Fatores prognósticos de melanoma cutâneo em um estudo de base populacional em Goiânia

É considerado o mais grave dos cânceres de pele por induzir metástase mais freqüente, apresentar uma alta mortalidade, sobretudo em lesões com diagnóstico tardio e por ser resistente as terapêuticas disponíveis (quimio e radioterapia). A presente pesquisa propõe-se a descrever os fatores prognósticos de pacientes com melanoma cutâneo do Registro de Câncer de Base Populacional de Goiânia (RCBPG), entre janeiro de 1998 a dezembro de 2003, período em que foram registrados 245 casos novos de melanoma. Destes, 185 foram incluídos no estudo por serem portadores de melanoma cutâneo residirem no município de Goiânia. Os sessenta pacientes restantes foram excluídos da pesquisa por apresentarem melanoma com primário desconhecido, melanoma extracutâneo ou por residirem em outro município. Foram coletados, por meio de um questionário, dados do RCBPG dos pacientes que apresentavam diagnóstico apenas de melanoma cutâneo primário. Após a coleta desses dados, foi realizado um estudo descritivo do tipo coorte de base populacional. A importância prognostica de quatro variáveis histológicas (Breslow, Clark, tipo histológico e número de neoplasias primárias) e três variáveis clínicas (sexo, localização da lesão e extensão) foram estudadas no grupo de 185 pacientes com melanoma cutâneo e foram comparados com a literatura mundial. Os resultados do estudo evidenciaram que os homens apresentaram melanomas mais espessos em comparação com as mulheres, dado que se refletiu em uma maior mortalidade e uma menor sobrevida para o sexo masculino. Nos homens, as lesões localizadas no tronco foram mais freqüentes e, nas mulheres, as das extremidades inferiores. A localização na cabeça e pescoço foi mais freqüente em homens e mulheres acima de sessenta anos. O melanoma extensivo superficial foi o tipo histológico mais freqüente. O melanoma nodular, o segundo de maior freqüência (17%), estava associado a tumores mais espessos. O melanoma cutâneo localizado, sem metástase detectada, foi à extensão de maior freqüência. A metástase à distância foi mais freqüente do que a loco-regional para linfonodos. As principais características dos pacientes e do tumor relacionados a pior prognóstico foram: melanomas do tipo histológico nodular (RH: 5,11; p=0,001) tumores com maior espessura (> 1,70 mm) (RH:12,7; p≥0,02) e os localizados nos pés (RH:4,84; p=0,005). Esses fatores contribuíram para um maior número de óbito e uma menor sobrevida dos pacientes. O aumento do nível de Clark apresentou relação direta com maior número de óbito e com menor sobrevida, o que, não se traduziu em uma associação significativa com pior prognóstico. _____________________________________________________________________________________ ABSTRACT / Melanoma is a cancer that affects mostly the skin. It is the 3rd most frequent type of cancer, after basal cell carcinomas and squamous carcinomas and is clinically characterized by the presence of an irregular hyperchromic macula. It is considered the most serious type of skin cancer because it induces metastases more frequently, the mortality rate is high, especially in lesions diagnosed at a later stage, and finally because it is resistant to the therapeutic options currently available (chemotherapy and radiotherapy).This study aims to describe prognostic factors of patients with cutaneous melanoma, listed in the Population-Based Cancer Registry of Goiânia (RCBPG), from January, 1998 to December, 2003. During that period of time, 245 new cases of melanoma were recorded. From these 245 cases, 185 were included in the study as these individuals with cutaneous melanoma lived in the county of Goiânia. The remaining 60 individuals were excluded because they presented with a melanoma with an unknown primary, ocular melanoma, melanoma of the mucosa, genital melanoma, visceral melanoma or because they lived in another county. RCBPG data from the patients with a diagnosis of primary cutaneous melanoma were obtained by means of a questionnaire and served to perform a population-base cohort descriptive study. The prognostic importance of four histological variables (Breslow, Clark, histogenic type and number of primary neoplasms) and three clinical variables (sex, lesion site and extension) were studied in the series of 185 patients with cutaneous melanoma. The results of this study showed that men presented with thicker melanomas, when compared to women. This information translated into higher mortality and shorter survival time for male individuals. Lesions located in the trunk, followed by those in the head and neck were more frequently found in men and lesions in the lower limbs, followed by locations in the head and neck were more commonly found in women. A head and neck location was more frequent in men and women above 60 years of age. Extensive superficial melanomas were the predominant histogenic type. Nodular melanomas were the second more frequent types (17%) and were associated to thicker tumors. The most frequent extension was the in situ cutaneous melanoma. Distant metastases were more frequent than those in loco or regional, for lymph nodes. During the time this study was conducted, no sentinel lymph node biopsies were performed to evaluate micrometastases of melanomas and this may have contributed to the result. The main characteristics of patients and of tumors related to the worst prognosis were: nodular melanomas (RH: 5,11; p=0,001), thicker tumors (>1.70 mm) (RH:12,7; p≥0,02) and those located in the feet (RH:4,84; p=0,005). These factors contributed to more deaths and a shorter survival time of the patients included in this study. The increase in the Clark level showed a direct relationship to a higher number of deaths, with a shorter survival time. However, this was not translated as a negative association with the worst prognosis in this study.

Estudos correlacionando o uso de Medicina Alternativa ou Complementar e a Dermatite Atópica não são raros fora do Brasil, porém são inéditos em nosso país. Medicina Alternativa ou Complementar (MAC) é definida como diagnóstico, tratamento e/ou prevenção que complementa a medicina convencional por contribuir como um todo, por satisfazer as demandas não encontradas na medicina ortodoxa ou por diversificar o quadro conceitual da medicina e inclui formas de terapia que não possuem bases científicas ou eficácia comprovadas por métodos científicos. O objetivo deste estudo foi de investigar a prevalência do uso da Medicina Alternativa ou Complementar por pacientes pediátricos com dermatite atópica, identificar os fatores que podem influenciar na decisão de usar tais terapias, identificar os fatores renda familiar, tamanho da família e escolaridade dos responsáveis/cuidadores dos pacientes, identificar a natureza dos recursos alternativos utilizados pelos pacientes participantes deste estudo, verificar se houve piora do quadro clínico após uso de MAC, verificar se o tempo de duração da doença está associado a maior probabilidade do uso de MAC e avaliar o fator custo do tratamento com recursos de MAC. Desenvolveu-se o estudo com 85 pacientes portadores de Dermatite Atópica, atendidas no Serviço de Dermatologia do Hospital Universitário de Brasília. Foi aplicado um questionário aos pais/cuidadores dos pacientes logo após a consulta médica. Um total de 54 (63,5%) de pacientes utilizou algum tipo de recurso de Medicina Alternativa ou Complementar, indicação de parentes e amigos foi a razão pela qual a maioria utilizou MAC. Os fatores renda familiar, tamanho da família e escolaridade dos pais/cuidadores não influenciaram na decisão de uso de recursos alternativos. Remédios à base de plantas e homeopatia foram os recursos alternativos mais utilizados. A cronicidade da doença está relacionada a maior probabilidade de uso de recursos alternativos. O custo do tratamento alternativo não influenciou o uso de MAC. Muitos dos tratamentos foram referidos pelos pacientes como inúteis ou como provocadores de deterioração do quadro clínico geral. _________________________________________________________________________________________ ABSTRACT / Atopic dermatitis is a common skin disease among children, with difficult management, that involves genetic and environmental factors. Studies correlating the use of Complementary or Alternative Medicine and atopic dermatitis are not rare out of Brazil, but are unpublished in our country. Complementary or Alternative Medicine (CAM) is defined as diagnosis, treatment and / or prevention which complements conventional medicine by contributing as a whole, by meeting the demands not found in orthodox medicine or by diversifying the conceptual framework of medicine and that includes forms of therapy that has no scientific basis or proven effectiveness by scientific methods. The objective of this study was to investigate the prevalence of use of CAM by pediatric patients with atopic dermatitis, identify the factors that may influence the decision to use such therapies, to identify the factors family income, family size and caretakers schooling, to identify the nature of the alternative resources used by patients participating in this study, to verify if there was worsening of symptoms after the use of MAC, to verify if the disease chronicity is associated with greater likelihood of using CAM and to evaluate the factor cost of alternative treatment. The study was developed with 85 patients with atopic dermatitis, that were seen at the Dermatology Service of the University Hospital of Brasília. We applied a questionnaire to parents / caregivers of patients after a medical consultation. A total of 54 (63.5%) of patients used some type of action for Complementary or Alternative Medicine, word by mouth was the most cited reason for trying CAM. The factors family income, family size and caretakers schooling did not influence the decision to use CAM. Herbal medicines and homeopathy were the most used alternative resources and 18,8% of patients mentioned some sort of skin worsening after CAM use. Disease chronicity is related to greater likelihood of using alternative resources. The alternative treatment cost did not influence the use of CAM. Many of these alternative treatments were reported by patients as useless or as provocative deterioration of clinical general.

Comprometimento nutricional do portador está relacionado com a gravidade das complicações, sendo as necessidades nutricionais do portador um reflexo da idade, extensão das bolhas, presença de infecção, necessidade de crescimento e manifestações clínicas. Objetivo: Investigar a adequação do consumo alimentar habitual de energia, da contribuição percentual de macronutrientes na dieta, da quantidade de proteínas, de ferro, zinco e fibra alimentar, bem como, a evolução antropométrica e as principais manifestações clinicas que afetam o consumo alimentar de dez portadores de Epidermólise Bolhosa, menores de 19 anos de idade, acompanhados nos ambulatórios de nutrição pediátrica e de dermatologia do Hospital Universitário de Brasília. Método: Trata-se de um estudo transversal analítico, do tipo estudo de uma série de casos, de uma coorte clínica de portadores de dois tipos diferenciados de epidermólise bolhosa a simples e a distrófica recessiva. E um recorte longitudinal de dados antropométricos desde a primeira consulta até maio de 2013. A primeira etapa do estudo refere-se à revisão de literatura acerca dos principais conceitos e aspectos do tratamento nutricional na EB. Foram coletados dados antropométricos, de consumo alimentar e informações complementares quanto às condições do nascimento, da alimentação no primeiro ano de vida e de manifestações clinicas que afetam o consumo alimentar, bem como identificação do percentual da superfície corporal atingida por bolhas. Elaboradas curvas de crescimento de acordo com os padrões da Organização Mundial da Saúde. O método de avaliação do consumo probabilidade de consumo inadequado para 90% dos casos. Consumo habitual de ferro e zinco se mostrou adequado para 90% dos casos. Adequação do consumo habitual de fibras apresentou-se inconclusiva para 90% dos casos. Conclusão: Dados do primeiro ano de vida irão contribuir para delineamento do perfil nutricional do portador. Monitoramento do peso e da altura deve ser feito em curto intervalo a fim de intervir em tempo oportuno. Estabelecimento das necessidades energéticas parece ser o maior desafio, devido à interação entre aumento das necessidades nutricionais pela atividade da doença, ao surgimento de manifestações clínicas que afetam o consumo alimentar e à necessidade de crescimento. ______________________________________________________________________________________ ABSTRACT / Introduction: Epidermolysis bullosa is composed of a group of inherited disorders characterized by the occurrence of blisters on the skin and mucous membranes as a result of mechanical trauma. Nutritional impairment carrier is associated with the severity of complications, and the nutritional requirements of patients reflects the age, extent of blistering, presence of infection, catch up growth and clinical manifestations. Purpose: Analyze habitual food intake, growth curves and main clinical manifestations that affect food consumption of ten patients with epidermolysis bullosa less than nineteen years followed at Hospital Universitário de Brasília between 2004 and 2013. Methods: This is an analytical cross-sectional study, comprises a series of cases study of a clinical cohort of patients with two different types of epidermolysis bullosa: simple and recessive dystrophic. In addition, there is a longitudinal cut of anthropometric data from the first visit until May 2013. The first stage of the study refers to the literature review of key concepts and aspects of nutrition therapy in EB. In the second stage, anthropometric data, food consumption and additional information regarding the conditions of birth, feeding in the first year of life and clinical manifestations that affect food consumption, as well as identifying the percentage of body surface affected by bubbles were collected. Then, growth curves prepared in accordance with the standards of the World Health Organization. The measurement of food intake was the dietary record of seven consecutive days. It was applied the Dietary Reference Intakes in assessing the adequacy of habitual protein intake in grams per day of iron, zinc and dietary fiber. Specific equation was used for EB patient and percentage of average need for estimating energy requirement. It was also used the acceptable macronutrient distribution ranges for evaluating the adequacy of the percentage contribution of macronutrients in total dietary energy. Results: Production of a review article entitled " Nutritional aspects of children and adolescents with epidermolysis bullosa: a literature review ". In a series of cases, All the patients were born at term, 90% with adequate birth weight. Age at diagnosis ranged from birthday until after the first year of life. Most patients (90%) received breastfeeding. Complications in the introduction of complementary foods were found in over half the cases of patients with dystrophic epidermolysis of the recessive type. Clinical manifestations were present in all patients. Microstomy reached 70% of the cases and blistering in the oral cavity 60% of cases. Dysphagia, chewing problems, ankyloglossia, odynophagia and gastroesophageal reflux also were identified. All patients were fed orally and the consistency of supply ranged from normal until liquid. As for the growth curves, a carrier of epidermolysis simplex type grew at or above the 85th percentile. All carriers of recessive dystrophic epidermolysis had growth in weight and height for age with values below the 3rd percentile, large on the bow curve. Adequacy of energy consumption in specific equation to estimate the energy requirements of patients showed deficient for more than half of patients with recessive dystrophic epidermolysis. Habitual protein intake showed a low probability of inadequate intake to 90% of cases. Habitual consumption of iron and zinc was adequate for 90% of cases. Adequacy of usual dietary fiber intake was inconclusive in 90% of cases. Conclusion: Data from the first year of life will help to design the nutritional profile of the carrier in the first year of life. Monitoring of weight and height should be done in a short period in order to intervene in a timely manner. Establishment of energy needs seems to be the biggest challenge, due to the interaction between increased nutritional needs for disease activity, the appearance of clinical symptoms that affect food consumption and the need for growth.

Prevenção e gerenciamento de lesões de pele : validação de um protocolo em uma Unidade de Terapia Intensiva

No. of bitstreams: 0 Previous issue date: 2018-11-29 / This is a methodological study, which the general aim was to validate a protocol of prevention and management of the healing of skin lesions, in an intensive care unit of Ceará. Its relevance is to improve the clinical and assistance practice and to promote ability and safety for the process of prevention, evaluation and monitorization of the tissue repair in the treatment of a pressure injury, among others. Guiding a performance based on the thinking to acting, consolidate by the scientific methods. The research was developed on the period of May to August in the year of 2018, after the Ethic Committee authorization, following the methodological propose. The phase of the data collection and evaluation occurred in a synchronic way, as follows: initially it was made an individual approach to the nurses, raising the importance of a prevention of skin lesions protocol and requesting the fill of the ¿pre-teste form¿ for the evaluation of the existent protocol, as well the knowledge about the PUSH scale (Pressure Ulcer Scale for Healing). Posteriorly, it was performed an expository class about the uses of the PUSH scale, raising a group discussion about the applicability of this scale on the management of the healing of the pressure lesions, listing the convergent and divergent. As well, the positives and negatives points of the prevention of skin lesions protocol. Before the systematic review executed on the first moment, as the data collected about the positives and negatives points of the protocol already implanted protocol, which was elaborated by the author. It was accomplished the necessary modifications in the prevention of skin lesions protocol, adding the professional¿s suggestions, the present scientific evidences and the PUSH scale. It was named Protocolo para Prevenção e Gerenciamento de Lesões de Pele (PPGLP). The same instrument was evaluated by judges, specialists¿ nurses in Dermatological Nursing from the Sociedade Brasileira de Feridas e Estética (SOBENFeE), with experience in skin lesions prevention and treatment, through an electronic form, built based on AGREE (Appaisal of Guidelines for Research & Evaluation). The data analysis was made through the adequacy of clinical guideline calculus proposed by the AGREE II. Followed as appropriate 75% of agreement, to consider the validated protocol. On the implantation moment, it was notorious the easy applicability of the PUSH scale and suitability of the conducts proposed in the protocol, as the nurses reports in the ¿Pre-test Form¿. The protocol had the consistent punctuations to the development of a good quality guideline, liable of implementation on the health service purpose to improve the assistance and clinical practice and promote more ability and safety to the prevention, evaluation and monitorization process of the tissue repair on the skin lesions treatment. / Trata-se de estudo metodológico, que tem como objetivo geral validar um protocolo de prevenção e gerenciamento da cicatrização de lesões de pele, em uma unidade de terapia intensiva do Ceará. Sua relevância está em aperfeiçoar a prática clínica e assistencial e dar maior habilidade e segurança para o processo de prevenção, avaliação e monitorização do reparo tissular no tratamento de lesão por pressão, dentre outras. Norteando uma atuação alicerçada no pensar para agir, com raízes no método científico. A pesquisa foi desenvolvida no período de maio a agosto do ano de 2018, após a autorização do Comitê de Ética, seguindo a metodologia proposta. A fase de coleta e análise de dados ocorreu de forma sincrônica, como se segue: realizada inicialmente uma abordagem individual com os enfermeiros, suscitando a importância do protocolo de prevenção de lesões de pele e solicitando o preenchimento do ¿formulário pré-teste¿ para avaliação do protocolo existente, bem como os saberes sobre a escala de PUSH (Pressure Ulcer Scale for Healing). Posteriormente, foi realizada uma aula expositiva a respeito do uso da escala de PUSH, suscitando uma discussão do grupo sobre aplicabilidade da mesma no monitoramento da cicatrização das lesões por pressão, elencando pontos convergentes e divergentes. Assim como, os pontos positivos e negativos do protocolo atual existente. Diante da revisão sistemática realizada no primeiro momento desse projeto, bem como dos dados coletados sobre os pontos positivos e negativos do protocolo já implantado no serviço, protocolo este elaborado pela autora. Foram realizadas as modificações necessárias no protocolo de prevenção de lesões de pele, acrescentando as sugestões dos profissionais, as evidências científicas atuais e a escala de PUSH. Este foi denominado de Protocolo para Prevenção e Gerenciamento de Lesões de Pele (PPGLP). O mesmo foi validado por juízes, enfermeiros especialistas em Enfermagem Dermatológica da Sociedade Brasileira de Feridas e Estética (SOBENFeE), com experiência na prevenção e tratamento de lesões de pele, por meio de um formulário eletrônico construído, baseado no AGREE (Appraisal of Guidelines for Research & Evaluation). A análise dos dados foi realizada mediante o cálculo da adequabilidade da diretriz clínica proposto pelo próprio AGREE II. Seguiu-se como adequado 75% de concordância, para considerar o protocolo validado. No momento de implantação do protocolo, foi notória a fácil aplicabilidade da escala de PUSH e adequabilidade das condutas propostas no protocolo, conforme relatos dos enfermeiros em resposta ao ¿Formulário pós-teste¿. O protocolo obteve pontuações condizentes com o desenvolvimento de uma diretriz de boa qualidade, passível de implementação no serviço de saúde com a finalidade de aperfeiçoar a prática clínica e assistencial e dar maior habilidade e segurança para o processo de prevenção, avaliação e monitorização do reparo tissular no tratamento de lesões de pele.

Com o objetivo de comparar diferentes produtos tópicos e procedimentos na busca da cicatrização da pele, foram utilizados doze lagartos adultos, machos, da espécie Tupinambis merianae. Em cada animal foram produzidas quatro lesões circulares na pele do dorso, dispostas em quadrilátero. Compostos três grupos de quatro animais, o Grupo 1 compreende o comparativo entre solução fisiológica e policresuleno; no Grupo 2, tintura de iodo a 2% e o não tratamento, considerado como controle; o Grupo 3 clorexidine e o enxerto autólogo de pele. As lesões do lado direito destinaram-se à análise macroscópica diária, com planimetria a cada sete dias e observação do tempo total de cicatrização. Das lesões esquerdas, foram coletadas amostras para biópsias e conseqüentes exames histopatológicos, a cada sete dias. Os lagartos apresentaram processo cicatricial semelhante aos mamíferos, notoriamente mais lento, entretanto. O enxerto não se mostrou vantajoso neste experimento, sofrendo necrose nos quatro animais. O policresuleno destacou-se por impermeabilizar a ferida em 4 dias, enquanto que os demais grupos obtiveram o mesmo efeito entre 8 e 12 dias. As lesões se mostraram completamente epitelizadas entre 40 e 53 dias, não havendo diferença entre os grupos quanto ao tempo total de cicatrização, retração tecidual e aparência da cicatriz / Abstract: Skin lesions in reptiles are caused by different factors and result in scarring and dysecdysis, predisposing to dermatitis. It may let the animal out of exhibitions or lead to the abandonment when created as pets. In order to compare different topical products and procedures for skin repair, twelve adult male tegu lizards (Tupinambis merianae) were used. Four circular square arranged skin lesions were created at the dorsum of each animal. Animals were divided in three groups (four animals/group) and each group was treated with two medications. In Group 1, animals were treated with saline and policresulen, in Group 2, with tincture of iodine and untreated (control group), and in Group 3, chlorhexidine and autologous skin graft. The lesions on the right side were macroscopicaly evaluated daily and the planimetry was realized every seven days. The left lesions were sampled every seven days for histopathological examination. The lizards showed healing process similar to mammals; however, it was notably slower. In this experiment, the skin graft showed no advantage, undergoing necrosis in all four animals. Policresulen closed the wounds in four days, while other groups had the same effect between 8 and 12 days. As avaliações e a padronização do HRM foram realizadas em cinco cavalos afetados (GA) e cinco não afetados (GC). Adicionalmente, cinco animais heterozigotos (GH) foram utilizados para padronizar o HRM. A ocorrência de heterozigotos foi determinada em 690 animais. Diversas regiões da pele foram mensuradas com cutímetro no GA e GC. Biópsias de pele foram submetidas aos exames histopatológico e ultraestrutural. Avaliação histopatológica foi realizada por dois patologistas. O exame oftalmológico incluiu, além das avaliações rotineiras, aferição dos diâmetros da córnea, paquimetria e biometria. Foi extraído DNA do sangue colhido do GA, GC, GH e de 690 cavalos e o HRM foi validado. Observou-se menor espessura de pele no GA. A sensibilidade e especificidade do diagnóstico histopatológico da pele dependeram do avaliador e da região, respectivamente. Foram observados menor espessura e maior curvatura e diâmetros da córnea no GA. O HRM apresentou elevadas acurácia e precisão. A frequência de heterozigotos foi de 4,7%. Apesar do padrão regional dos sinais dermatológicos, a diminuição da espessura da pele não é regional. Para o diagnóstico histopatológico, recomenda-se realizar biópsia de pele no pescoço, garupa ou dorso. A relevância clínica dos achados oftalmológicos deve ser investigada. O ensaio de HRM padronizado será útil na seleção dos acasalamentos, visando minimizar a ocorrência da doença / The present study was conducted to characterize the dermatological, ophthalmological, and morphological findings from horses affected with Hereditary Equine Regional Dermal Asthenia (HERDA) and to standardize a High Resolution Melting (HRM) genotyping assay to determine the frequency of carriers. The evaluations and HRM standardization were performed in five affected (AG) and five non-affected (CG) horses. Additionally, five heterozygous (HG) horses were used to HRM standardization. The frequency of carriers was determined in 690 horses. Several skin regions of both groups were measured with a cutimeter Skin biopsies were submitted to histopathological and ultrastructural evaluations. Histopathological evaluation was performed by two pathologists. Ophthalmology included, besides the routine evaluations, corneal diameters measurement, pachymetry, and biometry. HRM was validated using purified DNA from blood samples of the AG, CG, HG and 690 horses. Skin thickness decrease was observed in the AG. Histopathological sensitivity and specificity to diagnose HERDA was dependent on the evaluator and region, respectively. HERDA horses exhibited decreased corneal thickness and increased corneal curvature and corneal diameters. The HRM assay resulted in high accuracy and precision. The estimated carrier frequency was 4.7%. Despite of the regional pattern of the dermatological signs, the decrease of skin thickness from HERDA horses is not regional. Skin samples of the neck, croup or back are recommended to diagnose HERDA. The relevance of the ocular findings should be further investigated. The standardized HRM assay will be useful in the management of breeding programs to minimize the occurrence of this disease

Avaliar a sobrevida, o padrão de atividade da doença, o dano e o impacto no crescimento em uma coorte de LESJ. Os pacientes foram classificados pelos padrões de atividade ativo crônico (AC), recidivante-remitente (RR) e quiescente prolongado (QP) de acordo com o Modified SLEDAI-2K e ECLAM, pontuados em cada consulta e calculando-se a área sob a curva da atividade em intervalos de observação. O dano foi avaliado na última consulta pelo Pediatric SLICC/ACR Damage Index (Ped/SDI), para aqueles com acompanhamento por mais de um ano. Foi realizada análise de sobrevida considerando óbito, dano e atraso no crescimento como desfechos para a coorte e estratificada pelos padrões AC e RR. O modelo de Cox foi utilizado para identificar fatores preditivos para esses desfechos. Foram revisados 37 casos com mediana de idade ao diagnóstico de 11,2 anos e do tempo de acompanhamento de 2,7 anos. O padrão RR foi encontrado em 67,5%, AC em 29,8% e QP em 2,7%, segundo a classificação pelo Modified SLEDAI-2K. Pelo ECLAM, as frequências observadas foram 45,9%, 48,7% e 5,4%, respectivamente. A taxa de sobrevida em 5 anos foi 90%. Dano e atraso no crescimento foram observados em 62,5% e 31,3%, respectivamente. O tempo médio para o dano foi 4,8 ± 0,5 anos. A probabilidade de dano foi 10% com 1 ano, 15% com 2 anos, 32% com 3 anos e 47% com 5 anos de acompanhamento. Para o atraso no crescimento, as probabilidades foram 3%, 12%, 17% e 31% respectivamente, com tempo médio de 5,2 ± 0,4 anos. O atraso no crescimento foi associado com a ... / Activity and damage tools have been used to measure Juvenile Systemic Lupus Erythematosus (JSLE) outcome, aiming at assessing the real impact on morbidity. Evaluate survival, disease activity patterns, damage and growth failure in a cohort of JSLE patients. Patients were classified by disease activity patterns as chronic active (CA), relapsing-remitting (RR) and long quiescent (LQ) disease course according to Modified SLEDAI-2K and ECLAM, scored at each visit and summarized by the area under curve plotted between two evaluation intervals. Damage was scored at the last visit by the Pediatric SLICC/ACR Damage Index (Ped/SDI), selecting cases followed for more than one year. Survival curves considering the events death, damage and growth failure were plotted, for the whole cohort and stratified by the CA and RR patterns. Cox model was built to identify outcome predictors. Thirty seven patients with 11.2 years median age at diagnosis and 2.7 years median follow up were reviewed. The RR pattern was found in 67.5%, CA in 29.8% and LQ in 2.7%, according to Modified SLEDAI-2K. By ECLAM, the frequencies were 45.9%, 48.7% and 5.4%, respectively. The 5-year survival rate was 90%. Damage and growth failure were observed in 62.5% and 31.3%, respectively. The mean time for damage was 4.8 ± 0.5 years. A principal causadora da acne é a infecção pela bactéria Propionibacterium acnes (P. acnes). Atualmente, as opções de tratamento para acne vulgar são numerosas, porém muitas vezes com extensos efeitos colaterais. Fatores como resistência aos antibióticos, e a ação inicial lenta das terapias tópicas têm levado pesquisadores a buscar tratamentos alternativos. Desta forma, o presente estudo objetivou avaliar a eficácia da inativação fotodinâmica mediada por curcumina dissolvida em caldo (TrypcticSoyBroth) TSB e solução de sacarose 0,5% e a combinação dessa terapia com a tetraciclina, contra P. acnes em fase planctônica e biofilme. A viabilidade celular foi avaliada através da quantificação das colônias formadas por mililitro de amostra (UFC/mL). A concentração bactericida mínima (CBM) da tetraciclina sobre P. acnes foi determinada através do método de microdiluição em caldo. Foi realizada a avaliação da biomassa total e inibição da formação do biofilme através do método de coloração por cristal violeta 0,5%. A análise estatística dos dados foi feita através do teste de Análise de Variância (oneway ANOVA) com pós-teste de Tukey. The main cause of acne is an infection by the bacterium Propionibacterium acnes (P. acnes).Currently there are many treatment options for acne vulgaris, although often with extensive side effects. Factors such antibiotics resistance and slow initial action of topical therapies that led researchers to seek alternative treatments. Thus, the present study aimed to evaluate the efficacy of curcumin-mediated photodynamic inactivation in broth (Tryptic Soy Broth) TSB and 0.5% sucrose solution and the combination of this therapy with tetracycline against planktonic phase P. acnes and biofilm. Cell viability was evaluated by quantification of the colonies formed per milliliter of sample (CFU / mL). The minimum bactericidal concentration (MBC) of tetracycline over P. acnes was determined by the broth microdilution method. The evaluation of the total biomass and inhibition of the formation of the biofilm was performed through the method of staining by violet crystal 0.5%. The statistical analysis of the data was done through the variance analysis (ANOVA one way) test with Tukey's post test. The results of this study showed that curcumin-mediated photodynamic antimicrobial therapy (TFDa) was efficient in microbial reduction when quantification of the colonies was performed, compared to the negative control group in planktonic form as well as in biofilm both diluted in TSB broth and in solution sucrose 0.5% being able to reduce all bacterial load. A reduction of more than 28% of the total biofilm biomass and an inhibition of more than 70% was achieved after treatment. Existem várias opções de tratamento que variam de acordo com a gravidade da doença, porém são associados com reações indesejadas e resistência aos antimicrobianos. A terapia fotodinâmica antimicrobiana (TFDa) então é proposta como tratamento alternativo para inativar C. acnes juntamente com a incorporação do fotossensibilizador (FS) em uma formulação tópica para otimizar a terapia. Este trabalho objetivou avaliar a eficácia in vitro da TFDa mediada pelo azul de metileno (AM) incorporado em hidrogel de quitosana e poloxamer (HG-AM) contra C. acnes em fase planctônica e biofilme. A viabilidade celular após TFDa foi avaliada através da quantificação das colônias formadas por mililitro de amostra (UFC/mL). Para caracterizar estruturalmente os HGs foram realizadas as análises oscilatórias e de comportamento de fluxo em reômetro de estresse controlado com geometria placa-cone e a bioadesão em pele de orelha de porco utilizando um texturômetro. There are several treatments options that vary according to the severity of the disease, but are associated with unwanted reactions and antimicrobial resistance. Antimicrobial photodynamic therapy (aPDT) is then proposed as an alternative treatment to inactivate C. acnes together with the incorporation of the photosensitizer (PS) into a topical formulation to optimize therapy. This work aimed to evaluate the in vitro efficacy of methylene blue (MB) mediated TFDa incorporated in chitosan and poloxamer (HG-MB) hydrogels against planktonic and biofilm C. acnes. Cell viability after aPDT was evaluated by quantifying the colonies formed per milliliter of sample (CFU / mL). To characterize structurally the HG were performed the oscillatory and behavior of flow in a controlled stress rheometer with cone-plate geometry and bioadhesion in pig ear skin using a texture analyser. The statistical analysis of the data was the Analysis of Variance (one way ANOVA) with post-test of Tukey. The results of this study showed that aPDT was effective using HG-MB in planktonic phase. The minimum bactericidal concentration was 12.5 μg/mL associated with a fluence of 90 J/cm2 . The C. acnes biofilm was not totally eliminated by the aPDT with HG-MB under the conditions tested. O hormônio paratireoideo (PTH) e o índice ultravioleta (IUV), embora possivelmente envolvidos nessa associação, não têm sido considerados nessas pesquisas. Objetivos: Avaliar a concentração sérica da 25-hidroxivitamina D [25(OH)D] em pacientes com DA e em grupo controle; avaliar a adequabilidade dos valores de referência da 25(OH)D por meio de sua relação com o PTH molécula intacta (PTHi); identificar preditores independentes da gravidade clínica da DA e da concentração sérica de 25(OH)D. Métodos: Em estudo transversal com 106 pacientes de 0 a 18 anos de idade portadores de DA, um grupo controle foi pareado (1:1) com uma subamostra de 54 sujeitos com DA. Foram avaliados dados epidemiológicos, índice SCORing Atopic Dermatitis (SCORAD), testes laboratoriais e IUV. Resultados: O subgrupo DA apresentou maior concentração sérica de 25(OH)D e maior IUV - em média 3,39 ng/ml e 2,54 acima, respectivamente - que o grupo controle (p<0,05). Não houve associação significativa entre 25(OH)D e PTHi. Imunoglobulina E e IUV foram fatores independentes associados ao índice SCORAD. Por sua vez, fototipo, idade e IUV foram fatores independentes associados a 25(OH)D. Conclusões: A diferença da concentração de 25(OH)D entre casos e controles, embora significativa, pode não ter significância clínica e pode estar relacionada ao maior IUV médio encontrado nos casos com DA. A 25(OH)D e o PTHi não influenciaram o índice SCORAD. Estudos prospectivos com múltiplas dosagens de 25(OH)D são necessários para elucidar o papel da vitamina D na prevalência e gravidade da DA. Os pontos de corte adotados para a 25(OH)D parecem inadequados à amostra testada no tocante à supressão do PTHi. Uma vez que o IUV se mostrou significativamente associado com o SCORAD e com a 25(OH)D, esse índice pode atuar como fator de confusão e deve ser considerado em estudos envolvendo DA e vitamina D. / Introduction: The role played by vitamin D in atopic dermatitis (AD) is controversial and has been the focus of many studies. Although possibly involved in this association, parathyroid hormone (PTH) and ultraviolet index (UVI) have not been considered in such research. Objectives: To assess 25-hydroxy vitamin D [25(OH)D] serum levels in AD patients and a control group; to assess the adequacy of the cutoff point used for 25(OH)D investigating its relationship with intact PTH (iPTH); to identify the independent predictors of the AD clinical severity and 25(OH)D serum level. Methods: We conducted a cross-sectional study of 106 AD patients from 0 to 18 years old. A control group was matched (1:1) with a subsample of 54 participants with AD. Epidemiological data, SCORing Atopic Dermatitis (SCORAD) index, laboratory tests and UVI were assessed. Results: The AD subgroup had higher serum 25(OH)D level and higher UVI than control group – difference between the means of 3.39 ng/ml and 2.54, respectively (p<0.05). There was no significant correlation of iPTH with 25(OH)D. Immunoglobulin E and UVI were independent predictors associated with the SCORAD index. Skin type, age and UVI were independent predictors of 25(OH)D. Conclusions: The different levels of 25(OH)D between AD patients and controls may not have clinical consequences and may be related to the higher UVI found in AD subjects. The 25(OH)D and iPTH levels did not have an influence in SCORAD index. Prospective studies with multiple measurements of serum 25(OH)D levels are necessary to elucidate the role of vitamin D in prevalence and severity of AD. The cutoff point of 25(OH)D seems to be inappropriate to the sample at least with regard to the suppression of iPTH. Once UVI was significantly associated with SCORAD and with 25(OH)D, this index can work as a confounding factor and should be considered in studies involving AD and vitamin D.
