Internetförmedlad depressionsbehandling : En jämförelse mellan fysisk aktivitet och beteendeaktivering

Depression är en av vår tids vanligaste folksjukdomar, trots det söker bara hälften av alla drabbade vård. Ett sätt att nå ut till fler är genom internetbaserade behandlingsmetoder. Syftet med föreliggande studie var att undersöka effekten av fyra behandlarunderstödda internetadministrerade behandlingar vid mild till måttlig depression. Två av behandlingarna bygger på fysisk aktivitet och två på beteendeaktivering. Inom varje betingelse har rationalen manipulerats. Därtill används en kontrollgrupp. 312 deltagare randomiserades till de fem grupperna. Resultaten visar att samtliga behandlingar leder till minskade depressionssymtom, däremot gick det inte att påvisa några signifikanta mellangruppsskillnader. Samtliga grupper uppvisar en ökad fysik aktivitet och det gick att påvisa en signifikant mellangruppsskillnad då de grupper som inte fått någon rational ökat sin fysiska aktivitet signifikant mer än kontrollgruppen. En trolig anledning till att inga mellangruppsskillnader gällande depressionssymtom gick att identifiera är att även kontrollgruppen minskat sina symtom avsevärt. / Depression is one of the most widespread diseases of our time, but only fifty percent of the diseased seeks out for help. One way to reach out to more people is by offering internet based treatment methods. The purpose of this study is to investigate four therapist supported internet administrated treatments for mild to moderate depression. Two of the treatments are based on physical activity and two on behavioural activation. The treatment rationale in each condition has been manipulated. A control group has been used. 312 participants were randomized to the five groups. The results show that all the treatments leads to decreased symptoms of depression, but no in between group differences were found. All groups showed an increase of physical activity and there was a between group difference between the groups without rationale and the control group. The groups without rationale had increased their physical activity more than the control group. One possible explanation to the lack of between group differences on symptoms of depression is the fact that the control group also decreased their symptoms substantially. / Actua!

A dissertation submitted in partial fulfilment of the requirements for the degree of Masters of Arts (Clinical Psychology) in the Department of Psychology at the University of Zululand, South Africa, 2000. / This dissertation examines the concept of depression from the perspective that experiences of darkness and depth have validity, not as states to he avoided, rescued from or talked out of, but as creative opportunities. It also highlights that the journey into depths may be a perilous one, which should not be made unaccompanied. The process of reaching and holding the deep centre, individuation, is served only by willingness to stay with what may be highly uncomfortable experiences. What also becomes imperative is recognizing and honouring the call from depression. This initial dive into the depths may lead to.almost immediate resurfacing, which enables the inward-turned energy to be re¬directed outwards and ordinary life to be resumed, although with part of oneself still looking towards the centre. For Winnicott depression is regarded as having value. He felt that to enter, stay with and come out of depression was a triumph of integration. The argument in this dissertation is that creative work can come from depression as a means of finding the truth about oneself and thus increasing one's self-esteem. The creative impulse needs the depressed mood to flourish, in which sense, depression itself can be regarded as creative. The contention remains that it is the honouring of the journey into depression that is critical to the outcome.
