As estatísticas desfavoráveis aos homens são consequência de barreiras socioculturais e institucionais. Nesse contexto, com vistas a contribuir para a problematização do discurso sobre saúde e doença no segmento masculino, esta pesquisa teve como objetivos: descrever as representações sociais dos homens acerca do processo saúde-doença; e caracterizar as representações sociais do homem atendido nas Unidades de Atenção Primária à Saúde sobre a resposta à sua necessidade de saúde. O estudo desenvolveu-se a partir da abordagem social da pesquisa qualitativa tendo como princípio teórico as Representações Sociais. Os dados foram apreendidos por meio da entrevista semiestruturada junto a 51 usuários das UAPS de São João del-Rei/MG. Selecionou-se o método de análise temática proposto por Bardin. Para análise e interpretação dos dados, foram construídas as seguintes categorias: Homens tecendo representações sobre a saúde; Homens tecendo representações sobre a doença; A atenção primária à saúde na percepção dos homens; e A Atenção Primária à Saúde: motivações e (des) motivações dos homens para procurá-la. A partir da análise das categorias elencadas, vislumbrou-se a necessidade de considerar os homens em seus diferentes contextos e realidades, valorizando as diferenças culturais e de gênero. Dessa forma, estamos instrumentalizando-os para a adoção de comportamentos protetores da saúde e menos vulneráveis à doença e estimulando o desvinculamento com a cultura e costumes machistas prejudiciais. As representações dos homens evidenciam a necessidade do desenvolvimento de programas e capacitações dos profissionais para atuarem na estratégia saúde da família e do rompimento da população masculina com as amarras socioculturais para que os homens também utilizem as ações e serviços primários de saúde com qualidade e integralidade. / The male population presents high morbidity and mortality rates when compared to the female population. The unfavorable statistics for men are a consequence of socio-cultural and institutional barriers. In this context, in order to contribute to the discussion of issues relating about health and illness in the male segment, this study aimed to: describe men's social representations about the health-illness process; and characterize the social representations of men attended by Primary Health Care Units, regarding the response to their health needs. The study was developed from the social approach to qualitative research, based on Social Representations Theory. Data were collected through semistructured interviews with 51 users of the UAPS in São João del-Rei / MG. The thematic analysis method proposed by Bardin was selected. The following categories were constructed: Men forming representations about health; Men forming representations about illness; men's perception of primary health care; and Primary Health Care: motivations and disincentives for men to seek it. From the analysis of the categories listed, we discerned the need to consider men in their different contexts and realities, recognizing cultural and gender differences. Thus, we are providing them the means to adopt behaviors protective of health and less vulnerable to illness, and promoting a break with harmful macho culture and customs. The representations of these men show the need to develop programs and training for professionals to practice within the family health strategy, and to release the male population from socio-cultural constraints, so that men also make use of high-quality integrated primary health care actions and services.

O que essa pesquisa mostrou foi que, no Brasil, observa-se a difusão de um "erotismo politicamente correto" (GREGORI, 2010) pelo universo de produção, comercialização e consumo de bens eróticos, o mercado erótico. Erotismo que tem como características: O sexo/erotismo deslocado do seu sentido de transgressão e cada vez mais associado à fonte de prazer, saúde mental e corporal , em que uma vida sexual ativa se torna possível a todos os corpos e indivíduos, através do exercício e da domesticação desse corpo para o ato. Durante a pesquisa empírica realizada entre lojas e trabalhadores desse mercado erótico [pesquisa realizada de outubro de 2004 a dezembro de 2007], entrei em contato com um grande número de jornalistas e repórteres de revistas, que procuravam lojas de produtos eróticos para realizarem reportagens e matérias sobre sexo. Revistas como Nova, Marie Claire, Claúdia, Gloss. Essa circulação fez surgir a hipótese de que, talvez, extrapolando o mercado erótico, o erotismo politicamente correto também estaria sendo disseminado pelas revistas. Essas revistas poderiam ser vistas como manuais pedagógicos desse tipo de erotismo? Quais as especificidades ele receberia em suas páginas? O objetivo dessa dissertação é observar como e se ocorre a disseminação do erotismo politicamente correto nas páginas de duas revistas de circulação nacional Nova e Men's Health / Abstract: This dissertation begins in October 2004, when I worked as a research assistant on Thematic Project: Gender, corporeality of the Center for Gender Studies (Pagu.) What this research showed was that, in Brazil, there the diffusion of a "politically correct eroticism" (GREGORI, 201 O) in the universe of erotic market . Eroticism whose characteristics are: sex I eroticism displaced from their sense of transgression and increasingly more associated with the source of pleasure, mental health and body health. During the empirical research conducted between stores and employees of this erotic market [survey conducted from October 2004 to December 2007], I contacted a large number of journalists and reporters, making in this places articles about sex for magazines. This gave rise to the hypothesis that perhaps extrapolating the erotic market, the "politically correct eroticism" also is being disseminated by magazines. These magazines could be viewed as manuais of this kind of eroticism? Poor coordination and failure of systems in place appeared to characterise the many challenges. Gender inequalities and masculine dominated cultural practices like polygamy and widow inheritance are associated with consequences of ill health among women; including high HIV/AIDS prevalence, early marriage, high teenage pregnancies and high maternal mortality. Although these practices are in favor of men, they hardly protect them from the wrath of poor RH like STDS, HIV/AIDS, stressful big families and vast poverty. Hence, Tanzanian men are also victims of their own behavior. / South Africa

O projeto foi encaminhado ao Comitê de Ética e Pesquisa da Universidade Federal de Juiz de Fora, para cumprimento dos aspectos éticos e legais, atendendo à Resolução n° 466/2012 do Conselho Nacional de Saúde, sendo deferido com o parecer nº 525.352. O cenário constitui-se de um Hospital de Ensino da Zona da Mata Mineira e teve como participantes quinze homens que receberam o diagnóstico de câncer de próstata. Os depoimentos foram colhidos por meio da entrevista fenomenológica e emergiram, das estruturas essenciais, as Unidades de Significado. A compreensão vaga e mediana dos significados, primeiro momento metódico, possibilitou a elaboração do fio condutor para a análise interpretativa hermenêutica, segundo momento metódico. Desvelou-se à luz do pensamento de Heidegger que o homem, quando recebe o diagnóstico de câncer de próstata, sentese ameaçado pela doença e por suas consequências, desenvolvendo o movimento da angústia imprópria apresentando o temor, pavor, horror e terror. O ser-aí-apósdiagnóstico- de-câncer-de-próstata, lançado na sua facticidade, expressa um movimento de inautenticidade determinado pelo impessoal, ficando exposto ao falatório e à ambiguidade de tudo o diz a respeito ao câncer de próstata, e, assim, apresenta-se de-caídos. Evidenciaram-se as invisibilidades dos profissionais e serviços de saúde no atendimento a esse homem, marcados pela inautenticidade e impessoalidade. Este estudo, considerando os sentidos desvelados, enfatiza a contribuição para a melhoria da qualidade da assistência ao homem, ratificando ser necessário atender não somente à dimensão física, mas aos aspectos emocionais, sociais e espirituais, frente às atuais Políticas Públicas de Saúde do Homem. E que estas e outras reflexões possibilitam novas fontes de atuação à saúde do homem, além de contribuírem no atendimento às suas necessidades quando diagnosticado com câncer de próstata. / Qualitative research based on Heideggerian's theoretical, philosophical and methodological approach, in order to unveil the lived experiences of men after the diagnosis of prostate cancer. The project was submitted to the Research Ethics Committee of the Federal University of Juiz de Fora, for ethical and legal issues according to National Health Council Resolution n° 466/2012 and was accepted under n° 525.352. The research setting was a teaching hospital at Zona da Mata Mineira and the subjects were fifteen men diagnosed with prostate cancer. Interviews were conducted through phenomenological interviews and some units of signification have emerged from essential structures. Vague and median understanding, the first methodical moment, allowed the development of the central research thread towards the interpretive hermeneutic analysis, called the second methodical moment. In light of Heidegger's thought, it has been shown that the moment this man is diagnosed with prostate cancer he feels threatened by the disease and its consequences, reporting an improper anguish with fear, dread, horror and terror. The being-thereafter- diagnosis-of-prostate-cancer in his facticity reveals an inauthenticity determined by the impersonal being exposed to chattering and the ambiguity over all the aspects related to prostate cancer leading him to decay. The invisibilities of professionals and health services characterized by inauthenticity and impersonality were present in delivering care to man. Regarding the senses unveiled this study emphasizes the contribution provided to improve the quality care delivered to men showing how necessary it is to consider not only physical but emotional, social and spiritual aspects, in view of current Public Policies on Male Health. Furthermore these insights and many others may enable new sources of assistance targeting men’s health, helping to meet their needs when they have to face diagnosis of prostate cancer.

Men's perception regarding voluntary circumcision at a male clinic, Lesotho

Medical male circumcision prevalence in Lesotho remains at 23% even though efforts are made to encourage men to be circumcised. The purpose of this study was to explore and describe perceptions of men regarding male circumcision in a hospital in Morija, Lesotho and make recommendations on how to promote uptake of medical male circumcision. A descriptive, explorative and contextual qualitative design was used. In-depth, unstructured individual interviews were conducted on ten (10) uncircumcised men who were selected via purposive sampling. The findings revealed that men’s perceptions on circumcision can be classified under the following themes: perceived health beliefs of men about circumcision, perceived community-held beliefs about circumcision, men’s knowledge regarding circumcision, and perceived misconceptions about circumcision. It is recommended that knowledge on circumcision be reinforced and negative perceptions be corrected with the multi-sectoral approach to promote uptake of circumcision services. / Health Studies / M.A. (Public health)

Relationships between masculinity beliefs and colorectal cancer screening in male veterans

Indiana University-Purdue University Indianapolis (IUPUI) / Men’s adherence to masculinity norms has been implicated as a risk factor for unhealthy behaviors (e.g., drinking to intoxication, having unprotected sex with multiple, simultaneous partners) and lack of engagement in healthy behaviors (e.g., blood pressure screening, cholesterol screening, wearing protective clothing while in the sun, receipt of annual medical and dental exams) (Boman & Walker, 2010; Courtenay, 2000a, 2000b, 2011; Hammond, Matthews, & Corbie-Smith, 2010; Iwamoto, Cheng, Lee, Takamatsu, & Gordon, 2011; Locke & Mahalik, 2005; Mahalik, Lagan, & Morrison, 2006; Mahalik et al., 2003; Nicholas, 2000; Pachankis, Westmaas, & Dougherty, 2011; Pleck, Sonenstein, & Ku, 1993; Wade, 2009). Masculinity has been defined as behaviors, beliefs, and personality characteristics associated more often with men than women as well as characteristics and behaviors that society prescribes and reinforces in men (Thompson, Pleck, & Ferrera, 1992). Rooted in geographical, cultural, and temporal environments, diverse masculinities have emerged throughout the United States and the world (Connell, 1995; Courtenay, 2011). Traditional masculinity beliefs and behaviors in the United States include the sturdy oak (men should be tough, self-reliant, stoic, and confident), no sissy stuff (men should avoid feminine characteristics and behaviors), the big wheel (men should strive for success and status), and give ‘em hell (men should embrace aggressiveness, daring, and violence) (Brannon, 1976). Numerous qualitative studies have suggested that some men find cancer screening examinations involving the rectum (i.e., endoscopy for colorectal cancer [CRC] screening or digital rectal examination [DRE] for prostate cancer screening) an affront to their masculinity (see Table 1 for quotations from these studies) (Bass et al., 2011; Beeker, Kraft, Southwell, & Jorgensen, 2000; Getrich et al., 2012; Goldman, Diaz, & Kim, 2009; Harvey & Alston, 2011; Holt et al., 2009; Jilcott Pitts et al., 2013; Jones, Devers, Kuzel, & Woolf, 2010; Rivera-Ramos & Buki, 2011; Thompson, Reeder, & Abel, 2011; Wackerbarth, Peters, & Haist, 2005; Winterich et al., 2009). However, to the author’s knowledge, no quantitative studies have considered the role of masculinity in CRC screening adherence. Unfortunately, current CRC screening rates fall below the 70.5% Healthy People 2020 screening objective (U.S. Department of Health and Human Services, 2012).Research is needed to better understand relationships between men’s masculinity norms and CRC screening adherence so that interventions may be developed to reduce barriers to screening, improve screening rates, and, ultimately, decrease men’s mortality from CRC. The present study will address this gap in the literature by examining the masculinity norms and CRC screening adherence of male veterans aged 51-75 years who are at average CRC risk (Levin et al., 2008). First, the prevalence of CRC, its risk factors and warning signs as well as CRC screening techniques, screening rates, and characteristics of individuals who are adherent and non-adherent to CRC screening guidelines are summarized. Next, the concept of masculinity, theoretical and empirical support for studying masculinity norms within the context of CRC screening, and potential relationships between masculinity norms and colorectal cancer screening behaviors are described. Finally, the study methods, results, and future directions and limitations of this research are described.

A relação entre a experiência de abuso sexual na infância e queixas de dor pélvica crônica e disfunção sexual masculina / The relationship between childhood sexual abuse experience and complaints of chronic pelvic pain and male\'s with sexual dysfunction

INTRODUÇÃO: Estudos realizados com mulheres apontam para a importância de se fazer uma avaliação de ocorrência de abuso sexual infantil em pacientes com disfunções sexuais e dor pélvica crônica, pois o histórico de abuso aumenta o risco de apresentarem problemas sexuais na vida adulta. O Abuso sexual infantil é um fenômeno universal que atinge todas as idades, classes sociais e culturas. No Brasil é a segundo maior tipo de violência entre crianças de 0 a 9 anos. Diferentemente de mulheres, há poucos estudos relacionam a experiência de abuso sexual infantil e o desenvolvimento de queixas urológicas que afetem a função sexual masculina. OBJETIVO: Avaliar a relação entre a experiência de abuso sexual na infância e as queixas de dor pélvica crônica (DPC), disfunção erétil (DE) e/ou ejaculação precoce (EP). MÉTODO: Trata-se de um estudo cross-section. A amostra foi composta por oitenta homens, entre 30 e 59 anos, pacientes da Divisão de Clínica Urológica do Hospital das Clínicas, que apresentavam EP, DE e/ou DPC. Os dados foram coletados por um ano. O antecedente de abuso sexual na infância foi avaliado por meio do questionário de Childhood Sexual Experiences Questionnai, traduzido e testado para uso na pesquisa. Também foi avaliado o tipo de abuso, pessoas envolvidas, idade de inicio e tempo de duração do abuso, opção sexual, estado civil e fatores de risco (cardiopatia, hipertensão, diabetes e doenças psiquiáticas) para EP, DE e DPC. Na análise estatística o teste de Qui Quadrado foi utilizado para avaliar as variáveis categóricas e o GLM univariado para avaliar as variáveis contínuas, representadas por média ± desvio padrão. Os dados contínuos foram representados em frequência absoluta e percentual (%). RESULTADOS: A amostra se caracterizou por homens predominantemente heterossexuais e em união estável. A média de idade foi de 50,2 anos. O grupo com DE apresentou maior índice de diabetes comparado com os demais. Não houve diferença significativa entre os grupos em relação à hipertensão, cardiopatia e transtornos psiquiátricos. Não houve diferença entre os grupos quanta a prevalência de abuso. 10% do total da amostra relatam ter sofrido abuso com ou tentativa de penetração A média de idade do inicio do abuso foi de 9,6 anos. A maioria dos abusos durou mais de um ano sendo que em todos os episódios a pessoa envolvida era conhecida da criança. CONCLUSÃO: A alta prevalência de abuso sexual infantil na amostra revela um problema na notificação dos casos ocorridos com o gênero masculino, pelo estigma do abuso com meninos, conceito restrito de abuso (focados em casos graves), despreparo dos profissionais de saúde e educação em identificar os casos. A literatura indica maiores taxas de abuso com a população feminina, mesmo comparado com estes dados a prevalência de violência sexual infantil é maior nesta pesquisa, reforçando a hipótese de que o antecedente de violência sexual é um fator de risco para o desenvolvimento de problemas de saúde que afetam a sexualidade masculina / INTRODUCTION: Child sexual abuse is a universal phenomenon that affects all ages, social classes and cultures. In Brazil it is the second largest type of violence among children from 0 to 9 years old. Many studies point to the importance of evaluating the ocurrence of child sexual abuse in patients with sexual disfunctions and chronic pelvic pain, since the history of abuse increases the risc of presenting sexual problems in the adult life. Unlinke women, there are few studies that relate the experience of child sexual abuse and the development of utological complaints the affect male sexual function. OBJECTIVE: To evaluate the relationship of sexual abuse experience in childhood and chronic pelvic pain (CPP), erectile dysfunction (ED) and premature ejaculation (PE). METHOD: This is a cross sectional study. The sample consisted of eighty men, between 30 and 59 years old, patients of the Urological Clinic Dividion of the Hospital das Clínicas in São Paulo, Brazil. The patients presented chronic pelvic pain, erectile dysfunction and or premature ejaculation. Data were collected during one year. The history of childhood sexual abuse was assessed using the Childhood Sexual Experience Questionnaire, translated and tested for use in research. The type of abuse, people involved, age of onset and duration of abuse, sexual choice, marital status and risk factors (heart disease, hypertension, diabetes and psychiatric diseases) were also evaluated. Chi Square test was used to evaluate the categorical variables and univariate GLM to evaluate the continuous variables, represented by mean ± standard deviation. The continuous data were represented in absolute frequency and percentage (%). RESULTS: The sample was characterized by predominantly married and heterossexual men wich mean age of 50.2 years. The ED group presented a higher diabetes index compared to the CPP and PE groups. There were no statistical difference between groups in prevalence of abuse. The mean age at onset of abuse was 9.6 years. Most of the abuses lasted more than a year and in all episodes the person involved was known to the child. COCLUSION: The high prevalence of child sexual abuse in the sample reveals a problem in the notification of cases with the male gender. The stigma of child abuse, restricted concept of abuse (focused on serious cases), unprepared health professional in identify the cases. The literature indicates higher rates of abuse with the famale population. Even compared to these data, the prevalence of child sexual violence is higher in this research, reinforcing the hypothesis that the antecedente of sexual violence is a risk fator for the development of health problems which affect male sexuality

Inclusão do parceiro na assistência pré-natal / Inclusion of the male partner in prenatal care

ObjeEste estudo teve como objetivo primário avaliar a estratégia de incluir o parceiro na assistência pré-natal no município de Franca-Brasil. Constituíram os objetivos secundários: 1) Detectar a prevalência de parceiros com dosagens alteradas de colesterol total, HDL-colesterol, LDL-colesterol e triglicérides; 2) Detectar a prevalência de parceiros com dosagens alteradas de glicemia de jejum; 3) Detectar a prevalência de parceiros com HAS; 4) Detectar a prevalência da sífilis, HIV, hepatites B e C entre gestantes; 5) Verificar a taxa de DMG e de DM entre as gestantes; 6) Detectar a prevalência da sífilis, HIV, hepatites B e C entre os parceiros das gestantes; 7) Avaliar a motivação da equipe de saúde em acolher a Estratégia Pré-natal do Parceiro; 8) Avaliar a aceitação materna e do parceiro sobre a estratégia de inclusão do parceiro na assistência PN. Estudo quantitativo, utilizado o teste \"t\" de Student para comparações das variáveis paramétricas e o teste Qui-quadrado para as variáveis não paramétricas, considerando significativo o valor de p< 0,05 em quaisquer dos testes. Os resultados identificaram quatro parceiros com hipertensão arterial (3,4%) e 51 (43,2%) com intolerância à glicose, verificando-se que em quatro casos, a glicemia possibilitou diagnosticar diabetes mellitus. Constatou-se que a prevalência de dislipidemias dos parceiros foi de 84,4%. No tocante às sorologias identificou-se um casal (1,5%) com sorologia discordante para sífilis (gestante com sífilis) e outro casal, no qual o parceiro foi diagnosticado como portador da infecção pelo vírus da imunodeficiência humana (HIV), cujo diagnóstico só foi possível por sua participação no PNP. Foi possível identificar que 57 parceiros (48,3%) qualificaram as equipes de saúde com não motivadoras para a adoção da estratégia PNP. No entanto, 58 (49,1%) participaram por motivação própria. A percepção paterna sobre o PNP foi referida como positiva por 77,9% dos parceiros e 93 gestantes/mães (78,8%) consideraram importante e positiva a inclusão do parceiro no PN. Foi identificado que a inclusão do parceiro no PN foi importante no diagnóstico e tratamento precoces de doenças de transmissão vertical e crônicas e que é necessário incentivar a equipe de saúde a difundir os benefícios dessa participação no PN, contribuindo com a saúde da tríade mãe, filho e parceiro. / This study\'s primary objective was to analyze the strategy of including the male partner in prenatal care in the city of Franca, Brazil. The secondary objectives were: 1) To detect the prevalence of male partners with abnormal levels of total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides; 2) To detect the prevalence of male partners with abnormal levels of fasting blood glucose; 3) To detect the prevalence of systemic hypertension among male partners; 4) To detect the prevalence of syphilis, HIV, hepatitis B and hepatitis C among pregnant women; 5) To find out the frequency of Gestational diabetes mellitus (GDM) and of Diabetes mellitus (DM) among pregnant women; 6) To detect the prevalence of syphilis, HIV, hepatitis B and hepatitis C among the pregnant women\'s male partners; 7) To gauge the health team\'s motivation to implement the strategy of male involvement in prenatal care; 8) To assess maternal acceptance and male partner acceptance of the strategy of including the male partner in prenatal care. This was a quantitative study, which used the Student\'s t-test to compare parametric variables and the Chi-squared test for the nonparametric variables and which considered a p-value < 0.05 as significant in both tests. The results identified 4 male partners with hypertension (3.4%) and 51 male partners (43.2%) with glucose intolerance, of which four had a diagnosis of Diabetes mellitus as a result of the blood glucose test. We found that the Anexos 100 prevalence of dyslipidemia among the male partners was 84.4%. Regarding serology tests, we identified a couple (1.5%) with discordant syphilis serology (pregnant woman with syphilis) and another couple in which the male partner was diagnosed with Human Immunodeficiency Virus (HIV) infection with such diagnosis only having been possible because of his participation in prenatal care. We could identify that 57 male partners (48.3%) classified the health teams as not encouraging of the male involvement in prenatal care. However, 58 male partners (49.1%) participated by selfmotivation. The fathers\' perception of their involvement in prenatal care was referred to as positive by 77.9% of the male partners and 93 pregnant women / mothers (78.8%) considered the inclusion of the male partner in prenatal care important and positive. We identified that the inclusion of the male partner in prenatal care was important in the early diagnosis and early treatment of vertical transmission diseases and chronic diseases as well as that it is crucial to encourage health teams to communicate the benefits of the inclusion of the male partner in prenatal care, thus contributing to the health of the father- mother-child triad.

Busca por avaliação de próstata, disfunção erétil e demanda oculta de homens na Atenção Primária à Saúde / The search for prostate evaluation, erectile dysfunction and men\'s hidden agenda in Primary Health Care

Médicas e médicos de família e comunidade frequentemente atendem homens aparentemente assintomáticos que buscam avaliação da próstata, e percebem que muitos deles ocultam outras queixas, especialmente disfunção erétil - problema de grande interesse para homens e para a indústria farmacêutica. Como o rastreamento de câncer de próstata traz mais danos que benefícios, esses usuários se colocam sob risco de intervenções diagnósticas e terapêuticas questionáveis, com grande potencial de falsos-positivos e sequelas. As Unidades Básicas de Saúde (UBSs) no Brasil, por sua vez, historicamente se dedicaram à população feminina, e seus profissionais frequentemente reproduzem estereótipos de gênero que contribuem para a invisibilidade dos homens nesses serviços. O objetivo da pesquisa foi entender como a população masculina usa serviços de Atenção Primária à Saúde (APS) e como expressa suas queixas, a partir do caso dos homens que buscam avaliação de próstata ou têm questões referentes a disfunção erétil (ou outros problemas de sexualidade). Buscamos identificar articulações entre esses problemas e a demanda por rastreio, bem como entender como os serviços (particularmente os/as médicos/as) lidam com demandas expressas e subjacentes. A pesquisa é qualitativa e os dados foram produzidos através de entrevistas semiestruturadas com 16 profissionais e 15 usuários maiores de 18 anos da Estratégia Saúde da Família em três UBSs em São Paulo e duas em Mauá - homens em cujas consultas surgiram questões sobre próstata, disfunção erétil ou outros problemas de sexualidade. Como técnica auxiliar, observamos os cinco serviços participantes, pautados por roteiro específico e registrando as observações em diário de campo. A interpretação dos dados levou em conta os referenciais de gênero e construção social das masculinidades, além das noções de integralidade, prevenção quaternária e medicalização. A análise de conteúdo das entrevistas demonstrou como aspectos dos(as) profissionais se relacionam à forma como eles(as) percebem, acolhem e dão sentido às demandas dos homens, permitindo também apontar mudanças em curso. A observação dos serviços ajudou a identificar características de APS seletiva em UBSs mauaenses, favorecendo o desencontro entre as demandas e dificuldades dos homens e as ações oferecidas e idealizadas, e de uma APS mais abrangente nas UBSs paulistanas, que facilita a presença dos homens, mas nem sempre contempla questões de gênero. Entre os usuários, identificamos a incorporação de discursos médicos envolvendo risco e cuidado em saúde, nomeamos fatores de entrada, facilitação e manutenção de sua presença na UBS e apontamos fatores relacionados à expressão de suas demandas, mostrando como essa expressão pode influenciar o cuidado recebido e mesmo colocar o homem sob risco de iatrogenia. Discutimos possibilidades de superar olhares reducionistas e estereotipados e abordagens prostatocêntricas e medicalizadoras da saúde masculina. Com isso, contribuimos à diminuição da invisibilidade dos homens, positivando sua avaliação e cuidado integrais / Family physicians often meet apparently asymptomatic men who seek for prostate evaluation and perceive that many of them hide other complaints, especially erectile dysfunction - a matter of great interest to men and to the pharmaceutical industry. As the screening of prostate cancer brings more harm than good, these users are put at risk of questionable diagnostic and therapeutic interventions, with great potential for false positives and sequelae. Basic Health Units (BHU) in Brazil, in turn, was historically dedicated to the female population, and their professionals often reproduce gender stereotypes that contribute to the invisibility of men in these services. The objective of the research was to understand how the male population uses Primary Health Care (PHC) services and express their needs, considering the case of men who seek for prostate evaluation or have questions regarding erectile dysfunction (or other sexual problems). We seek to identify links between these problems and the demand for screening as well as understand how services (particularly doctors) deal with said and hidden agenda. The research is qualitative and data were produced using semistructured interviews with 16 professionals and 15 adult users of the Family Health Strategy in three BHUs in São Paulo and two in Mauá - men in whose consultations arose questions about prostate, erectile dysfunction or other sexual problems. As an auxiliary technique, we observed the five participating services, guided by specific script and recording observations in a field diary. Interpretation of data took into account the references of gender and the social construction of masculinities, as well as the notions of comprehensiveness, quaternary prevention and medicalization. The content analysis of the interviews demonstrated how aspects of the doctors relate to how they perceive, embrace and give meaning to the demands of men, allowing us also to identify changes in progress. Observation of services helped to identify characteristics of selective PHC in BHUs in Mauá (favoring the mismatch between men\'s needs and difficulties and the actions offered or idealized) and of a more comprehensive PHC in BHUs in São Paulo, which facilitates the presence of men, but not always consider gender issues. Among male users, we identified the incorporation of medical discourses involving risk and health care, named input, facilitating and maintaining factors for their presence in BHUs and pointed factors related to the expression of their needs, showing how this expression can influence the care received and even put the man at risk of iatrogenia. We discuss possibilities to overcome reductionist and stereotyped views as well as prostatocentric and medicalized approaches to men\'s health. Thus, we contribute to the lessening of the invisibility of men, supporting their comprehensive evaluation and care

Masculinidades, trabalho e cuidado em saúde: representações de trabalhadores no contexto de modernização do Porto de Santos-SP / Masculinities, work and health care: representations of workers in the context of modernization of the Port of Santos-SP

Esta tese investigou os significados atribuídos por trabalhadores portuáriosdo Porto de Santos/SP ao processo de Modernização dos Portos Brasileiros, implementado a partir da promulgação da Lei 8630/93 e da implantação do OGMO, no tocante à identidade de trabalhador portuário, processo saúde-adoecimento-cuidado, gênero e masculinidades. Foi utilizada metodologia qualitativa, com trabalho de campo realizado entre os anos de 2009/2010, com os instrumentos de observação etnográfica (dos locais de escalação para o trabalho e faixa cais do Porto de Santos) e 39 entrevistas em profundidade com diferentes categorias de trabalhadores portuários na ativa. O material produzido na pesquisa empírica constituiu-se de diários de campo e um banco de narrativas, sobre os quais foi aplicada análise por triangulação de métodos, seguindo o referencial hermenêutico , e a análise temática de conteúdo. A discussão sobre os dados obtidos a partir da análise foi divida em 3 capítulos representativos das categorias temáticas fundantes, identificadas no processo de impregnação: o primeiro sobre a caracterização dos sujeitos de pesquisa e seu universo simbólico, o segundo específico sobre a temática da saúde dos trabalhadores, e o último referente às representações de gênero e masculinidades. De acordo com os trabalhadores portuários, a promulgação da Lei 8630/93 modificou drasticamente a organização do trabalho portuário no Porto de Santos, trazendo alguns ganhos nos aspectos de saúde do trabalhador, como aqueles relacionados à exigência do uso de EPIs, controle de acidentes e exames médicos periódicos. Em contraposição, a mesma lei acarretou muitas perda objetivas e subjetivas, no sentido do esvaziamento da identidade do grupo, anteriormente pautada nas noções de ofício, e quanto aos atributos da masculinidade, baseada em valores como \'ser trabalhador\' e \'provedor da família\'. No quesito da relação entre saúdeadoecimento e cuidado, verificou-se uma percepção de insalubridade relacionada ao exercício do trabalho no Porto de Santos, considerado um ambiente permeado por muitos riscos pelos portuários. Por outro lado, apresentaram resistência à procura por tratamentos médicos e um comportamento de aversão aos afastamentos por motivos de saúde, sendo sua interface com o cuidado em saúde restrita aos exames periódicos exigidos pelo OGMO / We aimed to study the meaning of impacts that a Brazilian law (Lei de Modernização dos Portos Brasileiros 8630/93) produced in port workers of Port of Santos/SP, in a way to acess their port worker identity, health-illness care process, gender and masculinities themes. The empirical research which ground the discussion, had a qualitative approach and was perform between 2009/2010. It used as instruments for obtaining data ethnography observations (at workers selection places and pier) and 39 in-depth port workers interviews. From this point, were produced field diaries and an interview database that were analysed by metodos triangulation tecnic, according to the conceptual framework of hermeneutical referencial and content analysis. We shared the discussion in 3 chapters that brought out the leading temathics of our investigation: subjects characterization, their symbolic universe, workers health, gender and masculinities representations. According to port workers, that law dramatically modified their work organization, with some winning points like EPIs requirement, accident control and periodic exams. Beside that, port workers related considerable losses in the way of their brotherhood identity, based on craft skills and masculinities concepts, like \'be a worker\' and \'support a family\'. Port workers showed a perception about health-illness care process based in the idea that Port of Santos was a hazardous work place. In the other side, they avoid to look for medical treatments or absence from work for health problems. Their health care interface was restricted to periodic exams required from OGMO
