Consultants' and consultees' perceptions of the skills and characteristics important to the effectiveness of special education consultation in British Columbia

The purpose of this study was to replicate a study on consultation skills, done by Knoff, Hines, and Kromrey (1995) while expanding the sample base to include teaching assistants and parents. The Consultant Effectiveness Scale (CES), a questionnaire consisting of 75 consultant skills and characteristics, was used to survey the perceptions of 75 consultants and consultees (18 consultants, 23 teachers, 17 teaching assistants, and 17 parents) regarding the degree to which they believed each skill was important to the effectiveness of consultation. Results were examined for differences across groups, for consistency with the factor structure obtained by Knoff, Hines, and Kromrey (1995), and to determine the impact of demographic variables. A follow-up telephone interview was conducted with 2 consultants, 3 teachers, 3 teaching assistants, and 3 parents to verify questionnaire results. The ratings of respondent groups were compared and contrasted across the top 15 ranked items and several consistencies were found to exist across groups. Parents were found to have rated all items generally lower than consultants or teachers. A factor analysis was completed using the 52 items from the original four factors (Knoff, Hines, & Kromrey, 1995) and resulted in a high degree of consistency with the original factor structure generated by Knoff, Hines, and Kromrey. A one-way ANOVA was conducted and significant differences were found on three of the four factors with the consultants rating the items higher than the parents (p <.05). No significant differences were found across demographic variables. The results of the telephone interviews were consistent with the high ratings of many of the interpersonal and relationship-building skills and content skills identified in the questionnaire responses. Recommendations are made calling for future research to further validate the scale, and to confirm differences between respondent groups. The value of the scale and its contents are highlighted in the context of preservice and in service training for consultants. / Graduate

Evaluating the introduction of Team-Based Learning in a pharmacy consultation skills module

Yes / The learning and teaching strategy for a pharmacy consultation skills module was changed to Team-Based Learning (TBL) with the aim of motivating students to engage with course material pre-class and take deeper approaches to learning during class. Results from administering a validated TBL instrument suggests that students are more accountable and are satisfied with and have preference for TBL over traditional methods. Exam results show a 13% improvement in mean end-of-year examinations compared with pre-TBL results. Thematic analysis of written comments on the module evaluation survey suggest that they enjoyed learning using TBL and found it more engaging, stimulating and more effective for their learning; however, it could be improved through better management of workshop timings and more effective facilitation of discussion. TBL appears to have potential as a pedagogic approach in pharmacy education.

Empathy : its significance and monitoring in the dietetic consultation for chronic disease management

Communication between the patient and healthcare professional is pivotal in enabling effective self-care management to occur (Street et al., 2009) which in turn leads to improved health outcomes (Kravitz et al., 1993). However, there is little published data exploring the most effective tools for evaluating whether these communication skills occur and what particular attributes enhance the process (Goodchild, Skinner &amp; Parkin, 2005; Heisler et al., 2003; Parkin &amp; Skinner, 2003). This study aims to identify patient-centred communication skills occurring in consultations and to explore their link with a tool-recording agreement on reported decisions made. Quantitative analysis was conducted on 86 dietetic consultations across four outpatient diabetes services. Audio recordings from 20 of these were qualitatively analysed. Greater agreement on reported decisions correlated significantly with level of empathy demonstrated (t = .283, p = .0005). In consultations featuring agreement, dietitians expressed more empathy (p = .02), used more active listening skills, asked significantly more exploratory questions on self-care practices (U = 18.5, p = .007), provided significantly more supportive/collaborative information-giving exchanges (U = 11, p = .003) and were more likely to set an agreed agenda at the start of consultations. In contrast, consultations featuring disagreement had low levels of empathy, fewer active-listening skills and exploratory questions, significantly greater numbers of persuasive information-giving exchanges (U = 17, p = .007) and more recommending exchanges. Generally, agendas were not set. In conclusion, recording patient/healthcare professional agreement on reported decisions made during a consultation is a simple tool that can indicate the presence of patient-centred communication skills. Active-listening skills allow expressions of empathy that facilitate patient involvement and interactive dialogue. The measure of agreement should be used frequently as a marker of effective dietetic consultations and to provide further data on the relationship between patient-centred communication and implementation of behaviour change for improved health outcomes.

Improving community pharmacy consultations for people with depression

Aims The aims of this study were to increase community pharmacists’ willingness and confidence to provide consultations for people with depression, and to enhance patients’ awareness of the pharmacists’ developing role. Research Design To observe pharmacist-patient consultations, the researcher developed a patient scenario. Pharmacist knowledge and attitude questionnaires were adapted, and a skills observation checklist was developed. Assessments of patient satisfaction levels took place before and after pharmacist training, which included a simulated consultation and action planning. The consultations were both video recorded and observed. Participants undertook a short interview with the researcher and each pharmacist developed his/her own plan for continuing professional development (CPD). An exploration of the interview transcripts was undertaken qualitatively. A University Ethics Panel approved the project. Results There were twenty-two pharmacists who took part in the study, comprising eighteen males and four females. MPharm students comprised one female and two males, and community pharmacists made up three females and sixteen males. The quantitative results: It was found that pharmacists possessed appropriate knowledge (the mean score was 75%, which showed that the pharmacists were aware of the safety and action of anti-depressants). The mean score for attitude was 54%, which tends towards the positive. When observing the simulated consultations, the mean score for initiating a consultation session was recorded at 28%. This indicates that the pharmacists were not very interested in initiating rapport with patients. However, the highest mean score calculated was 61%, which corresponded with closing a consultation. For data collection and action, the mean scores were 42% and 35%, respectively, and this indicates the need for improvement in these areas. The qualitative findings: The pharmacists demonstrated good knowledge about anti-depressants and held positive attitudes towards people with depression. However, the pharmacists were not very willing to exercise the responsibilities of their extended role or provide additional services for patients. The pharmacists lacked certain skills and opportunities to be able to enhance the patients’ satisfaction. The pharmacists in this study needed to improve their soft skills in some areas and engage in mutual discussion with patients in order to enhance patients’ expectations with the service provided. Conclusion/discussion The knowledge and attitude of pharmacists were good, but their consultation skills could be improved. Although simulated consultation allowed pharmacists to review their skills and practice the apparent impact on patient care was limited. This study has enable greater understanding of pharmacist strategies when consulting people with depression, and the findings could be used by those developing training programmes for enhancing pharmacists skills.
