Dessa forma, buscamos analisar a atuação da TO nesse âmbito, algumas questões do espaço escolar, além de compreender histórias e experiências de adolescentes em situações de dificuldade de aprendizagem. Para, assim, identificar possíveis contribuições da profissão, com base nos pressupostos da Teoria da Subjetividade, no contexto da aprendizagem. Esta pesquisa tem como referencial metodológico a Epistemologia Qualitativa de González Rey (2002, 2005a, 2005b), assim as informações foram analisadas a partir de um processo construtivo interpretativo. O campo empírico ocorreu em dois contextos: um espaço terapêutico da TO pertencente à Secretaria de Saúde do DF, no qual foi observado um grupo para adolescentes com TDAH (Transtorno do Déficit de Atenção e Hiperatividade); e uma escola de ensino fundamental da rede pública de ensino, frequentada por todos os participantes do grupo. A partir desses ambientes, os colaboradores da pesquisa foram: uma terapeuta ocupacional, duas professoras e os adolescentes participantes das sessões grupais, dos quais três foram escolhidos ao longo da pesquisa para realização de estudos de casos. As relações observadas entre os contextos e atuações profissionais, assim como a compreensão da dinâmica subjetiva dos adolescentes observada nos estudos de casos, permitiu-nos refletir sobre a existência do transtorno orgânico e suas implicações nas dificuldades de aprendizagem. Percebemos que as principais barreiras no contexto da aprendizagem e desenvolvimento implicavam aspectos do contexto e história de vida dos adolescentes, assim como relações sociais estabelecidas e a construção de ambientes favoráveis para que eles pudessem se posicionar. Observamos que a principal contribuição da prática em TO ocorreu a partir da concretização de um espaço que possibilitou a participação e comunicação dos adolescentes. No contexto escolar, percebemos as implicações das relações sociais e dos espaços de comunicação estabelecidos entre professor-aluno no processo de aprendizagem. Dessa forma, refletimos que os comportamentos expressos pelos alunos estavam além de uma relação direta com as características orgânicas do transtorno, mas indicavam a manifestação de uma dinâmica subjetiva complexa, com implicações de diversas questões sociais e familiares, demonstrando uma necessidade de espaços e relações sociais favoráveis para que eles pudessem se posicionar. Portanto, consideramos que as principais ações da TO, com base na Teoria da Subjetividade, consistem em promover ambientes que valorizem a emergência do sujeito, a partir de ações e instrumentos que estimulem processos de desenvolvimento subjetivo, tendo as relações sociais um destaque nesse processo. A relevância dessa pesquisa foi a possibilidade de contribuir para a formação em TO, principalmente em relação ao contexto escolar, a partir de articulações entre campos teórico-práticos ainda não explorados, conforme os pressupostos da Teoria da Subjetividade. / This work depicts about the implications of the theory of the subjectivity of González Rey for the practice of occupational therapy (OT) in the educational context, based on the understanding of the subjective aspects of students who face situations of learning difficulties. In this way, we seek to analyze the performance of to in this context; some issues of school space, as well as understand stories and experiences of adolescents in situations of learning difficulties. So, identify possible contributions of the profession, based on the assumptions of the theory of subjectivity, to the context of learning. This research has as methodological Epistemology of González Rey (2002, 2005a, 2005b), so the information was analyzed from a constructive process. The Empirical field occurred in two contexts: a space of OT belonging to the Department of Health of the DF, which was an observed group for adolescents with ADHD (Attention Deficit Hyperactivity Disorder); and in a primary school of public education, attended by all the members of the group. From these environments, employees of the research were: an occupational therapist, two teachers and young participants of group sessions, three of which were chosen throughout the research to conduct case studies. The relationship observed between the contexts and professional performances, as well as the understanding of the dynamics of subjective adolescents of case studies allowed us to reflect on the existence of an organic disorder and its implications in learning difficulties. We realized that the main barriers in the context of learning and development were concerned with aspects of the context and history of life of adolescents, as well as in social relations established and in the construction of favorable environments so they could position themselves. We observed that the main contribution of the practice OT occurs from the implementation of a space that allowed the participation and communication of adolescents. In the school context we realize the implications of social relations and the spaces of communication established between teacher-student in the learning process. In this way, we reflected that the behaviors expressed by students, were beyond a direct relationship with the characteristics of organic disorder, but indicated the manifestation of a subjective dynamic complex, with implications of various social issues and family, demonstrating a need for spaces and social relations conducive so that they could be positioned. Therefore, we consider that the main actions of OT, based on the theory of subjectivity, are to promote environments that enhance the emergence of the subject, from actions and instruments that stimulate the development processes subjective, having the social relations a highlight in this process. The relevance of this research was to contribute to the formation at OT, especially in relation to the school context, from joints between fields theoretical-practical not yet explored, as the assumptions of the Theory of Subjectivity.

La especialidad de Terapia Ocupacional tiene como finalidad el desarrollo y mantenimiento de la salud a través de la ocupación, entendida esta desde el área de actividades de la vida diaria (AVD), de la productividad y del tiempo libre. Se contempla las diferentes situaciones que ocurren a lo largo de la vida, y se trabaja al respecto con las capacidades, deseos e intereses que traen los adultos mayores. Desde esta entrada profesional, se creó en la Universidad Nacional de Quilmes, varios talleres: Taller de Narración Oral, Taller de Movimiento, Taller de Danza terapia, Taller de Dibujo, Taller Literario, Taller de Radio, Taller de Coro, Taller para Estimular la Memoria.

InfluÃncia da mecÃnica respiratÃria sobre a assincronia paciente-ventilador, na ventilaÃÃo com pressÃo de suporte, com e sem sistema de disparo e ciclagem automÃticos, e na ventilaÃÃo assistida proporcional / Influence of respiratory mechanics on patient-ventilator asynchrony in pressure support ventilation with and without automatic triggering and cycling system and proportional assist ventilation

ContextualizaÃÃo: A obtenÃÃo de uma boa sincronia paciente-ventilador consiste em um dos maiores desafios no manejo da ventilaÃÃo mecÃnica (VM). A ventilaÃÃo com pressÃo de suporte ou pressure support ventilation (PSV) à uma modalidade ventilatÃria amplamente utilizada no processo de desmame da VM. A ventilaÃÃo assistida proporcional ou proportional assist ventilation (PAV) à uma modalidade de suporte ventilatÃrio onde o ventilador gera assistÃncia proporcional e instantÃnea aos esforÃos do paciente. O Auto-Trak digital consiste em uma tecnologia capaz de ajustar automaticamente, ciclo a ciclo, os mecanismos de disparo e ciclagem durante o modo PSV. Objetivos: Determinar a influÃncia da mecÃnica respiratÃria sobre a assincronia paciente-ventilador durante os modos PSV, com e sem sistema de disparo e ciclagem automÃticos e na PAV, em modelo pulmonar mecÃnico e identificar padrÃes nas curvas de ventilaÃÃo apresentadas na tela do ventilador que sejam relacionadas aos tipos de assincronia investigados. MÃtodos: Trata-se de estudo experimental, de bancada utilizando o simulador pulmonar mecÃnico, ASL 5000Â. Estudaram-se trÃs perfis de mecÃnica respiratÃria: normal, obstrutivo e restritivo, com variaÃÃo do tempo inspiratÃrio neural 0,5, 1,0, 1,5 e 2,0 segundos, sendo a intensidade mÃxima do esforÃo muscular (Pmus) fixada em -7.5 cmH2O, durante a VM nos modos PSV e PAV, em cinco ventiladores de UTI, de circuito duplo, e um ventilador de circuito Ãnico. O Auto-Trak foi estudado quando disponÃvel no ventilador. Os desfechos primÃrios foram: tempo de retardo inspiratÃrio e tempo de assincronia de ciclagem identificando, neste segundo caso, dois tipos possÃveis, ciclagem tardia ou precoce. AlÃm disso, procedeu-se a uma anÃlise por inspeÃÃo visual comparativa entre as curvas de mecÃnica: fluxo, VC e Pmus do ASL 5000 e as curvas na tela grÃfica do ventilador mecÃnico de VC, fluxo e pressÃo x tempo, na tentativa de se identificar padrÃes associados à assincronia, que fossem passÃveis de identificaÃÃo pela simples observaÃÃo na tela do ventilador pulmonar. Resultados: Houve marcante influÃncia da mecÃnica respiratÃria sobre a assincronia paciente-ventilador. O tempo de retardo inspiratÃrio foi maior e clinicamente significativo (> 100 ms) no perfil obstrutivo de mecÃnica respiratÃria, e foi menor, muitas vezes zero, no ventilador de circuito Ãnico; a assincronia de ciclagem foi comum no perfil obstrutivo, sendo predominantemente do tipo ciclagem tardia, enquanto no perfil restritivo predominou o tipo ciclagem precoce. O emprego do Auto-trak eliminou a ocorrÃncia de assincronia do tipo auto-disparo no ventilador de circuito Ãnico. A anÃlise visual das curvas detectou padrÃes de traÃados da curva de fluxo x tempo que sÃo caracterÃsticos de assincronia do tipo ciclagem precoce e ciclagem tardia e passÃveis de identificaÃÃo por inspeÃÃo visual direta na tela do ventilador. ConclusÃo: as assincronias de disparo e ciclagem entre o paciente e o ventilador sÃo a regra e nÃo a exceÃÃo durante os modos PSV e PAV, sendo estas influenciadas pela mecÃnica respiratÃria. O emprego do sistema Auto-trakÂ, mostrou benefÃcio durante o uso do ventilador de circuito Ãnico com melhora substancial do disparo. A inspeÃÃo visual da curva de fluxo na tela do ventilador pode favorecer a identificaÃÃo destes tipos de assincronia. Palavras-chave: / Obtaining a good patient-ventilator synchrony is one of the biggest challenges in the management of mechanical ventilation (MV). Pressure support ventilation (PSV) is a ventilator mode widely used in the MV weaning process. Proportional assist ventilation (PAV) is a mode of ventilator support in which the ventilator generates assistance proportional and instant to the efforts of the patient. Digital Auto-TrakTM consists in a technology capable of automatically adjusting, cycle by cycle, the mechanisms of triggering and cycling in PSV mode. Objectives: To determine the influence of respiratory mechanics on patient-ventilator asynchrony during PSV mode, with and without automatic triggering and cycling system, and PAV, in a mechanic lung model and to identify patterns on ventilation curves presented on the ventilator screen, which are related to the types of asynchrony investigated. Methods: This is an experimental bench study using the mechanic lung model, ASL 5000TM. Three profiles of respiratory mechanics were studied: normal, obstructive and restrictive, with variation of neural inspiratory time of 0.5, 1.0, 1.5 and 2.0 seconds, with maximum intensity of muscle effort (Pmus) fixed in -7.5 cmH2O, during MV in PSV and PAV modes, in five ICU ventilators, with double limb, and in one single limb ventilator. Auto-TrakTM was studied when avaliable in the ventilator. Primary outcomes were: inspiratory delay time and cycling asynchrony time identifying, in the second case, two possible types, late or premature cycling. Furthermore, we proceeded to an analysis by visual inspection of the: flow, VT and Pmus curves of ASL 5000TM and the: VT, flow and pressure curves on the ventilator screen in an attempt to identify patterns associated to asynchrony that would be identified through simple observation. Results: There was a marked influence of respiratory mechanics on patient-ventilator asynchrony. The inspiratory delay time was higher and clinically significant in the obstructive profile of respiratory mechanics, and lower, many times âzeroâ, with the single limb ventilator. Cycling asynchrony was common in the obstructive profile, predominantly the late cycling type, while in the restrictive profile, the premature cycling type predominated. The use of Auto-TrakTM system eliminated the occurrence of auto triggering asynchrony type in the single limb ventilator. Visual analysis of the curves detected patterns of flow x time curves that are characteristic of premature and late cycling asynchrony types and which can be identified by direct visual inspection of the ventilator screen. Conclusion: Triggering and cycling asynchronies between the patient and the ventilator are the rule rather than the exception during PSV and PAV modes, which are influenced by respiratory mechanics. The use of Auto-TrakTM system showed benefit during the use of the single limb ventilator, with substantial improvement of the triggering. Visual inspection of the flow curve on the ventilator screen may favor the identification of these types of asynchrony.

Posteriormente, ressaltaram-se alguns fragmentos do material clínico, para estudo da dinâmica de funcionamento do grupo. Por fim, na busca de uma contribuição ao estudo da atuação da terapia ocupacional na área de saúde mental elaboraram-se algumas reflexões e considerações referentes aos pressupostos teóricos utilizados para trabalhar com o grupo estudado / Abstract: This paper discuss some possibilities of actuation of occupational therapy psycodynamic approach - to a working group of psychotic patients. To accomplish this, systematic observations of the working group were conducted in a Psycho-social Treatment Center at Campinas - SP. As an introduction, several historical aspects were considered, mainly concemed to mental health and occupational therapy. In a second step, it was focused the method utilized to work with clinical material obtained from the observations. A characterization of the Psycho-social Treatment Center was performed, with respect to the technical team (staff), installations (physical area), working dynamics and general assistance developed. Several fragments of clinical material were selected and analyzed for the study of group dynamics. Para isso, são tomados como referência básica o símbolo em Cassirer, Freud e Ricoeur. É uma investigação epistemológica que parte de uma série de dúvidas sobre o exercício da terapia ocupacional: o que faz o terapeuta ocupacional? O que fala e do que fala o terapeuta ocupacional? As respostas para essas questões são encontradas nas teorías sobre o símbolo em Cassirer, Freud e Ricoeur, juntamente com o esforço para definir o tipo de ciência que é a terapia ocupacional. Em Cassirer o símbolo encontrado é racional, abstrato, uma representação, dirigindo a terapia ocupacional para as ciências formais. Em Freud o símbolo é empírico, psicológiCD, uma expressão. Encaminha a terapia ocupacional para as ciências empíricas. Por sua vez, o símbolo de Ricoeur é múltiplo e equívoco, fala da existência e oferece para a terapia ocupacional um caminho em direção às ciências humanas. O que resulta dísso é uma tripla fundamentação, aparentemente irreconciliáveis, e que, exatamente por ser múltipla, permite à terapia ocupacional dirigir-se a investigação da humanidade do homem, constituindo-se, portanto, numa ciência humana. Uma ciência que, procedendo através de uma interpretação, irá buscar compreender os múltiplos sentidos do homem, presentes objetificados - em seus fazeres, em suas atividades, em suas obras, enfim, nos símbolos. Uma ciência que tem como sujeito e objeto de seu conhecimento, o homem. Um homem que não é o homem natural, mas sim o homem que transforma a natureza em humanidade e porque é um homem que faz, que ao fazer simboliza e se objetiva e, com isso, torna-se ser de sua existência / Abstract: This research shows a discussion which objective is to understand the scientific character of occupational therapy from the concept of symbol. For such , the symbol in Cassirer, Freud and Ricouer are taken as basic reference. It is an epystemologic investigation that starts from a series of questions about the performance of occupational therapy: What does the occupational therapist do ? What and what about does the occupational therapists talk ? And What will the answers find, to these questions, in the theories about symbol in Cassirer, Freud and Ricouer, that bonds to the effort to define the kind of science occupational therapy is. In Cassirer the symbol that is found is a rational, abstract one, a representation, and that tums occupational therapy to the formal sciences. In Freud the symbol is empiric, psychological, an expression, and tums the occupational therapy to the empirical sciences. In a term, the symbol the symbol in Ricouer is multiple ' and equivocal, deals with the existence and shows the way of Humanistics to Occupational Therapy. The result is a triple fundamentation, apparently irreconcilables, but that, only because of being multiple, permits to occupational therapy to investigate the humanity of man, making itself therefore a human science. A science that proceeding through an interpretation will search for understanding the mltltiple meanings of man, that apparently are present - objectified - in its doings, in its activities, its opus, finally in its symbols. Os resultados revelaram maior propoção de portadores de visão subnormal (68,0%); leve predomínio do sexo masculino (57,0%), idade média de 32,2 anos; a maioria residente na cidade de Campinas (56,0%) e apresentando reduzido nível sócio-econômico. Adquiriram a deficiência visual após o nascimento 68,0% sendo a maioria na vida adulta (70,60/0). As causas mais freqüe!ltes de deficiência visual foram atrofia do nervo óptico, retinose pigmentar e catarata congênita. A maior proporção de encaminhamentos foi realizada pelo serviço de visão subnormal da Unicamp (48,0%). A maioria dos clientes mostrou conhecer o processo de reabilitação apresentando expectativas referentes ao trabalho desenvolvido no CEPRE. As opiniões em relação às limitações (68,0%) e dificuldades (88,0%) devidas à deficiência visual referem-se, principalmente, às relacionadas à locomoção. A aceitação do problema visual foi o fator mais apontado (89,5%) como mudança decorrente da participação no grupo de espera (atendimento interdisciplinar introdutório à reabilitação, em grupo, de início imediato após procura do serviço). Sugere-se que a área de reabilitação deva promover estudos da prevalência e das causas de deficiência visual na população adulta brasileira; prevenção de sua ocorrência; e efetivação de programas de reabilitação para atendimento imediato de portadores de deficiência visual / Abstract: The objective of the study of this work considered the characteristics of a clientele bearer of visual deficiency, their knowledge and opinion about the rehabilitation process performed at the Center of Studies and Research on Rehabilitation "Prof. Dr. Gabriel O. S. Porto" (CEPRE) at the School of Medical Science of the Campinas State University. We intended to contribute to the improvement on the dynamics of rehabilitation care at CEPRE and in similar services interested in the rehabilitation process of the bearers of visual deficiency. The sample constituted of 25 persons over the age of 12, bearing congenital or acquired visual deficiency, attending a rehabilitation process in the CEPRE during the period from 1994 to 1997. The results showed a majority of bearer of low vision (68,0%); a slight predominance of the male sex (57,0%), with an average age of 32.2 years, the majority residents of the city of Campinas (56,0%), with a low scholar and occupational leveI. Acquired the visual deficiency after birth (68,0%) being the majority in adulthood (70,6%). The more frequent courses were the atrophy of the optic nerve, pigmental retinose, and congenital cataract. The great majority of persons were directed to CEPRE by the Low Vision Service of UNICAMP (48,0%). The majority of the clients demonstrated to know about the rehabilitation service, presenting expectations relative to the work developed in the CEPRE. The opinions relative to the limitations (68,0%) and difficulties (88,0%) due to visual deficiency were, mainJ~ about locomotion. Among the opinions issued, related to the changes resulting from the participation in the caring of the waiting group (Interdisciplinary intervention, in group, withimmediate start after service search), there was a predominance in the acceptance of the visual deficiency (89,5%). We emphasize the possibilities of consciousness enlargement this use shows, that v/e consider essential for the psychoterapic intervention process. We conclude that, at the present stage of activity using as a therapeutical resource, we can’t discard totally any of the contributions that had been developed up to now, because if some of them offer to us fundament abour activity advantages as organizing and socializing making, other offer to us fundament about its effects on the psyche in terms of unconsciousness contents integration to consciousness: the repressed aspects, the introjected roles, the feelings and ways of dealing with the world (in its intentionally), the aspects of personal unconsciousness and the collective unconsciousness, not necessarily repressed, but as potentialities of being. / O objetivo desta pesquisa é o de descrever as bases teóricas que fundamentam a utilização da atividade em Psiquiatria e na Psicologia Dinâmica como o recurso terapêutico. Damos ênfase as possibilidades que esta utilização apresenta na ampliação da consciência, que consideramos como essencial no processo de intervenção psicoterápica. Em psiquiatria, abordamos a evolução da compreensão do papel da atividade no processo terapêutico e relacionamos tal compreensão às transformações do conceito de doença mental que foram concomitantemente tendo lugar nesta área. Para tanto, descrevemos duas linhas de trabalho especificamente vinculadas à questão da atividade: a Terapia Ocupacional e a Arte terapia. Abordamos, também, as diferentes formas de utilização da atividade como recurso terapêutico na Psicologia Dinâmica. Procuramos identificar em cada uma das teorias estudadas, - na Psicanálise, no Psicodrama, na Gestalt Terapia e na psicologia Analítica - a forma de utilização da atividade, sempre relacionando a à compreensão de ser humano e à compreensão dos objetivos do processo terapêutico por elas explicitados. Concluímos que no estágio atual da utilização da atividade como recurso psicoterápico, não podemos descartar em totalidade nenhuma das contribuições desenvolvidas até o momento, pois se algumas nos oferecem fundamentação sobre as vantagens da atividade enquanto fazer organizador e socializante, outras nos oferecem fundamentação sobre seus efeitos no psiquismo em termos de integração de conteúdos inconscientes à consciência: dos aspectos recalcados, dos papéis introjetados, dos sentimentos e formas de lidar com o mundo (em sua intencional idade), dos aspectos do inconsciente pessoal e do inconsciente coletivo não necessariamente recalcados, mas em forma de potencialidades de ser.

Na coletividade constitui-se nwn grave problema de Saúde Pública, principalmente nas mulheres, e preocupação para os profissionais do campo da Saúde Mental em geral e da Terapia Ocupacional em particular. Embora a literatura reconheça de modo unânime que o fenômeno da depressão é indissociável da experiência de vida do sujeito e das formas em que ocorre a sua inserção na cultura e na sociedade, nas múltiplas respostas apresentadas pela ciência ao problema predominam aquelas fragmentadas em disciplinas. Esta pesquisa objetiva contribuir para ampliar esta limitação teórico-metodológica na compreensão do fenômeno da depressão e da pessoa que se deprime. Para tanto, focalizamos o fenômeno da depressão com abordagem multidisciplinar de interface entre as Ciências Biológicas e Humanas, privilegiando a experiência do sujeito através das suas representações sociais. Este trabalho estuda as manifestações do sofrimento psíquico relacionado com a depressão no contexto do cotidiano do indivíduo, em uma população de 25 pessoas adultas, usuários de wn serviço de ambulatório de Terapia Ocupacional, com queixa e ou diagnóstico de depressão. Através da aplicação de questionário, entrevista aberta, entrevista semi-estruturada e trabalho de campo realizado durante o período de dois anos, analisa particularmente os aspectos socioculturais relativos à vulnerabilidade social relacionada à depressão. Ênfase especial foi dada aos diferentes processos que estão presentes nas situações de sucessivas rupturas dos vínculos sociais e relacionais que são discutidos através de algumas categorias tais como gênero, trabalho, isolamento e relações sociais no âmbito das transformações da sociedade moderna ocidental. Tendo-se em conta que é grande o nível de complexidade do fenômeno da depressão, como todos aqueles conexos aos distúrbios mentais, cujas causas na maioria são desconhecidas, esta pesquisa é uma aproximação, de wn ponto de vista multidisciplinar, à esta complexidade / Abstract: The depression, in its multiples expressions and manifestations of pain and suffering, presents itself as a problem to be investigated not only because it affects the physical and emotional integrity of the individual, .but. also his/her social role. In the community it constitutes a serious problem of public health, prevailing among women, and a serious marter to be dealt by professionals in the field of mental health particularly those ones concemed with Occupational Therapy. However literature proposes in an almost unanimous way that, the phenomenon of depression is inextricably linked to the experience of life of an individual and the way he/she is inserted in the context of society among the several answers given by science to the problem, prevail those fragmented in disciplines. Our research objects to contribute to amplify this limitation of a theorical-methodological order in the comprehension ofthe depression and the depressed humain being. To reach so, we focused on the marter trying to use a multidisciplinary approach, taking care not to separate Biological and Human Sciences, creating an interface to work, where both were of utmost importance. This work studies the manifestations of the psychic suffering related to depression, in the context of the daily life of the individual, and the social behavioral changes and the resulting problems of depression in a group of 25 people. The group was chosen among regular users (patients) ofthe Occupational Therapy clinic. AlI them adults, twenty -one women, four men, average age of 45 years complaining about and diagnosed with depression, through an application of a questionnaire and an interview. Each interview session lasted at least an hour, and the work field (analysis) was carried out during two years, looking into social and cultural aspects related to the social vulnerability related to depression. The different processes present in situations of successive ruptures in the social links and relations, plus the conflicts ofvalues, and social isolation present in the social and cultural insertion of the individual are vastly discussed through the geme, work and other aspects of the transformations of the modem Occidental Society. Taking into account the complexity of the depression phenomenon, as compared to all mental disturbances, which have clear roots and reasons unknown. Eficacia do metodo Meir Schneider de autocuidado em pessoas com distrofias musculares progressivas : ensaio clinico fase II

Nesse contexto discute-se a eficácia do método Meir Schneider de autocuidado (self-healing) baseado em ensaio clínico fase II em 12 pessoas com distrofias musculares progressivas, sob tratamento ambulatorial intensivo e não intensivo. Foram atendidos 7 portadores de Distrofia Muscular Duchenne- DMD (3 intensivos); 3 de distrofia miotônica de Steinert - DMS (1 intensivo) e 2 de outros tipos (1 intensivo). O tratamento intensivo foi ministrado em 3 sessões semanais e o não intensivo em 1 sessão semanal. O atendimento foi ministrado em universidade federal da região sudeste do país, em São Carlos - SP, no período de junho de 1995 a agosto de 1996. Dois instrumentos adotados em pesquisas multicêntricas internacionais foram escolhidos para avaliação das pessoas: o Protocolo de Avaliação Funcional Brooke et alli (1981), aplicado a cada 3 meses e o índice de Satisfação de Vida, forma A (McDOWELL&NEWMANN, 1987), cada 4 meses. Os resultados mais sensíveis ao método de autocuidado foram a força muscular e a satisfação de vida. Foram construídos no presente trabalho os indicadores funcionais, que tratam da comparação da força muscular dos movimentos complementares de três regiões corporais selecionadas: ombros (movimentos de abdução horizontal e adução horizontal); cotovelos (movimentos de extensão e flexão) e joelhos (movimentos de extensão e flexão). Estes indicadores associados à satisfação de vida representam o reconhecimento dos ganhos "dose-dependente" proporcionados pelo método tanto na forma intensiva quanto não intensiva. Nas 3 crianças com DM D do grupo intensivo, meninos de 5 e 6 anos, foi observada a equalização de valores nos indicadores funcionais o que promoveu maior coordenação e estabilização dos movimentos em ombros, cotovelos e joelhos. Também houve flutuação de força ao longo do tratamento mas com aumento global. As 4 crianças de DMD do grupo não intensivo, meninos de 7 a 10 anos, tiveram perda nos indicadores funcionais e na força muscular global, decorrente do crescimento corporal em peso e altura. Todas famílias de crianças com DMD dos grupos intensivo e não intensivo mantiveram sua satisfação de vida em níveis altos, mesmo em fases críticas, no primeiro grupo associada à confirmação diagnostica e no segundo à perda da marcha dos meninos em idade escolar. O estudo de 3 dos 5 casos tratados intensivamente, um de DMD (menino de 5 anos), um de DMS (mulher de 54 anos) e um de distrofia muscular congênita (menino de 4 anos) mostraram a relação da eficácia desse método com o suporte familiar para execução do programa domiciliar e com a capacidade intelectual da pessoa em atendimento. Concluiu-se que o método de autocuidado intensivo é eficaz em pessoas sem déficit intelectual e com suporte familiar. O método, de forma intensiva e não intensiva, mostrou-se eficaz no suporte emocional para lidar com as fases críticas da doença. É também parcialmente eficaz em pessoas resistentes à mudança de hábitos. Concluiu-se ainda que nos momentos de aceleração da doença o método deve ser intensificado mas respeitando-se a capacidade de adaptação da criança ou adulto, evitando-se tomar o atendimento nova fonte de desgaste. Muitas vezes é a família que requer maior atenção e o atendimento deve oferecer esse suporte para aliviar tensão corporal e psíquica e ministrar orientações gerais sobre a doença. Este estudo contribuiu para discriminar a "dose equivalente" do tratamento a ser ministrado segundo os padrões de cronicidade, expectativa de perdas e luto pessoal ou familiar, evitando superdosagem, conflitos e sobrecargas que poderiam obscurecer as vantagens apontadas como mais sensíveis. Sugere-se que o método Meir Schneider de autocuidado seja associado de forma intensiva ou não às terapias correntes como a terapia ocupacional e fisioterapia pois não só amplia o arsenal de recursos; modifica também a estratégia de intervenção e o autocuidado ainda evidencia a relação entre a terapêutica e a abordagem educativa com as pessoas portadoras e suas famílias / Abstract: People affected by Progressive Muscular Dystrophy (PMD) live on the edge of the medical knowledge and are constantly waiting for new research results on prevention, rehabilitation and even the possible cure of those who carry the trait. The efficacy of the Meir Schneider Self-healing Method is discussed in this context, based on a phase II clinical trial with twelve patients with various types of PMD. They underwent outpatient treatment on intensive sessions three times weekly and non-intensive sessions about once a week. Selected cases were seven patients with Duchenne Muscular Dystrophy - DMD (3 under intensive treatment); 3 with Steinert Miotonic Dystrophy - SMD (1 intensive) and 2 with other types (1 intensive). The intervention was carried out at a Health Center of a Federal Government University located in the southwestern Brazil at Sâo Carlos City, São Paulo State, from June 1995 through August 1996. For the purpose of building an evaluation protocol system, to measure the response to the treatments, two instruments were chosen from the literature: the Functional Evaluation of Brooke et alii (1981) that was taken every 3 months, and the Life Satisfaction Index - Form A recorded at every 4 months (McDowell & Newmann, 1987). The most sensible results of the self-healing method thai were drawn from the data were muscle strength and life satisfaction. As a consequence, they were used to built a set of Functional Indicators that dealt with comparisons of muscular strength of opposite complementary movements in three selected body regions: The shoulders (horizontal abduction and adduction); the elbows (extension and flexion) and the knees (extension and flexion) showed greatest sensibility. These functional indicators associated with life satisfaction bring up the evidences of the "dose-dependent" personal gains proportionated by the self-healing method either intensive or non-intensive. In the intensive treatment group 3 children under age 6 with DMD showed the equalization of values in their functional indicators that permitted them better coordination and more stable movements of shoulders, elbows and knees. Their global strength improved despite drifts occurred during the treatment.In the non intensive treatment group 4 children aged 7 to 10 presented losses in the same functional indicators and in their global strength that resulted of their body growth both in weight and height. All families of children with DMD both in the intensive and non intensive groups maintained a high level on their life satisfaction, even during critical phases of the children's diseases especially during the differential diagnosis period for the intensive group and the loss of walking capabilities for the school aged children in the non- intensive group. The Meir Schneider self-healing method has shown efficacy to gaining family support to carry on the exercises program at home and for personal involvement linked to preserved intellectual ability when 3 cases were especially described under clinical terms being: 1 boy aged 5 with DMD; a woman aged 54 with SMD; and 1 boy aged 4 with Congenital MD. The study states that the self-healing method in the intensive treatment shows efficacy for people with family support and with mental or intellectual impairment. This efficacy was also found when dealing with emotional needs in the critical phases of the disease both in the intensive and non-intensive treatments. It is also partially efficacious for people that are resistant to changes in personal habits. Another set of observations in the trial pointed out that when the disease is accelerating its physical losses the method must be intensified, but the therapist have to give way to the coping needs of the child or adult, in order not to transform the very treatment into a burden. In much of these occasions most of the therapist attention is to be focussed on the family offering support for relief of mental and physical tension and general advising about the disease. This study has contributed to distinguish the "dose-equivalent" treatment to be given to PMD patients according to their chronicity status, family and personal losses of self expectancy and psychic grief to avoid treatment overdose, personal conflicts and overload that could prevent from getting advantages on the more sensible indicators. A final suggestion is made, that the Meir Schneider self-healing method be associated with current treatments as occupational therapy and physiotherapy, because it widens the available tools; gives new insight to the intervention strategies besides also puts in evidence the relationships between therapy and an educational approach towards patients and their families / Doutorado / Doutor em Saude Coletiva
