatina reabilitados com prótese parcial removível instalada. Material e métodos: Pilares de 45 pacientes com fissuras labiopalatinas, de ambos os gêneros que utilizam prótese parcial removível, matriculados no Hospital de Reabilitação de Anomalias Craniofaciais, Universidade de São Paulo (HRAC-USP), com idade variando de 25 a 72 anos. Foram excluídos da amostra os indivíduos com alterações sistêmicas, doenças neurológicas, que tenham realizado tratamento periodontal prévio nos últimos seis meses, fumantes, gestantes, etílicos, usuários de drogas, que estavam fazendo uso de antidepressivos, antiinflamatórios, anticonvulsivantes, bloqueadores de cálcio ou qualquer fármaco que pudesse interferir nos resultados deste estudo. Os parâmetros clínicos avaliados por um único avaliador foram: medida de profundidade de sondagem, nível de inserção clínico, índice gengival, índice de placa, recessão gengival, quantidade de mucosa ceratinizada, com sonda periodontal, além das condições de higiene da prótese, classificação da área desdentada, classificação do tipo de grampo e foi aplicado o questionário de qualidade de vida. Resultados: Não houve diferença estatisticamente significante entre os dentes pilares e não pilares com exceção do nível de inserção clínico e profundidade de sondagem. As próteses de classe III foram as mais frequentes. Os escores 2 e 3 foram os mais frequentes dos índices de placa e Tarbet. O tipo de prótese, segundo a classificação de Kennedy não influenciou o OHIP-14. Os escores mais frequentes do OHIP-14 foram 0 e 1 . Conclusões: Os dentes pilares e não pilares das próteses parciais removíveis de indivíduos com fissura labiopalatina não apresentaram diferenças significativas nos parâmetros clínicos avaliados com exceção do nível de inserção clínico e profundidade de sondagem, aceitando-se parcialmente a hipótese nula. O tipo de grampo a classificação das próteses não tiveram influência na qualidade de vida. The prostheses of class III were the most frequent. The scores 2 and 3 were the most frequent plaque index and Tarbet. The type of prosthesis, according to the classification of Kennedy did not influence the OHIP-14. Scores Frequently OHIP-14 were 0 and 1. Objective: The purpose of this study was to evaluate the possible association between periodontal disease, preterm birth and low birth weight in pregnant women attended at the Basic Health Units and Family Health Strategies, in the city of Santa Maria, Rio Grande do Sul. In addition, to create a virtual learning technology in the form of a website, serving as a strategy for educating the oral health of pregnant women and their future babies. Methodology: The study was a cross-sectional, with a convenience sample, composed of 60 pregnant women attending the public service in the municipality of Santa Maria, RS. Data related to biological characteristics, socioeconomic factors and related to medical history, odontology and oral hygiene habits were obtained through a semi-structured questionnaire. Data on oral conditions were obtained from periodontal exams and data on the delivery of babies were obtained through the Pregnant Woman's Book. Results: Through the descriptive analysis, it was observed that the majority of the pregnant women were non-smokers (60.0%), self-reported white race (56.6%) and had schooling above elementary school (65.0%). Regarding dental clinical conditions, the prevalence of periodontitis was 28.33% in pregnant women. The percentage of gingival bleeding index was 34.9% and the supragingival index was 40.4%. Bleeding the probing was found in 25.6% of the sites. The mean PS and NIC were similar (1.85 and 1.87, respectively). The majority of patients presented PS and NIC ranging from 1-3mm (94.3% and 95.68%, respectively). Most births were at term (88.3%) and the vast majority of infants presented weight> 2,500 kg (91.6%). Conclusion: There was no association between periodontitis, preterm birth and low birth weight. However, the results of this study reveal a high prevalence of periodontitis in pregnant women, and it is necessary to act in a way to prevent its occurrence. It is noteworthy that as a product of this master's thesis, a website about dental prenatal care, oral health care and its importance for the pregnant woman and her baby were built. / Introdução: Os cuidados com a saúde bucal durante o período gestacional consistem em uma parte essencial de cuidados pré-natal, pois possibilita a prevenção e tratamento de doenças bucais que podem refletir sobre a saúde de ambos. Objetivo: O presente estudo teve como propósito avaliar a possível associação entre doença periodontal, parto prematuro e baixo peso ao nascer em gestantes atendidas em Unidades Básicas de Saúde e Estratégias de Saúde da Família, do município de Santa Maria, Rio Grande do Sul. Além disso, criar uma tecnologia virtual de aprendizagem, na forma de um website, servindo como estratégia de educação sobre a saúde bucal de gestantes e seus futuros bêbes. Metodologia: O delineamento do estudo foi do tipo transversal, com amostra de conveniência, composta por 60 gestantes que frequentavam o serviço público no município de Santa Maria, RS. Dados relacionados às características biológicas, fatores socioeconômicos e relativos à história médica, odontológica e hábitos de higiene bucal foram obtidos mediante questionário semiestruturado. Os dados a respeito das condições bucais foram obtidos a partir de exames periodontais e os dados referentes ao parto dos bebês foram obtidos através da Caderneta da Gestante. Resultados: Através da análise descritiva observou-se que a maioria das gestantes era não fumante (60.0%), raça auto-relatada branca (56.6%) e possuíam escolaridade acima do ensino fundamental completo (65,0%). Sobre as condições clínicas dentárias, a prevalência de periodontite foi de 28.33% nas gestantes. Sendo que a percentagem de índice de sangramento gengival foi de 34.9% e de cálculo supragengival de 40.4%. Sangramento a sondagem foi encontrado em 25.6% dos sítios. A média de PS e NIC foi similar (1.85 e 1.87 respectivamente). A maioria das pacientes apresentou PS e NIC variando entre 1-3mm (94.3% e 95.68% respectivamente). A maior parte dos nascimentos foi a termo (88.3%) e a grande maioria dos bebês apresentaram peso > 2.500 kg (91.6%). Conclusão: Não houve associação entre a periodontite, parto prematuro e baixo peso ao nascer. Entretanto, os resultados desse estudo revelam alta prevalência de periodontite nas gestantes, sendo necessário atuar de forma a prevenir a sua ocorrência. Destaca-se que como produto desta dissertação de mestrado, foi construído um website sobre o pré-natal odontológico, cuidados com a saúde bucal e sua importância para a gestante e seu bebê.
