Para o desenvolvimento do estudo, foram realizadas 6 (seis) cavidades ósseas Hemimandibulares em 4 (quatro) cães sem raça definida e sob anestesia geral. Foram formados 3 grupos: controle (preenchido por coágulo sangüíneo); PRP (preenchido por gel de PRP) e, PRP associado ao osso autógeno particulado. Após 120 dias, os animais foram sacrificados e as peças incluídas foram cortadas no sentido longitudinal e transversal e coradas pelo método da hematoxilina e eosina. Os resultados mostraram que nas cavidades preenchidas pelo gel de PRP havia a presença, em todos os animais, de tecido conjuntivo no centro da loja enquanto que no grupo da associação do PRP com osso autógeno a cavidade estava totalmente preenchida por osso neoformado. As partículas de osso autógeno remanescentes na cavidade estavam incorporadas ao osso neoformado ou com aspecto de parcialmente reabsorvidas. Assim, foi possível concluir, nas condições experimentais deste trabalho, que o gel de PRP não promove o reparo da cavidade enquanto que no grupo da associação do PRP com osso autógeno particulado houve reparo completo / The purpose of the present study was to evaluate histologically the platelet rich plasma (PRP) associated or not to the autogenous bone in the mandibular bone cavities repair in dogs. For the development of the study, 6 mandibular bone cavities were performed in 4 indefinite race dogs under general anaesthesia. Three groups were created: control (filled with blood clot); PRP (filled with PRP gel); and PRP associated to the particulated autogenous bone. After 120 days, the animals were killed and the embedded parts were longitudinally and transverselly sectioned and completed through the hematoxylin and eosin method. The results showed that in the cavities filled with PRP gel the connective tissue could be seen in the middle of the lodge in all of the animals, while in the group of PRP associated to autogenous bone the cavity was totally filled with new bone. The remnant autogenous bone particles in the cavity were incorporated to the new bone or suggested being partially resorbed. Consequently, it was possible to conclude, through the experimental conditions of this study, that the PRP gel did not promote the cavity repair while in the group of PRP associated to particulated autogenous bone the repair was complete

Influência da ovariectomia no processo de reparo de enxerto ósseo autógeno em bloco na mandíbula: estudo histológico em ratas adultas

Para isso, foram utilizadas 36 ratas fêmeas, com idade de 12 meses, divididas em grupo controle (GC) e grupo experimental (GE) de 18 animais cada. As ratas do GE foram submetidas à cirurgia de ovariectomia (Ov), enquanto as ratas do GC foram submetidas a simulação do mesmo procedimento cirúrgico, sem a remoção dos ovários (Si). Transcorridos 30 dias da Ov ou Si, todos os animais receberam enxerto ósseo autógeno em bloco na mandíbula, na região próxima ao ângulo, tendo como área doadora o osso parietal da calvária. Os animais de cada grupo foram divididos em subgrupos de 6 animais, os quais foram submetidos à eutanásia nos períodos de 7, 14 e 28 dias, após a realização do enxerto ósseo. Aos 7 dias, a interface enxerto-leito receptor no GC mostrou-se preenchida por um típico tecido de granulação, com discreta atividade de angiogênese na porção central, ao passo que no GE, havia ainda a presença de coágulo sanguíneo e um tecido de granulação em organização. Aos 14 dias, no GC, a interface apresentou-se parcialmente preenchida por matriz óssea neoformada, estabelecendo uma união do enxerto ao leito receptor, enquanto que no GE, a maior parte da interface encontrou-se preenchida por um típico tecido de granulação com nítida angiogênese e discreta atividade osteogênica, com neoformação de matriz óssea em algumas áreas. Aos 28 dias, o enxerto no GC apresentou-se histologicamente integrado ao leito receptor, com acentuada atividade osteogênica, ao passo que no GE a interface apresentou-se parcialmente preenchida por matriz óssea neoformada, encontrando-se também áreas de tecido conjuntivo entre o enxerto e o leito receptor... / The aim of this study was to investigate, by means of histological analysis, the influence of ovariectomy (estrogen depletion) on the repair process of autogenous block bone grafts. 36 female adult rats, age twelve months, were assigned to two groups (n=18): control group (CG) and experimental group (EG). The rats in the EG had an ovariectomy (Ovx) whereas those in the CG underwent the same surgical intervention but did not have their ovaries removed (Sham). Thirty days after Ovx or Sham all the animals received autogenous block bone grafts in the mandible, in the region near the mandibular angle. The donor area was the parietal bone. The animals of each group were euthanized at 7, 14 and 28 days postoperative. On the 7th postoperative day, the graft-recipient site interface in the CG appeared filled by a typical granulation tissue with discreet angiogenic activity in the central portion whereas the EG still exhibited a blood clot and an organizing granulation tissue. On the 14th postoperative day, the interface in the CG was partially filled with newly formed bone matrix establishing a union between the graft and the recipient site. The majority of the interface in the EG was filled by a typical granulation tissue with evident angiogenic activity, discreet osteogenic activity and newly formed bone matrix in some areas. On the 28th postoperative day, the graft in the CG appeared histologically integrated with the recipient site and exhibited pronounced osteogenic activity. On the other hand, the interface in the EG appeared partially filled by newly formed bone matrix with areas of connective tissue observed as well. Within the limitations of the study, it may be concluded that estrogen depletion caused by the ovariectomy hindered the healing process of autogenous block bone grafts placed in the mandible of female adult rats... Material e Métodos: Foram utilizados 40 ratos (Rattus norvegicus albinus, Wistar) machos, com aproximadamente 250 gramas divididos em dois grupos: o grupo I (Controle) recebeu injeção de tampão citrato, pela via endovenosa e o grupo II (Diabético) recebeu pela via intravenosa injeção de estreptozotocina dissolvida no tampão citrato para induzir o diabetes. Após controle glicêmico, anestesia, depilação e antissepsia foi realizada uma incisão longitudinal nas regiões antero-laterais de cada tibia. Após exposição das tíbias, duas cavidades cirúrgicas de 2mm de diâmetro foram preparadas com trefina em baixa-rotação refrigerada, uma no membro posterior direito e outra no membro posterior esquerdo, sendo a primeira preenchida com o osso autógeno particulado coletado durante a realização das duas cavidades e a segunda preenchida com coágulo sanguíneo e ambas recobertas pela membrana. Aos 10 e 30 dias pós-operatórios os animais foram eutanaziados e as peças obtidas foram processadas laboratorialmente para a realização de cortes semi-seriados com seis micrômetros de espessura e corados pela hematoxilina e eosina para análise histomorfométrica. Resultados e Conclusão: Concluiu-se que a reparação óssea foi maior com o emprego dos enxertos ósseos no grupo controle, embora as diferenças não foram estatisticamente significantes em nenhum período estudado e que, aos 30 dias, o processo de reparo das tíbias preenchidas... / Purpose: The aim of this study was to evaluate histomorphometric the behavior of membrane of a demineralized homogenous bone matrix and preserved in glycerin in the process of bony repair of surgical cavities filled out with autogenous bone accomplished in tibias of diabetic rats. Materials and Methods: Forty male rats weighing approximately 250 g were selected and assigned to two groups, as follows. Group I (Control) received a citrate buffer injection intravenously while Group II (Diabetic) received an intravenous injection of streptozotocin dissolved in citrate buffer to induce diabetes. After glycemic control, anesthesia, shaving and antisepsis, a longitudinal incision was made in the anterolateral areas of each tibia. After exhibition of the tibias, two surgical cavities of 2mm of diameter were prepared with trefina in refrigerated low-rotation, one in the right posterior member, other in the left subsequent member, being the first filled out with particulate autogenous bone collected during the accomplishment of the two cavities and second filled out with sanguine coagulum and both covered with the membrane. Ten and thirty days postoperatively, the animals were euthanized and the anatomical pieces were submitted to routine laboratorial processing and serially sectioned to obtain 6-μm-thick sections, which were stained by hematoxylin and eosin for histomorphometric analysis. Results and Conclusions: We concluded in the ended of the bony repair was larger with the employment of the bone grafts in the group control, although the differences don't have statistics significance in any studied period and that to the 30 days, the process of repair of the tibias filled out by graft of the group control it was more favorable quality than of the tibias under the same treatment of the diabetic group although such differences have not been statistics significance and the membrane was biocompatible in all the analyzed groups.

Materiais e métodos: Para a obtenção da membrana homógena foram utilizados 40 animais saudáveis, não incluídos no grupo experimental. No experimento foram utilizados 120 ratos (Rattus norvegicus albinus, Wistar) machos, com peso aproximado de 250 gramas, divididos em dois grupos: o grupo I (IC), sem alterações sistêmicas (Controle) recebeu injeção de tampão citrato a 0,01M, ph 4,5, pela via endovenosa e o grupo II (Diabético) ou IID recebeu pela mesma via de administração (veia peniana) injeção de estreptozotocina (Sigma-Aldrich) dissolvida em tampão citrato a 0,01M, ph 4,5, em uma concentração de 35mg/Kg. Após controle glicêmico os ratos sem alterações sistêmicas (grupo controle) e diabéticos foram subdivididos em três subgrupos de experimentos: SM - a cavidade cirúrgica da tíbia esquerda foi preenchida com enxertos ósseos autógenos, não sendo recoberta por membrana, MH - a cavidade também preenchida com enxertos ósseos foi recoberta por membrana homógena e MX - o recobrimento foi feito com membrana sintética de PTFe. Os animais foram eutanaziados aos 10 e 60 dias e as tíbias foram submetidas ao processamento laboratorial de rotina para análise histológica e histométrica. Resultados: Aos 10 dias não foram encontradas diferenças estatisticamente significantes entre os diabéticos e não-diabéticos que tiveram suas feridas recobertas ou não com as membranas. No entanto, nesse tempo, o tecido ósseo do grupo diabético apresentou-se qualitativamente pior se comparado ao do grupo controle. Results: At 10 days, there were no statistically significant differences between diabetic and non-diabetic rats which had their wounds covered or not covered with the membranes. However, at 10 days the bone tissue of the diabetic group was qualitatively worse in comparison to that of the control group. Possui algumas vantagens como a possibilidade de infusão de soluções em grande quantidade, simultaneamente e por tempo prolongado e como desvantagens podemos citar algumas complicações descritas em literatura como: infecção da inserção, túnel subcutâneo ou lúmen do cateter; migração da ponta; trombose de grandes ou pequenos vasos; rompimento ou perfuração com conseqüente extravasamento de soluções; obstrução por coágulos, medicações cristalizadas ou mecânica de um ou dos dois lumens. Na Enfermaria de Hematologia do Hospital das Clínicas da UNICAMP, este tipo de cateter é indicado para pacientes que serão submetidos à mobilização de células tronco hematopoéticas e transplante autólogo de medula óssea. Este estudo teve como objetivo quantificar e qualificar as complicações do cateter tipo Hickman de duplo lúmen, inseridos em pacientes que foram submetidos à mobilização de células tronco hematopoética e realização de transplante autólogo de medula óssea. Casuística e Métodos: um total de 55 cateteres tipo Hickman foram inseridos em 53 pacientes consecutivos submetidos à mobilização de células tronco hematopoética e transplante autólogo de medula óssea, no período de 24 de maio de 2000 à 29 de janeiro de 2003, na Enfermaria de Hematologia do Hospital das Clínicas da UNICAMP. Cinco pacientes foram excluidos do estudo. Todos os cateteres foram inseridos em Centro Círúrgico, sob anestesia local. Os dados para o estudo foram obtidos através da avaliação diária do cateter tipo Hickman quanto à sinais de infecção, fluxo e refluxo dos dois lumens, posicionamento, além de exames laboratoriais pertinentes. Resultados: os cateteres permaneceram locados por um mediana de 68 dias. Durante todo o período de observação p do estudo, foi registrado a freqüência de 96 cateteres com complicações relacionadas, 78% dos cateteres foram removidos devido à complicações e apenas dois permaneceram sem nenhum tipo de complicação no decorrer do período de observação. As causasde complicações que geraram a remoção dos cateteres foram: bacteremia (18%), colonização do cateter (14%), febre de origem indeterminada (12%), infecção do túnel (10%), óbito por septicemia (6%), septicemia (4%), problemas relacionados ao posicionamento ou fixação (12%) e trombose venosa profunda (2%). A complicação mais comum que gerou a remoção do cateter foi infecção. Discussão: a mediana de permanência, obtida em nosso estudo, é inferior a maioria dos dados descritos na literatura, porém encontra-se próxima, e até um pouco superior quando comparada à outros estudos que envolveram pacientes oncohematológicos. Em nosso estudo observamos elevado índice de infecção, quando comparado aos dados da literatura. Infecção da corrente sangüínea foi a ocorrência infecciosa mais freqüente (18%), seguida de colonização do cateter (14%), febre de origem indeterminada (12%), infecção do túnel (10%) e septicemia (10%). Alteração de permeabilidade foi observada em 10% dos cateteres, porém nenhum cateter foi removido por este motivo. Conclusão: um acesso venoso seguro é essencial para pacientes oncohematológicos. O cateter de Hickman mostrou ser factível e adequado para pacientes em IIlúbilizaçãode células tronco hematopoética e transplante autólogo de medula óssea, pois permite.a infusão de líquidos endovenosos em grande volume, por vias independentes e apre$enta fluxo suficiente para realização de aférese. Porém, em nosso serviço, a freqüência de complicações infecciosas relacionadas ao cateter é elevada, demonstrando. cuidados deficientes de manutenção / Abstract: Central venous catheters (CVCs) are essential for patients undergoing autologous bone marrow transplantation (ABMT). Hickman's catheter (HC) is a long-term, tunneled and cuffed venous access. This type of catheter constitutes a safe and useful line for administration of chemotherapy, antimicrobial agents, blood products, hydration, total parenteral nutrition and permits blood sampling and the accomplishment of leucoapheresis. Safety venous access is essential in patients with haematological diseases. Patients and Methods: a total of 55 Hickman's catheter were inserted in 53 consecutive patients undergoing ABMT, five patients were exc1udedof study. Fifty catheters were available for this study. All catheters were inserted in surgery room, with fluoroscopic guidance and local anaesthesia. AlI patients were followed daily during hospitalization time, from the time of catheter insertion to the time of catheter removal. A record of all complications and catheter loss cause were analyzed. Results: Catheters remained inserted in a median time of 68 days. A total of 96 episodes of complications with Hickman's catheter were documented. On1ytwo catheters did not present any complication and catheter 10ssrate due to complications was 78%. The most common complication for catheter requiring removal was infection. Discussion: this study aimed in a prospective observation defined the feasibility of this type of catheter in a general infirmary, in patients treated with high dose mobilizing chemotherapy. In our experience no data concerning catheter insertion are available, however no deaths related to the procedure occurred. The complication leading to catheter removal were infections (64%), tip migration (12%), thrombosis (2%), and others causes (22%). Difficulties of flow, particularly to collect samples, occurred in about 10% of cases but did not implicate in catheter removal. Therefore our study showed higher levei of infection compared to data from literature. Regarding the onset blood stream infection was the more frequent (18%), followed by catheter colonization (14%), pyrexia of unknown origin (PUü) (12%), tunnel infection (10%) and, septicemia (10%). Conclusion: A dependable central venous access is an essential prerequisite for the delivery of care to onco-hematological patients. The use of long-term venous access devices for chemotherapy, parenteral nutrition, antibiotic therapy and apheresis procedures is increasing. Acquisition of a reliable and sustained central venous access is a crucial point in the provision of comprehensive care to cancer patients. Many clinical adverse effects of antineoplastic drugs, particularly in heavily treated patients, as phlebitis, thrombosis, and fibrosis of peripheral veins, impose limitations on its repeated use. lndeed, patients needing to collect peripheral progenitor cell in order to perform autologous bone marrow transplantation should have a good access with ideal blood flow in the apheresis procedures. lndwelling Hickman's catheters are widely use and have proved to be a good devise for the delivery of drugs and apheresis procedure. ln addition, its use permits the best clínical control and monitoring of hydration, ions, central venous blood pressure, collection of samples for hematology, microbiology and biochemistry analysis. lndeed the uses of catheters are more cornfortable and lead a better quality of life during treatment / Mestrado / Clinica Medica / Clinica Medica

Materiais implantáveis e métodos de implantação são continuamente investigados. Este trabalho avaliou a utilização de enxerto de fragmento da faseia lata na lâmina própria da prega vocal de cães, estudando as alterações histológicas conseqüentes, quanto à reação inflamatória aguda e crônica, à fibrose e ã neovascularização. Doze cães adultos foram submetidos à enxertia de um fragmento de 0,3 X 0,3 cm da faseia lata na lâmina própria da prega vocal direita. Na prega vocal esquerda, o mesmo acesso cirúrgico foi reproduzido, sem colocação do enxerto. Após dois e seis meses, os animais foram sacrificados e suas pregas vocais estudadas histologicamente. A faseia não induziu resposta inflamatória, aguda ou crônica, em qualquer animal, nem desencadeou fibrose além da conseqüente ao procedimento cirúrgico, como observado no lado controle. Em todos os casos, identificou-se o tecido enxertado sem evidências de reabsorção e com aumento da neovascularização, demontrando que a faseia lata apresenta integração tecidual boa na camada superficial da lâmina própria de prega vocal de cão / Abstract: Satisfactory corrective treatment for sulcus, scar and senile alterations in the lamina propria of vocal folds is still under development. This study assessed the utilization of a fascia lata graft in the vocal fold lamina propria of dogs by analyzing the histological alterations induced by acute and chronic inflammatory reactions as well as the fibrosis triggered by graft procedure and persistence. A fragment measuring 0.3 X 0.3 cm of the fascia lata was grafted in the right vocal old lamina propria of 12 adult dogs. Identical surgical access in the left vocal fold was reproduced without introducing the graft. After periods of two and six months, the animals were sacrificed and a histological study of their vocal cords was performed. Besides the reactions resulting from the surgical procedure in the control side, the fascia did not induce an acute or chronic inflammatory reaction or trigger fibrosis in any of the animals. Histologicamente é formado pelo estroma e o parênquima, sendo seu arranjo celular diferenciado em polpas branca e vermelha. Os principais tipos celulares na estrutura esplênica são: linfócitos T e B, macrófagos, plasmócitos, fibrócitos, reticulócitos e células dendríticas. Os doentes vítimas de traumatismos esplênicos e esplenectomizados assumem um estado de asplenia e consequentemente adquirem grande susceptibilidade à sepse a qual pode ser rapidamente progressiva e fatal. Assim, têm surgido opções terapêuticas após as esplenectomias, utilizando técnicas cirúrgicas conservadoras (tratamento conservador, esplenectomia parcial e esplenorrafias com ou sem aplicação de agentes hemostáticos) e por fim as técnicas de autotransplante esplênico. Os objetivos desta pesquisa experimental foram analisar macro e microscopicamente a evolução tardia do autotransplante de fragmentos de baço na cavidade peritoneal, epíplon e mesentério, após 24 semanas de observação. Foram utilizados 52 ratos Wistar, machos e adultos, submetidos a esplenectomia total e divididos em quatro grupos. O grupo I (controle) foi formado por oito animais escolhidos aleatoriamente entre os três grupos estudados. O grupo II por dezessete animais com implante de fragmento de baço solto na cavidade peritoneal; grupo III por dezoito animais com implante no epíplon e o grupo IV por dezessete animais com implante fixado na raiz do mesentério. Na avaliação histológica foram adotados critérios qualitativos e quantitativos, com a contagem de elementos celulares e não celulares. Os resultados mostraram que se formaram aderências aos tecidos adjacentes e neovascularização em todos os fragmentos transplantados. O grupo II apresentou polpa branca e vascularização preservadas. No grupo III foram observados polpa branca com formações folicular e bainha linfóide, e a polpa vermelha em aspecto cordonal apesar de hemorrágica. No grupo IV foram observados alguns cortes histológicos com depleção de polpa branca e vermelha, enquanto outros evidenciavam melhor preservação das polpas. A contagem de linfócitos revelou diferença significativa entre os grupos I e II e o grupo I e IV (p<0,05). A contagem de macrófagos ativos revelou diferença significativa entre os grupos III e IV (p<0,05) e similaridade entre I e III (p>0,05). Os outros elementos: macrófagos ativos fagocitando hemossiderina, plasmócitos, fibroblastos, fibrócitos, células gigantes, monócitos, espaços intersticiais e fibras de colágeno, não apresentaram diferença significativa entre os grupos. O autotransplante esplênico é factível, sendo o grande epíplon o melhor local para a sua fixação. Esta pesquisa demonstrou por meio de análise histológica qualitativa e quantitativa que o tecido esplênico autotransplantado no epíplon preserva sua função de defesa dos organismos / Abstract: The spleen is a lymphoid organ that plays important part in the organism defense, participating in filtration processes, phagocytosis and immunoglobulin production. Histologically, is formed by a stroma and the parenquima being its cellular arrangement differentiated in white and red pulps. The main cellular types in the splenic structure are: T and B lymphocytes, macrophages, plasmocytes, fibrocytes, reticulocytes and dendritic cells. The patients victims of spleen traumatisms and splenectomized assume an asplenia state and consequently they acquire a larger susceptibility to the sepses, which can be quickly progressive and fatal. Thus, it has appeared therapeutic options after the splenectomies, using techniques of conservative surgery (conservative treatment, partial splenectomies and splenorrhaphies with or without application of haemostatics agents), as well as techniques of splenic autotransplantation. The objectives of the research were to evaluate macro and microscopically the late evolution of the fragments of spleen autotransplanted in the greater omentum, mesenterium and peritoneal cavity, after 24 weeks of observation. To perform the experimental study, were employed fifty two Wistar rats, males and adults, submitted to total splenectomy and divided in four groups. The group (control) I was formed by eight animals chosen aleatorily among the three groups studied. The group II - seventeen animals with implant of spleen fragment in the peritoneal cavity; group III - eighteen animals with implant in the omentum and group IV - seventeen animals with implant fixed in mesenterium root. In the histological analysis were adopted qualitative and quantitative criteria, with the counting of no cellular e cellular elements. The results showed adherences to the adjacent tissues and vascularization in all of the fragments transplanted. The group II presented white pulp and preserved vascularization. The group III showed white pulp with follicular formations and lymphoid tissue preserved, and the red pulp in cordon aspect and hemorrhagic. The group IV presented depletion of white and red pulp, while others evidenced better preservation of the pulps. The counting of lymphocytes revealed significant difference between the groups I and II and the group I and IV (p < 0.05). The counting of active macrophages revealed significant difference between the groups III and IV (p < 0.05) and similarity between I and III (p > 0.05). The other elements: active macrophages phagocyting hemosiderine, plasmocytes, fibroblasts, fibrocytes, giant cells, monocytes, interstitial spaces and fibers of collagen, did not show significant difference among the groups. The splenic autransplantation is feasible, being the better place the greater omentum. This research demonstrated through qualitative and quantitative histological analysis that the splenic tissue autotransplanted in the omentum of Wistar rats preserves its function of defense of the organisms / Mestrado / Pesquisa Experimental / Mestre em Cirurgia
