Eosinofilia esofágica em pacientes com anafilaxia à proteína do leite de vaca / Esophageal eosinophilia in patients with anaphylaxis to cow\'s milk protein

Esofagite Eosinofílica é uma doença inflamatória crônica restrita ao esôfago e imune mediada por antígenos. Sua prevalência descrita varia desde 0,4%, numa população geral, até 15% em pacientes com sintomas de disfagia. Já se conhece sua associação com doenças atópicas, anafilaxia e alergia alimentar, sendo o leite de vaca um dos principais alimentos envolvidos. Existem relatos recentes de casos em que pacientes foram diagnosticados com esofagite eosinofílica após serem submetidos à imunoterapia oral com o alimento causador de sua alergia alimentar mediada por IgE. Porém, em nenhum destes casos foi avaliado previamente se os mesmos pacientes já não apresentavam eosinofilia esofágica latente e/ou sintomas subjetivos sugestivos da doença. Considerando que, atualmente, um dos tratamentos mais promissores para alergia alimentar é a imunoterapia oral, justificou-se a necessidade de entender se esofagite eosinofílica seria de fato uma complicação do tratamento, ou se seria uma condição pré ou coexistente. Portanto, o objetivo deste trabalho foi avaliar a frequência de eosinofilia esofágica em pacientes com anafilaxia à proteína do leite de vaca. Foram analisados 89 pacientes matriculados no ambulatório de alergia alimentar do HC-FMUSP, com mediana de idade de 8 anos e que apresentavam anafilaxia ao leite de vaca. Todos foram submetidos à endoscopia digestiva alta com biópsias de esôfago, estomago e duodeno. Dados demográficos, comorbidades atópicas, uso de medicações e sintomas gastrointestinais foram analisados e comparados. A frequência de eosinofilia esofágica foi de 38,2% (34 de 89 pacientes). Em 15 dos 34 pacientes com eosinofilia esofágica, foi completada a investigação para esofagite eosinofílica com uso de inibidor de bomba de prótons em dose plena por 8 semanas antes de uma segunda endoscopia. Identificou-se, portanto, cinco pacientes (7,1%) com eosinofilia esofágica responsiva a inibidor de bomba de prótons e 10 pacientes com esofagite eosinofílica (14,2%). No grupo total de pacientes com eosinofilia esofágica (n=34) encontrou-se 29,4% de pacientes com quadro clínico gastrointestinal ausente; 23,5% oligossintomáticos, e apenas 47% com sintomas sugestivos de disfunção esofágica e, destes últimos, nem todos apresentavam sintomas esofágicos persistentes. Pode-se concluir que a frequência de esofagite eosinofílica descrita no grupo estudado foi significativamente superior à estimada na população geral e uma das mais altas descritas em grupos de pacientes com fatores de risco específicos. Também foi observada uma grande parcela de pacientes com eosinofilia esofágica, sendo muitos assintomáticos ou oligossintomáticos, surgindo o questionamento se esta não seria uma doença latente, de início precoce, insidioso e não relacionada diretamente como complicação de tratamentos atuais / Eosinophilic esophagitis is a chronic inflammatory disease, which occurs in the esophagus and is immune mediated by antigens. Its observed prevalence varies between 0.4% in the general population to 15% in patients with dysphagia. Its association with atopic diseases, anaphylaxis and food allergy has already been recognized. Cow\'s milk is one of the main food sources involved. There are recent reports of cases in which patients were diagnosed with eosinophilic esophagitis after being submitted to oral immunotherapy with the food that causes the IgE mediated allergy. However, in none of these cases was it previously determined if the same patients did not already present latent esophageal eosinophilia and/or subjective symptoms suggestive of the disease. Considering that, currently, one of the most promising treatment for food allergy is oral immunotherapy, the need to understand if eosinophilic esophagitis could be a treatment complication, or if it is a coexistent or preexistent condition, is justified. Therefore, the objective of this study was to evaluate esophageal eosinophilia frequency in patients with anaphylaxis to cow\'s milk protein. We analyzed eighty-nine patients registered in the Food Allergy Unit of the HCFMUSP, with a median age of 8 years, who presented cow\'s milk anaphylaxis. All of them were submitted to digestive endoscopy as well as esophagus, stomach, and duodenum biopsies. We also analyzed and compared demographic data, atopic comorbidities, use of medication, and gastrointestinal symptoms. The frequency of esophageal eosinophilia was 38.2% (34 of 89 patients). In 15 of the 34 patients with esophageal eosinophilia, full investigation for the disease was carried out using a proton pump inhibitor at full dose for eight weeks prior to a second endoscopy. From this, five patients (7.1%) had the proton pump inhibitor-responsive esophageal eosinophilia phenotype, and ten patients were diagnosed with eosinophilic esophagitis (14.2%). In the whole group of patients with esophageal eosinophilia (n = 34), it was found 29.4% of patients with an absent gastrointestinal clinical condition, 23.5% were oligosymptomatic, and only 47% had symptoms suggestive of esophagic dysfunction. Of these, not all presented persistent esophagic symptoms. It is possible to conclude that the frequency of eosinophilic esophagitis observed in this group was significantly higher than the estimated for the general population, and one of the highest observed in groups of patients with specific risk factors. A large portion of patients with esophageal eosinophilia were oligosymptomatic or asymptomatic, raising the question if this would not in fact be a latent disease, with a precocious beginning, insidious and not directly related to current treatments complications

Infiltrados eosinofílico e linfocítico em carcinoma espinocelular de lábio como fatores prognósticos / Eosinophilic and lymphocytic infiltrating in squamous-cell carcinoma of the lip as prognosis

O carcinoma espinocelular de lábio (CEC) é uma patologia relativamente freqüente e acomete preferencialmente indivíduos de pele clara e com antecedentes de exposição solar. A localização labial do CEC está associada ao comportamento clínico mais agressivo. Algumas características clínicas e histológicas, como tamanho do tumor, grau de diferenciação, espessura, invasão perineural e perivascular, atuam como fatores prognósticos do CEC de lábio. A presença do infiltrado inflamatório vem sendo associada ao comportamento de diversos tumores, dentre eles o CEC. Esse infiltrado é composto por linfócitos T, B, células natural killer, macrófagos e eosinófilos. O papel do infiltrado eosinofílico no CEC é bastante controverso, mostrando associação tanto com melhor como com pior prognóstico. Neste trabalho, tentamos relacionar os infiltrado eosinofílico e linfocítico entre si e estabelecer uma possível associação com o prognóstico do tumor. Foram avaliados 29 casos de CEC de lábio atendidos no ambulatório de cirurgia dermatológica do HC-FMUSP, no período de 1980 a 1989. Foram colhidos dados clínicos provenientes dos prontuários (sexo, idade, tabagismo, duração da lesão, presença de recidiva e metástase) e dados histológicos (grau de diferenciação, espessura, invasão perineural e perivascular, comprometimento da camada muscular, contagem de eosinófilos peritumoral e intratumoral, contagem de linfócitos) em lâminas coradas pela hematoxilia-eosina. A comparação entre a quantificação dos eosinófilos na área intra e peritumoral mostrou-se semelhante. Não foi encontrada significância estatística entre a contagem de eosinófilos tumorais e fatores que funcionam como prognósticos do CEC de lábio (espessura, duração da lesão, infiltrado perivascular e perineural, comprometimento da camada muscular, grau de diferenciação, presença de recidiva e metástase). O infiltrado linfocítico desempenha fator prognóstico nos casos de melanoma, porém, no CEC, ainda é controverso. Foi realizada a classificação semiquantitativa do infiltrado linfocítico e comparada às mesmas variáveis estudadas com os eosinófilos. Foi encontrada relação estatisticamente significativa entre maior infiltrado linfocítico e menor grau de diferenciação das células tumorais, sugerindo um papel importante dos antígenos de superfície presentes no desencadeamento da resposta imune linfocitária anti-tumoral. As outras comparações não se mostraram significativas. Tentamos relacionar ainda a quantidade de eosinófilos e linfócitos no infiltrado, e observamos que as duas populações de células se comportam de maneira independente. Concluímos então que os infiltrados eosinofílicos peritumoral e intratumoral apresentam quantidades semelhantes de eosinófilos; o infiltrado eosinofílico não tem relação com prognóstico do CEC de lábio; o número de linfócitos no infiltrado tumoral é maior em tumores mais indiferenciados; o infiltrado linfocítico não influencia no restante dos fatores que funcionam como prognóstico do CEC de lábio; a quantidade de eosinófilos e linfócitos no CEC de lábio comportam-se de maneira independentes / The squamous-cell carcinoma (SCC) is a pathology quite frequent in people in general, mostly in white persons with history of excessive exposition to the sun rays. The fact of the SCC is located on the lips is associated with the most aggressive clinical behavior. Some clinical and histological characteristics, as the size of the tumor, degree of differentiation, density, perineural and perivascular invasion, act as prognostic factors of SCC of the lip. The presence of the tumor-infiltrating has being associated with the behavior of several tumors, including SCC. This infiltrating is composed by lymphocytes T, B, natural killer cells, macrophages and eosinophils. The role of the eosinophilict infiltrating in SCC is controversial enough because it shows association with both best and worst prognosis. In this study, we tried to stablish relations between the eosinophilic and the lymphocytic infiltratings, to stablish a possibility of association with the tumor prognosis. In order to achieve our objective, we have analized 29 cases of SCC os lip observed in the ambulatory of dermatologic surgery of HC-FMUSP from 1980 to 1989. We have also collected clinical data from the patient\'s promptuaries (sex, age, tobaccoism, time of lesion, presence of recurrence and metastasis) and histological data (degree of differentiation, depth, perineural and perivascular invasion, infiltration of the muscle layer, counting of peritumoral and intratumoral eosinophils, counting of lymphocytes) in microscopic features stained with hematoxylin and eosin. When comparing quantities of eosinophils in the intra and peritumoral areas we found them similar. We have not found any significant differences between the counting of tumoral eosinophils and factors that work as SCC of lips prognosis (depth, time of lesion, perivascular and perineural invasion, infiltration of the muscle layer, degree of differentiation, presence of recurrence and metastasis). The lymphocytic infiltrating acts as prognostic factor in the cases involving melanoma, however, in CEC it is still controversial. We have proceeded a classification of the lymphocytic infiltrating and compared with the same fluctuations studied with the eosinophils. We have found a significant relationship between the higher lymphocytic infiltrating and a minor degree of differentiation of tumoral cells, which suggests that the surface antigens in the lymphocytic anti-tumor immune response have an important role. Other comparisons were not significant. We tried to list the quantity of eosinophils and lyphocytes in the infiltrating and we noted that two groups of cells had differents behaviors. We conclude, therefore, that: the peritumoral and intratumoral eosinophilic infiltrating present similar quantities of eosinophils; the eosinophilic infiltrating is not related to the SCC of lips prognosis; the number of lyphocytes in the tumoral infiltrating is higher in less differentiated tumors; the lymphcytic infiltrating has no influence in the other factors that work as lips\' CEC prognosis; and the quantity of eosinophils and lymphocites in the SCC of lips have independent behaviors

This study aimed to evaluate laboratory aspects (total IgE and eosinophils) in patients with Schistosoma mansoni and geohelminths in three localities in Rio Largo, Alagoas. Coprological survey was conducted in 3030 subjects and examined two slides for each stool sample by the Kato-Katz technique, blood count to determine the absolute and relative number of eosinophilic and quantification of serum total IgE using the automated chemiluminescence system (Automated Chemiluminescence Systems) ACS: 180 SIEMENS, adapted to the System II and MAGIC LITE using commercial kits in 547 individuals from three localities infested by the snail Biomphalaria glabrata, close to the river Mundaú and environmental similarities. The subjects were divided into four groups, G1 (co-infected with S. mansoni and geohelminths): n = 115 (21.02%) with mean age 21.18 ± 13.08 years, G2 (infected only by S. mansoni): n = 127 (23.22%) with mean age 29.59 ± 16.54 years; G3 (infected only by geohelminths): n = 149 (27.24%) with mean age 23.99 ± 15.52 years and G4 (negative for S. mansoni and geohelminths, the control group): n = 156 (28.52%) with mean age 25.44 ± 14.60 years. The parasitic load of S. mansoni was quantified and classified according to intensity of infection by the number of eggs per gram of feces detected. The study was approved by the Ethics Committee of the Federal University of Alagoas and implemented in accordance with the resolution MS-CNS 196/96. For processing and data analysis used the software SPSS 11.5. In all analysis was considered the 5% level of significance. The descriptive statistical analysis of qualitative and quantitative variables were performed by the distribution of relative frequencies and absolute position measurements (mean) and dispersion (standard deviation). He applied the normality test, t test, analysis of variance oneway (ANOVA), chi-square test, Fisher exact test, Pearson and Spearman correlation and linear regression. Of the 3030 samples, 1,209 (39.90%) were positive for any helminth, and 679 (22.41%) positive for A. lumbricoides, 546 (18.02%) for T. Trichiura, 220 (7.26%) to Ancyostomatidae and 242 (8.0%) for S. mansoni. There was no statistically significant difference (p>0.05) of individuals positive for S. mansoni separated by gender, age and locality, the mean parasite burden was moderate and 111 epg (minimum 24 and maximum of 4,080 epg). Eosinophils increased significantly with increasing parasite load (p<0.05). The absolute and relative eosinophilia appear to have greater significance in relation to group coinfected, because presented more frequently than the other groups. There were no significant differences (p>0.05) between worm burden of S. mansoni and total IgE levels, nor between serum total IgE and age. Thus the total IgE test was not considered relevant to the diagnosis of Schistosomiasis mansoni. There was no correlation between markers of total IgE and geohelminth infections, whether they are co-infected with S. mansoni. The results suggest the importance of including eosinophil counts in epidemiological surveys, the complementation of the findings and parasitological diagnosis of schistosomiasis, contributing to more precise investigations focused on the control of schistosomiasis in endemic areas. / Este estudo objetivou avaliar aspectos laboratoriais (IgE total e eosinófilos) em portadores de Schistosoma mansoni e geohelmintos em três localidades no município de Rio Largo, Alagoas. Foi realizado inquérito coprológico em 3.030 indivíduos e analisadas duas lâminas para cada amostra de fezes pela técnica de Kato-katz, hemograma para determinar o número absoluto e relativo de eosinífilos e quantificação sérica de IgE total utilizando o sistema de quimioluminescência automatizado (Automated Chemiluminescence Systems) ACS: 180 da SIEMENS, adaptado ao Sistema MAGIC LITE II e usando kits comerciais em 547 indivíduos de três localidades infestadas pelo molusco Biomphalaria glabrata, próximas ao rio Mundaú e com semelhanças ambientais. Os indivíduos foram divididos em 4 grupos, G1 (co-infectados por S. mansoni e geohelmintos): n=115 (21,02%) com média de idade 21,18 ± 13,08 anos; G2 (infectados apenas por S. mansoni): n=127 (23,22%) com média de idade 29,59 ± 16,54 anos; G3 (infectados exclusivamente por geohelmintos): n=149 (27,24%) com média de idade 23,99 ± 15,52 anos e G4 (negativos para S. mansoni e geohelmintos, considerado grupo controle): n=156 (28,52%) com média de idade 25,44 ± 14,60 anos. A carga parasitária do S. mansoni foi quantificada e classificada de acordo com a intensidade da infecção pelo número de ovos por grama de fezes detectados. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Alagoas e executado de acordo com a resolução CNS-MS 196/96. Para o processamento e análise dos dados utilizou-se o software SPSS 11.5. Em todas as análises foi considerado o nível de 5% de significância. A análise estatística descritiva das variáveis qualitativas e quantitativas foram realizadas através da distribuição de freqüências relativas e absolutas, medidas de posição (média aritmética) e dispersão (desvio padrão). Aplicou-se teste de normalidade, teste t, análise de variância one-way (ANOVA), teste do quiquadrado de Pearson, teste exato de Fisher, correlação de Pearson e Spearman e regressão linear simples. Das 3.030 amostras, 1.209 (39,90%) estavam positivas para algum helminto, sendo 679 (22,41%) positivas para A. lumbricoides , 546 (18,02%) para T. Trichiura, 220 (7,26%) para Ancyostomatidae e 242 (8,0%) para S. mansoni. Não houve diferença estatisticamente significante (p>0,05) dos indivíduos positivos para S. mansoni separados por gênero, faixa etária e localidades, a média da carga parasitária foi moderada e de 111 opg (mínimo de 24 e máximo de 4.080 opg). Os eosinófilos aumentavam significantemente conforme o aumento da carga parasitária (p<0,05). As eosinofilias absoluta e relativa parecem ter maior significado em relação ao grupo de co-infectados, pois apresentram maior frequência em relação aos demais grupos. Não foram observadas diferenças significantes (p>0,05) entre carga parasitária de S. mansoni e níveis de IgE total, nem entre níveis séricos de IgE total e faixa etária. Desta forma o teste de IgE total não foi considerado relevante para o diagnóstico de esquistossomose mansoni. Não houve correlação entre os marcadores de IgE total e as infecções geohelmínticas, independente de estarem co-infectados por S. mansoni. Os resultados sugerem a importância da inclusão de contagem de eosinófilos em inquéritos epidemiológicos, na complementação dos achados parasitológicos e no diagnóstico da esquistossomose, contribuindo para investigações mais precisas voltadas para o controle da esquistossomose mansônica nas áreas endêmicas.

Introdução: A dermatite atópica (DA) é uma doença cutânea inflamatória de caráter crônico, recidivante, em que o prurido intenso e a xerose cutânea são frequentes. A etiopatogenia da DA é multifatorial, envolvendo fatores genéticos, ambientais e imunológicos. Eosinófilos são leucócitos polimorfonucleares multifuncionais que estão implicados na patogênese de diversos processos inflamatórios, incluindo a DA. Além da produção e secreção de diversas proteínas presentes nos grânulos citoplasmáticos, os eosinófilos também apresentam potencial para secretar metaloproteinases, enzimas proteolíticas que degradam vários componentes da matriz extracelular, e estão presentes em diversos processos fisiológicos e patológicos. Objetivo: Avaliar: 1) o perfil fenotípico dos eosinófilos na dermatite atópica do adulto, através da expressão das moléculas CCR3, CD23, CD38, CD69 e CD62L; 2) o perfil funcional, a partir da secreção de metaloproteinases, inibidores teciduais de metaloproteinases e RANTES por eosinófilos purificados. Métodos: Foram incluídos 41 adultos diagnosticados com DA, de acordo com os critérios de Hanifin & Rajka e 45 controles adultos sadios. A gravidade da doença foi mensurada através do escore de gravidade EASI (Eczema Area and Severity Index). Eosinófilos (LIN 1- CCR3+) do sangue periférico foram analisados para os marcadores CCR3, CD38, CD69, CD23 e CD62L através da citometria de fluxo (LSRFortessa, BD Biosciences) a análise foi realizada com o FlowJo 7.5.6 software. Eosinófilos purificados de indivíduos com DA e indivíduos controles foram estimulados com enterotoxina de Staphylococcus aureus B (SEB) e FSL-1 (agonista de receptores Toll-like 2 e 6), e os sobrenadantes foram coletados para dosagem de metaloproteinases (MMPs), inibidores teciduais de metaloproteinases 1 e 2 (TIMP-1 e TIMP-2) e RANTES por ELISA e por Cytometric bead array. Resultados: Indivíduos com DA apresentaram maior frequência de eosinófilos (LIN1- CCR3+), relacionada à gravidade da doença. Observou-se também, que a frequência de CD62L (L-selectina) e de CD23 (receptor de baixa afinidade para IgE) em eosinófilos (LIN1- CCR3+) diminui em pacientes com DA. Os receptores de ativação precoce (CD69) e tardio (CD38) não mostraram diferença estatística entre os grupos analisados. Os níveis séricos de MMPs e de TIMPs foram similares entre os controles e pacientes. Ao analisarmos a secreção de MMPs e de (TIMPs), a partir de eosinófilos purificados de pacientes com dermatite atópica, observamos diminuição dos níveis basais de TIMP-1 e TIMP-2 e de RANTES. Conclusões: Na DA do adulto, o perfil fenotípico e funcional dos eosinófilos mostrou: perfil de ativação da fase aguda, com expressão aumentada de CCR3; potencial de migração elevado, em decorrência da diminuição da expressão de CD62L; falhas no processo de ativação dos eosinófilos via CD23, bem como, no remodelamento tecidual mediado por TIMP-1 e TIMP-2 e na quimotaxia mediada por RANTES / Introduction: Atopic dermatitis (AD) is an inflammatory, chronic and recurrent skin disease characterized by intense pruritus and xerosis. AD has a complex etiopathogenesis, which involves the influence of genetics, environment, and immunological disorders, among others. Eosinophils are multifunctional polymorphonuclear leukocytes that contribute to the pathogenesis of several inflammatory processes, such as AD. In addition to the production and secretion of diverse proteins of the cytoplasmic granules, eosinophils have also the potential to secrete metalloproteinases (MMPs), proteolytic enzymes with a primary role for degrading several extracellular matrix components, present in distinct physiological and pathological processes. Objective: To evaluate:1) the phenotypic profile of eosinophils in adults with atopic dermatitis through the expression of CCR3, CD23, CD38, CD69 and CD62L molecules; 2) the functional profile through secretion of MMPs, tissue inhibitors of metalloproteinases 1 and 2 ( TIMP-1 and TIMP-2) and RANTES by purified eosinophils. Methods: This work enrolled 41 patients with AD, diagnosed according to Hanifin & Rajka\'s criteria) and 45 healthy controls. Severity of the disease was established utilizing EASI (Eczema Area and Severity Index). Eosinophils (Lineage cocktail 1- CCR3+) from peripheral blood were analyzed for CCR3, CD38, CD69, CD23 and CD62L by flow cytometry (LSRFortessa, BD Biosciences), and analysis was performed using the FlowJo 7.5.6 software. Purified eosinophils were stimulated with Staphylococcus aureus enterotoxin B (SEB) FSL-1 (Toll-like receptor 2/6 agonist), and supernatants were collected for MMPs, TIMPs and RANTES secretion, evaluated by ELISA and cytometric bead array (CBA). Results: Patients with AD have a higher frequency of eosinophils (LIN1- CCR3+), related to disease severity. Moreover, the frequency of CD62L (L-selectin) and CD23 (low-affinity receptor for IgE) in (LIN1- CCR3+) eosinophils was reduced in individuals with AD. CD69 and CD38 (early and late activation receptors) did not show significant difference in the studied groups. Serum levels of MMPs and of TIMP-1 and TIMP-2 were similar in healthy controls and AD patients. When analyzing secretion of MMPs and TIMPs by purified eosinophils from AD individuals, we detected a decrease in baseline levels of TIMP-1, TIMP-2, and reduced RANTES-mediated chemotaxis. Conclusions: Eosinophils in AD exhibit an activation profile of acute phase, with enhanced CCR3 expression, high potential for migration due to reduced expression of CD62, defective activation mechanisms via CD23, altered tissue remodeling process mediated by TIMP-1 and TIMP-2 and reduced RANTES-mediated chemotaxis

Estando relacionados com o sistema de imunidade inata do organismo, eles representam uma linha fundamental de defesa contra invasão microbiana e, quando ativados, produzem uma série de mediadores solúveis que atuam nas respostas inflamatórias e alérgicas. A relação entre a atividade dos eosinófilos e plaquetas foi observada nas últimas décadas por muitos cientistas. Estas observações incluem o aumento do número de eosinófilos associados a desordens plaquetárias, incluindo alterações na cascata de coagulação e agregação plaquetária. Com base nessas observações, a interação entre os eosinófilos e plaquetas foram analisadas na agregação plaquetária. Plaquetas humanas foram incubadas com a fração citosólica de eosinófilos, linhagem celular promielocítica humana HL-60 clone 15 e proteína catiônica do eosinófilo (ECP). A agregação em plasma rico em plaquetas (PRP) foi induzida por difosfato de adenosina, fator de ativação plaquetária, ácido araquidônico e colágeno, e as plaquetas lavadas (PL) foram ativadas por trombina. A agregação induzida por todos os agonistas foi inibida de maneira concentração de células dependente pela fração citosólica de eosinófilos. Esta inibição foi apenas parcialmente revertida pela prévia incubação dos eosinófilos com L-Nitro-Arginina-metil-éster (L-NAME). A prévia incubação com indometacina não impediu a inibição induzida pela fração citosólica. A separação da fração citosólica de eosinófilos por gel filtração em Sephadex G-75 mostrou que a atividade inibitória foi concentrada na fração de peso molecular mais baixo. As células HL -60 clone 15 diferenciadas em eosinófilos por 5 e 7 dias foram capazes de inibir a agregação plaquetária. A proteína de ECP inibiu a agregação plaquetária em PRP e PL. Esta inibição foi mais evidente em PL, e o ensaio de citotoxicidade com MTT demonstrou a viabilidade de plaquetas testadas, indicando que a inibição observada pela proteína ECP não ocorre simplesmente pela morte celular. A proteína EDN, clonada e expressa em sistema eucarioto, também apresentou efeito de inibição sobre a agregação plaquetária em PRP, enquanto que a proteína MBP não apresentou efeito de inibição da agregação plaquetária significativo. Os nossos resultados indicam que os eosinófilos desempenham um papel fundamental na inibição da agregação plaquetária / Abstract: Eosinophils participate in allergic and inflammatory processes, being related to innate immunity system of the body, they represent a fundamental line of defense against microbial invasion and when activated produce a number of soluble mediators that act in inflammatory and allergic responses. The relationship between the activity of eosinophils and platelets has been observed in recent decades by many scientists. These observations include increased numbers of eosinophils associated with platelet disorders, including changes in the coagulation cascade and platelet aggregation. Based on these observations, the interaction between eosinophils and platelets in platelet aggregation was analyze. Human platelets were incubated with eosinophil cytosolic fraction, promyelocytic human HL-60 clone 15 cell lineage, and eosinophil cationic protein (ECP). Platelet rich plasma (PRP) aggregation was induced by adenosine diphosphate, platelet activating factor, arachidonic acid, and collagen, and washed platelets (WP) were activated by thrombin. Aggregation induced by all agonists was dose dependently inhibited by eosinophil cytosolic fraction. This inhibition was only partially reversed by previous incubation of the eosinophils with L-Nitro-Arginine-Methyl-Ester (L-NAME). Previous incubation with indomethacin did not prevent the cytosolic fraction induced inhibition. The separation of eosinophil cytosolic fraction by gel filtration on Sephadex G-75 showed that the inhibitory activity was concentrated in the lower molecular weight fraction. HL-60 clone 15 cells differentiated into eosinophils for 5 and 7 day were able to inhibit platelet aggregation. The ECP protein inhibited the platelet aggregation on PRP and WP. This inhibition was more evident in WP, and the citotoxicity MTT assay proved the viability of tested platelets, showing that the observed inhibition by the ECP protein does not occur simply by cell death. The EDN protein, cloned and expressed in eukaryotic system, also showed inhibitory effect on platelet aggregation in PRP, whereas the protein MBP had no effect significative inhibiting platelet aggregation. Rôle des éosinophiles et des cellules lymphoïdes innées dans l'asthme / Role of eosinophils and innate lymphoid cells in asthma

Dans la première partie de cette thèse, nous avons cherché à déterminer le rôle de l’éosinophile circulant dans l’asthme par une approche transcriptomique. Nos résultats suggèrent que l’éosinophile recruté sur un site inflammatoire présente des fonctions immunomodulatrices, importantes dans la réparation tissulaire et le retour à l’homéostasie. Dans la deuxième partie de cette thèse, nous avons étudié le rôle des cellules NK et des cellules lymphoïdes innées de type 2 (ILC2) dans l’inflammation asthmatique. Les cellules NK circulantes sont fortement activées dans l’asthme sévère et ont la capacité d’induire l’apoptose d’éosinophiles autologues in vitro. La PGD2 induit la production d’IL-13 par les ILC2 en synergie avec les cytokines épithéliales IL-25 et IL-33. Enfin, ces fonctions sont régulées par la LXA4. En conclusion, notre travail de thèse met en lumière de nouveaux rôles pour l’éosinophile et les cellules lymphoïdes innées dans l’immunopathologie de l’asthme. / In the first part of the thesis, we adopted a transcriptomic-based approach to investigate the activation state of circulating eosinophils in patients with asthma and other unrelated hypereosinophilic diseases. Taken together, our results, which suggest that esoinophils, recruited to inflammatory sites exhibit non-specific immunomodulatory functions important for tissue repair and homeostasis. In the second part of the thesis, we investigated the role of NK cells and type 2 innate lymphoid cells (ILC2) in asthma inflammation. Circulating NK cells are highly activated in severe asthma and promote apoptosis of autologous eosinophils in vitro. ILC2 generate interleukin-13 in response to prostaglandin D2 alone and in a synergistic manner with the airway epithelial cytokines IL-25 and IL-33. Finally, these functions are regulated by lipoxine A4. In conclusion, this thesis highlights new roles for eosinophils and innate lymphoid cells in asthma immunopathology.

Detection of the halogenating activity of heme peroxidases in leukocytes by aminophenyl fluorescein

The formation of hypochlorous and hypobromous acids by heme peroxidases is a key property of certain immune cells. These products are not only involved in defense against pathogenic microorganisms and in regulation of inflammatory processes, but contribute also to tissue damage in certain pathologies. After a short introduction about experimental approaches for the assessment of the halogenating activity in vitro and in cell suspensions, we are focusing on novel applications of fluorescent dye systems to detect the formation of hypochlorous acid (HOCl) in leukocytes. Special attention is directed to properties and applications of the non-fluorescent dye aminophenyl fluorescein that is converted by HOCl, HOBr, and other strong oxidants to fluorescein. This dye allows the detection of the halogenating activity in samples containing free myeloperoxidase and eosinophil peroxidase as well as in intact granulocytes using fluorescence spectroscopy and flow cytometry, respectively.

The inflammatory response against Cryptococcus neoformans is regulated by eosinophilic granulocytes and the interleukin-4/interleukin-4 receptor axis

Cytokines play an important regulatory role during immune responses against pathogens. The outcome of an induced cytokine pattern is determined by many factors. It strongly depends on the nature of the pathogen and the host’s ability to control the quality and strength of cytokine signals. In pulmonary infection with Cryptococcus neoformans T helper (Th) 1 and Th17 cell subsets and their associated cytokines confer protection, whereas a Th2-biased response with production of interleukin (IL) -4 confers susceptibility. Since inappropriate Th responses often lead to death in immunosuppressed human patients, especially HIV-1 infected patients, this work aimed to elucidate mechanisms of Th2 induction and regulation by assessing the Th2 hallmark cytokine IL-4 in an experimental model of cryptococcosis. Therefore, a kinetic study of IL-4 expression during 70 days after intranasal infection was performed in susceptible mice. The analyses included characterization of pulmonary leukocytes and Th cell cytokine profiling. IL-4 profiling revealed Cryptococcus-specific IL-4 production not before six weeks after infection. This unexpected finding was further validated by equal results observed in a kinetic study done in IL-4 reporter mice. These mice express a green fluorescent protein simultaneously to IL-4 expression in the same cell and this protein can be detected by flow cytometry. Two cellular sources of IL-4 were identified: Th2 cells were found as expected, but also, as shown for the first time, eosinophilic granulocytes could be demonstrated to secrete IL-4. Next, the influence of eosinophils on pulmonary inflammation and disease development was investigated using ΔdblGATA-1 mice constitutively devoid of eosinophilic granulocytes. Experiments with infected ΔdblGATA-1 mice revealed novel regulatory functions of eosinophils in cryptococcosis. In the absence of eosinophils pulmonary Th cell recruitment was significantly diminished. In addition, Th2 polarization was reduced in ΔdblGATA-1 mice as shown by reduced numbers of Th2 cells expressing the Th2-related surface marker T1/ST2 and reduced albeit not absent IL-4 production by Th cells. In addition to reduced IL-4 production, in the absence of eosinophils Th cells with enhanced interferon-γ and IL-17 production were observed. However, control of pulmonary fungal growth was only slightly enhanced in the absence of eosinophils and dissemination of cryptococci to the brain was unaltered. This may be related to the shared IL-4 production by not only eosinophils but also Th2 cells. Blocking more than one cellular source of IL-4 could be required to prevent immunopathology. To test the hypothesis of gradual IL-4-dependent immunopathology, experiments were conducted using mice expressing only one allele of the IL-4receptor (R) alpha (α) chain (+/-) instead of two (+/+). Indeed, mono-allelic expression of the IL-4Rα resulted in an intermediate expression of the IL-4R on the surface of myeloid and lymphoid cells indicating a gene-dosage effect for IL-4R expression. Infected IL-4Rα+/- mice displayed reduced susceptibility as compared with IL-4Rα+/+ mice, and IL-4Rα-/- mice completely lacking IL-4R expression were found to be protected with survival for the complete time period of the experiment (i.e. up to 275 days). Reduced susceptibility found in infected IL-4Rα+/- mice was associated with decreased serum levels of immunoglobulin E, reduced mucus production by airway epithelia, attenuation of airway hyper-reactivity, and reduced formation of alternatively activated macrophages in lung parenchyma – pathophysiological features, which are typically found in experimental models of asthma but also in asthma of humans and animals. Since no up-regulation of IL-4R by the infection with Cryptococcus neoformans was found, the experimental pulmonary infection model used appears to be a very sensitive low-level IL-4 system. This work highlights the outstanding role of IL-4 and its different cellular sources as well as its receptor in cryptococcosis and provides novel insights into pathogenesis. Moreover, a cellular (i.e. eosinophils) and a molecular (i.e. IL-4R) target for treatment of this mycosis and possibly of asthma is provided.
