Membrana de biopolímero de cana-de-açúcar como remendo em arterioplastias femorais em cães

Material e método: Oito cães adultos mestiços foram anestesiados por via endovenosa com cloridrato de cetamina na dose de 2,5 mg / Kg e brometo de pancurônio na dose de 0,2 mg / Kg, após indução com tiopental sódico na dose de 12,5 mg / Kg. Foi realizada tricotomia das regiões inguinais e a seguir procedeu-se a velocimetria Doppler percutânea das artérias femorais direita e esquerda para controle pré-operatório. Sob condições de assepsia e anti-sepsia os cães foram submetidos à dissecção das artérias femorais seguida da medida do diâmetro arterial e de uma arteriotomia longitudinal com retirada de um fragmento elíptico da parede arterial medindo 1,5 cm por um terço do diâmetro da artéria. Para a correção do defeito da parede arterial, os animais foram submetidos à arterioplastia femoral bilateral com remendos da membrana de biopolímero de cana-de-açúcar no lado esquerdo (grupo experimental) e de PTFE expandido (grupo controle) no lado direito. As suturas foram contínuas com fio de prolene ® 7.0. As membranas utilizadas como remendos são constituídas de um biopolímero obtido por síntese bacteriana a partir do melaço de cana-de-açúcar, produzidas na Estação Experimental de Cana-de-Açúcar de Carpina da Universidade Federal Rural de Pernambuco e processadas para aplicação como prótese cirúrgica no Núcleo de Cirurgia Experimental do Centro de Ciências da Saúde da Universidade Federal de Pernambuco. Na primeira semana pós-operatória os cães foram submetidos a avaliação clínica diária e semanal a partir do oitavo dia. A avaliação clínica consistiu no exame dos pulsos femorais, na avaliação da marcha e na observação da presença de tumor pulsátil, hematoma ou hemorragia e infecção da ferida operatória. Após 180 dias das arterioplastias, sob anestesia geral, foi realizada tricotomia das regiões inguinais e abdominal e procedeu-se nova fluxometria Doppler percutânea das artérias femorais . Os animais foram submetidos à dissecção das artérias femorais, medido o diâmetro arterial e realizada velocimetria Doppler trans-operatória em pontos proximal e distal à arterioplastia. A seguir foi realizada laparotomia e exposição da aorta abdominal para acesso arteriográfico. Através da velocimetria Doppler e da arteriografia foi pesquisada a ocorrência de trombose, aneurisma e falso-aneurisma. Os segmentos das artérias femorais com os remendos foram retirados para estudo histopatológico e os animais foram mortos com dose tóxica de anestésico. Resultados: No período de avaliação de 180 dias, nos dois grupos, não foram observados casos de infecção da ferida operatória , dilatação, ruptura, falso-aneurisma ou trombose. Nos dois grupos foi encontrada uma resposta inflamatória crônica com neutrófilos e linfócitos. Nas superfícies externa e interna dos remendos foi encontrada fibrose. No grupo controle ocorreu invasão dos remendos de e-PTFE por fibroblastos. Conclusões: Com base nos resultados obtidos com o modelo experimental utilizado, conclui-se que a membrana de biopolímero de cana-de-açúcar pode ser utilizada como remendo em arterioplastias femorais em cães

Objetivo: Medir a pulsatilidade da parede do aneurisma de aorta humano antes e depois de sua correção endoluminal. Método: Cinco aneurismas foram submetidos à perfusão pulsátil antes e depois do implante de uma endoprótese. Resultado: o nível da coluna de água oscilou durante a pulsação com variações de 17, 16, 13, 7 e 25 cm antes da colocação da endoprótese. Depois da prótese, a oscilação diminuiu em todos os casos para 13, 12, 9, 3,5 e 23 cm, respectivamente. Conclusão: A pulsação da endoprótese é transmitida à parede do aneurisma / Objective: To measure the pulsatility of human aortic aneurysms before and after exclusion with endograft. Method: Five aneurysms were submitted to pulsatile perfusion before and after implantation of a bifurcated endograft. Result: The level of the water column oscillated during pulsation, in each case, with an amplitude of 17, 16, 13, 7 and 25 cm before the endograft insertion. After that, the amplitudes dropped to, respectively 13, 12, 9, 3.5 and 23 cm.Conclusion: Pulsation of an endograft is transmitted to the aneurysm wall even in the absence of endoleak

INTRODUÇÃO: A implantação de stent auto-expansível de nitinol para o tratamento das lesões oclusivas femoro-poplíteas tem sido associado com maus resultados a longo prazo. O everolimus administrado via oral para inibir reestenoses do stent foi investigado recentemente em animais com bons resultados, porém sua segurança e eficácia não têm sido estudada em seres humanos. O propósito deste estudo piloto foi avaliar o impacto da terapia antiproliferativa com everolimus administrado via oral por 28 dias na diminuição de reestenose após implante de stent auto-expansível de nitinol para tratamento de lesões oclusivas da artéria femoral superficial. MÉTODOS E RESULTADOS: Trinta e quatro pacientes foram recrutados para este estudo randomizado, prospectivo. O grupo que recebeu everolimus via oral foi constituído por 15 pacientes e o grupo que não recebeu medicação composto por 19 pacientes. As características basais e do procedimento foram similares entre os dois grupos. Todos os pacientes tinham isquemia crônica do membro inferior e oclusão da artéria femoral superficial (média da lesão de 83,14 mm no grupo sem medicação e 105 mm no grupo everolimus). O objetivo primário do estudo foi a redução da porcentagem média do diâmetro da reestenose intra-stent após seis meses da angioplastia avaliada por angiografia quantitativa. A porcentagem média do diâmetro das reestenoses foi 46,9% no grupo tratado com everolimus e 44,5% no grupo que não recebeu a medicação (p=0,81). Não foram observados efeitos colaterais graves nos grupos. No acompanhamento clínico aos 24 meses não houve diferenças significativas entre os grupos em relação a eventos clínicos. A patência primária, primária assistida e secundária em 24 meses, foi 42%, 74% e 79% no grupo sem medicação e 27%, 73% e 73% no grupo tratado com everolimus. CONCLUSÃO: O everolimus via oral por 28 dias em doses altas é seguro e bem tolerado, com baixo índice de efeitos colaterais, porém não é eficaz na redução da porcentagem média do diâmetro da reestenose intra-stent em pacientes com implante de stents auto-expansíveis de nitinol nas lesões oclusivas complexas da artéria femoral superficial. / INTRODUCTION: The implantation of a self-expanding of nitinol stent in the treatment of femoropopliteal occlusive lesions has been associated with a poor outcome in a long term setting. Everolimus administered orally to inhibit restenosis of the stent was investigated recently in animals with good results, but its safety and efficacy has not been studied in humans. The purpose of this pilot study was to evaluate the impact of antiproliferative therapy with everolimus administered orally for 28 days in the reduction of restenosis after implantation of self-expandable nitinol stent for treatment of occlusive lesions of the superficial femoral artery. METHODS AND RESULTS: Thirty-four patients were recruited for this randomized, prospective study. The group that received oral Everolimus was consisted of 15 patients and the group that received no medication was 19 patients. The baseline characteristics and procedure were similar in both groups. All the patients had chronic lower limb ischemia and occlusion of the superficial femoral artery (mean of the lesion of 83.14 mm in the group without medication and 105 mm in the everolimus group). The primary objective of the study was to evaluate the reduction of the average percentage of the diameter of in-stent restenosis six months after angioplasty assessed by quantitative angiography. The in-stent mean percent diameter stenosis was 46.9% in the group treated with everolimus and 44.5% in the group that received no medication (p = 0.81). There were no serious side effects seen in either group in the clinical follow up at 24 months. There was no significant difference between groups in relation to clinical events. The primary patency, assisted primary and secondary in 24 months was 42%, 74% and 79% in the group without medication and 27%, 73% and 73% in the group treated with Everolimus. CONCLUSION: Everolimus administered orally for 28 consecutive days to stent implantation in high doses proves to be safe and well tolerated, with low rate of side effects, but it is not effective in reducing the average percentage of diameter of in-stent restenosis in patients with implantation of self-expandable nitinol stent in complex occlusive lesions of the superficial femoral artery.

Tratamento endovascular de pacientes com doenças da aorta torácica: avaliação de resultados em longo prazo.

Its incidence is low, but has been growing gradually due to the increase in life expectancy of the population, that in most cases, is associated with hypertension, smoking and diabetes mellitus. Advances in diagnostic techniques have also contributed to the identification of an increasing number of cases. The development of minimally invasive endovascular procedures has been used successfully in the treatment of these diseases, including patients with no indication for conventional surgical treatment. Objective: Evaluate the long-term results of endovascular treatment of patients with diseases of the thoracic aorta underwent implantation of self-expandable stent-grafts. Methods: This prospective study evaluated 112 patients who underwent percutaneous implantation of endoprosthesis of the thoracic aorta, from October 1998 to August 2013. Self-expandable endoprosthesis stent-graft Braile Biomédica ®, made of stainless steel and nitinol were employed. The occurrence of intraoperative and postoperative primary success, endoleaks, mortality, late evolution of the endoprosthesis and survival were evaluated in long term follow-up. Results: The mean time of the procedures was 72.66 ± 43.36 minutes (range 30-240 minutes). A total of 150 self-expandable stents were implanted in 112 patients, 61 (40.66%) of stainless steel and 89 (59.33%) of nitinol. The diameter and length of the stents of stainless steel and nitinol ranged from 24-45 mm (median 33) and 70-130 mm (median 90) 22-46 mm (median 35) and between 40 and 230 mm (median 110) respectively. The number of stents implanted per patient ranged from 1 to 4 (median 1). Primary success was observed in 100 (82.14%) of 112 patients treated. Immediate mortality occurred in seven (6.25%) patients, five (4.46%) from cardiovascular causes and two (1.78%) for non-cardiovascular causes. Late mortality occurred in 31 (27.68%) patients, 10 (8.93%) from cardiovascular causes, 12 (10.71%) for non-cardiovascular causes, two (1.78%) from natural causes-seven (6.25%) with no diagnosis. There hospital type I endoleaks occurred in four patients (3.57%), type II in five (4.46%) and three type IV (2.68%). Late endoleaks type I occurred in five (4.46%) patients and type IV in three (2.68%). Twenty-two patients (19.64%) had clinical complications in the immediate postoperative period, including nine (8.03%) pulmonary complications, four (3.57%) neurological abnormalities, three (2.67%) acute renal failure, two (1.78%) infections in the surgical incision, two (1.78%) with progression to post-implantation syndrome and one (0.89%) with laceration of the arterial access. Follow-up time ranged from 1 to 179 months (median 46). The acturial survival curve was 79,3% (IC95% 67,0-91,7) at 132 months free of death from cardiovascular causes. The logistic regression analysis showed that renal failure was the only risk factor that showed a statistically significant difference. Conclusions: The low levels of intra and postoperative complications demonstrate that the treatment is safe and effective. The high rate of survival after 132 months of follow-up for these critically ill patients show the benefits of endovascular technique to treatment of thoracic aorta diseases. / Introdução: Doenças da aorta torácica, incluindo as dissecções, aneurismas, pseudoaneurismas entre outras, são condições graves que trazem sérios riscos de morbimortalidade. Sua incidência é baixa, porém, vem crescendo gradativamente em virtude do aumento da expectativa de vida da população que, na maioria dos casos, está associada à hipertensão arterial, tabagismo e diabetes. Os avanços nas técnicas diagnósticas também têm contribuído para a identificação de um número cada vez maior de casos. O desenvolvimento de procedimentos endovasculares minimamente invasivos vem sendo utilizado com sucesso no tratamento destas doenças, inclusive em pacientes sem indicação para tratamento cirúrgico convencional. Objetivo: Avaliar os resultados em longo prazo do tratamento endovascular de pacientes portadores de doenças da aorta torácica submetidos ao implante de endopróteses autoexpansíveis stent-grafts. Casuística e Métodos: Estudo prospectivo que avaliou 112 pacientes submetidos ao implante percutâneo de endoprótese na aorta torácica, no período de outubro de 1998 a agosto de 2013. Foram empregadas endopróteses autoexpansíveis stent-grafts da Braile Biomédica®, confeccionadas em aço inox e nitinol. Foram avaliadas a ocorrência de complicações intra e pós-operatórias, sucesso primário, endoleak, mortalidade, evolução tardia da endoprótese e a sobrevivência em seguimento de longo prazo. Resultados. O tempo médio dos procedimentos foi de 72,66±43,36 minutos (variação 30-240 minutos). Foram implantadas um total de 150 endopróteses autoexpansíveis em 112 pacientes, sendo 61 (40,66%) de aço inox e 89 (59,33%) de nitinol. O diâmetro e comprimento dos stents de aço inox e de nitinol variaram de 24-45 mm (mediana 33) e 70-130 mm (mediana 90), 22-46 mm (mediana 35) e 40 a 230 mm (mediana 110), respectivamente. O número de endopróteses implantadas por paciente variou de 1 a 4 (mediana 1). Sucesso primário foi observado em 100 (82,14%) dos 112 pacientes tratados. A mortalidade hospitalar ocorreu em sete (6,25%) pacientes, cinco (4,46%) por causas cardiovasculares e dois (1,78%) por causas não cardiovasculares. A mortalidade tardia ocorreu em 31 (27,68%) pacientes, 10 (8,93%) por causas cardiovasculares, 12 (10,71%) por causas não cardiovasculares, dois (1,78%) por causas naturais e sete (6,25%) sem diagnóstico. No período hospitalar, ocorreu endoleak do tipo I em quatro pacientes (3,57%), tipo II em cinco (4,46%) e tipo IV em três (2,68%). Endoleak tardio do tipo I ocorreu em cinco (4,46%) pacientes e do tipo IV em três (2,68%). Vinte e dois pacientes (19,64%) apresentaram complicações clínicas no pós-operatório imediato, incluindo nove (8,03%) complicações pulmonares, quatro (3,57%) alterações neurológicas, três (2,67%) pacientes com insuficiência renal aguda, duas (1,78%) incisões cirúrgicas com infecção, duas (1,78%) síndromes pós-implante e uma (0,89%) laceração do acesso arterial. O tempo de seguimento variou de 1 a 179 meses (mediana 46). A curva atuarial de sobrevivência foi de 79,3% (IC95% 67,0-91,7) aos 132 meses livre de mortalidade por causas cardiovasculares. A análise de regressão logística mostrou que a insuficiência renal foi o único fator de risco para mortalidade que apresentou diferença estatisticamente significante. Conclusões. Os baixos índices de complicações intra e pós-operatórias demonstram que o tratamento é seguro e eficaz. O alto índice de sobrevivência após 132 meses de seguimento para estes pacientes graves mostram os benefícios da técnica endovascular no tratamento das doenças da aorta torácica.

Estudo experimental comparativo de remendos arteriais de polidimetilsiloxano com reforço de tecido de poliéster (PDMSr) versus politetrafluoretileno expandido (PTFEe) em aorta de coelhos / Patch of polydimethylsiloxane reinforced with polyester fabric for aortic angioplasty in rabbits

Introdução: Apesar de bons resultados descritos na literatura, o substituto ideal para a utilização como remendo no fechamento arterial ainda não existe. Por este motivo, ainda há espaço para a busca por remendo que seja biocompatível, e que apresente facilidade de manuseio e resultados satisfatórios. Avaliamos remendos arteriais de silicone (polidimetilsiloxano com reforço em poliéster - PDMSr) em comparação com remendos arteriais de PTFEe. Objetivo: O objetivo deste trabalho é comparar, em modelo experimental, em coelhos, os resultados de remendos arteriais feitos em PDMSr com remendos de PTFEe. Materiais e Método: A amostra foi definida em 10 animais que completassem todas as etapas da pesquisa em cada grupo. Os animais foram submetidos à laparotomia mediana e abertura longitudinal da aorta de aproximadamente 8mm, realizando-se o seu fechamento com remendo do grupo selecionado, seguido do fechamento por planos. Os animais foram mantidos em biotério até o 60º PO, quando, então, realizou-se arteriografia de controle, e análise macro e microscópica de peça. Resultados: Para se atingir a amostra desejada, foram necessários 12 procedimentos no grupo PDMSr e 16 no grupo PTFEe. Ocorreram 2 óbitos no grupo PDMSr e 6 no grupo PTFEe. Apesar do número maior de óbitos no grupo PTFEe não houve diferença estatística na sobrevida entre os grupos. Um animal do grupo PDMSr apresentou monoparesia em pata posterior direita e um animal do grupo PTFEe apresentou hérnia incisional. Não houve diferença estatística nas complicações entre os grupos. O tempo operatório foi estatisticamente maior no grupo PTFEe quando comparamos todos os animais, fato que não se repetiu quando excluímos os animais que faleceram. O peso e o tempo operatório foram estatisticamente maiores nos animais que morreram. Todos os animais que chegaram ao final do tempo de estudo apresentavam aorta patente. Na análise macroscópica da peça, houve, estatisticamente, maior reação tecidual periprótese no grupo PTFEe. A microscopia eletrônica de varredura evidenciou cobertura de todo o remendo de PDMSr por tecido similar ao endotélio, enquanto, nos remendos de PTFEe, o crescimento limitou-se às bordas da linha de sutura e ilhas isoladas no seu centro. Conclusão: O material estudado apresentou resultados comparáveis ao do PTFEe, porém com menor reação tecidual local e maior proliferação celular para a luz do vaso. Apesar de novos estudos serem necessários, inclusive para avaliação de uso em humanos, o presente estudo apresenta resultados promissores que encorajam a continuidade de sua pesquisa / Introduction: Although good results are reported for various materials for use as patches for arterial closure, as yet none of these is ideal. Therefore, research is continuing into development of a patch that is biocompatible and provides ease of handling, while having satisfactory outcomes. A new silicone arterial patch (polydimethylsiloxane reinforced with polyester fabric, PDMSr) was compared with patches made of expanded polytetrafluoroethylene (ePTFE). Objective: To compare the outcomes between arterial patches made of PDMSr with those made of ePTFE, in an experimental rabbit model. Materials and Method: Rabbits were placed in two groups, and received either PDMSr or ePTFE arterial patches (PDMSr group and ePTFE group, respectively). The animals underwent laparotomy and longitudinal opening of the aorta, which was then closed with the selected patch, followed by suture of all layers. The animals were kept in their cages until the 60th postoperative day, when arteriography, removal of the aorta, and macroscopic and optic and scanning electron microscopic analyzes of the aorta were performed. Ten rabbits from each group that had completed all stages of the research were included in analyses. Results: Twelve procedures were performed in the PDMSr group and 16 in the ePTFE group. There were 2 deaths in the PDMSr group and 6 in the ePTFE group. Despite the higher number of deaths in the ePTFE group, there was no statistical difference in survival rate between the groups. One animal in the PDMSr group developed monoparesis in its right hind paw and 1 animal in the ePTFE group had an incisional hernia. There was no statistical difference in complications between the groups. The operative time was significantly longer in the ePTFE group when all animals were included in analysis, but not when animals that died were excluded. Body weight was significantly greater and operative time was significantly longer in animals that died. Postsurgical aortic patency in the survival animals was 100% in both groups by arteriography. Macroscopically, tissue reaction around the prosthesis was greater in the ePTFE group (statistically significant). Microscopically, the PDMSr patches were entirely covered with a cellular endothelium-like tissue, while tissue growth on the ePTFE patches was limited to the edges of the suture line and to isolated central islands. Conclusion: The two materials showed comparable outcomes; however, PDMSr showed cellular proliferation to the entire graft, and less local inflammatory reaction compared with ePTFE. Although further studies are required, including assessment in humans, the results of the present study indicate that PDMSr shows promise as an arterial patch material

Estudo experimental comparativo de remendos arteriais de polidimetilsiloxano com reforço de tecido de poliéster (PDMSr) versus politetrafluoretileno expandido (PTFEe) em aorta de coelhos / Patch of polydimethylsiloxane reinforced with polyester fabric for aortic angioplasty in rabbits

Introdução: Apesar de bons resultados descritos na literatura, o substituto ideal para a utilização como remendo no fechamento arterial ainda não existe. Por este motivo, ainda há espaço para a busca por remendo que seja biocompatível, e que apresente facilidade de manuseio e resultados satisfatórios. Avaliamos remendos arteriais de silicone (polidimetilsiloxano com reforço em poliéster - PDMSr) em comparação com remendos arteriais de PTFEe. Objetivo: O objetivo deste trabalho é comparar, em modelo experimental, em coelhos, os resultados de remendos arteriais feitos em PDMSr com remendos de PTFEe. Materiais e Método: A amostra foi definida em 10 animais que completassem todas as etapas da pesquisa em cada grupo. Os animais foram submetidos à laparotomia mediana e abertura longitudinal da aorta de aproximadamente 8mm, realizando-se o seu fechamento com remendo do grupo selecionado, seguido do fechamento por planos. Os animais foram mantidos em biotério até o 60º PO, quando, então, realizou-se arteriografia de controle, e análise macro e microscópica de peça. Resultados: Para se atingir a amostra desejada, foram necessários 12 procedimentos no grupo PDMSr e 16 no grupo PTFEe. Ocorreram 2 óbitos no grupo PDMSr e 6 no grupo PTFEe. Apesar do número maior de óbitos no grupo PTFEe não houve diferença estatística na sobrevida entre os grupos.
