Psychosocial stress and health-related outcomes in chronic childhood asthma : using a biopsychosocial approach to understand transactional relationships across childhood and adolescence

[Truncated abstract] Despite significant scientific advances in tracking the complex physiological mechanisms that drive the asthma disease process, worldwide trends in childhood asthma continue to rise. This research sought to describe the relationships between psychosocial stress, psychosocial resources, asthma severity, and health-related outcomes from the standpoints of biopsychosocial and developmental theory. The research consisted of three studies based on a prospective study involving 2573 children from a community-based birth cohort. The cohort has been under active follow-up from birth and this thesis draws on data obtained at the 1, 2, 6, 10 and 13 year follow-ups . . . The final study undertook to describe the mediating influence of specific supportive behaviours provided by family members and peers to adolescents with asthma. Ninety-nine adolescents participating in the 13 year follow-up of the community cohort study completed a semi-structured interview adapted from the Diabetes Social Support Interview (La Greca et al., 1995) to assess supportive behaviours and the KINDL (Ravens-Sieberer & Bullinger, 1998) to assess QOL. It was predicted that illness-specific support would mediate the relationship between family dysfunction and quality of life. Qualitative analyses identified parents as important sources of tangible support, peers as important sources of companionship and emotional support, and siblings bridging the two, by providing tangible, companionship and emotional support to the adolescent. Mediator analyses found that specific parent behaviours perceived as unsupportive by the adolescent mediated the relationship between family dysfunction and self-oriented quality of life. Clinical implications support the integration of medical and psychological expertise in the treatment of asthma. Future research directions are also discussed.

A pesquisa é definida como descritiva, tendo utilizado metodologias quantitativas e qualitativas. O modelo conceitual que norteou esse trabalho foi o biopsicossocial e organizacional (BPSO) de Limongi-França (1996). O instrumento utilizado na pesquisa foi idealizado pela mesma autora e adaptado para o contexto do serviço público. Também foram utilizadas como técnicas de coleta de dados a entrevista semiestruturada e a análise documental. A análise quantitativa foi realizada por meio de análises descritivas, análise fatorial exploratória e confirmatória, enquanto os dados qualitativos foram analisados através da análise de conteúdo. Os resultados identificaram que os servidores se encontram moderadamente satisfeitos, com resultados significativamente diferentes entre as instituições. O confronto da percepção dos servidores com as práticas de gestão apontaram um desalinhamento entre as expectativas dos servidores e o que é possível ser ofertado pelas instituições, reforçando a importância de um diálogo. Existe um maior interesse na temática Qualidade de Vida no Trabalho por parte dos setores ligados à saúde do servidor, porém ainda é limitada à influência da Política de Atenção à Saúde e Segurança do Trabalho do Servidor Público Federal, ao passo que a Política Nacional de Desenvolvimento de Pessoal mostrou uma influência mais abrangente. Evidencia-se a ausência de um programa em Qualidade de Vida no Trabalho nos dois órgãos, mas os resultados mostram caminhos, apontados pelos próprios servidores, a serem tomados nesse sentido. Ressalta-se que o tema ainda se manifesta de forma ocasional em práticas pontuais, indicando a importância de a Qualidade de Vida no Trabalho avançar como um tema estrategicamente pensado e comum a toda instituição e reforça a importância de estudos que dirimam os entraves para a construção coletiva de um modelo em Qualidade de Vida no Trabalho na administração pública, e que contemplem satisfatoriamente todas as dimensões estudadas. / This research aimed to identify the perception of administrative federal employees in education about the Quality of Life at Work (QLW) in the Universidade Federal do Vale do São Francisco and Instituto Federal de Educação, Ciência e Tecnologia do Sertão Pernambucano contexts. This research is defined as exploratory and descriptive by using quantitative and qualitative methodologies. The conceptual model that guided this work was the biopsychosocial and organizational (BPSO-96) from Limongi-France (1996). The instrument used in the research was designed by the same author and adapted to the public service reality. Moreover, they were also used as data collection techniques the semi-structured interviews and document analysis. Quantitative analysis was performed using descriptive statistics, exploratory and confirmatory factor analysis, while qualitative data were analyzed using content analysis. The results showed that the federal employees are moderately satisfied, but also pointed to a significantly difference in the results between the two institutions. The confront of the federal employees' perceptions about management practices indicated a misalignment between the expectations of these employees and what can be offered by the institutions, reinforcing the importance of a dialogue. There is a greater interest in the subject Quality of Life at Work by the sectors related to employee's health, but the Health Care Policy is still limited to them, while the National Personal Development Policy showed a wider influence. The absence of a Quality of Life at Work program in the two institutes is evdente, but the results show paths indicated by the employees themselves to be taken accordingly. It is emphasized that the issue also manifests itself occasionally on specific practices, indicating the importance of Quality of Life at Work to advance as a strategically thought out theme common to every institution and reinforces the importance of studies that settle barriers to the collective construction of a Quality of Life at Work model in public administration and that satisfactorily address all of the studied dimensions.

Para conceituá-la, inicialmente, se utilizou o modelo exclusivamente médico lastreado em limitações físicas; passo seguinte se adotou o modelo social, cujo enfoque era a opressão social sofrida pelas pessoas com deficiência; e, atualmente, o modelo utilizado é o biopsicossocial, analisando-se a pessoa com deficiência por intermédio de perícia médica e social, conjugando a análise dos impedimentos corpóreos de longo prazo, frente às barreiras sociais, ambientais e atitudinais existentes. Para as legislações que usam o modelo médico restritivamente, o possuidor de visão monocular não se enquadra no conceito de pessoa com deficiência. Contudo, por todos os modelos, o médico, social ou biopsicossocial, é possível se considerar a pessoa com visão monocular como pessoa com deficiência visual. Nesse contexto, e com iguais propósitos, tem-se que analisar o conceito jurídico-constitucional da pessoa com deficiência visual, para fins de reconhecimento do direito ao gozo da aposentadoria especial nesta condição, aos possuidores de visão monocular. Apesar de serem possuidores de Cegueira ou Deficiência Visual Grave ou Moderada por critérios médicos, têm sido excluídas do acesso a vários direitos, inclusive desta espécie de aposentadoria, vez que, regra geral em termos legais expressos, não são considerados pessoas com deficiência visual. O objetivo deste trabalho, portanto, é analisar a evolução desses modelos e aferir o grau de proteção social hoje conferido à pessoa com deficiência visual, em especial no caso da visão monocular, principalmente no que concernente à concessão de aposentadoria especial da pessoa com deficiência. / According to the census done by Brazilian Institute of Geography and Statistics in 2010, was identified that 18.8% of the national population are persons with, at least, one kind of visual impairment. Before this expressive number, ifigures out that, it has been important to the legal word, the concept analisys of a person with visual impairment and its modifications along the time, specially, when, in the forensic and administrative social security praxis, it is noted a segregation of detaining persons with monocular vison deficiency at this legal concept. There is even, a legislation in force version, that it is still giving a concept surpassed of what, at the beginning, was used as an exclusively backed medical model considering the person with visual impairment. To concept it, the physical limitations; the next step is adopted a social model, whose approach was based in the social oppression suffered by the persons with deficiency; and, actually, this used model is the biopsychosocial, analysing the person with this disable throughout the social and medical expertise, joining the longterm bodily impediments analisys, facing the social, environmental and existing attitudinal barriers. For the legilations that use a restrict medical model, the monocular vision possessor does not fit as a person with deficiency. However, for all the kinds of models, like medical, social or biopsychosocial, it is possible to consider a person with monocular vison as one with visual impairment. In this context, and with same purposes, there is a way to analyse the conditions of a visual impairment person and the monocular vison possessor legal-constitutional concept, to recognize the rights of a special retirement enjoyment. In spite of being Severe or Moderate Vision or Blindness possessor by medical criteria, it has been excluded of many right accesses, including this kind of retirement, once that, the general rules, in expressive legal therms, are not considered persons with visual impairment. The purpose of this research is, therefore, to analyse these models evolution and to assess the degree of social protection to a person with visual impairment nowadays, in a special case, the monocular vision one, mainly in relation to the granting of special retirement of the disabled person.

Psychosocial factors involved in injuries sustained in long-distance running

The purpose of the study was to explore the psychosocial factors involved in the susceptibility, experience and rehabilitation of injuries sustained in long-distance running in order to develop a biopsychosocial theoretical model that will help explain the experience of injury and the successful rehabilitation thereof in long-distance running. The study was approached from a biopsychosocial theoretical perspective. A mixed methods research design with the purpose of expansion and using 15 case studies was employed. Purposive sampling with a snowballing effect was used to select the 15 long-distance runners who were injured or had recently recovered from injury. Semi-structured interviews were conducted with each of the participants. Furthermore, each participant completed the Myers-Briggs Type Indicator. An examination of the participants’ experiences of injury reveals that injury has an effect on and is affected by the following factors: running history and training program; perceived benefits and disadvantages of running; personal understanding of injury; personality factors; psychological responses to injury; perceived causes of injury; approach to rehabilitation; and coping mechanisms. Furthermore, there is an inherent interplay of biological or physical, psychological and social processes involved in the experience of injuries sustained in long-distance running. Based on the findings of the study, the biopsychosocial model of long-distance running injuries is proposed. Although the model emanates from the experiences of the 15 participants in the study, it may be applied to long-distance runners in general. A long-distance runner’s experience of running is the core of the model. The model depicts how each of the factors noted previously has an effect on and is affected by injury; thus, showing the integral relationship between injury and each of the factors. Furthermore, the biopsychosocial nature of the model is also portrayed in the model. A pilot exploration of the relationship between temperament and psychopathology in 12-18 year-old children born at extremely low birth weight

The aim of this study was to explore temperament, psychopathology and quality of life (QOL) in adolescents born at extremely low birth weight (ELBW), i.e., < 1000g. ELBW adolescents (N = 15) completed the Revised Cheek and Buss Scale and Mini International Neuropsychiatric Interview 5.0.0 (M.I.N.I.), while their legal guardians completed a biographical questionnaire and the Short Form 12 version 2. The median age of the sample was 13 SD = 2.526 years (60% male) and all participants spoke English. ELBW adolescents with social phobia (n = 6) were more shy (p = .041) and had poorer mental health-related QOL (p = .041) than those without such symptoms. The results suggest that ELBW could be a predisposing factor for increased shyness and psychological disorders including social phobia. Planning of interventions for ELBW individuals should therefore include strategies to prevent or mitigate the effects of these factors in adolescence / Psychology / M. A. (Clinical Psychology)

<div> <div> <div> <p>The current study examines the relationships between frequency of yoga practice and outcomes of self-compassion, couple satisfaction, and family functioning. Yoga and other forms of Eastern medicine have become increasingly popular in Western culture. Not only has yoga become more appealing to the general population, it has also become more widely accepted and has been more frequently integrated into various mental health treatments. Using a cross-sectional design, this study analyzed data from an online questionnaire regarding systemic outcomes of yoga participants (N = 115). A three-step hierarchical regression analysis was completed to test significance between predictor and outcome variables. The results showed a significant relationship between social reason for practicing yoga and family functioning. This study indicates that families who practice yoga together may have healthier family functioning. The results highlight the potential of yoga as a therapeutic intervention for clinicians working with families.<br> </p> </div> </div> </div>

BakgrundPatellatendinopati (PT) är förekommande hos personer som tränar rekreativt och bland elitidrottare men förekomsten bland inaktiva är okänd. Forskningen på PT har huvudsakligen varit fokuserad på de fysiska begränsningarna av tillståndet med viss framgång på utfall som smärta och funktion genom olika träningsprotokoll. Men PT tenderar att bli långvarigt och långt ifrån alla blir bra. Internationellt vetenskapligt konsensus inom tendinopati menar att det saknas kunskap om tillstånden ur ett bredare biopsykosocialt hänseende. Hittills har såvitt studieförfattarna vet, inga studier utforskat vilka erfarenheter personer med PT har av att leva med och rehabilitera besväret.SyfteAtt utforska erfarenheterna av att leva med och rehabilitera PT hos personer som rekreativt tränar/idrottar.MetodDetta är en deskriptiv intervjustudie med semi-strukturerade intervjuer där en kvalitativ induktiv ansats användes. En tematisk analys utfördes med en semantisk och induktiv inriktning. Totalt rekryterades tio deltagare.ResultatFem huvudteman identifierades ur datan: (1) Hot mot identiteten, (2) Att inte kunna leva sitt liv fullt ut, (3) En ogynnsam relation till smärta, (4) Att känna makt över sin situation återgav kontroll (5) Avgörande för rehab var att se helheten och inte bara ett knä.SlutsatsPT visar sig kunna vara ett tillstånd som innehåller ett större lidande utöver de uppenbara fysiska begränsningarna. Bland personerna som deltog framkom flertalet negativa kognitiva och emotionella erfarenheter och maladaptiva beteenden i relation till smärtan. Resultaten kan argumenteras ha en viktig klinisk innebörd. Omhändertagandet av PT bör ske personcentrerat med ett förhållningssätt förankrat i att smärta har en biopsykosocial inverkan. Av metodologiska skäl ska överförbarheten till en bredare population dock tolkas med viss försiktighet. / BackgroundPatellar tendinopathy (PT) is common within an active population, especially elite athletes, but less is known about the presence among sedentary people. Previous research has focused on the physical impairments with modest effect on outcomes like pain and function through different training protocols. Nonetheless, PT tends to be persistent and many fail to recover. Current international consensus highlights the lack of knowledge of tendinopathy through a biopsychosocial perspective. No studies have explored people with PT and their experiences with living and rehabilitating the condition.PurposeTo explore the experiences of living with and rehabilitating PT among people who exercise or do sports recreationally.MethodA descriptive interview study with semi-structured online interviews was performed. A total of 10 participants participated. A thematic analysis was performed with a semantic and inductive procedure.ResultsFive main-themes were identified from the data: (1) Threatened identity, (2) Not being able to live your life to the fullest, (3) A adverse relationship with pain, (4) To feel in charge over ones situation brought back control (5) To be seen as a whole and not just a knee was crucial to rehab.ConclusionPT has a larger impact beyond the physical impairments. Negative cognitions and emotions coupled with maladaptive behaviours in relation to the pain emerged to a considerable degree. These results could have an important clinical message, that the management of PT would benefit from being person-centred where the biopsychosocial impact of pain is acknowledged. Caution should be taken in regards to this study's transferability to a wider population due to methodological aspects.

The biopsychosocial factors influencing HIV/AIDS patient adherence to antiretroviral therapy (ART) : a social work study

The study emanates from the need to identify the biopsychosocial factors that influence patients’ adherence to antiretroviral therapy (ART) within the South African context The specific goal of the study was to explore these in order to make recommendations to enhance service delivery. Applied research was conducted, with its primary task being to stimulate thought and action concerning the challenges faced by patients who are on ART. In order to gather comprehensive data, the researcher engaged in a combination of the qualitative and quantitative approaches. For the qualitative case study the researcher made use of semi-structured interviews, utilizing the non-probability sampling method, aiming to understand and interpret the meaning that the multidisciplinary team accorded to matters of antiretroviral treatment. For the quantitative part of the study the probability random sampling method was made use of for the quantitative descriptive survey. Questionnaires were employed to collect data from 201 patients already on antiretroviral medication. The conclusions, which were drawn from the research findings, identified challenges to adherence to ART: the study confirmed that since the advent of combination antiretroviral therapy (HAART), HIV/AIDS has been transformed into a manageable and chronic condition, and has undoubtedly extended and improved the quality of life for people living with HIV/AIDS. However, it also confirmed that ART, is a complex intervention, which is accompanied by severe biopsychosocial implications, requiring near-perfect adherence in order to prevent the development of resistance. The impact that the various psychosocial needs of millions of HIV/AIDS people living on ART will have on current social structures and services, will tax the available professional social services, particularly the social work profession. The social correlation of HIV/AIDS and poverty is endorsed by the findings, confirming that the high level of unemployment, coupled with families who are headed by women and who receive little support, lead to almost total dependency on social security. The findings further indicate a specific relationship between socio-economic circumstances and the ability to adhere to ART. Empowering HIV/AIDS patients, to be able to adhere to ART, is therefore indicated, as is the further need for a regulator of HIV/AIDS support services, in order to protect and promote high standards of service delivery, especially counselling. / Thesis (DPhil)--University of Pretoria, 2007. / Social Work and Criminology / DPhil / unrestricted
