Problematika prevence dekubitů na anesteziologicko resuscitačním oddělení / Problematics of the prevention of pressure ulcers at anesthesiology and resuscitation department

The issue of pressure ulcers is related to limited mobility of P/Cs in bed, and is well known at present. With regard to the permanent increase of surviving P/Cs in critical conditions the prevention of pressure ulcers is a current nationwide topic. Nevertheless, statistical information admits the occurrence of pressure ulcers. The occurrence of pressure ulcers is a rapidly developing nursing problem. This problem has to be taken into account and its prevention has to start from the very beginning of the limited mobility of a P/C in bed.The theoretical part of the thesis is divided into three areas. The first one describes the quality of nursing care in general. The second area is directly focused on pressure ulcers. A chapter dealing particularly with the prevention of pressure ulcers at an ARD extends the last area of the theoretical part. Two goals were set for this thesis: 1. To map the roles of nurses (medical rescue workers) in the prevention of pressure ulcers at an ARD. 2. To map the factors affecting the occurrence of pressure ulcers at an ARD. Four research questions were formulated to achieve the goals: 1. What role does a nurse (a medical rescue worker) play in the prevention of pressure ulcers at an ARD? 2. What factors influence the occurrence of pressure ulcers at an ARD? 3. What information do nurses (medical rescue workers) working at an ARD miss? 4. What problems do nurses (medical rescue workers) working at an ARD have? The empiric part of the thesis was processed by a qualitative method in the forms of the semi-structured interview and involved observation. On the basis of the obtained data the research was extended by a chapter "The Occurrence of Pressure Ulcers", where the data obtained from the head nurses of the involved workplaces was processed and described to summarize the research. The elaboration and the processing of the research brought answers to all four research questions. The results of the research have revealed a number of roles that nurses play. The roles are: the provision of the prevention as such, detection of the first stage of a pressure ulcer, the care of a pressure ulcer already occurred and the prevention of complications. The internal and external factors influencing the occurrence of pressure ulcers are also describer here. The information on the issues of pressure ulcers prevention among nurses and medical rescue workers, the interest in the issues, where the results are surprising in the negative sense, where two thirds of the respondents showed disinterest. We have found that the respondents miss basic information like the classification of pressure ulcers, the information on modern prevention materials and on the healing and treatment of pressure ulcers. Problems in the prevention of pressure ulcers based on two sides were also found out, namely on the side of the management and on the side of the nursing care. On the basis of the circumstances ascertained within the research head nurses from the involved workplaces of the "X" region have been invited to participate in the organization of a seminar for nurses and medical rescue workers working at ARDs, focused on the topic: The issues of the prevention of pressure ulcers at an anaesthesiology and resuscitation department.

Design lůžka pro anesteziologicko-resustitační oddělení (ARO) / Design of Bed for Anesthesiology and Resuscitation Hospital Department

The thesis focuses on analysis and design of a special hospital bed designed for the department of anesthesiology and resuscitation. The design was based on the rules set by the ČSN EN 60601-2-52 norm. The bed is adjusted for the use at the department of anesthesiology and resuscitation not only with its look but also with its functions. The design emphasizes the ergonomics of the bed, safety and the needs of the patient as well as of the staff and also an easy maintenance of the bed. Some of the main innovative elements are: basal stimulation system placed directly in the bed, used colors, more infusion-rack holders or a headboard fixed to the construction of the bed if not needed. The bed was designed especially to provide more comfort to the patient.

Introduction: The dissertation of "The concept of basal stimulation from the point view of nurses working on Anaesthesiology and Resuscitation facility", discusses the matters of specifics and issues when it comes to providing a basal stimulation process within patient's hospitalization in a resuscitation unit. Patients in very complicated health conditions are hospitalized in a resuscitation unit, very often with symptoms of impaired consciousness. The concept of basal stimulation could very likely help patients' communication with other environment, with accepting and perceiving the actual situation as well as with recovering itself. The point of this dissertation is to chart the real and actual problematics of the basal stimulation used within resuscitation units, and also how nurses working in these units are bringing the concept to bear within their practice. Research methods and results: There are qualitative results posted within the empiric part of the research, as well as interviews with ten general educated nurses working in a resuscitation unit. All the results are collated and analysed in the final discussion after literal transcription of all the interviews. This dissertation deals with equipment handling problems of nurses on the Anaesthesiology and Resuscitation Department and training of new nurses for equipment handling. New nurses are therefore overloaded with lots of new information concerning nursing care, documentation, instruments, team etc. This dissertation consists of two parts ? theoretical and empirical one. The theoretical part deals with ARD description, monitoring, technical equipment, work of nurse on ARD and her adaptation process. The empirical part used a qualitative research method. Information was gathered from a semi-structured interview. The current state in context of nurse work with technical equipment on ARD was analysed in the first period of research. Tutorial material was created on the basis of gathered information which was applied in practice. Feedback on created tutorial material was found in the last period. The focus group was created by 10 respondents who work as general nurses on ARD in selected hospitals. Four goals and five research questions were stated during the investigation. The first goal was to map what technical equipment nurses on ARD are working with; the second goal was to find out how the training of new nurses on ARD is carried out; the third goal was to create training material concerning technology, not only for new nurses on ARD; and the fourth goal was to find out feedback on provided tutorial material from nurses on ARD. Research questions were: 1.What technical equipment do nurses operate on ARD? 2. How are new nurses on ARD trained? 3. What means are used during the training of new nurses? 4. According to nurses, what information regarding technology should a handbook for nurses on ARD contain? 5. What opinion do nurses on ARD have of created training material?The first goal was to map what technical equipment nurses on ARD are working with. From research results it is evident that nurses on ARD operate lots of equipment ? monitors, infusion pumps, linear pumps, ventilators, dialysis, suction bottles, defibrillators, laminar boxes, ICP monitors, patient lifters, medical heaters and coolers, nebulizers, plasmaferesis, PICCO, LIDCO, Vigileo, enternal pumps, anti-decubitus mattresses, anaesthetic equipment, EKG, haemoglobinmeters, glucometers, ABR analyser, bronchoscopes and Level. The second goal was to find out how the training of new nurses on ARD is carried out. Research results are different in this area. Every respondent undertook a different form of adaptation process in variously long intervals. The third goal was to create training material concerning technology not only for new nurses on ARD. Training material was created on the basis of discussion analysis with respondents and didactic principles had been kept during its creation (visualisation, connection theory with practise, scientism, adequacy, actuality, feedback, consciousness and activity, individualization and orderliness). The fourth goal was to find out feedback on provided tutorial material from nurses on ARD. Training material was given to all respondents for approximately 14 days and afterwards feedback was obtained. The result was that all respondents liked the training material very much. Research results will be provided to head nurses on ARD from chosen hospitals where the research was carried out. The created training material will be provided to ARD in hospital Pelhrimov, p. o. Specific training material for individual workplaces and equipment will be created for other hospitals on the basis of declared wishes of respondents. We anticipate that the training material will not only be beneficial for new nurses on ARD.

Analýza a inovace vybrané ošetřovatelské dokumentace používanaé na prscovišti intenzivní medicíny / Analysis and inovation selected nursing documentation used in the workplace intenzive medicine

The thesis topic: ?Analysis and Innovations of Selected Nursing Documentation Used in Intensive Care? was chosen deliberately for its topicality. Documentation is an important and integral part of treatment of patients. It is very important that nursing documentation suit nurses who work with it. Data should be clear and their recording easy and convenient for nurses. Duly managed nursing documentation should be beneficial for physicians as well. For the research, the anaesthetic resuscitation department in Jihlava was chosen. The thesis contained six defined objectives that have been met and resulting 6 research questions. Qualitative research was carried out in 3 stages. One research group consisted of nurses at the anaesthetic resuscitation department in Jihlava, the other research group consisted of doctors at the anaesthetic resuscitation department in Jihlava. In the first stage, an analysis of nursing documentation used at the anaesthetic resuscitation department in Jihlava was carried out and, subsequently, interviews with the nurses and doctors from this department were conducted. In the second phase, innovations of nursing documentation were made and the documentation was put into practice. The final stage of the thesis consisted of interviews with the same questions for the doctors and nurses at the anaesthetic resuscitation department in Jihlava, in which opinions of the nurses and doctors on the innovated documentation and its application in practice were surveyed. The objective was to analyze the existing nursing documentation, to innovate it on the basis of the interview results, to implement it, and then to find out whether it could be used in practice. Based on the interviews conducted, we found out that nurses do not like the nursing diagnoses and also record sheets, which are common for doctors and nurses, used their department. Based on these results, innovation of the nursing diagnoses was made and, at the same time, the department made an innovation of the recording sheet, and medical and nursing documentation was separated. After the documentation was introduced into practice, we interviewed the nurses and doctors again to find out whether the innovated documentation was better and whether it could be improved more. We found out that the documentation was better, more suitable for the respondents. The innovated documentation can be used in practice and is currently so at the anaesthetic resuscitation department in Jihlava.
