Transtornos de personalidade: história do conceito e controvérsias atuais. / Personality disorders: history of the concept and current controversies.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Este trabalho tem como objetivo analisar a estruturação do diagnóstico de Transtorno de Personalidade. Inicialmente, o trabalho percorre o território conceitual com o qual, desde sua origem na passagem do século XVIII para o XIX, a psiquiatria procurou nomear, explicar e compreender as personalidades consideradas anormais. Em seguida promove-se uma discussão acerca das concepções de personalidade, normalidade e patologia que circunscrevem a categoria, orientada a partir do estudo de seus diferentes modelos diagnósticos presentes no DSM-5. Por fim busca-se compreender a relevância atual do diagnóstico de transtorno de personalidade através da análise de alguns exemplos de seu uso em contextos médico, legal e literário. O objetivo é o de entrever o lugar ocupado por esse diagnóstico, especialmente o do tipo Antissocial, no imaginário cultural presente. / This work analyzes the processes through which the diagnosis of personality disorder has been structured. First, it covers the conceptual territory through which, from its origin in the passage of the eighteenth century to the nineteenth, psychiatry sought to name, to explain and to understand the so called abnormal personalities. This first step is followed by comments on the concepts of personality, normality and pathology that take part in the construction of this psychiatric category as it exists today. In order to do this, the structure of different diagnostic models present in the DSM-5 is analyzed. Finally, we seek to understand the current relevance of the diagnosis of personality disorder by taking into consideration examples of its use in medical, legal and literary contexts, in order to shed some light over the place occupied by this diagnosis, especially the anti-social type, in todays cultural landscape.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / O período de reestruturação e reordenação da cidade do Rio de Janeiro ao princípio da história republicana do Brasil, que culmina na chamada Belle Époque Carioca, é analisado aqui sob a ótica higienista. Neste sentido, a política assume uma postura de adequação do cotidiano embasada na medicina social e na intervenção sobre os costumes considerados indevidos, degenerantes. Um campo de conhecimento importante neste processo de adequação e imposição de uma nova ordem é a psiquiatria. Renovada pelas proposições de Juliano Moreira, a ciência seria responsável pela conceituação de novos padrões de anormalidade, as chamadas personalidades psicopáticas, e pelo afastamento destes perturbadores da ordem em relação ao convívio com a sociedade. / The period of restructuring and reordering of the city of Rio de Janeiro at the beginning of Brazils republican history, which culminates at the so called Belle Époque Carioca, is analyzed here under the perspective of the hygienism. In that way, politics assumes a posture of adequacy of the everyday based on social medicine and on intervention over the manners considered improper, degenerating. An important field of knowledge in this process of adequacy and imposition of a new order is psychiatry. Renewed by the propositions of Juliano Moreira, that science would be responsible by the conceptualization of new standards of abnormality, the so called psychopathic personalities, and by the withdrawal of these disturbers of the order in relation to the interaction with society.

Os Hospitais Gerais desenvolveram-se, ao longo da história da medicina, como ambientes em que não havia espaço para os loucos. A partir do século XX, começa a ocorrer uma mudança nesse paradigma, sobretudo nos EUA e na Europa, com a criação das UPHGs em diversos serviços por lá, e a consolidação da Interconsulta psiquiátrica como área de atuação da psiquiatria, que passa a estreitar o contato com o restante da medicina. Foi um movimento que rendeu diversos avanços tanto à psiquiatria quanto às outras áreas médicas. No Brasil, esse percurso começou na década de 1950 e ocorre de forma mais tímida. Apenas 1,1% dos Hospitais gerais contam com suporte da psiquiatria em nosso país atualmente. Contudo, houve espaço no Brasil para o desenvolvimento de projetos pioneiros nesse campo, cuja história não está registrada. A psiquiatria pernambucana foi protagonista na empreitada, inaugurando inclusive a integração da psiquiatria ao hospital geral no Brasil. Pernambuco também foi um dos pioneiros na introdução da clínica psicanalítica em hospitais gerais do serviço público. O presente trabalho pretende registrar a história dessas experiências. Para tanto, escolhemos os serviços da UFPE e do Hospital Barão de Lucena, sedes dos primeiros serviços de saúde mental em hospital geral de Pernambuco. Fez-se uma ampla pesquisa documental, além de serem entrevistados alguns dos principais atores da construção desses serviços. Apesar de difícil, a integração dos cuidados de saúde mental ao hospital geral na UFPE e no HBL conseguiu efetivar-se, a partir da psiquiatria na UFPE e da psicanálise no HBL. Foram espaços configurados e sustentados dentro de uma perspectiva de formação, para além da assistência, e acreditamos que sua longevidade tenha relação com isso. / The history of psychiatry, for the most part, is intertwined with the history of asylums. General Hospitals have developed mostly as environments in which there was no room for the insane. Since the early twentieth century, a paradigm shift takes place, specially in the US and Europe, with the creation of psychiatryc units in several General Hospitals there. It led to the consolidation of Consultation Liaison Psychiatry as an official sub-specialty of Psychiatry. Integration of Psychiatry and other medical specialties was a movement that yielded many advances in both psychiatry as other medical areas. In Brazil, this journey began in the 1950s and is more . Only 1.1% of brazilian general hospitals have psychiatric support nowadays. However, there was room in Brazil for the development of pioneering experiments in this field, whose history is not recorded. Pernambuco’s psychiatry had a key role in these experiments, inaugurating the integration of psychiatry to the general hospital in Brazil. Pernambuco was also one of the first places to have a psychoanalytic couch in public service facilities. This work aims to record the history of these pioneering experiences in the state of Pernambuco. Therefore, we chose the services of UFPE and the Hospital Barão de Lucena, headquarters of general Hospital Psychiatry in Pernambuco. An extensive literature research was done, along with statements collection from some of the major players in the construction of these services. Although difficult, the integration of mental health care to the general hospital at UFPE and HBL was sucessful, arising from psychiatry at UFPE and psychoanalysis at in HBL. They were configured spaces and sustained in a training perspective, and we believe that it explains their longevity.

ADHD in historical and comparative perspective : medical, educational and public approaches to childhood hyperactivity in the US and the UK, 1960-2010

Adding a much needed historical and comparative dimension to current debates about Attention Deficit Hyperactivity Disorder (ADHD), the present thesis provides an analysis of the changing construction and treatment of childhood hyperactivity in Britain and the United States, focusing on the period from 1960 to 2010. The focal point is the historical discrepancy between the two countries in diagnostic and therapeutic practices, and the question of how and why perspectives have increasingly converged over the past 20 years. Whereas British medical and educational professionals continued to rely on environmental explanations and interventions for the vast bulk of disruptive behaviour in school children, the American concept of hyperactivity disorder from the 1960s onwards became increasingly inclusive and biomedical in orientation. This expansion was closely related to the rise of psycho-stimulants as a widely employed treatment for hyperactivity and attention problems in the US. British and other European clinicians, on the other hand, resisted drug treatments up until the mid-1990s, when rates of diagnosis and prescription grew dramatically on both sides of the Atlantic. A key aim of this study is to explore and explain the rise of ADHD and Ritalin in both the American and British contexts, looking at the interplay of political, professional, institutional and socio-cultural factors that have contributed in each case. The study concentrates on three distinct but interconnected spheres which, both separately and in combination, have underpinned and shaped approaches to hyperactivity in the two countries: medicine, education and the wider public arena, represented by parent support groups. While chapters 2, 3 and 4 focus on the medical debates and practices surrounding hyperactivity, and the points of connection and disconnection between the two medico-psychiatric communities, chapters 5 and 6 examine the role of schooling, disability activism, and educational policy, especially that relating to special educational needs. Finally, chapter 7 explores the issue of parent activism which has been an important factor in the growth and critique of ADHD in both settings.

Transnational trauma : trauma and psychiatry in the world and Taiwan, 1945-1995

This study considers the history of trauma, both as a psychiatric concept and as a diagnosis, and its social and cultural representation from a transnational perspective after WWII. The intellectual evolution of trauma was determined by various medical, social and cultural variables, institutions, and people who wielded influence in the postwar world order as well as diverse local contexts. This thesis focuses on the globalisation and localisation of such concept and diagnosis shaped by international and local mental health experts at the World Health Organization and the National Taiwan University Hospital. Through the efforts of these experts, trauma not only became one of the most globally diffused psychiatric diagnoses, but also a hyperbole appropriated by Taiwanese psychiatrists to account for extreme forms of social suffering. Studies have criticised the universality and the Anglo-American-centred approach to the history of traumatic psychiatry. Scholars have also begun to explore transnational histories of psychiatry by systematically comparing or tracing the diffusion routes of psychiatric topics. Their methods of enquiry and problems solved, however, differ. My research analyses a disparate collection of evidence at the level of international organisations and from local aspects, allowing not only a critical reconsideration of trauma in the trend of global medicine, but also its reception, contestation and appropriation in the non-Western contexts. Guided by the works of medical historians, literary critics and cultural anthropologists, this project combines archival research with oral history interviews to challenge the existing historical accounts of trauma, and provide evidence of the limited capacity of globalised psychiatric norms and their reception and appropriation beyond the imagination of world citizenship. It argues that such scientific artefacts were not only produced through mutual reference between Eastern and Western experiences, but also measures of instrumental rationality employed by postwar internationalists to engineer their modernity in the Global South.

Esta dissertação analisa o Sanatório São José, uma instituição psiquiátrica particular criada no ano de 1934 na cidade de Porto Alegre, Rio Grande do Sul. O recorte temporal proposto para a pesquisa inicia com o contexto de criação do referido estabelecimento e encerra no decorrer da década de 1950 quando ocorre a introdução e a disseminação do uso de medicamentos para o tratamento psiquiátrico, o que imprime uma mudança no cotidiano e na prática institucional. Parte-se desta instituição para examinar a prática psiquiátrica de forma ampla, examina-se nesse sentido o contexto que legitimava o funcionamento de estabelecimentos desta natureza, a atuação dos psiquiatras como profissionais autorizados pela sociedade e as práticas terapêuticas desenvolvidas na instituição. Utilizam-se como fonte, sobretudo, as pastas dos pacientes internados neste estabelecimento, sendo estas de responsabilidade do Arquivo da Clínica São José. A pesquisa que segue esta dividida em três capítulos: no primeiro capítulo analisa-se o contexto da assistência psiquiátrica no Rio Grande do Sul durante as décadas de 1920 e 1930, compondo o ambiente no qual se desenvolveu a ideia da criação de um Sanatório particular. Os agentes envolvidos na criação desta instituição e os profissionais que atuaram naquele espaço – os psiquiatras e as religiosas – são analisados ainda neste espaço. No segundo capítulo, a partir dos dados presentes nos questionários iniciais – Boletins de internamento – e nas anotações clínicas presentes nas pastas dos pacientes, examinamos as concepções de alienação mental que compartilhavam aqueles que procuravam este estabelecimento, as formas como se justificava a internação, as atitudes e hábitos flagrantes de alienação mental. Da prevenção ao manejo do risco: contraste e indefinição na história recente da psiquiatria / From prevention to risk management: contrast and uncertainty in recent history of psychiatry

Conselho Nacional de Desenvolvimento Científico e Tecnológico / O presente trabalho tem como objetivo contrastar a noção de prevenção elaborada no contexto norte-americano dos anos 60 com a emergência do risco como noção alternativa de viés preventivista na psiquiatria contemporânea. Para tanto, analisamos as concepções e teorias que fundamentavam a prevenção nos anos 60, investigando como um determinado modo de explicar a doença se articulou a interesses profissionais e a demandas sociais daquele período, contribuindo para elevar a prevenção à ação primordial e objetivo fundamental do campo psiquiátrico. Em seguida, analisamos as concepções e teorias neurocientíficas recentes sobre a formação psicopatológica e as correlatas propostas de manejo do risco que começam a se configurar como alternativa aos discursos preventivos que predominaram no século XX. Finalmente, discutimos as transformações que explicam o declínio da prevenção e a ascensão das práticas de manejo do risco, associando as modificações do campo psiquiátrico às alterações no campo da saúde e no contexto cultural do final do século XX e início do século XXI. Os objetivos da discussão são: contrastar os dois discursos psiquiátricos, tecendo considerações sobre as marcantes diferenças entre o predomínio da prevenção e a lógica do risco e sobre possíveis similaridades ou continuidades; e examinar, de forma exploratória, algumas das consequências que a associação entre categorias psiquiátricas, risco e práticas de saúde contemporâneas pode promover nas formas de conhecer, tratar e vivenciar a patologia mental. / The purpose of this thesis is to contrast the notion of prevention developed in the United States during the 60s with the emergence of risk as an alternative notion of preventive bias in contemporary psychiatry. Therefore, I analyze concepts and theories underpinning prevention in the 60s, investigating how a particular way of explaining disease was articulated to professional interests and social demands, contributing to raise prevention as a fundamental action and main goal of the psychiatric field. Then I analyze recent neuroscientific concepts and theories on the formation of psychopathology and the related risk management proposals that begin to take shape as an alternative to preventive discourses that prevailed in the twentieth century. Finally, I discuss the transformations that explain the decline of prevention and the rise of risk management, connecting modifications in psychiatry to changes in health practices and in the cultural context of late twentieth and early twenty-first centuries. The objectives of the discussion are: to contrast the psychiatric discourses, considering the striking differences between the prevalence of prevention and the risk logic, as well as possible similarities; and to examine, in an exploratory way, some of the alterations that the association between psychiatric categories, risk and health practices may promote in contemporary ways of knowing, experiencing and treating mental pathology.
