Irreverence : a psychotherapeutic stance

The development of the concept of irreverence is examined in terms of its historical, theoretical and metatheoretical contexts. The underlying assumptions of the concepts of neutrality, curiosity, and irreverence are distinguished and contextualised. Neutrality is discussed with reference to Milan systemic therapy and first- and second-order cybernetics, while curiosity is examined in the light of constructivist and narrative approaches to psychotherapy. It is argued that these two concepts represent two sides of a dualism, which is transcended through irreverence. Irreverence is interpreted as a postmodern stance, involving the questioning and relativising of therapists' basic assumptions. The pragmatic components of an irreverent stance, namely self-reflexivity, orthogonality, flexibility and accountability, are explored with reference to related concepts in the work of other authors. Throughout the text metalogues are used in an attempt to engage reader and author in a collaborative enterprise of acknowledging and reevaluating their own basic assumptions. / Psychology / M.A. (Clinical Psychology)

This thesis deals with a multidisciplinary approach to clients suffering from schizophrenia. The main aim is to reflex the multidisciplinary approach to schizophrenic clients, appealing to social work and having respect to ethical principles of participating branches. The work includes chapters that are necessarily coherent to this topic. These are mainly the social work in the context of schizophrenia, specifics of schizophrenia, the structure of the multidisciplinary team, team work and all ethical aspects arising out of team work and individual experts' work. This thesis tries to link up theoretical and practical knowledge, mainly by means of casuistry and practical observations.

Inledning: Behovet av traumabehandling till flyktingar har ökat. Psykoterapeuten blir vittne till berättelser som handlar om lidande, rädsla och smärta. Syftet med denna studie är att undersöka hur psykoterapeuter upplever sitt arbete med behandling av krigs- och tortyrskadade flyktingar. Frågeställningar: 1. Hur upplever psykoterapeuter arbetet med krigs- och tortyrskadade flyktingar? 2. Använder sig psykoterapeuter av bemästringsstrategier för att hantera sina upplevelser i arbetet med krigs- och tortyrskadade flyktingar? Om så är fallet, vilka? Metod: Kvalitativ metod med fenomenologisk, deskriptiv ansats valdes. Fem psykoterapeuter som arbetar med krigs- och tortyrskadade flyktingar intervjuades. Resultat: Studiens resultat har en manifest del och en latent del. ”Att pendla mellan vanmakt och skapande förändring” är ett tema som representerar det underliggande, latenta budskapet, d.v.s. det som sägs ”mellan raderna” och representerar den röda tråd som finns i samtliga intervjuer. Det manifesta resultatet visar att terapeuterna upplever sitt arbete som ett privilegium och som betungande. Terapeuterna använder medvetna bemästringsstrategier för att hantera känslomässig stress. Diskussion: Terapeuternas beskrivning av sitt arbete med krigs- och tortyrskadade stämmer väl överens med tidigare forskning. Terapeuternas upplevelse av sitt arbete kan förstås genom begreppen motöverföring, projektiv identifikation, vikarierande traumatisering, sekundär traumatisk stress och posttraumatic growth. / Introduction: The need for trauma treatment for refugees has increased. The psychotherapist testifies to stories that concern suffering, fear and pain. The purpose of this study is to investigate how psychotherapists experience their work in dealing with war and torture injured refugees. Research questions: How do psychotherapists experience the work of war and torture injured refugees? Are psychotherapists using coping strategies to handle their experiences in the work of war and torture-injured refugees? If so, which? Method: Qualitative method with phenomenological, descriptive approach was chosen. Five psychotherapists working with war and torture injured refugees were interviewed. Results: ”Oscillating between powerlessness and creative change" is a theme that represents the underlying latent message, what is said "between the lines" and represents the red thread found in all the interviews. The manifest results show that the therapists experience their work as a privilege and as burdensome. The therapists use conscious coping strategies to deal with emotional stress. Discussion: The therapists' description of their work on war and torture injured is in line with previous research. Therapists' experience of their work can be understood by the concepts of transference, projective identification, vicarious traumatization, secondary traumatic stress and posttraumatic growth.

Behind training : differentiation of self of a psychotherapy trainee

Psychotherapy is an interpersonal encounter where a therapist collaborates with clients to facilitate a healing process. Due to the personal nature of the therapeutic encounter, the therapist requires the necessary skills and knowledge, as well as a differentiated sense of self. The importance of supporting the psychotherapy trainee’s own differentiation process seemed to have been neglected in research in recent years. In South Africa there has recently been increasing pressure to select larger groups of trainees which has the risk that the tending to differentiation would further be neglected. The purpose of this research study is to reemphasise the importance of differentiation of self of the psychotherapy trainee by examining my own process of differentiation during my psychotherapy training. This study introduces the reader to these concepts and explores psychotherapy training in general and the Unisa training method specifically. The research design of this study is autoethnography which falls in the realm of social constructionism and the coding method is Thematic Data Analysis. The research findings as reflected in the two global themes namely individuation; and gaining and strengthening authentic relationships, seem to accurately reflect the process of differentiation. The specific training method of the Unisa training team in combination with the way in which I engaged with this process seemed important for the facilitation of this process of differentiation. / Psychology / M.A. (Clinical Psychology)

Under pandemin COVID-19 har hälsovården tvingats till hastiga anpassningar i form av användning av vård via videolänk för kontinuitet, vilket berör även psykoterapi. Trots omfattande evidens för effektiviteten av terapi via videolänk finns fortfarande myter hos kliniker att den terapeutiska alliansen äventyras i det digitala forumet, vilket kan försvåra implementeringen. Studiens syfte var att utforska psykoterapeuters upplevelser av psykoterapi via videolänk, med särskilt fokus på den terapeutiska alliansen (TA). Genom semi-strukturerade intervjuer undersöktes psykoterapeuters föreställningar, anpassningar samt erfarenheter av denna omställning. Data analyserades utifrån kvalitativ innehållsanalys och resulterade i tre kategorier; Kommunikation, Den bärande relationen samt KBT-interventionerna. Huvudresultatet indikerar att klinikerna har behövt anpassa behandlingen kontextuellt, kommunikativt samt innehållsmässigt för att främja alliansen, och att TA går att utveckla över videolänk. Detta sammanfaller med tidigare internationella studiers resultat.  Utvecklingsområden som framkommit är aspekter självexponering som nytt fenomen och förändringar i maktstrukturen inom terapin. Omställningen har gått över förväntan positivt och delvis förändrat respondenternas inställning till videolänkterapi. Inom begränsningarna för denna studie beskrivs konsekvenserna och framtida forskningsriktningar. / During the pandemic COVID-19 the health care has suddenly been forced to adjust their work and start using video conference to keep the continuity, which also impacts psychotherapy. Despite extensive evidence of the efficiency of video conference psychotherapy there are still myths among clinicians that the therapeutic alliance can be endangered by the digital format, which can complicate the implementations. The aim of this study is to examine the psychotherapists' experiences of psychotherapy via video conference, with particular focus on the therapeutic alliance (TA). Through semi-structured interviews we examined the psychotherapists conceptions, adaptations and the experiences of this readjustment. Data were analyzed based on qualitative content analysis; Communication, The solid relation and the CBT-interventions. The main results indicated that the clinicians needed to adapt the treatment contextually, communicatively and in terms of content to frame TA, and that TA can be developed via video conference. This coincides with previous international research. Areas of development is the new phenomenon self-exposure and transformation/change of equalization of power.  The conversion has positively exceeded the expectations and has partly changed the respondents attitudes to therapy via video conference. Within the limitations of this study, implications and further directions for research are described.

Due to distance or financial problems, a rural psychotherapist cannot experience the privilege of supervision with an external supervisor. In such a situation selfsupervision could play an important role in the self-development, because it includes a search of self that will lead to more effectiveness in psychotherapy. Self-supervision focuses on the psychotherapist as a person and therapist, who he is, where he comes from, and in what direction he, as a psychotherapist, is developing within the psychotherapeutic process. It is important to any psychotherapist to strive towards a higher level of psychotherapy. This implies a process of selfinvestigation, dissection and self-evaluation. This process was conducted by means of diary entries in which the psychotherapist interpreted his daily experiences and events in terms of his past. This places the psychotherapist, with his family history and current interpersonal situation, as intrinsic parts of himself, in the foreground. In this process the psychotherapist takes centre stage with emphasis on his responsibility concerning the process of self-supervision. The problems addressed by this personally coloured, scientific study, are problems that the psychotherapist identified through the process of self-supervision and introspection. Such introspection leads to a deeper personal knowledge which the psychotherapist can use to his own benefit but also to the benefit of his family and client system. This study is an attempt to identify a new way of research. It is comprehensive and leads to personal fulfilment and deeper self-knowledge and is also a method by which other psychotherapists could discover themselves and their worlds. Psychotherapist development of trainee and qualified psychologists within the South African context : a qualitative study

This qualitative study aimed to trace the psychotherapeutic development of clinical and counselling psychologists across the careerspan within the South African context. Through purposive sampling 34 psychologists were recruited according to the inclusion criteria of the study. Five distinct career levels were explored, namely, student (n=10), intern (n=7), early career (n=7), experienced (n=5), and senior (n=5) psychologists in relation to the Society of Psychotherapy Research’s international model of psychotherapist development. By means of semi-structured interviews, couched in the social constructionist position, participants’ experiences were thematically analysed from over 600 pages of transcription. Additionally, the researcher’s personal journey of developing as a psychotherapist was reflected upon. The current study found that a number of aspects fostered the development of psychotherapists. Participants reflected on personal and familial wounding events that influenced them choosing a career as a psychologist; this awareness was gradual over their development. Across theoretical orientations, participants resisted adhering to one way of working in light of the diverse South African context. However, the need to limit the number of theoretical orientations taught within the first months of training was proposed. Professional sources of influence highlighted by participants were personal therapy and peer supervision or reading groups. Qualified professionals to a lesser extent made use of individual supervision which when pursued needed to be a ‘felt’ collaboration. Participants found as they developed they became more comfortable being a psychotherapist however the complexity of cases still kept them humble. Limitations of the study, potential future research directions as well as recommendations for practicing clinical and counselling psychology were outlined. / Psychology / Ph. D. (Psychology)

Emotional experiences of incestuous fathers : a social constructionist investigation

This study aimed to relate the emotions of two fathers regarding their incestuous behaviour. The epistemological framework is social constructionism. The `case-study approach' was chosen as the most suitable method to gain access to the information. Qualitative in-depth interviews with the fathers were conducted to collect data about their emotional experiences regarding their incestuous behaviour and Hermeneutics was used as method of data analysis. The emotional experiences of the participants regarding their incestuous behaviour were reconstructed in the form of prominent themes. Themes that seemed to re-occur in both participants' stories were identified and discussed as common themes. These themes included; ambivalence, powerlessness and guilt. This study provides a rich understanding of the emotional experiences of incestuous fathers. These experiences seem to be unstated or insufficiently emphasised in the traditional empirical, cause and effect, linear modernist literature. / Psychology / M.A. (Clinical Psychology)
