Análise bioética sobre o acesso dos usuários às unidades de tratamento intensivo no Brasil

No. of bitstreams: 1 2012_MarceloMoreiraCorgozinhocompleto.pdf: 1612368 bytes, checksum: 4e509f0aae8148cf26ffae6dd080b60a (MD5) / Realizou-se um estudo exploratório através do procedimento técnico de levantamento bibliográfico e análise documental de dados extraídos dos sites do Ministério da Saúde, Instituto Brasileiro de Geografia e Estatística e Associação de Medicina Intensiva Brasileira. O objetivo foi analisar a partir de conceitos bioéticos a cobertura assistencial em terapia intensiva por grandes regiões geográficas brasileiras. Foi possível estimar a necessidade de leitos de UTI através da Portaria 1101/2002 do Ministério da Saúde, que corresponde a 4 a 10% do total de leitos hospitalares ou 1 a 3 leitos para cada 10.000 habitantes, segundo a Associação de Medicina Intensiva Brasileira. Os resultados evidenciaram situações de iniquidade na distribuição de leitos de terapia intensiva no Brasil. As regiões Norte e Nordeste, as mais vulneráveis, dispõem das menores coberturas do país (0,7 e 0,8 leitos para cada 10.000 habitantes), enquanto que as regiões Sudeste, Sul e Centro-Oeste possuem as melhores coberturas (1,9; 1,7; 1,5 leitos para cada 10.000 habitantes, respectivamente). A falta de leitos em UTIs é um problema recorrente e que impede com que várias pessoas, das mais variadas idades e classes sociais tenham respeitados os direitos Constitucionais previstos. É dever do Estado realocar recursos suficientes, a fim de assegurar assistência aos cidadãos, principalmente aos mais necessitados. A Constituição Brasileira de 1988 trouxe a dignidade da pessoa humana como o valor maior do Estado, o núcleo constitucional supremo em torno do qual gravitam os demais direitos, sendo que o SUS representou um marco sanitário e democrático na saúde, mesmo com ressalvas quanto à equidade e à integralidade da assistência. A saúde consagrada como direito fundamental é um tema de extrema relevância social, econômica e científica, para o desenvolvimento do país. Nenhum país consegue se desenvolver e evoluir sem primeiro proporcionar condições mínimas de saúde para sua população. A análise dos resultados foi desenvolvida com base na Declaração Universal Sobre Bioética e Direitos Humanos, a saber, equidade em saúde, respeito pela vulnerabilidade humana e responsabilidade social compartilhada. No que tange à ética do século XXI, é fato a nova Bioética social e contextualizada que se consagrou após a Declaração Universal sobre Bioética e Direitos Humanos. É forte o apelo a favor dos indivíduos mais frágeis da sociedade, não promovendo uma situação passiva por parte destes, mas, intervindo de modo a fortalecê-los. Intervir para vê-los como sujeitos de valoração, assumindo uma posição digna e participativa na sociedade. Todos os membros da sociedade são trabalhadores sociais em potencial, portanto, responsáveis pelo processo de reconstrução social. ___________________________________________________________________________________ ABSTRACT / An exploratory research was undertaken through the technical procedure of literature review and data analysis from data extracted from the Ministry of Health, Brazilian Association of Critical Care Medicine and Brazilian Institute of Geography and Statistics websites. The goal is to analyze the assistance coverage in intensive care for major geographical regions, from concepts of the Bioethics. It was possible to estimate the need for ICU beds by the 1101/2002 Ordinance of the Ministry of Health, which corresponds to 4-10% of total hospital beds, or 1 to 3 beds per 10.000 inhabitants, according to Brazilian Association of Critical Care Medicine. It was identified that the North and Northeast regions, the most vulnerable, have the worst coverage in intensive care in the country (0,7 and 0,8 beds per 10.000 inhabitants), white the Southeast, South and Midwest have the best coverage ( 1.9, 1.7, 1.5 beds for every 10.000 inhabitants, respectively). The lack of beds in ICUs is a recurring problem and that impedes several people, of various ages and social classes, to have their provided Constitutional rights respected. It is the duty of the State to reallocate sufficient resources to ensure assistance to citizens, especially the needy. The Brazilian Constitution of 1988 brought the dignity of the human person as the largest value of the state, the supreme constitutional core, around which the other rights gravitate, while the SUS represented a sanitary and democratic mark in health, even with reservations about the assistance equity and integrality. The health enshrined as a fundamental right is a topic of extreme social, economic and scientific importance for the development of the country. No country can develop and evolve without firstly, providing the minimum conditions of health for its population. The analysis of the results was developed based on objectives of the Universal Declaration on Bioethics and Human Rights, namely, health equity, respect for human vulnerability and shared social responsibility. Regarding the ethics of the century XXI, is indeed the new Bioethics and social context that proved after the Declaration Universal on Bioethics and Human Rights. It is strong appeal in favor of the weaker individuals of society, not promoting a passive situation on their part, but intervening in order to strengthen them.
