A dengue é uma doença infecciosa viral transmitida pela picada de mosquitos do gênero Aedes e é um importante problema de saúde pública mundial. A infecção por qualquer um dos quatro sorotipos (DENV 1-4) pode apresentar diferentes quadros clínicos: pode ser assintomática, causar uma síndrome febril indiferenciada, ou a febre da dengue (DF), a evolução do quadro clínico pode levar a dengue hemorrágica com ou sem choque (DHF/DSS). Um número crescente de casos de infecção por dengue em crianças têm sido observados nos últimos anos. Estudos de prevalência da dengue em regiões endêmicas são importantes para avaliar a incidência da infecção por dengue em crianças. Assim, o objetivo deste estudo é investigar a prevalência de anticorpos anti-dengue IgM e IgG em crianças atendidas no Centro de Saúde Escola Dr. Edgard Aché, localizado na região oeste de Ribeirão Preto-SP. Crianças (n = 271) de 1-15 anos de idade foram recrutadas de março de 2010 até maio de 2011. Depois de um termo de consentimento ter sido assinado pelo responsável, uma amostra de sangue das crianças, participantes deste estudo, foi coletada. As crianças foram classificadas em assintomáticas (n= 174) ou sintomáticas (n = 97), quando estas apresentavam um ou mais de um sintoma sugestivo de dengue, de acordo com critérios da Organização Mundial de Saúde. Anticorpos IgM e IgG anti-dengue foram detectado nas amostras de soro por um ELISA de captura padronizado em nosso laboratório. O ELISA de captura para IgG mostrou uma positividade de 9,23% (25/271) e o ELISA de captura para IgM de 8,49% (23/271). O ELISA de captura para IgG foi positivo em 10,31% (10/97) das crianças sintomáticas e 8,62% (15/174) das crianças assintomáticas, enquanto que o ELISA de captura para IgM foi positivo em 15,46% (15/97) das crianças sintomáticas e 4,6% (8/174) em crianças assintomáticas. Este estudo mostrou a alta prevalência de anticorpos anti-dengue em crianças da região oeste do município de Ribeirão Preto; mostrou também que a infecção pode ser causada de forma assintomática, e que a infecção por este vírus pode ser um grave problema de saúde nesta população, servindo como um alerta às autoridades de saúde. / Dengue is an infectious viral disease transmitted by the biting of mosquitoes of Aedes genus and it is an important public health problem worldwide. Infection with any of the four serotypes (DENV 1-4) may be asymptomatic or causes illness ranging from mild viral syndrome, dengue fever (DF) to dengue hemorrhagic fever (DHF). An increasing number of dengue infection cases in children have been noted in the last years. A dengue surveillance study might be an important tool in endemic region to evaluate the incidence of dengue infection in children. Thus, the aim of this study was to investigate the prevalence of anti-dengue IgM and IgG antibodies in children in the Primary Health Care Center, Dr. Edgard Aché, located in the west region of Ribeirão Preto-SP. Children (n=271) from 1 to 15 years old were recruited during March 2010 until May 2011. After a signed consent by the person responsible for the children to participate in this study, a blood sample was collected. The children were classified in asymptomatic (n=174) or symptomatic (n=97) when they had more than one symptom suggestive of dengue according to the World Health Organization criterions. Anti-dengue IgM and IgG were detected in serum samples by a capture ELISA standardized in our laboratory. IgG capture ELISA was positive in 9,23% (25/271) and IgM capture ELISA in 8,49% (23/271) of the children. IgG capture ELISA was positive in 10,31% (10/97) of the symptomatic children and 8,62% (15/174) in asymptomatic children; while IgM capture ELISA was positive in 15,46% (15/97) of symptomatic children and 4,6% (8/174) in asymptomatic children. This study showed the high prevalence of anti-dengue antibodies in children in the west region of Ribeirão Preto. In addition, a high prevalence of dengue-infected children without symptoms was observed. The present survey demonstrated that dengue virus infection might be a problem in children\'s from Ribeirão Preto city and serve as an alert to the health authorities.

As hepatites virais A e E estão entre as doenças mais comuns nos seres humanos, relacionadas a países com baixo nível socioeconômico e condições inadequadas de higiene. São transmitidas por via fecal-oral, diretamente pessoa a pessoa ou por contaminação de água e alimentos. Um ciclo zoonótico da hepatite E (genótipo 3) ocorre principalmente pelo consumo de carne ou contato direto com suínos. A incidência de hepatite A vem reduzindo no país, mas pouco tem sido estudado sobre a prevalência em cidades pequenas do interior. Os estudos sobre a hepatite E no país ainda são limitados e sugerem que o genótipo 3 seja o responsável pela infecção. Este estudo buscou estimar a prevalência das hepatites A e E na população adulta de Cássia dos Coqueiros (aprox. 2600 hab.) e levantar possíveis fatores de risco para as infecções. Trata-se de um inquérito soroepidemiológico em 990 habitantes de Cássia dos Coqueiros com mais de 18 anos de idade, aproximadamente 51% de toda a população do município nesta faixa etária, com uma subamostragem de 248 indivíduos especificamente para pesquisa de hepatite E. As amostras de sangue foram coletadas entre os anos de 2011 e 2013 e aplicado um questionário contendo informações demográficas, socioeconômicas e de fatores de risco. Foram realizados exames imunológicos (ELISA) para detecção de anticorpos IgG para as hepatites A e E. As prevalências foram estimadas por faixa etária e calculados seus respectivos intervalos de confiança. Os dados foram submetidos a análises uni e multivariadas, em modelo de regressão logística log-binomial. As associações foram testadas medindo-se as Razões de Prevalência e seus respectivos intervalos de confiança (95%). A prevalência de hepatite A na população total foi 89,1% (IC95%= 87,1 - 90,9), aumentando de acordo com as faixas etárias. Observou-se na análise univariada associação com idade, unidade federada de origem, escolaridade, estrato econômico, antecedentes de hepatite e de contato domiciliar com casos desta doença, bem como história de eliminação de proglotes de Taenia sp nas fezes, mas após ajuste do modelo, apenas a variável idade permaneceu independentemente associada. A prevalência de hepatite E foi 20,7% (IC95%= 15,5 - 25,7), identificando-se associação apenas com a escolaridade na análise univariada, mas que deixou de ser associada após ser ajustada no modelo multivariado. Este é o primeiro estudo soroepidemiológico de base populacional comparando a presença de VHA e VHE numa cidade de porte pequeno com características rurais na região Sudeste do país. Cássia dos Coqueiros apresenta-se como área de baixa endemicidade para VHA, assim como as regiões Sul e Sudeste. A prevalência de VHE foi a maior encontrada em estudo de base populacional ou doadores de sangue no país, mostrando que a infecção ocorre de forma mais frequente do que se acreditava e que mais estudos são necessários para verificar o genótipo circulante e os fatores de risco para infecção, possivelmente pela transmissão zoonótica do VHE3. / Viral hepatitis A and E are among the most common diseases in humans, related to countries with low socioeconomic level and inadequate hygiene conditions. They are transmitted by fecal-oral route, directly person to person or by contamination of water and food. A zoonotic cycle of hepatitis E (genotype 3) occurs mainly through meat consumption or direct contact with swine. The incidence of hepatitis A has been declining in Brazil, but little has been studied about the prevalence in small towns in the interior. Studies on hepatitis E in the country are still limited and suggest that genotype 3 is responsible for the infection. This study aimed to estimate the prevalence of hepatitis A and E in the adult population of Cássia dos Coqueiros (approximately 2600 inhabitants) and to assess possible risk factors for infections. This is a seroprevalence survey of 990 inhabitants of Cássia dos Coqueiros, over 18 years of age, approximately 51% of the entire population of the municipality in this age group, with a subsampling of 248 individuals specifically for hepatitis E screening. Samples were collected between the years 2011 and 2013 and a questionnaire containing demographic, socioeconomic and risk factors information was applied. Immunological tests (ELISA) were performed to detect IgG antibodies to hepatitis A and E. The prevalences of hepatitis A and E were estimated by age group and their respective confidence intervals were calculated. The data were submitted to multivariate analysis using a log-binomial logistic regression model. Associations were tested by measuring the Prevalence Ratios and their respective confidence intervals (95%). The prevalence of hepatitis A in the total population was 89.1% (95% CI = 87.1-90.9), increasing according to age. The univariate analysis identified association with age, federated unit of origin, education level, economic strata, history of hepatitis and home contact with cases of this disease, as well as history of elimination of Taenia sp proglottes in feces. After adjustment of the model, only age remained independently associated. The prevalence of Hepatitis E was 20.7% (95% CI = 15.5 - 25.7), which was only associated with education level in the univariate analysis, but no longer associated with adjustment in the multivariate model. This is the first population-based seroprevalence study comparing the presence of HAV and HEV in a small city with rural characteristics in the Southeastern region of the country. According to the WHO criteria, Cássia dos Coqueiros seems to be as an area of low endemicity for HAV, as well as the South and Southeast regions of Brazil. The prevalence of HEV was the highest found in a population-based study or blood donors in the country, showing that the infection occurs more frequently than previously believed and that more studies are needed to verify the circulating genotype and the risk factors for infection, possibly due to the zoonotic transmission of HEV3.

Validação de estratégias a campo para o controle de Salmonella sp. na cadeia de produção de suínos

O Brasil ocupa uma posição de destaque mundial em relação à produção agropecuária, sendo necessário fornecer segurança microbiológica aos consumidores. Salmonella é um agente causador de infecções alimentares em seres humanos, de forma que os produtos de origem suína são responsáveis por cerca de 5-10% dos surtos em humanos. O controle depende do conhecimento da distribuição da bactéria desde o rebanho até o frigorífico. Em regiões com altas prevalências no campo, esforços direcionados primeiramente em reduzir a prevalência nos rebanhos visam minimizar os riscos de contaminação dos produtos. Neste sentido, medidas de biossegurança, seguindo boas práticas de produção agropecuária, são fundamentais. Além disso, a aplicação de intervenções complementares são, possivelmente, formas de reduzir a prevalência em um período de tempo mais curto. Desta forma o objetivo deste trabalho foi validar três estratégias: 1) utilização de um prebiótico Actigen®™ na ração dos animais, (PRE); 2) uma vacina viva Enterisol SC 54®, (VAC) e 3) o sistema de wean-to-finish, (WTF). Estes grupos foram comparados entre si e com o sistema tradicional em três sítios, o grupo controle (GC), frente à soroprevalência e contaminação em carcaças. Cada estratégia foi realizada em três repetições, sendo colhidas amostras de sangue de 55 animais de cada lote no dia do alojamento na terminação e quatro dias antes do abate. Suabe de 40 carcaças de cada lote foram colhidas antes do resfriamento. As soroprevalências e frequências de isolamento foram comparadas entre os grupos por meio de teste de qui-quadrado. A soroprevalência pré-abate foi estatisticamente menor no grupo PRE 50,3% em relação ao WTF, VAC e GC, com 99%, 96,9% e 98,8% respectivamente. As frequências de isolamentos em superfície de carcaça variaram de 0% a 29,1% nos grupos PRE e VAC respectivamente, sendo que ambas diferem significativamente entre si e dos grupos CG 18,33% e WTF 15% (p<0,05). Pode-se comprovar a eficácia do prebiótico em prevenir a infecção a campo frente às demais estratégias. Em relação às contaminações de carcaças, os resultados corroboram com os conhecimentos acerca do papel da pressão de infecção do campo nas contaminações na planta frigorífica. / Brazil has been increasing its worldwide position in relation to agricultural production, and is necessary providing food safety to consumers. Salmonella is a foodborne pathogen for humans and pork products play an important role in the amount of outbreaks. The control depends on the knowledge of the distribution of the bacteria occurrence from the herd to the slaughterhouse. In regions with high on farm prevalence, efforts are primarily directed to reduce the prevalence in the swine population in order to minimize the risks of products contamination. In this sense, biosecurity measures and good production practices are useful. Moreover, increase the knowledge about additional interventions, to reduce on farms prevalence in a shorter period of time, also is important. Therefore, the objective of this work was to validate three strategies: 1) use of Actigen ® ™ prebiotic in animal feed, (PRE); 2) a live vaccine Enterisol SC54® (VAC); and 3) the system of wean- to-finish, (WTF). Seroprevalence and contamination on carcasses surface in these groups were compared with the traditional system in three sites (the control group-CG). Each strategy was performed in three replicates, and blood samples were collected from 55 animals of each batch at the first day of finishing phase and four days before slaughter. Swabs of 40 carcasses were taken from each batch before chilling. The seroprevalence and isolation frequencies were compared between groups using logistic regression. The seroprevalence before slaughter was lower in PRE (50.3%) compared with the WTF, VAC and GC groups, with 99%, 96.9% and 98.79%, respectively. The frequency of Salmonella isolation was lower in PRE group 0%, when compared with the other groups (p<0.05). The results prove that prebiotic is able to prevent infection in the field compared to the other strategies. Regarding the carcass contamination, these finds are consistent with the knowledge on the role of infection pressure in the field contamination in the plant.

Epidemiologic and Economic Analysis of Avian Influenza in Nepal

Many countries, including Nepal, have been affected with highly pathogenic avian influenza (HPAI) outbreaks. There have been human mortalities in some countries and large numbers of poultry either died or were culled due to HPAI. The overall objective of this thesis was to improve our understanding of the epidemiology and economics of avian influenza (AI), and particularly HPAI, in Nepal. We determined the seroprevalence of and risk factors for AI virus antibodies presence in ducks in Kathmandu, Nepal. The estimated true prevalence of AI viruses (AIV) antibodies was 27.2% [95% Confidence Interval (CI): 24.6- 29.5]. Age of the ducks was identified as the only risk factor for AIV seropositivity. Ducks older than one year were more likely to be seropositive compared to ducks less than six months of age [Odds Ratio= 2.17 (95% CI: 1.07- 4.39)]. This study provided baseline information about seroprevalence of AIVs in Kathmandu that will benefit further research to differentiate the subtypes of AIVs circulating in Kathmandu. We also evaluated alternatives to the current control program (CCP) for HPAI in Nepal. The considered alternatives were: (i) absence of control measures (ACM) and (ii) vaccinating 60% of the domestic poultry flock twice per year. Cost-benefit analysis approach was used to evaluate the economic feasibility of the programs. In terms of the benefit-cost ratio, our findings indicated that there is a return of 1.96 dollars for every dollar spent in the CCP compared to ACM. The net present value of the CCP versus ACM was US$ 989,918. The vaccination program yielded a return of 2.41 dollars for every dollar spent when compared to the CCP. The net present value of vaccination versus implementing the CCP was US$ 13,745,454. These results support a continued investment into the CCP rather than ceasing to implement government regulated control measures and suggest that vaccination may be an even better control alternative. In summary, our studies have highlighted the value of epidemiologic and economic analysis in research of AI. Our results are expected to lead to an improved understanding and awareness of AI in Nepal and to formulation of better control strategies.

Seroprevalence and contamination on carcasses surface in these groups were compared with the traditional system in three sites (the control group-CG). Each strategy was performed in three replicates, and blood samples were collected from 55 animals of each batch at the first day of finishing phase and four days before slaughter. Swabs of 40 carcasses were taken from each batch before chilling. The seroprevalence and isolation frequencies were compared between groups using logistic regression. The seroprevalence before slaughter was lower in PRE (50.3%) compared with the WTF, VAC and GC groups, with 99%, 96.9% and 98.79%, respectively. The frequency of Salmonella isolation was lower in PRE group 0%, when compared with the other groups (p<0.05). The results prove that prebiotic is able to prevent infection in the field compared to the other strategies. Regarding the carcass contamination, these finds are consistent with the knowledge on the role of infection pressure in the field contamination in the plant.

Validação de estratégias a campo para o controle de Salmonella sp. na cadeia de produção de suínos

O Brasil ocupa uma posição de destaque mundial em relação à produção agropecuária, sendo necessário fornecer segurança microbiológica aos consumidores. Salmonella é um agente causador de infecções alimentares em seres humanos, de forma que os produtos de origem suína são responsáveis por cerca de 5-10% dos surtos em humanos. O controle depende do conhecimento da distribuição da bactéria desde o rebanho até o frigorífico. Em regiões com altas prevalências no campo, esforços direcionados primeiramente em reduzir a prevalência nos rebanhos visam minimizar os riscos de contaminação dos produtos. Neste sentido, medidas de biossegurança, seguindo boas práticas de produção agropecuária, são fundamentais. Além disso, a aplicação de intervenções complementares são, possivelmente, formas de reduzir a prevalência em um período de tempo mais curto. Desta forma o objetivo deste trabalho foi validar três estratégias: 1) utilização de um prebiótico Actigen®™ na ração dos animais, (PRE); 2) uma vacina viva Enterisol SC 54®, (VAC) e 3) o sistema de wean-to-finish, (WTF). Estes grupos foram comparados entre si e com o sistema tradicional em três sítios, o grupo controle (GC), frente à soroprevalência e contaminação em carcaças. Cada estratégia foi realizada em três repetições, sendo colhidas amostras de sangue de 55 animais de cada lote no dia do alojamento na terminação e quatro dias antes do abate. Suabe de 40 carcaças de cada lote foram colhidas antes do resfriamento. As soroprevalências e frequências de isolamento foram comparadas entre os grupos por meio de teste de qui-quadrado. A soroprevalência pré-abate foi estatisticamente menor no grupo PRE 50,3% em relação ao WTF, VAC e GC, com 99%, 96,9% e 98,8% respectivamente. As frequências de isolamentos em superfície de carcaça variaram de 0% a 29,1% nos grupos PRE e VAC respectivamente, sendo que ambas diferem significativamente entre si e dos grupos CG 18,33% e WTF 15% (p<0,05). Pode-se comprovar a eficácia do prebiótico em prevenir a infecção a campo frente às demais estratégias. Em relação às contaminações de carcaças, os resultados corroboram com os conhecimentos acerca do papel da pressão de infecção do campo nas contaminações na planta frigorífica. / Brazil has been increasing its worldwide position in relation to agricultural production, and is necessary providing food safety to consumers. Salmonella is a foodborne pathogen for humans and pork products play an important role in the amount of outbreaks. The control depends on the knowledge of the distribution of the bacteria occurrence from the herd to the slaughterhouse. In regions with high on farm prevalence, efforts are primarily directed to reduce the prevalence in the swine population in order to minimize the risks of products contamination. In this sense, biosecurity measures and good production practices are useful. Moreover, increase the knowledge about additional interventions, to reduce on farms prevalence in a shorter period of time, also is important. A dengue é uma doença infecciosa viral transmitida pela picada de mosquitos do gênero Aedes e é um importante problema de saúde pública mundial. A infecção por qualquer um dos quatro sorotipos (DENV 1-4) pode apresentar diferentes quadros clínicos: pode ser assintomática, causar uma síndrome febril indiferenciada, ou a febre da dengue (DF), a evolução do quadro clínico pode levar a dengue hemorrágica com ou sem choque (DHF/DSS). Um número crescente de casos de infecção por dengue em crianças têm sido observados nos últimos anos. Estudos de prevalência da dengue em regiões endêmicas são importantes para avaliar a incidência da infecção por dengue em crianças. Assim, o objetivo deste estudo é investigar a prevalência de anticorpos anti-dengue IgM e IgG em crianças atendidas no Centro de Saúde Escola Dr. Edgard Aché, localizado na região oeste de Ribeirão Preto-SP. Crianças (n = 271) de 1-15 anos de idade foram recrutadas de março de 2010 até maio de 2011. Depois de um termo de consentimento ter sido assinado pelo responsável, uma amostra de sangue das crianças, participantes deste estudo, foi coletada. As crianças foram classificadas em assintomáticas (n= 174) ou sintomáticas (n = 97), quando estas apresentavam um ou mais de um sintoma sugestivo de dengue, de acordo com critérios da Organização Mundial de Saúde. Anticorpos IgM e IgG anti-dengue foram detectado nas amostras de soro por um ELISA de captura padronizado em nosso laboratório. O ELISA de captura para IgG mostrou uma positividade de 9,23% (25/271) e o ELISA de captura para IgM de 8,49% (23/271). O ELISA de captura para IgG foi positivo em 10,31% (10/97) das crianças sintomáticas e 8,62% (15/174) das crianças assintomáticas, enquanto que o ELISA de captura para IgM foi positivo em 15,46% (15/97) das crianças sintomáticas e 4,6% (8/174) em crianças assintomáticas. Este estudo mostrou a alta prevalência de anticorpos anti-dengue em crianças da região oeste do município de Ribeirão Preto; mostrou também que a infecção pode ser causada de forma assintomática, e que a infecção por este vírus pode ser um grave problema de saúde nesta população, servindo como um alerta às autoridades de saúde. / Dengue is an infectious viral disease transmitted by the biting of mosquitoes of Aedes genus and it is an important public health problem worldwide. Infection with any of the four serotypes (DENV 1-4) may be asymptomatic or causes illness ranging from mild viral syndrome, dengue fever (DF) to dengue hemorrhagic fever (DHF). An increasing number of dengue infection cases in children have been noted in the last years. A dengue surveillance study might be an important tool in endemic region to evaluate the incidence of dengue infection in children. Thus, the aim of this study was to investigate the prevalence of anti-dengue IgM and IgG antibodies in children in the Primary Health Care Center, Dr. Edgard Aché, located in the west region of Ribeirão Preto-SP. Children (n=271) from 1 to 15 years old were recruited during March 2010 until May 2011. After a signed consent by the person responsible for the children to participate in this study, a blood sample was collected. The children were classified in asymptomatic (n=174) or symptomatic (n=97) when they had more than one symptom suggestive of dengue according to the World Health Organization criterions. Anti-dengue IgM and IgG were detected in serum samples by a capture ELISA standardized in our laboratory. IgG capture ELISA was positive in 9,23% (25/271) and IgM capture ELISA in 8,49% (23/271) of the children. IgG capture ELISA was positive in 10,31% (10/97) of the symptomatic children and 8,62% (15/174) in asymptomatic children; while IgM capture ELISA was positive in 15,46% (15/97) of symptomatic children and 4,6% (8/174) in asymptomatic children. This study showed the high prevalence of anti-dengue antibodies in children in the west region of Ribeirão Preto. In addition, a high prevalence of dengue-infected children without symptoms was observed. The present survey demonstrated that dengue virus infection might be a problem in children\'s from Ribeirão Preto city and serve as an alert to the health authorities.

Determination of the seroprevalence of Newcastle disease virus (Avian paramyxovirus type 1) in Zambian backyard chicken flocks

The specific objectives of this study were to determine the Newcastle disease virus (NDV) antibody titres from the chicken sera collected from various districts and provinces of Zambia and to determine the seroprevalence of ND in Zambian backyard chickens. Results showed that 73.9 % of the birds sampled tested positive for Newcastle disease (ND) antibodies. The seroprevalence of Newcastle disease virus (NDV) in Zambian backyard chicken flocks varied among the five provinces sampled, ranging from 82.6 % in Eastern Province to 48.3 % in Luapula Province. The seroprevalence of the virus also varied among the 11 districts sampled, ranging from 91.3 % in Monze District of Southern Province to 22.8 % in Mufulira District of the Copperbelt Province. The results indicated that the seroprevalence of ND in Zambia has increased since the last survey conducted in 1994. The data generated is expected to contribute towards a more clear understanding of the epidemiology of NDV that would ultimately contribute towards an improved ND control programme to benefit all stakeholders in Zambia. The Epidemiology of SARS-CoV-2 and Seasonal Respiratory Viruses in Ottawa, Ontario During the COVID-19 Pandemic

This thesis aims to describe the epidemiology of SARS-CoV-2 and other seasonal respiratory viruses (SRV) in the Ottawa region and determine if there are demographic or occupational risk factors associated with viral infection. This was accomplished in three articles: a rapid review of the literature evaluating SARS-CoV-2 seroprevalence in transit users, an account of SRV activity in Ottawa, Ontario from August, 2018 to January, 2022, and an analysis of coronavirus (CoV) incidence and prevalence in several at risk groups. Results indicated a reduction in SRV incidence during the COVID-19 pandemic correlated with the introduction of non-pharmaceutical public health measures. SARS-CoV-2 seroprevalence indicative of natural infection was not greater in population groups hypothesized to be at greater risk of infection, such as transit users and those exposed to children. Lower socioeconomic status and racial or ethnic minority were consistently associated with higher SARS-CoV-2 seroprevalence. SRVs are expected to resurge and co-circulate with SARS-CoV-2. Results from this thesis suggest those of ethnic minority, low income or immunocompromised individuals may at greater risk.
