Substituição do ligamento redondo por implante de fáscia lata bubalina preservada ou pino transarticular no tratamento da luxação coxofemural em cães.

Afecções ortopédicas são freqüentes em pequenos animais e dentre essas afecções destaca-se a luxação coxofemoral – a mais freqüente dentre as luxações. Devido a tal incidência, a luxação coxofemoral tornou-se objeto de inúmeras pesquisas em medicina veterinária, sendo o tratamento cirúrgico o mais efetivo para sua correção. Ainda não foi, no entanto, desenvolvida uma técnica cirúrgica preferencial, amplamente difundida nas rotinas hospitalares. O presente trabalho avalia comparativamente o implante de fáscia lata bubalina e de pino de Steinmann transarticular para a estabilização de articulações coxofemorais luxadas. Foram utilizados 26 cães, separados em dois grupos que foram também, por sua vez, subdivididos. O primeiro grupo foi denominado “grupo Experimental” e reuniu 16 animais: oito pertencentes ao subgrupo “pino” e oito ao subgrupo “fáscia”. O segundo grupo foi denominado “grupo Rotina” e constituído por dez animais provenientes da rotina de atendimentos do Hospital de Clínicas Veterinárias da Universidade Federal do Rio Grande do Sul (UFRGS), sendo igualmente subdividido nos subgrupos “pino” e “fáscia”, com cinco animais cada um. Os cães foram submetidos às respectivas técnicas de estabilização e acompanhados clínica e radiograficamente durante 60 dias, findos os quais os animais do grupo Experimental passaram por avaliação macroscópica direta das articulações, além da histopatologia e teste de tensiometria. A principal vantagem da fáscia respeita à deambulação dos animais, que apresentaram evolução pós-operatória significativamente precoce em relação ao grupo submetido ao implante do pino de Steinmann transarticular, além de menor grau de atrofia muscular. Os testes de tensiometria, avaliações radiográficas e exames histopatológicos não apresentaram diferenças estatisticamente significativas entre os grupos, evidenciando também que ambas as técnicas não geraram alterações deletérias à articulação operada. Destarte, conclui-se que a técnica de estabilização da articulação coxofemoral com implante de fáscia lata é eficaz e vantajosa quando comparada à técnica do pino transarticular. / The orthopedics disorders are very frequent in small animals and among them we can find the coxofemoral luxation as the most frequent disease. Because of a high incidence it became an important subject for veterinary researches. Surgery is the most effective treatment in cases of coxofomoral luxation, although the techniques that already exist are not so used in the clinics routine because of some difficulties these techniques present. In this paper the bubaline fascia lata implant and Steinmann transarticular pin were compared in surgeries for coxofemoral luxation. Twenty-six animals were used, divided into two groups that were subdivided. The first group was called “Grupo Experimental” and was formed by 16 animals, 8 animals in the subgroup called “pino” and the other 8 animals in the subgroup “fáscia”. The second group was called “Rotina” because these animals came from the routine of the clinic sector of Universidade Federal do Rio Grande do Sul Veterinary Hospital, and these groups were also subdivided in the subgroups “pino” and “fáscia”, with 5 animal each. These groups were submitted to the mentioned above stabilization techniques and after that, they were observed clinically and radiografically during 60 days. After that, it was realized a macroscopic study in the articulations of “Grupo Experimental”, besides an histopatological exam and a tensiometry test. The results showed that the technique with the bubaline fascia lata implant proved to be really efficient and it has some advantages comparing to the transarticular pin technique. The most important advantage is related to the deambulation, where the animals in this group showed a better evolution after the surgery. The level of muscular atrophy in this group was less marked. The tensiometry tests, the radiographic and histopatological exams did not present important differences between the two groups, but they were useful to show that the two techniques did not cause bad alterations in the worked articulation. In this sense it can be concluded that the stabilization of the coxofemoral articulation using bubaline fascia lata implant is a better technique comparing to the transarticular pin technique due to its advantages, mainly related to the deambulation and the muscular atrophy.

Após o aprofundamento do sulco troclear foi realizado o implante de um grampo metálico confeccionado com um pino intramedular de Steinmann, imediatamente proximal à crista troclear medial, dando continuidade à mesma, numa extensão equivalente à trajetória da patela. Em seguida a cápsula articular foi suturada em um padrão Sultan, iniciando-se pelo ligamento femuropatelar lateral, e a pele com pontos em U com fio de nylon 3-0. O tempo médio de apoio do membro foi de 5 dias, sendo que aos 10 dias de pós-operatório todos os cães apoiavam normalmente o membro. As avaliações radiográficas no período pós-operatório imediato, aos 30, 60 e 90 dias não revelaram alterações referentes a presença do grampo metálico, tais como áreas de osteólise ou migração do grampo. / This work shows a new technique for correction of patellar luxation, mainly for the pacients with high hiding patella. Twelve adult and healthy mongrel dogs with average weigh of 10 Kg were used for this study. The surgical technique consisted of skin and articular capsule parapatellar incision and after that was done the medial displacement of patella from trochlear groove which was deepened using a cylindrical rasp for metal, to remove the articular cartilage and subchondral bone tissue in a depth equivalent to the half of the patella thickness. After this, an implant of a metallic staple, confectioned using a Steinmamm intramedullary pin, was used proximal to the medial tochlear crest giving continuity in an extension equivalent to patella trajectory. Then the articular capsule was sutured using Sultan pattern from the lateral femuropatellar ligament and the skin closure was performed by U pattern, both with nylon 3-0. The average time of support of the affected limb was five days and after ten days of postoperative period, all dogs supported it normally. The radiological evaluations at immediate postoperative period and at 15, 30, 60 and 90 days, didn't show any alterations related to the presence of the metallic staple, like osteolytic lesions or migration of the staple. The histopathological evaluations of the femoral tochlea of the limb submitted to the surgery presented changes of the cartilaginous tissue for a fibrocartilagenous one. The histology of the patella and the implant site of the metallic staple didn't show any abnormalities when compared to the contra-lateral limb.

It is characterized by the medial displacement of the patella from its normal position in the trochlear groove, and surgical treatment is recommended from grade two dislocation. The objective of this study was to test if the creation of a complementary patellar ligament, manufactured from shredding of surplus retinaculum is a good technique to reinforce the patellar stabilization for the treatment of medial patellar luxation in dogs. We used 16 knees of 13 dogs, regardless of gender or age who had medial patellar luxation grades two, three and four, and among these, 8 were part of the B group in which we used the technique of synthetic patellar ligament for comparison. The animals were evaluated 30 and 90 days after surgery, for the parameters of pain, willingness to lift the contralateral limb, lameness and weight-bearing on the affected limb by the nonparametric Wilcoxon test for differences between ordered pairs. Since all animals showed functional recovery in 90 days, it is concluded that the proposed technique is also an appropriate surgical option for the strengthening of patellar stabilization in the treatment of medial patellar luxation in dogs. / A luxação patelar medial é um transtorno ortopédico frequentemente visto na clínica veterinária, acometendo especialmente cães de pequeno porte. Caracteriza-se pelo deslocamento medial da patela de sua posição normal no sulco troclear, sendo seu tratamento cirúrgico recomendado a partir do grau dois de luxação. Objetivou-se neste trabalho verificar se a criação de um ligamento patelar complementar confeccionado através do retalhamento do retináculo excedente constitui uma boa técnica de reforço da estabilização patelar para o tratamento da luxação medial de patela em cães. Foram utilizados 16 joelhos de 13 cães, sem distinção de sexo ou idade que apresentavam luxação de patela medial de graus dois, três e quatro, sendo que entre esses, 8 fizeram parte do grupo B onde se utilizou a técnica do ligamento patelar sintético para fazer a comparação. Os animais foram avaliados com 30 e 90 dias após a cirurgia, em relação aos parâmetros dor, disposição para levantar o membro contra-lateral, claudicação e suporte de peso no membro afetado. As diferenças entre os pares para esses parâmetros foram submetidas ao teste não paramétrico de Wilcoxon, evidenciando aos 30 dias melhor evolução estatística no grupo B, mas igual desempenho para ambos os grupo aos 90 dias. Conclui-se então, que a técnica proposta também constitui opção cirúrgica apropriada de reforço na estabilização patelar para o tratamento da luxação patelar medial em cães.

Efeito do alendronato de sódio em molares de rato em formação após luxação lateral / Effect of sodium alendronate on developing molars of young rats after lateral luxation

Os bisfosfonatos são drogas capazes de inibir a reabsorção óssea por meio de seu efeito direto sobre as células ósseas, interferindo na dinâmica dos tecidos mineralizados. O alendronato (ALN), um tipo de bisfosfonato nitrogenado, foi utilizado com o objetivo de investigar os seus efeitos sobre os tecidos dentários e periodontais após luxação lateral de molares com as raízes em desenvolvimento. Ratos Wistar com 21 dias de idade tiveram os segundos molares superiores luxados lateralmente. Doses diárias de 2,5 mg / kg de ALN começaram no dia seguinte à luxação; os controles receberam solução salina estéril. As maxilas foram fixadas, descalcificadas e incluídas em parafina ou em resina Spurr 7, 14 e 21 dias pós-luxação. Os cortes foram corados com H & E, incubados por histoquímica TRAP e imuno marcados para osteopontina (OPN), bem como para análise ultraestrutural. Após 21 dias, o ápice dos molares luxados sem ALN estava aberto e desorganizado, coberto por uma camada irregular de cemento celular. Os molares luxados dos animais tratados com ALN apresentaram alguns locais de anquilose, bem como lacunas de reabsorção na superfície do cemento. Os osteoclastos TRAP positivos foram mais numerosos no grupo ALN, apesar de sua aparência latente e sua localização, afastados das trabéculas ósseas, em relação aos controles, achado que foi confirmado com a análise ultraestrutural. A imunomarcação de OPN revelou uma linha grossa imunopositiva na dentina, que deve ter surgido a partir do momento da luxação, enquanto que as amostras tratadas com ALN não apresentaram alterações na dentina. Os resultados indicam que o alendronato inibe algumas alterações na dentina e na formação do cemento, induzidas pelo trauma dental de luxação. / Bisphosphonates are drugs that inhibit bone resorption through its direct effect on bone cells, interfering with the dynamics of mineralized tissues. Alendronate (ALN), a nitrogenated bisphosphonate, was used in order to investigate their effects on dental and periodontal tissues after lateral dislocation of molars with developing roots. Twenty one days old Wistar rats had their second molars laterally l. Daily doses of 2.5 mg / kg ALN started the day following the dislocation, while controls received saline solution. The maxillae were fixed, decalcified and embedded in paraffin or in Spurr resin after 7, 14 and 21 days post-dislocation. The sections were stained with H & E, incubated for TRAP, immunolabeled for osteopontin (OPN), and ultrastructurally analyzed by transmission electron microscopy. After 21 days, the apex of the luxated molar without ALN was open and disorganized, covered by an irregular layer of cellular cementum. The luxated molar from ALN-treated animals showed some areas of ankylosis and resorption lacunae on the cementum surface. TRAP-positive osteoclasts were more numerous in the ALN group, despite their latent appearance compared to controls, a finding that was ultrastructurally confirmed. OPN immunostaining revealed a thick immunopositive line in dentin, which must be resultant from the moment of dislocation, while the samples treated with ALN showed no changes in dentin. The results indicate that alendronate inhibits some changes in dentin and cementum formation induced by dental trauma of lateral luxation.

O mal alinhamento do membro pélvico de cães contribui para o desenvolvimento ósseo anormal e deslocamento medial ou lateral do aparelho extensor do quadríceps. De acordo com o aumento da incidência da luxação de patela em cães de médio e grande porte, o estudo do alinhamento do membro pélvico e das deformidades ósseas associadas recebeu destaque. Assim, o conhecimento do eixo mecânico e do alinhamento do membro pélvico, bem como de seus ângulos articulares no plano frontal, é essencial para o diagnóstico, classificação, quantificação do grau de desvio, e para o delineamento do tratamento cirúrgico corretivo. O presente estudo teve como objetivo mensurar e correlacionar os ângulos articulares do fêmur e da tíbia com o diferentes graus de luxação de patela. Foram obtidos exames radiográficos independentes do fêmur e da tíbia, nas projeções craniocaudal e caudocranial, respectivamente, de cães com e sem luxação de patela. As imagens radiográficas foram processadas digitalmente produzindo 92 exames radiográficos do membro pélvico em extensão total que foram divididos em grupos controle e luxação de patela. O grupo luxação foi subdividido de acordo com o tipo de luxação, medial ou lateral. Foram determinados os ângulos articulares, alinhamento e o eixo mecânico do membro pélvico correlacionando os resultados com a luxação de patela. Concluiu-se que o grau de luxação de patela interferiu na magnitude do desvio do eixo mecânico e no valor do ângulo mecânico femorotibial, e que o valor do ângulo mecânico lateral distal do fêmur foi maior nos cães com luxação de patela, sugerindo a presença de deformidade femoral distal. / The misalignment of the pelvic limb dogs contributes to abnormal bone development and medial or lateral displacement of the extensor mechanism. Increased incidences of the patellar dislocation in medium and large dogs stimulated alignment study and the associated bone deformities. Thus, knowledge of the mechanical axis and the pelvic limb alignment, as well as their joint angles in the frontal plane is essential for the diagnosis, classification, quantification of the degree of deviation and for the design of corrective surgery. The purpose of this study was to measure the femoral and tibial articular angles and and assess the relationship between it and the different degrees of patellar luxation. Independent radiographs of the femur and tibia of dogs with or without patellar luxation were obtained in the craniocaudal and caudocranial projections, respectively. The radiographic images were digitally processed producing 92 x-ray exams of the pelvic limb in full extension that were divided into control and patella luxation groups. The patellar luxation group was divided according to the type of dislocation, medial or lateral, and dislocation grade. The joint angles, alignment and the mechanical axis of the pelvic limb were determined and the results were correlated with the disease. In conclusion, the degree of patellar dislocation interfered with the magnitude of the mechanical axis deviation and the value of mechanical femorotibial angle, the value of the mechanical lateral distal femoral angle was higher in dogs with patellar dislocation, suggesting the presence of femoral distal deformity.

De acordo com o aumento da incidência da luxação de patela em cães de médio e grande porte, o estudo do alinhamento do membro pélvico e das deformidades ósseas associadas recebeu destaque. Assim, o conhecimento do eixo mecânico e do alinhamento do membro pélvico, bem como de seus ângulos articulares no plano frontal, é essencial para o diagnóstico, classificação, quantificação do grau de desvio, e para o delineamento do tratamento cirúrgico corretivo. O presente estudo teve como objetivo mensurar e correlacionar os ângulos articulares do fêmur e da tíbia com o diferentes graus de luxação de patela. Foram obtidos exames radiográficos independentes do fêmur e da tíbia, nas projeções craniocaudal e caudocranial, respectivamente, de cães com e sem luxação de patela. As imagens radiográficas foram processadas digitalmente produzindo 92 exames radiográficos do membro pélvico em extensão total que foram divididos em grupos controle e luxação de patela. O grupo luxação foi subdividido de acordo com o tipo de luxação, medial ou lateral. Foram determinados os ângulos articulares, alinhamento e o eixo mecânico do membro pélvico correlacionando os resultados com a luxação de patela. Concluiu-se que o grau de luxação de patela interferiu na magnitude do desvio do eixo mecânico e no valor do ângulo mecânico femorotibial, e que o valor do ângulo mecânico lateral distal do fêmur foi maior nos cães com luxação de patela, sugerindo a presença de deformidade femoral distal. / The misalignment of the pelvic limb dogs contributes to abnormal bone development and medial or lateral displacement of the extensor mechanism. Increased incidences of the patellar dislocation in medium and large dogs stimulated alignment study and the associated bone deformities. Thus, knowledge of the mechanical axis and the pelvic limb alignment, as well as their joint angles in the frontal plane is essential for the diagnosis, classification, quantification of the degree of deviation and for the design of corrective surgery. The purpose of this study was to measure the femoral and tibial articular angles and and assess the relationship between it and the different degrees of patellar luxation. Independent radiographs of the femur and tibia of dogs with or without patellar luxation were obtained in the craniocaudal and caudocranial projections, respectively. The radiographic images were digitally processed producing 92 x-ray exams of the pelvic limb in full extension that were divided into control and patella luxation groups. The patellar luxation group was divided according to the type of dislocation, medial or lateral, and dislocation grade. The joint angles, alignment and the mechanical axis of the pelvic limb were determined and the results were correlated with the disease. In conclusion, the degree of patellar dislocation interfered with the magnitude of the mechanical axis deviation and the value of mechanical femorotibial angle, the value of the mechanical lateral distal femoral angle was higher in dogs with patellar dislocation, suggesting the presence of femoral distal deformity.

Modélisation du fonctionnement de la Prothèse Totale de Hanche Double Mobilité - Compréhension des comportements biomécanique et tribologique. / Modeling the functioning of Dual Mobility Total Hip Arthroplasty – Comprehension of biomechanical and tribological behaviors

La Prothèse Totale de Hanche Double Mobilité (PTH-DM) a été inventée pour conjuguer l’usure faible du concept de low friction tout en proposant une solution au problème de la luxation. La Double Mobilité présente différentes composantes et ses indications sont diverses.Une étude clinique à long terme a permis de définir les avantages et échecs de ce concept. Une complication spécifique est la Luxation Intra Prothétique (LIP) ou usure de la collerette de rétention de l’insert en polyéthylène. Différentes formes peuvent être rencontrées.L’objectif principal de notre travail était de comprendre le fonctionnement de la Double Mobilité, c’est-à-dire le rôle respectif des 2 mobilités et de la collerette ainsi que leurs profils d’usure.Les objectifs secondaires étaient de comprendre les échecs, le descellement de cupule et la luxation intra-prothétique et de définir des pistes d’optimisation de l’usure.Plusieurs techniques d’analyse d’usure, à différentes échelles ont été élaborées. La profilométrie mécanique permet d’étudier la rugosité de surface, la microscopie électronique à balayage recherche les mécanismes d’usure. La cartographie CMM permet d’appréhender les paramètres de forme et d’ondulation. Nous proposons une nouvelle technique pour augmenter l’analyse qualitative de l’usure. L’analyse FE complète ces moyens, qui ont été appliquées sur des explants. 3 groupes d’explants, constitués par les inserts ayant fonctionnés plus de 15 ans sans LIP, les inserts issus de LIP et les inserts récents, le dessin et le matériau de l’insert ayant été modifiés.Les trois groupes ont été comparés. La LIP a pu être identifiée comme liée à l’usure de la collerette. Cette usure est majoritairement composée par l’usure externe de la collerette, elle-même liée aux propriétés du col de la tige fémorale.Les explants de plus de 15 ans ont présenté une usure d’environ 45 mm3/an, comparable aux implants métal-polyéthylène. Les méthodes standard d’analyse d’usure radiographique ne peuvent pas être appliquées à l’analyse des PTH-DM. Les dimensions de la cupule métallique et les caractéristiques du col fémoral prothétique étaient les seuls facteurs influant sur l’incidence de LIP. Une ébauche de modélisation de fonctionnement de la PTH-DM a pu être proposée en se basant sur les données des explants. La deuxième mobilité, par son rôle de réserve de mobilité, permet de diminuer l’usure globale et doit être encouragée. La collerette de rétention constitue également un point clé du concept et possède un rôle dans la survie. / Dual Mobility Total Hip Arthroplasty (DM-THA) was invented to combine the low wear of low friction while providing the solution to the problem of dislocation. Dual Mobility has different components and indications are diverse. A clinical long-term study of a PTH-DM first line has identified the benefits and failures of this concept. A specific complication is Intra Prosthetic Dislocation (IPD. Different types of IPD can be encountered. The main objective of our work was to understand the functioning of the Dual Mobility, the respective roles of the two mobilities and retention and their wear profiles. Secondary objectives were to understand the failures, loosening of the cup and intra-prosthetic dislocation and define an optimization of wear. Material and Methods: Several wear analysis techniques at different scales have been developed. Mechanical profilometry was used to study the surface roughness, the scanning electron microscopy researched the mechanisms of wear. CMM allowed us to understand the shape and waviness parameters. We suggested a new technique to increase the qualitative analysis of wear. The FE analysis would complete these means, which were applied to the explants. 3 groups of explants, formed by the liners having functioned for over 15 years without IPD, IPD and more recent liners, the design and material of the insert having been changed. Results: The three groups were compared. IPD has been identified as related to wear of the rim. This wear was mainly composed by the wear of the outer side of the rim, which was itself related to the properties of the neck of the femoral stem. Explants over 15 years showed wear about 45 mm3/year, which was at least comparable to standard metal on polyethylene implants. Standard wear analytical methods could not be applied to the analysis of DM-THA. The dimensions of the metal back and characteristics of the prosthetic femoral neck were the only factors affecting the incidence of IPD. Discussion: A schematic model of the functioning of DM-THA has been given based on the data of the explants. The second mobility, through its role as a reserve of mobility, reduces the overall wear and should be encouraged. The retentive rim is also a key point in the design and has a role in the implant survival. The wear data were still incomplete and there will be further work to improve our work at the statistical level.
