Avaliação de custo e da efetividade no tratamento da hepatite crônica C / Evaluation of cost and effectiveness of treatment for chronic hepatitis C

Introdução: Aproximadamente, 170 milhões de pessoas estão cronicamente infectadas pelo vírus da hepatite C (VHC). A hepatite C é considerada a principal causa de doença hepática, podendo evoluir para cirrose e carcinoma hepatocelular. O tratamento objetiva deter a progressão da doença hepática pela inibição da replicação viral. Objetivo: Avaliar a efetividade e os custos no tratamento da hepatite C. Casuística e Métodos: Estudo retrospectivo de coorte com 161 pacientes acompanhados por meio das prescrições médicas no período de julho de 2005 até outubro de 2007, em uma instituição terciária de assistência, ensino e pesquisa, que utiliza como tratamento interferon alfa (INF), Alfapeginterferona 2a 180mcg (PEG 2a) e Alfapeginterferona 2b 80mcg (PEG 2b). Custo do tratamento calculado com base nos valores apenas dos medicamentos de 2005 até 2010. Resultados: A média de idade foi de 53 anos (23-81, + 11,11); 51,6% dos pacientes eram do gênero feminino; 104 (64,6%) pacientes com genótipo 1, seguidos de 52 (32,3%) com genótipo 3. Cerca de 30 (18,6%) pacientes cirróticos. Foram encontrados cinco grupos de pacientes, de acordo com o tipo de interferon utilizado durante o tratamento; 55 (34,2%) pacientes com resposta virológica sustentada (RVS), 86 (53,4%) não respondedores (NR) e 20 (12,4%) pacientes outras respostas. O uso de terapia de suporte foi maior nos grupos tratados com Alfapeginterferona 24 pacientes (14,9%) quando comparados com interferon alfa, 3 pacientes (1,9%) (p< 0,0001). O custo do tratamento foi maior nos pacientes tratados com Alfapeginterferona 2a 180mcg ao se comparar com as outras alternativas encontradas no estudo. Conclusões: A RVS foi semelhante entre os grupos estudados, o interferon alfa apresentou menor uso de terapia de suporte, e o custo do tratamento do Alfapeginterferona 2b 80mcg foi menor do que o Alfapeginterferona 2a 180mcg. / Introduction: Approximately 170 million people are chronically infected with hepatitis C virus (HCV). Conclusions: SVR was similar between groups, the interferon alpha showed less use of supportive therapy and the treatment cost of peginterferon alpha 2b 80mcg was lower than the peginterferon alpha 2a 180mcg.

A hepatite C afeta cerca de 150 milhões de pessoas no mundo e é a razão mais comum de transplantes de fígado. A erradicação viral, por meio de tratamento medicamentoso, é a única intervenção que pode deter a progressão da doença, reduzir a mortalidade e melhorar a qualidade de vida dos pacientes. Em 2011, foi aprovado o boceprevir, um inibidor de protease, que passou a ser adicionado à terapia padrão dupla (interferon peguilado e ribavirina) pelo Protocolo Clínico brasileiro para tratamento de Hepatite C genótipo 1 em pacientes com grau de fibrose maior que F2. Devido ao alto custo de aquisição deste medicamento e à produção cada vez maior de novas tecnologias para essa área terapêutica, foi proposta essa pesquisa que tem como objetivo analisar o custo-efetividade da terapia tripla em relação à terapia dupla, no tratamento da hepatite C crônica genótipo 1 em pacientes virgens de tratamento para todos os graus de fibrose. Para tanto, foi construído um modelo de Markov com 15 estados de saúde representando a história natural da Hepatite C crônica. O modelo seguiu uma coorte hipotética pela vida toda, em que os custos foram expressos em Reais e os desfechos em anos de vida ganhos. A perspectiva adotada foi a do SUS. A RCEI calculada, com taxa de desconto de 5% para custos e desfechos, foi R$ 201.504,92 por ano de vida ganho. Considerando um limiar de custo efetividade de 3 vezes o valor do PIB per capita, segundo recomendação da OMS, a adição do boceprevir não foi custo-efetiva no tratamento de pacientes virgens em todos os graus de fibrose. Pela análise de sensibilidade, nenhuma variável teve grande impacto na RCEI, exceto quando a taxa de desconto aplicada em desfechos foi zerada, em que a terapia tripla passou a ser custo-efetiva. / The Hepatitis C virus affects around 150 million of people worldwide and it is the most common reason for liver transplantation. Viral eradication, by drug treatment, is the only therapeutic intervention that may halt the disease progression, reduce HCV-related mortality and improve the quality of life of infected patients. Boceprevir, a protease inhibitor, was approved in 2011, being to be added to standard of care (peguilated interferon-&#945; and ribavirin) by the Brazilian Protocol of treatment of genotype 1 Hepatitis C, in patients with degree of fibrosis greater than F2. Due to the high cost of acquisition of this drug and the increasing production of new technologies in this therapeutic area, the aim of this work was develop a cost-effectiveness analysis, comparing the triple therapy with the standard of care (double therapy) for treatment of genotype 1 chronic hepatitis C in treatment-naïve patients of all degrees of fibrosis. It was constructed a Markov Model with 15 health states representing the natural history of chronic Hepatitis C. The model followed a hypothetic cohort by lifetime, where costs were expressed in Reais and outcomes in life-years gained, under the perspective of Brazilian public health system. The calculated ICER, with discount rate of 5% to costs and outcomes, was R$201.504, 92 by life-years gained. Considering three times GDP per capita for cost-effectiveness threshold, according WHO recommendation, boceprevir was not cost-effective, when considered treatment-naïve patients of all degrees of fibrosis. By sensitivity analysis, none of the variables had a big impact in the ICER, except when it was stopped applying the discount rate in outcomes, in which the triple therapy became cost-effective.

Influência dos polimorfismos do gene IL-28B na resposta à terapia com interferon peguilado e ribavirina em pacientes tratados por hepatite C crônica / Influence of IL-28B gene polymorphisms in the response to pegylated interferon and ribavirin therapy in patients with chronic hepatitis C

Introdução: Estima-se que a infecção pelo vírus da hepatite C (VHC) acometa 3% da população global constituindo a maior causa de cirrose hepática e hepatocarcinoma. Nos pacientes infectados pelo genótipo 1 do VHC, a terapia em vida real com interferon peguilado (PEG-IFN) e ribavirina (RBV) resulta em 30% a 50% de resposta virológica sustentada (RVS). Este desfecho é determinado por fatores de associados ao VHC (carga viral, genótipo, quasispécies), características do hospedeiro (etnia, gênero, fatores genéticos, comorbidades, adesão) e fatores ligados aos medicamentos. Estudos de associação genética ampla (GWAS) têm demonstrado que a presença dos polimorfismos (SNP) no gene da interleucina 28B (IL-28B) associam-se a resposta à terapia baseada em interferon. Os SNPs rs12979860, rs8099917 e rs12980275 têm sido amplamente estudados e pacientes com o perfil favorável têm maiores chances de alcançar a RVS ou a eliminação espontânea do VHC. Objetivos: Avaliar a frequência relativa e a influência dos polimorfismos rs12979860 (C > T), rs8099917 (T > G) e rs12980275 (A > G) no gene IL-28B em brasileiros portadores de infecção crônica pelo VHC tratados com PEGIFN/ RBV em uma casuística de pacientes atendidos pelos médicos do Ambulatório de Hepatites DMIP/LIM-47 e avaliar retrospectivamente a possibilidade de predição de resposta à partir do perfil IL-28B na população estudada. Materiais e Métodos: Após aprovação ética, foram selecionados 171 pacientes com coleta retrospectiva e sistematizada das informações de interesse. O DNA destes pacientes foi purificado e foram desenhados primers e sondas específicas para a genotipagem dos SNPs através da técnica de reação em cadeia de polimerase em tempo real. Resultados: entre os SNPs selecionados na análise univariada rs12979860 e rs12980275 associaram-se com RVS (p < 0,05). rs8099917 não teve associação com RVS. Na análise multivariada apenas rs12980275 manteve associação com RVS (p < 0,05). Conclusão: Ao contrário do descrito na Literatura Internacional nos pacientes brasileiros apenas o pouco estudado SNP rs12980275 associou-se à RVS / Introduction: Currently, infection with hepatitis C virus (HCV) affects 3% of people worldwide, and is the major cause of chronic hepatitis C, liver cirrhosis and hepatocellular carcinoma. SNPs rs12979860, rs8099917 and rs12980275 have been widely studied and patients with the favorable profile are more likely to achieve SVR. Objectives: To evaluate the relative frequency and influence of the polymorphisms rs12979860 (C > T), rs8099917 (T > G) and rs12980275 (A > G) in Brazilian patients treated for chronic hepatitis C with PEG-IFN/RBV, and to evaluate, retrospectively, the prediction of the response possibility from IL-28B profile in the selected patients. Material and Methods: It was selected 171 patients. Medical history was evaluated retrospectively. A real time polymerase chain reaction assay was realized in order to analyze the polymorphisms. Results: In univariate analysis the polymorphisms rs12979860 and rs12980275 were associated with SVR (p < 0,05). The SNP rs8099917 was not associated with SVR. Multivariate analysis revealed that only rs12980275 maintained an association with SVR (p < 0,05). Conclusion: It is described on international literature that rs12979860 is strongly associated with SVR, however in Brazilian patients only the less studied SNP rs12980275 was associated with SVR

INTRODUÇÃO: A hepatite C crônica é uma das principais causas de hepatopatia em todo mundo. A coinfecção pelo vírus C (VHC) e o HIV não é incomum, pois ambos compartilham vias similares de transmissão. Recentemente, a terapêutica da hepatite C crônica foi radicalmente modificada com o advento das drogas antivirais de ação direta (DAAs), elevando as taxas de RVS mesmo na população coinfectada. O VHC é caracterizado pela sua alta taxa replicativa e por grande diversidade populacional. Substituições de ocorrência natural na protease viral associadas a resistência podem comprometer a terapêutica em alguns regimes baseados no uso de inibidores de protease (IPs). OBJETIVOS: Estimar a prevalência de polimorfismos e mutações de ocorrência natural associadas a resistência aos IPs em pacientes monoinfectados e coinfectados pelo VHC e HIV e identificar fatores clínicos e virológicos associados a presença de tais substituições. MATERIAIS E MÉTODOS: Dados epidemiológicos e clínicos foram obtidos de 247 pacientes (135 monoinfectados e 112 coinfectados pelo VHC e HIV). VHC RNA foi extraído do plasma dos indivíduos participantes e um fragmento de 765 pares de base da região NS3 foi amplificado e sequenciado por metodologia populacional (técnica de Sanger). O estadiamento da fibrose hepática foi realizado pelo escore não invasivo FIB- 4. RESULTADOS: 54 indivíduos (21,9%) apresentaram pelo menos uma substituição na região NS3/NS4A do VHC. Somente 14 pacientes (5,7%) apresentaram pelo menos uma mutação de resistência aos IPs (T54S, V55A ou Q80R). A Q80K não foi identificada em nenhuma das amostras. Não houve diferença entre monoinfectados e coinfectados quanto à ocorrência de polimorfismos ou mutações associadas a resistência. As variáveis independentemente associadas com substituições na região da protease foram infecção pelo VHC genótipo 1b, bilirrubinas totais > 1,5 vezes o LSN e níveis de albumina < 3,5 g/dL. Fibrose hepática avançada (FIB-4 > 3.25) não esteve associada a presença de substituições. A análise de diversidade nucleotídica na protease viral revelou maior heterogeneidade do VHC genótipo 1b em relação ao 1a. Contudo, a análise de pressão seletiva não demonstrou maior variabilidade de quasiespécies no grupo de hepatopatia avançada, achado este compatível com uma sequência genômica relativamente conservada. CONCLUSÕES: As substituições na região NS3/NS4 do VHC consistiram majoritariamente por polimorfismos naturais sem impacto clínico num eventual tratamento que envolva o uso de IPs. A prevalência de substituições associadas a resistência foi baixa e compatível com os valores informados pela maioria dos estudos nacionais e internacionais. A coinfecção pelo HIV não parece elevar a frequência de substituições na protease do VHC. A região NS3 do genótipo 1b foi altamente variável em relação ao genótipo 1a, reforçando o conceito de possíveis diferenças geográficas em relação ao perfil genético deste vírus / INTRODUCTION: Chronic hepatitis C is a major cause of liver disease worldwide. Hepatitis C vírus (HCV) and HIV coinfection is not uncommon due to similar transmission routes. Recently developed direct-acting antivirals drugs (DAAs) have increased the rate of SVR even in coinfected patients. HCV has a high replication rate and a lack of proofreading activity, leading to a greatly diverse viral population. Baseline spontaneously occurring resistance substitutions in the protease region may impair the rate of success in some protease inhibitors (PI) based regimens. OBJECTIVE: to determine the prevalence of naturally occurring polymorphisms and resistance associated variants to HCV PIs in mono and coinfected HCV HIV patients and to evaluate potential associations between amino acid substitutions in protease domain and clinical / virological features of those patients. METHODS: Clinical and epidemiological data were retrieved from medical records of 247 subjects in Brazil (135 HCV monoinfected and 112 HIV HCV coinfected patients). HCV-RNA was extracted from plasma and a fragment of 765 base pairs from the NS3 region was amplified and sequenced with Sanger-based technology. Fibrosis staging was assessed by non invasive score (FIB-4). RESULTS: Overall, 54 patients (21.9%) had at least one amino acid substitution in the NS3 region; only 14 patients (5.7%) harboured at least one resistance mutation (T54S, V55A, Q80R). Q80K mutation was not found in any sample. There was no difference between monoinfected and coinfected patients regarding the frequency of natural polymorphisms and resistance mutations. Variables independently associated with amino acid substitution were HCV subtype 1b, total bilirubin level > 1.5 ULN and albumin level < 3.5 g/dL. Advanced liver fibrosis (FIB-4 > 3.25) was not related to NS3 polymorphisms nor resistance associated variants. Examination of HCV protease nucleotide diversity revealed greater heterogeneity in subtype 1b than subtype 1a. Analysis of selective pressure did not reveal a greater quasispecies variability in advanced liver fibrosis group, being such finding consistent with a relatively conserved gene in this setting. CONCLUSION: Baseline HCV NS3 amino acid substitutions depicted herein were considered mostly natural polymorphisms with no clinical impact in a PI based therapy. The prevalence of resistance-associated substitutions was low and compatible with values reported by most national and international studies. HIV coinfection was not associated with a greater frequency of such substitutions in the studied sample. The NS3 region of genotype 1b was highly variable in relation to genotype 1a, highlighting geographic differences concerning HCV genetic profile

A prevalência de substituições associadas a resistência foi baixa e compatível com os valores informados pela maioria dos estudos nacionais e internacionais. A coinfecção pelo HIV não parece elevar a frequência de substituições na protease do VHC. A região NS3 do genótipo 1b foi altamente variável em relação ao genótipo 1a, reforçando o conceito de possíveis diferenças geográficas em relação ao perfil genético deste vírus / INTRODUCTION: Chronic hepatitis C is a major cause of liver disease worldwide. Hepatitis C vírus (HCV) and HIV coinfection is not uncommon due to similar transmission routes. Recently developed direct-acting antivirals drugs (DAAs) have increased the rate of SVR even in coinfected patients. HCV has a high replication rate and a lack of proofreading activity, leading to a greatly diverse viral population. Baseline spontaneously occurring resistance substitutions in the protease region may impair the rate of success in some protease inhibitors (PI) based regimens. OBJECTIVE: to determine the prevalence of naturally occurring polymorphisms and resistance associated variants to HCV PIs in mono and coinfected HCV HIV patients and to evaluate potential associations between amino acid substitutions in protease domain and clinical / virological features of those patients. METHODS: Clinical and epidemiological data were retrieved from medical records of 247 subjects in Brazil (135 HCV monoinfected and 112 HIV HCV coinfected patients). HCV-RNA was extracted from plasma and a fragment of 765 base pairs from the NS3 region was amplified and sequenced with Sanger-based technology. Fibrosis staging was assessed by non invasive score (FIB-4). RESULTS: Overall, 54 patients (21.9%) had at least one amino acid substitution in the NS3 region; only 14 patients (5.7%) harboured at least one resistance mutation (T54S, V55A, Q80R). Q80K mutation was not found in any sample. There was no difference between monoinfected and coinfected patients regarding the frequency of natural polymorphisms and resistance mutations. Variables independently associated with amino acid substitution were HCV subtype 1b, total bilirubin level > 1.5 ULN and albumin level < 3.5 g/dL. Advanced liver fibrosis (FIB-4 > 3.25) was not related to NS3 polymorphisms nor resistance associated variants. Examination of HCV protease nucleotide diversity revealed greater heterogeneity in subtype 1b than subtype 1a. Analysis of selective pressure did not reveal a greater quasispecies variability in advanced liver fibrosis group, being such finding consistent with a relatively conserved gene in this setting. CONCLUSION: Baseline HCV NS3 amino acid substitutions depicted herein were considered mostly natural polymorphisms with no clinical impact in a PI based therapy. The prevalence of resistance-associated substitutions was low and compatible with values reported by most national and international studies. HIV coinfection was not associated with a greater frequency of such substitutions in the studied sample. A hepatite C afeta cerca de 150 milhões de pessoas no mundo e é a razão mais comum de transplantes de fígado. A erradicação viral, por meio de tratamento medicamentoso, é a única intervenção que pode deter a progressão da doença, reduzir a mortalidade e melhorar a qualidade de vida dos pacientes. Em 2011, foi aprovado o boceprevir, um inibidor de protease, que passou a ser adicionado à terapia padrão dupla (interferon peguilado e ribavirina) pelo Protocolo Clínico brasileiro para tratamento de Hepatite C genótipo 1 em pacientes com grau de fibrose maior que F2. Devido ao alto custo de aquisição deste medicamento e à produção cada vez maior de novas tecnologias para essa área terapêutica, foi proposta essa pesquisa que tem como objetivo analisar o custo-efetividade da terapia tripla em relação à terapia dupla, no tratamento da hepatite C crônica genótipo 1 em pacientes virgens de tratamento para todos os graus de fibrose. Para tanto, foi construído um modelo de Markov com 15 estados de saúde representando a história natural da Hepatite C crônica. O modelo seguiu uma coorte hipotética pela vida toda, em que os custos foram expressos em Reais e os desfechos em anos de vida ganhos. A perspectiva adotada foi a do SUS. A RCEI calculada, com taxa de desconto de 5% para custos e desfechos, foi R$ 201.504,92 por ano de vida ganho. Considerando um limiar de custo efetividade de 3 vezes o valor do PIB per capita, segundo recomendação da OMS, a adição do boceprevir não foi custo-efetiva no tratamento de pacientes virgens em todos os graus de fibrose. Pela análise de sensibilidade, nenhuma variável teve grande impacto na RCEI, exceto quando a taxa de desconto aplicada em desfechos foi zerada, em que a terapia tripla passou a ser custo-efetiva. / The Hepatitis C virus affects around 150 million of people worldwide and it is the most common reason for liver transplantation. Viral eradication, by drug treatment, is the only therapeutic intervention that may halt the disease progression, reduce HCV-related mortality and improve the quality of life of infected patients. Boceprevir, a protease inhibitor, was approved in 2011, being to be added to standard of care (peguilated interferon-&#945; and ribavirin) by the Brazilian Protocol of treatment of genotype 1 Hepatitis C, in patients with degree of fibrosis greater than F2. Due to the high cost of acquisition of this drug and the increasing production of new technologies in this therapeutic area, the aim of this work was develop a cost-effectiveness analysis, comparing the triple therapy with the standard of care (double therapy) for treatment of genotype 1 chronic hepatitis C in treatment-naïve patients of all degrees of fibrosis. It was constructed a Markov Model with 15 health states representing the natural history of chronic Hepatitis C. The model followed a hypothetic cohort by lifetime, where costs were expressed in Reais and outcomes in life-years gained, under the perspective of Brazilian public health system. The calculated ICER, with discount rate of 5% to costs and outcomes, was R$201.504, 92 by life-years gained. Considering three times GDP per capita for cost-effectiveness threshold, according WHO recommendation, boceprevir was not cost-effective, when considered treatment-naïve patients of all degrees of fibrosis. By sensitivity analysis, none of the variables had a big impact in the ICER, except when it was stopped applying the discount rate in outcomes, in which the triple therapy became cost-effective.

Influência dos polimorfismos do gene IL-28B na resposta à terapia com interferon peguilado e ribavirina em pacientes tratados por hepatite C crônica / Influence of IL-28B gene polymorphisms in the response to pegylated interferon and ribavirin therapy in patients with chronic hepatitis C

Introdução: Estima-se que a infecção pelo vírus da hepatite C (VHC) acometa 3% da população global constituindo a maior causa de cirrose hepática e hepatocarcinoma. Nos pacientes infectados pelo genótipo 1 do VHC, a terapia em vida real com interferon peguilado (PEG-IFN) e ribavirina (RBV) resulta em 30% a 50% de resposta virológica sustentada (RVS). Este desfecho é determinado por fatores de associados ao VHC (carga viral, genótipo, quasispécies), características do hospedeiro (etnia, gênero, fatores genéticos, comorbidades, adesão) e fatores ligados aos medicamentos. Estudos de associação genética ampla (GWAS) têm demonstrado que a presença dos polimorfismos (SNP) no gene da interleucina 28B (IL-28B) associam-se a resposta à terapia baseada em interferon. Os SNPs rs12979860, rs8099917 e rs12980275 têm sido amplamente estudados e pacientes com o perfil favorável têm maiores chances de alcançar a RVS ou a eliminação espontânea do VHC. Objetivos: Avaliar a frequência relativa e a influência dos polimorfismos rs12979860 (C > T), rs8099917 (T > G) e rs12980275 (A > G) no gene IL-28B em brasileiros portadores de infecção crônica pelo VHC tratados com PEGIFN/ RBV em uma casuística de pacientes atendidos pelos médicos do Ambulatório de Hepatites DMIP/LIM-47 e avaliar retrospectivamente a possibilidade de predição de resposta à partir do perfil IL-28B na população estudada. Materiais e Métodos: Após aprovação ética, foram selecionados 171 pacientes com coleta retrospectiva e sistematizada das informações de interesse. O DNA destes pacientes foi purificado e foram desenhados primers e sondas específicas para a genotipagem dos SNPs através da técnica de reação em cadeia de polimerase em tempo real. Resultados: entre os SNPs selecionados na análise univariada rs12979860 e rs12980275 associaram-se com RVS (p < 0,05). rs8099917 não teve associação com RVS. Na análise multivariada apenas rs12980275 manteve associação com RVS (p < 0,05). Conclusão: Ao contrário do descrito na Literatura Internacional nos pacientes brasileiros apenas o pouco estudado SNP rs12980275 associou-se à RVS / Introduction: Currently, infection with hepatitis C virus (HCV) affects 3% of people worldwide, and is the major cause of chronic hepatitis C, liver cirrhosis and hepatocellular carcinoma. Objectives: To evaluate the relative frequency and influence of the polymorphisms rs12979860 (C > T), rs8099917 (T > G) and rs12980275 (A > G) in Brazilian patients treated for chronic hepatitis C with PEG-IFN/RBV, and to evaluate, retrospectively, the prediction of the response possibility from IL-28B profile in the selected patients. Material and Methods: It was selected 171 patients. Medical history was evaluated retrospectively. A real time polymerase chain reaction assay was realized in order to analyze the polymorphisms. Results: In univariate analysis the polymorphisms rs12979860 and rs12980275 were associated with SVR (p < 0,05). The SNP rs8099917 was not associated with SVR. Multivariate analysis revealed that only rs12980275 maintained an association with SVR (p < 0,05). Conclusion: It is described on international literature that rs12979860 is strongly associated with SVR, however in Brazilian patients only the less studied SNP rs12980275 was associated with SVR
