Esquemas disfuncionais em tentativa de suicídio: análise de sobreviventes por intoxicação exógena de um hospital público do Recife.

Estudos preliminares sugerem aspectos cognitivos semelhantes em indivíduos com comportamentos suicidas, mas não estabelecem ainda as características dos pensamentos mais freqüentes nesses tipos de processamento cognitivo. Diante disso, o objetivo geral do presente estudo foiidentificar asinstâncias anterioresao pensamento(esquemas), que estivessem possivelmente associados à tentativa de suicídio. A amostra foiconstituída por 240 pacientes de ambos os sexos, com idade entre 18 a68anos, 120 pacientes estavam internados no Hospital da Restauração, mediante atendimento clínico (grupo controle) e outros 120 foramadmitidosmediante tentativa de suicídio (grupo experimental). Os instrumentosutilizadosforamum questionário sóciobiodemográfico, oMini International Neuropsychiatric Interview (M.I.N.I.) e o questionário de esquemasde Young. Os resultadosforamprocessadose analisados estatisticamente através de técnicas de estatística descritiva, alémdos testes estatísticosQui-quadrado de Mc-Nemar, Qui-quadrado de Pearson ou o teste Exato de Fisher quando as condições para utilização do teste Qui-quadrado não foram verificadas (técnicas de estatística inferencial). A margem de erro utilizada na decisão dos testes estatísticos foi 5,0%. Os principais resultados encontrados demonstram semelhança entre os grupos estudados quanto ao perfil sóciobiodemográfico, maior presença de estressores no grupo experimental, sendo os principais a vivência de uma separação afetiva e a existência de conflitos familiares, a condição psiquiátrica mais identificada na população foi o transtorno de ansiedade generalizadae a mais associada ao comportamento suicida foi a depressão maior. Os esquemas mais associados à tentativa de suicídio foram abandono, padrões inflexíveis, privação emocional, vulnerabilidade a dores e doenças, autocontrole e autodisciplina insuficientes, desconfiança e abuso, isolamento social, fracasso, dependência e incompetência, merecimento, defectividade e vergonha e inibição emocional. A presente pesquisa conclui que o aspecto mais associado ao comportamento suicida na população estudada foi a presença de determinados esquemas disfuncionais. / Every year throughout the world, approximately one million people commit suicide and 10 to 20 millions make attempts to do that. Among the methods used in suicide attempts, the most common is the exogenic poisoning. Previous studies suggest similar cognitive aspects in individuals with suicide behavior. On the other hand, these studies do not yet characterize the most frequent thoughts of people who have these types of cognitive processing. Given this, the main goal of the present study was to identify the dysfunctional schemes which were probably associated to the suicide attempts.The sample was 240 patients of both gender, between 18 and 68 years of age. 120 patients were admitted at „Hospital da Restauração‟ by clinical attendance (control group) and theothers 120 were admitted by suicide attempt (experimental group). The instruments used were a sociobiodemographic questionnaire, the Mini International Neuropsychiatric Interview (M.N.I) and the Young‟s schemes questionnaire. The results were processed and statistically analyzed using descriptive satistical techniques. Besides this, the Chi-square Mc-Nemar and the Pearson‟s Chi-square test were used. The Fisher Exact test was employed when the conditions for using the chi-square test were not verified (techniques of inferential statistics). The margin of error used on the decision of the statistical test was 5,0%. The main outcome showed a similar picture regarding the sociobiodemographic profile in both of the groups studied. In addition, there were more stressors on the experimental group than the control group. Among these stressors, the main ones were the experience of an affective separation and the existence of family conflicts. The psychiatric condition most identifyed in the population was the TAG,and major depression was the most one associated with suicidal behavior. The schemes most related to suicide attempts were abandon, uncompromising standards, emotional deprivation, vulnerabilitytopainand to sickness, insufficient self-control and self-disciplin, distrust and abuse, social isolation, failure, dependence and incompetence, deserving, defectiveness and shame, emotional inhibition. The present research conclude that the aspect most associated with suicidal behavior in the studied population was the presence of such dysfunctional schemes.
