Use of Atypical Antipsychotics in a Community Mental Health Center: Evaluation of Dosing, Drug Combinations, and Gender

Class of 2005 Abstract / Objectives: To describe the dosing of atypical antipsychotics among outpatients diagnosed with schizophrenia at a community mental health center in Tucson, AZ, and to contrast this to dosing recommendations set by the manufacturer during clinical trials. Methods: A prescription database from January 01, 2004 to July 31, 2004 was used to evaluate the dosing of atypical antipsychotics (Abilify®, Clozaril®, Geodon®, Seroquel®, and Zyprexa®) in patients with schizophrenia. The average daily doses were evaluated for differences from recommended dosing using the physician desk reference. Differences in dosing were also analyzed for gender and monotherapy vs. patients taking multiple atypical antipsychotics. Results: Overall differences in dosing when comparing gender and monotherapy vs. patients on multiple atypicals were not significant for all five atypical antipsychotics studied. Overall, Geodon® had the highest percentage (51.9%) of patients above recommended guidelines while Clozaril® and Seroquel® had the highest percentage (70.6% and 47.4% respectively) below recommended guidelines. Implications: This study illustrates that dosing of these atypical antipsychotics at this outpatient community mental health center differs for many patients with schizophrenia from the guidelines set by the companies during clinical trials. This information will aid in the prescribing of physicians at this community mental health center and will possibly lead to larger studies to further look at reasons for these differences in dosing.

Thank you. / Learning problems present major theoretical and practical issues for the field of mental health. This thesis represents an inquiry into the causes of learning problems in children. Relevant theoretical themes viewed in the literature were compared with data collected from case records of school referred children. Review of the literature on learning problems is presented and six major contributing factors are discussed. The research sample was obtained from 26 school-referred children in a follow up study conducted by a child guidance clinic. Children presenting organic deficiencies or psychosis were ruled out. Information relating to psychodynamic issues, family and peer relationships, and school performance was obtained from case records. This information is tabulated and analyzed and compared with the six factors found in the literature on learning problems. Relative significance of each factor is assessed. In this study, the inappropriate handling of aggressive impulses was found to be the most significant factor in problems in learning. Additional conclusions are offered. Utilization of Depot Fluphenazine and Haloperidol in a Community Mental Health Center: A 12-Month Retrospective Analysis of Compliance, Hospitalization Data, and Cost of Care for Patients with Schizophrenia

Class of 2005 Abstract / Objectives: This retrospective study investigated the relationship between compliance, hospitalizations rates, and cost of care in an outpatient behavioral health facility over a 12-month period for schizophrenic patients treated with depot antipsychotic medications. Methods: Databases from COPE Behavioral Health Center in Tucson, AZ were utilized for administration of depot injections, hospitalizations, and cost of care for patients between July 1, 2003 and June 30, 2004. Results: Records were utilized for 103 patients receiving depot antipsychotics (n = 34 fluphenazine decanoate, n = 69 haloperidol decanoate). Increased number of injections received per year was associated with lower number of hospitalizations per year (p = 0.025 fluphenazine and p = 0.001 haloperidol). Also, increased number of hospitalizations was associated with increased total cost of care (p = 0.001 fluphenazine and p < 0.001 haloperidol). Implications: Patients with schizophrenia who received a greater number of depot antipsychotic injections had a lower number of hospitalizations during a 12-month period. Improved adherence with depot antipsychotics may improve clinical outcomes and reduce the total cost of care in patients with schizophrenia.

The aim of this diploma thesis is a design of a new mental health center. The mental health center is a brand new low-treshold element in the system for people with mental ilness. The building is located in Prague, Hrdlořezy, district of the Capital City of Prague. The building is a detached house, it has three floors, and one partial underground level. On the first floor, there is an entrance hall, administrative facilities, a bed unit, a dining area and last but not least, space for group and individual therapies. On the second and third floor, there are again bed units, administrativ facilities, but also space for leisure activities. The basement of the building consists of the technical backround of the building together with storage space. The proposed building is made up of cubic masses forming a rectangular floor plan with large undercovered atrium penetrating the center of the entire building. The building is designed as a monolithic reinforced concrete skeleton [frame on a base slab]. The skeleton infill masonry is designed from clay blocks. The roofing of this building is designed with a flat green vegetation roof.

Darbo tikslas. Įvertinti VšĮ Alytaus poliklinikos psichikos sveikatos centro pacientų lūkesčius dėl teikiamų paslaugų, informuotumą bei pasitenkinimą paslaugomis. Darbo uždaviniai: Išsiaiškinti VšĮ Alytaus poliklinikos psichikos sveikatos centro pacientų lūkesčius dėl teikiamų paslaugų; įvertinti pacientų informuotumą apie psichikos sveikatos centre teikiamas paslaugas; nustatyti pacientų požiūrį į teikiamas paslaugas ir jas teikiančius specialistus. Tyrimo metodika. Anoniminė anketinė psichikos sveikatos centro pacientų apklausa atlikta 2012 m. Išdalintos 410 anketų, visos jos grąžintos užpildytos. Duomenų statistinė analizė atlikta SPSS 20 programa. Kokybinių požymių palyginimui ir ryšių vertinimui taikytas χ2 arba Fišerio tikslusis kriterijus. Kokybinių požymių poriniams palyginimams taikytas z kriterijus. Statistinės hipotezės tikrintos, esant reikšmingumo lygmeniui α=0,05. Skirtumai ar ryšiai vertinti kaip statistiškai reikšmingi, kai p<0,05. Rezultatai. Daugumai pacientų, besilankančių psichikos sveikatos centre labai svarbus teigiamas personalo požiūris, bendradarbiavimas su gydytoju norint gauti informaciją apie savo sveikatos būklę bei teikiamą gydymą, galimybė gauti papildomas paslaugas, reikalingas paslaugas gauti laiku, taip pat pacientai tikėjosi, kad psichikos sveikatos centro aplinka bus maloni bei švari. Objectives of the study: clarify patients wishes for services provided in Alytus policlinic mental health centrum; evaluate patients informatikon for these services; set patients attitude to provided services and medical staff. Material and methods: A survey of patients using an anonymous questionnaire was conducted in 2012. 410 questionnaires were distributed and all returned back. Statistical data analysis was performed using “SPSS 20 for Windows” statistical software. Associations were tested using chi-square and z-test. Statistical significance level was set at p<0.05. Most of the patients want possitive staff attitude to themselves, cooperation with doctor to achieve information about their health status and treatment, also a possibility to get extra services, get services on time. They also suspected that environment in mental health center will be nice and cousy. Respect of medical staff was more important for women than for men. For older patients was more important to get services on time more than to get information about their health status. The request to get extra services emphasized more pensioners and people with higher education. A formative evaluation of a systemic infant mental health program designed to treat infants and their families through a rural community mental health center

Doctor of Philosophy / Department of Family Studies and Human Services / Anthony Jurich / Ann Murray / Despite the intensified research efforts into the field of Infant Mental Health and Marriage and Family Therapy, a truly systemically designed program has not been developed. This formative evaluation study illuminates the design phase, its developmental process, and the professional staff member’s experience of this newly implemented “Options” program. I focused specifically on Crawford County Community Mental Health Center’s innovative systemic approach to issues related to infant mental health. In this body of work, I describe the process of creating this innovative approach, identified how the program originators made decisions about their approach and how the approach is being operationalized on a daily basis by interviewing the clinicians, who are providing the services and the administrators who created and oversee the program. I utilized a qualitative approach in the design, transcription categorization, and data analysis. This formative evaluation used the “flashback approach” to tell the story of the evaluation findings, this included an Executive Summary. This study’s exploration yielded a clearer understanding of the developmental process of the infant mental health program and its initial implementation. The results of this evaluation revealed that there are a number of core program components (three levels of focus: child and family, program, and community and catchment area) that were organized and clearly disseminated throughout the staff. The interviews revealed that the program has encountered problematic issues including; policy and procedural agreements and mandates, staff turnover, program ownership and funding limitations. It grew increasingly clear that the value of the program’s positive impact on families outweighed the perceived hassle of establishing and implementing the program. This evaluation produced a number of program recommendations for program perpetuation and potential improvements. The program recommendations addressed the challenges facing the “Options Program” are explained. The future research implications of this formative evaluation are enumerated.
