Na busca de acelerar o reparo tendíneo, a laserterapia de baixa intensidade tem mostrado efeito benéfico. Objetivo: Avaliar o efeito da laserterapia de baixa intensidade na reparação do tendão calcâneo submetido à lesão parcial, em ratos. Métodos: Foram utilizados 60 ratos machos, distribuídos aleatoriamente em 5 grupos com 12 animais cada, sendo que 42 foram submetidos à lesão com queda de um peso de 186 gramas de uma altura de 20cm. No Grupo 1 (controle padrão) os animais não sofreram lesão tendínea e nem foram submetidos ao tratamento com o laser, no Grupo 2 (controle) os animais sofreram a lesão e não foram submetidos ao tratamento com laser, nos Grupos 3, 4 e 5 os animais sofreram lesão e foram submetidos ao tratamento com laser (830nm, P=40mW, F= 4J/cm 2 ) durante 3, 5 e 7 dias, respectivamente. Os animais dos grupos 2, 3, 4 e 5 foram submetidos a eutanásia no 8º dia após a lesão e avaliados por microscopia de polarização para analisar o grau de organização das fibras de colágeno. Resultados: Os grupos experimentais e o grupo controle padrão mostraram valores significativamente maiores em relação ao alinhamento das fibras de colágeno, comparados ao grupo controle, e não houve diferença significante quando comparados os Grupos 1 e 4, o mesmo acontecendo entre os Grupos 3 e 5. Conclusão: A terapia a laser de baixa intensidade é efetiva na melhora da organização das fibras colágenas do tendão calcâneo submetido à lesão parcial. / Introduction: Calcaneous tendon is one of the most frequently damaged tendons, and its healing may last from weeks to months to be completed. In the search after speeding tendon repair, low intensity laser therapy has showed favorable effect. Objective: To assess the effect of low intensity laser therapy on the process of tissue repair of calcaneous tendon, which underwent a partial lesion, in rats. Methods: Sixty male rats were used, randomly assigned to 5 groups containing 12 animals each one; 42 of 60 underwent lesion caused by dropping a 186 g weight from a 20 cm height. In Group 1 (standard control) animals did not suffer the lesion nor underwent laser therapy; in Group 2 (control) animals suffered the lesion but did not undergo laser therapy; in Groups 3, 4, and 5 animals suffered lesion and underwent laser therapy for 3, 5, and 7 days, respectively. Animals which suffered lesion were sacrificed on the 8 th day after the lesion and assessed by polarization microscopy to analyze the degree of collagen fibers organization. Results: Both experimental and standard control Groups presented significant values when compared with the control Groups, and there was not significant difference when Groups 1 and 4 were compared, and the same occurred between Groups 3 and 5. Conclusion: Low intensity laser therapy was effective for improving collagen fibers organization of the calcaneous tendon, after it underwent a partial lesion. / BV UNIFESP: Teses e dissertações

Objetivo: Avaliar o efeito da terapia a laser de baixa intensidade na viabilidade do retalho musculofasciocutâneo transverso do reto do abdome em ratos. Método: Foram utilizados 84 ratos Wistar-EPM1 que foram randomicamente distribuídos em 7 grupos com 12 animais cada. O Grupo 1 (simulação), grupo 2 (3J/cm2 com 1 ponto), grupo 3 (3J/cm2 24 pontos), grupo 4 (72J/cm2 1 ponto), grupo 5 (6J/cm2 1 ponto), grupo 6 (6J/cm2 24 pontos) e grupo 7 (144J/cm2 1 ponto). Todos os grupos experimentais foram submetidos a terapia a laser imediatamente após a realização do TRAM e nos outros dois dias subseqüentes. A porcentagem da área de necrose foi calculada no quarto dia pós-operatório por meio do gabarito de papel. Duas amostras foram coletadas por um punch para realizar as avaliações de mastócitos, vasos sanguíneos (1A4) e VEGF. Resultados: Foram encontradas diferenças significantes entre as porcentagens de necrose de todos os grupos, com os valores mais elevados para o grupo 1 comparado aos grupos experimentais. Entre os grupos 3 à 7 não foram encontradas diferenças significantes com p<0,292. Para as avaliações de mastócitos e VEGF os grupos 5 e 7 mostraram um aumento significante quando comparado aos outros grupos. Para avaliação de vasos sanguíneos (1A4) houve um aumento significante para os grupos 3 e 5 quando comparado aos outros grupos. Conclusão: A terapia a laser de baixa intensidade nas fluências de 6 e 144J/cm2 foi eficaz no aumento da viabilidade do retalho musculofasciocutâneo transverso do reto do abdome em ratos. / Introduction: The LLLT has been used with the aim to improve the perfusion vascular of the skin and musculocutaneous flaps. Objective: To assess the effect of low intensity laser therapy on the transverse rectus abdominis musculocutaneous flap (TRAM) viability. Methods: Were used 84 Wistar rats that random divided in 7 groups with 12 rats in each one. The Group 1 (sham laser), group 2 received 3J/cm2 with 1 point, group 3 (3J/cm2 24 points), group 4 (72J/cm2 1point), group 5 (6J/cm2 1point), group 6 (6J/cm2 24 points) and group 7 (144J/cm2 1 point). All experimental groups were undergoing to LLLT immediately after the TRAM operation, and at the next two subsequent days, and so, animals underwent 3 days with applications. The percentage of skin flap necrosis area was calculated on the fourth postoperative day through the paper template method and 2 samples of skin were collected by a 1 cm2 punch in order to perform mat cells evaluation, blood vessels with 1A4 and VEGF evaluations. Results: Were found statistically significant differences among necrosis percentage, and higher values were seen in Group 1, over all other values. Among Groups 3 to 7 statistically significant differences were not found, with p<0.292. To mast cells and VEGF evaluation the group 5 and 7 showed a significant increase when compared to another groups. The 1A4 evaluations showed that the groups 3 and 5 had a increase when compared to another groups. Conclusion: Low level laser therapy on the fluences 6 and 144J/cm2 was efficient to increase transverse rectus abdominis musculocutaneous flap (TRAM) viability in rats. / BV UNIFESP: Teses e dissertações

P. granatum (PNG) and low level laser irradiation (LLLI) have demonstrated anti-inflammatory properties and antioxidants. The aim of this study was to evaluate the anti-inflammatory and bone antiresorptive activities of PNG combined or not with LLLI in ligature-induced periodontitis in rats. Periodontitis was induced in 72 rats by inserting a nylon-3.0 around the left upper second molar. The contralateral hemimaxila was used as control. Groups of 6 animals received saline 0.9% (SAL), SAL+LLLI 4 J/cm2, PNG [60, 180, 540 (mg/kg)] or PNG 540 mg/kg+LLLI 4 J/cm2. The low intensity laser GaAlAs was applied immediately after surgery, while PNG was administered by gavage daily until day 11, when the animals were killed. The jaws were removed and processed for macroscopic, histometric, histological and immunohistochemical staining for TRAP. Gingival tissue samples were obtained to evaluate the activity of myeloperoxidase (MPO). Blood samples were collected for measurement of serum bone alkaline phosphatase (BALP), leukograms and analysis of liver and kidney function. Additionally, indexes of liver and kidney were performed. Rats were weighted daily. Periodontitis induction caused intense alveolar bone loss (ABL), reduction of BALP, cementum and periodontal ligament destructions, and intense leukocyte infiltration. Systemically, periodontitis induced leukocytosis and and did not alter hepatic transaminases, urea or creatinine serum levels, or liver and kidney indexes. The ABL was prevented by PNG (60= 34%, 180= 34%, 540= 42%), SAL+LLLI= 31% e PNG 540+LLLI= 48% (p<0.05). PNG 540 combined or not with LLLI decreased the alveolar bone resorption, the leukocyte infiltration and preserved the periodontium in the jaws in which the periodontitis was induced. Animals treated with PNG 540+LLLI showed an important decreasing of TRAP positive immunostaining (Periodontium Unchallenged = 0.0 ± 0.0, SAL= 0.016 ± 0.004; SAL+LLLI= 0.01 ± 0.003; PNG 540= 0.011 ± 0.003; PNG 540+LLLI = 0.002 ± 0.0003). The raise of MPO activity was prevented by PNG (60= 46%, 180= 49%, 540= 76%), SAL+LLLI= 54% e PNG 540+LLLI= 80% (p<0.05). In spite the reduction of BALP, seen in SAL group was not prevented by SAL+LLLI or PNG (p>0.05), the combined treatment with PNG 540+LLLI caused an increase on BALP serum levels (74.2 ± 5.4 U/l), when compared to SAL (44.9 ± 3.0 U/l) (p<0.05). No change was seen for hepatic transaminases, urea or creatinine serum levels, or in liver and kidney indexes, in all groups (p>0.05) and all groups prevented (p<0.05) the leukocytosis observed in SAL group. In short, PNG combined or not with LLLI reduced leukocyte infiltration and the number of neutrophils and reduced alveolar bone loss, at the expense of inhibiting osteoclast activation, without causing systemic changes / A periodontite é uma doença infecto-inflamatória crônica caracterizada por intensa perda óssea. P. granatum (PNG) e Irradiação com Laser de Baixa Intensidade (ILBI) têm demonstrado propriedades anti-inflamatórias e antioxidantes. O objetivo deste estudo foi avaliar as atividades anti-inflamatória e antirreabsortiva óssea da PNG combinada ou não com ILBI na periodontite induzida por ligadura em ratos. A periodontite foi induzida em 72 ratos Wistar através da inserção de um fio de nylon-3.0 em torno do segundo molar superior esquerdo. A hemimaxila contralateral foi utilizada como controle. Grupos de 6 animais receberam solução salina 0,9% (SAL), SAL+ILBI 4 J/cm2, PNG (60, 180, 540) mg/kg ou combinação de PNG 540 mg/kg+ILBI 4 J/cm2. O laser de baixa intensidade GaAlAs foi aplicado imediatamente após a cirurgia, enquanto a PNG foi administrada, por gavagem, diariamente, até o 11º dia, quando, então, os animais foram mortos, suas maxilas removidas e processadas para análises macroscópica, histométrica, histológica e marcação imunohistoquímica para TRAP. Amostras de tecido gengival foram obtidas para avaliação da atividade de mieloperoxidase (MPO). Foram coletadas amostras de sangue para dosagem dos níveis séricos de fosfatase alcalina óssea (FAO), leucograma e análise das funções hepática e renal. Adicionalmente, os índices hepático e renal foram realizados. Os ratos foram pesados diariamente. A indução da periodontite causou intensa perda óssea alveolar (POA), redução sérica da FAO, destruição do ligamento periodontal e do cemento, e intensa infiltração leucocitária. Sistemicamente, a periodontite induziu leucocitose e não causou alterações nos níveis séricos de transaminases hepáticas, ureia ou creatinina, bem como nos índices hepático e renal. A POA foi prevenida significantemente por PNG (60= 34%, 180= 34%, 540= 42%), SAL+ILBI= 31% e PNG 540+ILBI= 48% (p<0.05). PNG 540 combinada ou não com ILBI preveniu a perda óssea alveolar, a infiltração leucocitária e preservou o periodonto de maxilas nas quais a periodontite foi induzida. Animais tratados com PNG 540+ILBI mostraram significante redução na imunomarcação positiva para TRAP (Periodonto Não desafiado=0,0 ± 0,0; SAL= 0,016 ± 0,004; SAL+ILBI= 0,01 ± 0,003; PNG 540= 0,011 ± 0,003; PNG 540+ILBI= 0,002 ± 0,0003). O aumento da atividade de MPO foi prevenido por PNG (60= 46%, 180= 49%, 540= 76%), SAL+ILBI= 54% e PNG 540+ILBI= 80% (p<0.05). Embora a redução da FAO observada no grupo salina não tenha sido prevenida por PNG ou SAL+ILBI (p>0,05), PNG 540+ILBI causou um aumento significante nos níveis séricos de FAO (74,2 ± 5,4 U/l), quando comparado ao SAL (44,9 ± 3,0 U/l) (p<0,05). Nenhum grupo apresentou mudanças significantes nos níveis séricos de transaminases hepáticas, ureia ou creatinina ou nos respectivos índices hepático e renal (p>0,05), e todos os grupos preveniram (p<0,05) a leucocitose quando comparada ao grupo SAL. Em suma, PNG combinada ou não com ILBI reduziu a infiltração leucocitária e o número de neutrófilos além de reduzir a perda óssea alveolar, à custa da inibição da ativação de osteoclastos, sem causar alterações sistêmicas importantes

Os resultados mostram-se satisfatórios e indicam um futuro promissor para esse recurso terapêutico na Ortodontia, porém na literatura são encontradas divergências quanto às dosimetrias, o que torna difícil estabelecer um protocolo clínico e uma prescrição ideal para cada caso. Considerando a importância desse tema e a necessidade do conhecimento científico atual sobre os protocolos estudados e testados na literatura, este trabalho foi dividido em dois capítulos: O primeiro é uma revisão de literatura, que tem como objetivo analisar os efeitos da terapia laser de baixa intensidade no controle da dor e na movimentação ortodôntica, o outro é um estudo clínico dos efeitos da terapia laser de baixa intensidade na regeneração óssea da sutura palatina mediana após disjunção rápida da maxila. Para a revisão de literatura, foi realizada uma pesquisa na base de dados MEDLINE (1975 – 2014) com os descritores terapia laser de baixa intensidade, dor, movimentação e ortodontia. Foram identificados 65 artigos, dos quais foram selecionados dez, segundo os critérios de inclusão: artigos de estudos clínicos em humanos, que usaram para a terapia laser GaAlAs, e que apresentavam metodologia claramente descrita. Para o estudo clínico, foram selecionados 14 pacientes com idade entre oito e quatorze anos com deficiência transversa da maxila. Os pacientes foram submetidos à disjunção rápida da maxila com aparelho disjuntor de Hyrax, que foi ativado com o seguinte protocolo: uma volta completa após instalação, seguida de duas ativações diárias até ser obtida a sobrecorreção. Posteriormente, por meio de sorteio, os pacientes foram divididos em dois grupos - experimental (n=10) e controle (n=4). O grupo experimental foi submetido a 12 sessões de laser de baixa intensidade do tipo GaAlAs, com potência de 70mW e comprimento de onda na faixa do infravermelho (780 nm). As sessões foram realizadas duas vezes por semana no primeiro mês e uma vez por semana no segundo mês. A dose foi aplicada por dez segundos em quatro pontos equidistantes ao longo da sutura palatina mediana. Duas imagens tomográficas foram realizadas, em ambos os grupos, sendo a primeira (T0) logo após a disjunção e a segunda (T1) após quatro meses de controle. A regeneração óssea da sutura palatina mediana foi avaliada por meio da medição da densidade óptica das imagens tomográficas, usando o programa InVivo Dental 5.0, comparando T0 e T1 intragrupo, assim como comparando o grupo-controle com o experimental, com base no teste T de Student (α=0,05%). Foi observada diferença estatisticamente significante entre as densidades ópticas das tomografias T0 e T1 no grupo experimental (P=0,00), enquanto no grupo-controle essa diferença não foi significante (P=0,20). Quando os grupos foram comparados, maior densidade óptica foi observada no grupo experimental (p=0,05). Os resultados deste trabalho demonstraram que a irradiação laser de baixa intensidade (GaAlAs) pode ser usada como coadjuvante no tratamento ortodôntico, diminuindo a dor após a instalação de artifícios ortodônticos, acelerando a movimentação dentária e a regeneração óssea da sutura palatina mediana, após disjunção maxilar. Mais estudos, porém, são necessários para aprimorar os parâmetros de tratamento.

Different therapies, such as the application of growth factors and low level laser therapy (LLLT) are capable of biostimulating cells by increasing their proliferation and metabolism. The aim of this study was to evaluate: the functions of gingival fibroblasts obtained from young and elderly individuals (Study 1); the response of these cells exposed to tumor necrosis factor alpha - TNF-α (Study 2); and the effect of LLLT or epidermal growth factor - EGF on the viability and metabolism of these connective cell type (Study 3). Primary culture of gingival fibroblasts of 6 patients (3 young - Y and 3 elderly - E) were collected and cultured in complete culture medium (DMEM). In the Study 1 fibroblasts were seeded in 96-well plates in complete DMEM containing 10% of fetal bovine serum (FBS) for 24, 48 or 72 hours and subjected to evaluation of cell viability (MTT, n=36), total protein production (TP, n=36) and collagen synthesis (SR, n=36). In the Study 2, after 24-hour cell seeding, the complete DMEM was replaced by DMEM without FBS supplemented or not with TNF-α (100 ng/mL) which was kept in contact with cells for additional 24 hours. Then, the production of reactive oxygen TNF-α species (ROS, n=36), nitric oxide (NO, n=36) and synthesis of CCL5 chemokine (ELISA, n=36) were assessed. In the Study 3 fibroblasts were seeded in 24-well plates for 24 hours, and then exposed to EGF (100 μM) or submitted to low power laser irradiation (LASERTable device, InGaAsP - 780 ± 3nm; 25mW). Cell viability (Alamar Blue®, n=12), cell migration (Wound Healing, n=12), collagen synthesis (SR, n=12), and synthesis of vascular endothelial growth factor - VEGF (ELISA, n=12) were assessed 72 hours after the treatments. Na busca de acelerar o reparo tendíneo, a terapia a laser de baixa intensidade (TLBI), vem demonstrando efeitos satisfatórios. Objetivo: Avaliar a ação das diferentes potências da terapia a laser de baixa intensidade no reparo tendíneo, em ratos. Métodos: O laser utilizado foi (AsGaAl) 830nm, densidade de energia 30J/cm2 e potências de 40mW, 60mW, 80mW e 100mW, por um período de 5 dias consecutivos para todos os grupos. Todos os animais foram submetidos à lesão parcial do tendão calcâneo. No 6º dia após lesão, os tendões foram retirados e analisados por microscópio de polarização. Foi mensurada a organização das fibras de colágeno por meio da análise de birrefringência e verificada a predominância do tipo de colágeno no tendão calcâneo através da análise de picrosirius. Resultados: Os grupos tratados com TLBI mostraram uma melhora significante no realinhamento das fibras de colágeno, mas não apresentaram diferença significante entre si (p=0,139). Quanto à avaliação da quantidade de colágeno tipo III, foi observado que houve um aumento do percentual de colágeno, conforme o aumento da potência do laser (60 a 100mW) (p<0,01). Em relação ao colágeno tipo I, o grupo 80mW apresentou maior quantidade do que o grupo sham (p=0,05). Conclusão: A terapia a laser de baixa intensidade não apresentou diferença no realinhamento das fibras de colágenos entre diferentes potências. Potências a partir de 60mW aumentaram a quantidade de colágeno tipo III. Potência de 80mW aumentou a quantidade de colágeno tipo I e III. / Introduction: Calcaneous tendon is one of the most damaged tendons, and its healing may last from weeks to months to be completed. In the search after speeding tendon repair, low intensity laser therapy has shown favorable effect. Objective: To assess the effect of 40, 60, 80 and 100mW powers of low level laser on the process of tissue repair in partial lesion calcaneous tendon in rats. Methods: The was used 830-nm GaAlAs diode laser operating at output powers of 40, 60, 80 and 100 mW and energy density of 30 J/cm2 on the repair of partial calcaneal tendon ruptures in rats. A partial tendon rupture was induced in all animals, which were treated with laser irradiation for 5 consecutive days. Six days after injury, the injured tendons were removed and examined by polarized light microscopy. Results: Collagen fiber organization was evaluated by birefringence measurements, and collagen content was determined by picrosirius red staining. It was observed that the higher the output power (60-100 mW) the greater was the amount of type III collagen (P < 0.01). The amount of type I collagen was significantly greater (P = 0.05) in the 80-mW group than in the control group (sham stimulation). A non-statistically significant improvement in the realignment of collagen fibers was observed in the irradiated groups. Conclusion: Low-level laser therapy resulted in significantly greater amounts of type III collagen (output powers of 60 mW or more) and type I collagen (output power of 80 mW), however, no significant differences between groups were found in the realignment of collagen fibers. / TEDE / BV UNIFESP: Teses e dissertações

Assim, foi objetivo deste estudo, avaliar por meio de análise histomorfométrica e imunoistoquímica, a ação do laser em baixa intensidade (LLLT) sobre o processo de reparo após reimplante dentário em ratos. Sessenta ratos (Rattus norvegicus albinus, Wistar) tiveram seus incisivos superiores direitos extraídos e foram aleatoriamente divididos em seis grupos: C0, C30 e C45 considerados controles, onde o reimplante dentário foi realizado imediatamente, trinta e quarenta e cinco minutos após a exodontia respectivamente, sem nenhum tratamento do ligamento periodontal remanescente e L0, L30 e L45, onde o reimplante foi realizado nos mesmos tempos extra-alveolares dos controles, porém a superfície radicular e alvéolo, com remanescentes do ligamento periodontal, foram tratados com LLLT (Arseneto de Gálio e Alumínio) antes do reimplante. Os animais foram eutanasiados após 60 dias. Os resultados evidenciaram reabsorções radiculares externas por substituição e inflamatória em todos os grupos, não apresentando diferença significativa entre o grupo controle e tratado em cada período de tempo extra-alveolar (P>0,05). A anquilose, porém, foi maior no grupo L30 em comparação com o C30 (P<0,05). Na análise imunoistoquímica houve predominância da proteína RANKL sobre a RANK e OPG no reimplante imediato (p<0,05). No período extra-alveolar de 45 minutos houve predominância da RANK sobre a RANKL (p<0,05). A proteína TRAP foi predominante nos grupos tratados com LLLT no reimplante imediato e com 30 minutos de tempo extra-alveolar (p<0,05). Esses resultados levaram a concluir que o tratamento da superfície radicular e alvéolo com LLLT, nas... / The success of the replanted tooth, nowadays, still tends to be limited because great amount of teeth are lost because of progressive root resorption, justifying the search of new strategies that can control this problem. This study investigates by histomorphometric and immunohistochemical analysis the of low-level laser terapy (LLLT) influence on in the control of root resorption on the healing process after tooth replantation. Sixty Wistar rats were submitted to extraction of their upper right incisor and randomly were allocated to one of six experimental groups. The groups: C0, C30 and C45 had been considered controls, where the teeth were replanted immediately, thirty and forty five minutes after the tooth extraction respectively, without any treatment of the remaining periodontal ligament. In the groups L0, L30 and L45, the teeth were replanted immediately, thirty and forty five minutes after tooth extraction respectively; however the root surface and dental sockets with remainders of the periodontal ligament had been treated with LLLT (gallium-aluminium-arsinide). The animals were euthanasied after 60 days. The comparative analysis did not reveal statistically significant differences in external root resorption (P>0.05) between the treatment modalities when used for tooth replantation in each extra-alveolar period, but anchylosis was bigger in L30 (P<0.05) comparing to C30. Immunohistochemical analysis showed predominant expression of RANKL on RANK and OPG in the immediate replant (p<0.05). At 45 minutes extra-alveolar time, there was predominant expression of RANKL on RANK (p<0.05). TRAP protein showed predominant expression in the LLLT groups in immediate replant and 30 minutes extra-alveolar time (P<0.05). Entretanto, para a sobrevivência do enxerto, é necessário que o leito da ferida esteja saudável e com a presença de um tecido de granulação saudável. Objetivou-se com este trabalho foi comparar diferentes ondas do laser de baixa intensidade e do óleo de girassol ozonizado como potenciais estimulantes da cicatrização em enxertos aplicados em feridas sem tecido de granulação em coelhos. Para isso, foram elaborados dois experimentos onde o primeiro comparou a laser de baixa intensidade (LBI) de onda contínua com a LBI pulsada (Capítulo 2), no qual 31 coelhos foram distribuídos em três grupos, grupo controle (GC), grupo laser onda contínua (GLC) e grupo laser onda pulsada (GLP). O segundo experimento comparou a LBI de onda contínua com a aplicação tópica de óleo de girassol ozonizado (OGO) (Capítulo 3) e 32 coelhos também foram distribuídos em três grupos, sendo grupo controle (GC), grupo laser (GL) e grupo ozônio (GO3). No primeiro experimento, os grupos GLC e GLP apresentaram evolução semelhante, com processo cicatricial satisfatório do enxerto em ambos, ao contrário do grupo GC. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Cutaneous grafting is a simple and useful surgical technique for the repair of wounds, especially in which there is a difficulty in applying the primary closure, or other reconstructive technique. However, for graft survival, it is necessary a healthy granulation tissue in the wound bed. The aim of this study was to compare differents waves of low level laser therapy (LLLT) and ozonated sunflower oil as potential healing stimulators in grafts applied rabbits in fresh wounds. Two trials were carried out in which the first one compared LLLT continuous wave with LLLT pulsed (Chapter 2) in 31 rabbits divided into three groups: control group (CG), continuous wave laser group (GLC) and pulsed wave laser group (GLP). The second trial compared LLLT continuous with ozonized sunflower oil (OGO) (Chapter 3), and 32 rabbits were also divided into three groups, being control group (GC), laser group (GL) and ozone group (GO3). In the first trial, the GLC and GLP groups showed similar evolution, with a satisfactory healing process in both, unlike the CG group. These results were maintained in the microscopic evaluation since the GC showed a significant correlation with criteria related to graft failure. Uso do laser infravermelho em episiotomia: ensaio clínico aleatorizado / Use of infrared laser in episiotomy: a randomized controlled trial

Introdução: A episiotomia é uma ampliação cirúrgica do períneo amplamente utilizada na assistência ao parto, embora seu emprego rotineiro não seja justificado pelas evidências científicas. Está associada à dor e ao desconforto no período pós-parto. O Laser em Baixa Intensidade (LBI) vem se destacando na literatura como uma tecnologia promissora em relação ao tratamento de feridas. Apresenta efeitos de redução da dor, inflamação e estímulo à cicatrização. Objetivo: Avaliar os efeitos do laser em baixa intensidade na cicatrização da região perineal, na frequência e magnitude da dor perineal, após a episiotomia médio-lateral direita. Método: Ensaio clínico aleatorizado, paralelo e triplo cego, com uma amostra de 54 puérperas, divididas em grupo experimental (recebeu irradiação de laser) e controle (recebeu simulação de irradiação). As puérperas foram incluídas no estudo entre 6 e 10 horas após o parto no Alojamento Conjunto do Hospital Universitário da Universidade de São Paulo. Os critérios de inclusão foram: idade 18 anos, idade gestacional 37 e <42 semanas; sem parto vaginal anterior; parto espontâneo de feto único, vivo e em apresentação cefálica, com episiotomia médio-lateral direita; ausência de processo infeccioso, hemorroidas, hematomas ou varizes na região vulvoperineal; não ter realizado preparo da região perineal na gravidez; não ter feito uso de drogas fotossensibilizantes e sem intercorrências clínicas ou obstétricas. Foram excluídas do estudo mulheres que utilizaram qualquer produto diferente de água e sabão na região vulvoperineal. A intervenção com o laser consistiu em três irradiações (primeira: de 6 a 10 horas após o parto, segunda: 20 a 24 horas e terceira: 40 a 48 horas após a primeira aplicação), com laser diodo infravermelho, meio ativo semicondutor Gallium-Aluminum-Arsenide (GaAIAs), tamanho do spot de 0,04 cm2, densidade de energia de 5J/cm2, potência de 20 mW, duração da irradiação de 10 segundos por ponto. Em cada sessão, a episiotomia foi irradiada em nove pontos diferentes, com 0,2J por ponto e energia total de 1,8J por sessão. Na simulação da irradiação, a ponteira que emite o laser vermelho foi modificada pelo próprio fabricante, que substituiu o laser por uma luz guia. Em ambos os grupos, a cicatrização perineal foi avaliada em quatro momentos: antes das três irradiações e 7 a 10 dias após a alta hospitalar, por meio da escala Redness Edema Echymosis Discharge Aproximation (REEDA). A avaliação da dor perineal foi feita em sete ocasiões: antes e após as três irradiações e 7 a 10 dias, após a alta hospitalar pelo questionamento de presença ou ausência de dor e pela escala numérica de 0 a 10. A coleta de dados foi realizada entre junho e outubro de 2011. Resultados: Foram randomizadas 54 mulheres (29 no grupo experimental e 25 no grupo controle). Houve perda de seguimento de 11 mulheres na última avaliação (7 a 10 dias). Os grupos foram semelhantes quanto às variáveis: idade materna em anos completos; Índice de Massa Corporal; idade gestacional; peso do recém-nascido em gramas; Apgar de 1º, 5º e 10º minutos; perímetro cefálico em centímetros; extensão da episiotomia em centímetros; número de gestações, partos e abortos; cor; escolaridade; profissão; estado marital; presença de acompanhante; número de doses analgésicas e intervalo entre a ingestão do analgésico e a avaliação. A anestesia raquidiana foi usada com maior frequência no grupo controle (p=0,043). Quanto à cicatrização, os grupos não diferiram na escala REEDA em nenhuma das avaliações. Quanto às médias de dor perineal, os grupos diferiram nas seguintes ocasiões: o experimental apresentou maiores médias de dor na avaliação antes (grupo experimental 4,5; grupo controle 2,0; p=0,002), após a primeira irradiação (grupo experimental 4,1; grupo controle 2,0; p=0,008), e após a terceira irradiação (grupo experimental 1,5; grupo controle 0,6; p=0,019). Quanto à presença de dor, não houve diferença estatisticamente significante entre os grupos nos distintos momentos de avaliação. Não houve diferenças estatisticamente significantes entre os dois grupos entre a redução média de dor antes e após a irradiação laser, tanto em porcentagem quanto em relação a uma melhora <30% e 30%, nos três momentos em que a intervenção foi realizada. O procedimento foi considerado muito bom por 44,4% das mulheres, bom por 53,4% delas e ruim por 2,2% delas; 95,6% delas referiram o fariam novamente. Conclusão: O uso de LBI não teve efeito na cicatrização ou na frequência e na magnitude da dor perineal em mulheres com episiotomia, após o parto vaginal espontâneo. / Introduction: An episiotomy is a surgical widening of the perineum largely used in the childbirth, despite the fact that its routine use has not been justified by scientific evidence. It is associated with pain and discomfort in the postpartum period. The Low-Level Laser Therapy (LLLT) has been pointed out in the literature as a promising technology for the treatment of wounds. It has the effects of reducing pain and inflammation and healing stimulation. Objective: Evaluate the effects of low intensity laser therapy in the healing of the perineal region and in the frequency and magnitude of perineal pain, after right mediolateral episiotomy. Method: Parallel, triple blind randomized clinical trial, with a sample of 54 mothers who were divided into experimental (received laser irradiation) and control group (received simulated radiation). Postpartum women were included in the study from 6 to 10 hours after birth in the Rooming-in Unit of the Hospital of the University of São Paulo. Inclusion criteria were: age 18 years, gestational age 37 and <42 weeks, no previous vaginal delivery, to have a spontaneous delivery of a singleton fetus in cephalic presentation with right mediolateral episiotomy, absence of infection, hemorrhoids, bruises or varicose veins in the vulvoperineal region; no perineum preparation during pregnancy, no use of photosensitizing drugs and no clinical or obstetric complications. We excluded women who had used any product other than soap and water in the vulvoperineal region. The LLLT intervention consisted of three irradiations (first: from 6 to 10 hours after birth, second: from 20 to 24 hours after birth and third one from 40 to 48 hours after the first application), using infrared diode laser, with a semiconductor active medium Aluminum-Gallium-arsenide (GaAlAs), a size spot of 0.04 cm, 2 5J/cm2 energy density, power of 20 mW, and length of irradiation of 10 seconds per point. In each session, the episiotomy was irradiated in nine different points, with a total of 0.2 J per point and a total energy of 1.8 J per session. To simulate the irradiation, the tip that emits the red laser was modified by the manufacturer, who replaced the infrared laser by a guiding light in the same pen that emits the laser. In both groups, the perineal wound healing was assessed at four time moments: before the three irradiations and from 7 to 10 days after hospital discharge, through the scale Echymosis Discharge Aproximation Redness Edema (REEDA). The perineal pain was assessed in seven occasions: before and after the three sessions of irradiation and in one occasion from 7-10 days after birth, using numerical scale from 0 to 10 and questioning the woman on the presence or absence of pain. Data collection was carried out between June and October 2011. Results: We randomized 54 women (29 in the experimental group and 25 in the control group). Eleven women were lost in the follow-up, in the last evaluation (7-10 days). Both groups were similar with regard to the variables: maternal age in years, body mass index, gestational age, weight of the newborn in grams; Apgar score at 1, 5 and 10 minutes and head circumference in centimeters; episiotomy length in centimeters, number of pregnancies, births and miscarriages, skin color, education, profession, marital status, presence of caregiver, the number of analgesic doses and interval between the intake of analgesics and evaluation. The spinal anesthesia was more frequently used in the control group (p = 0.043). Regarding the healing, the groups did not differ in any assessments of the REEDA scale. Regarding the means of perineal pain, the groups differed on the following occasions: the experimental group had higher means of pain scores in the evaluation before (experimental group 4.5, control group 2.0, p = 0.002), and after the first irradiation (experimental group 4.1, control group 2.0, p = 0.008), and after the third irradiation (experimental group 1.5, control group 0.6, p = 0.019). Regarding the presence of pain, there was no statistically significant difference between groups in different stages of evaluation. There was no statistically significant differences between the two groups, both regarding the mean reduction of pain before and after laser irradiation in percentage and regarding an improvement <30% and 30% in the three moments when the intervention was performed. The procedure was evaluated as very good by 44.4% of women, as good by 53.4% and as bad by 2.2% of them; 95.6% of women who had the procedure reported that would have it again. Conclusion: The use of LLLT had no effect on wound healing or in the frequency and magnitude of perineal pain in women with episiotomies after spontaneous vaginal delivery, with the dosage and number of sessions used in this study.
