Estresse laboral na equipe de enfermagem do setor de emergências de um hospital no interior do estado de São Paulo e suas repercussões / Occupational stress in nursing staff in the emergency room of a hospital in the state of Sao Paulo and its repercussions.

Segundo a literatura os hospitais de emergência sofrem, mais do que outras unidades, os efeitos da falta de investimento no sistema público de saúde e de sua crise de governabilidade. O estresse tem despontado como fator desencadeante de doenças ocupacionais, prejudicando todos os atores envolvidos na assistência à saúde, pensando nisso, decidimos realizar uma pesquisa quantitativa e descritiva, realizada como estudo de caso em um hospital estadual no interior de São Paulo que atende Urgências Clínicas e Psiquiátricas, com o objetivo principal de: identificar possíveis causas do estresse entre trabalhadores de enfermagem das alas de emergência e comparar os estressores da Sala de Urgência (S.U.) e Emergências Psiquiátricas (P.Q.U.). O referencial teórico utilizado é o de Robert Karasek sobre demanda-controle de estresse. Na metodologia utilizamos a observação de campo, e após, aplicamos o instrumento Job Content Questionnaire. Encontramos Alpha de Conbrach negativo para três dimensões: DA (Autoridade decisória), PsD (Demanda psicológica) e JI (Insegurança no trabalho), e relacionamos isso com a inadequação transcultural da tradução da escala para a realidade brasileira, principalmente no que tange a estabilidade empregatícia. Na comparação entre os setores observamos que o setor S.U. apresentou escores mais altos para PsD (Demanda psicológica), PhD (Demanda física), CT (Controle no trabalho) e SD (Uso de habilidade). O Suporte Social foi maior para o setor P.Q.U. Concluímos que são necessários novos estudos com o mesmo objetivo para comparações e ainda sugerimos uma revisão da tradução da escala para o português com adequação a nossa realidade empregatícia. / According to the literature the emergency hospitals suffer more than other units, the effects of lack of investment in the public health system and its governability crisis. Stress has emerged as a triggering factor of occupational diseases, affecting all stakeholders in health care, thinking about it, we decided to perform a quantitative and descriptive study, conducted as a case study in a state hospital in São Paulo that meets Emergency Clinics Psychiatric and with the main objective to: identify possible causes of stress among nursing staff in wards, emergency and compare the stressors Room (ER) and Psychiatric Emergencies (PQU). The theoretical framework used is that of Robert Karasek demand-control over stress. Methodology used the field observation, and after we apply the instrument Job Content Questionnaire. Alpha Conbrach found negative for three dimensions: DA (Decision authority), PSD (Psychological demand) and JI (Job insecurity), and relate this to the inadequacy of cross-cultural translation of the scale for the Brazilian reality, especially regarding the stability employment. Comparing the sectors noted that the sector SU showed higher scores for PSD (Psychological demand), PhD (Physical demand), CT (Job control) and SD (Use skill). Social Support was highest for the sector P.Q.U.. We conclude that further studies are needed with the same goal for comparisons and further suggest a revision of the scale of the translation into Portuguese with our reality employment suitability.

Clima laboral y síndrome de Burnout en personal de una empresa agente de aduanas

Publicación a texto completo no autorizada por el autor / Estudia la relación existente entre las variables clima laboral y el síndrome de Burnout en los trabajadores de una empresa agente de aduanas, el estudio es de tipo descriptivo con un diseño descriptivo correlacional. El tipo de muestreo fue no probabilístico intencionado, con la participación de todo el personal de la empresa, siendo el total 110 participantes de ambos géneros, de diferentes grupos ocupacionales. Los instrumentos psicológicos utilizados en la medición de las variables de estudio fueron la Escala de clima laboral de Sonia Palma Carrillo (SPC-2004) y el Inventario Burnout de Maslach y Jackson (1981). Los resultados del estudio permitieron apreciar que el clima laboral de la empresa agente de aduanas es percibido por la mayoría de los trabajadores como favorable y muy favorable, por otro lado, el análisis del MBI señaló que la muestra reportó, de manera mayoritaria, nivel medio en la escala de cansancio emocional, nivel bajo en despersonalización y nivel bajo en realización personal. Respecto a la correlación entre variables, se encontró que el clima laboral. Tiene una relación estadísticamente significativa, negativa y media con la dimensión cansancio emocional del síndrome de burnout; además se relaciona significativa, negativa y moderadamente con la dimensión despersonalización del síndrome Burnout y finalmente se asocia de manera significativa, moderada y positivamente con la dimensión realización personal del síndrome Burnout. / Tesis

According to the literature studied, it is perceived that new forms of production and work organization were elaborated, with increasing changes in working conditions and forms. Faced with these changes, workers' health risk factors are increasingly close and real. Specifically in the professional practice of teaching activity, several psychosocial stressors are present, among them the dissatisfaction, the anguish, the demotivation, the fatigue, which can cause Burnout Syndrome. This reality provokes the need to implement a new logic for human existence in institutions, especially in educational institutions. This modern man is fragmented and directed especially to productivity and consumption. Hence, the need to look carefully at the work environment in order to promote health, providing a quality of life (QoL) favorable to collective health. Considering this new reality in the work world, this qualitative study, using the Focus Group technique (FG) and content analysis with the objective of analyzing the stress situations triggering Burnout Syndrome experienced by teachers of basic education of private confessional schools in Fortaleza, seeking to identify signs of Burnout Syndrome, as well as to know institutional actions directed to the promotion of health and quality of life of these teachers. As well as, to understand how teachers deal with this situation of stress and how they seek to overcome it. The findings obtained regarding the conception of professional stress and Burnout Syndrome expressed by teachers, related to the objective proposed in the research project, demonstrate fundamentally, the relevance given by the investigated teachers to the view that they need institutional actions directed to health promotion. / Ao longo da história, sabe-se a importância que o trabalho ganhou para a vida humana enquanto possibilidade de inserção das pessoas no mundo social e profissional. De acordo com a literatura estudada, percebe-se que foram elaboradas novas formas de produção e organização do trabalho, ocorrendo mudanças crescentes das condições e formas de trabalho. Diante dessas mudanças, os fatores de risco para a saúde dos trabalhadores estão cada vez mais próximos e reais. Especificamente no exercício profissional da atividade docente, diversos estressores psicossociais encontram-se presentes, dentre eles destacam-se a insatisfação, a angústia, a desmotivação, o cansaço, os quais podem ocasionar a Síndrome de Burnout. Essa realidade provoca a necessidade de se implementar uma nova lógica para a existência humana nas instituições, especialmente nas instituições de ensino. Esse homem moderno está fragmentado e direcionado especialmente à produtividade e ao consumo, por isso a necessidade de olhar cuidadosamente para o ambiente de trabalho, a fim de promover a saúde, proporcionando uma qualidade de vida (QV) favorável à saúde coletiva. Considerando essa nova realidade no mundo do trabalho, este estudo de abordagem qualitativa, utilizando-se da técnica de Grupo Focal (GF) e da análise de conteúdo, tem o objetivo de analisar as situações de estresse desencadeadoras da Síndrome de Burnout vivenciadas pelos professores da educação básica de escolas confessionais privadas de Fortaleza, buscando identificar indícios da Síndrome de Burnout, como também conhecer ações institucionais direcionadas à promoção da saúde e qualidade de vida desses professores, bem como compreender de que forma os professores lidam com essa situação de estresse e como buscam superá-la. Os achados obtidos concernentes à concepção de estresse profissional e Síndrome de Burnout externada pelos professores, relacionada ao objetivo proposto no projeto de pesquisa, demonstram, fundamentalmente, a relevância dada pelos professores investigados à visão de que necessitam de ações institucionais direcionadas à promoção da saúde.

Burnout, depression and job satisfaction in acute psychiatric and secure mental health settings

Chapter 1: Objective: The systematic review aimed to review the literature on burnout and its relationship to depression within the acute in-patient mental health services: psychiatric units and specifically, secure forensic mental health services. Methods: The review process included a systematic search across five databases (Medline, PsychINFO, Cinahl Plus, EMBASE and SCOPUS). Eligible studies included a cross-sectional design, using validated measures on burnout and depression. Results: A strong relationship between depression and emotional exhaustion was found. The relationship between depression and two other burnout dimensions (personal accomplishment, depersonalisation) was weaker and better explained in the context of other predicting (anxiety) and mediating (transformational leadership) variables. While depression severity across the studies was mostly mild with average burnout, service-specific variations were observed. Chapter 2: Objective: The empirical study aimed to explore any direct relationships of subjectively perceived understanding, predictability, control (job demands) with burnout and job satisfaction, and direct/in-direct effects of social support, psychological mindedness and psychological inflexibility (external and internal resources) on these relationships. Methods: Data was collected among Scottish National Health Service (NHS) employees (n=198) working in secure mental health services; forensic (58.65%) or intellectual disability (41.35%). Data gathered from the final sample of 141 nursing staff was analysed using t-tests, bi-variate correlations, hierarchical regressions and a series of mediation, moderation and moderated-mediation analyses. Results: The empirical study revealed that individual burnout dimensions were predicted by different job demands. Social support appeared as predictor rather than a moderator of job satisfaction and emotional exhaustion while psychological inflexibility was a mediator for job demands and burnout. Overall Conclusions: Concluding remarks for both, systematic review and empirical study, identify the need for further research, especially within the forensic mental health speciality. Both highlight that direct and in-direct effects may be important in explaining burnout while the empirical study makes further suggestion with regards to likely individualised pathways and two important resources of social support and psychological flexibility.

La presente investigación tuvo como finalidad determinar si existe diferencia en el nivel de satisfacción laboral y de síndrome de burnout entre el personal de un Policlínico y en el de una Comunidad Local de Administración de Salud, distrito La Victoria, Chiclayo – 2012, además, se complementó el estudio indicando el nivel en que se halló al personal, respecto a cada variable. Para ello, se aplicaron dos instrumentos a 92 trabajadores, la Escala de Satisfacción Laboral de Palma y el Inventario Burnout de Maslach, simultáneamente una encuesta y un consentimiento informado. La investigación arrojó que no existe diferencia entre el personal de ambas instituciones de salud en relación a ambas variables, hallándose niveles regulares de satisfacción laboral en las dos poblaciones y niveles bajos de síndrome de burnout. / Tesis

Esta investigación tuvo como propósito determinar las diferencias que existen entre los niveles de las dimensiones del Síndrome de Burnout entre el personal técnico de enfermería de dos centros hospitalarios estatales de las entidades MINSA y ESSALUD. Es un estudio de tipo comparativo y se trabajó con una población conformada por 100 técnicos del hospital estatal de la entidad MINSA y 100 técnicos del hospital estatal de la entidad ESSALUD (áreas de recuperación, esterilización, consultorios externos y emergencia). El instrumento utilizado fue el Inventario de Burnout de Maslach. Se encontró que existen diferencias significativas en la dimensiones de cansancio emocional y despersonalización, siendo los técnicos de enfermería del hospital estatal de la entidad MINSA los que alcanzan un mayor porcentaje en el nivel alto en las mencionadas dimensiones, sin embargo, no se encontraron diferencias significativas en la dimensión de realización personal. / Tesis

Burnout and Psychological Capital in Rural Critical Access Hospital Nurses

abstract: Job burnout, a prolonged reaction to job stress, includes mental and physical aspects of exhaustion related to professional work life. Linked to individual health-related problems, decreased job satisfaction, poor organizational commitment, and higher turnover, burnout poses a problem for both employees and organizations. The nursing profession identifies the prevalence of burnout and the resulting harmful effects in many settings, yet until now, rural critical access hospital settings have not been considered. To build and maintain a competent, healthy rural nursing workforce that responds innovatively to growing healthcare needs, it is important to examine burnout levels in rural nurses and to identify factors that might be associated with mitigating burnout. This study focuses on how psychological capital, socio-demographic and organizational work-related factors are associated with burnout in this population. This cross-sectional, descriptive correlational study employed the Maslach Burnout Inventory for Health Professionals, the Psychological Capital Questionnaire, and a sociodemographic questionnaire assessing individual and organizational work-related factors as self-report tools. Descriptive statistics, correlations, and regression analyses were performed to assess aspects of the nurses’ work environment, while describing the relationships among the variables.Means and standard deviations were examined across key variables and compared to reports from other studies. Hypotheses predicted psychological capital would be associated with burnout (negatively associated with emotional exhaustion and depersonalization, positively associated with personal accomplishment), and that individual sociodemographic and organizational work-related factors would also be associated with BO. It was further hypothesized that PsyCap would moderate the relationship between work-related factors and BO. Maslach Burnout Inventory results reveal similar findings to those in the global sample. However, levels of emotional exhaustion and professional accomplishment were greater in our rural nurse sample compared to published values. Higher levels of psychological capital were found to be related to decreases in depersonalization and correlated to greater professional accomplishment. Psychological capital was not found to moderate associations within this study. Intent to stay more than one year had a strong, negative correlation with emotional exhaustion. Se trata de un estudio descriptivo, transversal y relacional sobre Burnout, calidad de vida laboral y autocuidado en enfermeras que atienden y cuidan enfermos crónicos, terminales y complejos en los hospitales del sector público de Tamaulipas. La muestra no fue aleatoria. Estuvo compuesta por 185 profesionales de la enfermería que trabajan en siete hospitales del sector público de Tamaulipas. Además participó un grupo de enfermeras que trabajan en centros de salud de atención primaria. Esta investigación se justificó por el efecto en carga de trabajo y estrés que está teniendo el incremento de enfermos crónicos, terminales y complicados en unidades médicas presionadas por la demanda de servicios de consulta y hospitalización. Esto condiciona, que al ubicar como prioridad la atención del enfermo, se deje al margen la calidad de vida de los profesionales de la salud que prestan tal servicio, particularmente las enfermeras. El objetivo de la investigación es identificar es estado y características del Burnout como una de las expresiones de ese estrés que caracteriza la actual carga trabajo del personal de enfermería, así como de medir su asociación con factores relacionados a la calidad de vida profesional y el autocuidado. Se utilizaron tres instrumentos: el MBI de Maslach, la Encuesta de Calidad de Vida Profesional de la Secretaria de Salud de México y un test sobre autocuidado. La encuesta piloto, realizada en un hospital de Cd. Victoria, reportó un alfa de Cronbach de 0.82. La consistencia interna de la investigación en el grupo estudiado fue de 0.708. La prevalencia del Burnout en el grupo estudiado fue del 34.5%. Las variables que mostraron una asociación significativa con Burnout, fueron el estado civil, ser madre soltera, trabajar turno nocturno, trabajar más de 8 horas, la carga laboral, la percepción de la Calidad de Vida Profesional y el agotamiento emocional.
