Estudio experimental de la Osteonecrosis producida por Bisfosfonatos Nitrogenados

Se realizó un análisis del efecto profiláctico del tratamiento antibiótico en ratas Sprague-Dawley tratadas con dexametasona más pamidronato y sometidas a cirugía oral. Se incluyeron un total de 120 animales en un estudio prospectivo randomizado integrado por dos grupos con 60 animales cada uno, en tratamiento diario con dexametasona y pamidronato durante 7, 14 y 21 días. Todas fueron sometidas a extracción del molar derecho mandibular o maxilar. Además a un grupo se le añadió penicilina durante 7 días. Las ratas de ambos grupos se sacrificaron a los 14 y 28 días después de la extracción. En el grupo I, se documentó la osteonecrosis en 18 casos (34,6%). En el grupo II, se documentó osteonecrosis en 5 casos (9,61%) con diferencia estadísticamente significativa entre los dos grupos (p = 0,002). La adopción de una profilaxis antibiótica en los procedimientos dentales invasivos conduce a una reducción significativa de las osteonecrosis de los maxilares asociadas al uso de bisfosfonatos / An analysis is made of the prophylactic effect of antibiotic treatment in Sprague-Dawley rats treated with pamidronate plus dexamethasone and subjected to oral surgery. A total of 120 animals were included in a randomized prospective study composed of two groups with 60 animals each, in treatment with Dexamethasone and Pamidronate daily for 7, 14 and 21 days. All were subjected to right mandibular or maxillary molar extraction. Also a group with the addition of penicillin for 7 days. The rats in both groups were sacrificed 14 and 28 days after extraction. In group I, osteonecrosis was documented in 18 cases (34.6%)). In group II, osteonecrosis was documented in 5 cases ( 9.61%) the difference between the two groups being statistically significant (p= 0.002). The adoption antibiotic prophylaxis in invasive dental procedures results in a significant reduction in osteonecrosis of the jaws associated with bisphosphonate use.
