Aplicabilidade, validação e reprodutibilidade do Spinal Cord Independence Measure version III (SCIM III) nos pacientes com paraparesia espástica / Applicability, validation and reproducibility of the Spinal Cord Independence Measure version III (SCIM III) in patients with spastic paraplegia

Objetivo: verificar a aplicabilidade, reprodutibilidade e validade do SCIM III nos pacientes com paraparesia espástica. Método: estudo transversal incluindo 30 sujeitos (66% mulheres; 41,5±14,7 anos) com paraparesia espástica de etiologia genética, infecciosa ou a esclarecer que foram avaliados pela análise computadorizada da marcha, versão brasileira da SCIM III (0-100 pontos), MIF (18-126 pontos), por 2 examinadores (A e B) no mesmo dia e 1 semana depois (A). Resultados: o coeficiente de correlação intraclasse (CCI) para o uso da SCIM III indicou boa reprodutibilidade intra e inter-examinadores (CCI=0,9). A correlação de Spearman entre a SCIM III e a parte motora da MIF foi considerada adequada e estatisticamente significante (Spearman=0,6; p=0,001). A correlação entre as subescalas da SCIM III e os domínios da MIF foi considerada forte e significante para auto-cuidado (Spearman=0,8; p0,001) e moderada para transferências (Spearman=0,6; p=0,0005) e locomoção (Spearman=0,6; p=0,0006). A subescala mobilidade da SCIM III mostrou correlação positiva e significante para cadência (Spearman=0,8; p0,001), velocidade da marcha (Spearman=0,7; p0,001) e comprimento do passo (Spearman=0,6; p0,001). Conclusões: A SCIM III é um instrumento de avaliação funcional reprodutível intra e inter examinadores e capaz de avaliar o nível de independência dos indivíduos com paraparesia espástica. O SCIM III é mais sensível que a MIF, especialmente para pacientes com maior independência. Cadência, velocidade de marcha e comprimento do passo se correlacionaram com a subescala mobilidade / Purpose: to verify the applicability, reproducibility, and validity of the SCIM III patients with non-traumatic spastic paraplegia. Method: The cross-sectional study included 30 subjects (66% females; 41.5 ± 14.7 y) older with spastic paraparesis of any etiology were assessment by computerized gait analysis and with the Brazilian versions of SCIM III (0-100 points), FIM (18-126 points) by 2raters (A and B) at the same day and 1 week later (A). Results: The intraclass correlation coefficient (ICC) for the use of SCIM III indicated good intra and inter-evaluator reproducibility (ICC = 0.9). Correlation between the SCIM III and the motor FIM was appropriate (Spearman=0.6; p0.001). SCIM III subscales and the FIM domains correlated strongly for self-care (Spearman=0.8; p0.001), moderately for transfers (Spearman=0.6; p=0.0005) and locomotion (Spearman=0.6; p=0.0006). SCIM III mobility subscale positively correlated with the cadence (Spearman=0.8; p0.01), gait speed (Spearman=0.7; p0.01), and step length (Spearman=0.6;p0.01). Conclusions: SCIM III is a reproducible functional assessment instrument and capable of evaluating the level of independence of the individual with spastic paraplegia. The SCIM III is more sensitive than the FIM for non-traumatic spastic paraplegic patients with higher levels of independence. Linear gait parameters correlated with its mobility subscale.

Validação de escala para rastreamento de depressão em idosos: importância de um teste de aplicação rápida / Validation of a depression screening scale for the elderly: importance of a quick application test

INTRODUÇÃO: Os critérios diagnósticos atuais para as síndromes depressivas não identificam uma parcela dos idosos com sintomatologia depressiva clinicamente significativa. Instrumentos psicométricos específicos para esta população, que sejam rápidos e fáceis de aplicar tornam-se necessários. A partir de instrumentos previamente validados como as escalas GDS e CES-D, e de um consenso de especialistas foi desenvolvida uma escala de 10 itens para avaliar o construto clínico de depressão em pacientes idosos. OBJETIVO: Validação interna de uma nova escala de rastreio de sintomas depressivos em idosos (D10) desenvolvida no Programa Terceira Idade (PROTER), Instituto de Psiquiatria HC FMUSP. MÉTODOS: Uma amostra de conveniência composta por 62 sujeitos foi utilizada para o estudo de validade de critério (validade concorrente). Destes, 44 foram incluídos nos estudos de confiabilidade (consistência interna, estabilidade e equivalência) e de validade de construto (validades convergente e divergente). Todos os participantes incluídos tinham idade 60 anos, não apresentavam doenças médicas graves ou descompensadas, déficits cognitivo e funcional significativos (avaliados, respectivamente, pela M-CIRS, MMSE e B-ADL) e não apresentavam histórico passado ou atual sugestivo de síndrome psicótica. Os diagnósticos foram obtidos pela entrevista estruturada MINI-Plus (seções A, B, D, E, F e P) e a intensidade dos sintomas depressivos mensurados pela MADRS. Outros instrumentos foram utilizados (CAMCOG, IQCODE, CAGE, DUSI-R) no desenho da pesquisa, para reduzir a possibilidade de vieses sistemáticos. A distribuição da amostra foi avaliada pelo Teste Kolgomorov-Smirnov, e os índices de correlação de Pearson e Spearman foram utilizados para aferir a relação entre a D10 e as escalas MADRS e CAGE. A correlação entre os itens da D10 foram analisados pelos coeficientes de Correlação Intraclasse (CCI) e alfa de Cronbach. A acurácia do teste foi investigada pela análise das áreas sob as curvas ROC. RESULTADOS: A amostra (idade média de 72,35; SD=7,05) não apresentou diferenças significativas entre as variáveis sócio-demográficas, econômicas, clínicas, cognitivas e funcionais estudadas (p>0,05). O valor de alfa (consistência interna) manteve-se estável e elevado (=0,84±0,02) e os itens da D10 apresentaram boa equivalência (inter-rater) (rs=0,95, p0,01) e boa estabilidade (intra-rater) (rs=0,95, p0,01). A escala MADRS correlacionou-se significativamente com a D10 (rp=0,86, e rs=081, p0,01), não tendo se correlacionado com a CAGE (p>0,05). Esse achado foi reforçado pela correlação entre a CAGE e a DUSI-R (rp=0,31, p=0,005; e rs=0,29 p=0,01). A nota de corte de 6 pontos na D10 apresentou elevados níveis de sensibilidade (96,2%), especificidade (88,9%), valor preditivo positivo (86,2%), valor preditivo negativo (97%) e acurácia (91,9%). O tempo médio de aplicação do novo instrumento foi de 42±4 segundos. CONCLUSÕES: Os resultados obtidos indicaram que a escala D10 apresentou confiabilidade, validade interna, e acurácia adequadas para ser utilizada como um instrumento de aplicação rápida para o rastreio de sintomatologia depressiva clinicamente significativa em idosos / identify a portion of elderly people with clinically significant depressive symptoms. Specific psychometric instruments for this population with quick and easy application properties are of the essence. Based on previously validated instruments, such as GDS and CES-D scales, and on consensus among specialists, a 10-item scale was developed to assess the clinical depression construct in elderly patients. OBJECTIVES: Internal validation of a scale to screen depressive symptoms in elderly people (D10) developed in the Senior Citizens Program (locally, PROTER), at the Institute of Psychiatry of the HCFMUSP. METHODS: A convenience sample of 62 subjects was used for the criterion validity study (concurrent validity). Of these, 44 were included in the studies of reliability (internal consistency, stability and equivalence) and of the construct validity (convergent and divergent validity). All the participants included in the study were 60 years old, did not present any serious or uncontrolled medical conditions, significant cognitive or functional deficits (evaluated, respectively, through M-CIRS, MMSE and B-ADL), nor past or present history suggestive of psychotic syndrome. Diagnostics were obtained through the MINI-Plus structured interview (sections A, B, D, E, F and P), and the intensity of depressive symptoms through the MADRS. In the research design, other instruments were used (CAMCOG, IQCODE, CAGE, DUSI-R), aimed at reducing the possibility of systematic bias. The sample distribution was evaluated through the Kolgomorov- Smirnov Test, and the Pearson and Spearman correlation indexes were used to verify the relation between the D10 and the MADRS and CAGE scales. The correlation of the D10 items were analyzed through the Intraclass Correlation (ICC) and Cronbach\'s alpha. The test accuracy was investigated through the analysis of the areas under ROC curves. RESULTS: The sample (mean age of 72,35; SD=7,05) did not evidence significant differences in the socio-demographic, economic, clinical, cognitive and functional variables studied (p>0,05). The alpha value (internal consistency) remained stable and elevated (=0,84±0,02), D10 items presented good equivalence (inter-rater) (rs=0,95, p0,01) and good stability (intra-rater) (rs=0,95 p0,01). The MADRS scale correlated significantly with CAGE (p>0,05). This finding was reinforced by the correlation between CAGE and DUSI-R (rp=0,31 p=0,005; and rs=0,29 p=0,01). A cut-off score of 6 points in D10 presented high levels of sensitivity (96,2%), specificity (88,9%), positive predictive value (86,2%), negative predictive value (97%) and accuracy (91,9%). The average application time of the new instrument was 42±4 seconds. CONCLUSIONS: The results obtained indicate that the D10 scale presented adequate reliability, internal validity and accuracy for use as a quick application instrument to screen clinically significant depressive symptoms in the elderly

Orientador: Dionei Ramos / Resumo: Introdução: O tabagismo, bem como a abstinência deste, traz uma série de mudanças físicas e psicológicas. Na literatura a escala The Mood And Physical Symptoms Scale (MPSS) e a Wisconsin Smoking Withdrawal Scale (WSWS) são semelhantes e avaliam sintomas da abstinência do tabagismo, porém somente a WSWS é validada para o Brasil, sendo necessária a validação transcultural da MPSS. Além disso, é necessário verificar em momentos diferentes da abstinência, a responsividade das duas escalas a fim de quantificar corretamente os sintomas relacionados à Síndrome de Abstinência Tabagística (SAT). Objetivos: Verificar se MPSS é um instrumento válido e reprodutível para avaliar a presença de sintomas da SAT em tabagistas brasileiros e, determinar a diferença mínima clinicamente importante (DMI) da MPSS e WSWS após cessação. Métodos: Para a validação, 112 tabagistas responderam a escala MPSS e (WSWS) da seguinte forma: basal (avaliador 1 – MPSS e WSWS), após 30 minutos (avaliador 2 - MPSS) e após 15 dias (avaliador 1 - MPSS). Para a DMI foram avaliados 33 tabagistas após 24 horas e terceira semana após a cessação do tabagismo. Para reprodutibilidade na aplicação, e reaplicação da MPSS, o coeficiente de correlação intraclasse (CCI) foi utilizado, assim como o teste de Wilcoxon, para verificar se houve diferença entre as aplicações. In the literature, The Mood and Physical Symptoms Scale (MPSS) and The Wisconsin Smoking Withdrawal Scale (WSWS) are similar and evaluate smoking cessation symptoms, but only the WSWS is validated for Brazil, requiring cross-cultural validation of the MPSS. In addition, it is necessary to verify at different times of abstinence the responsiveness of the two scales in order to correctly quantify the symptoms related to the Smoking Abstinence Syndrome (SAS). Aim: Verify if MPSS scale is a valid and reproducible instrument to evaluate the presence of SAT symptoms in Brazilian smokers and to determine the Minimal Detectable Change (MDC) of MPSS and WSWS after cessation of smoking. Due to this, since 1999, an instrument has been being built according to the complaints of these professionals, and proposes to fulfill this necessity. To do so, it is necessary to check the reproducibility of the self-reference of the dimension of the vocal aspects in the questionnaire named Conditions of the Teacher s Vocal Production (CPV-P). Verify if a participant when stating that he has symptoms such as: hoarseness, tiredness when speaking, flaw in voice, among others, will confirm these vocal complaints a while later. OBJECTIVE: Evaluate the reproducibility of the questions regarding vocal dimension that are part of the instrument CPV-P. METHOD: A transversal observational study was performed with the objective of preceding the reproducibility test of one of the dimensions of the instrument. The sample was, by convenience, recruited in a sequence, among teachers with vocal disorders that looked for speech therapy treatment in a public hospital in the city of São Paulo. The questionnaire was filled in during two moments, with a 15 day interval between its applications. The variables where analyzed in a descriptive manner compared considering test and re-test through Kappa (p <0,05), to evaluate the concordance between the results of both instrument applications. RESULTS: The population was made up of 36 teachers of the public school of São Paulo city, the majority married (47,2%), with a college graduation (91,7%), between 11 and 20 years of professional experience (55,7%), that teach in pre-school (61,1%), work for more than 40 hours a week (27,8%), in only 1 school (58,3%) and with a defined class (80,6%). Regarding vocal symptoms, the highest measure of reliability was deep voice (k = 0,74) and the lowest, hoarseness (k = 0,52). Concerning the laryngopharyngeal sensations, the measure that presented highest level of reliability was pain while speaking (k = 0,59) and the lowest was hem (k = 0,36). In vocal habits at work, what presented highest measure of reliability was drinking water during class (k = 0,88) and what registered lowest measure of reliability was speaking in an open space (k = 0,41). In the vocal aspects of diverse nature, the highest level of reliability registered was missing work due to voice problems (k = 0,86) and what presented the lowest level was having received orientation about vocal health (k = 0,54). CONCLUSION: The results indicated that the dimensions of the vocal aspects of the questionnaire (CPV-P) presented in the reproducibility test and re-test study, concordance level between regular and excellent. The majority of the Kappa coefficients, for the items analyzed in the questionnaire, where higher than 0,50 and the results indicated that these symptoms/sensations and habits are trustworthy for epidemiological studies / INTRODUÇÃO: No Brasil, não existem instrumentos para avaliar a condição de produção vocal do professor, devido a isto, desde 1999, vem sendo construído um instrumento, a partir das queixas destes profissionais, que se propõe a suprir esta necessidade. Para tanto, é necessário constatar a reprodutibilidade da autorreferência da dimensão dos aspectos vocais do questionário denominado Condições de Produção Vocal do Professor (CPV-P). Verificar se um participante ao relatar que possui sintomas como: rouquidão, cansaço ao falar, falha na voz, entre outros, irá confirmar estas queixas vocais em um determinado tempo depois. OBJETIVO: Avaliar a reprodutibilidade das questões referentes à dimensão vocal, que fazem parte do instrumento CPV-P. MÉTODO: Foi realizado um estudo transversal observacional com o objetivo de proceder ao teste de reprodutibilidade de uma das dimensões do instrumento. A amostra foi por conveniência, recrutada de forma seqüencial, entre professoras com distúrbio de voz que procuraram tratamento fonoaudiológico em um hospital público da cidade de São Paulo. O questionário foi preenchido, em dois momentos, com intervalo de 15 dias entre as aplicações. As variáveis foram analisadas de forma descritiva e comparadas quanto à situação teste-reteste, por meio do teste Kappa (p <0,05), a fim de avaliar a concordância entre os resultados das duas aplicações do instrumento. RESULTADOS: A população foi composta por 36 professoras da rede municipal de São Paulo, a maioria casada (47,2%), com escolaridade superior (91,7%), entre 11 e 20 anos de profissão (55,7%), que leciona em educação infantil (61,1%), mais de 40h semanais (27,8%), em apenas uma escola (58,3%) e com classe definida (80,6%). Em relação aos sintomas vocais, a maior medida de confiabilidade foi voz grossa (k = 0,74) e a menor, rouquidão (k = 0,52). Quanto às sensações laringofaríngeas, a medida que apresentou maior índice de confiabilidade foi dor ao falar (k = 0,59) e a menor, o pigarro (k = 0,36). Em hábitos vocais no trabalho, o que apresentou maior medida de confiabilidade foi beber água durante a aula (k = 0,88) e o que registrou menor medida de confiabilidade foi falar em lugar aberto (k = 0,41). Nos aspectos vocais de natureza diversa, o maior índice de confiabilidade registrado foi faltar ao trabalho por problemas de voz (k = 0,86) e o que apresentou menor índice foi ter recebido orientação sobre saúde vocal (k = 0,54). CONCLUSÃO: Os resultados indicaram que a dimensão dos aspectos vocais do questionário (CPV-P) obteve, no estudo de reprodutibilidade teste-reteste, nível de concordância entre regular e ótimo. A maioria dos coeficientes Kappa, para os itens analisados do questionário, foram acima de 0,50 e os resultados indicaram que estes e sintomas/sensações e hábitos são confiáveis para uso em estudos epidemiológicos

No primeiro estudo utilizou-se 30 isolados clínicos e uma cepa de referência de C. albicans para obtenção de suspensões celulares (108ufc/mL) que foram plaqueadas nos meios específicos. Três variáveis independentes (nº de replicatas, cor de fundo e instrumento de leitura) originaram oito protocolos de leitura. A reprodutibilidade intra e interexaminadores da medida de produção de enzima (Pz) foi avaliada por meio do Coeficiente de Correlação Intraclasse (CCI). A enzima proteinase apresentou dois protocolos com ―Boa‖ reprodutibilidade intraexaminador (CCI ≥ 0,71) para todos os examinadores e o protocolo que utilizou leituras em triplicata sobre as fotografias contra luz natural atribuiu maior reprodutibilidade interexaminador para os valores de Pz-proteinase. Três protocolos com ‗Boa' concordância intraexaminador de Pz-fosfolipase foram selecionados para a análise interexaminador. Tanto as leituras em paquímetro como as realizadas em computador, em triplicata, mostraram reprodutibilidade ‗Muito Boa'. No estudo II, a concentração de CUR a 0,1µM foi utilizada associada a iluminação LED para promover dose subletal de IF. O grupo tratamento (C+L+) teve suas células fotossensibilizadas com CUR e iluminadas. O grupo controle positivo consistiu de amostras que receberam apenas CUR (C+L-), e o controle negativo não recebeu nem CUR nem luz (C-L-). A capacidade de adesão foi avaliada por meio das unidades formadoras de colônias (ufc/mL) e do método XTT. A formação de biofilme foi analisada ...(Resumo completo, clicar acesso eletrônico abaixo) / This study aimed to: I. to establish a reproducible protocol using the agar-Based methodology to evaluate the production of the phospholipase and proteinase by the species Candida albicans (C. albicans); II. to analyze the expression of virulence factors after photodynamic inactivation (PDI) mediated by curcumin (CUR). In the first study 30 clinical isolates and a reference strain of the C. albicans were used to obtain cell suspensions (108CFU/mL) that were plated in specific media. Three independent variables (number of replicates, background color and reading instrument) originated eight reading protocols. The intra- and inter-examiner reproducibilityof the enzyme production measure (Pz) was assessed using the intraclass correlation coefficient (ICC). The enzyme proteinase presented two protocols with good intra-rater reproducibility (ICC≥0.71) and the readings in triplicate against natural light showed the highest inter-examiner reproducibility for Pz-proteinase values. Three protocols with 'Good' intra-rater agreement for Pz-phospholipase were selected for interexaminer analysis. Caliper and computer readings, performed in triplicate, showed 'Very Good' reproducibility. In Study II, CUR at 0,1μM was used with LED light to promote sublethal dose of the PDI. The treatment group (C+L+) had its cells photosensitized with CUR and illuminated. The control group consisted of the samples that received only CUR (C+L-), and another which received neither CUR or light (C-L-). The adhesion ability was evaluated by means of CFU/mL and the XTT method. Biofilm formation was analyzed by means of CFU/mL, XTT and Crystal Violet (CV). Destes, 44 foram incluídos nos estudos de confiabilidade (consistência interna, estabilidade e equivalência) e de validade de construto (validades convergente e divergente). Todos os participantes incluídos tinham idade 60 anos, não apresentavam doenças médicas graves ou descompensadas, déficits cognitivo e funcional significativos (avaliados, respectivamente, pela M-CIRS, MMSE e B-ADL) e não apresentavam histórico passado ou atual sugestivo de síndrome psicótica. Os diagnósticos foram obtidos pela entrevista estruturada MINI-Plus (seções A, B, D, E, F e P) e a intensidade dos sintomas depressivos mensurados pela MADRS. Outros instrumentos foram utilizados (CAMCOG, IQCODE, CAGE, DUSI-R) no desenho da pesquisa, para reduzir a possibilidade de vieses sistemáticos. A distribuição da amostra foi avaliada pelo Teste Kolgomorov-Smirnov, e os índices de correlação de Pearson e Spearman foram utilizados para aferir a relação entre a D10 e as escalas MADRS e CAGE. A correlação entre os itens da D10 foram analisados pelos coeficientes de Correlação Intraclasse (CCI) e alfa de Cronbach. A acurácia do teste foi investigada pela análise das áreas sob as curvas ROC. RESULTADOS: A amostra (idade média de 72,35; SD=7,05) não apresentou diferenças significativas entre as variáveis sócio-demográficas, econômicas, clínicas, cognitivas e funcionais estudadas (p>0,05). O valor de alfa (consistência interna) manteve-se estável e elevado (=0,84±0,02) e os itens da D10 apresentaram boa equivalência (inter-rater) (rs=0,95, p0,01) e boa estabilidade (intra-rater) (rs=0,95, p0,01). A escala MADRS correlacionou-se significativamente com a D10 (rp=0,86, e rs=081, p0,01), não tendo se correlacionado com a CAGE (p>0,05). Esse achado foi reforçado pela correlação entre a CAGE e a DUSI-R (rp=0,31, p=0,005; e rs=0,29 p=0,01). A nota de corte de 6 pontos na D10 apresentou elevados níveis de sensibilidade (96,2%), especificidade (88,9%), valor preditivo positivo (86,2%), valor preditivo negativo (97%) e acurácia (91,9%). O tempo médio de aplicação do novo instrumento foi de 42±4 segundos. CONCLUSÕES: Os resultados obtidos indicaram que a escala D10 apresentou confiabilidade, validade interna, e acurácia adequadas para ser utilizada como um instrumento de aplicação rápida para o rastreio de sintomatologia depressiva clinicamente significativa em idosos / identify a portion of elderly people with clinically significant depressive symptoms. Specific psychometric instruments for this population with quick and easy application properties are of the essence. Based on previously validated instruments, such as GDS and CES-D scales, and on consensus among specialists, a 10-item scale was developed to assess the clinical depression construct in elderly patients. OBJECTIVES: Internal validation of a scale to screen depressive symptoms in elderly people (D10) developed in the Senior Citizens Program (locally, PROTER), at the Institute of Psychiatry of the HCFMUSP. METHODS: A convenience sample of 62 subjects was used for the criterion validity study (concurrent validity). Of these, 44 were included in the studies of reliability (internal consistency, stability and equivalence) and of the construct validity (convergent and divergent validity). All the participants included in the study were 60 years old, did not present any serious or uncontrolled medical conditions, significant cognitive or functional deficits (evaluated, respectively, through M-CIRS, MMSE and B-ADL), nor past or present history suggestive of psychotic syndrome. Diagnostics were obtained through the MINI-Plus structured interview (sections A, B, D, E, F and P), and the intensity of depressive symptoms through the MADRS. In the research design, other instruments were used (CAMCOG, IQCODE, CAGE, DUSI-R), aimed at reducing the possibility of systematic bias. The sample distribution was evaluated through the Kolgomorov- Smirnov Test, and the Pearson and Spearman correlation indexes were used to verify the relation between the D10 and the MADRS and CAGE scales. The correlation of the D10 items were analyzed through the Intraclass Correlation (ICC) and Cronbach\'s alpha. The test accuracy was investigated through the analysis of the areas under ROC curves. RESULTS: The sample (mean age of 72,35; SD=7,05) did not evidence significant differences in the socio-demographic, economic, clinical, cognitive and functional variables studied (p>0,05). The alpha value (internal consistency) remained stable and elevated (=0,84±0,02), D10 items presented good equivalence (inter-rater) (rs=0,95, p0,01) and good stability (intra-rater) (rs=0,95 p0,01). The MADRS scale correlated significantly with CAGE (p>0,05). This finding was reinforced by the correlation between CAGE and DUSI-R (rp=0,31 p=0,005; and rs=0,29 p=0,01). A cut-off score of 6 points in D10 presented high levels of sensitivity (96,2%), specificity (88,9%), positive predictive value (86,2%), negative predictive value (97%) and accuracy (91,9%). The average application time of the new instrument was 42±4 seconds. CONCLUSIONS: The results obtained indicate that the D10 scale presented adequate reliability, internal validity and accuracy for use as a quick application instrument to screen clinically significant depressive symptoms in the elderly

No Brasil, as percepções, conhecimentos e atitudes dos profissionais de saúde relacionados à SBE permanecem incertas. Conhecer como as práticas de SBE são implementadas, as barreiras e facilitadores para sua aplicação permite identificar quais são as necessidades educacionais e apoio para facilitar e promover a prática de SBE. Objetivo: Desenvolver e iniciar o processo de validação de um instrumento de pesquisa para avaliação da percepção, conhecimento e atitudes dos profissionais de saúde sobre as práticas de SBE. Método: Trata-se de estudo transversal de construção e validação de instrumento de pesquisa. Para selecionar os domínios a serem utilizados na elaboração do instrumento realizou-se a revisão da literatura científica. Utilizou-se a "Classification Rubric for Evidence Based Practice Assessment Tools in Education" (CREATE) como direcionamento para formulação do itens. In Brazil, perceptions, knowledge and attitudes of health professionals on EBHP remain uncertain. Knowing how to apply EBHP, as well as barriers and facilitators to their implementation, allows to identify the education and support necessary to facilitate and promote EBHP. Objective: To develop and start the validation process of a research instrument to assess the perception, knowledge and attitudes of health professionals on EBHP. Methods: This is a cross-sectional study for the development and validation of a research instrument. To select the domains to be used, the scientific literature was reviewed. The "Classification Rubric for Evidence Based Practice Assessment Tools in Education" (CREATE) was used as a guideline to formulate the items. Reprodutibilidade da curva de pressão intraocular de 24 horas em pacientes com glaucoma e suspeita de glaucoma / Reproducibility of 24-hour intraocular pressure curve in patients with glaucoma and suspected glaucoma

Foram estudados 33 indivíduos com glaucoma e suspeita de glaucoma sem tratamento. Todos os participantes foram submetidos a 3 CTDs de 24 horas. Os pacientes foram internados e as medidas foram realizadas às 9:00 hs, 12:00 hs, 15:00 hs, 18:00 hs, 21:00 hs, 24:00 hs e 6:00 hs com tonômetro de aplanação de Goldmann (TAG). A medida das 6:00 hs também foi realizada no leito, utilizando-se um tonômetro manual de Perkins antes da medida com o TAG. Uma segunda CTD de 24 horas foi realizada após um intervalo entre uma e três semanas e uma terceira após mesmo intervalo. A reprodutibilidade foi avaliada em cada horário da CTD de 24 horas com o coeficiente de correlação intraclasse (CCI) e os gráficos de Bland- Altman. Nos gráficos de Bland-Altman considerou-se como limite de concordância uma diferença de até 3 mmHg entre as medidas. O CCI variou de 0,736 a 0,917 nos diferentes horários, estando a maioria deles acima de 0,800. Nos gráficos de Bland- Altman, a porcentagem de pontos dentro do limite de concordância de 3 mmHg variou de 72,7 a 97,0%, sendo na maioria das observações acima de 80%. Concluiuse que as medidas da PIO em cada momento da CTD de 24 horas nas três repetições apresentaram excelente ou boa reprodutibilidade em indivíduos com glaucoma e suspeita de glaucoma / The reproducibility of measurements of intraocular pressure (IOP) at the same points of the 24-hour daily tension curve (DTC) with three replications was evaluated. Thirtythree untreated subjects with glaucoma and suspected glaucoma were studied. All participants underwent three 24-hour DTCs. Briefly, participants were hospitalized and IOP measurements obtained at 9:00 AM, noon, 3:00 PM, 6:00 PM, 9:00 PM, midnight and 6:00 AM using a Goldmann applanation tonometer. The 6:00 AM measurement was also obtained at bedside with a handheld Perkins tonometer prior to Goldmann tonometry. A second 24-hour DTC was performed 1 to 3 weeks after the first, and a third and final curve was obtained 1 to 3 weeks after the second. Reproducibility of measurements at each time point of the 24-hour DTC was assessed by means of the intraclass correlation coefficient (ICC) and Bland-Altman plots. A 3-mmHg difference in IOP was defined as the limit of agreement for Bland- Altman plots. ICCs ranged from 0.736 to 0.917 at different time points, with the majority exceeding 0.800. The percentage of points within the 3-mmHg limit of agreement on Bland-Altman plots ranged from 72.7% to 97.0%, exceeding 80% in most cases. In conclusion, in patients with glaucoma or suspected glaucoma, IOP measurements obtained at each time point of the 24-hour DTC showed good or excellent reproducibility across the three curves performed

Objetivos: Desenvolver e validar instrumento que auxilie o pediatra a determinar a probabilidade de ocorrência do abuso sexual em crianças. Métodos: Estudo de caso-controle com 201 crianças que consultaram em ambulatórios de pediatria e locais de referência para vítimas de abuso sexual, entre março e novembro de 2004: grupo caso (com suspeita ou revelação de abuso sexual) e grupo controle (sem suspeita de abuso sexual). Aplicou-se, junto aos responsáveis, um questionário com 18 itens e cinco opções de respostas segundo a escala Likert, abordando comportamento, sintomas físicos e emocionais apresentados pelas crianças. Excluíram-se nove crianças sem controle esfincteriano e um item respondido por poucas pessoas. A validade e consistência interna dos itens foram avaliadas com obtenção de coeficientes de correlação (Pearson, Spearman e Goodman-Kruskal), coeficiente α de Cronbach e cálculo da área da curva ROC. Calculou-se, após, a razão de verossimilhança (RV) e os valores preditivo positivos (VPP) para os cinco itens do questionário que apresentaram os melhores desempenhos. Resultados: Obteve-se um questionário composto pelos cinco itens que melhor discriminaram crianças com e sem abuso sexual em dois contextos. Cada criança recebeu um escore resultante da soma das respostas com pesos de 0 a 4 (amplitude de 0 a 20), o qual, através do teorema de Bayes (RV), indicou sua probabilidade pós-teste (VPP) de abuso sexual. Conclusões: O instrumento proposto é útil por ser de fácil aplicação, auxiliando o pediatra na identificação de crianças vítimas de abuso sexual. Ele fornecerá, conforme o escore obtido, a probabilidade (VPP) de abuso sexual, orientando na conduta de cuidado à criança. / Aims: Develop and validate an instrument to assist the paediatrician in determining the probability of the occurrence of sexual abuse in children. Methods: Case-control study with 201 children that attended the paediatric out-patient services and the referral facilities for victims of sexual abuse, between March and November 2004: case group (suspicion or disclosure of sexual abuse) and control group (without suspicion or disclosure of sexual abuse). The responsible adults were invited to answer a questionnaire containing 18 items and five answer options, in accordance with the Likert scale, that dealt with the behavioural, physical and emotional symptoms presented by the children. Nine children that lacked sphincter control and one item that was answered by few people were excluded. The validity and internal consistency of the items were assessed by obtaining the correlation coefficients with the (Pearson, Spearman and Goodman-Kruskal), Cronbach’s α coefficient and the calculation of the area of the ROC (Receiver Operator Characteristics) curve. Later, the Likelihood Ratio (LR) and the positive predictive values (PPVs) for the five items in the questionnaire that presented the best performance were calculated. Results: A questionnaire was obtained, composed of the five items that best discriminated children with or without sexual abuse in two contexts. Each child received a score consisting of the sum of the responses with weights from 0 to 4 (range from 0 to 20) that, through the Bayes theorem (LR), indicated his/her post-test probability (PPV) of sexual abuse. Conclusions: The proposed instrument is useful as it is easily applied, is of assistance to the paediatrician in the identification of child victims of sexual abuse. It will give, according to the score obtained, the probability (PPV) of sexual abuse, and so offer guidance in the care of the child.
