Objetivo: Identificar os fatores associados à SBH. Métodos: Foram realizados dois estudos. Uma revisão sistemática da literatura, cujo objetivo foi identificar os fatores associados à ocorrência da SBH em indivíduos com idade >40 anos, por meio da busca de artigos nas principais bases de dados em ciências da saúde. E um estudo transversal, realizado com idosas (>60 anos) da comunidade de Ceilândia-DF/Brasil, avaliadas por meio de entrevistas e questionários. Resultados: Para a revisão, os descritores utilizados foram “overactive bladder”, “risk factors” e “elderly" (em inglês). Encontrou-se 124 artigos, após o uso de filtros: tema principal - “bexiga urinária hiperativa”, idiomas - inglês, português e espanhol, período de 2006/2016, limite - “meia-idade” e “idoso” reduzindo este número para 57. Após leitura do título/resumo foram excluídos 43. Após leitura na íntegra, foram excluídos 9 artigos. Assim, 5 artigos foram selecionados, totalizando uma amostra de 15.190 homens e 20.053 mulheres. Para o segundo estudo, foram elegíveis 292 idosas, destas, 172 eram grupo caso (58,9%) e 120 (41,1%) grupo controle. Observou-se alta prevalência de SBH na população estudada. A análise multivariada apontou que vida sexual ativa reduz em 70,8% a chance de ter SBH; e cirurgias uroginecológicas aumentam em 3,098 vezes esse risco. Conclusão: A literatura apresenta como fatores associados à SBH a idade avançada; sexo feminino; o elevado IMC; a raça (afro-americano e hispânico para homens); etilismo; Diabetes Mellitus, sintomas miccionais e depressão para pessoas de meia-idade e idosos. Já no estudo transversal identificou-se como fator associado à SBH a cirurgia uroginecológica. Em contra partida, apresentar diagnóstico de diabetes e manter vida sexual ativa foram fatores protetores em mulheres idosas da comunidade de Ceilândia-DF. / Hyperactive Bladder Syndrome (SBH) is the urinary urgency with or without incontinence, polaciuria and nocturia, in the absence of other local pathologies. Objective: To identify the factors associated with SBH. Methods: Two studies were carried out. A systematic review of the literature, whose objective was to identify the factors associated with the occurrence of SBH in individuals aged> 40 years, through the search of articles in the main databases in health sciences. A cross-sectional study was conducted with elderly women (> 60 years) from the community of Ceilândia-DF / Brazil, evaluated through interviews and questionnaires. RESULTS: For the review, the descriptors used were "overactive bladder", "risk factors" and "elderly". There were 124 articles, after the use of filters: main theme - "overactive urinary bladder", languages - English, Portuguese and Spanish, period 2006/2016, limit "middle age" and "old", reducing this number to 57. After reading the title / abstract, 43 were excluded. After reading the entire article, 9 articles were excluded. For the second study, 292 elderly women were eligible, of which 172 were case group (58.9%) and 120 (41.1%) were the control group. The multivariate analysis showed that active sexual life reduces the chance of having SBH by 70.8%, and urogynecological surgeries increase by 3,098 times this risk Conclusion: The literature presents as associated factors SBH the old age; Female, high BMI, race (African American and Hispanic for men); Alcoholism; Diabetes Mellitus, voiding symptoms and depression for middle-aged and elderly people. In the cross-sectional study, urogynecological surgery was identified as a factor associated with SBH. In contrast, presenting a diagnosis of diabetes and maintaining active sexual life were protective factors in elderly women from the community of Ceilândia-DF.

O trato urinário, assim como outros sistemas, é influenciado pelos efeitos interativos e aditivos destas mudanças e pelo acúmulo de inúmeros problemas relacionados ao envelhecimento. A síndrome da bexiga hiperativa (SBH) é comum em idosos e é de grande importância a investigação de estratégias terapêuticas não-invasivas para uma melhor abordagem dessa população. Objetivo: Avaliar a eficácia da eletroestimulação transcutânea em nervo tibial posterior para o tratamento da SBH em idosas. Método: Trata-se de um ensaio clínico randomizado e controlado com mulheres idosas da comunidade (>60 anos) com SBH, que foram randomizadas em 3 grupos: G1 = eletroterapia em tibial posterior em limiar sensitivo, G2 = eletroterapia em tibial posterior em limiar motor, G3 = grupo controle, sem intervenção. As variáveis analisadas no estudo foram: (1) hábito urinário (número de espisódios de frequência miccional em 24 horas, noctúria, urgência e urge-incontinência) avaliado pelo diário miccional de três dias; (2) qualidade de vida, relacionada aos sintomas de BH e o grau de incômodo dos sintomas de frequência urinária diurna, noctúria, urgência e urge-incontinência, analisados pelo questionário ICIQ-OAB e (3) avaliação do desconforto com a eletroterapia por meio da escala visual analógica. O cálculo amostral sugeriu 19 voluntárias por grupo. A distribuição não normal dos dados foi identificada por meio do teste Kolmogorov-Smirnov. A comparação das variáveis contínuas entre os três grupos foi feita utilizando o teste Kruskal Wallis, seguida pelos testes Mann-Whitney U para realizar análise de comparações múltiplas intergrupo. A comparação das variáveis categóricas foi feita utilizando o teste qui-quadrado. Foi realizado análise de intenção de tratar e o nível de significância (α) de 0,05 foi considerado. Resultados: Foram selecionadas 130 idosas que atenderam aos critérios de elegibilidade, porém 29 foram excluídas. Foram randomizadas 101 pacientes, 39 no G1 (limiar sensitivo), 33 no G2 (limiar motor) e 29 no G3 (controle). Os grupos 1 e 2 apresentaram melhora significativa no pós-intervenção relacionada a qualidade de vida, enquanto o G3 não apresentou melhora significativa (p<0,001, p<0,001, p=0,474, respectivamente). O sintoma de maior incômodo tanto no G1 quanto no G2 foi a urgência, apresentando média de 7,09±3,12, 7,27±2,50, respectivamente. O G3 apresentou maior pontuação de grau de incômodo no sintoma de urge-incontinência (8,00± 3,73). No hábito urinário, tanto o G1 quanto o G2 apresentaram melhora significativa no pós-tratamento, para os parâmetros de frequência (p<0,001 para ambos), noctúria (p<0,001 para ambos), urgência (p<0,001 para ambos) e urge-incontinência (p<0,001, p=0,002). O G3 não apresentou melhora significativa. Conclusão: A eletroestimulação transcutânea no nervo tibial posterior mostrou-se eficaz no tratamento de SBH em idosas, não havendo diferença entre os limiares sensitivo e motor para as variáveis analisadas. __________________________________________________________________________________ ABSTRACT / Introduction: Aging is a continuous and inevitable process that affects humans. The urinary tract as well as other systems, is influenced by the interactive and additive effects of these changes and the accumulation of numerous problems related to aging. The overactive bladder syndrome (OABS) is common in the elderly is of great importance and the investigation of non-invasive therapeutic strategies for better treatment of this population. Objective: To evaluate the efficacy of transcutaneous electrical stimulation on posterior tibial nerve for the treatment of OABS in elderly women. Method: This is a randomized controlled trial with community elderly women (> 60 years) with OABS, who were randomized in 3 groups: G1 = electrotherapy in posterior tibial in sensory threshold, G2 = electrotherapy in posterior tibial in motor threshold, G3 = control group, without intervention. The variables analyzed in the study were: (1) urinary habit (number of episodes of voiding frequency in 24 hours, nocturia, urgency and urgeincontinence) assessed by voiding diary of three days; (2) quality of life related to symptoms of OAB and the degree of discomfort of the symptoms of daytime urinary frequency, nocturia, urgency and urgeincontinence, analyzed by questionnaire ICIQ-OAB, and (3) evaluation of discomfort of electrical stimulation through visual analogue scale. The sample size suggest 19 voluntary per group. The non normal distribution of data was done using Kolmogorov-Smirnov test. The comparison of continuous variables between groups was made using the Kruskal Wallis test followed by Mann-Whitney U tests to perform analysis of multiple intergroup comparisons. Comparison of categorical variables was performed using chi-square test. Intention to treat analysis and the significance level (α) of 0.05 were considered. Results: 130 elderly women who met the eligibility criteria were selected, but 29 were excluded. 101 patients, 39 were randomized in G1 (sensory threshold), 33 in G2 (motor threshold) and 29 in G3 (control). Groups 1 and 2 showed significant improvement in the post-intervention related to quality of life, while the G3 does not showed significant improvement (p <0.001, p <0.001, p = 0.474, respectively). The most bothersome symptom of both G1 and G2 was the urgency and averaged 7.09 ± 3.12, 7.27 ± 2.50, respectively. G3 presented higher scores of the level of discomfort in the symptom of urgeincontinence (8.00 ± 3.73). In urinary habits, both G1 and G2 showed significant improvement after treatment, for the parameters of frequency (p <0.001 for both), nocturia (p <0.001 for both), urgency (p <0.001 for both) and urgeincontinence (p <0.001, p = 0.002). The G3 showed no significant improvement. Conclusion: Transcutaneous electrical stimulation on posterior tibial nerve proved to be effective in treating OABS in elderly women, with no difference between the sensory and motor thresholds for the variables analyzed.
