Physiotherapy rehabilitation in the context of HIV and disability in KwaZulu-Natal.

Aim: The purpose of this study was to describe the experiences of people living with the human immunodeficiency virus who underwent a physiotherapy rehabilitation programme, with the aim of informing and improving future physiotherapy rehabilitation interventions. Methodology: Design: A qualitative research design was adopted, using the International Classification of Function, Disability and Health as a guiding framework. Population: All adult HIV positive patients who were referred for physiotherapy rehabilitation at a public-funded South African hospital during the course of a five week clinical block. Sample: Fourteen participants were eligible for the study. Eight of these participants, who were able to attend the post-rehabilitation interview, were considered for final analysis in the study. Study setting: A public-funded hospital within the eThekwini district of KwaZulu-Natal, South Africa. Research procedure: All eligible participants were requested to complete a questionnaire, the World Health Organisation Disability Assessment Schedule, prior to commencing a physiotherapy rehabilitation programme. After the period of rehabilitation, participants were interviewed using 14 open-ended questions designed to explore their experiences of this programme. Results: The questionnaire data described the participants’ demographics and illustrated the varying cognitive and physical challenges faced by these eight individuals. Content analysis of the eight interviews revealed the following themes: the participants’ knowledge of their health conditions and their prescribed medication, the impact of their illness on their impairments, activities and participation in their daily lives, the context in which these factors exist, the participants’ experience of physiotherapy rehabilitation and the barriers they faced in accessing continued rehabilitation. Conclusion: While participants reported mostly positive experiences related to physiotherapy rehabilitation, they face a number of barriers that limit their access to continued rehabilitation. It is hoped that the results of this study will assist in informing the development of future physiotherapy interventions, which are better designed to suit the needs of PLHIV in a South African public health context. / Thesis (M.Physio.)-University of KwaZulu-Natal, Westville, 2012.

Persons with physical disabilities� experiences of rehabilitation services at community health centres in Cape Town

<p>According to the United Nations, more than half a billion people (about 650 million) worldwide are disabled. Disability can have a vast impact on both the individual and the family. Rehabilitation is therefore a fundamental need for the persons with disability to achieve functional independence and have an improved quality of life. To enhance the effectiveness of rehabilitation, it is important to seek clients&rsquo / perspectives of the rehabilitation services and to incorporate these perspectives into the planning and delivery of rehabilitation services. In areas where rehabilitation services are available in South Africa, minimal research has been done to explore the clients&rsquo / experiences regarding provision of these services. The aim of this study&nbsp / therefore was to explore the persons with physical disabilities&rsquo / experiences of the rehabilitation services they received at community health centres (CHCs) in the Cape Town Metro Health&nbsp / District. Data was collected using a mixed methods design in the form of a sequential exploratory strategy. Qualitative data collection was done using in-depth interviews and this was followed&nbsp / by administration of an interview questionnaire. The questionnaire was developed using results from the in-depth interviews together with information from literature. Ten persons with physical&nbsp / isabilities, who had received rehabilitation services at participated in the in-depth interviews and 95 responded to the interview questionnaire. The interviews were tape-recorded and&nbsp / transcribed verbatim and they were analysed using predetermined themes. The SPSS version 16.0 was used to analyse the quantitative data which was presented in frequencies, medians, quartiles and percentages. The results&nbsp / of the study revealed that the participants experienced problems with getting transport to travel to the community health centres and getting adequate information from the service providers,&nbsp / &nbsp / particularly information regarding disability and support services available for them. Experiences regarding participants&rsquo / involvement in their rehabilitation were generally positive. Generally, the&nbsp / participants reported positive experiences regarding their interaction with service providers and family support and involvement and this study recommends the staff to maintain their standards&nbsp / &nbsp / regarding these two dimensions of rehabilitation. However most of the participants were not concerned about whether the service providers gave them an opportunity to express their preferences or not. The results indicate the need to improve transport services for persons with physical disabilities and to give them more information regarding support services. The service providers&nbsp / should also give the clients more opportunities to get involved in their rehabilitation and educate them about the benefits of them getting involved.</p>

This master thesis introduces the issue of individual concepts that affect the psychological and motor processes, occurring at people with physical disabilities. It addresses the definition of the basic concepts within both historical and present context. The thesis considers what kind of connection and relationships exist behind the failures of the body scheme and the human psyche, and in greater detail deals with methods that work to improve the quality of life for a person with a physical disability. The research part establishes the situation of comprehensive rehabilitation in the Czech Republic. It is developed according to the preliminary areas: lifelong learning, the status of the comprehensive rehabilitation and procedures used in caring for a person with a physical disability. The thesis deals with different problems in the area of decision making in further possible steps concerning the child care and also with the way how to deal with given problems. The aim is to point to the parents who have a child with combined disability and to illustrate their situation from the beginning, showing how they cope with the situation. The practical part contains a research which is based on the questionnaire filled in by parents who have a child with combined disability. The questionnaire focuses above all on the parents, their situation and the change of their lives after the birth of their child, partnership, environment, help and other organizations that frequent the parents and their children.

The diploma thesis deals with the coordinated rehabilitation. Coordinated rehabilitation is a coherent process of medical, educational, vocational and social rehabilitation, whose main objective is to minimalize the consequences, based on the individual's disability. Successful rehabilitation consists in ensuring early, continuous and coordinated efforts of the fastest and widest participation of persons with disabilities in the usual activities of life.The theoretical part consists of six basic chapters. The first one is devoted to the International Classification of Functioning, Disability and Health, which is followed by the subchapter dealing with disabilities and consequently mental disabilities. I have focused on people with mental disabilities, as an organization where research was conducted, providing services to these people. The second chapter contains information about coordinated rehabilitation. The remaining chapters deal with the individual components of a coordinated rehabilitation, which are medical, educational, vocational and social rehabilitation. The aims of this thesis is to determine how the system of coordinated rehabilitation is used in the Institute of social care Domečky. A partial aim is to determine the importance of a system of coordinated rehabilitation for clients of the Institute of social care Domečky. There are two research questions in the connection with the thesis aims. The main research question is: How are the individual components of a coordinated rehabilitation applied in the Institute of social care Domečky? The particular research question finds positives provides system of coordinated rehabilitation for clients of the Institute of social care Domečky.Qualitative research was chosen for the practical section of the thesis. I have used the questioning method and the technique of the semi-controlled dialoque, Results were evaluated using the method of make the clusters. The selected group consisted of 11 employees of the Institute of social care Domečky. The respondents have been chosen intentionally, according to the criterion of willingness to participate in research. The results are divided into two sections according to the research questions. The first section deals with the way the use of individual components of the coordinated rehabilitation in the Institute of social care Domečky. These results are also divided by the individual components of coordinated rehabilitation for greater clarity. The first part contains the identification of the respondents. Following parts: medical rehabilitation, educational rehabilitation, vocational rehabilitation and social rehabilitation.The second part of results includes the subjective opinions of the respondents on the benefits of coordinated rehabilitation and its benefit to the clients of the organization.The research has shown ignorance of the concept of coordinated rehabilitation. Within medical rehabilitation it was found that health care is provided by nurses and practical doctor. The organization also works with hospitals and polyclinics, which are next to the organization. The research, which is devoted to educational rehabilitation, results divided terminology used in special education. Vocational rehabilitation can be used by clients within a job in the kitchen and the laundry in the organization or employment in the civic association Pferda which offer a job in a cafe, a cleaning company and a bakery.Within the social rehabilitation it was described the relationship of key worker and client and the creation of individual planning. It was clarified the issue of social security benefits when clients receive invalidity pensions, care allowance, alternatively child benefit. From the perspective of respondents the benefit of coordinated rehabilitation is in complex care for clients that is devoted them.
