Instituto Oswaldo Cruz. Rio de Janeiro, RJ, Brasil / Os achados clínicos da leishmaniose visceral canina (LVC), assim como os da leishmaniose visceral humana (LVH) são variáveis e inespecíficos. Apesar da LVC se apresentar como uma doença que pode ser fatal e crônica, existem poucos estudos descrevendo as alterações clínicas e histopatológicas destas infecções. Desta forma, evidenciando a necessidade de obtenção de dados que pudessem contribuir nesses estudos, o objetivo deste trabalho foi estudar as alterações clínicas e oculares presentes em 25 cães com leishmaniose visceral provenientes da localidade Cidade Operária, São Luís - Maranhão/Brasil através da realização de avaliação clínica, testes oftalmológicos (Schirmer e colírio fluoresceína) além de dosagens hematológicas e bioquímicas, isolamento e caracterização do parasito em amostras de medula óssea, baço e humor aquoso, além de PCR. Os resultados mostraram que linfoadenopatia foi o sintoma mais frequente, seguido de caquexia, alopecia, lesão de pele, onicogrifose e hepatoesplenomegalia. As manifestações clínicas oculares observadas com maior frequência foram: presença de algum tipo de secreção e conjuntivite, seguido de blefarite, uveíte e opacidade de córnea. Oito animais foram positivos no teste de Schirmer e sete no colírio de fluoresceína indicando a presença de ceratoconjuntivite seca e úlcera de cornea, respectivamente As alterações hematológicas mais frequentes foram anemia e trombocitopenia, aumento sérico de proteínas totais e globulina e a diminuição de albumina. A alteração mais frequente observada em todos os animais foi a presença de infiltrado inflamatório do tipo plasmocitário. A análise histopatológica dos olhos dos cães mostrou lesões nas pálpebras, esclera, glândulas e musculatura retrorbital. Tendo a presença do parasito sido evidenciada em 16% dos animais. Estes resultados mostram que as manifestações oftalmológicas estão presentes em animais com LVC e podem ocorrer concomitantemente com outros sinais sistêmicos da doença. É importante considerar a leishmaniose como uma doença sistêmica, a qual afeta diferentes órgãos e sistemas. Sugerimos assim, que em áreas endêmicas, este agravo deve ser incluído como diagnóstico diferencial de cães com lesões oculares ou de etiologia desconhecida / Abstract: Clinical findings of canine visceral leishmaniasis (CVL), as well as of human visceral leishmaniasis (HVL) are variable and nonspecific. Although VC present as a disease that can be fatal and chronic few studies describing the clinical and histopathological changes of these infections. Thus, highlighting the need to obtain data that could contribute to these studies, the objective of this work was to study the clinical and ocular abnormalities present in 25 dogs with visceral leishmaniasis from the locality City Workers, Sao Luis - Maranhao / Brazil by conducting clinical, ophthalmological test (fluorescein drops and Schirmer) and hematological and biochemical measurements, isolation and characterization of the parasite in samples of bone marrow, spleen, and aqueous humor, and PCR. The results showed that lymphadenopathy was the most common symptom, followed by cachexia, alopecia, skin lesion, onychogryphosis, and hepatosplenomegaly. Ocular clinical manifestations observed most frequently were the presence of some type of secretion and conjunctivitis, followed by blepharitis, uveitis and corneal opacity. Eight animals were positive in the Schirmer test and seven in drops of fluorescein indicating the presence of keratoconjunctivitis sicca and cornea ulcer, respectively The most common hematologic abnormalities were anemia and thrombocytopenia, increased serum total protein and globulin and decreased albumin. The most frequent alteration observed in all animals was the presence of inflammatory infiltrate of plasma cell type. Histopathological analysis dogs eyes showed lesions on the eyelids, sclera, glands and muscles retrorbital. Since the presence of the parasite was observed in 16% of animals. These results show that the ocular manifestations are present in animals with LVC and can occur concurrently with other signs of systemic illness. It is important to consider leishmaniasis as a systemic disease, which affects different organs and systems. We suggest therefore that in endemic areas, this grievance should be included in the differential diagnosis of dogs with eye injuries or of unknown etiology

É importante que todo profissional de saúde tenha um conhecimento básico a respeito disso. A incidência de complicações oculares decorrentes da hanseníase tem diminuído devido ao diagnóstico precoce e instituição da PQT, porém elas ainda acontecem. As manifestações oculares mais frequentes nos hansenianos são: madarose, ceratite (devido à triquíase, lagoftalmo, diminuição ou ausência de sensibilidade corneana e secundário à infecção), conjuntivites e uveítes. Objetivos: Identificar manifestações oculares relacionadas com a hanseníase em pacientes recém diagnosticados, antes e durante o tratamento da hanseníase. Metodologia: Foi realizado um estudo de coorte prospectivo observacional, com pacientes de hanseníase no HUCAM. Foram recrutados 42 pacientes de Dezembro de 2007 à Julho de 2009, e submetidos a exame oftalmológico segundo protocolo pré estabelecido. Os pacientes foram examinados no momento do diagnóstico da hanseníase e no 3° e 6° mês de tratamento com poliquimioterapia. Resultados: A acuidade visual variou de 0,1 a 1,0 em cada olho (média de 0,7); manifestações oculares ocorreram em 39 pacientes (92,9%), porém, se consideradas as mais frequentemente relacionadas com a hanseníase, a freqüência foi de 23,8% .Redução do BUT e do Teste de Schirmer 1 foram as manifestações oculares mais frequentes (92,9% e 42,9% respectivamente). As manifestações oculares mais frequentes relacionadas com a hanseníase foram madarose de superclio (14,3%), seguida pela sensibilidade corneana diminuída e madarose de cílios (11,7%). Ceratite puntata estava presente 4,8%. Durante o período de 6 meses de seguimento, 9,5% dos pacientes apresentou aumento da PIO, 2,4% atrofia iriana e 9,5% ceratite. Conclusões: Houve uma maior prevalência de envolvimento ocular nas formas multibacilares em comparação com as formas paucibacilares. Madarose dos cílios, supercílio, sensibilidade corneana diminuída e atrofia da íris foram todos encontrados em níveis inferiores aos anteriormente descritos. Estes resultados sugerem que o envolvimento ocular no momento do diagnóstico de hanseníase foi ainda na fase inicial. A frequência de manifestações oculares e o fato de elas poderem ocorrer mesmo durante o tratamento reafirmam a necessidade de esses pacientes serem avaliados por um oftalmologista periodicamente. / Introduction: Leprosy is the disease that mostly frequently gives rise to disorders of the eye, so it is important that all professionals who treat leprosy patients have a basic understanding about these. Eye complications in leprosy have decreased, but they still happen. The ocular features most frenquently seen in leprosy patients are: madarosis, keratitis (due to a combination of trichiasis, lagophthalmos, loss of corneal sensation, and secondary infection), conjunctivitis, scleritis, episcleritis and uveitis. Objectives: To find out the presence of ocular manifestations in patients with leprosy before and during treatment. Methods: A cross-sectional prevalence survey was carried out with leprosy patients at University Hospital Cassiano Antônio de Morais in Vitória, Brazil. Forty two newly diagnosed, untreated leprosy patients were recruited from December 2007 to July 2009, and subjected to eye examination, in accordance with pre established protocol.They were examined before the beginning the leprosy treatment and followed up for 6 months. The data were crossed and analyzed statistically Results The visual acuity ranged from 0.1 to 1.0 at each eye (Mean 0.7); ocular manifestations occurred in 39 patients (92.9%), however considered due to leprosy in 10 (23.8%). Over all, reduction in tear film break-up time and in production of tear film were the most prevalent ocular complication (92.9% and 42.9 % respectively). The most prevalent ocular complication often related to leprosy was madarosis of superclia (14.3%), corneal sensitivity diminished and madarosis of cilia (11.7%). Punctate keratitis was presented in 2 patients (4.8%). During the follow up, 9,5% of patients had increased IOP, 2,4% íris atrophy and 9,5% puctata keratitis. Conclusions: There was a greater prevalence of ocular involvement in the multibacillary forms compared to the paucibacillary forms. Madarosis of the cilia, supercilia, diminished cornea sensitivity, involvement of the cornea and iris were all found at levels below those previously described. These results suggest that ocular involvement at the moment of diagnosis of leprosy was still in the initial stage. The prevalence of ocular complications and the fact that they can occur even during treatment, reassert the necessity of these patients be assessed by an ophthalmologist.

Objetivo: Identificar alterações na CFNR e no NO, por meio da tomografia de coerência óptica (OCT), em pacientes com glaucoma primário de ângulo aberto (GPAA), submetidos a tratamento clínico e cirúrgico (trabeculectomia). Métodos: Foi realizada análise prospectiva de 37 olhos de 25 pacientes avaliados antes e após os tratamentos por meio da OCT, num período pré-tratamento (1 semana a 1 mês) e com 1, 3 e 6 meses após o tratamento. Os parâmetros medidos pela OCT foram analisados pelo teste de igualdade de médias e, para correlação entre as variáveis e a Po (pressão intraocular), foi utilizado o método de Pearson. Resultados: Os pacientes apresentaram redução da Po acima de 30%. O grupo clínico apresentou aumento da espessura média da CFNR total (p=0,009) e inferior (p = 0,007). Houve aumento da espessura média da CFNR superior, temporal e nasal, aumento da área de rima e diminuição do volume de escavação, entretanto sem significância estatística. O grupo cirúrgico apresentou aumento da espessura média da CFRN inferior e redução do volume de escavação quando correlacionados com a Po (p=0,004 / p= 0,02). Houve aumento da CFRN temporal, diminuição da área de escavação e aumento da área de rima, entretanto sem significância estatística. Conclusão: O grupo clínico apresentou aumento significante na espessura da CFNR. O grupo cirúrgico apresentou diminuição do volume de escavação e aumento da espessura da CFRN dependentes da redução da Po. ______________________________________________________________________________ ABSTRACT / Introduction: Glaucoma is a progressive optic neuropathy, multifactorial, associated with visual field loss and changes in the optic nerve (ON) and retinal nerve fiber layer (RNFL). Objective: To identify changes in the RNFL and ON by optical coherence tomography (OCT) in patients with primary open angle glaucoma (POAG)undergoing clinical and surgical treatment (trabeculectomy). Methods: We performed a prospective analysis of 37 eyes of 25 patients before and after treatment, by means of OCT, a pre-treatment period (1 week to 1 month) and with 1, 3 and 6 months after treatment. The parameters measured by OCT were analyzed by the test of equality of means and correlations between IOP and variables we used the method of Pearson. Results: The patients showed reduction of IOP above 30%. The clinical group had an increase in average total (p= 0,009) and inferior RNFL thickness (p = 0.007). There was an increase in average superior, temporal and nasal RNFL thickness, increase in rim area and decreased in cup volume, however not statistically significant. The surgical group had an increase in average inferior RNFL thickness and reducing in cup volume when correlated with IOP (p = 0.004 / p = 0.02). There was an increase in temporal RNFL thickness, decrease in cup area and increase in rim area, though not statistically significant. Conclusion: The clinical group showed significant increase in RNFL thickness. The surgical group showed decrease in the cup volume and increased RNFL thickness dependent reduction of IOP.

Pacientes e métodos: Crianças portadoras de catarata congênita e do desenvolvimento, uni ou bilateral com indicação de tratamento cirúrgico com idades de zero a 12 anos. Foram excluídos olhos com diâmetro corneano horizontal menor que 10 mm ou outras alterações. Resultados: 50 olhos operados de 35 crianças sendo 18 masculinos (51,42%) e 17 femininos (48,58%). A idade variou de 2 meses a 12 anos. 20 olhos eram de crianças com menos de 1 ano de idade. 19 casos (76%) apresentaram catarata bilateral e 12 casos unilaterais (24%). Nenhuma complicação intra-operatória foi observada. Em relação a complicações pós-operatórias observou-se opacidade do eixo visual e necessidade de re-intevenção 6 olhos (12%), 4 olhos de crianças operadas com menos de 1 ano de idade (20% das crianças desta faixa etária) e 2 olhos de crianças maiores de 1 ano de idade (6,67% das crianças desta faixa etária). Conclusão: A técnica utilizada mostrou ser reprodutível, com complicações pósoperatórias leves e resultados refracionais adequados / Purpose: Evaluated the reproducibility, intraoperative and postoperative complications and refractive results by technique of primary IOL implantation followed by posterior capsulotomy and anterior vitrectomy via pars plana / plicata for treatment of congenital cataracts and development. Eyes less than 10 mm horizontal corneal diameter or other changes were excluded. Results: 50 eyes of 35 children and 18 males (51.42 %) and 17 female (48.58 %) . The age ranged from 2 months to 12 years. 20 eyes were of children under 1 year of age. 19 cases (76 %) had bilateral cataracts and 12 unilateral cases (24 %). No intraoperative complications were observed. Alterações oftalmológicas em pacientes com imunodeficiência comum variável / Ophthalmological changes in patients with common variable immunodeficiency

A imunodeficiência comum variável (ICV) é uma imunodeficiência predominantemente humoral, com prejuízo na produção de imunoglobulinas. A maioria dos pacientes apresenta infecções recorrentes. No entanto, os pacientes com ICV podem apresentar comorbidades não infecciosas, dentre elas, alterações oftalmológicas. Manifestações oftalmológicas são muito pouco conhecidas em pacientes com ICV, porém alguns relatos de casos já foram descritos na literatura. Este trabalho teve como objetivos realizar avaliação oftalmológica completa dos pacientes com ICV dos ambulatórios de Imunologia do Hospital das Clínicas, procurando correlacionar as manifestações oftalmológicas encontradas com processos infecciosos e inflamatórios. Foram avaliados 16 pacientes, com idades entre 10 e 50 anos, dos quais seis apresentaram alterações oftalmológicas (três pacientes com síndrome do olho seco, um paciente com estrabismo, um paciente com pterígio e um paciente com retinopatia hipertensiva e espessamento da parede arteriolar). Entretanto, pterígio e estrabismo foram considerados alterações casuais, não sendo associados à ICV. Dois pacientes com olho seco fizeram avaliação da lágrima, com a finalidade de diagnóstico precoce de possível síndrome de Sjögren, mas a avaliação foi normal. Desta forma, são necessários mais estudos e com maior número de pacientes com finalidade de estabelecer marcadores laboratoriais preditivos para lesões oftalmológicas / The common variable immunodeficiency (CVID) is a predominantly humoral immunodeficiency, with impairment in the production of immunoglobulins. Most patients have recurrent infections. However, patients with CVID may have noninfectious comorbidities, including ophthalmological changes. Ophthalmologic manifestations are very little known in patients with CVID, but some case reports have already been described in the literature. The aim of this study was to perform a complete ophthalmologic evaluation of patients with CVID who are seeing in the Immunology Ambulatory of the Hospital das Clínicas, seeking to correlate the ophthalmological manifestations found with infectious and inflammatory processes. Sixteen patients were evaluated, between the ages of 10 and 50 years, six of which presented alterations (three patients with dry eye syndrome, one patient with strabismus, one patient with pterygium and one patient with hypertensive retinopathy and arteriolar wall thickening). However, pterygium and strabismus were considered casual alterations and were not associated with CVID. Two patients with dry eye made a tear evaluation, in view of the early diagnosis of Sjögren\'s Syndrome, but it was normal. Thus, additional studies with a larger patient sample are necessary to establish predictive laboratory markers for ophthalmological injuries

Foram avaliados 16 pacientes, com idades entre 10 e 50 anos, dos quais seis apresentaram alterações oftalmológicas (três pacientes com síndrome do olho seco, um paciente com estrabismo, um paciente com pterígio e um paciente com retinopatia hipertensiva e espessamento da parede arteriolar). Entretanto, pterígio e estrabismo foram considerados alterações casuais, não sendo associados à ICV. Dois pacientes com olho seco fizeram avaliação da lágrima, com a finalidade de diagnóstico precoce de possível síndrome de Sjögren, mas a avaliação foi normal. Desta forma, são necessários mais estudos e com maior número de pacientes com finalidade de estabelecer marcadores laboratoriais preditivos para lesões oftalmológicas / The common variable immunodeficiency (CVID) is a predominantly humoral immunodeficiency, with impairment in the production of immunoglobulins. Most patients have recurrent infections. However, patients with CVID may have noninfectious comorbidities, including ophthalmological changes. Ophthalmologic manifestations are very little known in patients with CVID, but some case reports have already been described in the literature. The aim of this study was to perform a complete ophthalmologic evaluation of patients with CVID who are seeing in the Immunology Ambulatory of the Hospital das Clínicas, seeking to correlate the ophthalmological manifestations found with infectious and inflammatory processes. Sixteen patients were evaluated, between the ages of 10 and 50 years, six of which presented alterations (three patients with dry eye syndrome, one patient with strabismus, one patient with pterygium and one patient with hypertensive retinopathy and arteriolar wall thickening). However, pterygium and strabismus were considered casual alterations and were not associated with CVID. Two patients with dry eye made a tear evaluation, in view of the early diagnosis of Sjögren\'s Syndrome, but it was normal. Thus, additional studies with a larger patient sample are necessary to establish predictive laboratory markers for ophthalmological injuries

Introdução: A esclerodermia é rara doença do tecido conectivo que se manifesta através da esclerose cutânea e variável acometimento sistêmico. Duas categorias de esclerodermia são conhecidas: esclerose sistêmica, caracterizada por esclerose cutânea e acometimento visceral e a esclerodermia localizada ou morfeia que classicamente apresenta evolução benigna e autolimitada, confinada a pele e/ou tecidos subjacentes. Estudos recentes demonstram que a forma localizada possa eventualmente apresentar acometimento de órgãos internos e morbidade variável. Objetivo: Neste estudo objetivamos determinar as características demográficas, a prevalência de manifestações sistêmicas e alterações laboratoriais, bem como a associação com doenças autoimunes, em pacientes com esclerodermia da face. Métodos: Pacientes com esclerodermia localizada, incluindo os casos de esclerodermia em golpe de sabre, síndrome de Parry-Romberg e morfeia em placas com acometimento facial, foram avaliados e submetidos à investigação neurológica, incluindo exame clínico neurológico e ressonância magnética de crânio, e avaliação oftalmológica. Após 3 anos, o subgrupo de pacientes disponível para seguimento foi ressubmetido à ressonância magnética. Resultados: Foram estudados 34 pacientes com esclerodermia localizada da face. Deste total, 64,7% apresentavam uma ou mais manifestações extracutâneas, sendo cefaleia a queixa mais frequente, encontrada em 61,8% dos pacientes. Dos 23 pacientes submetidos à avaliação neurológica, 56,5% apresentaram alterações neurológicas possivelmente associadas à esclerodermia. Alterações à ressonância magnética foram observadas em 50% dos casos. O achado mais frequente foi a presença de lesões parenquimatosas com alteração de sinal em 50% dos pacientes. Dos pacientes que apresentavam alteração neurológica, 80% também apresentavam alguma alteração à ressonância magnética. Doze pacientes foram ressubmetidos a novo exame após 3 anos. Em todos os pacientes os achados de imagem se mantiveram inalterados. Durante esse intervalo de 3 anos, 25% dos pacientes apresentaram sinais de atividade da esclerodermia. Quanto à avaliação oftalmológica, 67,9% dos pacientes avaliados apresentaram alteração, sendo os achados mais frequentes a ocorrência de alterações orbiculares da esclerodermia (20,6%) e xeroftalmia (10,7%). Conclusão: Pacientes com esclerodermia localizada da face apresentam alta prevalência de alterações neurológicas e oftalmológicas. Two categories of scleroderma are known: systemic sclerosis, characterized by cutaneous sclerosis and visceral involvement and localized scleroderma or morphea which classically presents benign evolution and selflimited, confined to the skin and / or underlying tissue. Recent studies show that the localized form may possibly course with involvement of internal organs and variable morbidity. Objective: This study aimed to determine the demographic characteristics, the prevalence of systemic manifestations and laboratory findings, as well as the association with autoimmune diseases, in patients with scleroderma of the face. Methods: Patients with localized scleroderma, including cases of scleroderma en coup de sabre, Parry-Romberg syndrome and morphea plaque with facial involvement were evaluated and underwent neurological examination, including neurologic examination and magnetic resonance imaging, and ophthalmology evaluation. After 3 years, the subgroup of patients available for follow-up was subjected again to MRI. Results: We studied 34 patients with localized scleroderma of the face. Of this total, 64,7% had one or more extracutaneous manifestation, headache being the most frequent complaint found in 61,8% of patients. Of the 23 patients undergoing neurological evaluation, 56,5% had neurological changes possibly associated with scleroderma. MRI changes were observed in 50% of cases. The most frequent was the presence of parenchymal lesions with signal alteration in 50% of patients. Of the patients who had neurological deficits, 80% also had a change to MRI. Twelve patients were subjected again to another MRI scan after 3 years. In all patients, imaging findings were unchanged. During this interval of 3 years, 25% of patients showed signs of activity of scleroderma. As for ophthalmologic evaluation, 67,9% of patients showed abnormalities, with the most frequent findings being the occurrence of orbicular changes of scleroderma (20.6%) and xerophthalmia (10.7%). Conclusion: Patients with localized scleroderma face have a high prevalence of neurological and ophthalmological changes. Based on these findings, we suggest that all cases of localized scleroderma of the face should be thoroughly examined for the presence of systemic changes
