Dlouhodobé sledování pacientů po první atace schizofrenie z hlediska kognitivní výkonnosti a jejího vztahu k psychosociálnímu fungování / Long-term monitoring of patients after the first episode of schizophrenia in terms of cognitive performance and its relationship to psychosocial functioningPešková, Barbora January 2017 (has links)
Cognitive deficit is now considered to be a core feature of schizophrenia, which significantly influences psychosocial functioning in individuals with this illness. The aim of this study is to analyze the cognitive performance during the first year after the first episode of the illness and cognition related to psychosocial functioning. A total of 28 people hospitalized after the first psychotic episode of schizophrenia F20 according to ICD-10 participated in our study and underwent measurements of cognitive functions by the international MCCB battery. They were also evaluated by the PANSS (Scale of positive and negative symptoms) and PSP (range of personal and social performance). After one year from the first measurement for re-examination, 20 patients made an appearance. In 75% of the sample (n = 28), a cognitive deficit below 1.5 standard deviations below the normative average, i.e. below 35 T-score, was detected, at least in one of the measured domains. Speed of processing was the most impaired domain (T < 35), Reasoning and problem solving was the least attenuated domain (T > 40). The resulting analysis (paired t-test) showed a significant improvement one year after first-episode schizophrenia in domains Speed of processing (p = 0,018) and Reasoning and problem solving (p = 0,023). The...

Multimodalitní MR zobrazování patologických změn mozku u nemocných se schizofrenií / Multimodality MR Imaging of Pathological Changes in SchizophreniaSlezák, Ondřej January 2021 (has links)
Multimodality MR imaging of pathological changes in schizophrenia Aim: To prove structural changes of the neocortex and white matter of the brain indicating connectivity disorder in early phases of schizophrenia. Material and methods: A prospective monocentric study comparing a cohort of patients after the first episode of schizophrenia (on average 15.6 days after the initial hospitalization) with a control group of healthy persons. Probands were examined using a complex MRI protocol. Twenty-six patients and twenty-four healthy persons were examined in total. Three dimensional T1 and T2 data and DWI data were analyzed using TBSS FA, FBA a surface- based morphometry. Results: Large areas of dispersively decreased FA were found in patients compared to control group using TBSS. Several fixels of decreased FD metric were found using FBA in the anterior commissure of patients and one sporadic fixel of decreased FDC metric was found in frontal white matter of the brain. No statistically significant areas of cortical surface area and cortical thickness differences were found using SBM. Conclusions: Large areas of decreased microstructural integrity of the white matter of the brain were found. However, it was not possible to specify the nature of its corruption using FBA. Our findings indicate the crucial role of...

Čichové vnímání u první epizody schizofrenie a akutních a přechodných psychotických poruch / Olfactory Perception in First-Episode Schizophrenia and Acute and Transient Psychotic DisordersHofmannová, Zdena January 2019 (has links)
Research suggests that olfactory perception of patients with schizophrenia differs from healthy people. In the context of previously conducted research, the present thesis addresses the differences in the abilities of identification and discrimination of odours and assessment of perceived odour qualities (pleasantness, familiarity, intensity, edibility) between patients with schizophrenia or acute and transient psychotic disorders and healthy volunteers, taking into account gender differences; in addition, the differences in the relationship between identification of odours and evaluation of the perceived qualities of odours between patients and healthy persons were explored as well as relationship between the severity of negative symptoms and olfactory perception. In line with other studies, deficits were found in patients in identification and discrimination of odours and in the assessment of odour familiarity compared to healthy subjects, with no gender differences. There were no differences in the relationship between the identification of odours and assessment of the perceived qualities of odours between patients and volunteers, and association of olfactory abilities and the assessment of perceived qualities of odours with the severity of negative symptoms was found only for identification of...
