Yes / A significant proportion of healthcare that is delivered is wasteful, harmful and not evidence based. There are many wound care related guidelines, but their implementation in practice is variable. The Society of Tissue Viability (SoTV) published updated seating guidelines in 2017, but there is a lack of theoretical and conceptual clarity about how these guidelines are being utilised to inform clinical practice. Therefore, the aim of this paper is to generate a theory that can be used to incorporate the SoTV seating guidelines into policy and clinical practice.
Methods: We critically reflected on data from an evaluation study utilising systems-thinking approach, informed by implementation and safety science using wider literature as well as our expertise to generate a guideline implementation theory.
Discussion: Factors that facilitate or hinder the incorporation of the SoTV guidelines into policy and practice were characterised. We conceptualised the implementation of these guidelines into policy and practice into a Translation or Implementation into Policy or Practice (TIPP) theory with distinct stages, that we called liminal spaces. Knowledge of the guidelines, and the agency or authority to effect change, are key factors in the translation of these guidelines into clinical practice.
Conclusion: Our theory is that there are liminal spaces in the implementation trajectory of these guidelines into practice, which have their own characteristics. This theory provides a framework that can be used to underpin guidelines strategies to embed skin and wound care guidelines into policy and clinical practice in order to improve patient care.

Quality of life of adults with venous leg ulcersTheron, Bernhardett 08 1900 (has links)
Health related quality of life (HRQoL) refers to the impact of disease and treatment on disability and daily living. Living with a venous leg ulcer (VLU) has a negative impact on HRQoL. The aim of the present study was to investigate the aspects of HRQoL affected by VLUs in a local SA sample. A post hoc research design was utilised comparing 30 VLU patients and 30 non-VLU participants. Four hypotheses were investigated: (1) the experience of having a VLU on HRQoL, (2) the relation between gender and HRQoL of VLU patients, (3) the relation between age and HRQoL of VLU patients and (4) the interrelation between the underlying constructs of HRQoL of VLU patients. A structured questionnaire was used to obtain information on demographic variables and HRQoL using the SF-36 questionnaire. Interviews were conducted to obtain qualitative data to complete the investigation. Correlation analysis and t-tests confirmed that having a VLU negatively affected HRQoL and that interrelations existed among the constructs of HRQoL. There were no significant relations between gender, age and HRQoL of VLU patients. In line with the social constructivist approach, social support and restructuring a sense of meaning in life to increase HRQoL, are important goals in the management of VLUs. / Psychology / M.Sc. O conhecimento embasado nas melhores evidências científicas para a assistência de enfermagem às pessoas com feridas crônicas (FC) deve alicerçar o ensino da temática na formação do enfermeiro. Diante da relevância epidemiológica do problema, o docente de enfermagem se depara com o desafio de ensinar aos estudantes, o cuidado às pessoas com essa complicação, nos diversos contextos de atenção à saúde. A etapa da avaliação da FC é fundamental para identificar e descrever o processo de cicatrização e a seleção da terapia tópica, que o favoreça. A educação à distância (EAD) tem demonstrado ser uma estratégia efetiva para atualização do conhecimento. Diante disto, o objetivo deste estudo foi identificar o impacto da EAD, no conhecimento de docentes de enfermagem e enfermeiros vinculados ao ensino superior, acerca da avaliação de FC. Tratou-se de um estudo prospectivo, quase-experimental, com coleta de dados antes e após a intervenção. Após aprovação pelo Comitê de Ética em Pesquisa, foram convidados a participar da pesquisa, docentes e enfermeiros vinculados ao ensino, de instituições de ensino superior (IES) públicas e privadas, que ministrassem disciplinas teóricas e/ ou práticas, nos cursos de graduação em Enfermagem, de um município do interior do Estado de São Paulo. A amostra foi composta por 26 docentes e enfermeiros vinculados ao ensino, que participaram do curso de atualização \"Avaliação de Feridas Crônicas na Assistência de Enfermagem\" oferecido através do Ambiente Virtual de Aprendizagem (AVA) Moodle. Entre os participantes, a média de idade de 42,3 anos (DP 9,56), e todos eram do sexo feminino. O tempo médio de experiência no ensino foi de 11,16 anos (DP 8,02), 24 (92,3%) docentes afirmaram lecionar disciplinas teóricas e/ ou práticas, dos quais, 91,7% referiram atuar no cuidado de pessoas com FC, nos cenários de prática. A ferramenta mais utilizada para a busca de informações sobre \"feridas crônicas\" pela maioria dos participantes foi a internet. O aumento na porcentagem de acertos após a intervenção foi estatisticamente significante e o domínio que obteve maior número de acertos no pré e pós-teste foi \"Dimensionamento da ferida\", com melhor desempenho ao final do curso. A correlação entre o tempo de experiência na docência e o desempenho no teste de conhecimento foi negativa, e a associação entre a utilização de outras estratégias para busca de informações, tais como troca de experiência entre os pares, participação em eventos científicos acerca do tema, melhorou o desempenho dos participantes. A utilização da EAD, como ferramenta para a intervenção educativa, visando a divulgação de recomendações para a avaliação de feridas crônicas, impactou positivamente na aquisição de conhecimentos dos participantes. Considering the epidemiological relevance of this problem, nursing faculty are faced with the challenge of teaching students how to look after people with this complication in different contexts of health care. The assessment of CW is critical to identify and describe the healing process and the selection of the best topical therapy. The effectiveness of E-learning (EL) strategy to update knowledge has been demonstrated in other studies. Therefore, the aim of this study was to identify the impact of e-learning on CW assessment knowledge of teachers and nurses involved with higher education. A prospective and quasi-experimental study was conducted with data collection before and after the intervention. After the board of ethics approval, teachers and nurses were invited to participate in the research. These teachers and nurses involved in higher education institutions, both public and private, teach theoretical subjects and or practices in nursing undergraduate courses in the municipality in the state of Sao Paulo. The sample consisted of 26 teachers and nurses who attended the update course \"Assessment of Chronic Wounds in Nursing Care\" offered through the Virtual Learning Environment (VLE) Moodle. Among participants, we identified an average age of 42.3 years (SD=9.56), and regarding gender, all subjects were female. The average teaching experience was 11.16 years (SD=8.02), 24 (92.3%) teachers reported teaching theoretical subjects and/or practices, of which 91.7% reported working with CW care in practice scenarios. The most widely used tool to search for information about \"chronic wounds\" was the internet. The increase in the percentage of correct answers after the intervention was statistically significant and the item with highest number of correct answers in both pre and posttests was \"Dimensioning the wound\", with better performance at the end of the course. The correlation between times of teaching experience and performance on the knowledge test was negative, and the association between the use of other strategies to knowledge update (such as exchange of experience among peers, participation in scientific events on the theme) was found to improved performance of the participants. The use of EL as a tool for educational intervention focused on disseminating recommendations for CW assessment positively impacted the acquisition of knowledge among participants. Adopting technologies for the improvement of nursing faculty\'s abilities had outcomes that may support education and generate changes in the clinical nursing practice

The purpose of this thesis was to quantify the cost of care and treatment of pressure ulcers in Thomayer University Hospital with polyclinic (FTNsP) in Prague. Present pressure ulcer monitoring system was analyzed and a transformation of the records collected by this systém in the period 2005 - 2010 was performed in order to derive the necessary data - especially the number of patient-days, displayed by severity of the pressure ulcers. Data on expenditure of materials and pharmaceuticals was obtained by questionnaire survey, conducted in 2009 among the nursing staff. The description of the incidence of pressure ulcers in FTNsP, analysis of the effect of the use of anti-decubitus mattress on the incidence of pressure ulcers and bedsores costs per patient and for the entire hospital according to a single component of direct costs and costs of extended hospital stay are the results of the thesis.

Bakgrund Allmänsjuksköterskor kommer ofta i kontakt med personer med venösa bensår. Behandling och vård av dessa personer sker ofta inom primärvård, öppenvård eller hemsjukvård, men även på vårdavdelningar och andra sjukvårdsinstanser. Venösa bensår är en följd av underliggande kronisk venös insufficiens. Det är därför stor risk att såren återkommer efter läkning och behandlingen måste därför i många fall bli livslång. Trots detta finns få studier som fokuserar på personers upplevelser av behandling och vård vid venösa bensår. Syfte Syftet är att beskriva personers upplevelser av behandling och vård vid venösa bensår. Metod För att sammanfatta den forskning som i dagsläget finns kring ämnet har en litteraturöversikt gjorts. Sökningar har utförts i de vetenskapliga databaserna Pub Med och CINAHL och 16 artiklar identifierades. Dessa har kvalitetsgranskats genom Sophiahemmet Högskolas bedömningsinstrument. Resultat Litteraturöversiktens tre huvudkategorier, förhoppning och uppgivenhet, att leva ett begränsat liv och att inte bli sedd som person berättar om upplevelser som många personer med venösa bensår har av behandling och vård. Personer upplever att behandling och vård på något sätt begränsar eller påverkar dem. Vårdens kontinuitet, god kommunikation, och relationen till sjuksköterskan är av betydelse för upplevelsen av behandling och vård. Det finns brister i individanpassningen av behandling och vård som påverkar personer med venösa bensårs förtroende för vården. Många personer uppger en ovisshet som gör det svårt att hantera behandlingen psykologiskt. En stark önskan om läkning av de venösa bensåren är gemensamt för många studiedeltagare. Slutsats Det fattas specifik forskning om personers upplevelser. Sjuksköterskor som behandlar personer med venösa bensår saknar kunskaper om deras tillstånd och rekommenderad behandling. I denna litteraturöversikt framkommer ett behov av att skifta fokus från läkning av det venösa bensåret till personens livskvalitet. Sjuksköterskor behöver mer kunskaper om venösa bensår och en större förståelse för den livssituation som många personer med venösa bensår befinner sig i. Ökad nivå av personcentrering i behandling och vård av personer med venösa bensår bör därför eftersträvas, och förutsättningar måste skapas för sjuksköterskor att kunna erbjuda den specialiserade och kvalitativa vård som krävs. / Background Registered nurses frequently come into contact with persons living with venous leg ulcers, as these wounds are primarily treated and cared for by nurses. Persons with venous leg ulcers often experience underlying chronic venous insufficiency, which is the main cause of venous leg ulcers. This means that even once healed venous leg ulcers are likely to reoccur, and treatment is often life-long. In spite of this, very few studies and research projects focus on the person’s experiences of their ailment in relation to the treatment they receive. Aim To highlight persons’ experiences related to the treatment and care of venous ulcers. Method To summarize the research knowledge of this subject, a review of current literature was undertaken. Searches have been conducted in several online-based databases. Through the databases PubMed and CINAHL 16 articles were identified and then analyzed. Results Many persons with venous leg ulcers feel that treatment and care affects or impedes them in some way. Three core categories were identified as signifying for the result: hopefulness and uncertainty, an impaired life, and not being viewed as a person. Continuity of care, quality of communication, and the nurse-patient relationship are all key contributors to the experiences of treatment and care that persons with venous leg ulcers describe. The result implies that treatment and care of persons with venous leg ulcers is not satisfyingly individualized, which reduces the level of confidence that these persons feel for their healthcare providers. Persons with venous leg ulcers report that experiences of uncertainty make the treatment hard to handle psychologically. A strong desire for permanent healing is shared by persons with venous leg ulcers. Conclusions There is a shortage of specific research aimed at the experiences of persons with venous leg ulcers of aspects relating to their treatment and care. Many nurses lack vital knowledge regarding the treatment and care of these individuals. In this review a need for a shift of focus is perceived, from simply the healing of the venous leg ulcer, into also considering the quality of life of persons with venous leg ulcers. Nurses are in need of more knowledge of the condition, and more insight into the lives of persons with venous leg ulcers. A person-centred approach should therefore be sought, and conditions must be provided for nurses to expand their knowledge and be able to offer a more specialized and qualitative level of treatment and care.

Previous issue date: 2014-03-28 / Nenhuma / A úlcera de perna é considerada uma síndrome que se caracteriza pela perda irregular ou circunscrita do tegumento, que pode atingir o tecido subcutâneo e outros tecidos subjacentes, acomete as extremidades dos membros inferiores e pode ser desencadeada por diversos fatores. É identificada como ferida crônica, considerando o longo período de duração entre o tempo de abertura e a cicatrização da ferida, além de muita recorrência em um curto espaço de tempo. OBJETIVO: Verificar o método mais eficaz de avaliação sistematizada de úlceras de membros inferiores. MATERIAL E MÉTODO: Revisão sistemática da literatura nas bases de dados indexados Crochrane, Medline/Pubmed, Lilacs e MedCarib, utilizando os descritores úlcera, úlcera de perna, úlcera de pé, protocolos, cuidados de enfermagem e diagnóstico. A elaboração do protocolo seguiu os critérios do Manual Operacional de Diretrizes Clínicas/Protocolos Assistenciais da Gerência de Ensino e Pesquisa do Grupo Hospitalar Conceição (GEP-GHC, 2008) e da Metodología para la elaboración de guías de atención y protocolos (GÓMEZ, 2007). Para a análise dos resultados utilizou-se a estratégia de hierarquização dos níveis de evidência do sistema de classificação do Oxford Center for Evidence Based Medicine e a classificação do Grading of Recommendations Assessment, Development and Evaluation (MECHANICK et al., 2010; DEL MAR; SALISBURY; GLASZIOU, 2010). RESULTADOS: Foram encontrados 74 artigos na Biblioteca Crochrane, 174 na base de dados Medline/Pubmed, 31 na Lilacs e 7 na MedCarib. Após a aplicação dos critérios de inclusão e exclusão, além dos filtros de busca, restaram 179 artigos. Artigos em duplicidade ou sem disponibilidade de texto na integra foram excluídos, e também após a leitura de títulos e resumos, restando apenas 14. O guia prático de atendimento do enfermeiro na avaliação clínica de pacientes com úlceras de membros inferiores divide-se em transcendência e vulnerabilidade, magnitude, objetivo, diagnóstico: classificação de úlceras, tipos, avaliação da úlcera e perilesional. CONCLUSÃO: A elaboração do guia de prática clínica envolveu o detalhamento do processo de avaliação de úlceras de membros inferiores, buscando reunir as melhores práticas e evidências científicas encontradas na literatura. / A leg ulcer is considered a syndrome that is characterized by irregular or circumscribed loss of the integument, which may reach the subcutaneous tissue and other underlying tissues, commonly affects the lower extremities and can be triggered by many factors. Leg ulcers are identified as chronic wounds, considering the long duration of time between the opening and wound healing, plus plenty of recurrence in a short time. AIM: to determine the most effective method of systematic assessment of lower limb ulcers. MATERIAL AND METHODS: A systematic literature review on the basis of data indexed Crochrane, Medline/Pubmed, Lilacs and MedCarib, using the descriptors ulcers, leg ulcers, foot ulcer, protocols , nursing care and diagnosis. The preparation protocol followed the criteria of (GEP- GHC, 2008) Operating Manual of Clinical Guidelines/Protocols Relief of Management Education and Research Hospital Group Conception and Methodology of guides for it Elaboración of atencion y protocols (GOMEZ, 2007). For the analysis of the results was used for the ranking of levels of evidence classification of the Oxford Centre for Evidence Based Medicine and the Classification of the Grading of Recommendations Assessment, Development and Evaluation system strategy. (MECHANICK et al., 2010; DEL MAR; SALISBURY, GLASZIOU, 2010). RESULTS: There were 74 Crochrane Library, 174 in Medline / Pubmed, Lilacs in articles 31 and 7 in MedCarib data. After applying the inclusion and exclusion criteria, in addition to the search filter, 179 articles remained. Articles in duplicate, without the availability of full text, were excluded, as well as after reading the titles and abstracts, leaving only 14 items. The practical guide to the care of the nurses in the clinical assessment of patients with leg ulcers divided into transcendence and vulnerability, magnitude, objective diagnosis: classification of ulcers types of ulcer assessment and perilesional. CONCLUSION: This clinical practice guide involved the detailing of the lower extremity ulcers rating process, seeking to bring together the best practices and scientific evidence in the literature.
