Les stents coronariens sont des dispositifs médicaux utilisés pour soutenir mécaniquement les artères atteintes d'athérosclérose. Cette maladie se caractérise par l'accumulation de dépôts lipidiques à la surface des parois artérielles, ce qui provoque une diminution du diamètre effectif de l'artère et la perturbation du débit sanguin. Les stents sont principalement constitués d'une structure métallique recouverte d'un polymère. Malgré leur succès commercial, plusieurs lacunes au niveau de l'adhérence sont rapportées à la suite de leur expansion, telles que des délaminations et l'apparition de fissures. Ces défaillances affectent notamment l'efficacité des stents. L'objectif de cette thèse est donc d'étudier cette problématique en utilisant des revêtements fluorocarbonés déposés par plasmas. Ces travaux doctoraux proposent une nouvelle technique d'évaluation de l'adhérence appelée, le Small Punch Test. Cette méthode reproduit l'expansion d'un stent sur des échantillons plats ce qui facilite les caractérisations des revêtements après déformation. Des caractérisations au microscope électronique à balayage ainsi qu'au microscope à force atomique ont permis d'évaluer l'influence de l'épaisseur des revêtements sur leur cohésion. Des fissures ont été observées sur les échantillons possédant une épaisseur supérieure à 35 nm. Afin de déposer des revêtements à faible teneur en nano-pores et d'augmenter leur stabilité en milieu aqueux, ces travaux étudient également la croissance et les propriétés des revêtements fluorocarbonés en fonction de différents décapages plasmas. Les décapages d'hydrogène ont notamment conduit à la formation d'hydroxydes à la surface de l'acier qui ont agit comme initiateur à la polymérisation. Le revêtement ainsi formé possède une teneur réduite en nano-pores, est riche en CF2 et est composé de longues chaînes polymères tel qu'observé par XPS, ToF-SIMS et FTIR. Cependant, les décapages plasmas ont modifié et réduit l'épaisseur de la couche d'oxyde de l'acier inoxydable ce qui l'a rendue plus sensible à une post-oxydation

In Vitro Model of Vascular Healing in the Presence of BiomaterialsRose, Stacey Loren 16 November 2006 (has links)
Coronary artery stent placement has been a significant advance in the percutaneous treatment of atherosclerotic disease, and tissue engineered vascular grafts may provide a viable alternative to autologous segments for small diameter vessels. However, in-stent restenosis remains an important limitation, and tissue engineered grafts have poor patency and high risk of thrombus formation due to their inability to maintain a confluent, adherent, and quiescent endothelium. While animal models provide insight into the pathophysiology of these situations, elucidation of the relative importance of stent or graft components, hemodynamic factors, and molecular factors is difficult. Very little research has focused on bridging gaps in knowledge concerning blood/biomaterial interactions, blood/endothelial cell interactions, and endothelial cell/smooth muscle cell cross-talk. The work presented within this thesis will do just that. The objective of this thesis research was to elucidate the influence of biomaterial-induced activation of leukocytes on endothelial cell or smooth muscle cell phenotype, as well as endothelial cell/smooth muscle cell cross-talk in co-culture systems. Towards this goal, two complimentary in vitro endothelial cell/smooth muscle cell co-culture models with divergent smooth muscle cell phenotype were developed and characterized. Using these systems, it was found that the presence of more secretory smooth muscle cells (as would be seen in wound healing or disease) in general enhanced endothelial cell activation in response to biomaterial-pretreated monocytes, while the presence of less secretory smooth muscle cells (to model more quiescent smooth muscle cells found in uninjured healthy vessels) suppressed endothelial cell activation in response to biomaterial-pretreated monocytes (and neutrophils to a small degree). Additionally, biomaterial-pretreated monocytes and neutrophils amplified a smooth muscle cell phenotypic shift away from a more quiescent state. It is likely that the compounding effect of secretory smooth muscle cells and biomaterial-activated leukocytes are responsible for altered vascular wound healing upon implantation of stents or vascular grafts. Understanding the specific signals causing these effects, or signals delivered by contractile smooth muscle cells that limit these effects help to provide design criteria for development of devices or grafts capable of long term patency.

Objective - to assess possibilities and specific characteristics of pediatric cardiology in treatment of congenital heart diseases (CHD), to evaluate efficacy of curative per-catheter procedures by means of analysis of immediate and long-term results. Retrospective study. The data of 422 patients who underwent 467 CHD palliative-curative procedures during the period since 1971 till 2007 were analyzed. It was postulated that balloon atrial septostomy resulted in statistically significant increase of atrial septal defect, increase of arterial blood oxygen saturation and decrease of interatrial preasure gradient (PG). Balloon pulmonary valvulotomy (BPV) is one of the most common curative procedures; this procedure has an effect of marked decrease of pressure gradient between the right ventricle and right atrium; development of pulmonary artery valve insufficiency is the most common complication of this procedure. The long - term results of BPV are less positive when higher PG prior the procedure is present and residual PG after the procedure is 36mmHg and higher. It was postulated, that closure of small (less than 3 mm) persistent ductus arteriosus using Cook coils may compete with surgical treatment successfully. [to full text] / Disertacijos objektas yra nustatyti intervencinės pediatrinės kardiologijos galimybes ir ypatumus, gydant įgimtas širdies ydas (ĮŠY), įvertinti gydomųjų perkateterinių procedūrų efektingumą, remiantis ankstyvųjų ir vėlyvųjų rezultatų analize. Tai retrospektyvus tyrimas. Analizuoti 422 ligonių duomenys, kuriems 1971 - 2007 m. buvo atliekamos 467 įgimtų širdies ydų paliatyvinės - gydomosios procedūros. Nustatyta, kad po balioninės tarpprieširdinės pertvaros septostomijos, statistiškai reikšmingai padidėja prieširdžių pertvaros defektas, didėja arterinio kraujo įsotinimas deguonimi ir mažėja spaudimų skirtumas (SS) tarp prieširdžių. Balioninė plaučių arterijos valvuloplastika (BPV) yra viena iš dažniausiai taikomų gydomųjų procedūrų, jos efektas – ryškus SS tarp dešiniojo skilvelio ir plaučių arterijos (PA) sumažėjimas, o pagrindinė komplikacija – PA vožtuvo nesandarumo vystymasis. BPV vėlyvieji rezultatai blogesni, kai yra didelis SS prieš procedūrą, o po procedūros liekamasis SS ≥ 36mmHg. Nustatyta, kad mažų iki 3mm AAL kimšimas Cook spiralėmis gali sėkmingai konkuruoti su operaciniu gydymu. Rasta, kad aortos, tuščiųjų venų ir plaučių arterijos šakų balioninės plastikos efektas trumpalaikis, o gydymas stentais daug sėkmingesnis. Nustatyta, kad anomalinių įgimtų ir pooperacinių kraujagyslinių jungčių užkimšimas spiralėmis yra saugus ir efektyvus gydymo metodas.

Hemodynamic changes in intracranial aneurysms due to stent-induced vascular remodeling /Santos, Gabriel Bertacco dos. January 2018 (has links)
Orientador: José Luiz Gasche / Resumo: Originalmente, stents foram projetados para agir como barreiras mecânicas, impedindo a herniação de coils para a artéria-mãe. Recentemente, estudos mostraram que a atual geração de stents intracranianos auto-expansíveis altera a geometria local das artérias: um fenômeno com efeitos hemodinâmicos em parte incompreendidos. Nós realizamos simulações numéricas para avaliar a influência da remodelagem arterial induzida por stent sobre a hemodinâmica em aneurismas intracranianos. As simulações foram realizadas utilizando o software open-source OpenFOAM. O sangue foi modelado como fluido Newtoniano incompressível e as paredes arteriais foram consideradas rígidas. Para quantificar as alterações hemodinâmicas, avaliamos os parâmetros wall shear stress, WSS, e oscillatory shear index, OSI. Quatro geometrias reais de aneurismas intracranianos em bifurcações foram utilizadas. Em um aneurisma na bifurcação da artéria comunicante anterior (ACoA), um stent foi implantando, levando ao endireitamento das artérias que o receberam. Após o procedimento, os níveis de WSS e OSI aumentaram aproximadamente 60% e 25%, respectivamente. Em dois aneurismas em bifurcações da artéria cerebral média (MCA), dois stents foram implantados em uma configuração em “Y”, resultando em um endireitamento de ambas as artérias-filhas. The current generation of self-expanding intracranial stents has recently been shown to change the local vascular geometry, a phenomenon with unclear hemodynamic effects. We carried out numerical simulations to assess the role of stent-induced vascular remodeling in modifying intraaneurysmal hemodynamics. Simulations were performed using the open-source software OpenFOAM. Blood was assumed to behave as an incompressible Newtonian fluid; vessel walls were assumed to be rigid. Wall shear stress, WSS, and oscillatory shear index, OSI, were evaluated to quantify the hemodynamic changes in the aneurysm sac. Four pre- and post-stent patient-specific geometries of intracranial bifurcation aneurysm were used. In one aneurysm at the anterior communicating artery (ACoA) bifurcation, a single stent was deployed, resulting in straightening of the host vessels. After stenting, WSS and OSI increased by approximately 60% and 25%, respectively. In two aneurysms at middle cerebral artery (MCA) bifurcations, two stents in a “Y” configuration were deployed, resulting in straightening of both daughter arteries. Objetivo: Avaliar os resultados clínicos e radiológicos tardios de pacientes submetidos à angioplastia carotídea com colocação de stent (ACS) por doença aterosclerótica estenosante bulbar e determinar se o método é efetivo na prevenção de novos eventos isquêmicos cerebrais e se o método é durável na manutenção perviedade arterial. Métodos: Acompanhamos clinicamente e radiologicamente, por ultra-som Doppler (USD) e angiografia cerebral com subtração óssea digital (DSA), pacientes com estenose do bulbo carotídeo por doença aterosclerótica submetidos a ACS no período de janeiro de 2002 a maio de 2005 realizadas nos INRAD-HCFMUSP e Hospital Santa Marcelina de Itaquera. Resultados: 80 pacientes submetidos 84 ACS foram acompanhados clinicamente e radiologicamente num período entre 25 a 66 meses, média 43,8 meses, com um único caso de reestenose grave intra-stent (1,2%) e um único caso de novo evento isquêmico cerebral (1,3%). Conclusão: A ACS é um método durável na manutenção da perviedade do bulbo carotídeo e eficiente a longo prazo na redução de eventos isquêmicos cerebrais em pacientes com doença aterosclerótica do bulbo carotídeo. / Objective: to evaluate the long-term clinic and radiologic results for the patients submitted to angioplasty with stent implantation because estenotic atherosclerosis disease of the carotid bulb and to determine if ACS is effective to prevent the cerebral ischemic attack and durable to keep the carotid bulb opened. Methods: We follow-up clinically and radiologically, by ultrasound with color and spectral Doppler (USD) and with digital subtraction angiography (DSA), patients with atherosclerotic disease of the carotid bulb submitted to ACS realized between January of 2002 and may of 2005 in the INRAD-HCFMUS and Hospital Santa Marcelina de Itaquera. Results: 80 patients who underwent 84 ACS were evaluated clinically and radiologically in the long-term follow-up, 25 to 66 months, with media of the 43,8 months, with only one case of restenosis (1,2%) and only one case of new ischemic event (1,3%). Conclusion: The ACS é a effective method to keep the carotid bulb opened and it is efficient to prevent new ischemic event in patients with atherosclerotic disease of the carotid bulb.

Previous issue date: 2009-11-23 / Bakground: The presence of an increase in troponin I (cTnI) is common in patients who have undergone a percutaneous coronary intervention (PCI), but it is questionable if the myocardial lesion occurring in the intervention allows one to identify patients with a greater risk of presenting adverse cardiac events (ACE) .Objective : The overall objective of the study was to evaluate the immediate evolution of cardiac troponin I (cTnI) in patients undergoing elective coronary angioplasty with stenting, and also compare patients with serum changes after the procedure of coronary angioplasty stenting, with the clinical variables and the occurrence of adverse cardiac events (ACE) following 12 months.. Methods: This is a study of a prospective cut, which evaluates 49 consecutive patients who underwent coronary angioplasty with stent implantation. A cTnI serum dosage was done both before and after the procedure. Patients with a cut off >1.0 ng/mL before implantation were excluded. All the others received follow up for one year, to record the presence of ACE (death, acute myocardial infarction, another angioplasty). Results: cTnI serum elevations occurred in 21(45.7%) of the patients after the PCI. At the end of the follow up period, 11(23.9%) of the patients presented adverse cardiac events. There was no association between the cTnI serum elevation and the presence of ACE in the follow up. Conclusions: The elevation of cTnI found after a coronary angioplasty with stent implantation was frequent in our sample, but these elevations in serum did not show any association with clinical variables and the occurrence of adverse cardiac events following 12 months / A presença da elevação da troponina I(cTnI) é comum em pacientes submetidos a uma intervenção coronariana percutânea (ICP), porém é questionado se a lesão miocárdica

Metodologia: Este é um estudo de coorte prospectivo, que avaliou 49 pacientes consecutivos, submetidos à angioplastia coronariana com implante de stent. Application Of In Vivo Flow Profiling To Stented Human Coronary ArteriNanda, Hitesh 01 January 2004 (has links)
The study applies in vivo technique for profiling hemodynamics and wall shear stress (WSS) distribution in human coronary arteries. The methodology involves fusion of 2D Intra Vascular Ultra Sound and Bi-plane angiograms to reproduce the 3D arterial geometry. This geometry is then used in a Computational Fluid Dynamics (CFD) module for flow modeling. The Walburn and Schneck constitutive relation was used to represent the non-Newtonian blood rheology. The methodology is applied to study the relationship between WSS and Neointimal Hyperplasia (NIH) in two groups of diabetic patients after being treated separately with bare metal stents (BMS) and Sirolimus Eluting Stents (SES). The stent assignments were blinded until the end of the study. The study was repeated for the patients after 9 months. The predicted WSS ranged from (0.1- 8 N/m2) and was categorized into five classes: low ( < 1 N/m2); low-normal (1-2 N/m2); normal (2-3 N/m2); high-normal (3-4 N/m2); high ( > 4 N/m2). The results indicate NIH in 5 of the patients treated with BMS and none in SES cases. These results correlate with our predicted WSS distribution.

An Assessment of Novel Biodegradable Magnesium Alloys for Endovascular Biomaterial ApplicationsPersaud-Sharma, Dharam 10 June 2013 (has links)
Magnesium alloys have been widely explored as potential biomaterials, but several limitations to using these materials have prevented their widespread use, such as uncontrollable degradation kinetics which alter their mechanical properties. In an attempt to further the applicability of magnesium and its alloys for biomedical purposes, two novel magnesium alloys Mg-Zn-Cu and Mg-Zn-Se were developed with the expectation of improving upon the unfavorable qualities shown by similar magnesium based materials that have previously been explored. The overall performance of these novel magnesium alloys has been assessesed in three distinct phases of research: 1) analysing the mechanical properties of the as-cast magnesium alloys, 2) evaluating the biocompatibility of the as-cast magnesium alloys through the use of in-vitro cellular studies, and 3) profiling the degradation kinetics of the as-cast magnesium alloys through the use of electrochemical potentiodynamic polarization techqnique as well as gravimetric weight-loss methods. As compared to currently available shape memory alloys and degradable as-cast alloys, these experimental alloys possess superior as-cast mechanical properties with elongation at failure values of 12% and 13% for the Mg-Zn-Se and Mg-Zn-Se alloys, respectively. This is substantially higher than other as-cast magnesium alloys that have elongation at failure values that range from 7-10%. Biocompatibility tests revealed that both the Mg-Zn-Se and Mg-Zn-Cu alloys exhibit low cytotoxicity levels which are suitable for biomaterial applications. Gravimetric and electrochemical testing was indicative of the weight loss and initial corrosion behavior of the alloys once immersed within a simulated body fluid. The development of these novel as-cast magnesium alloys provide an advancement to the field of degradable metallic materials, while experimental results indicate their potential as cost-effective medical devices.
