Dentre essas células, nos últimos anos, os estudos têm se voltado para os neutrófilos, que exercem um importante papel efetor e modulador principalmente durante os estágios iniciais da infecção. As funções dessas células, incluindo as antimicrobicidas, podem ser reguladas por citocinas pró e anti-inflamatórias. Apesar dos mecanismos celulares e moleculares desse processo não estarem completamente entendidos, trabalhos nos últimos anos têm mostrado de forma bastante consistente o envolvimento dos “Toll Like Receptors” (TLRs). Neste contexto, os objetivos do presente trabalho foram: 1- Avaliar a expressão de TLR2 e TLR4 por neutrófilos humanos pré-ativados com GM-CSF, IL-15, TNF-a ou IFN-g e desafiados com cepa virulenta de Paracoccidioides brasiliensis cepa 18 (Pb18). 2- Avaliar a participação desses receptores na atividade fungicida, produção de H2O2 e das citocinas: IL-6, IL-8 e TNF-a e IL-10 por neutrófilos humanos pré-ativados com GM-CSF, IL-15, TNF-a, IFN-g e desafiados com Pb18. Nos ensaios referentes ao primeiro objetivo, as células foram tratadas por 18 horas com cada uma das citocinas ou LPS, posteriormente desafiadas com Pb 18 por 4 horas e avaliadas quanto a expressão de TLR2 e TLR4 por citometria de fluxo. Os resultados... / In paracoccidioidomycosis, phagocytic cells appear to provide one of the major lines of host defense. In this context, in last years, various studies have focused on the role of neutrophils that are involved in primary response to the fungus. Neutrophil functions are regulated by pro and anti-inflammatory cytokines. The molecular and cellular mechanisms involved in this process are not fully understood, but there are strong evidences about the involvement of “Toll Like Receptors” (TLRs). In this context, we aimed to evaluate TLR2 and TLR4 expression on human neutrophils primed with the cytokines GM-CSF, IL- 15, TNF-a or IFN-g and challenged with a virulent strain of Paracoccidioides brasiliensis (Pb18). Moreover, we asked if these receptors have a role on fungicidal activity, H2O2 and IL-6, IL-8, TNF-a and IL-10 production, by primed and challenged cells. In first assays, neutrophils were incubated with each of cytokines or LPS by 18 h, challenged with Pb18 by 4 h and evaluated by TLR2 and TLR4 expression by flow cytometry. In other assays, cells were incubated with each of cytokines or LPS by 18h, submitted to TLR2 and TLR4 blocking by specific monoclonal antibodies, challenged with Pb18 for 4 h and evaluated for fungicidal activity, H2O2 and IL-6, IL-8, TNF-a and IL-10 production, by ELISA. All cytokines increased TLR2 and TLR4 expression. Pb18 increased TLR2 expression inducing an additional effect to that of cytokines. On the contrary, it inhibited TLR4 expression. All cytokines increased neutrophils fungicidal activity, but this process was not associated with TLR2 or TLR4 expression. All cytokines and Pb18 increased H2O2 production, but in the same manner to fungicidal activity, the process was not associated to TLR2 or TLR4 expression. Embora vários dos mecanismos decorrentes dessa interação já estejam descritos, estudos sobre a participação de outros fatores potenciais podem ampliar sobremaneira o nosso entendimento sobre a imunopatogênese da PCM. Nesse sentido, os eicosanóides como as prostaglandinas (PGs) e leucotrienos (LTs) devem ser avaliados. Em trabalhos anteriores detectamos que o fungo induz a produção de PGs e LTs por monócitos humanos que, de uma forma autócrina, suprimem ou aumentam respectivamente, as atividades antifúngicas dessas células. No entanto, os estudos têm mostrado que além de induzir a produção de eicosanóides, os próprios fungos podem produzir esses mediadores, revelando um mecanismo de virulência que tem potencialmente grandes implicações para o entendimento dos mecanismos das infecções fúngicas. Baseados nesses estudos detectamos que o P. brasiliensis produz tanto PGs como LTs, usando fontes endógenas e exógenas de ácido araquidônico, e que ambos são importantes para a manutenção da viabilidade do fungo. Adicionalmente, os resultados sugeriram que o fungo utiliza as vias metabólicas da ciclooxigenase (COX) para a produção de PGs e da lipoxigenase (5-LO) para LTs. No presente trabalho, aprofundamos esses estudos objetivando avaliar se o fungo produz outras classes de eicosanóides, além da PGE2 e LTB4, se as PGs e LTs liberados pelo P. brasiliensis podem ser considerados como PGE2 e LTB4 autênticos, isto é semelhantes aos detectados para os mamíferos, assim como estabelecer de uma forma definitiva se a COX e a 5-LO são as vias metabólicas utilizadas... / Paracoccidioidomycosis (PCM) is a systemic mycosis caused by the dimorphic fungus Paracoccidioides brasiliensis. The establishment of paracoccidioidomycosis-infection or the evolution to paracoccidioidomycosis-disease in its more or less severe forms, primarily depends on complex host-parasite interaction, in which immune response mechanisms play an essential role. While many of these mechanisms have been elucidated, studies on the participation of other potential factors can greatly expand our understanding of PCM immunopathogenesis. Accordingly, eicosanoids such as prostaglandins (PGs) and leukotrienes (LTs) should be evaluated. In previous studies we found that the fungus induces the production of prostaglandins and leukotrienes by human monocytes that in an autocrine way suppress and increase, respectively, the antifungal activities of these cells. However, studies have shown that in addition to inducing the production of eicosanoids, fungi themselves can produce these mediators, showing a mechanism for virulence that has potentially important implications for understanding the mechanisms of fungal pathogenicity. Based on these studies, we found that P. brasiliensis produces both PGs and LTs, using exogenous and endogenous sources of arachidonic acid and that both are important for fungus viability. Additionally, the results suggested that the fungus uses cyclooxygenase (COX) and lipoxygenase (5-LO) metabolic pathways for PGs and LTs production, respectively. Here, we profound these studies evaluating whether PGs and LTs released by P. brasiliensis can be considered as authentic LTB4 and PGE2, i.e. similar to those detected for mammals. The host innate immune response against the fungus has been well characterized and several studies have clearly shown the important role played by phagocytic cells. Our laboratory has studied the relationship between human neutrophils (PMNs)/ Pb, focusing the effector mechanisms of these cells against the fungus. However, in last years, studies have shown that in addition to their phagocytic and killer functions, PMNs can modulate and instruct the immune response, since these cells have been shown to produce and release several cytokines. Thus, we evaluated whether PMNs stimulated with Pb can modulate the immune response to a Th1 phenotype through the production of IFN-γ (IFN-gamma), as well as the role of “pattern-recognition receptors” (PRRs) such as TLR2, TLR4 and Dectin-1 in this production. Paracoccidioidomicose e neoplasia

Pacientes com paracoccidioidomicose (PCM) podem apresentar comorbidades de natureza não infecciosa, tais como a doença de Hodgkin, leucemias e carcinomas. Neste sentido, o presente trabalho tem como objetivo avaliar clinicamente a associação entre paracoccidioidomicose e neoplasia. Foi realizado um estudo retrospectivo durante um período de 35 anos (1972- 2007), das fichas clínicas de 802 pacientes com diagnóstico da micose, provenientes dos arquivos do Laboratório de Micologia, Santa Casa - Complexo Hospitalar, Porto Alegre. Vinte e quatro casos (3,18%) apresentaram concomitantemente algum tipo de neoplasia. Foram estudados aspectos demográficos, hábitos de vida, manifestações clínicas, radiológicas e laboratoriais, evolução e tratamento realizado em ambas as doenças. O diagnóstico da paracoccidioidomicose foi feito através do exame micológico direto, histopatologia e/ou soromicologia; todos os casos de neoplasia tiveram confirmação histopatológica e/ou citopatológica. Todos os pacientes eram do sexo masculino, com idade entre 40 e 65 anos, com uma mediana de 53 anos. Manifestações relacionadas ao aparelho respiratório foram as principais queixas quando do diagnóstico da micose. Predominou o envolvimento pulmonar seguido por tegumentar e ganglionar. Em 12 pacientes o diagnóstico da PCM precedeu o da neoplasia, em 10 foram simultâneos, e em dois a neoplasia foi identificada anteriormente à micose. Houve concomitância de lesões entre as duas doenças em 66,6% dos casos. Os carcinomas representaram 95,8% das neoplasias, dos quais o de pulmão foi o mais freqüente, com 15 casos (62,5%); um paciente tinha doença de Hodgkin. A diferença entre a freqüência de carcinoma brônquico na população estudada e na população sem a micose foi estatisticamente significante (p = 0,0015). A terapêutica da PCM consistiu, isoladamente, de sulfas (29,2%), sulfametoxazol-trimetoprim (20,8%), cetoconazol (12,5%), itraconazol (12,5%) e anfotericina B (4,2%); houve combinação entre drogas em dois casos e substituição em uma oportunidade. A conduta antineoplásica mais empregada foi cirúrgica (11/24) seguida da radioterápica (9/24), quimioterápica (3/24) de um não se obteve relato. Cura micológica foi observada em doze casos, em nove a evolução é desconhecida e três pacientes foram ao óbito. Em conjunto, os resultados mostraram que existe uma associação entre a paracoccidioidomicose e neoplasia. / Patients with paracoccidioidomycosis may have comorbidities of non-infectious, such as Hodgkin's disease, leukaemia and cancers. Thus, the present study aims to evaluate the clinical association between the paracoccidioidomycosis and neoplasia. A retrospective study was conducted over a period of 35 years (1972-2007) through the revision of chips clinics from 802 patients with clinical diagnosis of mycosis, from the archives of Mycology Laboratory, Santa Casa - Complex Hospital, Porto Alegre. Twenty-four cases (3.18%) had some form of cancer concurrently. They were studied demographics, habits of life, leading clinical, radiological and laboratory, as well as developments in treatment done in both diseases. Paracoccidioidomycosis was diagnosed based on direct mycological exam, serologic and/or histopathologic evidence. All cases of cancer were confirmed by histopathologic and/or cytopathologic examination. All patients were male, aged between 40 and 65 years, with a median of 53 years. Disturbs related to the respiratory system were the main complaints. Lung involvement was the most predominant, followed by the tegumentary and lymphatic. In 12 patients the diagnosis of paracoccidioidomycosis preceded that of neoplasia, 10 were simultaneous, and in two to neoplasia was identified prior to mycosis. There was a concomitance between both of the illnesses in 66,6% of the cases. The cancers accounted for 95.8% of the cancers of the lung, which was the most frequent, with 15 cases (62.5%); one patient had Hodgkin's disease. The difference between the frequency of bronchial carcinoma in the studied population and the population without mycosis was statistically significant (p = 0.0015). The paracoccidioidomycosis therapy consisted alone of sulfas (29,2%), sulfamethoxazoletrimethoprim (20,8%), ketoconazole (12,5%), itraconazole (12,5%) and amphotericin B (4,2%); there was a drug combination in both cases and a substitution in a given opportunity as well. The most used antineoplastic procedure was the surgical one (11/24), followed by radiotherapy (9/24) and chemotherapy (3/24). The mycological cure was noticed in twelve cases; in nine of the cases the evolution is unknown and three patients died. Together, the results showed that there is an association between paracoccidioidomycosis and cancer.

Influência do cobre no padrão de expressão de genes envolvidos na interação de Paracoccidioides brasiliensis com a matriz extracelular

Uma das estratégias possivelmente utilizadas pelo patógeno seria a expressão de genes envolvendo adaptação às condições do hospedeiro, que pode também estar relacionadas à captação de micronutrientes. A matriz extracelular (MEC) desempenha um papel importante na regulação da adesão celular, diferenciação, migração e proliferação das células. Este estudo propõe uma análise de transcritos e de proteínas expressas em condições de depleção de cobre, na presença de quatro matrizes extracelulares – laminina, fibronectina e colágeno I e IV, mimetizando as condições de infecção por P. brasiliensis por meio das técnicas de RDA (Representational Difference Analysis) e eletroforese bidimensional. Para isso, o isolado Pb01 foi cultivado por 3 horas no meio quimicamente definido (MVM), depletado de cobre (Cu) e a seguir colocado em contato com os quatro diferentes componentes da MEC e a adesão foi avaliada por citometria de fluxo. Um aumento significativo (p ≤ 0,05) na adesão frente a todos os componentes da MEC foi observado quando o fungo foi cultivado sem Cu. Então, o RNA e os extratos protéicos foram obtidos de Pb sem Cu e Pb sem Cu em contato com os diferentes componentes da MEC. Ensaios de RDA foram realizados para demonstrar os genes envolvidos neste processo. Duas hibridizações foram realizadas nas proporções de 1:10 e 1:100 de tester e driver, respectivamente, com um excesso de driver para remover as seqüências comuns em ambas condições. Os produtos diferencialmente expressos foram amplificados, resultando em padrões distintos que foram seqüenciados... / Paracoccidioides brasiliensis (Pb) is the etiologic agent of paracoccidioidomycosis, a systemic mycosis of great importance in Brazil, which has the highest concentration of endemic areas for this disease in the world. One of the strategies used by the pathogen may be the expression of proteins related to the adaptation to the host conditions, which may also be related to the uptake of micronutrients. Extracellular matrix (ECM) plays an important role in the regulation of cell adhesion, differentiation, migration and proliferation of cells. This study proposes an analysis of transcripts and proteins expressed in condition of copper depletion in the presence of four components of extracellular matrix - laminin, fibronectin and collagen I and IV, mimicking the conditions of infection by P. brasiliensis, using techniques of RDA (Representational Difference Analysis) and two-dimensional electrophoresis. For this, we cultured the Pb 01 strain at 3 hours in a chemically defined media (MVM) with depletion copper (Cu). After, the fungus was placed at contact with the four differents ECM components and the adhesion was evaluated by flow cytometry. A significant increase of binding (p ≤ 0,05) to all ECM components was observed when the fungal was cultured without Cu. So RNA and protein extracts were obtained of Pb without Cu, and Pb without Cu in contact with the differents ECM components. RDA assay was developed to demonstrate the genes involved in this process. Two hybridizations were performed in the proportions of 1:10 and 1:100 of tester and driver respectively, with an excess of driver to remove the common sequences in both conditions. Devido à rica composição nutricional e suas propriedades medicinais o cogumelo comestível Agaricus blazei é amplamente utilizado na medicina tradicional. Dessa forma, o objetivo do presente trabalho foi avaliar se a suplementação nutricional de camundongos infectados por P. brasiliensis, com o cogumelo A. blazei, contribui para a manutenção de seu estado nutricional e de sua competência imunológica. Para isso, camundongos BALB/c foram infectados por via intratraqueal com leveduras de Pb 18 e suplementados dieteticamente com o cogumelo acrescentado à ração comercial. Os animais foram sacrificados aos 7 ou 14 dias pós-infecção para avaliar o grau de infecção no pulmão, baço e fígado e do efeito do tratamento sobre a capacidade de resposta imune celular. Os animais infectados apresentaram perda de peso no início da infecção, com recuperação após duas semanas. Os animais infectados e suplementados com ração 15% tiveram maior perda de peso, bem como menor nível de recuperação em relação ao peso inicial. Os parâmetros bioquímicos indicam que as funções hepáticas e renais sofreram alterações decorrentes da infecção mas os efeitos da suplementação nutricional foram de pouca relevância. O tratamento provocou exacerbação da infecção na fase inicial (7 dias), acompanhada de intensa redução da atividade linfoproliferativa. A resistência dos animais parece restaurada aos 14 dias nos animais que receberam 5% de cogumelo na ração, observando-se redução de fungos viáveis nos pulmões, restauração da capacidade de resposta proliferativa dos esplenócitos, aumento nos níveis de TNF-a e redução de IL-4 nos pulmões e discreto aumento de IFN-g no baço... / Paracoccidioides is one of the main systemic mycosis in Latin America and the host resistance to the fungus Paracoccidioides brasiliensis, is strongly dependent on the immune responsiveness. Due to it rich nutritional composition and medicinal properties, the edible mushroom Agaricus blazei is widely used as traditional medicine. Then, our goal was to investigate whether the nutritional supplementation with A. blazei helps P. brasiliensis-infected mice to keep their nutritional status and immunocompetence. For this purpose, BALB/c mice were infected for intratracheal route with Pb 18 yeasts and fed with comercial chow supplemented with dried and powdered A. blazei fruiting bodies. Animals were evaluated at the 7th and 14th day on the infection levels, in lung, spleen and liver, and on cellular immunoresponsiveness. Infected mice showed weight loose in the beginning of the infection that was partially restored after 2 weeks. Infected animals, supplemented with 15% of A. blazei showed the higher weight loose, as well as lower restoring levels. Biochemical parameters indicate that the infection itself interfered in the liver and kidney functions but no relevant effect of nutritional supplementation was detected. The treatment induced infection exacerbation at the first week that was followed by intense decrease of lymphoproliferative activity of spleen cells. The resistance of mice against P. brasiliensis appears to be restored by the 14th day in those mice eating feed with 5% A. blazei. This view is supported by the observation that such animals have lower number of viable fungus inside the lungs than controls (group Pb), and also restored the lymphoproliferative activity. We also observed increased levels of TNF-a and decreased levels of IL-4 in the lungs and slight increase of INF-g in the spleen. Altogether, our results partially supported our initial hypothesis... O objetivo do presente trabalho foi avaliar o efeito da IL-15 sobre a atividade fungicida de neutrófilos humanos desafiados com cepa virulenta do P. brasiliensis (Pb 18). Adicionalmente, testamos se essa atividade seria mediada por produtos do metabolismo oxidativo como anion superóxido e H2O2 e se estaria associada a alterações nos níveis das citocinas IL-8 e TNF- alfa. Neutrófilos do sangue periférico de indivíduos saudáveis , foram incubados na ausência ou presença de IL-15 (12,5 a 250 ng/mL) por 18h a 370C em tensão de 5% de CO2, infectados com Pb 18 por 4h e avaliados quanto a atividade fungicida , produção de ânion superóxido, H2O2 e quantificação nos sobrenadantes das citocinas TNF-a e IL-8. A préincubação de neutrófilos com IL-15 induziu um aumento significativo na atividade fungicida dessas células de maneira dose dependente. Adicionalmente após a ativação, foi detectado um aumento na produção de 51 anion superóxido e H2O2 por essas células, sugerindo a participação desses metabólitos como moléculas efetoras da atividade fungicida. O desafio das células em presença de catalase, levou a uma diminuição significativa da atividade fungicida comprovando o papel da H2O2 nesse processo. No entanto, os níveis de... / Interleukin-15 is a cytokine produced by a wide range of different cell types, including macrophages in response to lipopolysaccharide or microbial infection. This cytokine may play a crucial role in the activation of phagocytic cells against pathogens, mainly during innate immune response. The effects of IL-15 on human polimorphonuclear leukocyte fungicidal activity against high virulent P. brasiliensis strain were investigated. Prétreatment with IL-15 for 18h, increased neutrophils fungicidal activity in a concentration dependent manner. In addition, exposure to IL-15 induced enhanced neutrophil oxidative burst evaluated by superóxide anion and H2O2 release. Catalase inhibits fungicidal activity confirming a role for H2O2 in fungus Killing. In contrast, IL-8 and TNF-alfa levels were not affected by IL-15 suggesting that its effects were not mediated by these cytokines. Together, the results showed that IL-15 is a potent stimulant of antifungal activities of human neutrophils, at least in part, by a mechanism dependent on oxidative metabolism.

Afeta 0,7% a 3% das crianças na faixa etária pré-escolar, e está, nas crianças, relacionada com a hipertrofia das tonsilas palatina e/ou faríngeas. Em adultos, a SAHOS foi relacionada como fator de risco para doenças cardiovasculares e distúrbios metabólicos. Em crianças, há poucos relatos de alterações cardíacas como cor pulmonale ou HAS. Porém, as evidências fisiopatológicas da SAHOS permitem suspeitar que haja alterações estruturais e funcionais cardíacas, notadamente do ventrículo direito. Objetivos: Avaliar a função cardíaca de crianças com distúrbios respiratórios obstrutivos por hipertrofia das tonsilas, antes e após a cirurgia de adeno- e/ou tonsilectomia. Casuística e Métodos: Foram estudadas 40 crianças, de ambos os sexos, com idade entre 3 a 11 anos, das quais 30 estavam em seguimento no Ambulatório de Distúrbios do Sono, da disciplina de Otorrinolaringologia, FMB – UNESP, aguardando cirurgia de adeno- e/ou tonsilectomia por hipertrofia das tonsilas e distúrbios respiratórios obstrutivos, caracterizados por roncos, pausas respiratórias referidas e sono agitado. As outras 10 crianças eram controles saudáveis, os dois grupos estando homogêneos em relação ao sexo, idade, peso e altura. Todas as 40 crianças foram submetidas... / Introduction: Obstructive sleep apnea is characterized by intermittent partial or complete obstruction of the upper airway, causing hypoxemia, hypercapnia and sleep fragmentation. It affects 0,7% to 3% of the pre-school children, and in children it is closely related to enlarged tonsils. In adults, sleep apnea has been described as na independent risk factor for cardiovascular disease and metabolic disorders. In children, there a few studies for cardiovascular disfunction, most of them related to cor pulmonale or hypertension. Even though, the pathophysiologic mechanism of OSA permit to suspect of structural and functional cardiac changes, mostly of the right chambers. Aim: To study the heart function in children with sleep-related breathing disorders and enlarged tonsils, before and after adeno- and/or tonsillectomy. Patients and Methods: We studied 40 children, of both genders, aged between 3 and 11 years. Thirty children were at follow-up of Botucatu Medical School – State University of São Paulo due to hypertrophy of the tonsils and clincal complaints of Obstructive Sleep Apnea, as snoring, referred apneas and restless sleep. The other ten children were healthy controls; both groups were homogeneous in gender, age, weight and height. All 40 children were submitted to echocardiogram, analysing the four chambers, in systole and diastole. We obtained measures for diameter, area, variation of area, ejection fraction and the flow of tricuspid, pulmonary, mitral and aortic valves. Special attention was given to the right chambers, obtaining diameter, area and variation of area, measures not routinely in an echocardiogram. A maioria dos pacientes com a FC da doença, mesmo após tratamento eficaz, apresenta sequelas, principalmente fibrose pulmonar (FP) e enfisema. Os problemas sociais, econômicos e psicológicos desencadeados pela FP são subestimados. Apesar da fibrogênese na PCM ser reconhecida como um processo precoce, seus mecanismos não são totalmente conhecidos. No presente estudo, determinamos o perfil proteômico sérico de pacientes com PCM-FC com diferentes desfechos de FP. Vinte e nove pacientes com PCM pulmonar foram acompanhados periodicamente e as amostras de soro sanguíneo foram coletadas em quatro momentos: antes do tratamento (M0), cura clínica (M1), cura sorológica (M2) e cura aparente (M3). O grupo controle foi constituído por amostras de quinze indivíduos fumantes, pareados com idade e sexo dos pacientes avaliados. Ao final do tratamento (M3), os pacientes foram submetidos à avaliação de tomografia computadorizada e dois grupos de pacientes foram formados com base nos resultados de PF: PF menor (n = 17) e PF maior (n = 12). As proteínas de elevada abundância no soro foram removidas e, em seguida, submetidas à análise proteômica shotgun através do sistema nanoACQUITY UPLC-Xevo QTof MS. A identificação e quantificação das proteínas foram realizadas usando o software PLGS Expression E. Análise de enriquecimento e análise de rede de interação proteína-proteína (PPI) foram realizadas usando o software String. Antes do tratamento, os pacientes com PF menor, em comparação aos controles, apresentavam 91 proteínas diferencialmente expressas (60 sub-reguladas 31 superreguladas). Soro de pacientes com maior PF apresentaram 70 proteínas diferencialmente expressas (66 sub-reguladas 4 reguladas). A maioria dessas proteínas estavam envolvidas na regulação do processo celular e resposta inflamatória. Nas condições ensaiadas, o estudo permitiu identificar as proteínas APOH, A2M, GC e SERPINA1 como alvos envolvidos nos mecanismos moleculares da fibrogênse pulmonar na paracoccidioidomicose. Foi observado, ainda, que o momento de cura clínica apresentou maior atividade pró-fibrótica durante o seguimento dos pacientes. Além disso, foram identificadas 20 proteínas candidatas a biomarcadores no prognóstico da fibrose pulmonar, conforme segue: família das hemoglobulinas (HBB, HBD, HBG2, HBE1, HBG1, HBA1), ENO3, A2M, APOH, GC, IGHG-1, -2 e 3, IGLC1, -2 e -3, HP, SERPINA1, LRG1 e PZP. Nossos achados fornecem novos alvos para estudos futuros envolvendo a FP na PCM, assim como biomarcadores séricos em potencial. / Paracoccidioidomycosis (PCM) is systemic mycosis caused by fungi of the genus Paracoccidioides; its main clinical forms are acute / subacute, chronic (CF) and residual. Most FC-PCM patients, even after effective treatment, present sequelae, mainly pulmonary fibrosis (PF) and emphysema. The social, economic and psychological problems triggered by PF are underestimated. Although fibrogenesis in PCM is recognized as an early process, its mechanisms are not fully known. In the present study, we aimed to determine the serum proteomic profile of patients with FC-PCM with different (PF) outcomes. Twenty-nine patients with pulmonary PCM patients were followed up periodically, and blood serum samples were collected in four moments: before treatment (M0), clinical cure (M1), serological cure (M2), and apparent cure (M3). For control group (n=15), serum samples of sex and age-matched smokers were also collected. At the end of treatment (M3), the patients were submitted to CT Scan evaluation and two groups of patients were formed based on PF outcomes: minor PF (n=17) and major PF (n=12). High-abundance proteins were removed from serum and submitted to shotgun proteomic assay using a nanoACQUITY UPLC-Xevo QTof MS system. The identification and quantification of proteins were performed using PLGS Expression E software. Enrichment analysis and protein-protein interaction (PPI) network analysis were performed using String software. Before treatment, patients with minor PF, in comparison to controls, showed 91 differentially expressed proteins (60 down-regulated and 31 up-regulated). Serum of patients with major PF exhibited 70 differentially expressed proteins (66 down-regulated and 4 up-regulated). Most of these proteins are involved in the regulation of cellular process and inflammatory response. The results allowed the identification of APOH, A2M, GC and SERPINA1 as targets involved in the molecular mechanisms of pulmonary fibrogenesis in paracoccidioidomycosis. It was also observed that the clinical cure presented greater pro-fibrotic activity during the follow-up of the patients. In addition, 20 candidate biomarkers were identified in the prognosis of pulmonary fibrosis, as follows: hemoglobin family (HBB, HBD, HBG2, HBE1, HBG1, HBA1), ENO3, A2M, APOH, GC, IGHG-1, -2 and 3, IGLC1, -2 and -3, HP, SERPINA1, LRG1 and PZP.
